Abstract:Utilizing a suitable combination of lactide and glycolide in a copolymer would optimize the degradation rate of a scaffold upon implantation in situ. Moreover, 3D printing technology enables customizing the shape of the scaffold to biometric data from CT and MRI scans. A previous in vitro study has shown that novel 3D-printed poly-lactic-co-glycolic acid (PLGA) scaffolds had good biocompatibility and mechanical properties comparable with human cancellous bone, while they could support proliferation and osteogenic differentiation of osteoblasts. Based on the previous study, this study evaluated PLGA scaffolds for bone regeneration within a rabbit model. The scaffolds were implanted at two sites on the same animal, within the periosteum and within bi-cortical bone defects on the iliac crest. Subsequently, the efficacy of bone regeneration within the implanted scaffolds was evaluated at 4, 12 and 24 weeks post-surgery through histological analysis. In both the intra-periosteum and iliac bone defect models, the implanted scaffolds facilitated new bone tissue formation and maturation over the time course of 24 weeks, even though there was initially observed to be little tissue ingrowth within the scaffolds at 4 weeks post-surgery. Hence, the 3D-printed porous PLGA scaffolds investigated in this study displayed good biocompatibility and are osteoconductive in both the intra-periosteum and iliac bone defect models.

Abstract:INTRODUCTION: Dengue fever is a viral disease transmitted by the Aedes aegypti Linn. (1792) (Diptera: Culicidae) mosquito, which is endemic in several regions of Brazil. Alternative methods for the control of the vector include botanical insecticides, which offer advantages such as lower environmental contamination levels and less likelihood of resistant populations. Thus, in this study, the ability of botanical insecticide formulations to inhibit the activity of the liver enzymes serum cholinesterase and malate dehydrogenase was evaluated. METHODS: Inhibition profiles were assessed using in vitro assays for cholinesterase and malate dehydrogenase activity and quantitated by ultraviolet-visible spectroscopy at 410nm to 340nm. RESULTS Insecticide products formulated from cashew nutshell liquid [A] and ricinoleic acid [B] showed cholinesterase activity levels of 6.26IU/mL and 6.61IU/mL, respectively, while the control level for cholinesterase was 5-12IU/mL. The products did not affect the level of 0.44IU/mL established for malate dehydrogenase, as the levels produced by [A] and [B] were 0.43IU/mL and 0.45IU/mL, respectively. CONCLUSIONS Our findings show that in vitro testing of the formulated products at concentrations lethal to A. aegypti did not affect the activity of cholinesterase and malate dehydrogenase, indicating the safety of these products.
Comparison Study Of Three Hydroxyapatite-Based Bone Substitutes In A Calvarial Defect Model In Rabbits

Abstract:Purpose: The aim of this study was to investigate the in vivo performance, including bone regeneration and biomaterial degradation, of three hydroxyapatite-based bone substitutes at surgically created defects in the cranium of rabbits. Materials and Methods: A bovine-derived hydroxyapatite (BHA), a synthetic hydroxyapatite (SHA), and a nano-hydroxyapatite with collagen I (nHAC) composite were characterized and then placed in calvarial defects in rabbits. After surgery at 6 weeks and 12 weeks, the animals were sacrificed for further microcomputed tomography, histologic, and histomorphometric analyses. The amounts of newly formed bone, residual grafting materials, connective tissues, and tartrate-resistant acid phosphatase staining positive multinucleate cells were evaluated. Results: The three hydroxyapatite-based bone substitutes displayed different surface topography. BHA had the highest amount of newly formed bone at both 6 weeks and 12 weeks, yet no significant difference was found between the three groups at 6 weeks. The amount of residual materials was significantly higher in the SHA group than the other groups at both time points. The amount of connective tissue was highest in BHA. More multinucleated cells were observed in the nHAC group. Conclusion: The three tested bone substitutes are suitable for grafting defects related to early bone healing. BHA had a better impact on bone formation with the prolongation of time. An integration of collagen type I and nano-hydroxyapatite may not improve bone regeneration of pure synthetic hydroxyapatite but helps to modify the degradation rate of pure hydroxyapatite.

