Abstract:Background The optimal management of febrile stem cell transplant (SCT) patients presenting without severe neutropenia (absolute neutrophil count [ANC] ≥ 500/μL) is unclear. The authors have developed iterative risk prediction models (Esbenshade Vanderbilt [EsVan] models) that reliably predict bloodstream infections (BSIs) in the febrile general pediatric oncology population without severe neutropenia, but SCT‐specific data are limited. Methods All SCTs occurring from May 2005 to November 2019 at a single institution were identified. Episodes of fever with a central venous catheter and ANC values ≥ 500/μL were abstracted. All previous versions of the EsVan model were applied to the SCT data, and c‐statistics were generated. The models were additionally applied to each type of transplant (autologous/allogeneic), and a new allogeneic model that further adjusted for metrics of immunosuppression, Esbenshade Vanderbilt Allogeneic SCT Model (EsVanAlloSCT), was developed and internally validated. Results For 429 SCT episodes (221 autologous and 208 allogeneic), the BSI incidence was 19.6% (84 of 429), and it was higher in allogeneic transplant patients (25.5%) than autologous transplant patients (14.0%; p < .01). All versions of the EsVan model performed well for the overall SCT cohort (c‐statistics, 0.759–0.795). The EsVan models performed better for the autologous episodes (c‐statistics, 0.869–0.881) than the allogeneic SCT episodes (c‐statistics, 0.678–0.717). The new allogeneic transplant–specific model, EsVanAlloSCT, which added an adjustment for the extent of immunosuppression, yielded a c‐statistic of 0.792 (bootstrap‐corrected, 0.750). Conclusions The EsVan models work exceptionally well when they are applied to autologous SCT, but they work less well for allogeneic SCT. EsVanAlloSCT appears to improve the predictive ability in allogeneic SCT, but it will need additional external validation.

Predicting fever in neutropenia with safety-relevant events in children undergoing chemotherapy for cancer: The prospective multicenter SPOG 2015 FN Definition Study

Abstract:Background: Fever in neutropenia (FN) remains a frequent complication in pediatric patients undergoing chemotherapy for cancer. Preventive strategies, like primary antibiotic prophylaxis, need to be evidence-based. Procedure: Data on pediatric patients with any malignancy from the prospective multicenter SPOG 2015 FN Definition Study (NCT02324231) were analyzed. A score predicting the risk to develop FN with safety-relevant events (SRE; bacteremia, severe sepsis, intensive care unit admission, death) was developed using multivariate mixed Poisson regression. Its predictive performance was assessed by internal cross-validation and compared with the performance of published rules. Results: In 238 patients, 318 FN episodes were recorded, including 53 (17%) with bacteremia and 68 (21%) with SRE. The risk-prediction score used three variables: chemotherapy intensity, defined according to the expected duration of severe neutropenia, time since diagnosis, and type of malignancy. Its cross-validated performance, assessed by the time needed to cover (TNC) one event, exceeded the performance of published rules. A clinically useful score threshold of ≥11 resulted in 2.3% time at risk and 4.1 months TNC. Using external information on efficacy and timing of intermittent antibiotic prophylaxis, 4.3 months of prophylaxis were needed to prevent one FN with bacteremia, and 5.2 months to prevent one FN with SRE, using a threshold of ≥11. Conclusions: This score, based on three routinely accessible characteristics, accurately identifies pediatric patients at risk to develop FN with SRE during chemotherapy. The score can help to design clinical decision rules on targeted primary antibiotic prophylaxis and corresponding efficacy studies.

