Abstract:Background Sleep behavior (eg, sleep duration, sleep quality, and nap) is closely related to many chronic conditions. However, less is known about its association with multiple chronic conditions (multimorbidity), particularly evidence from cohort studies. Methods Data were collected from a cohort of 8 937 individuals aged 45 and older from the China Health and Retirement Longitudinal Study (2011-2018). Sleep duration, sleep quality, and nap duration were collected in 2011 and 2013. Progression of multimorbidity was defined as the first report of 2 or more chronic conditions for participants without multimorbidity or the new report of 1 or more conditions for those with multimorbidity. Cox regression models were performed to calculate the hazard ratios and 95% confidence intervals (CIs) of the associations between sleep behaviors and the progression of multimorbidity. Results Short sleep duration and poor sleep quality were associated with the progression of multimorbidity independently and jointly, especially in those less than 65 years and females. The U-shaped dose-response relationships were observed between nighttime and total sleep duration and the progression of multimorbidity. Persistent short and unsteadily changed sleep behaviors increased the risk of multimorbidity progression. Individuals sleeping <= 5 h/night with 5-7 restless days/week had 1.53 times higher risk of multimorbidity progression (95% CI: 1.37-1.71), compared to those sleeping 7-8 h/night with Conclusions Short sleep duration and poor sleep quality were independently and jointly associated with a higher risk of multimorbidity progression in a mid-to-older population. Optimal sleep duration and sleep quality should be emphasized in multimorbidity prevention and control.

Abstract:AIM:The effect of sleep duration on cognitive function has been reported. However, the studies about the combined effects of total sleep duration and midday napping on cognition in elders were limited and inconclusive. We aimed to investigate the associations between total sleep duration, midday napping and cognitive function among middle-aged and older Chinese adults.METHODS:Based on the 3rd wave of the China Health and Retirement Longitudinal Study (CHARLS) in 2015, a total of 9218 participants aged ≥45 years with completed cognition measurements were included. Cognitive functions were assessed by a combined global cognition score of episodic memory and mental status. Information about sleep-related variables, demographic characteristics, and health status were collected by validated questionnaires. Multivariate linear regression models were performed to evaluate the associations between total sleep duration, midday napping, and cognitive function. Stratified analyses were used to explore the potential effect modifier.RESULTS:Overall, the global cognition score was 10.38 ± 4.30 among the participants (mean age: 61.5 ± 8.7 years). For sleep duration, both short sleep and long sleep duration were significantly associated with the increased risk of cognitive impairment after controlling for demographics and other confounders. Compared with sleeping for 7h per day, the adjusted β-coefficient and 95% CI of the risk of cognitive impairment was -0.967 (95% CIs: -1.191, -0.742) for ≤5h, -0.257 (-0.498, -0.016) for 6h, -0.424 (-0.650, -0.198) for 8h and -0.664 (-0.876, -0.452) for ≥9h. The combined effect analysis indicated that subjects with extended or without naps had a significantly higher risk of cognitive impairment in the ≤5h sleep time group (ref: 7h) and in extended nappers or without naps group (ref: short nappers). Subjects with extended naps or without naps might increase the risk of cognitive impairment, especially those having ≤5h total sleep time. Stratified analysis showed that participants aged ≥60 years without social activity increased the risk of cognitive decline.CONCLUSION:An inverted U-shaped association was observed between total sleep duration and cognitive function in Chinese elders, especially in those aged ≥60 years or without social activity. Short midday naps could mitigate the deleterious effects of poor sleep quality and shorter sleep duration on cognitive function. The findings could help us identify the vulnerable population and decrease the burden of cognitive impairment.

