Abstract:Objective To investigate the effects of a multidisciplinary round electronic checklist in ICU. Methods From October 2016 to September 2017,a total of 1 172 patients were provided with a multidisciplinary round electronic checklist. Another 886 patients with routine rounds between October 2015 to September 2016 were retrospectively analyzed. The duration of mechanical ventilation and ICU stay,the rate of nosocomial infection, the percentages of catheter days(central line catheter days,artificial airway days and urinary catheter days) and clinical staffs' satisfaction with the multidisciplinary collaboration were compared. Results The duration of mechanical ventilation and ICU stay in the intervention group were 1.42(0.68,3.63)days and 3.19(2.48,5.40)days respectively,in the control group were 2.16(1.40, 4.77) days and 4.14(3.46, 6.92) days respectively. The differences between two groups were statistically significant(t=2.084, 2.247, P <0.05). The application of a multidisciplinary round electronic checklist shortened the duration of mechanical ventilation and ICU stay. The percentages of catheter days and the rate of catheter-associated urinary tract infection were 0(0/6 269),60.34%(6 269/10 389)respectively,in the control group were 0.93‰(6/6 485),67.02%(6 485/9 676)respectively.The differences between two groups were statistically significant(χ2=96.187,Fisher's exact test=0.031,P<0.05).Besides,the level of multidisciplinary collaboration in ICU was greatly improved in the intervention group, 3.03 ± 0.11 vs. 4.13 ± 0.39 (t=24.587, P < 0.05). Conclusions A multidisciplinary round electronic checklist in ICU can improve the care quality and multidisciplinary collaboration.

Abstract:Aim The study aimed to evaluate the reliability and usability of the CARE‐Radiology checklist in assessing radiological case reports and provide a basis for its broader adoption and optimization. Methods Ten randomly selected radiological case reports published in scientific journals in 2020 were evaluated using the CARE‐Radiology checklist. Twenty‐six experts from 10 countries were invited to independently assess all ten reports. The reliability of the checklist was measured using Fleiss' Kappa, and Cronbach's alpha coefficient. Usability was evaluated by recording the time taken to complete the assessments and requesting the evaluators to rate each item on a Likert scale for its easiness of use. Results The median time for evaluating one radiological case report was 15 min. The overall agreement among evaluators showed moderate reliability with a Kappa value of 0.47 and a Cronbach's alpha of 0.51. The mean compliance rate for the items of CARE‐Radiology was 61.8%, with some items exceeding 90% compliance. Items related to abstracts and keywords had the lowest compliance rates. The evaluators found most items easy to understand, with a few exceptions. Conclusions The CARE‐Radiology checklist is relatively easy for researchers to use and understand. Continuous feedback is necessary for future revisions and updates, to enhance the effectiveness of the checklist, and to improve user experience.

Abstract:Introduction: Health information systems play an important role in improving the quality of patient care and patient safety. to ensure their effectiveness and efficiency, they need to be evaluated. Although HIS evaluation has been investigated in many studies, there is no consensus on which aspects of HIS to evaluate. The aim of this study is to identify the indicators for the evaluation of health information systems and to provide an overview of the criteria devised and studies conducted.Methods: An umbrella review was performed exploring databases PubMed, Science direct, Web of Science, Science, and IEEE while following the PRISMA protocol. Articles were reviewed by two authors independently using the covidence tool to check the inclusion criteria and to extract the data items. Risk of bias was assessed using ROBIS and AMSTAR.Result: All included studies showed a high risk of bias according to ROBIS criteria. The extracted evaluation criteria were classified into 13 categories. Most of the studies believe that a more reliable and standardized tool is needed for the evaluation of health information systems. Two studies mentioned that surveys and questionnaires were the most commonly used method for evaluation of the systems. Summative evaluation was the most used method in two studies and the least used method in another one study.Conclusion: All the included studies had high risk of bias. Accordingly, further research and evidence is needed in this field. Most of the studies highlighted the need for more reliable and standardized tools for evaluation of health information systems.