Abstract:Background Fever in neutropenia (FN) remains a serious complication of childhood cancer therapy. Clinical decision rules (CDRs) are recommended to help distinguish between children at high and low risk of severe infection. The aim of this analysis was to develop new CDRs for three different outcomes and to externally validate published CDRs. Procedure Children undergoing chemotherapy for cancer were observed in a prospective multicenter study. CDRs predicting low from high risk infection regarding three outcomes (bacteremia, serious medical complications (SMC), safety relevant events (SRE)) were developed from multivariable regression models. Their predictive performance was assessed by internal cross-validation. Published CDRs suitable for validation were identified by literature search. Parameters of predictive performance were compared to assess reproducibility. Results In 158 patients recruited between April 2016 and August 2018, 360 FN episodes were recorded, including 56 (16%) with bacteremia, 30 (8%) with SMC and 72 (20%) with SRE. The CDRs for bacteremia and SRE used four characteristics (type of malignancy, severely reduced general condition, leucocyte count <0.3 G/L, bone marrow involvement), the CDR for SMC two characteristics (severely reduced general condition and platelet count <50 G/L). Eleven published CDRs were analyzed. Six CDRs showed reproducibility, but only one in both sensitivity and specificity. Conclusions This analysis developed CDRs predicting bacteremia, SMC or SRE at presentation with FN. In addition, it identified six published CDRs that show some reproducibility. Validation of CDRs is fundamental to find the best balance between sensitivity and specificity, and will help to further improve management of FN.

Abstract:OBJECTIVE: To derive, cross validate, and externally validate a clinical prediction model that assesses the risks of different serious bacterial infections in children with fever at the emergency department.DESIGN: Prospective observational diagnostic study.SETTING: Three paediatric emergency care units: two in the Netherlands and one in the United Kingdom.PARTICIPANTS: Children with fever, aged 1 month to 15 years, at three paediatric emergency care units: Rotterdam (n=1750) and the Hague (n=967), the Netherlands, and Coventry (n=487), United Kingdom. A prediction model was constructed using multivariable polytomous logistic regression analysis and included the predefined predictor variables age, duration of fever, tachycardia, temperature, tachypnoea, ill appearance, chest wall retractions, prolonged capillary refill time (>3 seconds), oxygen saturation <94%, and C reactive protein.MAIN OUTCOME MEASURES: Pneumonia, other serious bacterial infections (SBIs, including septicaemia/meningitis, urinary tract infections, and others), and no SBIs.RESULTS: Oxygen saturation <94% and presence of tachypnoea were important predictors of pneumonia. A raised C reactive protein level predicted the presence of both pneumonia and other SBIs, whereas chest wall retractions and oxygen saturation <94% were useful to rule out the presence of other SBIs. Discriminative ability (C statistic) to predict pneumonia was 0.81 (95% confidence interval 0.73 to 0.88); for other SBIs this was even better: 0.86 (0.79 to 0.92). Risk thresholds of 10% or more were useful to identify children with serious bacterial infections; risk thresholds less than 2.5% were useful to rule out the presence of serious bacterial infections. External validation showed good discrimination for the prediction of pneumonia (0.81, 0.69 to 0.93); discriminative ability for the prediction of other SBIs was lower (0.69, 0.53 to 0.86).CONCLUSION: A validated prediction model, including clinical signs, symptoms, and C reactive protein level, was useful for estimating the likelihood of pneumonia and other SBIs in children with fever, such as septicaemia/meningitis and urinary tract infections.

Abstract:Purpose: Sepsis and bacteraemia are both referred to as bloodstream infection (BSI). It contributed majorly to the mortality of the children with blood cancer. In order to reveal the clinical features of BSI and improve its outcome, we performed the investigation among the inpatients in haematology/oncology department. Methods: Neutropenia was defined as an absolute neutrophil count (ANC) in peripheral blood of <0.5×10 9 /L. Microbiologically documented infection (MDI) was defined when causative pathogen was isolated from blood. Drug susceptibility test was performed using the VITEK-60 Auto Microbic System and Kirby-Baue disk diffusion method. Relative cytokines in the sera were detected through flow cytometry (FCM). All of the data were analysed with STATA software (version 9.0). Results: The incidence of BSI was 10.73% (161/1500), the rate of MDI for BSI was 43.48% (70/161). There was no statistical significance between non-MDI and MDI groups on the clinical characteristics. Total 79 strains were detected from 70 blood samples. Gram-positive bacteria, gram-negative bacteria and fungi accounted for 55.70% (44/79), 43.04% (34/79) and 1.27% (1/79), respectively. The most common pathogens were Staphylococcus epidermidis (20.25%), Escherichia coli (15.19%) and Klebsiella pneumoniae (15.19%). Staphylococcus showed no/low resistance to linezolid, vancomycin and furantoin. Gram-negative bacteria showed no/low resistance to cefoxitin, imipenem, piperacillin/tazobactam and amikacin. The mortality of BSI was 3.1% (5/161). Conclusions: BSI had no specific signs and symptoms clinically, and gram- positive bacteria were the dominant pathogens. Antibiotics selection according to the susceptibility test was superior to the empirical treatment. Improved prevention, early detection, and advanced therapeutic strategies might decrease the incidence of the BSI and improve its treatment outcome.

