Improving Emergency Airway Knowledge and Self-Efficacy Levels of Outpatient Gastroenterology Staff via Implementation of Online Education and In Situ Simulation

Abstract:Twenty percent of all ambulatory surgery cases utilizing monitored anesthesia care and sedation report at least one perioperative respiratory complication such as bronchospasm, hypoxia, laryngospasm, or aspiration (). However, the national Standards of Practice for both surgical technicians and ambulatory care nurses do not mandate emergency airway education beyond cardiopulmonary resuscitation and Basic Life Support training. A local outpatient gastroenterology clinic noticed the gap in education, and the anesthesia team decided to implement an evidence-based dual-factorial quality improvement project utilizing online education and in situ simulation. First, registered nurses and procedural technologists completed a test to assess their baseline knowledge and airway emergency performance self-efficacy levels. Then an online module was distributed that included information on the 3 most common anesthesia airway emergencies in the outpatient setting: laryngospasm, aspiration, and obstruction with resultant hypoxemia. Next, participants completed an in situ simulation of the 3 airway emergencies using low-fidelity mannequins. A post-education assessment was distributed after completion of the simulation training and again at 6 weeks and 3 months post-implementation. The data collected showed a statistically significant increase in both knowledge scores and levels of self-efficacy at 6 weeks and 3 months posteducation (p < .001).
A Simulated Tracheostomy Tube Change Educational Intervention to Promote Competency Among Novice Healthcare Professionals: A Repeated Measures Study

Abstract:Objective: To evaluate an educational intervention to promote confidence, knowledge, and skills in tracheostomy tube change among nursing students. Methods: The study, conducted at the at the Johns Hopkins Center for Immersive Learning and Digital Innovation, enrolled nursing students without prior experience in tracheostomy tube change. The intervention included a pre-recorded presentation, faculty demonstrations with a Tracheostomy Care Training Simulation Model, and participant practice demonstrating skills. Primary outcomes included confidence, knowledge, and competency with tracheostomy tube changes. Secondary outcomes included number of attempts required to achieve competency and time required per attempt. The study followed STROBE guidelines with repeated measure design. Results: Participants in the study (n=50) had a mean age of 30 years, were predominantly female (83%) with a bachelor's degree (76%), most often in the third semester of nursing school (45%). Participants showed a mean improvement of 3.58 points out of five (SD: 0.56, P<0.001) across 11 pre- and post-test items. Every confidence assessment improved, with the largest increase in assessing tube placement. Knowledge assessments improved across all eight test items in the first test-retest interval, showing an improvement of 1.14 points out of five (SD: 0.89, P<0.001). Competency assessment improved in the first test-retest interval of 1.01 points out of five (SD: 0.65, P<0.001). On serial assessments, time to complete tracheostomy tube change decreased from 2.39 to 0.60 minutes. Faculty deemed 95% of participants competent after only one skill testing iteration. Conclusion: An educational intervention, combining digital presentations with interactive faculty-led simulations and practical skill assessments, successfully elevated nursing students' confidence, knowledge, and competency in tracheostomy tube changes.
Mastering tracheostomy care: Refresher programme for tracheostomy training for nurses: Comparison of two training methods based on hands-on simulation-based training alone versus additional complementary self-directed e-learning

Abstract:Background Effective clinical education is essential for managing tracheostomy patients safely and efficiently. Simulation-based training has shown greater efficacy than traditional methods in various clinical settings. Our internal training programme, called the Tracheostomy Refresher Program (TRP) was used to enhance nurses' skills in tracheostomy care. Aim/objective The aim of this study was to evaluate the impact of the TRP on nurses' self-reported knowledge and confidence and psychomotor skills comparing hands-on simulation-based training alone (TRP-S) with both the simulation-based training and the e-learning component (TRP-S + e). Methods The study was conducted at a large tertiary hospital in Singapore from February 2022 to October 2022, focussing on the TRP. Participants were divided into two cohorts: those receiving TRP-S and those receiving additional complementary TRP-S + e. All participants completed theory tests and affective questionnaires before and after the training to assess knowledge and attitudes. At the same time, their psychomotor skills were evaluated during the simulation using a standardised checklist. The two cohorts were then compared based on the results of these pretests and post-tests and the psychomotor skills assessment to evaluate the effectiveness of the additional e-learning component. Results Participants reported significantly enhanced confidence, knowledge, and psychomotor skills in tracheostomy care post training (p < 0.001 for all). The TRP-S + e cohort showed significantly higher knowledge and confidence scores than the TRP-S cohort (p < 0.001 for both). Conclusion Our study suggests that a TRP incorporating hands-on simulation-based training with or without e-learning significantly improved self-reported knowledge, confidence, and psychomotor skills in tracheostomy care. Future research should explore the optimal duration, engagement strategies, and cost-effectiveness of such educational techniques and whether similar approaches can be applied for other clinical skills.

