Abstract:The development of ustekinumab as a first‐in‐class anti‐interleukin (IL) 12/23p40 therapeutic agent for psoriasis represents an important example of modern and rational drug design and development. Psoriasis is a chronic, systemic, immune‐mediated skin disorder with considerable clinical, psychosocial, and economic burden. Ustekinumab is a human monoclonal antibody (mAb) that binds the p40 subunit common to IL‐12 and IL‐23, key cytokines in psoriasis pathogenesis. The therapeutic mAb was developed using human gamma‐1 immunoglobulin (IgG)‐expressing transgenic mice, which created a molecule with endogenous IgG1 biologic properties and low immunogenicity. Ustekinumab was well tolerated in clinical studies and yielded rapid, significant, and sustained efficacy plus improved quality of life/work performance and reduced depression/anxiety. Its pharmacologic properties afford the most convenient dosing regimen among approved biologics, representing a significant advancement in the treatment of moderate to severe psoriasis. Ustekinumab also holds promise for other immune‐mediated disorders with significant unmet need.
Behavioral Intervention Strategies to Nudge Hand Hygiene

Abstract:Maintaining optimal hand hygiene is an important strategy for infection control and prevention. However, how to increase adherence to hand hygiene practices has been a major challenge to prevent infectious diseases and reduce hospital acquired infections (HAIs), especially in the critical period of COVID-19 epidemic regular prevention and control. There are great differences in the effectiveness between different hand hygiene behavior intervention strategies, and the best hand hygiene intervention practice is still in development and requires further investigation. In order to develop intervention strategies for health care researchers and practitioners, the current research systematically summarizes hand hygiene behavior intervention strategies from the perspective of “nudge” for the first time. Traditional hand hygiene interventions are usually based on knowledge sharing and health education, which only produce weak or modest effects on hand hygiene practices. Recent research in behavioral science provides insights for developing effective behavioral interventions to optimize hand hygiene practices by helping people form better hand hygiene habits. Traditional intervention strategies tend to rely on people’s ability to engage in rational thinking and the availability of cognitive resources, that is, encouraging people to reflect on their hand hygiene behaviors and enhance their handwashing motivation in a rational and conscious way. The influences of external environmental factors are rarely taken into account in traditional intervention strategies. Similarly, the role of personal psychological factors is often ignored, so this kind of hand hygiene intervention strategy often shows limited effectiveness and low sustainability. In addition, there are many other common obstacles such as limited accessibility of hand hygiene products, people's overconfidence in their immune system to prevent infection, inertia, and habitual forgetting. Many research results show that even if traditional intervention strategies can increase risk awareness of poor hand hygiene and enhance hand washing intention in the target population, they may not lead to effective behavior changes in hand hygiene. Inspired by the research from behavioral sciences, researchers have tried to promote experiential, unconscious, and automatically triggered hand hygiene behavior through interventions of specific psychological or external environmental factors, so as to help people overcome the gap between hand washing intention and behaviors. Based on different influential mechanisms, hand hygiene nudging strategies can be classified into four categories and nine subordinate categories, including providing decision information (simplifying information, providing feedback, and harvesting the impact of social norms), optimizing decision options (simplifying options and making original options more attractive), influencing decision structure (increasing the accessibility and visibility of favorable options), and reminding decision direction (direct reminder and environmental hint). Previous studies have shown that behavioral science-based hand hygiene interventions, in general, achieved positive effects at a fairly low cost, which are worthy of further application. However, there are still many disputes concerning ethics and effectiveness for nudging intervention. Among these disputes, the two points often mentioned are whether nudging limits the decision-makers' freedom of independent choice and damages their ability of independent choice. These two disputes may have a relatively small impact on hand hygiene promotion, and the form of nudging intervention is more easily accepted by the public, which may be due to the fact that handwashing behavior essentially has a certain degree of “injunctive norm” tendency (i.e., the vast majority of people agree with maintaining hand hygiene). In addition, previous research on hand hygiene nudging intervention also has many limitations, including low accuracy of the evaluation criteria, sustainability, and generalizability of nudging strategies. Further research is warranted to develop more effective hand hygiene nudging interventions and apply them to diverse social contexts. The effectiveness of multi-facet nudging strategies has also been confirmed in hand hygiene practices, suggesting that another future research direction is to construct a hand hygiene nudging strategy classification system similar to the Behavior Change Technique (BCT) taxonomy, so as to design multi-facet nudging strategies for promoting hand hygiene in a specific social context. Based on China's national conditions, traditional hand hygiene intervention strategies such as health education cannot be completely abandoned. This kind of comprehensive new model of “traditional intervention + nudging strategies” and the personalization and specialization of nudging intervention strategies might be the focus of hand hygiene behavior promotion intervention in the future. However, there is still a lack of hand hygiene nudging intervention in Chinese sociocultural contexts. Another direction for future research is to carry out such nudging interventions in hospitals, schools, communities, and other public places based on the theory of behavioral change, so as to contribute to the prevention and control of infectious diseases and improve public health.
A clinical trial evaluation of handwashing products and educational resources to improve hand hygiene in paediatric patients and school children