Abstract:Abstract Background Objectives were to build a machine learning algorithm to identify bloodstream infection (BSI) among pediatric patients with cancer and hematopoietic stem cell transplantation (HSCT) recipients, and to compare this approach with presence of neutropenia to identify BSI. Methods We included patients 0–18 years of age at cancer diagnosis or HSCT between January 2009 and November 2018. Eligible blood cultures were those with no previous blood culture (regardless of result) within 7 days. The primary outcome was BSI. Four machine learning algorithms were used: elastic net, support vector machine and two implementations of gradient boosting machine (GBM and XGBoost). Model training and evaluation were performed using temporally disjoint training (60%), validation (20%) and test (20%) sets. The best model was compared to neutropenia alone in the test set. Results Of 11,183 eligible blood cultures, 624 (5.6%) were positive. The best model in the validation set was GBM, which achieved an area-under-the-receiver-operator-curve (AUROC) of 0.74 in the test set. Among the 2236 in the test set, the number of false positives and specificity of GBM vs. neutropenia were 508 vs. 592 and 0.76 vs. 0.72 respectively. Among 139 test set BSIs, six (4.3%) non-neutropenic patients were identified by GBM. All received antibiotics prior to culture result availability. Conclusions We developed a machine learning algorithm to classify BSI. GBM achieved an AUROC of 0.74 and identified 4.3% additional true cases in the test set. The machine learning algorithm did not perform substantially better than using presence of neutropenia alone to predict BSI.

Abstract:BackgroundBacteremia is a common infectious complication among pediatric oncology patients, yet management practices are not standardized. Antibiotic treatment courses as short as seven days are supported by literature and could improve patient quality of life and resource use. However, it is not known if short courses are associated with greater treatment failure in immunocompromised patients.MethodsWe conducted a retrospective study at St. Jude Children's Research Hospital, including patients with a laboratory confirmed bloodstream bacterial infection between April 1, 2022, and March 31, 2023. Clinical characteristics such as age, gender, type of malignancy, phase of treatment, underlying chronic conditions, and clinical sites of infection were collected. Antibiotics used, duration of therapy, and route of administration were also gathered to examine clinical management. Clinical outcomes were measured by documenting ICU admission, oxygen, fluid and vasopressor requirements, date of hospital discharge, bacteremia recurrence, and mortality.Results86 patients contributed 112 bacteremia episodes that met eligibility criteria. Severe neutropenia was present at onset in 68% of all episodes and 32% of patients did not have neutrophil recovery over 500 cells/ml during the episode. Gram-negative organisms accounted for 52% of isolated organisms, with the most common organisms isolated overall being Escherichia coli (19%), followed by Klebsiella pneumoniae (14%), and Viridans group streptococcus (14%). The median length of antibiotic treatment was 12 days. Treatment duration was not associated with oncologic diagnosis or Gram-negative versus Gram-positive infection.ConclusionBacteremia occurs more frequently in patients who are neutropenic, especially those who are severely neutropenic with an absolute neutrophil count less than 500 cells/mL. Treatment duration is not associated with primary cancer diagnosis or type of bacteremia. In this single high resource setting, bloodstream infections resulted in significant morbidity, resource use, and a mortality rate of 8%, highlighting opportunities for investigation to improve clinical practices.