Abstract:Objective: The complicated association of daytime napping, biological aging and cognitive function remains inconclusive. We aimed to evaluate the cross-sectional and longitudinal associations of daytime napping and two aging measures with cognition and to examine whether napping affects cognition through a more advanced state of aging. Methods: Data was collected from the China Health and Retirement Longitudinal Study. Napping was self-reported. We calculated two published biological aging measures: Klemera and Doubal biological age (KDM-BA) and physiological dysregulation (PD), which derived information from clinical biomarkers. Cognitive z-scores were calculated at each wave. Linear mixed models were used to explore the longitudinal association between napping, two aging measures, and cognitive decline. Mediation analyses were performed to assess the mediating effects of biological age acceleration on the association between napping and cognition. Results: Participants aged over 45 years were included in the analyses. Non-nappers had greater KDM-BA and PD [LS means (LSM) = 0.255, p = 0.007; LSM = 0.085, p = 0.011] and faster cognitive decline (LSM = -0.061, p = 0.005)compared to moderate nappers (30-90 min/nap). KDM-BA (β = -0.007, p = 0.018) and PD (β = -0.034, p < 0.001) showed a negative association with overall cognitive z scores. KDM-BA and PD partially mediated the effect of napping on cognition. Conclusion: In middle-aged and older Chinese, compared to moderate nappers, non-nappers seem to experience a more advanced state of aging and increased rates of cognitive decline. The aging status possibly mediates the association between napping and cognition. Moderate napping shows promise in promoting healthy aging and reducing the burden of cognitive decline in Chinese middle-aged and older adults.
Association of postlunch napping duration and night-time sleep duration with cognitive impairment in Chinese elderly: a cross-sectional study.

Abstract:Objective To analyse the independent and combined associations of postlunch napping duration and night-time sleep duration with risk of cognitive impairment among Chinese elderly. Design A cross-sectional study. Setting We analysed the data from Zhejiang Ageing and Health Cohort, a population-based survey of seven counties located in Zhejiang province in eastern China. Participants 10 740 participants aged 60 years or older were included in final analysis. Primary and secondary outcome measures Cognitive impairment was assessed through Mini-Mental State Examination. Data on sleep-related characteristics was collected in the behavioural habits section within the questionnaire. Results Relative to participants with 1-30 min of postlunch napping, those who did not nap and who napped longer had significantly higher risks for cognitive impairment. OR of cognitive impairment were 1.41 (95% CI 1.14 to 1.75) for participants with longer night-time sleep duration (>= 9 hours), compared with those sleeping 7-8.9 hours. In addition, combined effects were further identified. Participants with both longer night-time sleep duration (>= 9 hours) and longer postlunch napping duration (>60 min) (OR=2.01, 95% CI 1.30 to 3.13), as well as those with both longer night-time sleep duration (>= 9 hours) and appropriate postlunch napping duration (1-30 min) (OR=2.01, 95% CI 1.20 to 3.38), showed significantly higher risk of cognitive impairment than those with sleeping 7-8 hours and napping 1-30 min. Meanwhile, a 34% increase in odds of cognitive impairment was observed in participants with both shorter night-time sleep duration (5-6.9 hours) and no napping. Conclusion Both postlunch napping duration and night-time sleep duration were independently and jointly associated with cognitive impairment, which needs verification in prospective studies.

These features include the substantial role of externalities, the high degree of specialization, the critical role of quality and timeliness, and the high level of knowledge required to participate in the health care system at all levels. These characteristics have important implications for the design of health policy in general, and especially for a decentralized system of service delivery and sector management. This chapter outlines the decentralization health policies and programs of Indonesia, the Philippines, and Vietnam, focusing on the period 1985–2003, spanning the years before and after significant decentralization began in these countries. The chapter also points to areas where reforms may facilitate more effective health care delivery.

Abstract:Background Little was known about the longitudinal associations between daytime napping and cognitive function in China. Thus, the study aimed to explore the cross-sectional and the longitudinal relationship between daytime napping and cognitive performance in the elderly Chinese population. Methods The data was from the China Health and Retirement Longitudinal Study (CHARLS). Daytime napping was self-reported. Cognitive function was assessed via a structured questionnaire in two dimensions: episodic memory and mental status. Linear regression and mixed-effect model were applied to explore the association between daytime napping and cognitive function. Results A total of 2,875 and 2,440 participants aged over 65 years were included in the cross-sectional and the longitudinal studies, respectively. In the cross-sectional study, non-nappers and extended nappers had significantly lower global cognition scores ( P <0.01), as well as significantly lower scores for episodic memory ( P <0.05) and mental status ( P <0.01), compared with moderate nappers. In the longitudinal analysis, no napping and extended napping were significantly associated with global cognitive decline ( P <0.05) and only extended napping showed the significant association for the decline in episodic memory as well as mental status ( P <0.01). Limitations Daytime napping duration was self-reported by participants. Conclusion The study found a longitudinal association between extended napping duration and worse cognitive function.

