Abstract:Objective:To evaluate the value of S/D ratio of umbilical arteries (UA) and Z-scores of multiple ultrasound parameters in the diagnosis of fetal growth restriction (FGR) in pregnant women with gestational diabetes mellitus (GDM).Methods:Three thousand four hundred and nine GDM women with singleton pregnancy who underwent routine prenatal examination and delivery at Jinhua Maternal and Child Health Care Hospital from June 2016 to October 2021 were selected as research subjects. Pregnant women with both GDM and FGR were used included in an observation group (35 cases), and the other pregnant women served as a control group (3374 cases). Fetal biometric parameters including UA S/D ratio, growth parameters, and Z-scores of HC, AC, and HC/AC ratio of the two groups at 28 and 32 weeks of gestation were compared, and subsequently receiver operating characteristic (ROC) curves were produced and area under the curve (AUC) values were calculated.Results:BPD, HC, AC, and FL in the observation group at 28 and 32 weeks of gestation were significantly decreased compared to those of the control group, and HC/AC ratio was significantly increased compared to that of the control group (P＜0.05). The UA S/D ratio and Z-scores of HC/AC ratio in the observation group at 28 and 32 weeks of gestation were significantly increased compared to those of the control group, and the Z-score of AC was significantly decreased compared to that in the control group (P＜0.05). The AUC values of S/D ratio, Z-score of AC, and Z-score of HC/AC ratio in the diagnosis of FGR at 28 and 32 weeks of gestation were 0.820, 0.984, and 0.948, and 0.855, 0.941, and 0.981, respectively.Conclusion:The UA S/D ratio and Z-scores of multiple ultrasound biometric parameters can accurately and reliably evaluate the fetal growth and development, which can provide important information for the clinical diagnosis of FGR in pregnant women with GDM.
[Predictive Value of Umbilical Cord Blood Bilirubin Level for Subsequent Neonatal Jaundice].

Abstract:OBJECTIVE:To investigate the predictive value of umbilical cord serum (UCS) bilirubin for subsequent jaundice in healthy term newborns. METHODS:Five hundred and twenty-three healthy term newborns (275 boys, 248 girls) were selected. The cord blood total serum bilirubin concentration and the serum albumin concentration were determined. All the infants were assessed for jaundice daily by measurement of transcutaneous bilirubin (TCB). When the infant's TCB was >or= 18 within the first 24 h after birth, >or= 21 at 48 h, >or= 25 at or after 72 h, the venous total serum bilirubin (TSB) was determined and treatment against jaundice was applied as needed. The infants were aligned into four groups according to their UCS bilirubin levels, starting from < 30 micromol/L(group 1); >or= 30 micromol/L(group 2); >or= 36 micromol/L(group 3); >or= 42 micromol/L(group 4). The frequency of hyperbilirubinemia and phototherapy (PT) were compared among the four groups. An analysis of UCS bilirubin as a predictor of later development of jaundice was performed. The characteristics of the infants who became jaundiced (jaundiced group) were compared with the normal infants (non-jaundiced group). RESULTS:A clear correlation between UCS bilirubin level and the development of hyperbilirubinemia was found in all populations of the four groups. Only eight of the 194 infants in group 1 showed a TCB index >or= 25. TSB values > 205 micromol/L but < 257 micromol/L were observed in 2 newborns. None of the infants in this group showed TSB > 257 micromol/L or needed PT. Thirty-two infants in group 2 showed TCB >or= 25, 12 infants had TSB > 205 micromol/L but < 257 micromol/L, 2 infants had TSB > 205 micromol/L and received PT. In group 3, one infant developed hyperbilirubinemia at 48 h after birth and received PT. Thirty-nine infants showed TCB >or= 25, 16 infants TSB > 205 micromol/L but < 257 micromol/L, 2 infants had TSB > 205 micromol/L and also received PT. In group 4, 4 infants showed a range of TSB from 200 to 215 micromol/L at 48 h and received PT. Twenty-two infants showed TCB >or= 25, 17 of them showed TSB > 205 micromol/L but < 257 micromol/L, and 5 of them had TSB > 205 micromol/L and received PT. The frequency of TSB > 205 micromol/L increased from 1.03% in group 1, 5.77% in group 2, 19.75% in group 3 and to 42.5% in group 4. None of the 194 newborns in group 1 needed phototherapy, whereas 0.96%, 3.70% and 22.5% of the newborns in groups 2 - 4, needed PT. The frequency of patients with hyperbilirubinemia or phototherapy increased with increasing UCS bilirubin levels. For the prediction of TCB >or= 25 using a UCS bilirubin cut-off level, such as >or= 35 micromol/L, we found a positive predictive value of 45.68% and sensitivity of 68.27%. It is significant to predict neonatal jaundice by UCS bilirubin levels (P < 0.001). In the jaundiced group (TCB >or= 25) UCS bilirubin levels were significantly higher than those in the non-jaundiced group (t = 10.96, P < 0.001). No significant differences were found in the cord blood serum albumin concentration (t = 2.38, P > 0.05), the gestational age (t = -0.90, P > 0.05), and birthweight (t = 0.10, P > 0.05) between the jaundiced and non-jaundiced groups. CONCLUSIONS:UCS bilirubin level is useful in predicting the subsequent jaundice in healthy term infants. The use of UCS bilirubin values may help detect infants at low or high risk for hyperbilirubinemia and minimize an unnecessary prolongation of hospitalization.
Role of umbilical cord C-peptide levels in early prediction of hypoglycemia in infants of diabetic mothers

