Abstract:Context:The treatment of diabetic nephropathy (DN) is still quite limited. DN remains poorly understood due to the complexity of and differences in its etiology. Therefore, potential biomarkers for diagnosis and targeted treatments are urgently needed.Objective:The study aimed to analyze the associations between circulating total bile acid (TBA) levels and the risk of DN in Chinese patients with type 2 diabetes mellitus (T2DM) and to determine the differences in the TBA levels of males and females, including pre- and postmenopausal women, to find clues for the screening of DN.Design:The research team performed a retrospective study.Setting:The study took place at the Second Affiliated Hospital at the School of Medicine of Zhejiang University in Zhejiang, China.Participants:Participants were 1785 T2DM patients admitted to the hospital between April 2008 and November 2013.Groups:The research team separated participants into three groups: (1) the normoalbuminuria or normal group, with a UACR <30 mg/g·Cr (2) the microalbuminuria (MAU) group, with a UACR of 30-299 mg/g·Cr; and (3) the macroalbuminuria (MAC) group, with a UACR of ≥300 mg/g·Cr.Outcome Measures:Between the three groups, the research team compared: (1) the demographic and clinic characteristics of the normal, MAU, and MAC groups; (2) TBA distribution by age; (3) TBA distribution by gender; and (4) TBA quartiles. The team also examined the associations between TBA and albuminuria, identifying the odds ratios (OR) and relevant 95% confidence intervals (CI) using multiple logistic regression.Results:The study found that: (1) the MAC group's TBA was significantly lower than those of the normal and MAU groups; (2) the TBA of postmenopausal women was significantly higher than that of premenopausal women; (3) the incidence of MAC was obviously increased with TBA levels; (4) the risks for MAU group didn't change significantly with increasing TBA levels; (5) the MAC group's odds ratios (ORs) were 0.61 between Q2 and Q1, 0.44 between Q3 and Q1, and 0.38 between Q4 and Q1; and (6) for men and postmenopausal women, the TBA levels of those in Q3 and Q4 might decrease the risk of MAC, whereas no such correlation existed for MAU.Conclusions:An independent negative association exists between TBA levels and MAC in T2DM. The decrease of circulating TBA might be a prospective clinical factor for determining established DN, especially for males and postmenopausal females.

Abstract:Background Type 2 diabetic kidney disease is the most common cause of chronic kidney diseases (CKD) and end-stage renal diseases (ESRD). Although kidney biopsy is considered as the ‘gold standard’ for diabetic kidney disease (DKD) diagnosis, it is an invasive procedure, and the diagnosis can be influenced by sampling bias and personal judgement. It is desirable to establish a non-invasive procedure that can complement kidney biopsy in diagnosis and tracking the DKD progress. Methods In this cross-sectional study, we collected 252 urine samples, including 134 uncomplicated diabetes, 65 DKD, 40 CKD without diabetes and 13 follow-up diabetic samples, and analyzed the urine proteomes with liquid chromatography coupled with tandem mass spectrometry (LC–MS/MS). We built logistic regression models to distinguish uncomplicated diabetes, DKD and other CKDs. Results We quantified 559 ± 202 gene products (GPs) (Mean ± SD) on a single sample and 2946 GPs in total. Based on logistic regression models, DKD patients could be differentiated from the uncomplicated diabetic patients with 2 urinary proteins (AUC = 0.928), and the stage 3 (DKD3) and stage 4 (DKD4) DKD patients with 3 urinary proteins (AUC = 0.949). These results were validated in an independent dataset. Finally, a 4-protein classifier identified putative pre-DKD3 patients, who showed DKD3 proteomic features but were not diagnosed by clinical standards. Follow-up studies on 11 patients indicated that 2 putative pre-DKD patients have progressed to DKD3. Conclusions Our study demonstrated the potential for urinary proteomics as a noninvasive method for DKD diagnosis and identifying high-risk patients for progression monitoring.
Urinary RBP As an Independent Predictor of Renal Outcome in Diabetic Nephropathy.