Abstract:Using artificial biomaterials in bone regenerative medicine for highly efficient osteoconduction into the bone defect to decrease the bone healing time is still a challenge. In this research, magnesium (Mg)-doped wollastonite (similar to 10% Mg was substituted for calcium (Ca) in beta-CaSiO3) (CSi-Mg10) bioceramic scaffolds with ultrahigh mechanical strength were fabricated using ceramic ink writing three dimensional (3D) printing. To evaluate the potential of other additives on the new bone regeneration efficiency, beta-tricalcium phosphate (beta-TCP) was introduced to the CSi-Mg10 ceramic ink at a concentration of 15% and the biphasic bioceramic scaffolds (CSi-Mg10/TCP15) were also fabricated using 3D printing. The mechanical characterization indicated that introduction of beta-TCP led to nearly 50% mechanical decay, although the effect of the two heating schedules (one- and two-step sintering) on the compressive and flexural strengths of the scaffolds was significantly different. The bone regeneration results in critical sized calvarial defect of rabbits showed that the CSi-Mg10/TCP15 scaffolds displayed a markedly higher osteogenic capability than those on the CSi-Mg10 and beta-TCP scaffolds after eight weeks, and reached similar to 35% new bone tissue regeneration at 12 weeks postoperatively. These findings demonstrate that the CSi-Mg10/TCP15 bioceramic scaffolds can be well suited for stimulating in situ bone regeneration and for use in tissue engineering applications.
[A Study on the Repair of Bone Defects with Deproteinized Bone Surrounded by Titanium Mesh and Osteoblasts].

Abstract:To investigate the ability of composite graft of osteobalsts and deproteinized bone-titanium mesh (DPB-TM) scaffold to repair cranial bone defect. 30 rabbits were randomly divided into 3 groups. The passage 3 fetal rabbit osteoblasts were seeded into porous DPB-TM scaffolds at the density of 5 x 10(6) ml(-1) as the experimental group. The same defects were respectively reconstructed by DPB-TM or osteoblasts as the control groups. After 12 weeks, the result was evaluated by three-dimensional computed tomographic scanning, gross inspection, scanning electron microscopy, histological examination and mechanics test, respectively. In the experimental group, bone trabecula was observed to pass the defect and interface was mixed. No demarcation between the region of the bone defect and the normal bone was observed. There was plenty of new bone on the scaffold. Part of the scaffold was absorbed. In view of mechanics, the intensity of artificial bone (18.93+/-1.12 MPa) was higher than that of normal bone (16.96+/-1.60 MPa) (P<0.05). In the control groups, only fibrous tissue was observed in the defect region, there was no new bone formation. The tissue engineering bone constructed by osteoblasts and DPB-TM scaffold can be applied to the repair of bone defect.

Abstract:This study investigates the effect of scaffold architecture on bone regeneration, focusing on 3D-printed polylactic acid–bioceramic calcium phosphate (PLA-bioCaP) composite scaffolds in rabbit femoral condyle critical defects. We explored two distinct scaffold designs to assess their influence on bone healing and scaffold performance. Structures with alternate (0°/90°) and helical (0°/45°/90°/135°/180°) laydown patterns were manufactured with a 3D printer using a fused deposition modeling technique. The scaffolds were meticulously characterized for pore size, strut thickness, porosity, pore accessibility, and mechanical properties. The in vivo efficacy of these scaffolds was evaluated using a femoral condyle critical defect model in eight skeletally mature New Zealand White rabbits. Then, the results were analyzed micro-tomographically, histologically, and histomorphometrically. Our findings indicate that both scaffold architectures are biocompatible and support bone formation. The helical scaffolds, characterized by larger pore sizes and higher porosity, demonstrated significantly greater bone regeneration than the alternate structures. However, their lower mechanical strength presented limitations for use in load-bearing sites.