Abstract:Abstract Background Until now, diabetes during pregnancy has been associated with a high risk of maternal, fetal, and neonatal morbidities and mortalities. The main aim of this study was to evaluate the risk factors of hypoglycemia in infants of diabetic mothers (IDMs) and to study the relationship between umbilical cord (UC) C peptide levels and the risk of developing hypoglycemia. Material and methods UC blood C-peptide and serial serum blood glucose measurements were done for all included singleton newborns born to diabetic mothers during the study period. Maternal and neonatal data such as gestational age, maternal age, maternal weight, types of diabetics and its control, maternal glycated hemoglobin (HbA1C), birth weight, Apgar score, and neonatal complete blood picture were collected. Results In total, 83 IDMs met the inclusion criteria. Fifty-four (65.06%) developed hypoglycemia and 29 (34.94%) remained normoglycemic. However, there were no significant differences between hypoglycemic and normoglycemic IDMs in terms of types of maternal diabetics ( P value = 0.41), its duration (P value = 0.43). The hypoglycemia peak occurred within the first 3 h of life, with 33.11 ± 8.84 mg/dl for the hypoglycemia group and 54.10 ± 6.66 mg/dl for the normoglycemic group ( P value < 0.0001). Most of the babies had no hypoglycemic manifestation (96.30%). Neonates with hypoglycemia their mothers had poor diabetes control in the last trimester (HbA1C 7.09 ± 0.96%) compared to normoglycemic babies (HbA1C 6.11 ± 0.38%), ( P -value < 0.0001). The mean (SD) of UC C-peptide level in hypoglycemic neonates increased to 1.73 ± 1.07 ng/ml compared to normoglycemic ones with 1.08 ± 0.81 ng/ml ( P value = 0.005). Conclusion Poor diabetes control, especially in the last trimester, is associated with neonatal hypoglycemia. Increased UC C-peptide levels could be used as an early indicator for the risk of developing neonatal hypoglycemia and a predictor for babies need neonatal admission.

Abstract:objective To investigate the clinical significance of ultrasonic umbilical artery flow analysis in pregnaney with gestational diabetes mellitus(GDM).Methods Ultrasonic Doppler umbilical artery blood flowmeter was used to detect the systolic/diastolic blood pressure(S/D)ratio,pulse index,and resistance index of umbilical artery in 102 GDM women,aged(29±4),and 105 aged-matched normal pregnant women.The delivering and pefinatal outcomes were observed. Results The S/D ratio,pulse index.and resistance index of the GDM women were 2.55±0.52,1.03±0.23,and 0.59±0.68respectively,all significantly higher than those of the control group(2.11±0.22,0.81±0.13,and 0.52±0.47 respectively.all P＜0.01.The rates of premature rupture of fetal membrane,postpartum hemerrhage,fetal macrosomia,and bad outcome of perinatal infant in the GDM group were 12.7%,7.8%,17.7%,and 12.7%respectively,all significantly higher than those in the control group(1.9%,1.9%,6.7%,and 1.9%,all P＜0.05).The rates of premature labor,premature rupture of fetal membrane,postpartum hemorrhage,fetal macrosomia,and bad outcome of pefinatal infant of the GDM women with the S/D.S/D≥2.5(n=47)were 36.2%,21.3%,17.0%,55.3%,and 21.3%respectively,all significantly higher than those of the GDM women with the S/D＜2.5(n=55)12.7%,3.6%,1.8%,7.3%and 3.6%,(all P＜0.05).Conclusion Ultrasonic umbilical artery flow analysis in pregnancy with GDM is a good method to forecast the placenta function and perinatal outcomes.The poor perinatal outcome is associated with the S/D≥2.5 in GDM.
Cord Blood C-peptide, Insulin, HbA1c, and Lipids Levels in Small- and Large-for-Gestational-Age Newborns

Abstract:Background: Small-and large-for-gestational-age (SGA, LGA) newborns are associated with metabolic syndrome in their later life. Cord blood C-peptide, insulin, glycosylated hemoglobin (HbA1c), and lipids levels may be altered in SGA and LGA newborns; however, the results are conflicting. Therefore, this study aimed to determine the effect of cord blood markers on SGA and LGA newborns.Material/Methods: This was a prospective cohort study and included 2873 term newborns of non-diabetic women. Among these newborns, 83 (2.9%) were SGA, 2236 (77.8%) were appropriate-for-gestational-age (AGA), and 554 (19.3%) were LGA newborns. Cord blood C-peptide, insulin, HbA1c, triglyceride (TG), total cholesterol (TC), high-density lipoprotein cholesterol (HDL-C), and low-density lipoprotein cholesterol (LDL-C) levels were measured. The chi-square, Kruskal-Wallis, and Mann-Whitney tests were used to analyze categorical variables and continuous variables, respectively. Multinomial logistic regression analysis was used to determine the independent effect of these variables on SGA and LGA newborns.Results: Cord serum TG level was significantly higher in the SGA group than in AGA and LGA groups (p<0.05). The LGA group had significantly higher cord serum insulin level than AGA and SGA groups (p<0.05). After adjustment for confounding variables, including maternal age, parity, pre-pregnancy body mass index (BMI), education, annual household income, pregnancy-induced hypertension (PIH), mode of delivery, and newborn sex, high TG and insulin levels remained significantly associated with SGA and LGA newborns, respectively (p<0.05).Conclusions: High cord serum TG and insulin levels are independently associated with SGA and LGA newborns, respectively.
Correlation Between the First Trimester Two-Hour Postprandial Blood Glucose Greater Than 110 mg/dL for the Prediction of Gestational Diabetes Mellitus