Abstract:Background: Bloodstream infection (BSI) remains a considerable cause of morbidity and mortality for cancer patients. With a better understanding of it, better methods can be used. The primary objective of this study was to describe the characteristics of BSIs in our institution, and the second was to determine the possible risk factors associated with them. Materials and Methods: Data of cancer patients from 2009 to 2015 at our institution were included. Medical information and blood cultures were analyzed to determine the BSI rate. The χ2 and Fisher exact tests were used for categorical data and to determine risk factors associated with BSIs and pathogens. Results: A total of 565 (8.6%) events were diagnosed with BSIs. Although Gram-negative bacteria (52.6%) were the most commonly isolated pathogens, Gram-positive microorganisms (45.0%) were also prevalent. Oral and gastrointestinal organisms were common. Pathogens were more likely to be identified in patients with central venous catheters and in patients with prolonged neutropenia (P<0.05). Conclusions: This study provides updated epidemiology in BSIs and helps with the prevention and management of suspected BSIs in vulnerable patients. Better anti-infection therapy could be provided to these patients based on the isolated pathogens.
Risk of Mortality in Adult Cancer Febrile Neutropenia Patients with a Machine Learning Approach.

Abstract:e13562 Background: Febrile neutropenia (FN) is a major dose-limiting toxicity of myelosuppressive therapy in cancer patients and impose substantial morbidity and mortality risks. We analyzed National Inpatient Sample (NIS) database to understand the trend in FN and predict mortality in cancer patients who experienced FN using a machine learning (ML) approach. Methods: We used the Healthcare Cost and Utilization Project (HCUP) Nationwide Inpatient Sample (NIS) data. Cancer discharges were identified using the HCUP Clinical Classifications Software (CCS) categorizations (11-45), within which FN patients were identified with International Classification of Diseases, Ninth Revision, Clinical Modification (ICD-9-CM) codes (ICD-9-CM: 7806 and 288, 2841 or 1125). We examined trends in FN discharges and mortality rates using during 2003 – 2015, and then built various models on NIS 2013 data to predict mortality. Results: The percentage of discharges secondary to cancer-related FN kept increasing from 0.2% in 2003 and stabilized around 2% since 2010. However, associated cost has continued to rise (average cost of $36,853 vs $105,912 respectively, in 2003 and 2015). The unadjusted mortality rate also kept increasing from 2.2% in 2003 and has stabilized around ~ 4.2% since 2010. The logistic regression model showed significantly higher odds of mortality with advancing age (2.8%, 3.8%, 7%, 8.3% respectively, for age group 18-44, 45-64, 65-84, 85+). Presence of bacteremia compared to no bacteremia (4.5 vs, 3.3, P < 0.01), non-teaching hospitals compared to teaching hospital (4.9% vs, 4.3%, P < 0.01), and comorbidities such as hypertension, paralysis, hypothyroidism, depression showed higher risk of mortality (P < 0.05). Patients admitted over weekend and transferred from another institute had higher odds of mortality (P < 0.01). Conclusions: Even in this era of widespread use of granulocyte colony-stimulating factor for primary prophylaxis and less cytotoxic immunotherapy, mortality from FN remains significantly higher and continues to impose significant financial burden. Identification of risk factors and predicting mortality risk will help clinicians take better therapeutic decisions and thus patient survival.
The Role of Metagenomics Next-Generation Sequencing (mNGS) in Early Diagnosis of Bloodstream Infection in Hematologic Patients with Febrile Neutropenia: A Multicenter Prospective Study

