Assessment of secondary cancer risks within non-target organs during proton therapy for lung cancer: A Monte Carlo study

Abstract:Proton therapy is a rapidly progressing modality with a significant impact on lung cancer treatment. However, there are concerns about the subsequent effects of secondary radiation in out-of-field organs. Thus, the present study aimed to evaluate the risk of subsequent secondary cancers within non-target organs during proton therapy for lung cancer. A Monte Carlo model of the International Commission on Radiological Protection (ICRP) 110 male phantom was employed to calculate the absorbed dose associated with secondary photons and neutrons within out-of-field organs for different tumor locations. The risk of induced secondary cancers was then estimated using the Biological Effects of Ionizing Radiation Committee (BEIR) VII and National Council on Radiation Protection and Measurements (NCRP) 116 risk models. Organs close to the tumor, such as the heart, esophagus, thymus, and liver, received the highest equivalent doses. The calculated equivalent doses increased as the tumor depth increased from 4-8 cm to 12-16 cm. The contribution of neutrons to the total equivalent dose was dominant (up to 90%) in most of the organs studied. The calculated risks of secondary cancers were higher in the liver and esophagus compared with other organs when using the BEIR risk model. The maximum risk value was obtained for the left lung when the NCRP 116 risk model was used. Furthermore, the estimated risks of secondary malignancies increased with the tumor depth using both risk models. The calculated risks of radiation-induced secondary cancers were relatively lower than the baseline cancer risks. However, extra attention is warranted to minimize subsequent secondary cancers after proton therapy for lung cancer.

Abstract:Introduction: Early cancer detection combined with new treatment technologies has resulted in higher numbers of long‐term cancer survivors. The risk of radiation‐induced secondary cancers to tissues away from the PTV is a growing concern in particular for pediatric patients. The focus of this project is to use whole‐body pediatric phantoms in Monte Carlo dose calculations in order to determine the effective dose from secondary radiation in patients undergoing proton treatment. Methods: Age and gender specific pediatric phantoms have been implemented into the Geant4 Monte Carlo package for organ dose calculations. A proton therapy treatment plan for a pediatric head and neck tumor case was chosen to address the significance of age dependent phantoms for radiation protection calculations. To mimic radiation therapy treatment, the setup of the phantom position was based on field parameters (based on a full treatment head model), including gantry angle, couch angle, and iso‐center position. We distinguish between secondary radiation from the treatment head and secondary radiation generated within the patient. Results: Results using an adult phantom as well as phantoms of a 4‐year old female and an 11‐year old male were analyzed. Organ doses and radiation and tissue weighting factors were used to calculate the effective dose. For proton treatments with double‐scattering system, range modulator and aperture, a significant number of secondary neutrons are generated in the treatment head. More important, differences between phantoms (age dependent) were found with respect to dose to specific organs and relative importance of neutrons generated in the patient versus neutrons from the treatment head. Conclusion: We present results of doses to various anatomic sites in the human body for whole‐body phantoms. The magnitude of secondary dose in organs/tissues depends on the distance from the PTV. For the first time, the significance of age‐dependent phantoms for secondary dose calculations was studied.

Abstract:Epithelial sodium channels (ENaC) are regulated by various intracellular and extracellular factors including divalent cations. We studied the inhibitory effect and mechanism of external Ni(2+) on cloned mouse alpha-beta-gamma ENaC expressed in Xenopus oocytes. Ni(2+) reduced amiloride-sensitive Na(+) currents of the wild type mouse ENaC in a dose-dependent manner. The Ni(2+) block was fast and partially reversible at low concentrations and irreversible at high concentrations. ENaC inhibition by Ni(2+) was accompanied by moderate inward rectification at concentrations higher than 0.1 mm. ENaC currents were also blocked by the histidine-reactive reagent diethyl pyrocarbonate. Pretreatment of the oocytes with the reagent reduced Ni(2+) inhibition of the remaining current. Mutations at alphaHis(282) and gammaHis(239) located within the extracellular loops significantly decreased Ni(2+) inhibition of ENaC currents. The mutation alphaH282D or double mutations alphaH282R/gammaH239R eliminated Ni(2+) block. All mutations at gammaHis(239) eliminated Ni(2+)-induced inward current rectification. Ni(2+) block was significantly enhanced by introduction of a histidine at alphaArg(280). Lowering extracellular pH to 5.5 and 4.4 decreased or eliminated Ni(2+) block. Although alphaH282C-beta-gamma channels were partially inhibited by the sulfhydryl-reactive reagent [2-(trimethylammonium)ethyl] methanethiosulfonate bromide (MTSET), alpha-beta-gamma H239C channels were insensitive to MTSET. From patch clamp studies, Ni(2+) did not affect unitary current but decreased open probability when perfused into the recording pipette. Our results suggest that external Ni(2+) reduces ENaC open probability by binding to a site consisting of alphaHis(282) and gammaHis(239) and that these histidine residues may participate in ENaC gating.