Abstract:Aim: To determine the correlation between first-trimester two-hour postprandial blood glucose (PPBG) > 110 mg/dL for predicting gestational diabetes mellitus (GDM). Methods: This prospective cohort study enrolled 200 women between 8 and 10 weeks of gestation from February 2022 to February 2024. All recruited pregnant women underwent testing for two-hour PPBS at 8-10 weeks and were followed up till delivery. GDM screening was done during 14-16, 24-28, and 32-34 weeks of gestation. Results: Amongst women having PPBS > 110 mg/dL, 95.9% developed GDM, while in the group with PPBS < 110 mg/dL, only 4% developed GDM. In the PPBS > 110 mg/dL group, a significantly higher number of women were in the older age group (p < 0.049), had a higher BMI (p < 0.001), a family history of diabetes (p < 0.001), and previous history of abortion (p < 0.001). Women with PPBS > 110 mg/dL had significantly higher rates of cesarean section (p < 0.01), preterm delivery (p < 0.001), and macrosomia (p < 0.001). A positive correlation (r = 0.677; p < 0.001) was observed between first trimester two-hour PPBS and cord blood glucose levels. Similarly, a positive correlation (r = 0.465; p < 0.001) was present between insulin levels measured during the first trimester with cord blood insulin. The area under the curve (AUC) for PPBS was 0.969 (p < 0.001) with 95% CI: 0.933-0.988. PPBS > 110 mg/dL has a sensitivity of 95.9%, specificity of 95.6%, positive predictive value (PPV) of 95.9%, negative predictive value (NPV) of 95.7%, and diagnostic accuracy of 95.77% to predict GDM. Conclusion: PPBS > 110 mg/dL at two hours exhibits high levels of diagnostic accuracy for the prediction of GDM and is associated with adverse fetomaternal outcomes. PPBG is a superior, physiologic, and low-cost option compared to HbA1c for early prediction of GDM and can also be performed as a simple point-of-care test with a glucometer at home or in the periphery by healthcare workers (HCW) and in wellness centers.
Glycosylated hemoglobin level in diagnosis and management of gestational diabetes mellitus

Abstract:Objective To evaluate the value of glycosylated hemoglobin (HbAlc) level in glucose monitoring,insulin therapy and neonatal birth weight prognosis in gestational diabetes mellitus (GDM) patients.Methods Data of 1074 women who were diagnosed as GDM and delivered at Peking University First Hospital between January 1,2005 and August 31,2011 were collected and retrospective analysis was conducted.The relationship of HbAlc level with plasma glucose level of oral glucose tolerance test (OGTT),insulin administration and neonatal birth weight were investigated with Pearson correlation analysis,Logistic regression analysis or receiver operating characteristic (ROC) curve.Results OGTT was done on 1074 women with (31.8± 4.0) years old during the second and third trimester and diagnosed GDM at (27.6 ± 3.4) weeks of gestational age.Among them,glucose level of fasting and 1 and 2 hours after OGTT were (5.2 ± 0.7),(10.9± 1.4) and (9.4±1.5) mmol/L.The levels of HbAlcwere (5.57±0.48)％ at the fist time of HbAlc testing at (31.8±4.3) weeks of gestational age.(1)The average value of HbAlc in 726 women determined at one month after OGTT was (5.54±0.47)％,and increased from (5.34±0.41)％ in women with one abnormal item in OGTT to (5.47±0.41)％ and (5.71±0.46)％ in women with two or three abnormal items inOGTT (t=3.025 and 6.399,all P＜0.01).(2) HbAlc level in women received insulin therapy was higher than those who did not [(5.78±0.58) ％ vs (5.42±0.37) ％,t=9.431,P＜0.01].The sensitivity and specificity were 57.8％ and 75.5％ respectively in predicting insulin requirement in women with GDM at the cut-off value of 5.67％ for HbAlc (Logistics analysis:OR＝6.847,95％CI:4.588-10.218,P＜0.01).(3) Maternal HbAlc levelat the third trimester was higher in large for gestational age(LGA) group than that in non-LGA group [(5.75±0.52)％ vs (5.54±0.42)％,t=6.845,P＜0.01].The sensitivity and specificity were 44.8％ and 73.8％ respectively in predicting LGA at the cut-off value of 5.75％ for HbAlc (Logistics analysis:OR=2.187,95％CI:2.097-3.783,P＜0.01).(4) Maternal HbAlc level at the third trimester was higher in macrosomia group than that in non-macrosomia group [(5.88±0.53)％ vs (5.57±0.45)％,t=5.990,P＜0.01].The sensitivity and specificity were 50.4％ and 79.8％ respectively in predicting macrosomia at the cut-off value of 5.85％ for HbAlc (Logistics analysis:OR=3.299,95％CI:2.237-4.865,P＜0.01).Conclusions HbAlc level at the time of GDM diagnosis could imply the severity of disease and might predict insulin requirement.While maternal HbAlc level at the third trimester may predict the occurrence of LGA and macrosomia.
The Area under the Curve (AUC) of Oral Glucose Tolerance Test (OGTT) Could Be a Measure Method of Hyperglycemia in All Pregnant Women