Abstract:Introduction: It is widely acknowledged that good hand hygiene (HH) is an important non-pharmaceutical method for reducing the transmission of infectious diseases. Children are at high risk of infection due to their immature immune systems. Hospital transmitted infections are a cause for concern worldwide, with poor HH suggested to be responsible for up to 20% of cases. Patients, in particular paediatric patients, are often overlooked when it comes to the promotion of hand hygiene compliance (HHC) in hospitals. This report describes the clinical evaluation of the 'Soaper Stars'; a collection of child-friendly HH products with linked educational resource, developed using the COM-B approach to behaviour change, and designed to encourage correct HH in paediatric patients and in schools. Method: The Soaper Star products were distributed on paediatric wards in five UK hospitals, and the use of the products around mealtimes was evaluated. Workshops teaching the 'why when and how' of handwashing were run in four UK primary schools with pre and post evaluations conducted to establish impact on knowledge. Over 300 children were involved. Results: The Soaper Stars products stimulated a 38% increase in HHC compared to when only hospital-issued products were available, and verbal feedback from families indicated that having the Soaper Star products encouraged improved HHC by all visitors, not just the patient. Workshops in four schools (283 pupils) showed an increase in knowledge around the transmission of infection and the need for good HH that was sustained for at least 4 weeks. Conclusion: The results of this study demonstrate that providing children with the age-appropriate knowledge about why HH is necessary, and the child-friendly means to maintain their HH, will lead to greater HHC, not just by individual children, but also their families.

Abstract:Contagious diseases that affect young children place a great burden on them and their families. Proper hand hygiene is an important measure to reduce the disease burden, however, its implementation in day care centres is challenging. This paper introduces a digital intervention to support independent and good handwashing among young children. The intervention leverages animated instructions triggered by water and soap use, together with a symbolic reward shown to children on a screen during and immediately after handwashing. We tested the intervention in a pre-registered, cluster-randomised controlled field trial in 4 day care centres in Finland and Germany with 162 children over 42 days. The intervention increased soaping time, used as a proxy for handwashing quality, by 5.30 s (+ 62%, p < 0.001). The effect occurs immediately at the onset of the intervention and is maintained throughout the intervention phase.

Abstract:BackgroundThis article presents the development of a school handwashing programme in two different sub-Saharan countries that applies the RANAS (risk, attitudes, norms, ability, and self-regulation) systematic approach to behaviour change.MethodsInterviews were conducted with 669 children enrolled in 20 primary schools in Burundi and 524 children in 20 primary schools in Zimbabwe. Regression analyses were used to assess the influence of the RANAS behavioural determinants on reported handwashing frequencies.ResultsThe results revealed that, in both countries, a programme targeting social norms and self-efficacy would be most effective. In Burundi, raising the children’s perceived severity of the consequences of contracting diarrhoea, and in Zimbabwe, increasing the children’s health knowledge should be part of the programme.ConclusionsThe school handwashing programme should create awareness of the benefits of handwashing through educational activities, raise the children’s ability and confidence in washing hands at school through infrastructural improvements, and highlight the normality of washing hands at school through events and poster creation.
A cross-sectional survey assessing the influence of theoretically informed behavioural factors on hand hygiene across seven countries during the COVID-19 pandemic

