A novel approach to the immediate effects of electroacupuncture on dry eye: A case series

Abstract:Introduction: Dry eye disease (DED) is an inflammatory disorder that shares several features with autoimmune diseases. Research suggests that electroacupuncture (EA) is a promising alternative treatment option for this disorder; however, evidence of its immediate efficacy is limited. Case presentation: Three patients were diagnosed with DED, and used artificial tears or anti-inflammatory drugs for long-term relief of eye symptoms. However, the cure rates were low, and the side effects were high. To improve ocular symptoms, quality of life, and physical and mental health, the patients sought alternative complementary therapies and received electroacupuncture therapy. All patients showed significant improvements in fatigue and dryness of the ocular surface after 3-4 days of treatment, and follow-up after 4 weeks showed no tendency for recurrence. No adverse reactions or unexpected events were observed during treatment. Conclusion: We propose an innovative electroacupuncture treatment aimed at fewer acupuncture points, shorter periods, and faster healing that improves the symptoms of patients with DED in the short term. Continuous current stimulation by anatomically positioned needles on both sides of the lacrimal gland and the creation of an electric field may improve the endogenous mechanisms of DED.

Abstract:Dry eye is an ocular surface disease caused by multiple factors,which is characterized by imbalance of tear film homeostasis,changes of ocular surface micro-environments and ocular uncomfortable symptoms.Tear film instability,tear hyperosmolarity,ocular surface inflammation and damage and corneal neurosensory abnormalities play etiological roles in the process of dry eye.Dry eye seriously affects work and life quality of the patients.With the progress of science and technology,the treatment of dry eye is becoming more diversified.The therapeutic methods are different for mild,moderate and serous dry eye.The choice of therapies actually depends on the individual state and condition of therapeutic institutions.A systemic,standardized and comprehensive therapy is associated with the improvement of treatment outcome.This article systematically summarizes the traditional and emerging treatments of dry eye,which are clinically feasible and can provide reference for choosing treatment methods.

Abstract:Professor LIN Guo-hua is the representative inheritor of Lingnan fire needling therapy. He proposed that the clinical application of Lingnan fire needling is mainly based on pattern differentiation according to eight priciples, as well as in coordination with the theories of meridians and zang-fu organs. He believes that dry eye syndrome can be treated from the aspects of deficiency of liver and kidney, heart yin deficiency, and qi and yin deficiency. Through deeply digging the connotation of ancient classics, and combined his clinical practice using Lingnan fire needling, professor LIN created a unique therapy that select "Neichengqi"as the main acupoint combined with Xialeidian, Dagukong (EX-UE 5), Xiaogukong (EX-UE 6) and Shaoze (SI 1) in the treatment of dry eye syndrome. In this paper, from the etiology and pathogenesis of dry eye syndrome and therapeutic characteristics of Lingnan fire needling, the thoughts of syndrome differentiation and characteristics of acupoints selection of professor LIN were analyzed by a typical case, and it is expect to enrich the clinical treatment plan for dry eye syndrome.

Abstract:Aim: To discuss the immunological mechanism in electroacupuncture (EA) treatment of dry eye syndrome (DES) by targeting the changes in conjunctival cytokine expression profile.Method: Eligible DES patients were randomized into an EA group (EAG) or an acupuncture group (AG). The ocular surface disease index (OSDI), amount of tear production, and tear film break-up time (BUT) were observed to evaluate the efficacy. Conjunctival cells were collected from both effective and invalid cases to observe the expressions of cytokines by protein microarray. Gene Ontology (GO) and Kyoto Encyclopedia of Genes and Genomes (KEGG) were used for functional cluster and signaling pathway analysis of the differentially expressed proteins. Enzyme-linked immunosorbent assay (ELISA) was used to verify the specific differential proteins.Result: After treatment, OSDI dropped and BUT extended in both groups, and the tear production increased only in the EAG (all P < .01). Compared with the AG, the improvement in tear production was more significant in the EAG (P < .01). There were 17 differentially expressed conjunctival cytokines between the effective and invalid cases in the EAG, and those expressed higher than the limit of detection (LOD) included monocyte chemoattractant protein 1 (MCP-1), macrophage colony-stimulating factor (M-CSF), regulated on activation in normal T-cell expressed and secreted (RANTES) and tissue inhibitor of metalloproteinases 1 (TIMP-1). GO analysis showed that the differential cytokines were mainly involved in cellular interaction, signaling pathways and reactions to stimuli. KEGG analysis revealed that the signaling pathways of these cytokines were mainly responsible for interactions between cytokines or between cytokines and their receptors, such as Jak-STAT signaling pathway, chemokine signaling pathway, and tumor necrosis factor signaling pathway.Conclusion: EA can effectively treat DES by improving the symptoms, increasing tear secretion and extending BUT, which is possibly related to its regulation on the conjunctival cytokine expressions.