Abstract:Objective: We adopted the area under the curve (AUC) of oral glucose tolerance test (OGTT) as a measure method of the severity of maternal hyperglycemia and investigated its relationship with adverse perinatal outcomes among women with and without gestational diabetes mellitus (GDM). Research design and methods: This is a retrospective cohort study. Our study group collected the medical records of 15,296 women who received perinatal care in 15 hospitals in Beijing and who delivered from July 1, 2013, to December 31, 2013. And several original articles on this cohort have been published. In this study, we analyze the relationship between AUC and adverse perinatal outcomes, so that in multiple pregnant cases, patients with pre-pregnancy diabetes, hypertension, and abnormal kidney function and those who did not receive a 75-g OGTT were excluded. A Chi-squared test and logistic regression analysis were used to determine the associations. Results: In total, 13,561 women were included. As the AUC of OGTT increased, the prevalence of macrosomia (odds ratio [OR] 1.059, 95% confidence interval [95% CI] 1.029 - 1.090, p < 0.001) and hypertensive diseases (OR 1.106, 95% CI 1.064 - 1.149, p < 0.001) also increased. For patients with same levels of AUC values, no significant differences in the risk of macrosomia, preterm birth and neonatal complications were observed between the GDM and non-GDM groups. Women with an AUC higher than 14.20 (mmol * h/L) had a higher risk of adverse outcomes regardless of the presence of GDM. Conclusions: The AUC could be a measure method of the severity of maternal hyperglycemia, and women with a high AUC should undergo aggressive management to avoid adverse outcomes regardless of the presence of GDM.
Predictive capacity of fetal pancreatic circumference for gestational diabetes mellitus

Abstract:Objective: To assess the capacity of fetal pancreatic size, before standard blood testing for screening and diagnosis, to predict maternal gestational diabetes mellitus (GDM). Methods: This was a retrospective cohort study of low-risk pregnant women recruited during routine second-trimester fetal anatomical screening at 20-25 weeks' gestation at two ultrasound units in Israel between 2017 and 2020. The predictive performance of fetal pancreatic circumference ≥ 80th and ≥ 90th centiles and glucose challenge test (GCT) was examined for the outcome of GDM. The independent-samples t-test was used to compare mean pancreatic circumference centile between pregnancies with GDM and those without GDM. Diagnostic performance was evaluated with 2 × 2 contingency tables and receiver-operating-characteristics (ROC) curves. Results: Overall, 195 women were selected for statistical analysis. Twenty-four (12.3%) women were diagnosed subsequently with GDM. The mean ± SD pancreatic circumference centile was significantly higher in the GDM group compared with the non-GDM group (82.4 ± 14.6 vs 62.8 ± 27.6; P < 0.001). The pancreatic circumference centile was correlated positively with the estimated fetal weight centile (Pearson's coefficient, 0.243; P = 0.001). The 80th centile cut-off for pancreatic circumference had the highest sensitivity (70.8%) and positive predictive value (23.3%) for future maternal GDM, with the best trade-off between sensitivity and specificity achieved at the 75th centile cut-off (sensitivity, 79%; specificity, 60%). The GCT had better specificity (90.2%) and negative predictive value (97.9%) compared with both cut-offs in pancreatic circumference. The area under the ROC curve was higher for pancreatic circumference compared with GCT (0.71 vs 0.64) and only the former was statistically significant (P = 0.001). Conclusions: Fetal pancreatic circumference has a higher positive predictive capacity compared with GCT. Measuring pancreatic circumference can identify pregnancies at high risk for maternal GDM, thereby promoting earlier diagnosis and treatment, decreasing the time period during which the fetus is exposed to high maternal glucose levels and improving infant outcome. © 2024 The Authors. Early blood glucose screening in asymptomatic high-risk neonates

Abstract:Objectives: Detecting and treating severe hypoglycemia promptly after birth is crucial due to its association with adverse long-term neurodevelopmental outcomes. However, limited data are available on the optimal timing of glucose screening in asymptomatic high-risk neonates prone to hypoglycemia. Risk factors associated with asymptomatic high-risk neonates include late prematurity ≥35 and <37 weeks gestation (LPT), small-for-gestational-age (SGA), large-for-gestational-age (LGA), and infant-of-a-diabetic mother (IDM). This study aims to determine the incidence and the impact of individual risk factors on early hypoglycemia (defined as blood glucose ≤25 mg/dL in the initial hour after birth) in asymptomatic high-risk neonates. Methods: All asymptomatic high-risk neonates ≥35 weeks gestation underwent early blood glucose screening within the first hour after birth (n=1,690). A 2-year retrospective analysis was conducted to assess the incidence of early neonatal hypoglycemia in this cohort and its association with hypoglycemia risk factors. Results: Out of the 9,919 births, 1,690 neonates (17 %) had risk factors for neonatal hypoglycemia, prompting screening within the first hour after birth. Incidence rates for blood glucose ≤25 mg/dL and ≤15 mg/dL were 3.1 and 0.89 %, respectively. Of concern, approximately 0.5 % of all asymptomatic at-risk neonates had a blood glucose value of ≤10 mg/dL. LPT and LGA were the risk factors significantly associated with early neonatal hypoglycemia. Conclusions: Asymptomatic high-risk neonates, particularly LPT and LGA neonates, may develop early severe neonatal hypoglycemia identified by blood glucose screening in the first hour of life. Additional investigation is necessary to establish protocols for screening and managing asymptomatic high-risk neonates.
Predictive Value of Early Pregnancy Fasting Blood Glucose for Development of Gestational Diabetes Mellitus