Abstract:An educational project in medical undergraduate otorhinolaryngology teaching-learningmethodology was designed with the aimof objectively studying the impact of self-directed learning on the topic of tracheostomy, since the latter has been included by the National Medical Commission as a core-competence for students. 150 third-year medical undergraduates participated in the study. They were divided into two groups, namely A and B. Group A was guided to undertake self-directed learning on tracheostomy while Group B underwent a lecture on the sametopic. Thereafter, both the groups responded to a validated feedback performa. Both the groups were assessed before and after the teaching-learning sessions by way of validated multiple-choice questions. Seventy-eight percent of participants from Group A agreed with the fact that they had acquired adequate knowledge on tracheostomy following the self-directed learning session as compared to 93.3% of those in Group B. Lacunae in the knowledge at the end of the teaching sessions were found to be more (27.4%) in Group B as compared to Group A (22%). The results obtained through the preand post-tests found Group A students to be performing significantly better than those in Group B, indicating that self-directed learning had been a more effective method for teaching the skills of tracheostomy as compared to the conventional lecture involving a large group. Self-directed learning, when used alone for a topic like tracheostomy, is more effective than a lecture in terms of reduced lacunae in knowledge and students performance in examinations.

Abstract:A retrospective study was performed on 54 patients with intracranial mass lesions who underwent closed needle biopsy using freehand and stereotactic biopsy techniques. A diagnosis of brain neoplasm was established in 44 of 54 patients, and nonneoplastic abnormalities were discovered in an additional six cases. Permanent neurologic worsening was noted in two instances following biopsy, and a single patient died as a result of the procedure. Closed needle biopsy, particulary when done stereotactically, appears to be a resonably safe and efficacious means of establishing a tissue diagnosis in carefully selected patients suspected of harboring a deep-seated supratentorial cerebral neoplasm.

Abstract:The past decade has witnessed unprecedented progress in tracheostomy care, through communication, dissemination, and implementation of key drivers including interprofessional education, team-based care, standardized protocols, patient and family engagement, and data-driven practice. Improved safety, efficiency, and quality of tracheostomy care reflects contributions from fields of competency-based education, evidence-based practice, and quality improvement. These elements are interconnected, reinforcing one another to enhance patient care. Competency-based interactive education emphasizes active and practical learning through simulations and case studies, which enhance the clinical skills essential for high-quality care. These educational strategies are grounded in clinical research, ensuring that care practices are continually updated and aligned with the latest evidence, thereby bridging the gap between research findings and clinical application. Quality improvement processes such as Plan-Do-Study-Act (PDSA) cycles refine care delivery in real-world settings. Implementation science promotes the uptake of evidence-based practices, ensuring that discoveries translate to improved health outcomes, quality of care, and overall system performance. In each of these domains, patient and family engagement ensures alignment with patient needs and values. The Global Tracheostomy Collaborative leverages this integrated approach through international educational symposia and webinars, comprehensive data analyses, and a learning community that promotes innovative technologies like in situ simulation and augmented and virtual reality. Together, these approaches enhance the learning and application of best practices in tracheostomy care. The continuous, dynamic interaction of education, research, and quality improvement, grounded in patient-centered care, fosters excellence and innovation in care of patients with tracheostomy.