Abstract:BACKGROUND: The ideal bone repair materials must be biocompatible and absorbable, have the porosity that is conducive to the vascularization and rapidly replaced by new tissue, as well as a three-dimensional structure similar to bone tissue. OBJECTIVE: To test the effect of poly(lactic-co-glycolic acid)/tricalcium phosphate (PLGA/TCP) scaffolds prepared by rapid prototyping technology loaded with bone morphogenetic protein in the repair of radial defect in rabbits. METHODS: The lactic acid/glycolic acid copolymer dissolved in 1,4-dioxane and mixed with calcium phosphate powder to prepare a liquid slurry, then placing into a biomaterial rapid prototyping machine TissForm™, thus a cylindrical artificial bone carrier at the diameter of 5 mm and length of 15 mm was obtained. According to the standard of 15 mg in each material, active artificial bone materials were prepared by use of pre-wet, negative pressure of bone morphogenetic protein, freeze-drying. Twenty New Zealand rabbits were used to prepare a 15-mm radial defect, which was treated by the implantation of active artificial bone loaded with and without bone morphogenetic protein. Then results of imaging, histology, scaffolds degrade rates and bone mineral density was appraised to examine the repairing effects of the scaffolds. RESULTS AND CONCLUSION: At 12 weeks, all defects in the experimental group were radiographically repaired. New bony callus was observed to connect with the defect ends and then mould, the scaffold was nearly completely absorbed, and no significant differences were observed compared with control group. PLGA/TCP scaffolds prepared by rapid forming technology loaded with bone morphogenetic protein can effectively repair the 15-mm long bone defects in rabbits, its degradation rate well matched the bone formation rate.
Biodegradable 3D printed HA/CMCS/PDA scaffold for repairing lacunar bone defect

Abstract:Three-dimensional (3D) printing technology has attracted considerable focus for preparing porous bone repair scaffolds to promote bone regeneration. Inspired by organic-inorganic components and the porous structure of natural bone, novel porous degradable scaffolds have been printed using hydroxyapatite (HA), carboxymethyl chitosan (CMCS), and polydopamine (PDA). The well-designed HA/CMCS/PDA scaffolds exhibited a porous structure with 60.5 ± 4.6% porosity and 415 ± 87 μm in mode pore diameter. The weight loss percentage (WL%) of the HA/CMCS/PDA scaffolds reached about 17% during a 10-week degradation in vitro. The degradation process between the CMCS and HA induced the release of calcium ions. Using commercial product as the contrast material, the osteogenic properties of the scaffolds were assessed in vivo. The implantation and degradation of HA/CMCS/PDA scaffolds had no adverse effects on the kidney and liver of rabbits with no inflammatory response in the implantation sites. The micro-CT and histology data suggested that the HA/CMCS/PDA scaffolds could effectively stimulate new bone formation within the femoral lacuna defect region of rabbits versus blank control at 12 weeks after implantation. Surface cortical bone was generated in the defect area in the HA/CMCS/PDA group; the defect in the blank group remained obvious. HA/CMCS/PDA scaffolds had excellent biodegradability matching the formation of new bone during implantation. In conclusion, 3D-printed HA/CMCS/PDA scaffolds have remarkable potential as a new material for repairing bone defects.
Experimental Research on Repairing Long Bone Defects of Rabbit with Large Segments of Simulated-Bioactive Bone Substitute

Abstract:Objective To observe the effects of large segments of simulated-bioactive bone substitutes made from PLLA (poly-left-lactic acid), TCP (tricalcium phosphate) and rhBMP-2 (recombinant human bone morphogenetic protein 2) on repairing long bone defects of radius in rabbits. Methods Large segments of PLLA-TCP porous carrier were made with a rapid forming machine. Then rhBMP-2 was compounded through negative pressure to produce large segments of simulated-bioactive bone substitute. 36 New Zealand rabbits were divided into 3 groups randomly. Operations were done to make a 15 mm bone defect at the middle-superior segment of right radius in each rabbit. In the experiment group, a large segment of simulated-bioactive bone substitute was implanted into the defect; in the control group A, a large segmental carrier was implanted; in control group B, nothing was implanted. The results were evaluated by radiograph, histology and BMD (bone mineral density), and the degrading rate of the carrier was analyzed by an image analysis system. Results (1) Radiography: In the experiment group, by 12 weeks after operation, the defect was repaired by newly-formed callus, and the repairing rate was 100%. By 24 weeks postoperatively, the callus molded well, and the medullary cavity formed. In the 2 control groups, by 24 weeks after operation, no callus formed and the defects were not repaired. (2) Histology: In the experiment group, by 12 weeks after operation, the outer layer of callus was cortical bone, and the central part was the carrier where there were bone islands. By 24 weeks, medullary cavity formed between cortical bone and the carrier. (3) The BMD of the new callus in 12 weeks and 24 weeks after operation were 70.6%and 96.8%of the normal radius. (4) In 12 weeks postoperatively, the degrading rate of the carrier was 38.3%in the experiment group, 31.4%in the control group. In 24 weeks, they were 54.2%and 43.4%respectively. Conclusion The large segment of simulated-bioactive bone substitute has satisfactory effects on repairing the bone defects.
Three-Dimensional-Printed Spherical Hollow Structural Scaffolds for Guiding Critical-Sized Bone Regeneration