Abstract:Introduction: Motoric cognitive risk syndrome (MCR), characterized by subjective cognitive complaints and slow gait in older populations, is associated with sleep duration. However, the association between MCR and daytime nap duration has not been thoroughly explored. Methods: Baseline data from the China Health and Retirement Longitudinal Study (CHARLS) were used in this study. MCR was defined as the coexistence of subjective cognitive complaints and objective slow gait speed without a history of dementia or mobility disability. Daytime nap duration was categorized into four groups: no napping, short napping (<30 min), moderate napping (30-89 min) and extended napping (≥90 min). Multivariable logistic regression models were used to explore the association of daytime napping duration and MCR. Results: A total of 4230 individuals aged ≥60 were included in the current analysis, of which 463 were diagnosed with MCR. Moderate napping of 30-89 min per day was found to be significantly associated with lower odds of MCR compared with the reference group of no napping. In subgroup analysis, individuals with sleep durations of <7 h per night had lower odds of MCR in the model that adjusted for all potential confounders with ≥30 min daytime nap duration compared with no napping. Interestingly, for people with a night sleep duration of 7-8 h, only those with a moderate nap of 30-89 min had lower odds of MCR than non-nappers after adjustment for potential confounders. Conclusion: A moderate nap of 30-89 min could lower the odds of MCR, especially for older adults with a night sleep duration of ≤8 h.
The Relationship Between Depression, Daytime Napping, Daytime Dysfunction, and Snoring in 0.5 Million Chinese Populations: Exploring the Effects of Socio-Economic Status and Age

Abstract:Background Daytime napping has been postulated as both a protective and a risk factor for depression in previous studies. In addition to these conflicting results, research gaps also exist with regard to controlling confounding bias between daytime napping and depression and examining the potential association within the Chinese population. To facilitate the prevention and diagnosis of depression, this study aims to provide insight into the association of daytime napping and depression in 0.5 million Chinese adults by fully controlling confounders, and further examine the modifying effects of socio-economic status (SES) and age. Methods Data were drawn from the baseline of a Chinese cohort study of 0.5 million adults. Depressive status was measured by the Composite International Diagnostic Inventory (CIDI). Logistic regression models were used to examine the association between depression and daytime napping adjusted for SES, sleep-related factors, lifestyle factors and related diseases. Further stratified analyses were conducted to identify the modifying effects of socio-economic status and age. Results The odds ratio of depression by daytime napping was 1.15 (95% CI: 1.01–1.31) in females and 1.42 (95% CI: 1.18–1.71) in males. Factors including living in a rural area (OR = 1.31, 95% CI: 1.13–1.52), receiving less education (OR = 1.42, 95% CI: 1.22–1.66), getting married (OR = 1.24, 95% CI: 1.10–1.40) and being 45–65 years old (OR = 1.29, 95% CI: 1.12–1.49) had a modifying effect on daytime napping and depression that could strengthen the association. Conclusions A significantly positive association was found between depression and daytime napping, as well as daytime dysfunction, snoring and both shorter and longer sleep duration. Lower SES and age could possibly modify the association. Further clinical or epidemiological studies are needed to investigate the mechanism and facilitate the prevention of depression.