Association of a Perioperative Education Program With Unplanned Readmission Following Total Laryngectomy

Abstract:Importance: Patients undergoing total laryngectomy (TL) are at high risk for 30-day hospital readmission. Strategies to decrease the readmission rate remain unknown. Objectives: To assess the association of a comprehensive perioperative TL education program with unplanned readmissions; to determine the program’s association with the rate of readmissions for stomal or tracheoesophageal puncture (TEP) complications and patient and caregiver knowledge of and preparedness for TL. Design, Setting, and Participants: This single-institution prospective pilot study was conducted between December 1, 2014, and November 30, 2016, among 50 patients undergoing a perioperative TL education program at a tertiary care academic medical center. Intervention: The perioperative TL education program consisted of speech-language pathology counseling, a hands-on class with an otolaryngology nurse educator, a TL “Journal Guide” booklet, and a prehospital discharge competency assessment. A family member or friend acting as a laryngectomy coach accompanied patients throughout. Main Outcomes and Measures: The primary outcome was the rate of 30-day unplanned readmission. Secondary measures included the rate of readmission for stomal or TEP complications and change in knowledge of and preparedness for TL. Results: Of the 50 patients (12 women and 38 men; median age, 61 years [range, 47-86 years]) who underwent the TL education program, the 30-day unplanned readmission rate was 20% (n=10). Only 1 patient (2%) had a readmission for a stomal or TEP complication. Patients increased their TL knowledge (median improvement in TL knowledge test score, 3.5 [95% CI, 2.8-4.2] of 12) and preparedness (median improvement in TL preparedness score, 3.1 [95% CI, 2.4-3.8] of 10) after undergoing the intervention. Conclusions and Relevance: This prospective pilot study evaluated an intervention to decrease unplanned readmission in head and neck oncology patients. It provides data indicating that a comprehensive perioperative TL education program is feasible. This program has the potential to decrease 30-day readmission for stomal or TEP complications and merits further study in a larger, multicenter clinical trial.
Inpatient Nursing and Parental Comfort in Managing Pediatric Tracheostomy Care and Emergencies

Abstract:Importance: Tracheostomy is a critical and often life-saving intervention, but associated risks are not negligible. The vulnerability of the pediatric population underlies the importance of caregiver comfort and competence in tracheostomy care. Objective: To assess inpatient nursing staff and parental perspectives in managing tracheostomy care. Design, setting, and participants: Cross-sectional analysis of survey data from (1) a volunteer sample of inpatient nurses in a tertiary care, freestanding pediatric hospital in the Midwest, assigned to clinical wards that provide care for children with tracheostomy tubes and (2) a consecutive sample of families whose child underwent tracheostomy tube placement at the same institution between March 1 and December 31, 2013. Main outcomes and measures: Nurse and parental comfort in managing acute and established tracheostomy tubes. Nursing data were analyzed with attention to years' experience and primary unit of practice. Results: Respondents included 129 of 820 nurses (16% response rate) and family members of 19 of 38 children (50% response rate). When queried about changing established tracheostomies, 59 of 128 nurses (46%) reported being "totally comfortable," including 46 of 82 intensive care unit (ICU) nurses (56%) vs 13 of 46 floor nurses (28%) (P = .002) and 48 of 80 nurses with at least 5 years' experience (60%) vs 12 of 49 less experienced nurses (24%) (P < .001). For managing accidental decannulation of a fresh tracheostomy, 61 nurses (47%) described being completely uncomfortable, including 27 of 83 ICU nurses (33%) vs 34 of 46 floor nurses (73%) (P = .006), and 33 of 80 nurses with at least 5 years' experience (41% ) vs 28 of 49 less experienced nurses (57%) (P = .03). Most families felt prepared for discharge (16 of 17 [94%]) and found the health care team accessible (16 of 17 [94%]), although only 5 of 18 families (28%) indicated that tracheostomy teaching was consistent. Conclusions and relevance: Nurses' comfort with tracheostomy was higher among nurses with at least 5 years' experience and primary ICU location. Whereas parental comfort with tracheostomy care was high, lack of consistent instruction highlights the role for standardized education in tracheostomy care.