Abstract:Objective To evaluate the value of early pregnancy fasting plasma glucose(FPG) to predict gestational diabetes mellitus(GDM) in pregnant women with or without GDM risk factors. Methods From June 2013 to September 2014, 3 223 singleton healthy pregnant women in Peking University First Hospital were recruited and their demographic/medical information were collected using. Based on age, pre-pregnancy body mass index (BMI) and family history of diabetes, participants were grouped into GDM risk factors positive group (n=1 341) and GDM risk factors negative group (n=1 892). T-test andχ2 test were used for between-group comparison. Receiver operating characteristic (ROC) curve was used to evaluate the prediction of early pregnancy FPG for GDM. Results (1) Pregnant women with GDM risk factors had significantly higher FPG levels in early pregnancy compared to those without GDM risk factors ((5.1±0.4)vs (5.0±0.4) mmol/L, t=6.44, P<0.001).(2)Compared to women with FPG<4.75 mmol/L and those without GDM risk factors, women with GDM risk factors and with 4.75≤FPG<5.00 mmol/L, 5.00≤FPG<5.25 mmol/L and FPG≥5.25 mmol/L had a significant increased risk of developing GDM, the OR values were 1.84(95%CI 1.26-2.70),2.46 (95%CI 1.71-3.52) and 6.99 (95%CI 5.01-9.77) respectively;while in GDM risk factors negative group, only those with FPG≥5.25 mmol/L had a significant increased risk of developing GDM, and the OR value was 2.55 (95%CI 1.80-3.60). There was approximately a 1.6-fold increase (95%CI 1.45-1.98) in the risk of developing GDM for each 0.5 mmol/L increment in FPG in women without GDM risk factors, while a 2.33-flod increase (95%CI 1.96-2.61) in women with GDM risk factors. (3) Based on the ROC analysis, the area under the curve of early pregnancy FPG in predicting GDM for women with and without GDM risk factors was 0.694(95%CI 0.661-0.732) and 0.620 (95%CI 0.580-0.662),respectively. The optimal cut off value was 5.17 mmol/L and 5.09 mmol/L respectively. Conclusions Early pregnancy FPG has greater effect on predicting GDM in women with GDM risk factors. The optimal cut off values of FPG in predicting GDM are different between women with and without GDM risk factors.

Abstract:Objective To explore the feasibility of Guardian real-time continuous monitoring system (GRT) in high-risk infants with hypoglycemia.Methods The glucose of 72 infants admitted to Neonatal Intensive Care Unit (NICU) of Children's Hospital of Fudan University between May 2015 and December 2016 were detected by GRT (provided by Medtronic),there were 43 males and 29 females with gestational age [(35.25 ± 4.45) weeks],and birth weight [(2 385.69 ± 1 062.63) g].At the same time,the capillary glucose was monitored intermittently detected by using ACCU-CHEK(R).Results A total of 1 134 paired glucose levels were collected.A good correlation between the paired capillary[(5.23 ± 1.96) mmol/L] and GRT continuous monitor measurements [(5.19 ± 1.99) mmol/L] was found(r =0.88,P ＜ O.05),and the value of mean absolute difference (MAD) was 11.88％.Through the Clarke error grid analysis,there were 98.24 ％ points located in region A and region B,only 20 paired(1.76％) glucose located in region D.Hypoglycemia was defined as glucose concentration ＜ 2.6 mmol/L,while hyperglycemia was defined as glucose concentration ＞ 7.0 mmol/L.A high/low blood glucose detected by peripheral blood glucose monitoring is a high/low blood glucose event,and a high/low blood glucose detected by GRT in a continuous period is a high/low blood glucose event.Twelve cases of hypoglycemia,26 episodes of hypoglycemia,29 cases of hyperglycemia,and 151 episodes of hyperglycemia were detected by using fast blood glucose meter.Twenty-six cases of hypoglycemia,88 episodes of hypoglycemia,38 cases of hyperglycemia,and 229 episodes of hyperglycemia were detected by using GRT.Sixty-two episodes of hypoglycemia were not detected by the fast blood glucose meter,with 14 episodes lasting longer than 30 min,4longer than 60 min,and 5 longer than 90 min.There were 78 hyperglycemic episodes that were not detected by the fast blood glucose meter,with 44 episodes lasting longer than 30 min,15 longer than 60 min,and 70 longer than 90 min.There were significant differences in the testing of hypoglycemia and pathoglycemia(x2 =7.00,18.60;all P ＜ 0.05),but,there was no significant difference in the testing of hyperglycemia between the 2 kinds of detection(x2 =2.26,P ＞0.05).GRT was acceptable to parents,medical staffs and children as only 3 infants bled when the continuous glucose sensor was imbedded,but there was no progressive increase in blood loss.There was no redness,edema,infections,or effusions at the insertion sites.Conclusions The use of GRT continuous monitoring system in high-risk infants of hypoglycemia was feasible.Compared with the conventional intermittent peripheral blood glucose monitoring,more abnormal blood glucose events and their duration could be detected.
Blood Sugar Management in Delivery for Pregnant Women with Abnormal Glucose Metabolism Associated with Neonatal Hypoglycemia