Abstract:Background: Bloodstream infection (BSI) is a lethal complication in hematologic patients with febrile neutropenia (FN). The delayed diagnosis and intervention will lead to significant mortality. Unfortunately, the traditional golden standard diagnostic method using blood culture (BC) can only identify pathogens in 20-30% patients with BSI, and usually needs more than 48 hours. Therefore, novel methods with fast turnover, improved sensitivity and specificity are needed. Metagenomics next-generation sequencing (mNGS) shows promise as diagnostic testing for difficult and critical infectious diseases in immunodeficient hosts, but its significance in BSI of hematological patients remains unclear, and has not been evaluated in prospective study. Aims: To evaluate the ability of mNGS to identify infectious etiologies in FN patients with BSI. Method: This study was prospective performed in four Chinese hematologic centers since October 2021. For patients≥15 years with hematologic diseases, patients were screened at the onset of neutropenia. At the onset of FN, Peripheral blood specimens were collected per subject for simultaneous BC and mNGS (separate sequencing for plasma and blood cells). All patients will be followed for 14 days, or until death before 14 days. The intervention was per the attending physician. The primary endpoints of this study is the sensitivity and specificity of mNGS. Thepositive agreement was defined as mNGS identified at least one same pathogen as initial BC. The negative agreement was defined as both mNGS and BC were negative. And the adjudication of the result of BC, mNGS, and clinical diagnosis was evaluated by 4 independent specialists in hematology. Clinical adjudication categorized mNGS results as: Definite, mNGS identified at least one pathogen also found by BC or SMT with in 7 days; Probable, mNGS result was concordant with a clinical diagnosis of BSI in FN; Possible, mNGS result was consistent with BSI but not considered a common cause of FN; Unlikely, mNGS result was not a plausible cause of BSI in FN. This study was registered on ClinicalTrials.gov ID: NCT05149547. This is an interim analysis of 134 patients enrolled in this multicenter, prospective observational study. Results: Te basic information of the patients was summarized in table 1. Clinical adjudication categorized mNGS results as: Definite (N=24); Probable (N=21); Possible (N=7); Unlikely (N=49). Definite, probable and possible cases were regarded as true positives. The positive percent agreement (PPA) and negative percent agreement (NPA) with BC were 80.0% (20/25) and 25.7% (28/109) respectively; 44% of mNGS results were polymicrobial (except viruses). According to the clinical adjudication, mNGS sensitivity and specificity were 94.6% (52/55) and 38.0% (30/79) respectively (table 2 and table 3). Compared with BC, the reporting time of mNGS was significant shorter (24.8±4.0 vs. 72.3±21.2 h, p < 0.05). Conclusions: Compared with conventional BC, mNGS might provide an early and rapid microbiological diagnosis of BSI in hematologic patients with FN. Procalcitonin for safe reduction of unnecessary blood cultures in the emergency department: Development and validation of a prediction model

Abstract:Objectives: Blood cultures (BCs) are commonly ordered in emergency departments (EDs), while a minority yields a relevant pathogen. Diagnostic stewardship is needed to safely reduce unnecessary BCs. We aimed to develop and validate a bacteremia prediction model for ED patients, with specific focus on the benefit of incorporating procalcitonin. Methods: We included adult patients with suspected bacteremia from a Dutch ED for a one-year period. We defined 23 candidate predictors for a "full model", of which nine were used for an automatable "basic model". Variations of both models with C-reactive protein and procalcitonin were constructed using LASSO regression, with bootstrapping for internal validation. External validation was done in an independent cohort of patients with confirmed infection from 71 Spanish EDs. We assessed discriminative performance using the C-statistic and calibration with calibration curves. Clinical usefulness was evaluated by sensitivity, specificity, saved BCs, and Net Benefit. Results: Among 2111 patients in the derivation cohort (mean age 63 years, 46% male), 273 (13%) had bacteremia, versus 896 (20%) in the external cohort (n = 4436). Adding procalcitonin substantially improved performance for all models. The basic model with procalcitonin showed most promise, with a C-statistic of 0.87 (0.86-0.88) upon external validation. At a 5% risk threshold, it showed a sensitivity of 99% and could have saved 29% of BCs while only missing 10 out of 896 (1.1%) bacteremia patients. Conclusions: Procalcitonin-based bacteremia prediction models can safely reduce unnecessary BCs at the ED. Further validation is needed across a broader range of healthcare settings.
Outpatient Management of Fever and Neutropenia in Low-risk Children with Solid Tumors: A Quality Improvement Initiative