Abstract:Background and aims: Prevention of cancer and risk assessment activity is highly information – intensively task, yet health care providers don't have appropriate technologies available to access, manage and interpret varied information. Also decision making modalities for cancer screening for many conditions and different stages have become increasingly complex. Evidence based systems facilitate decision making for the delivery of cancer screening services. The aim of this article is to designing evidence based Risk assessment system for cancer screening. Methods: This was a developing study. The first phase was a comparative study that performed by using secondary data extracted from literature review. Three courtiers (Canada, Australia and United States) were selected from 25 countries that are member in the international Cancer Screening Network (ICSN). National guidelines of colorectal cancer screening were approved in the next step. The scond phase was estimating of survival rate of covered populations. The Naive Bayes classifier was selected as one of the data mining technique for estimating. Finally evidence based hybrid decision support system was implemented. Programming language of designed web base system is Javascript. An integrated development environment (IDE) and database of system are respectively Jetbrain webstorm and MySQL. Results: In this study, screening evidence base system was surveyed of six dimensions. These dimensions were general specification, functions, technologies, data resources, users, manual and standards of screening information system. We approved four risk assessment guidelines (High risk, increased, average and low risk), clinical criteria for hereditary syndromes and roadmap of genetic and pathologic analysis. Designed intelligent hybrid system is integrated with registry system. This web base system has detected risk assessment groups by approved guidelines. This system determined 595 individuals with high risk, 185 individuals with increased risk, 20 individuals with average risk and 16 individuals with low risk up to July 30, 2016. Also screening recommendation screening of system was demonstrated by risk groups. Precision of system for detecting of risk groups was 100%. Evidence based system has estimated survival rates of covered populations with precision of 95.6%. Conclusion: This review was presented that evidence based system has improved real-time decision making process. This system has managed vast operation of cancer screening. One of the suggestions for future research is the national integrated evidence based network for all cancers.

Abstract:Introduction: Electronic health records are critical for diagnosis and treatment, but their development can be challenging and time-consuming process. Personal health records provide a practical and accessible solution by empowering patients to take an active role in their care. This study presents a personalized electronic health record system for chronic kidney disease patients, highlighting its potential benefits on patient care.Material and Methods: In 2021, a practical study was conducted to develop and evaluate a personal health records software specifically for patients with chronic kidney disease. The analysis phase involved a literature review and validation process to review the dataset and key software functions. The study primarily focuses on the implementation process and exploratory usability evaluation results of the software. The development and evaluation process involved a health information management specialist and a medical informatics expert, and the software was evaluated by health information technology students using the Nielsen usability evaluation checklist.Results: In the evaluation of the produced software, 12 issues were identified, of which three were related to the principle of user freedom and mastery over the system, three were related to the principle of consistency and standards, one was related to the principle of aiding users in diagnosis, and five were related to error prevention.Conclusion: This study emphasizes the importance of compliance with clinical standards and user experience in developing successful personal health record software. Previous failures in similar systems have been attributed to non-compliance and unmet user expectations. The study aims to identify obstacles and improve the personal health records production process.

Abstract:As part of its data modernization initiative (DMI), the Centers for Disease Control and Prevention, Division of Cancer Prevention and Control is testing and implementing innovative solutions to improve cancer surveillance data quality and timeliness. We describe a consensus-based effort to create a framework to guide the evaluation of cancer surveillance modernization efforts by addressing specific context, processes, and costs related to cancer registration. We drew on prior theories, consulted with experts, and sought feedback from cancer registry staff. We developed the cancer surveillance systems, context, outcomes, and process evaluation (CS-SCOPE) framework to explain the ways in which cancer registry data quality, timeliness, and efficiency are impacted by external and internal contextual factors and interrelated process and content factors. The framework includes implementation measures to understand acceptability of process changes along with outcome measures to assess DMI initiation and ongoing sustainability. The framework's components and structures can be tailored for use in other DMI evaluations.
Evaluating information management system in epidemic infectious diseases in Iran

