Abstract:OBJECTIVE:To evaluate hippocampal volume changes and neuropsychological performances in patients with either amnestic mild cognitive impairment (aMCI) or Alzheimer's disease (AD). METHODS:Thirty-eight AD dementia, 22 aMCI patients, and 20 healthy controls were enrolled. Bilateral hippocampal volume was measured concurrently with mini-mental state examination (MMSE), auditory verbal learning test (AVLT), Boston naming test (BNT), and activities of daily living (ADL) test. Baseline and two additional follow-up examinations were conducted. RESULTS:Baseline hippocampal volumes were significantly smaller in AD group than that in aMCI and control groups. MMSE, AVLT, ADL, and BNT scores for the AD group were significantly different from that of both aMCI and control groups. Baseline hippocampal volumes were positively correlated with MMSE and AVLT scores in AD and aMCI patients. At follow-up, left hippocampal volume loss was positively correlated with decreased MMSE and AVLT scores both in AD and aMCI groups, while right hippocampal volume loss was positively associated with decreased AVLT performance only in AD group. Increased ADL and decreased BNT scores were positively associated with left hippocampal volume reduction only in the AD group. CONCLUSIONS:Current findings provide evidence of a close relationship between hippocampal volume and cognitive performances in patients with AD and aMCI, both at baseline and over follow-up.
Relationship Between Afternoon Napping and Cognitive Function in the Ageing Chinese Population

Abstract:BackgroundSeveral studies have shown that afternoon napping promotes cognitive function in the elderly; on the other hand, some studies have shown opposite results. This current study further examined the relationship between afternoon napping and cognitive function in the ageing Chinese population.MethodsA total of 2214 elderly were included (napping group: n=1534; non-napping group: n=680). They all received cognitive evaluations by the Beijing version of the Montreal Cognitive Assessment, the Mini-Mental State Examination, and the Chinese version of the Neuropsychological Test Battery. Among all the subjects, 739 elderly volunteered to take blood lipid tests.ResultsSignificant differences in cognitive function and blood lipids were observed between the napping and the non-napping groups. Afternoon napping was associated with better cognitive function including orientation, language, and memory in the present study. Subjects with the habit of afternoon napping also showed a higher level of triglyceride than the non-napping subjects.ConclusionThe results demonstrated that afternoon napping was related to better cognitive function in the Chinese ageing population.

Abstract:Apolipoprotein E (ApoE) ε4 allele and cerebral blood flow (CBF) changes are related to the increased risk of cognitive impairment independently. However, whether there are interactions between ApoE ε4 and CBF on memory performance in older adults with normal cognition remains unknown. This study determined whether the association between CBF and memory performance could be moderated by ApoE ε4 within a sample of cognitively normal older adults from the ADNI. 62 participants, including 23 with ApoE ε4 (ApoE ε4+) and 39 without ApoE ε4 (ApoE ε4−), underwent resting CBF measurement and memory testing. CBF was measured by arterial spin labeling MRI and memory performance was evaluated by the Rey Auditory Verbal Learning Test. By using linear regression models, CBF was negatively associated with memory function in ApoE ε4+ group, whereas positively in ApoE ε4− group by contrast. This study suggests that different CBF-memory relationships can be detected in cognitively normal ApoE ε4 carriers compared to ApoE ε4 non-carriers. Associations between hyperperfusion and worse memory performance in ApoE ε4 carriers may reflect vascular and/or cellular dysfunction.
Longitudinal Associations between Daytime Napping and Cognitive Function in Chinese Older Adults

Abstract:Background Little was known about the longitudinal associations between daytime napping and cognitive function in China. Thus, the study aimed to explore the cross-sectional and the longitudinal relationship between daytime napping and cognitive performance in the elderly Chinese population. Methods The data was from the China Health and Retirement Longitudinal Study (CHARLS). Daytime napping was self-reported. Cognitive function was assessed via a structured questionnaire in two dimensions: episodic memory and mental status. Linear regression and mixed-effect model were applied to explore the association between daytime napping and cognitive function. Results A total of 2,875 and 2,440 participants aged over 65 years were included in the cross-sectional and the longitudinal studies, respectively. In the cross-sectional study, non-nappers and extended nappers had significantly lower global cognition scores ( P <0.01), as well as significantly lower scores for episodic memory ( P <0.05) and mental status ( P <0.01), compared with moderate nappers. In the longitudinal analysis, no napping and extended napping were significantly associated with global cognitive decline ( P <0.05) and only extended napping showed the significant association for the decline in episodic memory as well as mental status ( P <0.01). Limitations Daytime napping duration was self-reported by participants. Conclusion The study found a longitudinal association between extended napping duration and worse cognitive function.

