Abstract:Background and objectives: Virtual reality simulation (VRS) can be used to complement experiential learning, as it enables nursing students to further learn and refine nursing skills outside of the clinical setting. However, gathering evidence for its effectiveness as a teaching method in achieving learning outcomes is still ongoing, and thus there is a lack of systematic synthesis. The objective of this systematic literature review is to analyze VRS scenarios with a high level of immersion and their impact on learning outcomes in nursing education.Methods: A literature search was performed in the MEDLINE, CINAHL, and ERIC databases in November 2022. As a result, fifteen studies were included and analyzed using deductive content analysis.Results: The studies reported twelve different scenarios for virtual reality simulations with high levels of immersion, the focus of which was on acute critical care, broader nursing processes, neonatal and pediatric care, single nursing interventions, and observation of patients’ symptoms. The associated learning objectives were mainly achieved in the domains of cognition and psychomotor skills.Conclusions: There are several VRS scenarios that show potential for use in nursing education. The VRS scenarios are effective in improving learning outcomes, particularly those related to knowledge and skills. Overall, the supportive body of evidence gained through this review may help nurse educators in integrating virtual simulations in their curricula. In the future, nursing and adult learning theories should be given greater consideration, and the aspect of affective learning could be included in design and implementation. Moreover, future research could benefit from exploring the long-term effects of learning after using VRS with a high level of immersion to provide valuable evidence for developing VRS teaching methods in nursing.

Abstract:Virtual reality simulation offers students the opportunity to acquire clinical and psychomotor skills in a safe and interactive environment. This study describes the usability of virtual reality simulation among undergraduate nursing and midwifery students. Participants were recruited using convenience and snowball sampling and engaged in a 20-minute virtual reality simulation scenario of their choice. They then completed a 21-item survey comprising a sociodemographic questionnaire, the System Usability Scale, a satisfaction questionnaire, and open-ended questions. Quantitative data were analyzed using descriptive statistics, and qualitative data were analyzed using deductive content analysis. Forty-three students participated in this study. The mean (SD) System Usability Scale score was 75.87 (13.7), indicating that virtual reality simulation was acceptable. Almost all participants were either “extremely satisfied” or “somewhat satisfied” with virtual reality simulation, which was perceived as informative and enjoyable, fostering safe and self-directed learning without causing patient harm. Participants recommended using virtual reality simulation to practice clinical skills, prepare for clinical placements, and learn about rare clinical situations. Virtual reality simulation needs to be underpinned by a strong pedagogy and aligned with learning outcomes. Educators and students should be trained in virtual reality simulation prior to its integration into the curriculum.

Abstract:Nursing schools struggle to increase enrollments due to insufficient faculty and limitations in clinical site placements. New strategies must be employed to optimize learning, engage learners, and provide methods to ensure competency in future nursing graduates. A convenience sample of junior-level prelicensure nursing students (n = 48) were assigned to the immersive virtual reality (VR) group (n = 24) or the hospital-based group (n = 24). This mixed-methods quasi-experimental pilot study examined the effectiveness of an immersive VR simulation training program to replace pediatric clinical practice. The intervention group participated in immersive VR. The control group received the usual inpatient clinical training. Both groups then completed a traditional in-person simulation to compare performance outcomes. Performance scores for the immersive VR training group were significantly higher for total performance compared to the hospital-based training group. These preliminary results offer promise and demonstrate the potential of VR in the future of nursing education.

Abstract:Background: The purpose of this mixed-methods study was to examine the relationship between virtual reality simulation (VRS) and student satisfaction and self-confidence in a health assessment laboratory course. Methods: Second-year students (n = 37) completed a postoperative respiratory distress scenario using Elsevier’s Simulation Learning System with Virtual Reality. All participants completed the Satisfaction and Self-Confidence in Learning Scale; a subset participated in 1:1 semistructured interviews. Results: Satisfaction and self-confidence scores were strongly correlated. VRS experiences of fidelity, communication confidence and competence, learning with peers, integrated learning and critical thinking, and a safe space to learn were related to students’ satisfaction and self-confidence. Conclusions: VRS experiences are correlated with high student satisfaction and self-confidence.