Abstract:Background. Renal tubular impairment is prevalent in diabetic nephropathy (DN) and the histological severity predicted renal outcome. Biomarkers of tubular injury also increased in the urine of DN patients. The retrospective study aimed to assess the prognostic value of clinically widely applied urinary tubular injury markers, retinol-binding protein (RBP), beta 2-microglobulin (beta 2-MG) and N-acetyl-beta-D-glucosaminidase (NAG) in DN. Method. A total of 305 patients with biopsy-proven DN were enrolled. The baseline urine total protein and components including albumin, IgG, RBP, beta 2-MG and NAG were retrieved from medical records. The primary outcome was end stage renal disease (ESRD). Cox proportional hazard analysis and restricted cubic splines were performed to evaluate the association of parameters with ESRD. Nomograms were constructed and concordance index (C-index) was used to measure the prediction ability. Result. The levels of urinary RBP, beta 2-MG and NAG were positively correlated with the severity of interstitial fibrosis and tubular atrophy (IFTA). Positive correlations were also observed among beta 2-MG, NAG and mesangial expansion. Urinary RBP was not correlated with any glomerular lesions. Urinary RBP, beta 2-MG and NAG were risk factors for ESRD in hazard analysis with adjustment for age, gender and body mass index (BMI). The hazard ratios increased with the increment of baseline levels. In the multivariate Cox model including serum creatinine (SCr), total urinary protein, urinary albumin, urinary IgG and the tubular injury biomarkers, urinary RBP (with every g/mol.Cr increase: HR 1.06, 95% CI 1.03-1.10, p =0.001) remained as an independent risk factor for ESRD in DN patients. Patients were divided by the medium value of urinary RBP into the low RBP and high RBP groups. Survival analysis showed that significantly more patients in the high RBP progressed to ESRD compared to those in the low RBP group (p =0.02) when urinary total protein was less than 3.5 g/g. The C-index of the nomogram incorporating age, gender, BMI, SCr and total urine protein was 0.757. The value increased to 0.777 after adding urinary RBP to the model. Conclusions. Urinary RBP excretion was only correlated with the severity of IFTA and independently predicted ESRD in DN patients.
Comparison of urinary vitamin D binding protein with albumin-creatinine ratio in type 2 Diabetes Mellitus as an early screening tool for diabetic nephropathy

Abstract:ABSTRACT Introduction:- Diabetes mellitus (DM) is a group of metabolic disorders. Diabetic nephropathy is one of the chronic complications of DM, leading to End stage renal disease (ESRD). A current diagnostic criterion for diabetic nephropathy is measurement of microalbuminuria, which is 30 to 300 mg of albumin/24 hour’s urine or ratio of albumin to creatinine (ACR) in the range of 30 to 300 mg/g in random urine sample but it shows inadequate sensitivity for the early detection of diabetic nephropathy. Methodology:- This was a comparative cross sectional study, which comprised of seventy five study subjects and were distributed into three study groups with 25 subjects in each group, having age in the range of 40-50 years.Group-1 comprising controls (without diabetes mellitus),Group-2 contained diabetes mellitus patients with normoalbuminuria. Group-3 composed of diabetes mellitus patients with microalbuminuria. Vitamin D binding protein, urine creatinine and albumin was measured from the urine sample of each study subjects. Determination of creatinine in urine was performed by jaffe method and albumin in urine was determined by immunoturbidimetric method. Vitamin D binding protein was measured by ELISA method. Levels of VDBP and albumin were normalized with urine creatinine and expressed as VDBP creatinine ratio as (ng/mg) and albumin creatinine ratio as (mg/g) in the spot urine sample. Results:-Results of this study showed that level of Vitamin D binding protein was significantly increased in diabetes mellitus in comparison to control subjects. Conclusion:- Our results proposes that urinary vitamin D binding protein levels is likely to become a useful biomarker for the early detection of diabetic nephropathy in diabetic patients which can be helpful in early treatment and will help to manage diabetic nephropathy. Levels of VDBP and albumin were normalized with urine creatinine and expressed as VDBP creatinine ratio as (ng/mg) and albumin creatinine ratio as (mg/g) in the spot urine sample. Results: Urinary VDBP levels among the three groups were as the highest Median, IQR (Q1- Q3) values were observed in group 3 as 1056 ng/mg,(905 ng/mg – 1215 ng/mg) followed by group 2 as 442 ng/mg,(381.50 ng/mg – 523 ng/mg) and group 1 as 98 ng/mg,(73.50 ng/mg – 149 ng/mg) respectively, and a statistical significant difference was observed among the three groups with a p- value of 0.000. Results of this study showed that level of Vitamin D binding protein was significantly increased in diabetes mellitus in comparison to control subjects. Conclusion: Results suggest that urinary vitamin D binding protein level is likely to become a useful biomarker for the early detection and management of diabetic nephropathy in Type 2 diabetic patients.