Association of postlunch napping duration and night-time sleep duration with cognitive impairment in Chinese elderly: a cross-sectional study.

Abstract:Objective To analyse the independent and combined associations of postlunch napping duration and night-time sleep duration with risk of cognitive impairment among Chinese elderly. Design A cross-sectional study. Setting We analysed the data from Zhejiang Ageing and Health Cohort, a population-based survey of seven counties located in Zhejiang province in eastern China. Participants 10 740 participants aged 60 years or older were included in final analysis. Primary and secondary outcome measures Cognitive impairment was assessed through Mini-Mental State Examination. Data on sleep-related characteristics was collected in the behavioural habits section within the questionnaire. Results Relative to participants with 1-30 min of postlunch napping, those who did not nap and who napped longer had significantly higher risks for cognitive impairment. OR of cognitive impairment were 1.41 (95% CI 1.14 to 1.75) for participants with longer night-time sleep duration (>= 9 hours), compared with those sleeping 7-8.9 hours. In addition, combined effects were further identified. Participants with both longer night-time sleep duration (>= 9 hours) and longer postlunch napping duration (>60 min) (OR=2.01, 95% CI 1.30 to 3.13), as well as those with both longer night-time sleep duration (>= 9 hours) and appropriate postlunch napping duration (1-30 min) (OR=2.01, 95% CI 1.20 to 3.38), showed significantly higher risk of cognitive impairment than those with sleeping 7-8 hours and napping 1-30 min. Meanwhile, a 34% increase in odds of cognitive impairment was observed in participants with both shorter night-time sleep duration (5-6.9 hours) and no napping. Conclusion Both postlunch napping duration and night-time sleep duration were independently and jointly associated with cognitive impairment, which needs verification in prospective studies.
Daytime napping, biological aging and cognitive function among middle-aged and older Chinese: insights from the China health and retirement longitudinal study

Abstract:Objective: The complicated association of daytime napping, biological aging and cognitive function remains inconclusive. We aimed to evaluate the cross-sectional and longitudinal associations of daytime napping and two aging measures with cognition and to examine whether napping affects cognition through a more advanced state of aging. Methods: Data was collected from the China Health and Retirement Longitudinal Study. Napping was self-reported. We calculated two published biological aging measures: Klemera and Doubal biological age (KDM-BA) and physiological dysregulation (PD), which derived information from clinical biomarkers. Cognitive z-scores were calculated at each wave. Linear mixed models were used to explore the longitudinal association between napping, two aging measures, and cognitive decline. Mediation analyses were performed to assess the mediating effects of biological age acceleration on the association between napping and cognition. Results: Participants aged over 45 years were included in the analyses. Non-nappers had greater KDM-BA and PD [LS means (LSM) = 0.255, p = 0.007; LSM = 0.085, p = 0.011] and faster cognitive decline (LSM = -0.061, p = 0.005)compared to moderate nappers (30-90 min/nap). KDM-BA (β = -0.007, p = 0.018) and PD (β = -0.034, p < 0.001) showed a negative association with overall cognitive z scores. KDM-BA and PD partially mediated the effect of napping on cognition. Conclusion: In middle-aged and older Chinese, compared to moderate nappers, non-nappers seem to experience a more advanced state of aging and increased rates of cognitive decline. The aging status possibly mediates the association between napping and cognition. Moderate napping shows promise in promoting healthy aging and reducing the burden of cognitive decline in Chinese middle-aged and older adults.
Associations Between Total Sleep Duration and Cognitive Function among Middle-Aged and Older Chinese Adults: Does Midday Napping Have an Effect on It?