Abstract:The use of 60Co teletherapy unit for the treatment of unilateral retinoblastoma (Rb) patients is a very common procedure in many developing countries including Tanzania. The aim of this study was to estimate organ-specific absorbed doses from an external beam radiation therapy 60Co unit for unilateral Rb and to assess the risks of the patients developing a secondary primary cancer. The absorbed dose estimations were based on a Monte Carlo method and a set of age-dependent computational male phantoms. The estimated doses were used to calculate the secondary cancer risks in out-of-field organs using the Biological Effects of Ionising Radiation VII risk models. The survival information and baseline cancer risks were based on relevant statistics for the Tanzanian population. The resulting out-of-field organ doses data showed that organs which are close to the target volume, such as the brain, salivary glands and thyroid glands, received the highest absorbed dose from scattered photons during the treatment of Rb. It was also found that the resulting photons dose to specific organs depends on the patient's age. Younger patients are more sensitive to radiation and also received higher dose contributions from the treatment head due to a larger part of the body exposed to the photon radiation. In all sites considered, the overall risks associated with radiation-induced secondary cancer were relatively lower than the baseline risks. Thus, the results in this article can help to provide good estimations of radiation-induced secondary cancer after radiation treatment of unilateral Rb using 60Co teletherapy unit in Tanzania and other developing countries.

Abstract:Purpose: With the average age of radiation therapy patients decreasing due to early cancer detection and a larger proportion of patients being cured and thus at risk for treatment complications, there has to be a growing concern for the risk of developing cancer due to radiation treatments, in particular for pediatric and young patients. Age and gender specific whole‐body phantoms have been implemented in Geant4 in order to determine the dose from secondary radiation in patients undergoing protontreatment.Method and Materials: In a systematic study, we have used three methods to calculate the neutronorgan equivalent dose to 14 organs distal to the target volume for six circular fields of varying diameters and range/modulation width combinations and for two patient fields that were previously used to treat two pediatric patients at the F. H. Burr Proton Therapy Center of Massachusetts General Hospital for brain lesions. Results using an adult phantom as well as phantoms of a 9‐month old male, a 4‐year old female, an 8‐year old female, an 11‐year old male and a 14‐year old male were analyzed. Results: The relative contribution to organdose from secondary radiation from the treatment head and secondary radiation generated within the patient was evaluated. We found that the organ equivalent dose has a marked dependence on the phantom age, the distance between the organ and the PTV as well as on field characteristics. Overall, the organ equivalent doses were below 3 mSv per treatment Gy. Conclusions: The organ equivalent neutrondose for protontreatments of brain lesions in organs distal to the target was calculated for six age‐dependent phantoms. We found that the dose decreases with patient age and also depends on field size, beam energy and modulation. Work supported by NIH/NCI RO1 CA 116743.

Abstract:Objective:Image-guided and adaptive proton therapy rely on daily CBCT or CT imaging, which increases radiation dose and radiation-induced cancer risk. Online adaptation however also reduces setup uncertainty, and the additional risk might be compensated by reducing the setup robustness margin. This work developed a framework to investigate how much this robustness margin should be reduced to offset the secondary cancer risk from additional imaging dose and applied it to proton therapy for head-and-neck cancer. Approach:For five patients with head-and-neck cancer, voxel-wise CT and CBCT imaging doses were estimated with Monte Carlo radiation transport simulations, calibrated with air and PMMA phantom measurements. The total dose of several image-guided and adaptive treatments protocols was calculated by summing the planning CT dose, daily and weekly CBCT or CT dose, and therapy dose, robustly optimized with setup margins between 0 and 4 mm. These were compared to a reference protocol with 4 mm setup margin without daily imaging. All plans further used 3% range robustness. Organ-wise excess absolute risk (EAR) of cancer was calculated with three models to determine at which setup margin the total EAR of image-guided and adaptive treatment protocols was equal to the total EAR of the reference. Results:The difference between the simulated and measured CT and CBCT doses was within 10%. Using the Monte Carlo models, we found that a 1 mm setup margin reduction was sufficient for most patients, treatment protocols, and cancer risk models to compensate the additional risk \imposed by daily and weekly imaging. For some protocols, even a smaller reduction sufficed, depending on the imaging frequency and type. The risk reduction by reducing the margin was mainly due to reducing the risk for carcinomas in the brain and, for some patients, the oral cavity. Significance: Our framework allows to compare an increased imaging dose with the reduced treatment dose from margin reductions in terms of radiation-induced cancer risk. It is extendable to different treatment sites, modalities, and imaging protocols, in clinic-specific or even patient-specific assessments.