Abstract:Introduction Dry eye (DE) is a multifactorial ocular surface disease causing considerable medical, social and financial implications. Currently, there is no recognised long-term, effective treatment to alleviate DE. Clinical evidence shows that electroacupuncture (EA) can improve DE symptoms, tear secretion and tear film stability, but it remains controversial whether it is just a placebo effect. We aim to provide solid clinical evidence for the EA treatment of DE. Methods and analysis This is a multicentre, randomised, sham-controlled trial. A total of 168 patients with DE will be enrolled and randomly assigned to EA or sham EA groups to receive 4-week consecutive treatments and follow-up for 24 weeks. The primary outcome is the change in the non-invasive tear break-up time (NIBUT) from baseline to week 4. The secondary outcomes include tear meniscus height, the Schirmer I test, corneal and conjunctival sensation, the ocular surface disease index, corneal fluorescein staining, the numerical rating scale and the Chinese DE-related quality of life scale. Ethics and dissemination The trial protocol and informed consent were approved by the Ethics Committee of Yueyang Hospital of Integrated Traditional Chinese and Western Medicine Affiliated to Shanghai University of Traditional Chinese Medicine (identifier: 2021–119), Shanghai Eye Disease Prevention and Treatment Center (identifier: 2022SQ003) and Eye and ENT Hospital of Fudan University (identifier: 2022014). Electroacupuncture modulates the TLR4-NF-κB inflammatory signaling pathway to attenuate ocular surface inflammation in dry eyes of type 2 diabetic rats

Abstract:Bioinformatics analysis was performed to reveal the underlying pathogenesis of type 2 diabetes (T2DM) dry eye(DE) and to predict the core targets and potential pathways for electroacupuncture (EA) treatment of T2DM DE, in which key targets such as Toll-likereceptor4 (TLR4), NF-κB and Tumor necrosis factor-α (TNF-α) may be involved. Next, streptozotocin and a high-fat diet were used to generate T2DM-DE rats. Randomly picked EA, fluorometholone, model, and sham EA groups were created from successfully modelled T2DM DE rats. Six more rats were chosen as the blank group from among the normal rats. The results of DE index showed that EA improved the ocular surface symptoms.HE staining showed that EA attenuated the pathological changes in the cornea, conjunctiva and lacrimal gland of T2DM DE rats. EA decreased the expression of TLR4, MyD88, P-NF-κB P65, and TNF-α in the cornea, conjunctiva, and lacrimal gland, in accordance with immunofluorescence and Western blot data. Thus, EA reduced ocular surface symptoms and improved pathological changes of cornea, conjunctiva, and lacrimal gland induced by T2DM DE inT2DM DE rats, and the mechanism may be related to the inhibition of overactivation of the TLR4/NF-κB signaling pathway by EA and thus attenuating ocular surface inflammation.
Effect of Acupuncture Treatment on the Ocular Pain, Mental State and Ocular Surface Characteristics of Patients with Dry Eye Disease: A Non-Randomized Pilot Study

Abstract:Background: Ocular pain is a prevalent symptom of dry eye disease (DED), which often accompanies potential psychological issues. The study aimed to explore whether acupuncture could improve ocular pain, mental state, and dry eye parameters in patients with DED. Methods: The non-randomized pilot study included 48 patients divided into two groups: the acupuncture group (n=27) and the 0.3% sodium hyaluronate (SH) group (n=21). Participants in the acupuncture group underwent treatments on six bilateral acupuncture points (BL1, BL2, ST1, LI 20, SI1 and SI3) 3 times per week for 4 weeks. Patients in the SH group received 0.3% SH 4 times per day for 4 weeks. Ocular pain was assessed using the numerical rating scale (NRS), and mental state was evaluated through the self-rating anxiety scale (SAS) and self-rating depression scale (SDS). Ocular surface parameters, concentrations of inflammatory cytokines, and corneal nerve morphological indicators were measured at baseline, the first week, and the fourth week. Randomization procedures were not used in this study, and outcome assessors and statistical analysts were blinded. Results: Compared with baseline, both NRS scores (from 5.91 ± 1.52 to 1.94 ± 1.57) and ocular surface discomfort index (OSDI) scores (from 49.75 ± 14.92 to 29.64 ± 18.79) were decreased after 1 and 4 weeks of treatment in both groups (all p < 0.05). At 4 weeks, the acupuncture group showed significant improvements, including increased tear break-up time (TBUT) and corneal perception, decreased SAS and SDS scores, and reduced concentrations of interleukin (IL)-1β, IL-6, and tumor necrosis factor (TNF)-α concentration in tears (all p < 0.05). These changes were not observed in the SH group (all p > 0.05). Conclusion: Acupuncture treatment could improve ocular surface characteristics in patients with DED, and more importantly, it alleviates their ocular pain and depressive state. The anti-inflammatory effect of acupuncture may be involved in this process. Future research with larger, randomized controlled trials (RCTs) is necessary to confirm these findings and clarify the mechanisms involved.
[Progress of treatment on oculomotor paralysis with electroacupuncture]