Abstract:AIM:The effect of sleep duration on cognitive function has been reported. However, the studies about the combined effects of total sleep duration and midday napping on cognition in elders were limited and inconclusive. We aimed to investigate the associations between total sleep duration, midday napping and cognitive function among middle-aged and older Chinese adults.METHODS:Based on the 3rd wave of the China Health and Retirement Longitudinal Study (CHARLS) in 2015, a total of 9218 participants aged ≥45 years with completed cognition measurements were included. Cognitive functions were assessed by a combined global cognition score of episodic memory and mental status. Information about sleep-related variables, demographic characteristics, and health status were collected by validated questionnaires. Multivariate linear regression models were performed to evaluate the associations between total sleep duration, midday napping, and cognitive function. Stratified analyses were used to explore the potential effect modifier.RESULTS:Overall, the global cognition score was 10.38 ± 4.30 among the participants (mean age: 61.5 ± 8.7 years). For sleep duration, both short sleep and long sleep duration were significantly associated with the increased risk of cognitive impairment after controlling for demographics and other confounders. Compared with sleeping for 7h per day, the adjusted β-coefficient and 95% CI of the risk of cognitive impairment was -0.967 (95% CIs: -1.191, -0.742) for ≤5h, -0.257 (-0.498, -0.016) for 6h, -0.424 (-0.650, -0.198) for 8h and -0.664 (-0.876, -0.452) for ≥9h. The combined effect analysis indicated that subjects with extended or without naps had a significantly higher risk of cognitive impairment in the ≤5h sleep time group (ref: 7h) and in extended nappers or without naps group (ref: short nappers). Subjects with extended naps or without naps might increase the risk of cognitive impairment, especially those having ≤5h total sleep time. Stratified analysis showed that participants aged ≥60 years without social activity increased the risk of cognitive decline.CONCLUSION:An inverted U-shaped association was observed between total sleep duration and cognitive function in Chinese elders, especially in those aged ≥60 years or without social activity. Short midday naps could mitigate the deleterious effects of poor sleep quality and shorter sleep duration on cognitive function. The findings could help us identify the vulnerable population and decrease the burden of cognitive impairment.

