Abstract:The development of ustekinumab as a first‐in‐class anti‐interleukin (IL) 12/23p40 therapeutic agent for psoriasis represents an important example of modern and rational drug design and development. Psoriasis is a chronic, systemic, immune‐mediated skin disorder with considerable clinical, psychosocial, and economic burden. Ustekinumab is a human monoclonal antibody (mAb) that binds the p40 subunit common to IL‐12 and IL‐23, key cytokines in psoriasis pathogenesis. The therapeutic mAb was developed using human gamma‐1 immunoglobulin (IgG)‐expressing transgenic mice, which created a molecule with endogenous IgG1 biologic properties and low immunogenicity. Ustekinumab was well tolerated in clinical studies and yielded rapid, significant, and sustained efficacy plus improved quality of life/work performance and reduced depression/anxiety. Its pharmacologic properties afford the most convenient dosing regimen among approved biologics, representing a significant advancement in the treatment of moderate to severe psoriasis. Ustekinumab also holds promise for other immune‐mediated disorders with significant unmet need.
Immediate influences of hygiene education sessions on handwashing behaviors of selected Nepali students

Abstract:Abstract Appropriate handwashing can prevent 50–70% of water and foodborne infections. However, schoolchildren who are in the formative stage of life, particularly in low-income countries like Nepal, are deprived of such a lifesaving skill. This study investigates the effectiveness of a school-based participatory action research intervention to promote handwashing with soap among basic level community school students in Nepal. Teachers, the school management committee, the participatory action research committee, and child-club members actively participated in designing and implementing the intervention. This study employed a semi-structured interview with the headteacher, five focus group discussions, and spot observations during 50 handwashing with soap events involving students, to collect the data. This study assesses the handwashing situation of students before and after the intervention. As part of the intervention, participatory teaching methods such as singing, drawing, showing a video, games, and demonstrations were used. Findings from basic level students who actively participated in hygiene sessions and increased their handwashing with soap before meals and after toilet use were used as a comparison to baseline. Participants reported that the intervention was perceived positively, pragmatic, and cost-effective. This intervention study concluded that handwashing behaviors improved because of the influence of sensitization sessions and demonstrations about handwashing. However, some predominant issues in the teaching of handwashing practices include limited hygiene contents in the curriculum and the practical use of teaching and learning activities. The lack of availability of soap at handwashing stations is the main barrier in sustaining handwashing behaviors in schoolchildren.

Abstract:This paper explores school as the site for examining hygiene and cleanliness. The aims of the study were to examine aspects of school hygiene, understand children’s needs and to equip primary school children with tools and capacities to develop and sustain body and environmental hygiene through an intervention. Children studying in primary classes from twelve schools run by the local urban municipal body in Delhi participated in the study. 846 and 656 children were interviewed during pre and post intervention phases respectively. Data was collected in the form of observations, interviews and children’s responses on hygiene practices. Data was coded and subjected to statistical analysis. Based on data obtained in the baseline assessment, a Health Hygiene Education Module was designed. The pedagogy was based on discussions, demonstrations, storytelling, activities, interactive games and takeaways. Post intervention, a significant increase in percentage of children who reported following effective practices in aspects of oral hygiene, handwashing and use of toilets. Children were motivated and keen to learn. However, lack of infrastructure, resources and basic facilities like running water and soap was observed. Supervision and monitoring of all aspects of hygiene in the school needs to be strengthened

Abstract:A study regarding poor hygiene and inadequate sanitary conditions play major roles in the increased burden of communicable diseases. The study evaluated the knowledge, attitude, practice of hygiene among rural school children in Ethiopia; about 669 students were in grades 1-6, and they were interviewed by trained staff. Data consisted of hygiene and hand washing practices, knowledge about sanitation, personal hygiene characteristics. Approximately 52% of students were having adequate knowledge of hygiene, only 36.2% reported using soap, and of the 76.7% of students who reported hand washing after defecation, only 14.8% reported actually following this practice. Study findings underscore the need for more hand washing and hygiene education in schools. The first concept in personal hygiene is the positive and negative emotions that affect physical health, such as feeling pride in being neat and clean or feeling frustration in using hygiene tools. The second concept is personal hygiene practices such as hand washing, oral hygiene etc. The third concept is the fact that germs can lead to common diseases such as the flu or a cold. Nurses can use various techniques to help kids remember these.