Abstract:Health information management is a vital and constructive component of the health system, refers to the process of producing and collecting, organising and storing, analysing, disseminating and using information. The aim of this study was to evaluate the strengths and weaknesses of the information management system in epidemic infectious diseases in Iran, specifically focusing on the registration, reporting, quality, confidentiality, and security of infectious disease data. This assessment was conducted from the perspective of policymakers and experts responsible for data registration and reporting. After examining the processes of registering and reporting infectious disease data and interviewing experts, a researcher-designed questionnaire was prepared to evaluate the infectious disease information management system. To assess the content validity of the Content Validity Index and Content Validity Ratio Index, a questionnaire was utilized. The reliability of the questionnaire was confirmed using Cronbach's alpha. By employing purposeful sampling and adhering to the inclusion criteria, 150 participants were included in the study. Questionnaires were distributed via email, WhatsApp, or Telegram to employees at various levels of Iran's health and treatment systems who were responsible for registering and reporting infectious disease data. The study encompassed 100 participants who successfully concluded the research. The results highlight that the key strength of healthcare data registration lies in its ability to "depict the epidemic curve during outbreaks of infectious diseases." Conversely, a notable weakness was the "insufficient collaboration from non-academic sectors (e.g., clinics, private laboratories) in registering and reporting infectious diseases. The present study's findings suggest that the issue lies not in the framework itself, but rather in the execution and functionality of the strategies. We can cultivate a repository of reliable and beneficial data by incorporating initiatives like training programs, enforcing regulations with consequences for inadequate data documentation, offering both material and motivational rewards, and streamlining all data collection and reporting systems.
An Automated Data Verification Approach for Improving Data Quality in a Clinical Registry.

Abstract:BACKGROUND AND OBJECTIVE:The quality of data is crucial for clinical registry studies as it impacts credibility. In the regular practice of most such studies, a vulnerability arises from researchers recording data on paper-based case report forms (CRFs) and further transcribing them onto registry databases. To ensure the quality of data, verifying data in the registry is necessary. However, traditional manual data verification methods are time-consuming, labor-intensive and of limited-effect. As paper-based CRFs and electronic medical records (EMRs) are two sources for verification, we propose an automated data verification approach based on the techniques of optical character recognition (OCR) and information retrieval to identify data errors in a registry more efficiently.METHODS:Three steps are involved to develop the automated verification approach. First, we analyze the scanned images of paper-based CRFs with machine learning enhanced OCR to recognize the checkbox marks and hand-writing. Then, we retrieve the related patient information from the EMRs using natural language processing (NLP) techniques. Finally, we compare the retrieved information in the previous two steps with the data in the registry, and synthesize the results accordingly. The proposed automated method has been applied in a Chinese registry study and the difference between automated and manual approach has been evaluated.RESULTS:The automated approach has been implemented in The Chinese Coronary Artery Disease Registry. For CRF data recognition, the accuracy of recognition for checkboxes marks and hand-writing are 0.93 and 0.74, respectively. For EMR data extraction, the accuracy of information retrieval from textual electronic medical records is 0.97. The accuracy, recall and time consumption of the automated approach are 0.93, 0.96 and 0.5 h, better than the corresponding values of the manual approach, which are 0.92, 0.71 and 7.5 h.CONCLUSIONS:Compared to the manual data verification approach, the automated approach enhances the recall of identify data errors and has a higher accuracy. The time consumed is far less. The results show that the automated approach is more effective and efficient for identifying incomplete data and incorrect data in a registry. The proposed approach has potential to improve the quality of registry data.
An Intelligent Support System for Patient Safety Checklists

Abstract:Safety checklists have gain increasingly attention in recent years as an effective way of standardizing healthcare.They serve as a quality improving tool by enabling providers checking the variance between the actual situation and the best practice prior to the critical activities.However,the traditional checklists in paper form are lack of integration with clinical workflow,specification to individual patients and information system support,which increases the workload of providers and leads to ineffective implementation.This study decomposed safety checklist knowledge into 4 layers,i.e.clinical workflow,checklist form,medical algorithm and supplementary information,aiming to make checklists adjustable and executable by computerized systems.A support system had been developed according to the checklist representation and execution mechanism.The system was able to reduce the providers' workload by integrating checklist into the daily care process of providers for individual patients and providing related patient data and knowledge.A percutaneous coronary intervention peri-operative checklist set had been used to validate the proposed system.By using the system,the process was decomposed into 11 scenarios integrated with the actual clinical workflow.In each scenario,a group of clinical algorithms was applied to produce checkable items.The results indicated that the proposed approach effectively supported the representation and execution of checklists and providers could have benefit from the system by using the process-oriented and patient-specific checklists.