Abstract:Background: In order to lessen the burden of Alzheimer's disease (AD), timely and efficient management and intervention methods for mild cognitive impairment (MCI) are crucial. MCI is seen as a transitional stage between normal aging and dementia. Although sarcopenia is an important risk factor for MCI, it is unclear what factors mediates and regulates the brain-muscle communication. Our objective was to investigate the indirect moderating effects of sleep duration and leisure activity on depressive symptoms, sarcopenia and MCI. Method: Panel data from the 2015 China Health and Retirement Longitudinal Study (CHARLS) database was used in this investigation. we used Bootstrap sampling to determine the relationship between sleep duration, leisure activity, depressive symptoms, sarcopenia, and MCI in mediation and indirect moderation models. The outcome measurements were odds ratio (OR) and confidence interval (CI). Result: After adjusting for confounding variables, we discovered that sarcopenia and its traits, such as handgrip strength, gait speed, standing test, and muscle mass, were significantly correlated with MCI. Second, the results implied that depressive symptoms played a role in modulating the link between physical function, muscle strength, and MCI. This moderating effect was impacted by short sleep duration and moderate to high levels of leisure activities. Conclusion: We discovered that MCI was highly correlated not only with physical function and muscle strength but also with depressed symptoms, which acted as a partially mediating factor in this connection. Handgrip strength, gait speed, and standing test mediated the correction of MCI caused by depression symptoms. Importantly, leisure activities and sleep duration had indirect moderating effects on the above associations, and future management policies should take these factors into account.

Abstract:The current study examined the relationship between napping duration and metabolic syndrome (MetS) among Chinese middle-aged and older adults over 4 years. We included 4,526 individuals aged ≥50 years who completed the 2011 and 2015 waves of the China Health and Retirement Longitudinal Study. General linear models were conducted to analyze the association of napping duration (none, 1 to 29 minutes, 30 to 59 minutes, 60 to 89 minutes, and ≥90 minutes) with MetS. At baseline, a higher prevalence of MetS was observed among participants with longer napping durations (60 to 89 minutes or ≥90 minutes) compared to non-nappers (odds ratio [OR] = 1.27, OR = 1.51, respectively). Among all participants, a napping duration ≥90 minutes at baseline was correlated with an increased risk of MetS 4 years later (OR = 1.58). Among participants without MetS at baseline, excessive napping (≥90 minutes) was predictive of a higher incidence of MetS 4 years later (OR = 1.46). Results revealed that excessive napping was related to an increased prevalence and incidence of MetS among Chinese middle-aged and older adults. [Research in Gerontological Nursing, xx(x), xx-xx.].

Abstract:BackgroundSeveral studies have shown that afternoon napping promotes cognitive function in the elderly; on the other hand, some studies have shown opposite results. This current study further examined the relationship between afternoon napping and cognitive function in the ageing Chinese population.MethodsA total of 2214 elderly were included (napping group: n=1534; non-napping group: n=680). They all received cognitive evaluations by the Beijing version of the Montreal Cognitive Assessment, the Mini-Mental State Examination, and the Chinese version of the Neuropsychological Test Battery. Among all the subjects, 739 elderly volunteered to take blood lipid tests.ResultsSignificant differences in cognitive function and blood lipids were observed between the napping and the non-napping groups. Afternoon napping was associated with better cognitive function including orientation, language, and memory in the present study. Subjects with the habit of afternoon napping also showed a higher level of triglyceride than the non-napping subjects.ConclusionThe results demonstrated that afternoon napping was related to better cognitive function in the Chinese ageing population.