We would like to congratulate the authors on their publication as it highlights a number of important issues regarding tracheostomy management in anaesthesia practice. We would like to make some comments regarding the case report. We agree with the authors recommendation that an anaesthesia specific guideline should be used for every patient with a tracheostomy undergoing surgery. Crises can challenge even the most experienced clinicians and cognitive aids can provide valuable real time decision support (2,3). In the case report Villasenor and colleagues identified a number of actions that could have been undertaken if a well-designed cognitive aid had been available and they presented a new checklist and guideline (1). Key points they emphasized were: having awareness of the age of the tracheostomy and the relative difficulty of previous upper airway management, confirmation of a patent airway with capnography before commencing anaesthesia, the importance of passing a suction catheter to confirm tube patency and manage obstruction, the value of deflating the cuff and the importance of prompt tube removal (1). The Austin Hospital, Melbourne, Australia has developed a double sided, A3 sized Emergency Tracheostomy Management cognitive aid which travels with the patient wherever they go in the hospital including the operating room (4). The major content source for our cognitive aid was the National Tracheostomy Safety guidelines and algorithm (5). We suggest that the use of our cognitive aid (or equivalent) would have prevented and/or enabled more rapid management of this challenging situation. The cognitive aid is navigated by a cognitive aid reader who is separate to the team leader (3). The front side of the cognitive aid is for primary responders who are the bedside/ward staff. This page also contains tracheostomy and upper airway information. The second side of the cognitive aid is the advanced responders' side, which is to be used by the code blue team and in critical care environments including intensive care, emergency department and the operating room. The advanced responders' side of the cognitive aid could have been used in this case as follows: We have observed in simulation on multiple occasions the phenomenon of an experienced clinician passing a suction catheter into a completely blocked tracheostomy and incorrectly reaching the conclusion that the tube is patent. We suggest to learners to gauge depth of the suction catheter on the spare tracheostomy tube at the bedside before inserting it into the patient. In this case the clinician may not have appreciated that the suction catheter had not passed the entire length of the tracheostomy tube. If the team had been using the Austin cognitive aid, failure of passage of a suction catheter to stabilise the patient would still have led them on to the next step (step 3) cuff deflation and if the patient remained unstable to continue until the emergency was resolved. Cognitive aids should be developed in a comprehensive manner by diverse teams (6,7). The emergency tracheostomy management cognitive aid in our hospital is part of hospital-wide, system level approach to tracheostomy care led by the Tracheostomy Review and Management Service (TRAMS). Endotracheal Intubation Performance in Novices: Comparing the Training Effect Resulting From Two Airway Management Training Simulators

Abstract:Introduction: Between 2011 and 2014, the Combat Casualty Training Consortium research study sought to evaluate all aspects of combat casualty care, including mortality, with a special focus on the incidence and causes of potentially preventable deaths among American combat fatalities. This study identified a major training gap in critical airway management. Because of the high rate of morbidity and mortality associated with poor or incorrect airway management, an effort to address this training gap was necessary. Materials and methods: This experiment compared the training using the Advanced Joint Airway Management System, a novel high-fidelity airway simulator, relative to one of the most utilized simulators for endotracheal intubation (ETI) via a parallel group randomized control trial design. Before training, participants (n = 19) attempted an ETI on a cadaver using direct laryngoscopy. Performance during the attempt was recorded and scored by trained blinded raters. Participants were then randomly allocated to either novel or conventional training. Post-training, participants completed a second ETI under the same parameters. Analysis was completed via 2 × 2 mixed analysis of variance for (1) ETI Score, (2) Errors, (3) Critical Failures, and (4) Duration, across both the pre- and post-training tests and between the two groups. Results: A priori power analysis required a total sample size of 84 participants in this experimental design. Consequently, this study is under-powered to reach statistical significance. Scores for all trainees did improve with training, yet analyses did not reveal a difference in overall ETI score between the novel and conventional training group, at baseline or at the post-training test (P = .249). Conclusion: The Advanced Joint Airway Management System simulator presents a training effect that is comparable to the conventional training model. However, given this study's small sample size, these results must be considered preliminary and further research is merited to draw firm conclusions about its impact on trainee performance. Future studies engaging larger cohorts of trainees and exploring the other capabilities of the Advanced Joint Airway Management System (cricothyroidotomy, needle chess decompression) are needed to further examine the educational potential of this novel airway management training system.

Abstract:Background and objectives: Tracheostomy is one of the oldest known surgical procedures. Tracheostomy patients are at high risk for airway obstruction, impaired ventilation, and infection as well as other lethal complications. Skilled bedside nursing care can prevent these complications. Therefore, this study aimed to evaluate the effect of progressive tracheostomy care practice on nurses and physiotherapists in the ICU and RCU in Erbil City-Iraq. Methods: A quasi-experimental study was conducted at four Intensive Care Units (ICU) and Respiratory Care Unit (RCU) Hospitals (Hawler Teaching Hospital, East Emergency Hospital, West Emergency Hospital, and Lalav RCU Hospital) in Erbil City/Iraqi Kurdistan Region. Nurses and physiotherapists from hospitals were selected as the control group, and study group respectively. Nurses and physiotherapists were evaluated through the demographic data questionnaire and tracheostomy care (preparatory phase, performance phase, and follow-up phase) scale. Results: A total of 90 nurses and physiotherapists (40 nurses and 5 physiotherapists from the interventional group and 38 nurses and 7 physiotherapists from the control group) participated. At the end of the study, there were significant improvements in tracheostomy care practice (P-value was 0.001). Conclusion: Nurses and physiotherapists had incompetent practice regarding tracheostomy care (TC) in the ICU, and RCU and the nurses and physiotherapists who received progressive tracheostomy care procedure training had more knowledge and practice than those who did not attend this program. Attempts should focus on improving tracheostomy care in ICUs and RCUs and establish management guidelines.