Abstract:AIM The purpose of this article was to evaluate the ability of an interactive virtual reality (VR) platform guided by standards of best practice to provide an effective immersive learning environment. We specifically evaluated usability of the platform and learners' perceptions of the experience. BACKGROUND A variety of strategies are needed to train a highly competent nursing workforce. METHODS We conducted a quantitative cross-sectional study to evaluate the VR experience using the System Usability Scale (SUS)® and the Simulation Effectiveness Tool-Modified (SET-M). RESULTS Post-simulation evaluations were completed by 127 prelicensure and 28 advanced practice students. On the SUS scale, students found the overall VR system easy to navigate, and on the SET-M, they rated the VR experience positively. CONCLUSION Immersive technology such as VR with a defined curriculum and facilitated debriefing can be valuable for student learning and may ultimately effect patient care. Abstract:With increasing use of virtual reality simulation (VRS) in nursing education and given the vast array of technologies available, a variety of levels of immersion and experiences can be provided to students. This study explored two different levels of immersive VRS capability. Study participants included baccalaureate nursing students from three universities across four campuses. Students were trained in the skill of decontamination using traditional methods or with VRS options of mouse and keyboard or head-mounted display technology. Results of focus group interviews reflect the student experience and satisfaction with two different immersive levels of VRS.

Abstract:Aims: To examine the effectiveness of a virtual reality simulation (VRS) triage in improving students' learning motivation, attitudes, satisfaction and learning experiences in an emergency nursing course. Background: VR facilitates the realization of clinical scenarios that are significant for teaching emergency nursing. Triage presents a unique challenge in emergency nursing courses, where practical learning opportunities are seldom available. Design: The study employed a convergent parallel mixed-methods design. Methods: A total of 164 third-year nursing students were recruited from an emergency nursing course. The experimental group of 73 students participated in a VRS triage program and the control group of 91 students received a traditional program based on written case studies. The learning outcomes were evaluated using the Attention, Relevance, Confidence and Satisfaction motivational learning inventory together with questionnaires on learning attitudes and satisfaction. Qualitative data were collected through phenomenographic interviews. Results: The learning outcomes from the experimental group showed significant improvement in motivational learning inventory scores, with the greatest increase in satisfaction, followed by confidence, attention and relevance. Qualitative interviews with 30 students revealed four main themes in their learning experiences: "reflecting a real-world clinical environment," "enabling the internalization of knowledge," "enhancing clinical skills proficiency," and "cultivating a positive attitude toward patient care." The core theme was the establishment of professional core competency. Conclusions: VRS learning shows positive outcomes in enriching students' learning experiences. This study contributes to the literature on the use of VR technology in nursing education by offering insights into the effectiveness of VRS learning.
A mixed-methods feasibility study to assess the acceptability and applicability of immersive virtual reality sepsis game as an adjunct to nursing education

Abstract:Background: Virtual Reality (VR) simulation has been a topic of interest in recent years as an innovative strategy for healthcare education. Although there are a handful of studies evaluating VR simulation on knowledge, motivation, and satisfaction; there is a paucity of evidence to evaluate the effectiveness, acceptability and usability of 'Immersive' VR (IVR) simulation in nursing students. Objectives: A two-stage sequential mixed-methods feasibility study underpinned by gaming theory investigated; (1) the impact of IVR sepsis game on pre-registration nurses' self-efficacy and, (2) their perceptions of the acceptability and applicability of IVR sepsis game as an adjunct to nursing simulation education. Methods: The IVR simulation intervention was designed in collaboration with serious game specialists. Stage one collated pre and post-intervention self-efficacy scores with 19 pre-registration nurses using the validated instrument, Nursing Anxiety and Self-Confidence with Clinical Decision Making (NASC-CDM©) scale. Stage two used a descriptive qualitative approach to explore student nurses' perceptions of the game. Results: In stage one, pre and post-test scores revealed significant increase in self-confidence (26.1%, P < 0.001) and a significant decrease in anxiety (23.4%, P < 0.001). Stage two qualitative responses revealed four over-arching themes: acceptability, applicability, areas of improvement of IVR sepsis game and limitations of IVR game. Conclusion: IVR simulation show promise as an adjunct for nurse simulation and it appears to increase self-efficacy in pre-registration nursing students. Further testing with a sufficiently powered sample size will ratify findings and provide effective solutions to distance and online learning.

Abstract:BackgroundSimulation education is needed to train a highly competent nursing workforce. In-person simulation requires many resources, such as faculty, space, and time, that can negatively affect its feasibility. These barriers have prompted educators to seek new technologies to provide experiential learning opportunities, such as virtual simulation (VS). The COVID-19 pandemic further highlighted the need for educators to investigate alternative learning opportunities.MethodsA quantitative cross-sectional study was conducted with pre-licensure and advanced practice nursing students. They participated in a VS learning experience that was evaluated using the System Usability Scale (SUS)®, the Clinical Learning Environment Comparison Survey (CLECS 2.0), and the Simulation Effectiveness Tool-Modified (SET-M).Result197 pre-licensure and 11 advanced nursing practice students completed post-simulation evaluations. The results from the SUS scale found the overall virtual system effective. Evaluating the CLECS tool found students' perceived experience was relatively similar for face-to-face simulation and screen-based simulation. The SET-M indicated students rated the VS simulation experience positively.ConclusionVS for nursing education can be an effective experiential learning tool.