Abstract:Objective To analyze the relationship between maternal blood glucose level and the incidence of neonatal hypoglycemia,and evaluate ways and significances of maternal blood glucose management of gestational diabetes mellitus(GDM)women during labor.Methods A retrospective analysis was conducted on 435 cases from August 2002 to October 2007 of singleton pregnancy with abnormal gestational glucose tolerance,whose blood glucose levels were monitored during labor,were subdivided into 206 cases with the gestational impaired glucose tolerance group(GIGT group),136 cases and 93 cases with the gestational diabetes mellitus A1(GDMA1 group)and A2 group(GDMA2 group), respectively.Results The morbidity of neonatal hypoglycemia in GIGT group,GDMA1 group and GDMA2 group was 2.9%,2.9% and 1.1%,respectively(P=0.602).Within the 3 gestational groups of abnormal glucose metabolism,no significant difference was found in the incidence of neonatal hypoglycemia between women with the blood glucose7.8 mmol/L and those with the blood glucose≥7.8 mmol/L occurring in labor without insulin treatment[the gestational impaired glucose tolerance group was 3% vs.1.3%;the gestational diabetes mellitus A1 group was 0% vs.1.8%;the gestational diabetes mellitus A2 group was 0% vs.0%(P0.05)].No correlation was detected between the immediate neonatal blood glucose value at birth and maternal mean blood glucose during labor in 3 groups(P= 0.260,0.861,0.296).Conclusion With strict glycemia control for pregnant women and appropriate management of newborns,maternal hyperglycemia of the women with gestational diabetes mellitus during labor has limited effect on the level and the incidence of neonatal hypoglycemia.Blood glucose assay and immediate feed for high risk neonates might have more clinical importance.

Abstract:Early intervention of gestational diabetes mellitus (GDM) is effective in reducing pregnancy disorders. Fetal growth, measured by routine ultrasound scan a few weeks earlier before GDM diagnosis, might be useful to identify women at high risk of GDM. In the study, generalized estimating equations were applied to examine the associations between ultrasonic indicators of abnormal fetal growth at 22–24 weeks and the risk of subsequent GDM diagnosis. Of 44,179 deliveries, 8324 (18.8%) were diagnosed with GDM between 24 and 28 weeks. At 22–24 weeks, fetal head circumference (HC) < 10th, fetal femur length (FL) < 10th, and estimated fetal weight (EFW) < 10th percentile were associated with 13% to 17% increased risks of maternal GDM diagnosis. Small fetal size appeared to be especially predictive of GDM among women who were parous. Fetal growth in the highest decile of abdominal circumference (AC), HC, FL and EFW was not associated with risk of subsequent GDM. The observed mean difference in fetal size across gestation by GDM was small; there was less than 1 mm difference for AC, HC, and FL, and less than 5 g for EFW before 24 weeks. Despite similar mean fetal growth among women who were and were not later diagnosed with GDM, mothers with fetuses in the lowest decile of HC, FL and EFW at 22–24 weeks tended to have higher risk of GDM.

Abstract:Neonatal hypoglycemia (NH) is the most common metabolic disturbance affecting newborns, and has been linked with poor academic performance [1]. Many knowledge gaps persist regarding its definition, long-term impact, screening and management, and proper measurement and monitoring of glucose concentrations [2]. Current hypoglycemia screening involves intermittent glucose measurement using point-of-care (POC) glucometers. Samples are obtained every few hours per NH screening protocols. Additional painful needle sticks are performed for (1) laboratory confirmation of low POC values, (2) serial pre-prandial testing, and (3) placement of intravenous catheters for parenteral treatment of NH. Continuous glucose monitoring (CGM), in contrast, holds promise as a less painful method of evaluating glucose concentrations continuously in order to identify dysglycemia and improve family-centered care and parental satisfaction. It requires only one needle stick during CGM sensor placement and reports interstitial glucose concentrations every 5 min. Repeated and prolonged hypoglycemia as well as glycemic instability are associated with adverse neurological outcomes; [2] these episodes cannot be fully evaluated with current intermittent POC technology. Thus, CGM has the potential for increased detection of these potentially harmful episodes, which will enable timely treatment. The single center REACT feasibility pilot trial evaluating the use of CGM in very preterm infants reported low bias (−4.9 mg/dL, 95% CI −6.3 to −3.4 mg/dL, n = 20) when the CGM was calibrated every 12 h [3, 4]. An agreement study of glucose concentrations between CGM (that does not require calibration) and intermittent values in at-risk late preterm and term newborns, who represent the majority of newborns with hypoglycemia, however, is lacking. Thus, our pilot study aimed to assess the accuracy of CGM in at-risk newborns.