Abstract:Aim: This study aimed to assess the correlation between serum concentration of 25-hydroxyvitamin D and albuminuria progression of diabetic kidney disease (DKD) and to use bioinformatics methods to determine the potential mechanism in the pathological process of advanced DKD. Methods: A total of 178 type 1 diabetes mellitus (T1DM) patients with microalbuminuria complications who were hospitalized at least twice (with an interval > 24 months) in the Department of Endocrinology of The First Affiliated Hospital of USTC were included in this study. According to the urinary albumin creatinine ratio (UACR), we classified DKD stages as follows: microalbuminuria (UACR, 30-300 mg/g), and macroalbuminuria (UACR, >300 mg/g). We divided the patients into DKD progression (N=44) and stable group (N=134) on account of urinary albumin-to-creatinine ratio (UACR) by at least two randomized measurements. Stable group was defined as UACR between 30 and 300 mg/g, whereas progression group was defined as UACR >300 mg/g at the end of follow-up. Data were obtained from participants' medical records, and the 25-hydroxyvitamin D level was categorized into three groups as follows: G1 (N=45), <10 ng/mL; G2 (N=80), 10-20 ng/ml; and G3 (N=53), ≥20 ng/mL. The Nephroseq database (http://v5.nephroseq.org) was used to identify VDR expression in diabetic nephropathy. The dataset GSE142025 from GEO (http://www.ncbi.nlm.nih.gov/geo) was downloaded. After stratification by the median-centered log2 VDR expression value, the 21 advanced DKD samples were divided into two groups (low VDR expression group and high VDR expression group). Gene set enrichment analysis (GSEA) (http://software.broadinstitute.org/gsea/index.jsp). Differentially expressed genes (DEGs) were screened by the limma package (adjusted p 1). The Gene Ontology (GO; http://www.geneontology.org/) database and pathway analysis within the Kyoto Encyclopedia of Genes and Genomes (KEGG; https://www.kegg.jp/) were performed using the R package ClusterProfile. The CIBERSORT (Cell type Identification By Estimating Relative Subsets Of known RNA Transcripts) algorithm was utilized for calculating the infiltrated immune cells in advanced kidney tissues. Results: 1) A multivariate Cox regression analysis revealed that DR (diabetic retinopathy), eGFR (estimated glomerular filtration rate), and 25-hydroxyvitamin D were significant independent predictors of DKD progression (HR: 2.57, 95% CI: 1.44.4.24, p=0.007; HR: 2.13, 95% CI: 1.58.3.79, p = 0.011; HR: 0.732, 95% CI: 0.232–0.816, p = 0.023, respectively). 2) Kaplan–Meier survival curves of DKD progression by serum 25-hydroxyvitamin D stratification showed that the G2 and G3 groups were significantly different when compared with the G1 group (log-rank χ2 = 14.69, p <0.001; χ2 = 28.26, p <0.001, respectively). 3) There was a weak negative correlation between 25-hydroxyvitamin D level and UACR at baseline,and the overall mean rate of change in eGFR was 1.121 ± 0.19 ml/min/1.73 m2/year. Neither crude nor adjusted rate of decline in eGFR was significantly different among patients classified according to baseline serum 25-hydroxyvitamin D levels (all p<0.05). 4) The high expression of VDR group was most positively correlated with enriched gene sets like reactome innate immune system and reactome G alpha I signaling events when compared with the low expression of VDR group. 5) The CIBERSORT algorithm showed decreased M2 macrophage infiltration in advanced kidneys in comparison to low VDR expression and high VDR expression. Conclusion: This study concluded that low 25-hydroxyvitamin D levels can predict an increased risk of DKD albuminuria progression and eGFR decline. Decreased M2 macrophage infiltration may be a potential mechanism involved in this pathogenesis.