Abstract:Background: As per the WHO-UNICEF monitoring report 2012, around 626 million people, or half of the population of India defecate in the open; this explains the high burden of communicable diseases in developing countries like India. In this context present study attempts to assess the effectiveness of school-based awareness programme on toilette etiquettes and sanitation in primary school-going children (mean age 9.27 years.) of Mumbai city. Methods: This is a comparative, before and after study between the interventional group and the comparison group. The study was carried out between July 2016 to March 2017, end line survey was carried out approximate 3 months after programme intervention for both the groups. Stratified random sampling was used to select 3737 students in baseline and 3230 students in end line from 130 primary schools run by municipal corporation of greater Mumbai (MCGM).Results: Knowledge and attitude significantly improved in the end line than the baseline survey exorbitantly for the interventional group than the comparison group. Although the comparison group also exhibited some improvement but there was a marked difference as when compared to the interventional group.Conclusions: An innovative education cum entertainment intervention programmes on basic health and hygiene habits focused on toilette etiquettes and sanitation was conducted. This has influenced the KAP among primary school children of Mumbai city. Though individual behaviour is difficult to mould, short to medium term effects of innovative programmes like WASH are encouraging.

Abstract:Purpose of the study: This research aims to evaluate the effectiveness of using video media in increasing students' knowledge and attitudes about the importance of washing hands with soap at Public Middle School 31 North Bengkulu Methodology: This research uses quantitative methods with a pre-experimental One Group Pretest-Posttest Design. The research population was class III students at Public Middle School 31 North Bengkulu, with a sample of 35 students selected using a purposive sampling technique. Data analysis was carried out using the Wilcoxon test to test the difference between pretest and posttest results. Main Findings: This study concludes that the use of video media as a health promotion tool significantly increases the knowledge and attitudes of students at Public Middle School 31 North Bengkulu towards the importance of washing hands with soap. Video media is proven to be more effective than conventional methods in attracting attention and increasing student participation during the learning process Novelty/Originality of this study: This research is very urgent considering the high rate of transmission of infectious diseases among adolescents which can have a significant impact on their health and school attendance. Infectious diseases such as flu, diarrhea and respiratory infections often spread quickly in the school environment, causing high levels of absenteeism and reducing learning productivity.

Abstract:Background: Most of the morbidities among school children are preventable by ensuring healthy practices regarding personal hygiene and diet. The current study aimed to assess effectiveness of a health education programme regarding personal hygiene and diet in improving the morbidity profile of the adolescent girls in an urban slum. Methods: A quasi experimental study was conducted in two Government secondary schools located in a slum area of Kolkata. The study consisted of three steps. First a baseline survey with the help of a predesigned pretested questionnaire and checklist was done to find out the socio-demographic information and existing knowledge, attitude, practice of personal hygiene and dietary habits of the students. Morbidity profile of the children was also assessed. This was followed by an intervention phase of 6 months during which weekly lecture and demonstration classes were taken in the study school. Second step was reassessment of morbidities by clinical examination at post intervention. Third step was to follow them for another 3 months to reassess their morbidities. Results: There was statistically significant decrease in the prevalence of morbidities in study school from the pre-test level to post-test level as compared to the control school, but the effect was not sustained at follow up as shown by increase in mean morbidity score at 9 months follow up than 6 months post-intervention level in the study school. Conclusions: Sustained behavioral change programme regarding proper practice of personal hygiene and diet could significantly decrease the prevalence of major morbidities encountered in adolescent girl students.

Abstract:Contagious diseases that affect young children place a great burden on them and their families. Proper hand hygiene is an important measure to reduce the disease burden, however, its implementation in day care centres is challenging. This paper introduces a digital intervention to support independent and good handwashing among young children. The intervention leverages animated instructions triggered by water and soap use, together with a symbolic reward shown to children on a screen during and immediately after handwashing. We tested the intervention in a pre-registered, cluster-randomised controlled field trial in 4 day care centres in Finland and Germany with 162 children over 42 days. The intervention increased soaping time, used as a proxy for handwashing quality, by 5.30 s (+ 62%, p < 0.001). The effect occurs immediately at the onset of the intervention and is maintained throughout the intervention phase.