Abstract:Background and aims: One of the primary duties that circulating and scrub nurses fulfill is to record instruments used and procedures adopted during a surgical operation. An appropriate checklist based on evidence and guidelines can reduce the occurrence of legal and ethical complications in this field. The present research was performed to explore perspectives of operating room personnel on both the existing checklist and the improved one that was produced on the basis of evidence and guidelines available in Isfahan's Alzahra University Hospital. Methods: This one-group and two-stage trial study examined perspectives of 28 operating room personnel on both the existing Surgical Safety Checklist and the improved one. The latter was provided on the basis of books, results of studies, and qualified specialists' attitudes. The first section of the questionnaire was distributed to the sample in order to conduct an opinion poll about the present checklist. Next, they were asked to have the improved checklist for 3 weeks. Finally, another opinion poll was carried out about this checklist. The data were analyzed using the SPSS software as well as descriptive and analytical statistical tests. Results: Of the respondents, 25% (7 persons) and 75% (21 persons) were men and women respectively. The majority of the respondents, i.e. 60% (17 persons), had a bachelor's degree. Moreover, 21.6% of the sample had more than 5 years of professional experience. The items in the improved surgical safety checklist were as follows: including a space for the signature of the scrub nurse and personnel during the shift change and mentioning instruments used during an operation as well as different phases of surgery one by one. In general, the personnel's perspectives on the improved checklist, compared with the present checklist, were statistically significantly different (p<0.05). That is to say the improved checklist is a more comprehensive guide than the existing checklist as for keeping a checklist of instruments applied and procedures adopted. Furthermore, it is crucial to mention names of instruments on the checklist completely and individually. Conclusion: It is essential to keep a complete and comprehensive checklist of procedures implemented and instruments used during a surgical operation. It is therefore better to improve and complete the Surgical Safety Checklist on the basis of existing evidence and results so as to enhance operating room personnel's performance.

A qualitative study of design stakeholders' views of developing and implementing a registry-based learning health system

Abstract:Background: New opportunities to record, collate, and analyze routine patient data have prompted optimism about the potential of learning health systems. However, real-life examples of such systems remain rare and few have been exposed to study. We aimed to examine the views of design stakeholders on designing and implementing a US-based registry-enabled care and learning system for cystic fibrosis (RCLS-CF). Methods: We conducted a two-phase qualitative study with stakeholders involved in designing, implementing, and using the RCLS-CF. First, we conducted semi-structured interviews with 19 program personnels involved in design and delivery of the program. We then undertook 11 follow-up interviews. Analysis of interviews was based on the constant comparative method, supported by NVivo software. Results: The organizing principle for the RCLS-CF was a shift to more partnership-based relationships between patients and clinicians, founded in values of co-production, and facilitated by technology-enabled data sharing. Participants proposed that, for the system to be successful, the data it collects must be both clinically useful and meaningful to patients and clinicians. They suggested that the prerequisites included a technological infrastructure capable of supporting data entry and joint decision-making in an accessible way, and a set of social conditions, including willingness from patients and clinicians alike to work together in new ways that build on the expertise of both parties. Follow-up interviews highlighted some of the obstacles, including technical challenges and practical constraints on refiguring relationships between clinicians and patients. Conclusions: The values and vision underlying the RCLS-CF were shared and clearly and consistently articulated by design stakeholders. The challenges to realization were often not at the level of principle, but were both practical and social in character. Lessons from this study may be useful to other systems looking to harness the power of "big data" registries, including patient-reported data, for care, research, and quality improvement.

Abstract:Over the past years the number of medical registries has increased sharply. Their value strongly depends on the quality of the data contained in the registry. To optimize data quality, special procedures have to be followed. A literature review and a case study of data quality formed the basis for the development of a framework of procedures for data quality assurance in medical registries. Procedures in the framework have been divided into procedures for the co-ordinating center of the registry (central) and procedures for the centers where the data are collected (local). These central and local procedures are further subdivided into (a) the prevention of insufficient data quality, (b) the detection of imperfect data and their causes, and (c) actions to be taken / corrections. The framework can be used to set up a new registry or to identify procedures in existing registries that need adjustment to improve data quality.