Abstract:Abstract Background Human hygiene behaviours influence the transmission of infectious diseases. Changing maladaptive hygiene habits has the potential to improve public health. Parents and teachers can play an important role in disinfecting surface areas and in helping children develop healthful handwashing habits. The current study aims to inform a future intervention that will help parents and teachers take up this role using a theoretically and empirically informed behaviour change model called the Capabilities-Opportunities-Motivations-Behaviour (COM-B) model. Methods A cross-sectional online survey was designed to measure participants’ capabilities, opportunities, and motivations to [1] increase their children’s handwashing with soap and [2] increase their cleaning of surface areas. Additional items captured how often participants believed their children washed their hands. The final survey was administered early in the coronavirus pandemic (May and June 2020) to 3975 participants from Australia, China, India, Indonesia, Saudi Arabia, South Africa, and the United Kingdom. Participants self-identified as mums, dads, or teachers of children 5 to 10 years old. ANOVAs analyses were used to compare participant capabilities, opportunities, and motivations across countries for handwashing and surface disinfecting. Multiple regressions analyses were conducted for each country to assess the predictive relationship between the COM-B components and children’s handwashing. Results The ANOVA analyses revealed that India had the lowest levels of capability, opportunity, and motivation, for both hand hygiene and surface cleaning. The regression analyses revealed that for Australia, Indonesia, and South Africa, the capability component was the only significant predictor of children’s handwashing. For India, capability and opportunity were significant. For the United Kingdom, capability and motivation were significant. Lastly, for Saudi Arabia all components were significant. Conclusions The discussion explores how the Behaviour Change Wheel methodology could be used to guide further intervention development with community stakeholders in each country. Of the countries assessed, India offers the greatest room for improvement, and behaviour change techniques that influence people’s capability and opportunities should be prioritised there.

Abstract:Children attending kindergarten are at high risk for contracting infections, for which hand hygiene (HH) has been recognized as the most cost-effective prevention measure globally. Kindergarten teachers' HH behavior plays a vital role in encouraging favorable hygiene techniques and environment. This study aims to evaluate the effectiveness of a multimodal intervention at changing kindergarten teachers' HH behavior and social cognitive factors that influences HH behavior in China. The intervention named "Clean Hands, Happy Life" includes HH products with refills, reminders and cues for action, a kick-off event with awards, and training programs. We evaluated the intervention using a self-administrative questionnaire with a stratified random sample of 12 kindergartens. Two surveys was completed by 176 teachers at baseline and 185 after the 6-month intervention. Compared with the baseline scores, there was a significant improvement in the overall self-reported HH compliance of teachers (9.38 vs. 9.68 out of 10, p = 0.006), as well as teachers' perceived disease susceptibility, disease severity and behavioral control after the intervention (p<0.05). We found that teachers' HH compliance was likely to be higher among those who have better HH guideline awareness (β = 0.48, p<0.01) and perceived behavioral control (β = 0.26, p = 0.01), which explained 24.2% of the variance of self-reported compliance of teachers at baseline. The assessed intervention may provide Chinese kindergarten teachers with behavioral skills and cognitions that associated with the compliance of HH behavior. We thus recommend future intervention studies consider our HH behavior change techniques, address multiple social cognitive determinants of HH behavior and include the change of targeted influences in the impact evaluation.

Abstract:Objective: The present study determined the feasibility and initial efficacy of a kindergarten-based, family-involved intervention in improving children's hand hygiene (HH) behaviors. Design: A cluster-randomized controlled trial was performed, with a cluster defined as a kindergarten class. Sample: Participants were recruited from 20 classes in six kindergartens. A total of 289 children and their families were enrolled in the intervention group, and 293 children and their families were enrolled in the control group. Measurements: HH behavior and a related knowledge survey, as well as data on absences due to infection, were collected. Intervention: An 8-week training session on HH for children and an education program combining a seminar and WeChat groups for parents were provided to participants in the intervention group. Results: Two HH behaviors of children, namely, HH after playing outside and 7-stage HH compliance, were significantly different between the two groups after the intervention. The two HH behaviors and knowledge of infections of parents/legal guardians in the intervention group were better than those in the control group after the intervention. The number of absences due to infections in children was lower in the intervention group than in the control group. Conclusions: Kindergarten-based, family-involved interventions effectively improved the HH behavior of kindergarten children and decreased absences due to infections.