Abstract:Accumulation of iodine and copper in the node, paranodular and contralateral (not affected tissue by node) tissues of thyroid gland in relation to the level of metal-binding proteins, potential antioxidants and oxidative changes in tissue was investigated. To assess the severity of the pathological process the molecular markers of cytotoxicity were used. The reduction of total iodine (by 19.5%), increase of inorganic iodine fraction (by 82.4%) and total copper content (twice) in paranodular and nodular tissues compared with contrlateral part have been established. Excess of copper in goitrous-changes tissue was partially accumulated in the metallothioneins. The level of metal-binding form of metallothioneins and reserve of free thiols of these proteins was higher two-three times and lower content of reduced glutathione in node-affected tissue compared to the contralateral part. Signs of cytotoxicity among them: higher cathepsine D free activity (up to 84.6% and 134.4% in paranodular tissue and node respectively) and higher level of DNA strand breaks in the node (up to 22.6%) were observed. In paranodular tissue the range of indices variability compared with parenchyma of contralateral part is shorter than in the node. Thus, under low level of iodine organification and high copper level in goitrous-modified tissue of thyroid gland metallothionein may provide a partial compensatory effect on prooxidative processes.
Knowledge, attitudes and practices of nurses regarding airway clearance in neurocritical illness patients: A cross-sectional study

Abstract:Aim: We aimed to assess the level of knowledge, attitudes and practices regarding airway clearance among nurses and explore the factors affecting the knowledge, attitudes and practices. Design: A questionnaire-based cross-sectional study. Background: Airway clearance is an important method of eliminating excess secretions. In neuroscience nursing, nurses are important executors of airway management, and their knowledge, attitudes and practices can influence the effectiveness of airway clearance. Methods: This study was conducted from July to September 2023 in four hospitals in Jiangsu Province, China. A structured questionnaire about airway clearance was designed and used to collect the data. The nurses used this questionnaire to self-rate. The STROBE checklist for cross-sectional studies was followed. Results: The age, work experience, highest educational attainment and technical title of the nurses can significantly influence their knowledge. The age, highest educational attainment and technical title of the nurses can significantly impact their attitudes. Practice scores were significantly influenced by age, work experience, technical title, whether the nurses had received any training on airway clearance techniques, and whether the department developed procedures for implementing the airway clearance technology. Nurses' attitudes were significantly associated with knowledge and practice, and there was no significant correlation between knowledge and practice. Conclusion: This study showed that age, work experience, highest educational attainment and training were related to knowledge, attitudes and practices. These findings suggest that nursing managers can conduct airway clearance training according to age group, working experience and education level of the nurses. Impact: The findings show that the level of knowledge, attitudes and practices related to airway clearance in neuroscience nursing among nurses were acceptable, which means that nurses can better perform airway management on patients. These findings serve as a significant reference for designing an airway clearance education for nurses and meet the needs of nurses in clinical nursing practice.

Abstract:Transgender individuals face significant health disparities. Laryngologists play a key role in gender‐affirmation care. This is the first survey to assess educational experience and state of transgender care among practicing US‐based laryngologists. We attempt to spotlight the degree by which practicing laryngologists are familiar, competent, and comfortable with transgender care. Objectives Transgender individuals face significant health disparities including deficiencies in physician education, knowledge, and comfort with transgender health care. As the prevalence of the transgender population increases more individuals may seek gender‐affirming surgery. Herein, we present a survey study which presents data on (1) the current practice patterns, (2) the familiarity with, (3) the perception of, and (4) the future educational goals of transgender health care among laryngologists in the United States. Methods A cross‐sectional survey study of practicing laryngologists in the United States.