Abstract:Purpose: The present work aimed to evaluate organ doses and related risk for cancer from external beam radiation treatment (EBRT) and high-dose-rate (HDR) brachytherapy (BT) involving Co-60 source for patients with cervical carcinoma in Tanzania based on Monte Carlo methods and to evaluate the secondary cancer risks in their lifetime. Methods: EBRT and HDR-BR were modelled by using the MCNPX Monte Carlo (MC) code. The MC simulations were performed by using validated models and isocentric irradiation of an adult female computational phantom. The organ doses and cancer risks estimates were obtained. Results: The highest absorbed doses of 6.98 x 10(-2) and 5.74 x 10(-2) Sv/Gy were recorded in the bladder for BT and EBRT. The higher risk was found for colon at 1.06 x 10(-3) in the HDR-BT and 9.75 x 10(-5) in the EBRT per 100,000 population at exposure age of 35 years than in the other organs. The risk magnitude decreased with increasing age at exposure. In general, the secondary cancer risks in all sites considered from EBRT and HDR-BR for cervical cancer patient were lower than the baseline risks. Conclusions: The chances of developing secondary cancer take years following radiation therapy are extremely low, but the results of present study can support to establish a future database on secondary cancer risks involving radiation therapy in patients with cervical cancer by using HDR-BR and EBRT with Co-60 source in Tanzania and other developing countries.

Abstract:Purpose: To determine the effective secondary neutron dose to patients undergoing proton therapy. Goal was to separate the secondary radiation generated from the proton treatment nozzle from the one generated inside the patient. Further, the secondary dose should be calculated for specific organs independently. Method and Materials: GEANT4 based Monte Carlo methods were used to study the neutron induced secondary dose deposited to healthy organs/tissues during proton therapy. A whole‐body patient model, VIP‐Man, was implemented as patient model. Two proton treatment plans for tumor in the lung tumor and paranasal sinus were applied to VIP‐Man. Internal (generated inside the patient body) and external (generated in the treatment nozzle) neutrons were studied separately. The averaged absorbed dose and neutron quality factor were deduced for each organ. The ICRP‐60 tissue weighting factors were used to calculate the whole body effective dose. Results: The magnitude of secondary dose in organs/tissues depends on the distance to the primarily irradiated volume. In general, the averaged secondary dose is at least three orders of magnitudes lower than the planned dose in GTV. The neutron quality factor is calculated as to be ∼6 since the neutron fluence includes a large fraction of thermal and low energy neutrons. Neutrons are mainly generated in the treatment nozzle. Results show that the whole body effective dose for the two proton plans is 0.162 Sv and 0.027 Sv, respectively. The statistic error of the estimated effective dose is better than 0.5% and 2% for internal and external neutrons, respectively. Conclusion: Our VIP‐Man Monte Carlo method is an effective tool for secondary dose calculations in patient geometry. Secondary dose can be calculated organ specific. Our method can be used for any treatment modality and is planned to serve as basis for more quantitative evaluation of secondary cancer risk following radiation therapy.

Abstract:Purpose: It has been long known that patients treated with radiation carry a risk of developing second cancers in their lifetimes as a result of their treatments. In addition to hosting a symposium on “Secondary Cancer Risks for Emerging Radiation Treatments,” the AAPM has recently formed Task Group 158 that will provide recommendations on the dosimetry protocols for quantifying scattered radiation dose outside the treatment volume. Measurements are useful in providing a relative comparison of different treatment modalities or beam characteristics. On the other hand, more precise organ dose equivalent values need to be calculated from Monte Carlo simulations and anatomically realistic computational phantoms. This study outlines a computational platform that integrates anatomically‐refined virtual patient models with a detailed model of a medical accelerator head to calculate scattered doses to patients treated with radiation. Method and Materials: A detailed model of a 6‐MV Varian Clinac 2100C was developed in MCNPX. Several virtual patient models have been integrated with the accelerator head model in MCNPX. Various conventional and IMRT treatment plans have been considered. Results: Average organ doses are significantly different than point‐wise measurement values. The differences can be attributed to various sources of scattered radiation that deposits dose in the organ. Large dose gradients will be present throughout organs at short and intermediate distances from the tumor volume, while organs at longer distances will have a more uniform dose distribution. Conclusion: A computational platform to calculate organ dose from scattered radiation during radiation treatments has been demonstrated. Results indicate that average organ/tissue doses are significantly different than point‐wise measurements. Organ doses calculated in this manner will likely provide more accurate organ dose information for epidemiologic studies and for assessing the risk during the treatment planning.