Abstract:The treatment of bone tissue defects continues to be a complex medical issue. Recently, three-dimensional (3D)-printed scaffold technology for bone tissue engineering (BTE) has emerged as an important therapeutic approach for bone defect repair. Despite the potential of BTE scaffolds to contribute to long-term bone reconstruction, there are certain challenges associated with it including the impediment of bone growth within the scaffolds and vascular infiltration. These difficulties can be resolved by using scaffold structural modification strategies that can effectively guide bone regeneration. This study involved the preparation of biphasic calcium phosphate spherical hollow structural scaffolds (SHSS) with varying pore sizes using 3D printing (photopolymerized via digital light processing). The chemical compositions, microscopic morphologies, mechanical properties, biocompatibilities, osteogenic properties, and impact on repairing critical-sized bone defects of SHSS were assessed through characterization analyses, in vitro cytological assays, and in vivo biological experiments. The results revealed the biomimetic properties of SHSS and their favorable biocompatibility. The scaffolds stimulated cell adhesion, proliferation, differentiation, and migration and facilitated the expression of osteogenic genes and proteins, including Col-1, OCN, and OPN. Furthermore, they could effectively repair a critical-sized bone defect in a rabbit femoral condyle by establishing an osteogenic platform and guiding bone regeneration in the defect region. This innovative strategy presents a novel therapeutic approach for assessing critical-sized bone defects.

Biomechanical Effects of 3D-Printed Bioceramic Scaffolds With Porous Gradient Structures on the Regeneration of Alveolar Bone Defect: A Comprehensive Study

Abstract:In the repair of alveolar bone defect, the microstructure of bone graft scaffolds is pivotal for their biological and biomechanical properties. However, it is currently controversial whether gradient structures perform better in biology and biomechanics than homogeneous structures when considering microstructural design. In this research, bioactive ceramic scaffolds with different porous gradient structures were designed and fabricated by 3D printing technology. Compression test, finite element analysis (FEA) revealed statistically significant differences in the biomechanical properties of three types of scaffolds. The mechanical properties of scaffolds approached the natural cancellous bone, and scaffolds with pore size decreased from the center to the perimeter (GII) had superior mechanical properties among the three groups. While in the simulation of Computational Fluid Dynamics (CFD), scaffolds with pore size increased from the center to the perimeter (GI) possessed the best permeability and largest flow velocity. Scaffolds were cultured in vitro with rBMSC or implanted in vivo for 4 or 8 weeks. Porous ceramics showed excellent biocompatibility. Results of in vivo were analysed by using micro-CT, concentric rings and VG staining. The GI was superior to the other groups with respect to osteogenicity. The Un (uniformed pore size) was slightly inferior to the GII. The concentric rings analysis demonstrated that the new bone in the GI was distributed in the periphery of defect area, whereas the GII was distributed in the center region. This study offers basic strategies and concepts for future design and development of scaffolds for the clinical restoration of alveolar bone defect.

