Abstract:Objective:To compare the efficacy of oral sulfate solution (OSS) and polyethylene glycol (PEG) electrolyte powder for colonoscopy bowel preparation.Methods:A total of 283 randomized patients from 9 centers in China taking OSS ( n=143) or PEG ( n=140) using two-day split bowel preparation regimen received colonoscopy and assessment. The primary index was the bowel preparation success rate [global Boston bowel preparation scale (BBPS)≥ 6 by independent assessment center]. Secondary indices included BBPS global and segmental scores, investigator satisfaction (5-point Likert scale) with the quality of bowel preparation, patient satisfaction assessed by questionnaires, and patient tolerance assessed by Sharma scale. Compliance and safety were compared between the two groups. Results:The bowel preparation success rates were 100.0% for OSS and 99.3% for PEG [adjusted difference 0.7% (95% CI: -5.3% - 6.7%), P<0.001 for non-inferiority]. The BBPS global score in OSS group was significantly higher than that in PEG group (8.1 VS 7.7, P<0.001). The segment BBPS scores were also higher in OSS group than those in PEG group for all 3 segments (right colon: 2.4 VS 2.3, P=0.002; transverse colon: 2.8 VS 2.7, P=0.018; left colon: 2.8 VS 2.7, P=0.007). Investigator Likert score in the OSS group was significantly higher than that in the PEG group (2.6 VS 2.3, P<0.001). There was no significant difference in compliance between OSS and PEG, except for the second dose (90.9% VS 82.6%, P=0.039). There was no significant difference in patient satisfaction, Sharma score or proportion of patients with tolerance-related symptoms between the two groups. Safety was comparable between the two groups, and all adverse events were mild to moderate. Conclusion:OSS has comparable efficacy with PEG, with higher BBPS scores in all segments, better investigator satisfaction, better compliance in split dose, and comparable patient tolerance and safety.
Exploration of an Improved Bowel Preparation for Inflammatory Bowel Disease Patients

Abstract:Objective To improve the quality of colonoscopy examination by exploring an improved pre-preparation method for patients with inflammatory bowel disease (IBD).Methods A prospective study was carried out.Patients with IBD undergoing colonoscopy at the Center of Inflammatory Bowel Diseases of Sir Run Run Shaw Hospital between December 2016 and March 2017 were divided into control group and trial group according to the order of examination time.The control group received no slag diet 1 day before bowel preparation,and the trial group received no slag diet for breakfast and lunch and oral enteral nutrition solution for dinner.The bowel cleanliness was assessed by the Boston bowel preparation scale (BBPS).Results A total of 114 IBD patients were enrolled with 57 cases in each control and trial group.The BBPS scores in the left hemicolon of both groups were higher than those in the right hemicolon in the same group (control group:2.77 ± 0.54 vs.2.28 ± 0.80,P =0.0002;trial group:2.91 ± 0.28 vs.2.63 ± 0.59,P =0.0015).Compared with control group,trial group had higher BBPS score of right hemicolon (2.63 ± 0.59 vs.2.28 ± 0.80,P =0.009),higher BBPS score of transverse colon (2.98 ±0.13 vs.2.81 ±0.58,P=0.028) and higher total BBPS score (8.53±0.66 vs.7.86± 1.56,P=0.004).There was no statistically significant difference in BBPS score of left hemicolon between two groups.No obvious adverse reactions and complications occurred during the study.Conclusions The bowel preparation cleansing effect of the trial group is better than that of the control group,which is mainly present in right hemicolon and transverse colon.It suggests that for whole colon examination,no slag diet plus oral enteral nutrition solution dinner 1 day before the bowel cleansing will have a better effect,and if for left hemicolon examination only,normal no slag diet 1 day before the bowel cleansing is suitable.

Abstract:Effective bowel preparation is crucial for successful inpatient colonoscopies, which tend to be more challenging than outpatient procedures. The quality of bowel cleansing directly affects diagnostic yield and therapeutic intervention success during colonoscopies. Hospitalized patients often face higher risks of poor preparation due to factors like older age, decreased mobility, and multiple comorbidities. Inadequate or incomplete preparation can lead to limitations such as delayed procedures, increased costs, and compromised patient safety. Higher-volume preparations can exacerbate these challenges, leading to complications such as electrolyte imbalances, nausea, vomiting, abdominal discomfort, and aspiration.