Abstract:Objective: To explore the associations of urinary retinol binding protein (RBP) and β2-microglobulin (β2-MG) with urinary albumin to creatinine ratio (UACR) and renal function in hospitalized patients with type 2 diabetes mellitus (T2DM). Methods: A total of 1 030 Chinese patients with T2DM were included in this study. The subjects were divided into the UACR normal group (<30 mg/g), microalbuminuria group (30-300 mg/g) and macroalbuminuria group (>300 mg/g). Patients with normal UACR were further divided into two groups according to the estimated glomerular filtration rate (eGFR): the eGFR low group (<90 ml·min-1·1.73m-2) and the normal eGFR group (≥90 ml·min-1·1.73m-2). Urine RBP and β2-MG levels among the groups were compared. Multiple linear regression analyses were applied to evaluate risk factors of urine RBP and β2-MG. Results: In all patients (n=1 030), urine RBP and β2-MG increased gradually with the increase of UACR across the three groups, the proportions of abnormal urine RBP (>0.7 mg/L) and β2-MG (>370 μg/L) in these groups were 3.8%, 8.5%, 39.0% (P<0.001), and 12.9%, 26.7%, 46.8% (P<0.001), respectively. In the UACR normal group (n=788), 12.2% of the patients were with eGFR<90 ml·min-1·1.73m-2. The proportion of abnormal β2-MG (>370 μg/L) was higher in the eGFR low group than that in the eGFR normal group (29.2% vs. 10.7%, P<0.001). Multivariate linear stepwise regression analyses were performed using natural logarithm of urine RBP or β2-MG as dependent variable, and showed that urine RBP was independently associated with UACR (β=0.0005, P<0.001), serum creatinine (β=0.006, P<0.001) and glycosylated hemoglobin A1c (β=0.050, P=0.001), and β2-MG was independently correlated with UACR (β=0.000 4, P<0.001), serum creatinine (β=0.011, P<0.001), systolic blood pressure (β=0.005, P=0.031) and fasting blood-glucose (β=0.027, P=0.046). Conclusions: Urine RBP and β2-MG are positively associated with high UACR and impaired renal function in T2DM patients, and these changes could occur before UACR and eGFR turned out to be abnormal. It is recommended that urine RBP and β2-MG be detected as early as possible to identify diabetic kidney disease in patients with normal UACR and eGFR.
Associations of Bone Mineral Density and Bone Metabolism Indices with Urine Albumin to Creatinine Ratio in Chinese Patients with Type 2 Diabetes.

Abstract:OBJECTIVE:To identify correlations of bone mineral density (BMD) and bone metabolism indices with the urine albumin to creatinine ratio (ACR) as an indicator of nephropathy in Chinese patients with type 2 diabetes (T2D). METHODS:In this retrospective analysis, 297 patients with T2D were divided into 3 groups according to the urine ACR. Patients' data were analyzed to identify associations of general conditions, blood glucose level, lipid levels, and uric acid level with BMD and bone metabolism indices. RESULTS:BMD at every location tested (femoral neck, trochanter, inside hip, Ward's triangle, total hip, and lumbar vertebrae) was negatively correlated with the urine ACR (all p<0.05). Osteocalcin, beta-C-terminal telopeptide (β-CTX), and procollagen type 1 N- peptide (P1NP) were positively correlated with urine ACR (all p<0.05). Finally, 25-hydroxyvitamin D [25(OH)D] was negatively correlated with urine ACR (p<0.05). Multiple regression analysis with adjustment for age, body mass index, disease duration, and other clinical measurements revealed no significant correlation between urine ACR and BMD measurements or β-CTX (p>0.05). However, significant correlations remained between urine ACR and osteocalcin, P1NP, and 25(OH)D (p<0.05). The same results were obtained for postmenopausal women specifically, with the exception of a significant correlation between the ACR and β-CTX (p<0.05). CONCLUSION:In the early stage of diabetic nephropathy, BMD changes and bone transformation acceleration may occur, and the acceleration of bone transformation may occur before the change in BMD. Therefore, it is important to monitor bone metabolism indices in the early stage of diabetic nephropathy in T2D patients.
Correlations of Serum Retinol-Binding Protein and Stromal Cell-Derived Factor-1 with Renal Function in Patients with Diabetic Kidney Disease

Abstract:BACKGROUND: The aim is to investigate the correlations of serum retinol-binding protein (RBP) and stromal cell-derived factor-1 (SDF-1) with renal function in patients with diabetic kidney disease (DKD).METHODS: A total of 438 patients with type 2 diabetes mellitus (T2DM) treated from October 2017 to October 2020 were enrolled in this prospective study and divided into simple T2DM and DKD groups. According to urinary albumin-to-creatinine ratio (UACR), DKD patients were divided into moderate, severe, and nephrotic groups. They were assigned to one of the following categories of estimated glomerular filtration rate (eGFR): G1, G2, G3a, G3b, G4, and G5 stages. The correlations of RBP and SDF-1 with renal function were analyzed.RESULTS: The DKD group had a longer T2DM course and higher RBP, uric acid (UA), blood urea nitrogen (BUN), β2-microglobulin (β2-MG), serum creatinine (Scr) levels and UACR, and lower SDF-1 level and eGFR than those of simple T2DM group (p < 0.05). The areas under the receiver operating characteristic curves of RBP and SDF-1 for identifying DKD were 0.903 and 0.868, and the optimal cutoff values were 70.71 mg/L and 5.69 ng/mL, respectively. With increasing urinary albumin and clinical stage, RBP, UA, BUN, β2-MG and Scr levels and UACR significantly rose, while SDF-1 level and eGFR declined (p < 0.05). In patients with DKD, RBP was correlated positively with UACR, UA, BUN, β2-MG, and Scr (r = 0.764/0.787/0.693/0.577/0.801, p < 0.0001), and negatively with eGFR (r = -0.782, p < 0.0001). SDF-1 was correlated negatively with UACR, UA, BUN, β2-MG and Scr (r = -0.744/-0.794/-0.666/-0.605/-0.820, p < 0.0001), and positively with eGFR (r = 0.767, p < 0.0001). The multiple linear regression equation was RBP = 29.852 + 0.007 x UACR + 0.101 x UA + 0.497 x BUN + 0.034 x Scr-0.083 x eGFR (p < 0.001).CONCLUSIONS: RBP and SDF-1 can identify DKD in patients with T2DM, and the degree of renal function damage is correlated positively with RBP and negatively with SDF-1. Elevated levels of UA, BUN, Scr and UACR as well as reduced eGFR are risk factors for evaluating RBP.