Abstract:Abstract Introduction Prior studies of older adults have demonstrated that the development and progression of Alzheimer’s disease are tightly linked with longer daytime naps in a potentially bidirectional manner. It remains unclear whether daytime sleep behaviors in middle-to-older aged adults are linked to the risk for dementia. Methods We studied 85,037 cognitively intact non-shift-workers (baseline age=63 years, SD=8, range: 43-69; 57% female) in the UK Biobank cohort who completed 7-day actigraphy and had been followed for up to 8 years. We implemented the Cole-Kripke algorithm to identify daytime sleep episodes and considered three types of napping metrics: (1) mean nap duration between 9am-7pm; (2) intra-individual variability (individual SD) in the nap duration between 9am-7pm; (3) timing of napping quantified as the percentage of nap duration in each 2-h bin between 9am-7pm (9-11am, 11am-1pm, 1-3pm, 3-5pm, 5-7pm). Incidences of all-cause dementia and mild cognitive impairment (MCI) were identified via ICD-10 codes. Nap variables were square-root transformed to correct for skewness. We conducted Cox proportional hazards models, adjusting for demographic characteristics, lifestyle, comorbidities, polygenic genetic risk score for Alzheimer's disease, nighttime sleep, and chronotype. Results Almost all participants (99.97%) napped at least once. Median nap duration was 0.40 hours per day (IQR=0.20-0.78), and intra-individual variability of nap duration was 0.39 (IQR=0.19-0.69) hours. Twenty-eight percent (IQR=12%-52%) of the naps were taken between 9-11am, 5% (IQR=0%-17%) between 11am-1pm, 9% (IQR=0%-22%) between 1-3pm, 16% (IQR=1%-29%) between 3-5pm, and 17% (IQR=4%-32%) between 5-7pm. During follow-up, 557 (0.66%) participants developed MCI or dementia on average 4.62 (range: 0.01-7.98) years after baseline. Longer nap duration (for 1-SD, HR=1.21, 95%CI: 1.12-1.31, p< 0.0001), larger intra-individual variability (for 1-SD, HR=1.14, 95%CI: 1.04-1.24, p=0.004), and higher percentage of naps between 1-3pm (for 1-SD, HR=1.11, 95%CI: 1.02-1.22, p=0.017) were associated with increased risk for MCI/dementia. Conclusion Longer and increased intra-individual variability in daytime nap as well as higher percentage of naps in the early afternoon are associated with a greater risk of MCI/dementia in middle-to-older aged adults. These findings highlight the potential importance of monitoring napping in screening for the risk for developing dementia. Abstract:Abstract Introduction Recent evidence suggests that older adults with more frequent and longer daytime naps have an increased risk of Alzheimer’s dementia (AD). We aim to further elucidate whether the timing of daytime napping impacts AD risk. Methods Data were from 1,203 participants in the Rush Memory and Aging Project (mean age at actigraphy=80.85±7.30; 77% female). Each participant wore an accelerometer (Actical) for up to 14 (mean 9.57±1.17) days and have been followed for up to 17 (mean 6.92±4.12) years with annual clinical assessment to determine AD status. We used a validated algorithm to identify daytime nap episodes between 9am-7pm and determined the timing of naps based on the midpoint of each nap. We computed the proportion of naps within each 2-h window for each participant, i.e., morning (9-11am), noon (11am-1pm), early afternoon (1-3pm), late afternoon (3-5pm), and early evening (5-7pm). Cox proportional hazards models were performed to examine the associations of napping timing with risk of AD, with adjustment for age, sex, education years, and nighttime sleep duration. Results Among all participants, 357 (30%) developed AD during the follow-up (mean time to AD: 6.34±3.98 years). Taking all naps in the morning, compared to taking all naps at other times, is associated with a 1.9-fold increased AD risk (HR [95%CI]=1.93[1.002-3.58], p=0.04). This association was only observed in women (HR=2.22[1.08-4.38], p=0.03) but not in men (HR=1.01[0.18-4.38], p=0.99). Moreover, the association between naps in the morning and AD risk was attenuated when additionally adjusted for total nap duration and frequency, BMI, and intradaily variability in rhythms (HR=1.71[0.85-3.43], p=0.13). The proportions of naps during other times of the day were not associated with AD risk (noon: HR=1.02[0.44-2.23]; early afternoon: HR=1.28[0.72-2.22]; late afternoon: HR=0.62[0.33-1.15]; early evening: HR=0.62[0.30-1.25]; ps>0.05). Conclusion More naps in the morning were associated with an elevated risk of developing AD in older adults. A higher propensity of napping in the morning may be an indicator of disturbances in sleep-wake regulation. Future studies are warranted to test whether interventions to consolidate nap timing or limit napping in the morning can improve neurocognitive outcomes. Abstract:Study objectives: Increasing evidence suggests that napping is associated with cognitive impairment and dementia, but the conclusions are inconsistent. Moreover, the extent of the risk is uncertain. We therefore conducted a systematic review and meta-analysis to quantify the connection between napping and cognitive impairment. Methods: We performed a systematic search of PubMed, EMBASE, Web of Science, and Cochrane Library for studies that were published up to June 2023, and assessed associations between napping and cognitive impairment. Odds ratios (ORs) and 95% confidence intervals (CIs) were calculated as the effect sizes for all studies. Heterogeneity and potential publication biases were assessed. Results: A total of 4535 papers were retrieved, with 20 reports assessing the relationships between napping and cognitive impairment. Pooled analysis indicated that napping was associated with dementia (OR = 1.14; 95% CI: 1.07-1.21). Importantly, we found that those napping longer than 30, 45, and 60 min/day were 35%, 41%, and 40%, respectively, more likely to have an increased risk of cognitive impairment (30 min: OR = 1.35; 95% CI: 1.24-1.48; 45 min: OR = 1.41; 95% CI: 1.27-1.58; 60 min: OR = 1.40; 95% CI: 1.26-1.56). North America and Europe showed that associations existed between napping and cognitive impairment (North America: OR = 1.15; 95% CI: 1.04-1.27; Europe: OR = 1.13; 95% CI: 1.08-1.18). Conclusions: This meta-analysis indicated associations between long napping durations and cognitive impairment or dementia, suggesting that longer napping might be a potential risk factor of adverse cognitive outcomes.
Systematic review and meta-analyses on the effects of afternoon napping on cognition