Abstract:BACKGROUND: Research data from patient reports indicate that the least bearable part of colonoscopy is the administration of laxatives for bowel preparation.AIM: To observe the intestinal cleansing efficacy and safety of sodium picosulfate/magnesium citrate and to discuss the patients' experiences due to the procedure.METHODS: Subjects hospitalized in the International Medical Center Ward of Peking University International Hospital, Beijing, China, from April 29 to October 29, 2020, for whom the colonoscopy was planned, were enrolled. Bowel preparation was performed using sodium picosulfate/magnesium citrate. The effect of bowel cleansing was evaluated according to the Ottawa Bowel Preparation Scale, defecation conditions and adverse reactions were recorded, and the comfort level and subjective satisfaction concerning medication were evaluated by the visual analogue scale/score (VAS).RESULTS: The bowel preparation procedure was planned for all patients enrolled, which included 42 males and 22 females. The results showed an average liquid rehydration volume of 3000 mL, an average onset of action for the first dose at 89.04 min, an average number of bowel movements of 4.3 following the first dose, an average onset of action for the second dose at 38.90 min and an average number of bowel movements of 5.0 after the second dose. The total average Ottawa Bowel Preparation Scale score was 3.6, with 93.55% of bowel preparations in the "qualified" and 67.74% in the "excellent" grade. The average VAS score of effect on sleep was 0, and the average VAS score of perianal pain was also 0. The average VAS score for ease of taking and taste perception of the bowel cleanser was 10. Side effects included mild to moderate nausea (15.63%), mild vomiting (4.69%), mild to moderate abdominal pain (7.81%), mild to moderate abdominal distension (20.31%), mild palpitation (7.81%) and mild dizziness (4.69%).CONCLUSION: Sodium picosulfate/magnesium citrate is effective and safe for bowel preparation before colonoscopy with high subjective patient acceptance, thus improving overall patient compliance.

Real-word Evaluation of Differences in Bowel Preparation for Colonoscopy Between the Digestive and the Non-Digestive Physicians: A Retrospective Study

Abstract:IntroductionUsing real-world data, we compared the quality of bowel preparation instructed by the digestive and non-digestive physicians in outpatients for colonoscopy and identified potential risk factors.MethodsThis was a retrospective study based on real-world data, which were collected from the Ningbo First Hospital in China from December 2019 to October 2020. Outpatients included were classified into the digestive and the non-digestive physician groups according to the referring physician. The primary outcome was adequate bowel preparation measured by the Boston Bowel Preparation Scale (BBPS), namely, a BBPS score of 2 or higher in any colonic segment and a total score ≥ 6. Secondary outcomes included the total mean BBPS scores and possible risk factors associated with poor bowel preparation.ResultsThere were 671 outpatients included, with 392 in the digestive physician group and 279 in the non-digestive physician group. Adequate bowel preparation was 84.2% in the digestive physician group and 71.0% in the non-digestive physician group (odds ratio [OR]: 1.50, p < 0.001), and the latter had lower total mean BBPS scores (6.12 ± 1.33 vs. 6.66 ± 1.29, p < 0.001). The non-digestive physician was an independent risk factor according to the multivariate logistic regression analysis (OR: 0.45, p < 0.001).ConclusionThe quality of bowel preparations instructed by non-digestive physicians was inferior to digestive physicians, which was a factor potentially associated with poor bowel preparation (ClinicalTrials.gov number: NCT04738578).

Abstract:Colorectal cancer screening relies on adequate bowel preparation to ensure clear visualization during colonoscopy. Glucagon-like peptide-1 receptor agonists (GLP-1 RAs) are integral in managing diabetes and weight, yet their effects on gastrointestinal motility, such as delaying gastric emptying and reducing small bowel transit time, can impact bowel preparation. Current guidelines do not specifically address the interaction between GLP-1 RAs and bowel preparation. This retrospective study evaluated the association between GLP1- RAs and the quality of bowel preparation in patients undergoing colonoscopy as assessed by the Boston Bowel Preparation Score (BBPS).