Abstract:Background Nonimmersive virtual reality (NVR) provides a window into a virtual world that is displayed on a computer screen. The technology recreates real clinical scenarios and provides engaging learning experiences. As a novel teaching modality, NVR has been applied in nursing education with varying results. Methods Meta-analysis. Results Thirteen trials involving 896 participants were included. Nursing students receiving NVR simulation showed greater increases in knowledge (posttest minus pretest scores) than those receiving mannequin-based simulation (SMD = 0.42, 95%CI = [0.08,0.77], p = .02), didactic education (SMD = 0.54, 95%CI = [0.21,0.88], p = .001), and nonintervention strategies (SMD = 0.87, 95%CI = [0.53,1.21], p < .001). Students receiving NVR simulation performed better (posttest scores only) than those receiving mannequin-based simulation (SMD = 0.58, 95%CI = [0.34,0.82], p < .001), didactic education (SMD = 1.72, 95%CI = [1.15,2.29], p < .001), and nonintervention strategies (SMD = 0.39, 95%CI = [0.03,0.75], p = .03). Students receiving NVR simulation reported higher self-efficacy/confidence (posttest scores only, SMD = 0.45, 95%CI = [0.20,0.70], p < .001) and satisfaction (posttest scores only, SMD = 0.92, 95%CI = [0.67,1.17], p < .001) than those receiving other educational strategies. Conclusion NVR simulation can effectively improve knowledge, performance, and self-efficacy/confidence of nursing students, and they are more satisfied with this teaching method. As a promising learning strategy, it may complement mannequin-based simulation to better prepare nursing students for clinical practice.

Virtual reality in midwifery education: A mixed methods study to assess learning and understanding

Abstract:Background: Virtual reality learning environments (VRLEs) are a potentially valuable learning tool that have recently increased in popularity due to widespread availability and decreased cost. VRLEs can provide an immersive learning environment that increases the understanding of three-dimensional relationships between anatomical structures. However, there is a paucity of evidence in the literature supporting its use within Midwifery education. Objectives: To explore the effectiveness of a VRLE as an educational tool in midwifery education. Setting: A large University in Ireland, with institutional ethical approval. Participants: Undergraduate and graduate degree midwifery students. Design: A descriptive qualitative and quantitative study was carried out. Data collection was carried out between September 2020 and March 2021. Methods: Participants underwent a VRLE lesson based on the topic of fetal lie, position, and presentation in pregnancy. A multiple-choice questionnaire was used to quantitatively evaluate knowledge before and immediately after the intervention, and knowledge retention after one week. Qualitative data was collected using open-ended questions in the questionnaire. The primary outcome was a difference in pre- and post-intervention knowledge scores. Data was analysed using repeated measures one-way ANOVA. Qualitative data was analysed using thematic analysis and simple content analysis. All students participated in the quantitative and qualitative components of the study. Secondary outcomes included participant satisfaction and self-confidence in learning which were analysed using thematic analysis. The side effect profile of the virtual reality device was also explored using open-ended questions in the questionnaire. Results: Forty-one midwifery students participated in the study, with a 100 % participation and response rate. Repeated measures one-way ANOVA revealed no statistically significant differences in knowledge scores pre- and post-intervention. Participants rated high satisfaction and self-confidence scores with regard to the VRLE as a learning modality. Side effects most commonly experienced by participants included dizziness (49 %), disorientation (30 %) and symptoms similar to motion sickness (32 %). The following themes were identified: "Learning in 3D", "The Power of Visual Learning", "The value of Educational Technology", "Learning can be fun and enjoyable". Conclusions: This study showed that the VRLE had no impact on knowledge gain, though high levels of satisfaction and self-confidence indicate a positive response to the VRLE. VRLEs are a potentially valuable learning tool to help enhance the student learning experience, promoting increased engagement, satisfaction, and self-confidence with the learning material.
360-Degree Virtual Reality Utilising Head-Mounted Devices in Undergraduate Nursing and Midwifery Education: A Scoping Review