Abstract:Background: Temporary low plasma glucose concentrations are common in healthy newborns. Although there is no uniform definition of neonatal hypoglycemia, there is a consensus in the current literature that plasma glucose concentrations should be measured in infants at risk. Known risk groups for transient neonatal hypoglycemia include infants of diabetic mothers (IDM), large (LGA) or small (SGA) for gestational age and late preterm (LPT) infants.Objectives: The aim of this retrospective trial was to determine the incidence of hypoglycemia and the impact of the application of a 2011 revised guideline in respect of additional feeding or i.v. glucose administration, admission to a neonatal ward and the number of blood samples taken.Methods: During the period 1 January 2015 to 31 January 2016, the plasma glucose concentrations of all infants at risk were determined. They were screened over a period of 24 hours or until plasma glucose concentration was >45 mg/dL on three occasions. Hypoglycemia was defined as a plasma glucose concentration <40 mg/dL, regardless of the age of the infant.Results: One hundred and thirty-six (13.6%) out of 1017 newborns were identified as at-risk patients, 119 (87.5%) of whom were included in the final data evaluation. Ten study participants had more than one risk factor and 32 (26.9%) newborns (male:female = 1.1:1) had a total of 40 hypoglycemic episodes. Three (9.4%) out of these 32 newborns had to be transferred to the neonatal ward for i.v. glucose treatment. The mean number of blood samples taken was 7.6 ± 2.4.Conclusions: The incidence of hypoglycemia in the studied infants at risk was 27%, and 19.7 blood samples had to be taken to detect one episode of low glucose concentration. Neonatal hypoglycemia can be recognized and avoided in time, which justifies the establishment of a standardized plasma glucose measurement protocol in newborn infants at risk.Brief RationaleFollowing a considerable number of sources, it is recommended that infants at risk be identified, low plasma glucose concentrations prevented and, if necessary, the affected neonates cared for. Our data show that the risk group for neonatal hypoglycemia comprised about one-tenth of all infants at our nursery and hypoglycemia occurred in one-fourth. These results are in accordance with the recommendations to implement this protocol as a screening tool in neonates.
Evaluating predictive values of umbilical cord arterial lactate for adverse newborn outcomes among term‐births in northern Uganda: A cross sectional analytical study

Abstract:Objective Birth asphyxia is one of the leading causes of death for neonates worldwide. Lack of an objective cost effective test to predict poor newborn outcomes at birth affects the ability to respond appropriately. This study determined predictive values of umbilical cord arterial lactate in relation to adverse neonatal outcomes. Methods This was a cross‐sectional analytical study conducted between March 2018 and March 2019 at two hospitals in Northern Uganda. A total of 2655 women admitted for birth and their newborns were recruited. At birth, umbilical cord arterial blood was tested for lactate using the Nova Biomedical StatStrip Xpress meter. Apgar scores were assessed at 5 min by trained research midwives. Area under the receiver operator characteristics curve (AUROC) was calculated relating umbilical arterial lactate (UAL) levels and four outcomes. We modeled the best lactate cutoff level associated with the highest AUROC for the four outcomes. Results The estimated AUROC for lactate was: 88.42% for Apgar score <7 at 5 min, 83.35% for resuscitation with bag and mask, 84.55% for oxygen therapy after resuscitation and 87.72% for admission to neonatal care unit. The UAL cutoff value of 5.5 mmol/L was associated with the best AUROC of between 75.81% to 81.75% for the four adverse outcomes with no significant differences when adjusted for infectious disease parameters. The sensitivity, specificity, PPV, and NPV were; 78.95%, 86.48%, 23.54%, and 98.73% for Apgar scores <7 at 5 min, 64.40%, 88.11%, 36.59%, and 95.87% for resuscitation with bag and mask, 67.17%, 87.20%, 30.23%, and 96.99% for oxygen therapy after resuscitation, and 77.17%, 86.15%, 22.27%, and 98.65% for admission to the special care unit, respectively. Conclusion Umbilical cord lactate point‐of‐care (POC) estimate of ≥5.5 mmol/L predicts adverse neonatal outcomes. This test may be used to trigger early interventions and intensified neonatal care complementing the clinical Apgar score assessment in settings like Uganda.

Abstract:Background: Glycated albumin (GA) levels have been considered as a promising biomarker for estimating glycemic control during pregnancy, but the relationship between GA levels and the incidence of adverse pregnancy outcomes in women with gestational diabetes mellitus (GDM) remains unclear. Our study aimed to investigate the relationship between GA levels during the third trimester and 13 different adverse pregnancy-related outcomes among women with GDM in China. Methods: We retrospectively extracted clinical data from the medical records of 819 pregnant women with GDM who underwent prenatal examinations and child delivery at the Affiliated Hospital of Qingdao University between January 2022 and October 2022. The cohort was divided into GA-high (GA-H) and GA-low (GA-L) groups based on the median GA level of 10.6%. Then, the incidence rates of 13 specific adverse pregnancy outcomes were compared between the two groups. Furthermore, we estimated the mean GA levels in pregnant GDM women with or without specific adverse outcomes. Multivariate logistic regression analysis was performed to assess whether the GA levels (high or low) were independent risk factors for specific adverse outcomes in pregnant women with GDM. Receiver operating characteristic (ROC) curve analysis was performed to assess the predictive value of GA levels for the adverse pregnant outcomes in women with GDM. E-value for sensitivity analysis was performed to assess the robustness of the findings to unmeasured confoundings. Results: We included 819 pregnant women with GDM, whose average age was 33.09 ± 4.47 years, average pre-pregnancy BMI was 23.51 ± 3.67 kg/m2, and the average gestational week in which GDM diagnosed was 24.80 ± 1.79 weeks. The analysis showed that 80.71% (661/819) pregnant women with GDM were associated with adverse pregnancy-related outcomes. Pregnant women in the GA-L group showed higher incidence of the premature rupture of membranes (PROM), whereas those in the GA-H group showed higher incidence of neonatal hypoglycemia. The GA levels showed acceptable clinical performance for predicting neonatal hypoglycemia with an area under the ROC curve (AUC) value of 0.700 (P = 0.010), sensitivity of 71.4%, and specificity of 70.2%. The optimal cut off value for GA was 11.55%. Conclusions: This study demonstrated that GA levels were significantly associated with specific adverse pregnancy outcomes, especially PROM and neonatal hypoglycemia. Furthermore, GA levels in the third trimester showed acceptable clinical performance for predicting neonatal hypoglycemia among pregnant women with GDM. In the future, the potential role of GA as a predictor of adverse pregnancy outcomes need to be further confirmed and explored in GDM women.
Application of glycemic qualification rate based on fingerstick glucose monitoring in women with gestational diabetes mellitus