Abstract:Naps are increasingly considered a means to boost cognitive performance. We quantified the cognitive effects of napping in 60 samples from 54 studies. 52 samples evaluated memory. We first evaluated effect sizes for all tests together, before separately assessing their effects on memory, vigilance, speed of processing and executive function. We next examined whether nap effects were moderated by study features of age, nap length, nap start time, habituality and prior sleep restriction. Naps showed significant benefits for the total aggregate of cognitive tests (Cohen's d = 0.379, CI95 = 0.296-0.462). Significant domain specific effects were present for declarative (Cohen's d = 0.376, CI95 = 0.269-0.482) and procedural memory (Cohen's d = 0.494, CI95 = 0.301-0.686), vigilance (Cohen's d = 0.610, CI95 = 0.291-0.929) and speed of processing (Cohen's d = 0.211, CI95 = 0.052-0.369). There were no significant moderation effects of any of the study features. Nap effects were of comparable magnitude across subgroups of each of the 5 moderators (Q values = 0.009 to 8.572, p values > 0.116). Afternoon naps have a small to medium benefit over multiple cognitive tests. These effects transcend age, nap duration and tentatively, habituality and prior nocturnal sleep.
Effects of Anosognosia on Static and Dynamic Amplitudes of Low-Frequency Fluctuation in Mild Cognitive Impairment

Abstract:Background: Anosognosia is a significant symptom in patients with mild cognitive impairment (MCI) while the underlying neurological mechanism behind it is still unclear.Methods: A total of 121 subjects were included and classified into three groups, including 39 normal controls (NCs), 42 individuals with MCI without anosognosia (MCI-NA), and 40 individuals with MCI with anosognosia (MCI-A), based on their everyday cognition (ECog) questionnaire (discrepancy score). Resting-state functional MRIs were acquired from all the subjects, and the static amplitudes of low-frequency fluctuation (sALFF) and dynamic ALFF (dALFF) variance were investigated to evaluate the intrinsic functional network strength and stability, respectively, and both were corrected by age, sex, education, and gray matter volume. Eventually, correlation analyses were conducted to explore the relationship between brain activity changes and cognitive status in all the subjects.Results: No significant difference was found between MCI-A and MCI-NA (P > 0.05) in cognitive ability. Regarding intrinsic brain activity, MCI-A had increased sALFF and dALFF variance in the anterior cingulate cortex (ACC) relative to MCI-NA, as well as decreased sALFF and dALFF variance in the precuneus relative to MCI-NA and controls. Moreover, MCI-A had decreased sALFF in the inferior temporal gyrus (ITG) and paracentral lobule (PCL) compared to MCI-NA. Among all the subjects, correlation analyses showed that the sALFF and dALFF variance in the precuneus was related to the Ecog discrepancy score (r = 0.232 and 0.235, respectively), immediate story recall (r = 0.200 and 0.277, respectively), and delayed story recall (r = 0.255 and 0.298, respectively).Conclusion: Alterations of intrinsic brain activation in the ACC and precuneus seem to be associated with the anosognosia symptom in patients with MCI.
Daytime Napping, Incident Atrial Fibrillation, and Dynamic Transitions With Dementia

Abstract:Background: Associations between napping and incident atrial fibrillation (AF) remain unknown, and few studies have accounted for dynamic transitions between AF and dementia. Objectives: The purpose of this study was to evaluate associations between napping with incident AF and the dynamic transitions of AF and dementia, as well as the mediation pathway of left ventricular (LV) size and function. Methods: A total of 476,588 participants from UK Biobank were included. Napping frequency and other sleep behaviors were evaluated. Incident AF, dementia, and mortality were ascertained via linkage to external registry databases. LV size and function indices were obtained from cardiovascular magnetic resonance imaging phenotypes. A multistate survival analysis was conducted to examine daytime napping in relation to dynamic transitions. Weighed AF genetic risk score was calculated. Results: Frequent daytime napping, compared to never/rarely napping, was associated with a 1.17-fold AF risk (HR: 1.17; 95% CI: 1.12-1.22), which persisted after controlling for other sleep behaviors. Genetic predisposition significantly modified associations between napping and AF (P for interaction <0.001), with stronger associations observed in those of low and moderate genetic risk. LV ejection fraction significantly mediated 26.2% (95% CI: 4.2%-74.1%) of associations between napping and AF. Frequent napping was also associated with a 1.27-fold risk of transition from AF to comorbidity of AF and dementia. Conclusions: Our findings highlight the potential importance of screening for napping in view of the association with incident AF and dementia. Routine evaluations of the LV ejection fraction could be warranted to timely identify early indications of AF onset among habitual nappers.
Increased Functional Connectivity Between Nucleus Basalis of Meynert and Amygdala in Cognitively Intact Elderly along the Alzheimer’s Continuum