Abstract:Introduction Compliance with hand hygiene recommendations in hospital is typically less than 50%. Such low compliance inevitably contributes to hospital-acquired infections that negatively affect patients’ well-being and hospitals’ finances. The design of the present study is predicated on the assumption that most people who fail to clean their hands are not doing so intentionally, they just forget. The present study will test whether psychological priming can be used to increase the number of people who clean their hands on entering a ward. Here, we present the protocol for this study. Methods and analysis The study will use a randomised cross-over design. During the study, each of four wards will be observed during four conditions: olfactory prime, visual prime, both primes and neither prime. Each condition will be experienced for 42 days followed by a 7-day washout period (total duration of trial=189 days). We will record the number of people who enter each ward and whether they clean their hands during observation sessions, the amount of cleaning material used from the dispensers each week and the number of hospital-acquired infections that occur in each period. The outcomes will be compared using a regression analysis. Following the initial trail, the most effective priming condition will be rolled out for 3 months in all the wards. Ethics and dissemination Research ethics approval was obtained from the South Central—Oxford C Research Ethics Committee (16/SC/0554), the Health Regulatory Authority and the sponsor. Interventions to improve hand hygiene compliance in patient care

Abstract:Background: Health care-associated infection is a major cause of morbidity and mortality. Hand hygiene is regarded as an effective preventive measure. Objectives: To update the review done in 2007, to assess the short and longer-term success of strategies to improve hand hygiene compliance and to determine whether a sustained increase in hand hygiene compliance can reduce rates of health care-associated infection. Search strategy: We conducted electronic searches of: the Cochrane Central Register of Controlled Trials; the Cochrane Effective Practice and Organisation of Care Group specialised register of trials; MEDLINE; PubMed; EMBASE; CINAHL; and the BNI. Originally searched to July 2006, for the update databases were searched from August 2006 until November 2009. Selection criteria: Randomised controlled trials, controlled clinical trials, controlled before and after studies, and interrupted time series analyses meeting explicit entry and quality criteria used by the Cochrane Effective Practice and Organisation of Care Group were eligible for inclusion. Studies reporting indicators of hand hygiene compliance and proxy indicators such as product use were considered. Self-reported data were not considered a valid measure of compliance. Studies to promote hand hygiene compliance as part of a care bundle approach were included, providing data relating specifically to hand hygiene were presented separately. Studies were excluded if hand hygiene was assessed in simulations, non-clinical settings or the operating theatre setting. Data collection and analysis: Two reviewers independently extracted data and assessed data quality. Main results: Four studies met the criteria for the review: two from the original review and two from the update. Two studies evaluated simple education initiatives, one using a randomized clinical trial design and the other a controlled before and after design. Both measured hand hygiene compliance by direct observation. The other two studies were both interrupted times series studies. One study presented three separate interventions within the same paper: simple substitutions of product and two multifaceted campaigns, one of which included involving practitioners in making decisions about choice of hand hygiene products and the components of the hand hygiene program. The other study also presented two separate multifaceted campaigns, one of which involved application of social marketing theory. In these two studies follow-up data collection continued beyond twelve months, and a proxy measure of hand hygiene compliance (product use) was recorded. Microbiological data were recorded in one study. Hand hygiene compliance increased for one of the studies where it was measured by direct observation, but the results from the other study were not conclusive. Product use increased in the two studies in which it was reported, with inconsistent results reported for one initiative. MRSA incidence decreased in the one study reporting microbiological data. Authors' conclusions: The quality of intervention studies intended to increase hand hygiene compliance remains disappointing. Although multifaceted campaigns with social marketing or staff involvement appear to have an effect, there is insufficient evidence to draw a firm conclusion. There remains an urgent need to undertake methodologically robust research to explore the effectiveness of soundly designed and implemented interventions to increase hand hygiene compliance.