Abstract:Background: Management of febrile neutropenia in pediatric oncology usually requires inpatient parenteral antibiotics after initial evaluation, but some patients at lower risk of sepsis could be safely managed outpatient. We describe a quality improvement project to increase outpatient management of fever and neutropenia. Methods: We designed a standardized algorithm for children with a solid tumor diagnosis and low risk for bacteremia. The aim was to achieve outpatient management for at least 80% of eligible patients within 20 months of project initiation. We used plan-do-study-act cycles to improve algorithm compliance, including optimizing medical record decision support, developing targeted educational materials and outreach, and restructuring outpatient processes to allow for close follow-up. We surveyed patients (age ≥12 y) and parents/caregivers to assess the impact of outpatient management. Results: The initiative led to 71% (n = 34) of eligible patients being managed as outpatients. Six percent (n = 2) of patients developed bacteremia, resulting in hospital admission. Fifteen of 26 parents/caregivers and five of 11 patients approached completed the survey. For the preferred setting of febrile neutropenia management, 83% of patients preferred to be home versus 40% of parents/caregivers. No patient expressed any of the three highest ratings in the question exploring fear regarding outpatient febrile neutropenia management versus 67% of parents/caregivers. Conclusions: Some children with a solid tumor diagnosis at low risk for bacteremia are safely managed for febrile neutropenia as outpatients. Targeted efforts to engage parents/caregivers early in this practice change are necessary for success.
Procalcitonin and Interleukin-10 May Assist in Early Prediction of Bacteraemia in Children With Cancer and Febrile Neutropenia

Abstract:Objectives: Febrile neutropenia (FN) causes treatment disruption and unplanned hospitalization in children with cancer. Serum biomarkers are infrequently used to stratify these patients into high or low risk for serious infection. This study investigated plasma abundance of cytokines in children with FN and their ability to predict bacteraemia. Methods: Thirty-three plasma cytokines, C-reactive protein (CRP) and procalcitonin (PCT) were measured using ELISA assays in samples taken at FN presentation (n = 79) and within 8-24 h (Day 2; n = 31). Optimal thresholds for prediction of bacteraemia were identified and the predictive ability of biomarkers in addition to routinely available clinical variables was assessed. Results: The median age of included FN episodes was 6.0 years and eight (10%) had a bacteraemia. On presentation, elevated PCT, IL-10 and Mip1-beta were significantly associated with bacteraemia, while CRP, IL-6 and IL-8 were not. The combination of PCT (≥0.425 ng/ml) and IL-10 (≥4.37 pg/ml) had a sensitivity of 100% (95% CI 68.8-100%) and specificity of 89% (95% CI 80.0-95.0%) for prediction of bacteraemia, correctly identifying all eight bacteraemia episodes and classifying 16 FN episodes as high-risk. There was limited additive benefit of incorporating clinical variables to this model. On Day 2, there was an 11-fold increase in PCT in episodes with a bacteraemia which was significantly higher than that observed in the non-bacteraemia episodes. Conclusion: Elevated PCT and IL-10 accurately identified all bacteraemia episodes in our FN cohort and may enhance the early risk stratification process in this population. Prospective validation and implementation is required to determine the impact on health service utilisation.

Abstract:Background: Empirical antibacterial therapy for febrile neutropenia reduces mortality due to Gram-negative blood stream infections (BSIs). Pediatric guidelines recommend monotherapy with an antipseudomonal beta-lactam or a carbapenem and to add a second anti-Gram-negative agent in selected situations. We evaluated the changes in the proportions of resistance of beta-lactam monotherapies vs. their combination with amikacin, and the possible impact on ICU admission or death. Results: 797 BSIs due to Gram-negative bacteria in 685 patients were included. Combination therapies with amikacin had a lower percentage of isolates resistant to one or to both drugs compared with the respective monotherapy. The highest OR for ICU admission was observed when both drugs of the combination of meropenem–amikacin were resistant. Mortality was significantly associated with relapse or the progression of the underlying malignancy, and resistance to both drugs of the combinations of cefepime–amikacin or meropenem–amikacin. Methods: This study was based on data collected for a large multinational study, in which the susceptibility of Gram-negative bloodstream isolates was categorized following either EUCAST or CLSI according to local laboratory standards. An escalation antibiogram was generated for each selected drug. For resistant bacteria, the conditional susceptibility probability on resistance was calculated. Conclusions: In pediatric cancer patients with Gram-negative BSIs, the proportion of the resistant organism correlates with ICU admission or death, which may be reduced by combination therapy. In patients with suspected or confirmed Gram-negative BSIs that are not-improving or deteriorating under monotherapy, escalation to meropenem may represent the best option. Amikacin should be preferred when combination therapy is considered with ciprofloxacin as an alternative in the case of impaired renal function.