Abstract:To explore the appropriate application of glycemic qualification rate (GQR) calculated by fingerstick blood glucose (BG) monitoring for patients with gestational diabetes mellitus (GDM) by analyzing the relationship between BG control and adverse pregnancy outcomes. Fingerstick Blood Glucose data during the second and third trimester of singleton pregnant women diagnosed with GDM were collected. GQR which is defined as the percentage of fingerstick BG values reaching the targets of BG control in a period of time was calculated. Patients were divided into three groups according to tertiles (tertile 1, GQR <56.25%; tertile 2, GQR 56.25–75%; and tertile 3, GQR ≥75%). Pregnant outcomes were compared among the three groups. Univariate analysis and logistic regression were performed to analyze the potential relationship between GQR and pregnancy outcomes. Receiver operating characteristic (ROC) curves were calculated to determine the cutoff values. We also explored that whether twice or three times monitoring per day would be adequate for GQR calculation, so we brought in two or three glucose measuring times per day to explore the relationship between new GQR and adverse outcomes. A total of 311 patients diagnosed with GDM were analyzed. In univariate analysis, the incidences of cesarean section of tertile 1–3 groups were 61.4%, 58.7%, and 44.9%, respectively ( p < .05). The incidences of neonatal hypoglycemia of tertiles 1–3 groups were 19.8%, 18.6%, and 8.7% ( p < .05). The difference of composite outcomes was statistically significant ( p = .001). After adjustment, the patients with worse BG control (lower GQR) had higher risk of cesarean section (tertile 1 – aOR = 2.029, 1.128–3.648), neonatal hypoglycemia (tertile 1: aOR = 2.498, 1.082–5.766) as well as composite outcomes. The ROC curve of GQR indicated the predictive value for neonatal hypoglycemia (area under the ROC curve (AUC) 0.612 (0.532–0.692)) and neonatal composite outcomes (AUC 0.593 (0.528–0.657)) with optimal cutoff values of 81.1% and 73.5%, respectively. We also explored that whether twice or three times monitoring per day would be adequate for GQR calculation. The result showed that GQR only calculated by FBG + 2hPG after lunch (2h AL) per day also had well relationship with cesarean section (tertile 1: OR = 2.412, 1.322–4.398), neonatal hypoglycemia (tertile 1: aOR = 4.497, 1.607–12.586), and neonatal composite outcomes (tertile 1: aOR = 1.959, 95% confidence interval (CI): 1.114–3.444, p = .020). The GQR calculated by the easily applicable fingerstick BG is related to occurrence of cesarean section and neonatal hypoglycemia in GDM women. GQR ≥ 80% is recommended for better pregnancy outcomes. As for the number of points monitoring per day, GQR calculated by FBG + 2h AL was an optimal option for better pregnancy outcomes if mothers needed to simplify the process of monitoring.

Abstract:Background Continuous glucose monitoring (CGM) has shown potential in improving maternal and neonatal out-comes in individuals with type 1/2 diabetes, but data in gestational diabetes mellitus (GDM) is limited. We aimed to explore the relationship between CGM-derived metrics during pregnancy and pregnancy outcomes among women with GDM.Methods We recruited 1302 pregnant women with GDM at a mean gestational age of 26.0 weeks and followed them until delivery. Participants underwent a 14-day CGM measurement upon recruitment. The primary outcome was any adverse pregnancy outcome, defined as having at least one of the outcomes: preterm birth, large-for-gestational-age (LGA) birth, fetal distress, premature rupture of membranes, and neonatal intensive care unit (NICU) admission. The individual outcomes included in the primary outcome were considered as secondary outcomes. We conducted multivariable logistic regression to evaluate the association of CGM-derived metrics with these outcomes.Findings Per 1-SD difference in time above range (TAR), glucose area under the curve (AUC), nighttime mean blood glucose (MBG), daytime MBG, and daily MBG was associated with higher risk of any adverse pregnancy outcome, with odds ratio: 1.22 (95% CI 1.08-1.36), 1.22 (95% CI 1.09-1.37), 1.18 (95% CI 1.05-1.32), 1.21 (95% CI 1.07-1.35), and 1.22 (95% CI 1.09-1.37), respectively. Time in range, TAR, AUC, nighttime MBG, daytime MBG, daily MBG, and mean amplitude of glucose excursions were positively associated, while time blow range was inversely associated with the risk of LGA. Additionally, higher value for TAR was associated with higher risk of NICU admission. We further summarized the potential thresholds of TAR (2.5%) and daily MBG (4.8 mmol/L) to distinguish individuals with and without any adverse pregnancy outcome.Interpretation The CGM-derived metrics may help identify individuals at higher risk of adverse pregnancy outcomes. These CGM biomarkers could serve as potential new intervention targets to maintain a healthy pregnancy status among women with GDM.