Abstract:Abstract Purpose Proper handwashing helps prevent the spread of communicable diseases. The aim of our study was to analyse and compare children's knowledge and skills in hand hygiene before and after school interventions in order to evaluate the effectiveness of our peer education programme. Materials/methods In our longitudinal study, short- and long-term changes in the knowledge, hand-washing skills and health behaviour of 224 lower, upper and secondary school students were assessed. Our measurements were performed with a self-administered, anonymous questionnaire and the Semmelweis Scanner. Results As a result of the intervention, the proportion of correct answers increased significantly both in the short term and in the long run compared to the input measurements, but age differences did not disappear for most variables. There is a difference in the process of learning theoretical knowledge and practical skills. Areas not used for handwashing in the paediatric population are different from those described for adults in the literature. There was no significant difference between the mean scores of the right and left hands. Conclusions There was a significant positive change in both theory and practice of handwashing. In education, emphasis should also be put on long-term retention of theoretical knowledge in age-specific health promotion programmes within the paediatric population.
The effects of school-based hygiene intervention programme: Systematic review and meta-analysis

Abstract:Children are susceptible to infections due to frequent participation in school group activities and their often-suboptimal hygiene practices. Frequent infections in children affect school attendance, academic performances, and general health. The effectiveness of school-based hygiene-related intervention programmes need to be informed by updated high-quality synthesised evidence. In this systematic review, we searched PubMed and Cochrane CENTRAL for randomised and non-randomised interventional studies that evaluated school-based hygiene-related interventions. We assessed risk-of-bias (Cochrane risk-of-bias 2 tool), performed random-effect meta-analysis (RevMan 5.4) and rated certainty-of-evidence (GRADE). Thirty-nine trials (41 reports), published from 2011 to 2024 from 22 countries were included. Twenty-three studies contributed data for meta-analysis. All school-based interventions were compared with standard curriculum and demonstrated very low to low certainty-of-evidence due to study methodological limitations and imprecision. Hand-body hygiene interventions may improve knowledge, attitudes and practices (SMD 2.30, 95%CI 1.17 to 3.44, 6 studies, 7301 participants), increase handwashing practices (RR 1.75, 95%CI 1.41 to 2.17, 5 studies, 5479 participants), and reduce infection-related absenteeism (RR 0.74, 95%CI 0.66 to 0.83, 5 studies, 1017852 observations). Genital hygiene interventions may improve attitude (SMD 6.53, 95%CI 2.40 to 10.66, 2 studies, 2644 participants) and practices (RR 2.44, 95%CI 1.28 to 4.68, 1 study, 1201 participants). However, intervention effects on oral hygiene appeared mixed, with worsening of the oral hygiene score (SMD 3.12, 95%CI 1.87 to 4.37, 2 studies, 652 participants) but improved dental hygiene (SMD -0.33, 95%CI -0.53 to -0.13, 3 studies, 4824 participants) and dental caries scores (SMD -0.34, 95%CI -0.52 to -0.16, 4 studies, 2352 participants). Limited evidence suggests that interventions targeting hand-body and genital hygiene practices may improve knowledge, practices, and infection-related absenteeism. However, the effects on oral hygiene intervention appeared mixed. Future research should strengthen randomisation and intervention documentation, and evaluate hygiene-related behaviour, academic performances and health outcomes.

Social-cognitive antecedents of hand washing: Action control bridges the planning-behaviour gap

Abstract:Objectives: To examine motivational and volitional factors for hand washing in young adults, using the Health Action Process Approach (HAPA) as a theoretical framework. Design: In a longitudinal design with two measurement points, six weeks apart, university students (N = 440) completed paper-based questionnaires. Main outcome measures: Prior hand washing frequency, self-efficacy, outcome expectancies, intention and action planning were measured at baseline, and coping planning, action control and hand washing frequency were measured at follow-up. Results: A theory-based structural equation model was specified. In line with the HAPA, the motivational factors of self-efficacy and outcome expectancies predicted intention, whereas the volitional factors of planning and action control mediated between intention and changes in hand washing frequency. Action control was confirmed as the most proximal factor on hand washing behaviour, thus representing a bridge of the planning-behaviour gap. Conclusions: Both motivational and volitional processes are important to consider in the improvement of hand hygiene practices. Moreover, the statistically significant effects for planning and action control illustrate the importance of these key self-regulatory factors in the prediction of hand hygiene. The current study highlights the importance of adopting models that account for motivational and volitional factors to better understand hand washing behaviour.
Effectiveness of a continuous handwashing education program with multiple activities at a Japanese kindergarten school