Abstract:Background: A growing body of research reported the degeneration of the basal forebrain (BF) cholinergic system in the early course of Alzheimer’s disease (AD). However, functional changes of the BF in asymptomatic individuals along the Alzheimer’s continuum remain unclear. Methods: A total of 229 cognitively intact participants were included from the Alzheimer's Disease Neuroimaging Initiative dataset and further divided into four groups based on the “A/T” profile using amyloid and tau positron emission tomography (PET). All A-T+ subjects were excluded. One hundred and seventy-three subjects along the Alzheimer’s continuum (A-T-, A+ T-, A+ T+) were used for further study. The seed-based functional connectivity (FC) maps of the BF subregions (Ch1-3 and Ch4 [nucleus basalis of Meynert, NBM]) with whole-brain voxels were constructed. Analyses of covariance to detect the between-group differences and to further investigated the relations between FC values and AD biomarkers or cognition. Results: We found increased FC between right Ch4 and bilateral amygdala among three groups, and the FC value could well distinguish between the A-T- group and the Alzheimer’s continuum groups. Furthermore, increased FC between the Ch4 and amygdala was associated with higher pathological burden reflected by amyloid and tau PET in the entire population as well as better logistic memory function in A + T+ group. Conclusion: Our study demonstrated the NBM functional connectivity increased in cognitively normal elderly along the Alzheimer’s continuum, which indicated a potential compensatory mechanism to counteract pathological changes in AD and maintain intact cognitive function.
The Relationship Between Activity Level and Cognitive Function in Chinese Community-Dwelling Elderly

Abstract:ABSTRACT To study the relationship between daily activity level and cognitive function in community-dwelling elderly. We collected demographic features, cognitive function, activity level and self-rating depression scale scores in 53 community-dwelling olderly aged 60 years or above. The activity level and moderate-to-vigorous physical activity (MVPA) time were assessed by using the accelerometer for 7 consecutive days. We compared activity level, MVPA time and depression scores between cognitive impaired and normal groups. Cognitive functions were compared in groups with different MVPA level, and the correlation between cognitive function and MVPA time was analysed. Of the 53 subjects, 27 had varying degrees of cognitive impairment. Individuals with cognitive impairment shown significantly shorter MVPA time and higher depression score compared to the cognitive normal group (P < 0.05). After controlling for confounding factors (age, BMI), MVPA time was associated with cognitive function (r = 0.358, P = 0.009). The memory factor score correlated with MVPA time (r = 0.357, P = 0.012) and mean activity level (r = 0.287, P = 0.046). Moderate-to-vigorous physical activity in the elderly was positively related to their cognitive function. Strengthening daily activities may beneficial for the elderly to maintain better cognitive function.

Abstract:Background: As one of the key features of sleep, sleep duration (SD) has been confirmed to be associated with multiple health outcomes. However, the link between SD and cognitive function (CF) is still not well understood. Methods: We employed a combined approach utilizing data from the National Health and Nutrition Examination Survey (NHANES 2011-2014) and Mendelian Randomization (MR) methods to investigate the relationship between SD and CF. In the NHANES cross-sectional analysis, the association between these variables was primarily examined through multivariate linear regression to explore direct correlations and utilized smoothing curve fitting to assess potential nonlinear relationships. To ensure the robustness of our findings, subgroup analyses were also conducted. MR analysis was used to assess the causal relationship between SD and sleeplessness on CF. After excluding confounding factors, univariate and multivariate MR were performed using inverse variance weighting (IVW) as the main analysis method, and sensitivity analysis was performed. Results: The results of our cross-sectional study indicate a notable negative association between SD and CF, forming an inverted U-shaped curve with the inflection point occurring at SD = 6 h. This relationship remains consistent and robust across subgroup analyses differentiated by variables such as age, levels of physical activity, and frequency of alcohol intake. In MR analysis, IVW analysis showed no causal relationship between SD and sleeplessness on CF (Both P > 0.05). Conclusion: Cross-sectional studies suggest the existence of an inverted U-shaped correlation between SD and CF among the elderly. However, MR analysis did not reveal a causal relationship between SD and CF, which the lack of nonlinear MR analysis may limit. These findings provide evidence from a sleep perspective for optimizing cognitive strategies in older adults.