Abstract:Abstract Introduction Prior studies of older adults have demonstrated that the development and progression of Alzheimer’s disease are tightly linked with longer daytime naps in a potentially bidirectional manner. It remains unclear whether daytime sleep behaviors in middle-to-older aged adults are linked to the risk for dementia. Methods We studied 85,037 cognitively intact non-shift-workers (baseline age=63 years, SD=8, range: 43-69; 57% female) in the UK Biobank cohort who completed 7-day actigraphy and had been followed for up to 8 years. We implemented the Cole-Kripke algorithm to identify daytime sleep episodes and considered three types of napping metrics: (1) mean nap duration between 9am-7pm; (2) intra-individual variability (individual SD) in the nap duration between 9am-7pm; (3) timing of napping quantified as the percentage of nap duration in each 2-h bin between 9am-7pm (9-11am, 11am-1pm, 1-3pm, 3-5pm, 5-7pm). Incidences of all-cause dementia and mild cognitive impairment (MCI) were identified via ICD-10 codes. Nap variables were square-root transformed to correct for skewness. We conducted Cox proportional hazards models, adjusting for demographic characteristics, lifestyle, comorbidities, polygenic genetic risk score for Alzheimer's disease, nighttime sleep, and chronotype. Results Almost all participants (99.97%) napped at least once. Median nap duration was 0.40 hours per day (IQR=0.20-0.78), and intra-individual variability of nap duration was 0.39 (IQR=0.19-0.69) hours. Twenty-eight percent (IQR=12%-52%) of the naps were taken between 9-11am, 5% (IQR=0%-17%) between 11am-1pm, 9% (IQR=0%-22%) between 1-3pm, 16% (IQR=1%-29%) between 3-5pm, and 17% (IQR=4%-32%) between 5-7pm. During follow-up, 557 (0.66%) participants developed MCI or dementia on average 4.62 (range: 0.01-7.98) years after baseline. Longer nap duration (for 1-SD, HR=1.21, 95%CI: 1.12-1.31, p< 0.0001), larger intra-individual variability (for 1-SD, HR=1.14, 95%CI: 1.04-1.24, p=0.004), and higher percentage of naps between 1-3pm (for 1-SD, HR=1.11, 95%CI: 1.02-1.22, p=0.017) were associated with increased risk for MCI/dementia. Conclusion Longer and increased intra-individual variability in daytime nap as well as higher percentage of naps in the early afternoon are associated with a greater risk of MCI/dementia in middle-to-older aged adults. These findings highlight the potential importance of monitoring napping in screening for the risk for developing dementia. Abstract:STUDY OBJECTIVES:To examine the association between sleep-related factors and memory impairment. DESIGN:Cross-sectional study SETTING:Community-based study in Guangzhou, China. PARTICIPANTS:28,670 older Chinese (20,776 women and 7,894 men) aged 50 to 85 years. MEASUREMENTS AND RESULTS:Demographic and socioeconomic data, sleep-related factors, and cognitive function were collected by face-to-face interview. Potential confounders, such as employment and occupational status, smoking, alcohol and tea use, physical activity, self-rated health, anthropometry, blood pressure, and fasting plasma glucose and lipids were measured. After adjusting for multiple potential confounders, an inverted U-shaped association between sleep duration and delayed word recall test (DWRT) score, a validated measure of memory impairment, was found, with 7 to 8 h of habitual sleep duration showing the highest score (P-values for trend from 3 to 7 h and from 7 to ≥ 10 h were all ≤ 0.001). Compared to sleep duration of 7 h, the adjusted odds ratio for memory impairment from the sleep duration of 3 to 4 or ≥ 10 h was 1.29 (95% confidence interval 1.07-1.56) and 1.52 (1.25-1.86), respectively. Subjects with daily napping, morning tiredness, or insomnia had significantly lower DWRT scores than those without (P ranged from < 0.001 to 0.01). CONCLUSIONS:Short or long sleep duration was an important sleep-related factor independently associated with memory impairment and may be a useful marker for increased risk of cognitive impairment in older people.

A total of 2,959 valid samples were included, and the relationship between sleep and depression was explored by logistic regression, restricted cubic spline, and isotemporal substitution model. Results: In the cross-sectional analysis, no statistical relationship was observed between napping time and depression in the elderly. The optimal sleep interval for the elderly at night is 6-7.5 hours, and the health benefits are the largest. A sleep duration of < 6 hours at night (OR = 2.25, 95% CI: 1.90 to 2.65) was associated with a high likelihood of depression. The probability of depression in the elderly continues to decrease with the increase of time after the nighttime sleep duration reaches 6 hours and is at the lowest level of about 7.5 hours. Moreover, the probability of depression will increase after the sleep duration exceeds 9.5 hours. In the range of 6-7.5 hours of recommended sleep duration, the likelihood of depression in the elderly will be reduced by 0.311 for every 30-minute increase in nighttime sleep time instead of noon sleep time. Conclusion: The duration of nighttime sleep and the probability of depression have a U-shaped relationship. The likelihood of depression was lowest in the elderly who slept for 6-8 hours at night, and the likelihood of depression could be reduced by increasing the nighttime sleep time instead of napping time within the optimal nighttime sleep range.