Abstract:To examine the effect of bone tissue engineering material constructed via 3D printed nanohydroxyapatite/silk fibroin/polycaprolactone (nHA/SF/PCL) artificial bone scaffolds and doubletransfected BMP-2/VEGF mesenchymal stem cells in repairing rabbit radial bone defects, 60 New Zealand rabbits were randomly selected, and with the establishment of a 15 mm bone defect model upon the middle of the anterior right radius bone. The rabbits were randomly divided into 4 groups, with each group composed of 15 rabbits: Group A (nHA/SF/PCL BMP-2/VEGF), Group B (simple nHA/SF/PCL), Group C (simple BMP-2/VEGF), Group D (blank control). Our team recorded the eating, movement, redness, exudation, and so forth across each sample group following the operation. The bone protein expression levels were measured via western blotting following 12 weeks post-operation. Histological observation was carried performed via tissue sections. Following the operation procedure, the wound healed healthily across the 4 experimental groups, no serious infections occurred, only a few rabbits exhibited redness, exudation, pus, and other phenomena, and there was no significant difference present in each group ( P > 0.05). The A group took the shortest time to recover its autonomous activity ability, followed by the B group, and the D group took the longest time to recover its autonomous activity ability. From western blotting tests, the A group possessed the highest BMP-2 and OCN expression levels, and have significant difference existed with the B, C, and D group ( P < 0.05). The order of expression level of BMP-2 and OCN across the other three groups were group C > group B > group D, respectively. The results of a histological section performed at 12 weeks post-operation revealed that there was no immune response present and no adverse inflammatory reaction occurring within each experimental group. Among the rabbits, the A group exhibited an apparent plate layer new bone formation, bone trabeculae were arranged regularly, and there was no obvious space within the new bone tissue; furthermore, the repair of the bone defect was completed. The B group still exhibited some bone scaffold remaining within the implantation area, and there was a small gap between the new bone tissue, and several bone defects had not been repaired. The C group of bone defects presented only a small number of new bone formation, and bone defect repair effect was not obvious. The D group still possessed large bone defects and exhibited poor repair of bone defects. The nHA/SF/PCL composite of artificial bone scaffold and double-transfected BMP-2/VEGF mesenchymal stem cells was effective in repairing radial bone defects within rabbits, and was superior to using nHA/SF/PCL bone scaffolds alone.
3D-printed HA15-loaded β-Tricalcium Phosphate/Poly (Lactic-co-glycolic acid) Bone Tissue Scaffold Promotes Bone Regeneration in Rabbit Radial Defects

Abstract:In this study, a β-tricalcium phosphate (β-TCP)/poly (lactic-co-glycolic acid) (PLGA) bone tissue scaffold was loaded with osteogenesis-promoting drug HA15 and constructed by three-dimensional (3D) printing technology. This drug delivery system with favorable biomechanical properties, bone conduction function, and local release of osteogenic drugs could provide the basis for the treatment of bone defects. The biomechanical properties of the scaffold were investigated by compressive testing, showing comparable biomechanical properties with cancellous bone tissue. Furthermore, the microstructure, pore morphology, and condition were studied. Moreover, the drug release concentration, the effect of anti-tuberculosis drugs in vitro and in rabbit radial defects, and the ability of the scaffold to repair the defects were studied. The results show that the scaffold loaded with HA15 can promote cell differentiation into osteoblasts in vitro, targeting HSPA5. The micro-computed tomography scans showed that after 12 weeks of scaffold implantation, the defect of the rabbit radius was repaired and the peripheral blood vessels were regenerated. Thus, HA15 can target HSPA5 to inhibit endoplasmic reticulum stress which finally leads to promotion of osteogenesis, bone regeneration, and angiogenesis in the rabbit bone defect model. Overall, the 3D-printed β-TCP/PLGA-loaded HA15 bone tissue scaffold can be used as a substitute material for the treatment of bone defects because of its unique biomechanical properties and bone conductivity.