Abstract:Background and aims: Systems existing in hospital or clinic settings offer services within the physical environment. Examples of such systems include picture archiving and communication systems, which provide remote services for patients. To develop a successful system, methods like software development life cycles (SDLCs) and design techniques, such as prototyping, are needed. This study aimed to specify requirements, design, and evaluation of dental image exchange and management system using a user-centered approach. Methods: This cross-sectional study was conducted in four phases, each corresponding to different stages of SDLCs. User-needs data were used to gathered by interviews and observations. A prototype was developed using object-oriented programming and presented to users for feedback. Finally, focus group was used to finalized the prototype into the desired system. Results: User needs were identified and prioritized from the outset, with ease of use, security, and mobile apps being their most essential requirements. The prototype underwent several iterations of design and evaluation in focus group sessions until users were satisfied, and their feedback was incorporated. Eventually, the prototype was refined into the final system with users' consent. Conclusion: The study revealed that instant access to information, voluntary participation, user interface (UI) design, and usefulness were critical variables for users and should be integral to any system. Successful implementation of such a system requires careful consideration of end-users' needs and their application to the system. Moreover, integrating the system with electronic health records can further enhance the treatment process and the efficiency of medical staff. The voluntary perspective of users played a significant role in achieving an exemplary UI and overall satisfaction with the system. Developers and policymakers should consider these aspects in similar system development projects.
Evaluating Psychometric Properties of the Sultan Bin Abdulaziz Humanitarian City (SBAHC) Nurses' Electronic Handover Tool

Abstract:Introduction: Handover is crucial during the transfer of patient care between healthcare professionals.Successful handover maintains patients' safety and high-quality care. The electronic handover of patients' information is achievable, feasible, and potentially improves the quality of patient care. However, the introduction of the electronic handover is relatively recent and challenging for healthcare providers, especially nurses. Objectives: Since Sultan Bin Abdulaziz Humanitarian City (SBAHC) has recently implemented an electronic handover system among nurses, the main aim of the current study was to develop a tool to assess the perception and barriers to electronic handover systems among nurses working at SBAHC and evaluate its psychometric properties. Methods: The tool content and face validity were assessed by the content validity ratio (CVR). Exploratory and confirmatory factor analysis were used as validity methods, while the test-retest and inter-item consistency were used as reliability measures. The sample size was planned as five times the number of questions and a total of 200 nurses participated in the study. Results: The criteria required for factor analysis were met as indicated by the results of the Kaiser-Mayer-Olkin test and Bartlett's test of sphericity. The reliability results showed a Cronbach's alpha coefficient ranging from 0.858 to 0.910 for the perception subscale and from 0.564 to 0.789 for the barrier subscale, and an overall interclass correlation of 0.986, p<0.001. Conclusions: The developed SBAHC electronic handover tool was valid and reliable, and it is advisable to consider it at the initial stages of implanting an electronic handover system to identify obstacles that are faced by the staff to be considered and addressed by the higher management.

Abstract:Polycystic ovary syndrome (PCOS) is the most common endocrine disorder affecting women of reproductive age. It increases the risk of type 2 diabetes, cardiovascular disease, endometrial cancer, infertility, gestational diabetes, preeclampsia, and preterm birth. Accurately identifying predictors of these health risks is crucial. Electronic health records (EHRs) offer an affordable approach, however, the validity and reliability of EHRs for PCOS diagnosis are unclear. A scoping review of the literature on the prevalence and reliability of the diagnosis of PCOS using EHRs was performed. An analysis of the feasibility of obtaining diagnostic variables from a PCOS patient database was also carried out. Eight studies met the criteria. The prevalence of PCOS ranged from 0.27% to 5.8%. Reliability varied, with one study reporting a sensitivity of 50% and a specificity of 29%. Another study found a 74.4% agreement between international classification of disease (ICD) codes and clinical criteria. The database analysis found only 13.7%, 8%, and 7.5% of women had all the necessary variables for an objective diagnosis of PCOS using the Rotterdam, National Institutes of Health (NIH), and Androgen Excess and PCOS Society (AEPCOS) criteria, respectively. Using EHRs results in an underestimation of PCOS prevalence compared to other diagnostic criteria, and many women identified may not meet the complete diagnostic criteria. These findings have implications for future research studies on PCOS prevalence and related health risks.