Abstract:Purpose: This study aimed to investigate the prevalence of depressive symptoms and the association between multimorbidity and depressive symptoms among the elderly in eastern China. Patients and methods: A multicenter cross-sectional study was conducted in four cities (Jianggan, Yiwu and Anji in Zhejiang Province and Taixing in Jiangsu Province) in eastern China. We collected the information on 27 chronic conditions through the self-reported medical history and used the Geriatric Depression Scale (GDS) short form to evaluate depressive symptoms. Multivariate Logistic regression was used to evaluate the relationship between multimorbidity and depressive symptoms. Results: Five thousand two hundred and ninety-six participants were included into the current study, among which 2687 (50.74%) were female, with the mean +/- SD age 72.0 +/- 8.1 years old. The overall prevalence of depressive symptoms in eastern China was 23.5%. And the percentage of multimorbidity in depressed participants was higher compared with non-depressed participants (50.8% vs 38.8%, P<0.001). The univariate model and adjusted model suggested that participants with multiple chronic diseases were more likely to have a depressive symptom (adjusted OR=1.42; 95% CIs 1.19-1.70). Conclusion: Depressive symptom was significantly associated with multimorbidity among the community-dwelling elderly population in eastern China.
Study on the association of midday napping with risks of cardiovascular diseases incidence among Huixian resident in Henan Province

Abstract:Objective To understand the association of daytime napping with risk of cardiovascular disease（CVD） incidence among Henan Huixian residents,and provide the theory basis for health education of daytime napping habit in this area. Methods The data were from Henan Xinxiang Huixian data of China Chronic Diseases Prospective Study,59 866 residents（30-79 year old）in Henan Huixian served as the subjects. The investigation was performed with the electronic questionnaire（including sociodemographic characteristics,lifestyles,disease history,family history,mental state,and sleeping habits）. The following-up was conducted by different ways,the following up outcome were the diagnosis of CVD（except for nonlethal hypertension）,death,loss of following up or December 31,2017. Logistic regression（categorical variable） and linear regression continuous variables were used to describe the baseline characteristics after adjusting age and gender in residents with different midday napping habits. Cox proportional hazard regression was conducted. The sub-group analysis was performed to analyze the inter-reaction. The used software was Stata 15.0. Results Of 59 866 subjects,73.7% of subjects had summer midday napping habit,13.9% of subjects had perennial midday napping habit. During a mean follow-up of 9.2±3.1 years,17 142 cases of CVD were diagnosed,CVD incidence was 28.6%,and the crude cumulative incidence density was 29.2/1 000 person-years. Multivariate cox regression analyses showed that after adjusting potential confounding factors,as compared with residents without midday napping habit,the residents with summer midday napping habit had the higher risk of CVD（HR=1.07）,and the residents with perennial midday napping habit had the higher risk of CVD,ischemic heart diseases,cerebrovascular diseases and ischemic stroke（HR values were 1.11,1.12,1.15and 1.14）,P＜0.05. The inter-reaction analysis showed that age and gender had modified effect on association of midday napping with CVD（P values of inter-reaction were 0.001 and 0.024）. In residents with ＜65 year old（HR values were 1.10 and 1.16） and females（HR values were 1.09 and 1.17）,the association of summer midday napping habit and perennial midday napping habit with high risk of CVD was found. Conclusion Among Henan Huixian residents,the perennial midday napping might be associated with the increased risk of CVD incidence. It should pay attention to perennial midday napping,as a possible risk factor for CVD.

Abstract:Background: The relationship between daytime napping and depression remains debatable. Thus, a meta-analysis in this study was conducted to evaluate the relationship between daytime napping and depression. Methods: The PubMed, Embase, Web of Science, and China National Knowledge Infrastructure databases were searched up to February 2022, and the reference lists of the included studies were also retrieved. A random-effects model was used to estimate the combined effect size. Results: Nine studies with 649,111 participants were included in the final analysis. The pooled odds ratio (OR) was 1.15 (95% confidence interval: 1.01–1.31) with a significant heterogeneity ( I 2 = 91.3%, P for heterogeneity <0.001), and the results demonstrated an increased risk of depressive symptoms among daytime nappers. Visual inspection of the funnel plot and Egger's and Begg's tests identified no obvious evidence of publication bias. Conclusion: This meta-analysis indicates that daytime naps are a predictor of depression. The effects of daytime napping on depression may vary depending on the characteristics of people, the pattern of naps, and the individual's sleep experience. The findings may have significant implications for future research on depression.