To prepare a three-dimensional (3D) printing polylactic acid glycolic acid (PLGA) scaffold with bone morphogenetic protein-9 (BMP-9) and P-15 peptide hydrogel and evaluate its application in treating bone defects in rabbits. Methods. 3D printing PLGA scaffolds were formed and scanned by electron microscopy. Their X-ray diffraction (XRD), in vitro degradation, and compressive strength were characterized. BMP-9 and P-15 hydrogels were prepared. Flow cytometry was used to detect apoptosis, and an electron microscope was used to evaluate cell adhesion to scaffolds. Alkaline phosphatase (ALP), type 1 collagen (Col-I), osteocalcin (OCN), runt-related transcription factor 2 (RUNX2), and osterix (SP7) were detected by western blotting. MicroCT was used to detect new bone formation, and bone tissue-related protein expressions were determined in the rabbit model with bone defects. Results. The 3D printing scaffolds were cylindrical, and the inner diameter of the scaffolds was about 1 mm. The bread peak with wide distribution showed that the 3D printing only involved a physical change, which did not change the properties of the materials. The degradation rate of scaffolds was 9.38%, which met the requirements of properties of biological scaffolds. The water absorption of the support was about 9.09%, and the compressive strength was 15.83 N/mm2. In the coculture of bone marrow mesenchymal stem cells (BMSCs) with scaffolds, the 2% polypeptide hydrogel showed the most obvious activity in promoting the differentiation of BMSCs. Flow cytometry showed that the 0% and 2% groups did not cause obvious apoptosis compared with the control group. Scaffolds with 2% and 4% polypeptide promoted the expression of ALP, COL-1, OCN, RUNX2, and Sp7 in BMSCs. In vivo experiments showed that the expression of ALP, COL-1, OCN, RUNX2, and Sp7 protein in the 2% polypeptide scaffold group increased significantly compared with the model group. MicroCT detection demonstrated that the 2% polypeptide scaffold had good bone repair ability. Conclusion. The PLGA scaffolds combined with BMP-9 and P-15 peptide hydrogels had good biological and mechanical properties and could repair bone defects in rabbits.

Abstract:Implanting artificial biomaterial implants into alveolar bone defects with individual shape and appropriate mechanical strength is still a challenge. In this study, bioceramic scaffolds, which can precisely match the mandibular defects in macro and micro, were manufactured by the 3-dimensional (3D) printing technique according to the computed tomography (CT) image. To evaluate the stimulatory effect of the material substrate on bone tissue regeneration in situ in a rabbit mandibular alveolar bone defect model, implants made with the newly developed, mechanically strong ~10% Mg-substituted wollastonite (Ca 90% Mg 10% SiO 3 ; CSi-Mg10) were fabricated, implanted into the bone defects, and compared with implants made with the typical Ca-phosphate and Ca-silicate porous bioceramics, such as β-tricalcium phosphate (TCP), wollastonite (CaSiO 3 ; CSi), and bredigite (Bred). The initial physicochemical tests indicated that although the CSi-Mg10 scaffolds had the largest pore dimension, they had the lowest porosity mainly due to the significant linear shrinkage of the scaffolds during sintering. Compared with the sparingly dissolvable TCP scaffolds (~2% weight loss) and superfast dissolvable (in Tris buffer within 6 wk) pure CSi and Bred scaffolds (~12% and ~14% weight loss, respectively), the CSi-Mg10 exhibited a mild in vitro biodissolution and moderate weight loss of ~7%. In addition, the CSi-Mg10 scaffolds showed a considerable initial flexural strength (31 MPa) and maintained very high flexural resistance during soaking in Tris buffer. The in vivo results revealed that the CSi-Mg10 scaffolds have markedly higher osteogenic capability than those on the TCP, CSi, and Bred scaffolds after 16 wk. These results suggest a promising potential application of customized CSi-Mg10 3D robocast scaffolds in the clinic, especially for repair of alveolar bone defects.