Abstract:BACKGROUND: Control of bacterial and fungal infections is critical to improving outcomes in hematological neoplastic diseases of children and adolescents. In this study, a retrospective analysis of our previous studies on febrile neutropenia was performed to investigate bacteremia.PROCEDURE: From August 2008 to December 2023, five antibiotic studies were performed for febrile and neutropenic pediatric patients who had been treated with chemotherapy, immunosuppressive therapy, or had received stem cell transplantation in the pediatric unit at Sapporo Hokuyu Hospital. The rate of positive blood culture, detected bacteria, and susceptibility of several types of antibiotics in febrile episodes were investigated.RESULTS: Blood culture was positive in 133 of 1604 febrile episodes of 329 patients. Detected bacteria were Gram-positive cocci (61.2 %), Gram-negative bacilli (27.6 %), Gram-negative cocci (0.7 %), and Gram-positive bacilli (10.4 %). The incidence of bacteremia over time showed a decreasing trend with each passing year. In particular, the incidence of bacteremia was around 10 % in 2008-2013, whereas it was often below 5 % after 2020; this decrease was statistically significant. Although almost all detected bacteria and their susceptibilities to antibiotics (piperacillin/tazobactam, meropenem, ceftazidime, and cefozopran) did not change over time, all Escherichia coli detected after 2014 were extended-spectrum β-lactamase-producing bacteria.