Abstract:Background: As one of the key features of sleep, sleep duration (SD) has been confirmed to be associated with multiple health outcomes. However, the link between SD and cognitive function (CF) is still not well understood. Methods: We employed a combined approach utilizing data from the National Health and Nutrition Examination Survey (NHANES 2011-2014) and Mendelian Randomization (MR) methods to investigate the relationship between SD and CF. In the NHANES cross-sectional analysis, the association between these variables was primarily examined through multivariate linear regression to explore direct correlations and utilized smoothing curve fitting to assess potential nonlinear relationships. To ensure the robustness of our findings, subgroup analyses were also conducted. MR analysis was used to assess the causal relationship between SD and sleeplessness on CF. After excluding confounding factors, univariate and multivariate MR were performed using inverse variance weighting (IVW) as the main analysis method, and sensitivity analysis was performed. Results: The results of our cross-sectional study indicate a notable negative association between SD and CF, forming an inverted U-shaped curve with the inflection point occurring at SD = 6 h. This relationship remains consistent and robust across subgroup analyses differentiated by variables such as age, levels of physical activity, and frequency of alcohol intake. In MR analysis, IVW analysis showed no causal relationship between SD and sleeplessness on CF (Both P > 0.05). Conclusion: Cross-sectional studies suggest the existence of an inverted U-shaped correlation between SD and CF among the elderly. However, MR analysis did not reveal a causal relationship between SD and CF, which the lack of nonlinear MR analysis may limit. These findings provide evidence from a sleep perspective for optimizing cognitive strategies in older adults.
Associations between Sleep Quality, Depression, and Mild Cognitive Impairment in Community-dwelling Older Persons

Abstract:Abstract Background Few studies have examined the trilateral relationships among sleep quality, depression, and cognitive function. Therefore, this study aimed to explore these relationships in an older population. Furthermore, we examined mediation models for sleep quality, depression, and mild cognitive impairment (MCI). Methods From December 2020 to July 2021, a cross-sectional survey was conducted among 5000 older people 60 years and over in Shanghai communities, of whom 4897 (97.9%) provided responses, using a random cluster sampling method. Sleep quality and depression status were assessed using the Pittsburgh Sleep Quality Index (PSQI) and 30-item Geriatric Depression Scale (GDS-30), respectively. The Barthel Index (BADL) and Ascertain Dementia 8 (AD-8) scales were combined to assess cognitive function. T-tests and chi-square tests were used to compare the sociodemographic characteristics of the participants. Pearson’s correlations were used to determine the direction and strength of the relationships between the primary variables. We further assessed the mediating role of sleep quality and depression. Results The prevalence of MCI was 21.7%. After adjusting for covariates, there were bidirectional relationships between sleep quality and depression and between MCI and depression. The significant mediating effect of sleep quality on the association between depressive symptoms and cognition function (95% CI: 0.0011 to 0.0026, p = 0.001) and mediated19.29% of the total effect of depressive symptoms on cognitive function. The mediation analysis showed that sleep quality had an indirect effect on cognitive function through the mediating variable of depression (95% CI: 0.0030 to 0.0050, p = 0.001) and mediated 36.50% of the total effect of sleep quality on cognitive function. Meanwhile, sleep quality partially mediated the relationship between cognitive function and depression symptoms (95% CI: 0.4594 to 0.7643, p = 0.001) and mediated26.77% of the total effect of cognitive function on depression. Conclusion Sleep quality and depression had significant direct (independent) and indirect (as a mediator) effects on MCI. We believe that improving screening for depression and sleep quality among older persons could significantly contribute to their health and well-being.