Abstract:<h3 class="Heading">Background</h3><p class="Para">Absenteeism amongst pre-school children is often due to illnesses such as hand, foot, and mouth disease, acute gastroenteritis, cold and flu, which are easily spread amongst them. This is because of weak immunity and lack of knowledge on proper hand hygiene. This quasi-experimental study assessed the efficacy of an intervention consisting of a hand hygiene education programme, along with digital tools in bringing about a change in behaviour and health conditions amongst pre-school children in Klang Valley, Malaysia.</p><h3 class="Heading">Methods</h3><p class="Para">A total of 377 school children, male and female, aged 5–6 years old, participated and were assigned to either the intervention or a control group. During the 2 months intervention period, children in the test group were trained on proper hand hygiene practices and techniques with the aid of the interactive android-based tablets. The numbers of absent days of all the children were recorded for 2 months before the intervention and during the intervention.</p><h3 class="Heading">Results</h3><p class="Para">In the test group, there was a 25% increase in the total number of absent days from the pre-intervention period to the intervention period, a much lesser increment observed as compared to that of control group in which the increase was much higher at 89%. Results showed a significant difference (<em class="EmphasisTypeItalic">P</em> < 0·05) between the absenteeism rates for the test and control group during the intervention period.</p><h3 class="Heading">Conclusion</h3><p class="Para">These results suggest that proper education and intervention increase hand hygiene compliance, which may help decrease school absenteeism due to illness; however, a longer study duration may be necessary to evaluate the benefit further.</p>

Abstract:BackgroundThis article presents the development of a school handwashing programme in two different sub-Saharan countries that applies the RANAS (risk, attitudes, norms, ability, and self-regulation) systematic approach to behaviour change.MethodsInterviews were conducted with 669 children enrolled in 20 primary schools in Burundi and 524 children in 20 primary schools in Zimbabwe. Regression analyses were used to assess the influence of the RANAS behavioural determinants on reported handwashing frequencies.ResultsThe results revealed that, in both countries, a programme targeting social norms and self-efficacy would be most effective. In Burundi, raising the children’s perceived severity of the consequences of contracting diarrhoea, and in Zimbabwe, increasing the children’s health knowledge should be part of the programme.ConclusionsThe school handwashing programme should create awareness of the benefits of handwashing through educational activities, raise the children’s ability and confidence in washing hands at school through infrastructural improvements, and highlight the normality of washing hands at school through events and poster creation.

Abstract:Hygiene is the science of health and its maintenance. Hygiene status of children is an index of national investment in the development of its man power. It is influenced by social, familial and individual factors as well as the children’s knowledge of health on personal hygiene, comfort and basic needs. The utility of games is as a teaching strategy of health professionals. The main objectives of the study were to assess existing level of awareness regarding healthy habits among school children and to check the effect of Modified snake & ladder game in improving the awareness of children regarding healthy habits. Research design selected for this study was pre experimental one group pretest posttest design with a sample size of 60. Samples of the study were given opportunity to play and earn awareness through the modified snake &ladder game. Their awareness was assessed before and after the game section. Data was analyzed by using descriptive and inferential statistics such as mean, standard deviation and chi square test. In data analysis the mean of pre-test score was 13.83 and mean of post-test score was 18.63. The mean difference was 4.8. The standard deviation of pre-test stress score was 01.07 and post-test stress score was 01.60. The calculated’ ‘t’ value was 21.24, which was greater than the table value(1.67).The study concluded that snake and ladder game was effective in improving the awareness of school children on healthy habits.