Abstract:OBJECTIVE:To evaluate the repairing effect of the rabbits radial defects of by polyester/tricalcium phosphate scaffolds prepared by rapid forming technology loaded with bovine BMP, and find a new carrier for growth factor.METHODS:Polyester/Tricalcium phosphate scaffolds prepared by rapid prototyping (RP) technology loaded with and without bovine BMP were used to repair the 15 mm radial defect of rabbit. Then results of radiography, histology, scaffolds degrade rates and bone density were appraised to examine the repairing effects of the scaffolds at 12 weeks.RESULTS:At 12 weeks, all defects treated with bBMP were radiographically repaired. No radii implanted polyester/tricalcium phosphate scaffolds alone showed radiographic and historical union. At experimental groups, longitudinal alignment of lamellar structure was observed histologically at 12 weeks, indicating that remodeling of regenerated bone almost completed, the scaffolds degradation rates were different by 12 weeks, and no abnormalities were observed in the surrounding soft tissue in all groups.CONCLUSION:Polyester/tricalcium phosphate scaffolds prepared by rapid prototyping technology loaded with bovine BMP can repair the rabbits radical defects. As for the effects, the poly (L-lactic-co-glycolide)/tricalcium phosphate (PLGA/TCP) scaffold are ideal and better than poly (L-lacide-co-D, L-lactide)/tricalcium phosphate (PDLLA/TCP) scaffold, but the poly (L-lactic acid)/tricalcium phosphate (PLLA/TCP) is not so good for its low degradation rates.

Abstract:Despite strong interest in improving care for high-risk elders, demonstration projects typically show negative results. This paper examines one large foundation-sponsored initiative to gain insight on why success often is so elusive. The findings indicate that specific flaws in concept, design, and implementation each make it more challenging for demonstrations to achieve their intended goals, especially those involving cost and utilization reductions. We speculate that part of the reason for this is that organizational and political processes lead to fundamentally conservative demonstrations that assume that small amounts of resources directed at incremental change can be effective in generating substantial change in organizations and can do so rapidly.
Repair of Critical‐Sized Long Bone Defects Using Dipyridamole‐Augmented 3D‐Printed Bioactive Ceramic Scaffolds

Abstract:There are over 2 million long bone defects treated in the USA annually, of which ~5% will not heal without significant surgical intervention. While autogenous grafting is standard of care in simple defects, a customized scaffold for large defects in unlimited quantities is not available. Recently, a three‐dimensionally (3D) printed bioactive ceramic (3DPBC) scaffold has been successfully utilized in the of repair critical sized long bone defects in vivo. In this study, 3DPBC scaffolds were augmented with Dipyridamole, an adenosine A2A receptor (A2AR) indirect agonist, because of its known effect to enhance bone formation. Critical‐sized full thickness segmental defects (~11mm x full thickness) defects were created in the radial diaphysis in New Zealand White rabbits (n=24). A customized 3DPBC scaffold composed of β‐tricalcium phosphate was placed into the defect site. Groups included scaffolds that were collagen‐coated (COLL), or immersed in 10μM, 100μM, or 1000μM Dipyridamole solution. Animals were euthanized 8 weeks post‐operatively and the radii/ulna‐scaffold complex retrieved, en bloc, for micro‐CT, histological and mechanical analysis. Bone growth was assessed exclusively within scaffold pores and evaluated by microCT and advanced reconstruction software. Biomechanical properties were evaluated utilizing nanoindentation to assess the newly regenerated bone for elastic modulus (E) and hardness (H). MicroCT reconstructions illustrated bone in‐growth throughout the scaffold, with an increase in bone volume dependent on the Dipyridamole dosage. Histological evaluation did not indicate any adverse immune response while revealing progressive remodeling of bone. Abstract:Large size bone defects typically need a long recovery period of more than three months, while a rapid repair of large bone defects is of great important and presents a significant challenge in clinical. Osteoinduction and osteoconduction provide different pathways and biofunctions for bone repair. Here, a biomimetic bone spatial architecture scaffold loaded with drug were constructed for satisfied both of these two bone repair biofunctions. Contacted with the host bone tissue, the HAp scaffold facilitates osteoconduction mainly through the outer layer of the scaffold. Meanwhile, GelMA loaded drugs primarily facilitates osteoinduction in the inner layer of the scaffold by establishing an osteogenic microenvironment. In vitro cell experiments confirmed that HAp/GelMA/IC showed excellent biocompatibility and osteogenic differentiation. An animal model of rabbit femoral condyle defect confirmed that HAp/GelMA/IC promoted osteoinduction and osteoconduction biofunctions. This dual biofunctions design provided a promising strategy for fast bone reconstruction in the large size bone defects repair.