Abstract:Introduction: Motoric cognitive risk syndrome (MCR), characterized by subjective cognitive complaints and slow gait in older populations, is associated with sleep duration. However, the association between MCR and daytime nap duration has not been thoroughly explored. Methods: Baseline data from the China Health and Retirement Longitudinal Study (CHARLS) were used in this study. MCR was defined as the coexistence of subjective cognitive complaints and objective slow gait speed without a history of dementia or mobility disability. Daytime nap duration was categorized into four groups: no napping, short napping (<30 min), moderate napping (30-89 min) and extended napping (≥90 min). Multivariable logistic regression models were used to explore the association of daytime napping duration and MCR. Results: A total of 4230 individuals aged ≥60 were included in the current analysis, of which 463 were diagnosed with MCR. Moderate napping of 30-89 min per day was found to be significantly associated with lower odds of MCR compared with the reference group of no napping. In subgroup analysis, individuals with sleep durations of <7 h per night had lower odds of MCR in the model that adjusted for all potential confounders with ≥30 min daytime nap duration compared with no napping. Interestingly, for people with a night sleep duration of 7-8 h, only those with a moderate nap of 30-89 min had lower odds of MCR than non-nappers after adjustment for potential confounders. Conclusion: A moderate nap of 30-89 min could lower the odds of MCR, especially for older adults with a night sleep duration of ≤8 h.

Abstract:The apolipoprotein E (APOE) ε4 allele is the best-known genetic risk factor for developing sporadic Alzheimer’s disease (AD). Recently, the resting-state functional magnetic resonance imaging (rsfMRI) has been widely used to investigate brain functional differences between ε4 carriers and non-carriers. Two fundamental approaches have dominated the rsfMRI related literature to date: seed-based correlation analysis and independent component analysis (ICA). However, these studies had to select regions of interest or subnetwork to conduct functional connectivity analysis, which may result in selection bias. In the current study, we used a model-free, data-driven approach, the eigenvector centrality mapping (ECM) to detect the difference of interconnectedness of brain areas between cognitively intact APOE ε4 carriers and ε3 homozygotes. Moreover, we investigated whether changes in eigenvector centrality (EC) were related to behavior data and to AD related pathology. The current study included 13 cognitively intact, elderly APOE ε4 carriers (9F, 75.24 ±5.92y, with at least one copy of APOE ε4 allele) and 22 well-matched ε3 homozygotes (9F, 78.01 ±5.85y). Based on Matlab, the fastECM (Wink et al., 2012) was used to assess the brain interconnectedness. The AD related pathology was measured by aβ42, total-tau (t-tau) and phosphorylated-tau (p-tau) in cerebrospinal fluid (CSF). All subjects underwent a battery of neuropsychological tests assessed their general cognitive functional and other cognitive domains including memory, executive function and language ability. Comparisons of ECM between two groups were conducted, and subsequently, the correlation between the differential EC values and cognitive ability as well as AD related pathology was analyzed. This study suggests that the presence of the APOE ε4 allele was linked with functional connectivity decrease in certain nodes of brain networks, which contributed to verbal memory function and may reflect accumulated AD-related pathological concentrations.

Abstract:Though sleep disturbance constitutes the risk factor for Alzheimer's disease (AD), the underlying mechanism is still unclear. This study aims to explore the interaction between sleep disturbances and AD on brain function. We included 192 normal controls, 111 mild cognitive impairment (MCI), and 30 AD patients, with either poor or normal sleep (PS, NS, respectively). To explore the strength and stability of brain activity, we used static amplitude of low-frequency fluctuation (sALFF) and dynamic ALFF (dALFF) variance. Further, we examined white matter hyperintensities (WMH) and amyloid PET deposition, representing the vascular risk factor and AD-related hallmark, respectively. We observed that sleep disturbance significantly interacted with disease severity, exposing distinct effects on sALFF and dALFF variance. Interestingly, PS groups showed the dALFF variance trajectory of initially increased, then decreased and finally increased along the AD spectrum, while showing the opposite trajectory of sALFF. Further correlation analysis showed that the WMH burden correlates with dALFF variance in PS groups. Conclusively, our study suggested that sleep disturbance interacts with AD severity, expressing as effects of compensatory in MCI and de-compensatory in AD, respectively. Further, vascular impairment might act as important pathogenesis underlying the interaction effect between sleep and AD.