Abstract:Background Glucagon-like peptide-1 receptor agonists (GLP-1RAs) affect gastrointestinal motility, slowing gastric emptying and colonic transit. GLP-1RAs have an impact on gastric residue before endoscopy, but only limited data are available regarding its effect on the adequacy of colonic preparation. We investigated the association between GLP-1RA use and inadequate bowel preparation (IBP) for colonoscopy. Methods We performed a multicenter retrospective study with GLP-1RA cases matched with controls (using propensity scores for age, sex, diabetes mellitus [DM], obesity, and co-morbidities). Data on demographics, medication use, procedural indications, and colonoscopy findings were collected. IBP ("poor preparation" on Aronchik scale or Boston Bowel preparation scale <5) was the primary outcome. Results 4876 patients treated with GLP-1RAs were included in the analysis and compared with 4876 controls selected from 333 648 patients without GLP-1RA use. Among the GLP-1RA patients, 10% (n = 487) had IBP compared with 197 (4%) of the control group ( P <0.001). Subgroup analysis showed a higher rate of IBP among diabetic patients treated with GLP-1RA (284/2364 [12%]) than among diabetic patients without GLP-1RA treatment (118/2364 [5%]; P <0.001). Additionally, 203/2512 nondiabetic patients treated with GLP-1RAs had IBP (8%) compared with 79 of the nondiabetic non-GLP-1RA group (3%; P <0.001). On multivariate analysis, diabetes and GLP-1RA use were both found to be independent risk factors for IBP (odds ratio [OR] 1.4 and OR 2.7, respectively; both P <0.001). Conclusion Our findings highlight the necessity for special attention and tailored recommendations for both diabetic and nondiabetic patients treated with GLP-1RAs in terms of colonic preparation prior to colonoscopy. Abstract:Abstract Background Colonoscopy is important in the management inflammatory bowel disease. Adequate bowel preparation (IBD) is a prerequisite for colonoscopy and may be difficult to achieve in setting of IBD Methods We performed a systematic review and network meta-analysis of randomised trials of various bowel preparations used before colonoscopy in IBD. Various electronic databases were searched to identify eligible studies. A frequentist network meta-analysis was done to compare the effectiveness of various bowel preparations. Risk of bias was assessed using Cochrane risk of bias tool 2.0. Other outcomes were assessed qualitatively. Results We included 7 trials including 960 patients. All bowel preparation had a similar effectiveness of bowel preparation as compared to 4 litres of polyethylene glycol (PEG) (OR when compared with PEG4L: OSS- 1.1043 95% CI 0.6553-1.8610; PEG/Ascorbate 2L- 0.9838, 0.6519-1.4848; PEG 1L -1.0011, 0.5522-1.8150; PEG2L plus bisacodyl -1.0864, 0.7130-1.6554; PEG 4L plus simethicone -1.0069, 0.6721-1.5084; PEG/SPMC 1.5L -0.9966, 0.5569-1.7835; SPMC 2L -1.0996, 0.6123-1.9745 had a similar effectiveness. Low-volume preparations had better compliance, tolerance, and acceptance, and willingness to repeat. No difference in additional outcomes like change in disease activity after colonoscopy, procedure-related outcome after colonoscopy like cecal intubation rate, and change in electrolyte levels were found. Conclusion Various bowel preparations had similar effectiveness in respect to colonoscopy preparation in IBD patients. Low-volume preparations have better compliance, tolerance, and acceptance. The systematic review was limited by a small number of included RCTs.

Abstract:Study design: Randomised controlled trial. Objective: This study aimed to determine the effectiveness of a preoperative bowel preparation protocol comprising bisacodyl to minimize postoperative gastrointestinal morbidities and the hospital length of stay for patients with adolescent idiopathic scoliosis. Summary of background data: Patients who undergo scoliosis correction surgery frequently experience postoperative gastrointestinal morbidities and a prolonged hospital length of stay. Emesis, paralytic ileus and constipation are the most common gastrointestinal morbidities. Opioid medication is a well-known risk factor for gastrointestinal complications after scoliosis correction surgery. Methods: Eighty-seven patients (22 boys [25.3%] and 65 girls [74.7%]) with a mean age of 17.7 years (standard deviation [SD], ±2.2 years) diagnosed with adolescent idiopathic scoliosis were enrolled in this study and randomized into 2 groups. Group A comprised 44 patients who received a preoperative bowel preparation comprising bisacodyl. Group B comprised 43 patients who did not receive any preoperative medication. Demographic data, height, weight, medical and surgical comorbidities, Risser status, number of instrumented levels and preoperative opioid consumption of all patients were evaluated. Results: Group A experienced fewer postoperative abdominal symptoms than group B. The mean hospital length of stay was 4.1 days (SD, ±.6 days; median, 4 days; range, 3-5 days) for group A; however, it was 5.3 days (SD, ±.8 days; median, 5 days; range, 4-7 days) for group B (P = .01). Conclusion: The use of a bowel preparation protocol before scoliosis correction surgery for patients with adolescent idiopathic scoliosis can effectively decrease postoperative gastrointestinal morbidities and the hospital length of stay.
A Randomized Study on the Effect of Different Bowel Preparation Methods Before Colonoscopy in Elderly Patients