Abstract:Abstract Purpose Proper handwashing helps prevent the spread of communicable diseases. The aim of our study was to analyse and compare children's knowledge and skills in hand hygiene before and after school interventions in order to evaluate the effectiveness of our peer education programme. Materials/methods In our longitudinal study, short- and long-term changes in the knowledge, hand-washing skills and health behaviour of 224 lower, upper and secondary school students were assessed. Our measurements were performed with a self-administered, anonymous questionnaire and the Semmelweis Scanner. Results As a result of the intervention, the proportion of correct answers increased significantly both in the short term and in the long run compared to the input measurements, but age differences did not disappear for most variables. There is a difference in the process of learning theoretical knowledge and practical skills. Areas not used for handwashing in the paediatric population are different from those described for adults in the literature. There was no significant difference between the mean scores of the right and left hands. Conclusions There was a significant positive change in both theory and practice of handwashing. In education, emphasis should also be put on long-term retention of theoretical knowledge in age-specific health promotion programmes within the paediatric population.

Abstract:Background: Improper food production, handling, and preparation techniques have direct influence on health. A food borne disease is a disease usually either infectious or toxic in nature, caused by agents that enter the body through ingestion of food. The objective of the present study was to assess the food safety knowledge and hygienic practices among food handlers and to assess the improvement in knowledge and hygienic practices after educational intervention.Methods: This was a longitudinal educational interventional study, carried out during October to December 2017 among 50 randomly selected food handlers in and around Gandhi Medical College campus, where the study subjects were interviewed by using a questionnaire in local language regarding their knowledge and practices of food handling, and were then later educated on proper food handling practices by lecture and demonstration method followed by a post intervention interview after a gap of 2 weeks.Results: Out of 50 food handlers, 62% had knowledge of the food borne diseases and it raises up to 100% post intervention. During pre-intervention, only 78% of the participants used to wash their hands before preparing meal and after intervention 92% were washing their hands before preparing meal.Conclusions: Food safety knowledge and hygienic practices among food handlers improved by 73% after the educational intervention. Maximum improvement was seen in the use of gloves, caps and hand washing practices.

Abstract:Abstract Background Human hygiene behaviours influence the transmission of infectious diseases. Changing maladaptive hygiene habits has the potential to improve public health. Parents and teachers can play an important role in disinfecting surface areas and in helping children develop healthful handwashing habits. The current study aims to inform a future intervention that will help parents and teachers take up this role using a theoretically and empirically informed behaviour change model called the Capabilities-Opportunities-Motivations-Behaviour (COM-B) model. Methods A cross-sectional online survey was designed to measure participants’ capabilities, opportunities, and motivations to [1] increase their children’s handwashing with soap and [2] increase their cleaning of surface areas. Additional items captured how often participants believed their children washed their hands. The final survey was administered early in the coronavirus pandemic (May and June 2020) to 3975 participants from Australia, China, India, Indonesia, Saudi Arabia, South Africa, and the United Kingdom. Participants self-identified as mums, dads, or teachers of children 5 to 10 years old. ANOVAs analyses were used to compare participant capabilities, opportunities, and motivations across countries for handwashing and surface disinfecting. Multiple regressions analyses were conducted for each country to assess the predictive relationship between the COM-B components and children’s handwashing. Results The ANOVA analyses revealed that India had the lowest levels of capability, opportunity, and motivation, for both hand hygiene and surface cleaning. The regression analyses revealed that for Australia, Indonesia, and South Africa, the capability component was the only significant predictor of children’s handwashing. For India, capability and opportunity were significant. For the United Kingdom, capability and motivation were significant. Lastly, for Saudi Arabia all components were significant. Conclusions The discussion explores how the Behaviour Change Wheel methodology could be used to guide further intervention development with community stakeholders in each country. Of the countries assessed, India offers the greatest room for improvement, and behaviour change techniques that influence people’s capability and opportunities should be prioritised there.

Abstract:Aims This study aimed to examine the effect of the hand hygiene education on middle school students' behavioral scores related to hand hygiene practices. Design The research adheres to a quasi‐experimental design suitable for the pre‐test Posttest model with a control group. Methods The population of the study consists of a total of 1159 students, comprising 5th, 6th, 7th, and 8th graders. During the first semester of the 2021–2022 academic year, face‐to‐face education was conducted at a middle school located in Ankara. Two face‐to‐face training sessions were conducted to increase awareness of hand hygiene. Results The groups indicate homogeneity in terms of demographic characteristics. The mean hand hygiene behavior score was 56.45 ± 8.03 before the education in the intervention group, which increased to 59.79 ± 7.62 after the education. In the control group, the mean score was 55.98 ± 7.89 before the education and 57.15 ± 7.63 after the education. It was found that after the educational program, there was a statistically significant increase in hand hygiene behavior scores of the groups. Conclusion The research findings indicate a beneficial impact of hand hygiene education on students' average behavioral scores. Impact To foster lasting behavioral changes from a young age, it is advisable to implement hygiene education in schools periodically, including targeted interventions for middle school students.