Abstract:Aims/Introduction: Diabetic kidney disease (DKD) is defined as diabetes with impaired renal function, elevated urinary albumin excretion, or both. DKD is one of the most common microvascular complications of diabetes and plays an important role in the cause of end-stage renal disease (ESRD). About 5% of people with type 2 diabetes (T2DM) already have kidney damage at the time they are diagnosed, but other triggers of renal insufficiency, such as obesity, hyperlipidemia, glomerular atherosclerosis are often present, making it difficult to define "diabetic kidney disease" or "diabetic nephropathy" precisely in epidemiology or clinical practice. Therefore, the aim of this study is to identify diabetic patients with CKD at an early stage, and evaluate the value of tubular injury markers including α1-microglobulin (α1-MG), β2-microglobulin (β2-MG), N-acetyl-beta-D-glucosaminidase (NAG) and Urinary retinol binding protein (URBP) in the development of diabetes to DKD. Materials and methods: We recruited a total of 182 hospitalized patients with T2DM in the First Affiliated Hospital of Zhengzhou University from February 2018 to April 2023. We collected basic clinical characteristics and laboratory biochemical parameters of the patients. Based on their levels of urinary albumin creatinine ratio (UACR) and glomerular filtration rate (GFR), patients were divided into DM group (UACR≤30 mg/g and eGFR≥90 mL/min/1.73 m2, n = 63) and DKD group (UACR>30 mg/g or eGFR<90 mL/min/1.73 m2, n = 119) excluding other causes of chronic kidney disease. We further developed diagnostic models to improve the ability to predict the risk of developing DKD by screening potential risk factors using univariate and multivariate logistic regression analysis. Calibration plots and curve analysis were used to validate the model and clinical usefulness. Next, we screened patients with relatively normal estimated glomerular filtration rate (eGFR) (≥90 mL/min/1.73 m2) to investigate whether tubular injury markers could accurately predict the risk of DKD in patients with normal renal function. We defined the rate of GFR decline as a prognostic indicator of renal function in patients and collected the information of the re-hospitalized DKD patients to determine whether the relevant indicators had an impact on the renal prognosis. Results: The patients with DKD had higher levels of tubular injury markers than patients with DM. URBP, α1-MG, eGFR were statistically different in both univariate and multivariate logistic regression analyses and displayed great predictive power after modeling with an area under curve of 0.987. The calibration curve showed medium agreement. Decision curve showed it would add more net benefits for clinical decision. After adjusting eGFR and serum creatinine (Scr), URBP was demonstrated to be associated with early renal function impairment. Conclusion: Tubular injury markers play an important role in early diabetic renal function impairment.
Expression of Angiopoetin-Like Protein-4 and Kidney Injury Molecule-1 as Preliminary Diagnostic Markers for Diabetes-Related Kidney Disease: A Single Center-Based Cross-Sectional Study

Abstract:The purpose of the study was to examine the urinary levels of kidney injury molecule-1 (KIM-1) and angiopoietin-like protein-4 (ANGPTL-4) in individuals with diabetic kidney disease (DKD) and their association with established DKD diagnostic markers such as albuminuria and estimated glomerular filtration rate (eGFR). Levels of ANGPTL-4 and KIM-1 were estimated in urine samples. A total of 135 participants were recruited into three groups: 45 diabetes type 2 patients in the control group and 90 DKD patients in two disease groups. Concentrations of ANGPTL-4 and KIM-1 were conclusively related to the urinary albumin-creatinine ratio (UACR). Also, the levels of both ANGPTL-4 and KIM-1 were negatively associated with the eGFR. Multivariable Poisson regression analysis showed that urinary ANGPTL-4 (PR: 3.40; 95% CI: 2.32 to 4.98; p < 0.001) and KIM-1 (PR: 1.25; 95% CI: 1.14 to 1.38; p < 0.001) were prevalent in DKD patients. Receiver operating characteristic (ROC) analysis of urinary ANGPTL-4 and KIM-1 in the combined form resulted in an area under curve (AUC) of 0.967 (95%CI: 0.932-1.000; p < 0.0001) in the microalbuminuria group and 1 (95%CI: 1.000-1.000; p < 0.0001) in the macroalbuminuria group. The association of urinary levels of ANGPTL-4 and KIM-1 with UACR and eGFR and significant prevalence in the diabetic kidney disease population illustrates the diagnostic potential of these biomarkers.