Abstract:Chronic obstructive pulmonary disease (COPD) is one of the most severe public health problems worldwide. Pervasive computing technology creates a new opportunity to redesign the traditional pattern of medical system. While many pervasive healthcare systems are currently found in the literature, there is little published research on the effectiveness of these paradigms in the medical context. This paper designs and validates a rule-based ontology framework for COPD patients. Unlike conventional systems, this work presents a new vision of telemedicine and remote care solutions that will promote individual self-management and autonomy for COPD patients through an advanced decision-making technique. Rules accuracy estimates were 89% for monitoring vital signs, and environmental factors, and 87% for nutrition facts, and physical activities.

Abstract:BackgroundThe purpose of this study was to advance a checklist of evaluative criteria designed to assess patient-reported outcome (PRO) measures’ developmental measurement properties and applicability, which can be used by systematic reviewers, researchers, and clinicians with a varied range of expertise in psychometric measure development methodology.MethodsA directed literature search was performed to identify original studies, textbooks, consensus guidelines, and published reports that propose criteria for assessing the quality of PRO measures. Recommendations from these sources were iteratively distilled into a checklist of key attributes. Preliminary items underwent evaluation through 24 cognitive interviews with clinicians and quantitative researchers. Six measurement theory methodological novices independently applied the final checklist to assess six PRO measures encompassing a variety of methods, applications, and clinical constructs. Agreement between novice and expert scores was assessed.ResultsThe distillation process yielded an 18-item checklist with six domains: (1) conceptual model, (2) content validity, (3) reliability, (4) construct validity, (5) scoring and interpretation, and (6) respondent burden and presentation. With minimal instruction, good agreement in checklist item ratings was achieved between quantitative researchers with expertise in measurement theory and less experienced clinicians (mean kappa 0.70; range 0.66–0.87).ConclusionsWe present a simplified checklist that can help guide systematic reviewers, researchers, and clinicians with varied measurement theory expertise to evaluate the strengths and weakness of candidate PRO measures’ developmental properties and the appropriateness for specific applications.

Abstract:Synopsis Of an original sample of 705 elderly (aged over 65) community residents 71% were rescreened after two years and four months using Short CARE. Thirty-one (6·7%) subjects were identified as possible new cases of dementia, implying a mean annual inception rate among the survivors of 26 per 1000. These were subjected to full clinical assessment, which yielded 10 cases of Alzheimer's disease (8 per 1000) and 5 multi-infarct dementia (4 per 1000). The remainder of the cases consisted of 6 with a dementia secondary to other causes, 1 case of mixed dementia and there was insufficient information to classify the remaining 3. Six were found not to be suffering from dementia. This yields an overall annual incidence rate for dementia among the survivors of 20 per 1000.

Abstract:Donor‐specific HLA antibody (DSA)‐mediated graft injury is the major cause of kidney loss. Among DSA characteristics, graft homing has been suggested as an indicator of severe tissue damage. We analyzed the role of de novo DSA (dnDSA) graft homing on kidney transplantation outcome. Graft biopsy specimens and parallel sera from 48 nonsensitized pediatric kidney recipients were analyzed. Serum samples and eluates from graft biopsy specimens were tested for the presence of dnDSAs with flow bead technology. Intragraft dnDSAs (gDSAs) were never detected in the absence of serum dnDSAs (sDSAs), whereas in the presence of sDSAs, gDSAs were demonstrated in 72% of biopsy specimens. A significantly higher homing capability was expressed by class II sDSAs endowed with high mean fluorescence intensity and C3d‐ and/or C1q‐fixing properties. In patients with available sequential biopsy specimens, we detected gDSAs before the appearance of antibody‐mediated rejection. In sDSA‐positive patients, gDSA positivity did not allow stratification for antibody‐mediated graft lesions and graft loss. However, a consistent detection of skewed unique DSA specificities was observed over time within the graft, likely responsible for the damage. Our results indicate that gDSAs could represent an instrumental tool to identify, among sDSAs, clinically relevant antibody specificities requiring monitoring and possibly guiding patient management.