Abstract:Collecting information from the database of China National Knowledge Information (CNKI) and VIP database of Chinese Journals, an overall analysis and review on treatment of oculomotor paralysis with electroacupuncture in recently years are made in the paper. The electroacupuncture, which can provide constant stimulation and reinforce the effect of acupuncture, is the major therapy to treat oculomotor paralysis. However, the best stimulate threshold of electroacupuncture on oculorotary muscles has not been reported yet. The parameters of electroacupuncture adopted in clinic are not standardized, which lead to unfavorable therapeutic effect as well as unclear mechanism of treatment. Therefore, it is suggested that the study should focus on enhancing the research level and broadening researcher's mind to explore the best parameter for the electroacupuncture stimulation on the oculorotary muscles, to make clear the mechanism of treatment and to search for valuable observation indicators in the future.
Acupuncture for Dry Eye Disease after Recovery from COVID-19: A Protocol for Systematic Review and Meta-Analysis

Abstract:Dry eye disease (DED) is a condition occurring worldwide. Studies have found that coronavirus disease 2019 (COVID-19) patients may have persistent dry eye symptoms for weeks and months after recovery, and the prevalence of dry eye is higher in COVID-19 patients than in people without COVID-19 infection. As one of the common ophthalmic diseases, the clinical application of acupuncture in the treatment of DED is not widely used nowadays.According to the retrieval strategies, randomized controlled trials (RCT) on the acupuncture for DED after recovery from COVID-19 were obtained from Embase, The Cochrane Library, Web of Science, Chinese National Knowledge Infrastructure database, Chinese Biomedical Database, Chinese Science and Technology Periodical database, The WanFang database. Studies were screened based on inclusion and exclusion criteria, and the Cochrane risk bias assessment tool was used to evaluate the quality of the studies. The meta-analysis was performed using Review Manager (RevMan 5.3) and STATA 14.2 software. Ultimately, the evidentiary grade for the results will be evaluated.The study will provide a high-quality and convincing assessment of the efficacy and safety of acupuncture for DED after recovery from COVID-19 and will be published in peer-reviewed journals.Our findings will provide references for future clinical decision and guidance development.
Curative effect of needling in 3 acupoints around eye and Fengchi (GB20) on optic atrophy

Abstract:Objective To study the curative effect of needling in 3 acupoints around eye[Jingming(BL1),Chengqi(ST1) and Shangming]and Fengchi(GB20) on optic atrophy.Methods The patients(n = 40) were randomly divided into treatment group and control group(each n = 20) by applying prospective randomized controlled method.Control group was given Chinese medicinal decoction and support therapy of nerve nutrition,and treatment group was given acupuncture in Jingming(BL1),Chengqi(ST1),Shangming and Fengchi(GB20) once a day besides of the same treatment as control group.After 4 w,the changes of vision,visual field and visual evoked potential(VEP) were observed in two groups.Results The total effective rate was 81%in treatment group and 52%in control group.After treatment and in treatment group,vision was significantly improved,mean light sensitivity(MS)increased significantly,mean defect(MD) decreased significantly and evoked potential amplitude increased significantly(P 0.05).Conclusion Needling in 3 acupoints around eye and Fengchi(GB20) has good curative effect on optic atrophy.
Transcutaneous Electrical Acupoint Stimulation (Teas) In The Treatment of Dry Eye Disease