Abstract:Formation of proper handwashing techniques and habits from childhood is important for disease prevention. However, there are few studies that comprehensively and longitudinally evaluate the effectiveness of handwashing education for kindergarteners. This study aims to evaluate the effectiveness of continuous handwashing education using multiple activities to improve handwashing practices and skills among first- to third-grade students at a kindergarten in central Japan. A quasi-experimental one group pre- and post-test design was used. The education program consisted of three activities: (i) a 1-day teaching session by a researcher in January 2021, (ii) a 1-month follow-up activity led by kindergarten teachers and (iii) a 1-month follow-up activity led by parents at home, both occurring from late January to late February 2021. The study used questionnaires and handwashing skill experiments to investigate the kindergarteners' handwashing practices and comprehensive handwashing skills (handwashing steps, handwashing time, rinsing time and areas of the hands left unwashed) before and after Activities 1, 2 and 3. Data were obtained from 56 kindergarteners (64.4%). Second and third graders showed a significant improvement in their handwashing practices after coughing or sneezing. With the exception of rinsing time, handwashing skills significantly improved in all grades after the 1-day teaching session. After 1-month follow-up activities, the number of areas left unwashed by first graders significantly decreased, and the score for handwashing steps significantly improved. This study indicated that continuous handwashing education is partially effective at improving and maintaining handwashing practices and skills, except for rinsing time, among kindergarteners of all grades.
Hand hygiene of medical students and resident physicians: predictors of attitudes and behaviour

Abstract:OBJECTIVES: We measured medical students' and resident trainees' hand hygiene behaviour, knowledge and attitudes in order to identify important predictors of hand hygiene behaviour in this population.METHODS: An anonymous, web-based questionnaire was distributed to medical students and residents at Stanford University School of Medicine in August of 2012. The questionnaire included questions regarding participants' behaviour, knowledge, attitude and experiences about hand hygiene. Behaviour, knowledge and attitude indices were scaled from 0 to 1, with 1 representing superior responses. Using multivariate regression, we identified positive and negative predictors of superior hand hygiene behaviour. We investigated effectiveness of interventions, barriers and comfort reminding others.RESULTS: 280 participants (111 students and 169 residents) completed the questionnaire (response rate 27.8%). Residents and medical students reported hand hygiene behaviour compliance of 0.45 and 0.55, respectively (p=0.02). Resident and medical student knowledge was 0.80 and 0.73, respectively (p=0.001). The attitude index for residents was 0.56 and 0.55 for medical students. Regression analysis identified experiences as predictors of hand hygiene behaviour (both positive and negative influence). Knowledge was not a significant predictor of behaviour, but a working gel dispenser and observing attending physicians with good hand hygiene practices were reported by both groups as the most effective strategy in influencing trainees.CONCLUSIONS: Medical students and residents have similar attitudes about hand hygiene, but differ in their level of knowledge and compliance. Concerns about hierarchy may have a significant negative impact on hand hygiene advocacy.

Abstract:BACKGROUND: Children's knowledge on hygiene is often considered to be provided by parents or schools. It is something with a direct impact on health and linked with socio-cultural factors. Primary care plays a vital role in health education and promotion, so focusing on hygiene habits is part of primary prevention.OBJECTIVES: Improving the knowledge on hygiene and promoting healthy behaviors in students from the 4th to the 6th grade.MATERIALS AND METHODS: Longitudinal, prospective study. Hygiene habits were assessed through a questionnaire, followed by a brief health education session organized by healthcare professionals at schools. Hygiene habits were then reassessed using the original questionnaire.RESULTS: A total of 812 questionnaires (354 in the pre-session phase and 458 in the post-session phase) were collected, filled in by students with ages ranging between 8 and 16 (average, 10.8), being 52% boys. There were significant increases in the score in both genders, but this increase was slightly higher for boys, with an average increase of 0.98 points (P < 0.05, independent samples t test). The 9 year old group (in the first evaluation; 10 years old post-talk) had the highest increase in score, with an average increase of 1.33 points in the questionnaire.CONCLUSION: Interventions conducted by healthcare professionals at schools have a positive effect for health education. The resulting scores revealed an improvement regarding hygiene habits namely in younger students. This may suggest that interventions on hygiene habits should take place at younger ages, so to have a greater impact in behavioral change.