Abstract:Background: Bloodstream infection (BSI) is a lethal complication in hematologic patients with febrile neutropenia (FN). The delayed diagnosis and intervention will lead to significant mortality. Unfortunately, the traditional golden standard diagnostic method using blood culture (BC) can only identified pathogens in 20-30% patients with BSI, and usually needs more than 48 hours. Therefore, novel methods with fast turnover, improved sensitivity and specificity is needed. Metagenomics next-generation sequencing (mNGS) shows promise as diagnostic testing for difficult and critical infectious diseases in immunodeficient hosts. We recently established a novel dual mNGS approach detecting using plasma cfDNA and blood corpuscle. Still, its significance in BSI of hematological patients remains unclear and has not been evaluated in a prospective study. Aims: To evaluate the ability of dual mNGS to early identify infectious etiologies in FN patients with BSI. Methods: This study was prospective performed in four Chinese hematologic centers since October 2021. For consecutive patients aged ≥15 years with hematologic diseases, patients were screened at the onset of neutropenia. At the onset of FN, peripheral blood specimens were collected per subject for simultaneous BC and dual mNGS (separate sequencing for plasma and blood corpuscle). All patients will be followed for 14 days, or until death before 14 days. The intervention was per the attending physician. The results of dual mNGS bio-informatic analysis were determined according to the reference literature, the abundance in each sample and relative to other samples in the cohort. Clinical physician and mNGS analysis team were double-blind. The primary endpoint of this study was the diagnostic performance of dual mNGS. The positive agreement was defined as mNGS identified at least one same pathogen as initial BC. The negative agreement was defined as both mNGS and BC were negative. And the adjudication of the result of BC, mNGS, and clinical diagnosis was evaluated by 4 independent specialists in hematology. This study was registerd on ClinicalTrials.gov ID: NCT05149547. Results: During the study period, 300 FN events were enrolled in the final analysis. According to the clinical adjudication, the 300 FN events were adjudged as 62 Definite BSI, 61 Probable BSI, 116 infectious FN other than BSI, 55 Non-infectious FN events and 6 FN of indeterminate cause. Among the 62 definite BSI, clinical adjudication categorized dual mNGS results as: Definite (N=49); Probable (N=7); Possible (N=3); Unlikely (N=1); Negative (N=2). Definite, probable and possible cases were regarded as true positives. The sensitivity, specificity, positive predictive value (PPV) and negative predictive value (NPV) of dual mNGS were 95.2%, 94.6%,95.2%, 94.6%, respectively. The positive percent agreement (PPA) and negative percent agreement (NPA) of dual mNGS with BC were 79.0%(49/62) and 34.9%(83/238) respectively. Compared with BC, the diagnostic time of dual mNGS was significant shorter (39.7±15.0 vs. 119.8±31.9 h, p < 0.0001). Real-time availability of dual mNGS results could have allowed early optimization of agents in 52.3% events, by addition or escalation of antibacterial in 31.7%, and antimicrobial narrowing in 20.6%. Summary/Conclusion: Our findings suggest that a novel dual mNGS approach has excellent diagnostic performance in early diagnosis of BSI in FN patients with hematological disorders. Abstract:Background The aim of this study was to investigate the applicability of the central line‐associated bloodstream infection (CLABSI) criteria of the Centers for Disease Control and Prevention in pediatric oncology patients. Methods Bacteremia episodes from 2020 to 2022 from a prospective cohort of pediatric oncology patients with a central venous catheter were included. Episodes were classified by three medical experts following the CLABSI criteria as either a CLABSI or non‐CLABSI (i.e., contamination, other infection source, or mucosal barrier injury‐laboratory confirmed bloodstream infection (MBI‐LCBI)). Subsequently, they were asked if and why they (dis)agreed with this diagnosis following the criteria. The primary outcome was the percentage of episodes where the experts clinically disagreed with the diagnosis given following the CLABSI criteria. Results Overall, 84 bacteremia episodes in 71 patients were evaluated. Following the CLABSI criteria, 34 (40%) episodes were classified as CLABSIs and 50 (60%) as non‐CLABSIs. In 11 (13%) cases the experts clinically disagreed with the diagnosis following the CLABSI criteria. A comprehensive assessment of the prolonged febrile neutropenia evaluation in pediatric oncology patients

Abstract:Background: Pediatric oncology patients with prolonged (≥96 hours) febrile neutropenia (absolute neutrophil count < 500/μL) often undergo an evaluation for invasive fungal disease (IFD) and other infections. Current literature suggests that beta-D-glucan (BDG), galactomannan, bronchoalveolar lavage (BAL), and computed tomography (CT) scans (sinus, chest, and abdomen/pelvis) may help determine a diagnosis in this population. Methods: In a retrospective cohort study of all cancer/stem cell transplant patients (diagnosed 2005-2019) from one pediatric hospital, all episodes with prolonged febrile neutropenia or IFD evaluations (defined as sending a fungal biomarker or performing a CT scan to assess for infection) were identified. Results: In total, 503 episodes met inclusion criteria and 64% underwent IFD evaluations. In total, 36.4% of episodes documented an infection after initiation of prolonged febrile evaluation, most commonly Clostridioides difficile colitis (6.4%) followed by a true bacterial bloodstream infection (BSI) (5.2%), proven/probable IFD (4.8%), and positive respiratory pathogen panel (3.6%). There was no difference in sinus CTs showing sinusitis (74% vs 63%, p = 0.46), whereas 32% of abdomen/pelvis CTs led to a non-IFD diagnosis, and 25% of chest CTs showed possible pneumonia. On chest CT, the positive predictive value (PPV) for IFD was 19% for nodules and 14% for tree and bud lesions. BDG had a PPV of 25% for IFD and GM 50%. BAL diagnosed IFD once and pneumocystis jirovecii pneumonia twice. Conclusions: Chest CTs and abdomen/pelvis CTs provide clinically relevant information during the prolonged febrile neutropenia evaluation, whereas BDG, galactomannan, BAL, and sinus CTs have less certain utility.