Abstract:Purpose: To assess effectiveness and safety of the Transcutaneous Electrical Acupoint Stimulation (TEAS) treatment in alleviating symptoms and signs of dry eye disease (DED).Methods: Patients diagnosed with DED at the Peking University Third Hospital Eye Center from December 2020 to February 2021 were randomly assigned in a 1:1:1 ratio to 5mA, 3mA, or 0mA Group respectively. DED signs and symptoms were evaluated before and 30min after treatment. We compared the clinical improvement among the three groups and between each two groups. Results: A total of 63 patients were included. 5mA Group had best efficacy in all the signs and most of symptoms (P<0.05), and the symptoms scores in 3mA Group were also significantly improved after treatment except pain, watering and increased secretions (P<0.05). In the comparisons between before and after treatment between each two groups, 5mA Group showed greater improvement in the signs and most of symptoms than other two groups(P<0.05). In addition, 3mA Group had greater improvement than 0mA Group in the symptoms of asthenopia (P=0.018) and blurred vision (P=0.001). During the treatment, we hadn’t gotten any adverse events from patients.Conclusions: TEAS treatment was effective in the treatment of DED, and 5mA provided greater signs and symptoms relief. TEAS might be a new therapeutic option for the treatment of DED.

Abstract:Objective To summarize and critically assess the evidence from randomized controlled trials (RCTs) of acupuncture in treating dry eye syndrome (DES) according to the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) statement and the Cochrane Collaboration recommendations. Methods A search of PubMed, the Cochrane Central Register of Controlled Trials, ClinicalTrials.gov and Embase was made from their inception to August 2016, as well as Chinese, Japanese, and Korean databases. Two reviewers independently selected RCTs and assessed the methodological quality. Meta-analysis and the level of evidence were processed by RevMan 5.3 and the Grading of Recommendations Assessment, Development and Evaluation (GRADE) approach. Results After selection, 8 trials were subjected to our systematic review. The methodological quality was low generally. The 3-10 weeks follow-up showed that acupuncture improved the tear film break-up time (BUT) (MD=1.33, 95% CI=1.01-1.66, 619 participants). The mean difference of Schirmer’s test was 1.73 mm (95%CI=1.28-2.18, 618 participants) between the acupuncture group and the control group. The subjective variables exhibited no significant differences. Conclusion The low methodological quality of the trials does not suggest drawing firm conclusions on the value of acupuncture therapy for DES. Acupuncture treatment may have some effects on the tear film BUT and Schirmer’s test, but not on the subjective symptoms. Well-planned large-scale high-quality RCTs are needed to make it clear whether acupuncture is effective in treating DES.
Effect of Acupuncture on Tear Proteomics in Patients with Video Display Terminal-Related Dry Eye

Abstract:Acupuncture is widely used to treat dry eye disease (DED), but its effect has not been reported in treating video display terminal (VDT)-related dry eye, and the mechanism of acupuncture on VDT-related dry eye is also unknown. In our study, the tear proteome was compared with identifying possible mechanisms and biomarkers for predicting acupuncture effectiveness in VDT-related dry eye. The results showed that the ocular surface disease index scores were significantly different between the acupuncture group (AC group) and artificial tears group (AT group) at the end of the study, whereas tear film breakup time (TFBUT) and Schirmer I test (SIT) were not significantly different between the groups. Proteome changes pre- and post-treatment in the AC group were associated with B cell-related immune processes, inflammation, glycolysis, and actin cytoskeleton. Furthermore, the proteins hexosaminidase A and mannose-binding lectin 1 could prospectively predict whether acupuncture treatment was effective. Therefore, we believe that acupuncture can provide greater improvement in the clinical symptoms of VDT-related dry eye than artificial tears. The mechanism of acupuncture in VDT-related dry eye treatment may be associated with glycolysis- and actin cytoskeleton remodeling-mediated inflammatory and immune processes. Additionally, hexosaminidase A and mannose-binding lectin 1 are biomarkers for predicting the efficacy of acupuncture for VDT-related dry eye.

Abstract:Diabetic retinopathy(DR) is one of the most common and serious complications of diabetes.Through the study of ancient medicals and based on many years of clinical experiences,Professor GAO Yan-bin infers that DR is particularly closely related to the liver and kidney.Using these two organs as the theoretical foundation,the root of eye diseases can be grasped.The theory of collateral disease coincides and is responsible throughout the whole process of DR.Using syndrome differentiation,he holds the idea that with deficiency as the basic pattern and excess as the syndrome,the main reason for DR is qi,blood,yin and yang deficiency,while blood stagnation in the eye meridian is pathogen,The rules of treatment are tonifying qi and enriching yin,supplementing and nourishing liver and kidney,removing blood stasis and promoting circulation.By combining overall symptoms with fundus changes,together with dynamic differentiation and flexible treatment,satisfactory results have been obtained in clinical practice.