Abstract:Chronic kidney disease (CKD) is a growing health concern with a complex etiological landscape. Among the numerous factors implicated, vitamin D binding protein (VDBP) has emerged as a focal point of scientific studies because of its critical role in vitamin D metabolism and immune modulation. The relationship between VDBP and CKD reveals a complex web of molecular and biochemical details that have great potential for improving diagnostic understanding and treatment strategies for CKD. This review summarizes the multifaceted roles of VDBP, including its molecular dynamics, interactions with vitamin D, and subsequent implications for kidney function. The main focus of the discussion is how VDBP affects bone mineral homeostasis, highlighted by the dysregulation of calcium and phosphorus metabolism, which is a part of the pathophysiology of CKD. The discussion also touches on the immunomodulatory scope of VDBP and how it may reduce the chronic inflammatory environment that accompanies CKD. The diagnostic potential of VDBP as a biomarker for CKD has been rigorously examined, highlighting its capacity to improve early detection and prognostic assessment. Modification of VDBP activity has the potential to slow the course of CKD and improve patient outcomes. Furthermore, a detailed examination of the genetic polymorphisms of VDBP and their implications for CKD susceptibility and treatment responsiveness provides a perspective for personalized medical methods. Prospects for the future depend on the expansion of studies that try to understand the molecular mechanisms underlying the VDBP-CKD interaction, in addition to clinical trials that evaluate the effectiveness of VDBP-focused treatment approaches.

Does serum kidney injury molecule-1 predict early diabetic nephropathy: A comparative study with microalbuminuria

Abstract:Introduction: Diabetic nephropathy (DN) is a multifactorial disease, one of the most common complications of diabetes and a major cause of chronic kidney disease. Kidney injury molecule-1 (KIM-1) is a sensitive and specific marker of kidney injury as well as a predictor of prognosis. Objective: The present study aimed to investigate the usefulness of serum KIM-1 as an early marker of DN. Patients and Methods: The present study included total 75 participants, among whom 25 nondiabetic participants were chosen as controls. The 50 diabetic participants were divided into two groups according to urine protein/creatinine ratio (UPCR) as participants with normoalbuminuria (T2DM patients without nephropathy) and microalbuminuria (T2DM patients with nephropathy). The complete blood count, blood glucose, HbA1c, serum electrolytes, and creatinine levels were measured using standard laboratory techniques, and serum KIM-1 levels were measured by sandwich enzyme-linked immunosorbent assay. Results: There was a significant difference in the mean serum KIM-1 between the control and diabetics without microalbuminuria ( P = 0.0001). Patients with longer duration of diabetes had a higher serum KIM-1 values ( P = 0.05 in DM without microalbuminuria; P = 0.007 for DM with microalbuminuria). Serum KIM-1 did not correlate with UPCR in controls ( P = ‒0.167), in diabetics with microalbuminuria ( P = 0.487). However, there was a significant correlation observed between UPCR and serum KIM-1 in diabetics without microalbuminuria ( P = 0.04). Conclusion: The present study observed significantly increased levels of serum KIM-1 in both the diabetic groups compared to controls. Moreover, serum KIM-1 positively correlated with the duration of diabetes. Therefore, serum KIM-1 may be used as an early diagnostic marker to predict nephropathy among diabetes in our population.
Evaluation of Urinary Biomarkers of Proximal Tubular Injury, Inflammation, and Fibrosis in Patients With Albuminuric and Nonalbuminuric Diabetic Kidney Disease