Abstract:Immersive Virtual Reality (IVR) is a promising tool for improving the teaching and learning of nursing and midwifery students. However, the preexisting literature does not comprehensively examine scenario development, theoretical underpinnings, duration, and debriefing techniques. The aim of this review was to assess the available evidence of how 360-degree Virtual Reality (VR) utilising head-mounted devices has been used in undergraduate nursing and midwifery education programmes and to explore the potential pedagogical value based on Kirkpatrick’s evaluation model. This review followed the Joanna Briggs Institute (JBI) methodology. A comprehensive electronic search was conducted across five databases. All studies published in English between 2007–2022 were included, regardless of design, if the focus was undergraduate nursing and midwifery programmes and utilised fully immersive 360-degree VR scenarios. Out of an initial pool of 1700 articles, 26 were selected for final inclusion. The findings indicated a limited diversity in scenario design, with only one study employing a participatory approach. Within the Kirkpatrick model, the most measurable outcomes were found at level 2. The main drawback observed in interventional studies was the absence of a theoretical framework and debriefing. The review concludes that the increased use of fully IVR in nursing education has improved student learning outcomes; however, published literature on midwifery education is scarce.

Abstract:Background Simulations have become widely used in health care education to enhance learner preparedness. Virtual simulations, both screen- or headset-based, provide nursing students with the ability and flexibility to practice patient care at their convenience and obtain valuable automatic feedback before treating patients. While cheaper than in-person simulations and effective in teaching students, limited work exists to provide guidance concerning which modality type (screen-based or headset) offers an optimal interactive learning experience. Methods We compared nursing students' (n = 29) performance (Oxford Medical Simulation software-generated score), ratings of usability (system usability scale and handheld augmented reality usability scale), cognitive load demands (Leppink's cognitive load scale), and emotions (medical emotions scale) between the two modalities, while keeping the virtual simulation constant. Results Both modalities yielded statistically comparable results in performance scores, usability, cognitive resources, and emotional experiences as determined by statistical analysis. Conclusion Results suggest that nursing students have similar experiences and benefits from using headset and screen-based virtual simulation modalities. Therefore, educators have the flexibility to choose between screen-based and headset modalities for virtual simulation training.

Abstract:The purpose of this study is to confirm the effectiveness of the problem solving process, critical thinking disposition, and self-directed learning ability effect between the group who received virtual reality simulation training online for 3rd grade nursing students and the group who received clinical practice training offline for them. The experimental group (18 people) was asked to evaluate patient care and provide feedback through VSim Scenarios (The National League for Nursing & Laerdal Medical), an online interactive virtual simulation. The control group (19 people) was given offline regular practical training. Data analysis was using SPSS25.0. As a result of the study, it was found that the experimental group who received virtual reality simulation practical education using online had a significantly higher problem solving process (Z=-2.194, p=.028) than the control group. Processes such as patient safety, communication, physical assessment, and drug injection appear to have had a positive effect on solving the problem by providing patient information through scenario learning, pre-learning quizzes, and providing feedback to patients after nursing in a virtual simulation space. In conclusion, in order to confirm the effectiveness of online virtual reality simulation education, it is necessary to continuously study by applying various variables.

Abstract:Background Virtual reality enables reproducing scenarios that are difficult to access in clinical education. Its immersive qualities and ability to isolate students from external auditory and visual distractions may enhance engagement over other simulation modalities. Methods This quasi-experimental study compared the effects of virtual reality and hybrid simulations (i.e., actor and manikin) on nursing students' engagement. Data on satisfaction, confidence in learning, cognitive load, mental effort, and clinical reasoning were also collected. The study included 179 nursing students who completed a post-simulation survey. Results Participants in the hybrid group reported higher engagement, satisfaction, and mental effort than the virtual reality group. No significant differences regarding confidence, cognitive load, and clinical reasoning were found. Conclusion Even if virtual reality emerges as a promising tool for simulation-based education, certain inherent aspects of traditional simulation modalities may contribute to enhanced engagement, such as human presence and opportunities for natural interactions.

Abstract:The use of simulation in the undergraduate nursing curriculum is gaining popularity and is becoming a foundation of many nursing programs. The purpose of this paper is to highlight a new simulation teaching strategy, virtual reality (VR) simulation, which capitalizes on the technological skills of the new generation student. This small-scale pilot study focused on improving interpersonal skills in senior level nursing students using VR simulation. In this study, a repeated-measure design was used to evaluate the effectiveness of VR simulation on improving student’s performance over a series of two VR scenarios. Using the Emergency Medicine Crisis Resource Management (EMCRM) tool, student performance was evaluated. Overall, the total EMCRM score improved but not significantly. The subscale areas of communication ( P = .047 , 95% CI: − 1.06 , − .007 ) and professional behavior ( P = .003 , 95% CI: − 1.12 , − .303 ) did show a significant improvement between the two scenario exposures. Findings from this study show the potential for virtual reality simulations to have an impact on nursing student performance.