Abstract:Objective To investigate the clinical effects of different bowel preparation methods before colonoscopy in elderly patients.Methods A total of 84 elderly patients undergoing colonoscopy during July 2015 and July 2016 were divided into two groups(n=42 for each group). Repeat group was treated with 1 L Polyethylene Glycol(PEG)orally for bowel preparation at night before the colonoscopy and 2 L PEG on the day of colonoscopy respectively. Single group was treated with 3 L PEG for bowel preparation on the day of colonoscopy. In the two groups, bowel preparation efficacy, patient's compliance, tolerability and adverse reactions were evaluated.Results According to the Boston bowel preparation scale(BBPS), the score of total colon cleaning effect was(7.49±1.33) in repeat group and (5.29±1.45) in single group with a significant difference(t=2.78, P<0.05), and side colon cleaning effect were also statistically significant (P<0.05). Bowel preparation efficacy, patient's compliance, tolerability and adverse reactions in two groups were not statistically significant (P>0.05).Conclusions It is the proper method for elderly patients who are treated with PEG orally and repeatedly before colonoscopy.

Abstract:Objective: To determine the caecal intubation time depending on bowel preparation as per Boston Bowel Preparation Scale. Methods: This cross-sectional study was conducted at Dr. Ruth K. M. Pfau, Civil Hospital Karachi between August 2018 to February 2019. A total of 201 patients were included in the study. Time was recorded from insertion of colonoscope to the time required to reach the cecum. Bowel preparation was graded during withdrawal of colonoscope by using Boston Bowel Preparation Scale. Pearson Correlation test was used to study correlation of BBPS scores with CIT, gender, BMI, adenoma and polyp detection. Results: In this study 201 patients undergoing colonoscopy were included. Mean ±SD of age of patients was 36.9 ±15.8 years. Out of the 201 patients 112 (56%) were males and 89 (44%) were females. The results of our study showed that increased Boston Bowel Preparation Scale Scores were associated with decreased caecal intubation time. The mean CIT was 10.7 ±5.4 minutes and Pearson correlation was significant at 0.002. Significant correlations of BBPS were also found with BMI and adenoma detection. Conclusion: The diagnostic effectiveness of colonoscopy depends upon the quality of the preparation. A novel strategy for improving bowel preparation based on social software-enhanced education: A prospective, multicenter, randomized controlled study

Abstract:Background and aim: The compliance and timeliness of oral laxatives have always been the key factors restricting bowel preparation (BP). We have constructed a novel enhanced-educational content and process based on social software (SS) for BP to optimize these issues. Methods: A multicenter, prospective, randomized controlled study was conducted at 13 hospitals in China from December 2019 to December 2020. A total of 1774 enrollees received standard instructions for BP and were randomly assigned (1:1) to the SS group (SSG) that received a smartphone-based enhanced-education strategy starting 4 h before colonoscopy or the control group (CG). Results: A total of 3034 consecutive outpatient colonoscopy patients were assessed for eligibility, and 1774 were enrolled and randomly assigned. Ultimately, data from 1747 (SSG vs CG: 875 vs 872) enrollees were collected. The BP adequacy rate was 92.22% (95% CI: 90.46-93.98) in the SSG vs 88.05% (95% CI: 85.91-90.18) in the CG (P = 0.005), and the total Boston Bowel Preparation Scale scores (6.89 ± 1.15 vs 6.67 ± 1.15, P < 0.001) of those in the SSG were significantly higher than those in the CG. The average number of polyps detected in the SSG was considerably higher than that in the CG (0.84 ± 2.00 vs 0.53 ± 1.19, P = 0.037), and the average diameter of the polyps was significantly lower than that of the control group (4.0 ± 2.5 vs 4.9 ± 3.7, P < 0.001). Conclusions: This SS-enhanced education strategy can improve the BP adequacy rate and increase the average number of polyps detected, especially those of small diameter.