Abstract:IntroductionAlbuminuric and non-albuminuric pathways contribute to diabetic kidney disease (DKD). Proximal tubule and inflammation play important roles in these processes. There is a need for urinary biomarker/s to detect early kidney damage and predict progression.MethodsNine urinary biomarkers were measured at baseline in 400 patients with diabetes. Correlation and multivariate logistic and linear regression analyses were performed to assess the association of biomarkers with CKD and progression.ResultsIn ACR<3 cohort, only biomarker significantly associated with eGFR<60mL/min was N-acetyl-β-d-glucosaminidase (NAG). A combination of ACR and monocyte chemotactic protein−1 (MCP1) were significantly associated with stage2 CKD in this cohort Logistic models: In patients with all levels of albuminuria, ACR, RBP and MCP1 were associated with progression. A model including MCP1, Interleukin 6 (IL6) and neutrophil gelatinase-associated lipocalin (NGAL) showed significant association with progression to CKD3/4 in ACR<3 cohort. Linear mixed model regression analyses demonstrated MCP1, RBP and ACR as significant proteins associated with progression to stage3 or worse while MCP1 was the only significant biomarker in ACR<3 cohort. Time-to-event and Cox Proportional Hazard models confirmed significant hazard ratios for progression for ACR, RBP and MCP1 with significant differences noted between quantiles of biomarkers for ACR, RBP and MCP1.ConclusionIn this study of diabetic patients with single baseline measurements of urinary biomarkers, Albumin, RBP and MCP1 were significantly associated with CKD progression at all levels of albuminuria. Inflammatory cytokines, NGAL and MCP1 were associated with progression in patients without albuminuria. NAG demonstrated significant association with eGFR<60 in ACR<3 cohort.

Abstract:Objectives: Diabetic kidney disease (DKD) is one of the most common chronic complications in diabetic patients, and there are major limitations in its pathological diagnosis. This study's objectives were to examine the changes in serum insulin-like growth factor-1 (IGF-1) and interleukin-6 (IL-6) levels in DKD patients with various urinary albumin/creatinine ratio (ACR) and to evaluate the utility of these two biological markers in the clinical diagnosis of the condition. Methods: We chose 80 type 2 diabetic patients as the experimental group and 20 healthy normal participants as the control group. The experimental group was split into three groups based on the ACR range: diabetes without nephropathy group (ACR < 30 mg/g), microalbuminuric group (30 < ACR 300 mg/g). The levels of serum IL-6 and IGF-1 were assessed in each trial participant. Results: Serum IGF-1 was higher in the experimental group than in the control group ( P < 0.01), and serum IL-6 levels were also higher than in the control group ( P < 0.001). In DKD patients, serum levels of IL-6 and IGF-1 tended to rise when ACR levels rose. By Pearson correlation analysis, serum IGF-1 and IL-6 were positively correlated with ACR (r = 0.765 and r = 0.651, all P < 0.001) and negatively correlated with eGFR (r = -0.389 and r = -0.364, all P < 0.01). Additionally, the receiver operating characteristic (ROC) characteristic curve showed that the area under the curve (AUC) values for serum IGF-1 and IL-6 were 0.9056 and 0.7850, respectively, while the AUR value for both combined was 0.9367. Conclusion: Serum IGF-1 and IL-6 levels can be used to diagnose DKD, and the combined analysis of these two indicators can improve the sensitivity and specificity of the disease diagnosis.

Abstract:Objective To measure the levels of urinary KIM-1 and serum cystatin C in type 2 diabetic patients(DM) with different degree of albuminuria and to evaluate the value of these biomarkers in predicting the early diabetic nephropathy(DN).Methods One hundred and eighteen subjects with type 2 diabetes mellitus and 41 control subjects were recruited; In the diabetic patients,44 were normoalbuminuric,41 were microalbuminuric,and 33 were macroalbuminuric,according to their urine albumin / creatinine ratio(UACR).Collecting fresh morning urine and plasma samples to detect the levels of KIM-1 and Cystatin C.Results The levels of urinary KIM-1(UKIM-1) were significantly higher in the DM group than in the control group,but no significantly difference between DM groups.The levels of serum cystatin C were significantly higher in the DM group than in the control group,which increased corresponding with UACR.UKIM-1 and serum Cystatin C were positively related with UACR(r = 0.427,0.726,both P < 0.001) and negatively related with eGFR(improved simplified MDRD formula)(r =- 0.008,- 0.791,for Cystatin C P < 0.05).Conclusion Our study indicates that UKIM-1 may be useful to assess early kidney injury prior to microalbuminuria,and serum Cystatin C can predict the severity of all stages of DN.