Abstract:Standard methods for risk assessments resulting from human exposures to mixed radiation fields in Space consisting of different particle types and energies rely upon quality factors. These are generally defined as a function of linear energy transfer (LET) and are assumed to be proportional to the risk. In this approach, it is further assumed that the risks for single exposures from each of the radiation types add linearly. Although risks of cancer from acute exposures to photon radiations have been measured in humans, quality factors for protons and ions of heavier atomic mass are generally inferred from animal and/or cellular data. Because only a small amount of data exists for such particles, this group has been examining tumourigenesis initiated by energetic protons and iron ions. In this study, 741 female Sprague-Dawley rats were irradiated or sham irradiated at approximately 60 days of age with 250 MeV protons, 1 GeV/nucleon iron ions or both protons and iron ions. The results suggest that the risk of mammary tumours in the rats sequentially irradiated with 1 GeV/nucleon 56Fe ions and 250 MeV protons is less than additive. These data in conjunction with earlier results further suggest that risk assessments in terms of only mean LETs of the primary cosmic rays may be insufficient to accurately evaluate the relative risks of each type of particle in a radiation field of mixed radiation qualities.
Risk of Second Primary Cancers among Long-Term Survivors of Breast Cancer

Abstract:Purpose: The current study explored the risk of developing second primary cancers (SPCs) among long-term early-stage breast cancer survivors and identified risk factors to build an externally validated clinical prediction model. Methods: The cumulative incidence of SPCs was calculated by Gray method among survivors of early-stage initial primary breast cancer (IPBC). Comparisons of treatment-related risk by selected organ sites were performed. A nomogram was established to estimate the individual risk of developing SPCs based on the multivariate Fine and Gray risk model. Decision curve analysis (DCA) was used to evaluate clinical usefulness of the model. Results: The cumulative incidence of developing SPCs after early-stage IPBC was 7.43% at 10 years, 14.41% at 15 years, and 20.08% at 20 years. Radiotherapy was associated with elevated risks of any SPCs and with elevated risks of lung cancer (SHR: 1.109; P = 0.045), breast cancer (SHR: 1.389; P < 0.001), and AML (SHR: 1.298; P = 0.045). Chemotherapy was significantly associated with a declined risk of any SPCs, with decreased risks of lung (SHR: 0.895; P = 0.015) and breast cancers (SHR: 0.891; P < 0.001), as well as elevated risks of other leukemias (SHR: 1.408; P = 0.002). HR-positive status was associated with decreased risks of any SPCs; with decreased risks of breast (SHR: 0.842; P < 0.001) and ovarian cancers (SHR: 0.483; P < 0.001); and with elevated risks of urinary tract cancers (SHR: 1.214; P = 0.029). Conclusion: We found that the cumulative incidence of developing SPCs increased over time and did not plateau. Risk factors for developing SPCs identified by our study were not consistent with those of previous studies. The prediction model can help identify individuals at higher risk of SPCs.
EVALUATION OF SECONDARY DOSE AND CANCER RISK FOR OUT-OF-FIELD ORGAN IN ESOPHAGEAL CANCER IMRT IN A CHINESE HOSPITAL USING ATOM PHANTOM MEASUREMENTS.