Abstract:Background and Aim Sodium picosulfate plus magnesium citrate (SP + MC) is a well‐tolerated bowel preparation agent. However, Japan currently approves only two methods of taking SP + MC: the day‐before and split‐dose preparation, without approval of same‐day preparations. This study aimed to evaluate the efficacy and safety of same‐day SP + MC preparations. Methods This was a multicenter, single‐arm, nonrandomized, open‐label study. We enrolled 145 Japanese patients between June and December 2023. The patients received two sachets of SP + MC dissolved in 300 ml of water and 1200 mL or more of clear liquid on the day of colonoscopy. Bowel cleansing efficacy, adverse events (AEs), and patient satisfaction were evaluated. Results Of the enrolled patients, 137 underwent colonoscopy according to our protocol. Bowel preparation was adequate in 133 patients (97.1%). The mean total Boston Bowel Preparation Score was 8.3 ± 1.2. Five patients experienced AEs (3.6%): two (1.5%), abdominal pain; one (0.73%), ischemic enteritis; one (0.73%), vomiting or nausea; and one (0.73%), headache. All AEs were treated conservatively. None of the patients exhibited abnormal blood test results or clinical symptoms after receiving SP + MC. Regarding patient satisfaction, all patients were able to take SP + MC as directed; 136 (99.2%) expressed a preference for this bowel preparation for future colonoscopies. Conclusion The same‐day SP + MC preparation showed high bowel‐cleansing efficacy and satisfaction in Japanese patients without serious AEs.

A Systematic Review of Exercise Therapy for Bowel Preparation: Evidence to Guide Practice

Abstract:This systematic review aims to evaluate (1) the effectiveness of exercise therapy in bowel preparation for colonoscopy, and (2) the characteristics of exercise programs for bowel preparation. Systematic searches were done in PubMed, EMBASE, the Cochrane Library, Web of Science, and CINAHL from inception to November 2022. Randomized controlled trials and quasi-experimental studies assessing the efficacy of exercise during bowel preparation were included in this review. Two reviewers independently assessed the methodological quality using a modified Downs and Black checklist. A narrative synthesis was conducted. A total of five studies (1,109 participants) were included in this review. In all eligible studies, the characteristics of the exercise programs varied and included mainly two types of exercise (walking and yoga), various amount of exercise (3,000-10,000 steps or 0.5-1.9 hours), and two exercise timing (during and 1 hour after taking the laxative). Available evidence indicated that exercise therapy is effective in improving the quality of bowel preparation. However, there was insufficient high-quality evidence to conclude the effects on procedure-related indicators, adverse events, and willingness to repeat preparation. Exercise should be recommended as an important part of routine bowel preparation for patients undergoing colonoscopy to improve the quality of bowel preparation. More rigorous studies focusing on the effects on procedure-related indicators, adverse events, and willingness to repeat preparation are needed. To ensure the effectiveness and safety of the intervention, it is critical to establish a standard, well-structured exercise program for bowel preparation.

Abstract:Background: Adequate bowel preparation is one of the most important prerequisite during the colonoscopy. It is conductive to detect the polypus, adenoma and early colorectal carcinoma and decrease the rate of overuse of medical resource. BianShi Moxibustion(BSM) is one of the most important Traditional Chinese Medicine(TCM) treatment options, which is called TCM proper technology. BSM is applied to prevent and cure digestive system disorder widely, including functional constipation(FC), irritable bowel syndrome(IBS), functional dyspepsia(FD) etc. we speculate that BSM can improve the bowel preparation quality through ameliorating the function of digestive tract, especially in high risk of inadequate bowel preparation patients, including elderly individual, diabetes mellitus, neurodegenerative changes etc. Hence, the objective of this research is to evaluate the efficacy of BSM on the quality of bowel preparation before colonoscopy. Method: This is a randomized triple-blinded , single-center, prospective study. We will recruit 72 inpatients who are scheduled to undergo colonoscopy for screening and diagnostic or therapeutical purposes for the first time will be randomized to assign into treatment group or control group at a ratio of 1:1. The intervention plan in the treatment group consists of 3L polyethylene glycol(PEG) solution plus BSM(Treatment period is 3 days, Qd). In the control group, the plan is 3L PEG plus sham BSM. Boston Bowel Preparation Scale(BBPS) will be used to evaluate the efficacy of the bowel preparation and regarded as the primary outcome measure. The secondary outcomes including caecal intubation rate, the willingness to repeat colonoscopy, the tolerance of bowel preparation regimens, the rate of adverse events. Discussion: The aim of this clinical trial is to confirm the influence of BSM on the quality of bowel preparation. The hypothesis of this study is that the BSM has a positive role on the quality of bowel preparation. The routine bowel preparation regime combines with TCM proper technology will enhance bowel preparation quality and ameliorate the subjective feeling of participants. In addition, it will provide the reliable evidence of bowel preparation prior to colonoscopy from the point of view of integrative medicine.