Abstract:Proteinuria and serum creatinine are commonly employed biomarkers for monitoring the advancement of diabetic nephropathy, but they may not comprehensively capture all types of lesions and are indicative of declining glomerular filtration rate only in the later stages. Recent research has identified biomarkers reflecting different aspects of diabetic kidney disease (DKD) that can provide information on the risk of DKD progression and associated poor prognosis. This review outlines several biomarkers, such as renal injury molecule-1, Neutrophil gelatinase-associated lipocalin, heat shock protein 72, soluble urokinase plasminogen activator receptor, angiopoetin-Like protein-4, monocyte chemoattractant protein-1, and liver-type fatty acid-binding protein. The research revealed that, following adjustingments for serum creatinine or estimated glomerular filtration rate and proteinuria, the concentrations of plasma and urine biomarkers remained elevated in individuals with diabetic kidney disease and were additionally correlated with the progression of DKD. These new biomarkers represent different biological pathways such as tubular injury, glomerular injury, tubular dysfunction, and inflammation, and can be used as liquid biopsies to better characterize the disease of diabetic nephropathy. Novel blood and urine biomarkers improve clinicians' ability to assess the prognosis of DKD progression and may improve understanding of the pathophysiology of DKD. However, for preclinical biomarkers associated with DKD outcomes, there was considerable heterogeneity between study cohorts and designs, limiting the comparison of prognostic performance between different studies. Larger clinical studies are needed to determine how these biomarkers could be used in the clinic to improve the clinical management of DKD.
Changes of Urinary L-FABP, KIM-1, NGAL and Serum Cystatin C in Diabetic Nephropathy and Its Clinical Value

Abstract:Objective To investigate the changes of urinary liver-type fatty acid binding protein (L-FABP),kidney injury molecule-1 (KIM-1),neutrophil gelatinase-associated lipocalin (NGAL) and serum cystatin C in diabetic nephropathy and its clinical value.Methods A total of 118 patients diagnosed with type 2 diabetes in the Department of Endocrinology of Affiliated Hospital of Sichuan Medical University from October 2011 to October 2012 were recruited in this study.According to their urinary albumin-to-creatinine ratio(UACR),patients were divided into normal albuminuria group(n =45),microalbuminuria group (n =42),and macroalbuminuria group (n =43).A total of 41 healthy subjects were enrolled as normal control group(n =41).Enzyme-linked immunosorbent assays were used to measure the levels of urinary L-FABP,KIM-1 and NGAL.Turbidimetric inhibition immuno-assay was used to measure the expression of serum cystatin C.All the urinary indicators were adjusted by the level of urine creatinine.Changes of biomarkers in each group and their crrelations between UACR,eGFR were also compared.Results Compared with normal control group,urinary L-FABP and serum cystatin C were significantly increased in diabetic groups (x2 =77.959,104.003,all P ＜ 0.05);urinary KIM-1 was also significantly increased (x2 =29.711,P ＜ 0.05).Urinary NGAL was increased in microalbuminuria group and macroalbuminuria group compared with normal control group and normal albuminuria group(x2 =23.833,P ＜ 0.05),but there were no significant difference between normal albuminuria group and normal control group.Urinary L-FABP,KIM-1,NGAL and serum cystatin C were positively correlated with UACR (r =0.719,0.427,0.327,0.726,all P ＜ 0.01);while L-FABP and serum cystatin C were negatively correlated with eGFR (r =-0.301,-0.791,all P ＜ 0.01).Conclusion Urinary L-FABP,KIM-1,NGAL and serum cystatin C are increased in early diabetic nephropathy,while urinary L-FABP and serum cystatin C may be the better biomarkers to predict diabetic renal injury.

Abstract:Objective. To investigate the associations of 25-(OH)D andβ-cell function or insulin resistance or albuminuria in Chinese type 2 diabetic patients.Methods. In total, 1408 type 2 diabetic patients without vitamin D supplement were included in this retrospective study.Results. Comparison between patients with and without 25-(OH)D deficiency indicated that, compared with patients with 25-(OH)D ≥ 50 nmol/L, patients with 25-(OH)D < 50 nmol/L showed a higher level of urine albumin-creatinine ratio (ACR) (90.15±10.30 mg/g versus52.79±14.97 mg/g). Multiple regression analysis indicated that 25-(OH)D was independently and negatively correlated with urine ACR (OR=0.985, 95%CI 0.972–0.999,P=0.03), adjusted by age, diabetic duration, HBP duration, SBP, HbA1c, creatinine, LDL-C, triglyceride, total cholesterol, and HDL-C. Compared with patients with normal level of urine ACR, patients with higher level of urine ACR showed a significant lower level of 25-(OH)D (34.49±13.52 nmol/L versus37.46±13.6 nmol/L,P=0.00). Analysis of the associations of 25-(OH)D andβ-cell function or insulin resistance showed that 25-(OH)D may not correlate withβ-cell function or insulin resistance.Conclusion. 25-(OH)D was independently associated with albuminuria in Chinese type 2 diabetic patients but was not associated withβ-cell function or insulin resistance.