Methods The present systematic review and meta-analysis were applied according to the Preferred Reporting Items for Systematic reviews and Meta-Analyses (PRISMA) statement. The PubMed, Embase, CINAHL, ProQuest, Cochrane Library, Web of Science, and Scopus were searched for relevant articles in the English language. The methodologies of the studies evaluated were assessed using Cochrane Risk of Bias2 (ROB 2) tool and Joanna Briggs Institute (JBI) assessment tool. We took the learning satisfaction, knowledge, and skill performance of nursing students as the primary outcomes, and nursing students' self-efficacy, learning motivation, cognitive load, clinical reasoning, and communication ability were assessment as secondary outcomes. The meta-analysis was performed using R 4.3.2 software according to PRISMA guidelines. Heterogeneity was assessed by I 2 and P statistics. Standardized mean difference (SMD) and 95 % confidence intervals (CIs) were used as effective indicators. Results Twenty-six studies were reviewed, which involved 1815 nursing students. The results showed that virtual reality teaching, especially immersive virtual reality, was effective in improving nursing students' learning satisfaction (SMD: 0.82, 95%CI: 0.53–1.11, P < 0.001), knowledge (SMD: 0.56, 95%CI: 0.34–0.77, P < 0.001), skill performance (SMD: 1.13, 95 % CI: 0.68–1.57, P < 0.001), and self-efficacy (SMD: 0.64, 95%CI: 0.21,1.07, P < 0.001) compared to traditional teaching methods. However, the effects of virtual reality technology on nursing students' motivation, cognitive load, clinical reasoning, and communication ability were not significant and require further research. Conclusions The results of this study show that virtual reality technology has a positive impact on nursing students. Nonetheless, it is crucial not to underestimate the effectiveness of traditional education methods, and future research could analyze the impact of different populations on nursing education while improving virtual reality technology, to more comprehensively explore how to improve the quality of nursing education. Moreover, it is imperative to emphasize the integration of virtual education interventions with real-world experiences promptly. This integration is essential for bridging the gap between the virtual learning environment and real-life scenarios effectively. Abstract:Aim: To provide insights into the optimal use of virtual reality (VR) in nursing education by evaluating pre-registration nursing students' experiences in conducting holistic patient assessments while interacting with artificial intelligence (AI)-led patients. Specifically, this project evaluation compares the use of two different VR scenarios, one employing a menu-based interface and another using AI voice-controlled technology. Methods: Eleven pre-registration adult nursing students from two UK universities were selected through purposeful sampling to participate in the two VR simulations. Data collection and analysis: This included qualitative insights gathered from three focus group sessions, audio-recorded and thematically analysed to classify and describe students' experiences. Findings: Four key themes emerged: technological literacy, VR as a learning tool, the road of learning, and transition to independence. Advantages across both methods of VR-AI interaction and their particular challenges were identified and described for each key theme. Conclusion: VR with AI-led patient technology in pre-registration nursing education positively contributes to the curriculum by exposing students to problem-based learning situations and use of a multiplicity of skills in a safe environment. Although both methods are relevant for developing proficiencies around holistic patient assessment, there are advantages and limitations to each. Students perceived the voice-controlled technology as more intuitive with a more natural method of communication, whereas the menu-based interaction gave students more structure and guidance.
Use and Effectiveness of Virtual Simulations in Nursing Student Education: An Umbrella Review

Abstract:Computer-based virtual simulation in nursing is a rapidly developing field. A summary of current research could benefit nursing faculty who are incorporating this pedagogy into online teaching due to social distancing requirements. This umbrella review aimed to explore the use and effectiveness of virtual simulations in prelicensure nursing education. Our umbrella review approach was informed by The Johanna Briggs Institute guidelines. The 18 included reviews captured the experiences of over 7600 nursing students who engaged with five different virtual (screen-based) simulation modalities. Results indicated that virtual simulations can be effective in developing nursing students' knowledge and psychomotor and psychosocial skills, for example, in medication administration and communication. There was no study that showed a conclusive benefit of virtual simulations for developing clinical reasoning. Students enjoyed virtual simulations, claiming they were accessible, fun, and engaging ways to learn. Many benefits of virtual simulations and some technological challenges were identified in a mapped model. In the future, more rigorous and experimental studies are needed to confirm effects on learning outcomes. Nursing curriculum designers need to define the place and purposes of the pedagogy to ensure that knowledge and skills learned during virtual simulations can be applied to real-world, patient-centered care.