Abstract:BACKGROUND: Children's knowledge on hygiene is often considered to be provided by parents or schools. It is something with a direct impact on health and linked with socio-cultural factors. Primary care plays a vital role in health education and promotion, so focusing on hygiene habits is part of primary prevention.OBJECTIVES: Improving the knowledge on hygiene and promoting healthy behaviors in students from the 4<sup>th</sup> to the 6<sup>th</sup> grade.MATERIALS AND METHODS: Longitudinal, prospective study. Hygiene habits were assessed through a questionnaire, followed by a brief health education session organized by healthcare professionals at schools. Hygiene habits were then reassessed using the original questionnaire.RESULTS: A total of 812 questionnaires (354 in the pre-session phase and 458 in the post-session phase) were collected, filled in by students with ages ranging between 8 and 16 (average, 10.8), being 52% boys. There were significant increases in the score in both genders, but this increase was slightly higher for boys, with an average increase of 0.98 points (<i>P</i> < 0.05, independent samples t test). The 9 year old group (in the first evaluation; 10 years old post-talk) had the highest increase in score, with an average increase of 1.33 points in the questionnaire.CONCLUSION: Interventions conducted by healthcare professionals at schools have a positive effect for health education. The resulting scores revealed an improvement regarding hygiene habits namely in younger students. This may suggest that interventions on hygiene habits should take place at younger ages, so to have a greater impact in behavioral change.

Abstract:Children are susceptible to infections due to frequent participation in school group activities and their often-suboptimal hygiene practices. Frequent infections in children affect school attendance, academic performances, and general health. The effectiveness of school-based hygiene-related intervention programmes need to be informed by updated high-quality synthesised evidence. In this systematic review, we searched PubMed and Cochrane CENTRAL for randomised and non-randomised interventional studies that evaluated school-based hygiene-related interventions. We assessed risk-of-bias (Cochrane risk-of-bias 2 tool), performed random-effect meta-analysis (RevMan 5.4) and rated certainty-of-evidence (GRADE). Thirty-nine trials (41 reports), published from 2011 to 2024 from 22 countries were included. Twenty-three studies contributed data for meta-analysis. All school-based interventions were compared with standard curriculum and demonstrated very low to low certainty-of-evidence due to study methodological limitations and imprecision. Hand-body hygiene interventions may improve knowledge, attitudes and practices (SMD 2.30, 95%CI 1.17 to 3.44, 6 studies, 7301 participants), increase handwashing practices (RR 1.75, 95%CI 1.41 to 2.17, 5 studies, 5479 participants), and reduce infection-related absenteeism (RR 0.74, 95%CI 0.66 to 0.83, 5 studies, 1017852 observations). Genital hygiene interventions may improve attitude (SMD 6.53, 95%CI 2.40 to 10.66, 2 studies, 2644 participants) and practices (RR 2.44, 95%CI 1.28 to 4.68, 1 study, 1201 participants). However, intervention effects on oral hygiene appeared mixed, with worsening of the oral hygiene score (SMD 3.12, 95%CI 1.87 to 4.37, 2 studies, 652 participants) but improved dental hygiene (SMD -0.33, 95%CI -0.53 to -0.13, 3 studies, 4824 participants) and dental caries scores (SMD -0.34, 95%CI -0.52 to -0.16, 4 studies, 2352 participants). Limited evidence suggests that interventions targeting hand-body and genital hygiene practices may improve knowledge, practices, and infection-related absenteeism. However, the effects on oral hygiene intervention appeared mixed. Future research should strengthen randomisation and intervention documentation, and evaluate hygiene-related behaviour, academic performances and health outcomes.