Abstract:'s experience is introduced in the treatment of cortical blindness with the comprehensive acupuncture therapy for benefiting and resolving stasis. The comprehensive therapy includes acupuncture at Sishencong (EX-HN 1), Xinming No.1 (Extra), Shang Jianming, Chengqi (ST 1), Xinming No.2 (Extra), optic area and visual area (scalp acupuncture); acupoint injection applied alternatively at Qiuhou (EX-HN 7) and Taiyang (EX-HN 5); the auricular point paste stimulation at Brain Point, Shenmen (TF), Gan (CO), Shen (CO), Yan (LO), Eye No.1 and Eye No.2; as well as the tapping therapy with skin needle at Zhengguang No.1 and Zhengguang No.2. Additionally, 3 typical cases of cortical blindness are introduced. The coordination of the different acupuncture techniques is analyzed so as to explain its actions as benefiting , resolving stasis, promoting blood circulation and nourishment of the eyes.

Abstract:Objective: To investigate the application characteristics of electroacupuncture (EA) in the treatment of peripheral neuropathy, so as to provide a basis for clinical use of EA therapy. Methods: Keywords of "electroacupuncture""peripheral neuropathy" "facial paralysis" "trigeminal neuralgia" "sciatica" "common peroneal nerve injury" "diabetic peripheral neuropathy" "intercostal neuralgia" "gluteal epithelial neuritis" "ulnar nerve injury" "median nerve paralysis" "postherpetic neuralgia", and "great occipital neuralgia" were used to search articles in both English and Chinese published from 1999 to 2019 in databases of CNKI, Wanfang, VIP, CBM, Ovid, PubMed and Embase and related books such as electroacupuncture, and neurology, followed by establishing a database of "Electroacupuncture Treatment of Peripheral Neuropathy". Then, the collected articles were put into statistical analysis after sorting, screening, input, checking, and data extracting by using data mining technology and statistical software EpiData. Results: Of the searched 1 528 papers, 778 were eligible, involving 13 types of peripheral neuropathy which the facial paralysis and facial spasm were most frequently seen, followed by trigeminal neuralgia and sciatica, with an effective rate being above 90% for nearly all the 13 diseases. The acupoints employed were chiefly those close to the affected area and distribute along the nerve trunk.In addition, about the needling techniques, the penetration needling was frequently used, and the triple needling, quintuple needling and accompanied needling were also used. Regarding the related needle manipulations, the uniform reinforcing-reduction technique was most frequently used. The duration of EA was 30 min, with a highest stimulating frequency of 50 Hz. The acupoint injection was frequently supplemented, followed by moxibustion, and the treatment sessions were usually about 30 times. Conclusion: EA therapy is frequently used in the treatment of peripheral neuropathy, and has some characteristics in acupoint selection, stimulating parameters and some additional needling techniques.
[Efficacy observation on electroacupuncture in the treatment of oculomotor impairment caused by ophthalmic nerve injury]

Abstract:Objective: To observe the difference in the clinical efficacy on oculomotor impairment between electroacupuncture and acupuncture and explore the best therapeutic method in the treatment of this disease. Methods: Sixty cases of oculomotor impairment were randomized into an electroacupuncture group and an acupuncture group, 30 cases in each one. In the electroacupuncture group, the points were selected on extraocular muscles, the internal needling technique in the eye was used in combination of electroacupuncture therapy. In the acupuncture group, the points and needling technique were same as the electroacupuncture group, but without electric stimulation applied. The treatment was given 5 times a week, 15 treatments made one session. After 3 sessions of treatment, the clinical efficacy, palpebral fissure size, pupil size, oculomotor range and the recovery in diplopia were compared before and after treatment in the two groups. Results: In the electroacupuncture group, the palpebral fissure size was (9.79+/-2.65)mm and the eyeball shifting distance was (18.12+/-1. 30)mm, which were hig-her than (8.23+/-2.74)mm and (16.71+/-1. 44)mm respectively in the acupuncture group. In the electroacupuncture group, the pupil diameter was (0. 44 +/-0. 42)mm, which was less than (0. 72 +/- 0. 53)mm in the acupuncture group, indicating the significant difference (all P<0. 05). The cured rate was 63. 33% (19/30) and the total effective rate was 93.33% (28/30) in the electroacupuncture group, which was better than 36.67% (11/30) and 83. 333 (25/30) in the acupuncture group separately, indicating the significant difference (all P<0. 05). Conclusion: Electroacupuncture presents the obvious advantages in the treatment of oculomotor impairment, characterized as quick and high effect, short duration of treatment and remarkable improvements in clinical symptoms, there are important significance for the improvement of survival quality of patients.