Abstract:There have been few studies on the secondary cancer after radiation treatment in Chinese hospitals. This study has measured out-of-field absorbed organ doses from intensity-modulated radiation therapy (IMRT) radiotherapy for esophageal cancer in a Chinese hospital and evaluated the risks of secondary cancer. The dose measurements were based on the thermo-luminescence dosemeter (TLD) and the ATOM(A (R)) phantom, which represents an adult male. Over 100 TLD chips were placed in 35 different organ sites and one group of the same TLDs was set as background contrast. All TLDs were calibrated against the same Linac accelerator performing an IMRT plan for esophageal cancer. The measured doses were used to calculate the secondary cancer risks according to biological effects of ionizing radiation (BEIR) VII methodology. The baseline risks and survival data were based on relevant statistics for the Chinese population. It is found that the out-of-field organ doses depended greatly on the distance between organ sites and the target isocenter. The organ doses decreased exponentially as the distance from the target isocenter increased, and, for distances < 15 cm, the organ doses fell off more rapidly and almost decreased by 99.55%. When compared with the calculation results by the Pinnacle treatment planning system (TPS), most of the out-of-field organ doses were underestimated in the TPS and the percentage of underestimation reached 100% for distant organs such as the bladder, prostate and testis. These trends are due to a known fact that out-of-field organs received secondary radiation resulted from patients and collimator scattering as well as leakage in the gantry head. The higher lifetime attribute risks (LARs) per 100 000 population were in the lower esophagus (186) and lungs (93.2) near the target. But all LARs of considered organs were found to be less than the baseline cancer risks. Results in this article can help to provide a database about the effect of radiotherapy-induced secondary cancer incidence during esophageal cancer treatment in China.
Radiation Therapy for Stage IIA/B Seminoma: Modeling Secondary Cancer Risk for Protons and VMAT versus 3D Photons

Abstract:Radiation therapy (RT) is an effective treatment for stage IIA and select stage IIB seminomas. However, given the long life expectancy of seminoma patients, there are concerns about the risk of secondary cancers from RT. This study assessed differences in secondary cancer risk for stage II seminoma patients following proton pencil-beam scanning (PBS) and photon VMAT, compared to 3D conformal photon RT. Ten seminoma patients, five with a IIA staging who received 30 GyRBE and five with a IIB staging who received 36 GyRBE, had three RT plans generated. Doses to organs at risk (OAR) were evaluated, and secondary cancer risks were calculated as the Excess Absolute Risk (EAR) and Lifetime Attributable Risk (LAR). PBS reduced the mean OAR dose by 60% on average compared to 3D, and reduced the EAR and LAR for all OAR, with the greatest reductions seen for the bowel, liver, and stomach. VMAT reduced high doses but increased the low-dose bath, leading to an increased EAR and LAR for some OAR. PBS provided superior dosimetric sparing of OAR compared to 3D and VMAT in stage II seminoma cases, with models demonstrating that this may reduce secondary cancer risk. Therefore, proton therapy shows the potential to reduce acute and late side effects of RT for this population.

Radiation-Related New Primary Solid Cancers in the Childhood Cancer Survivor Study: Comparative Radiation Dose Response and Modification of Treatment Effects

Abstract:Objectives: The majority of childhood cancer patients now achieve long-term survival, but the treatments that cured their malignancy often put them at risk of adverse health outcomes years later. New cancers are among the most serious of these late effects. The aims of this review are to compare and contrast radiation dose-response relationships for new solid cancers in a large cohort of childhood cancer survivors and to discuss interactions among treatment and host factors. Methods: This review is based on previously published site-specific analyses for subsequent primary cancers of the brain, breast, thyroid gland, bone and soft tissue, salivary glands, and skin among 12,268 5-year childhood cancer survivors in the Childhood Cancer Survivor Study. Analyses included tumor site-specific, individual radiation dose reconstruction based on radiation therapy records. Radiation-related second cancer risks were estimated using conditional logistic or Poisson regression models for excess relative risk (ERR). Results: Linear dose-response relationships over a wide range of radiation dose (0-50 Gy) were seen for all cancer sites except the thyroid gland. The steepest slopes occurred for sarcoma, meningioma, and nonmelanoma skin cancer (ERR/Gy > 1.00), with glioma and cancers of the breast and salivary glands forming a second group (ERR/Gy = 0.27-0.36). The relative risk for thyroid cancer increased up to 15-20 Gy and then decreased with increasing dose. The risk of thyroid cancer also was positively associated with chemotherapy, but the chemotherapy effect was not seen among those who also received very high doses of radiation to the thyroid. The excess risk of radiation-related breast cancer was sharply reduced among women who received 5 Gy or more to the ovaries. Conclusions: The results suggest that the effect of high-dose irradiation is consistent with a linear dose-response for most organs, but they also reveal important organ-specific and host-specific differences in susceptibility and interactions between different aspects of treatment.
Spatial correlation of linear energy transfer and relative biological effectiveness with suspected treatment-related toxicities following proton therapy for intracranial tumors

Abstract:Purpose: The enhanced relative biological effectiveness (RBE) at the end of the proton range might increase the risk of radiation-induced toxicities. This is of special concern for intracranial treatments where several critical organs at risk (OARs) surround the tumor. In the light of this, a retrospective analysis of dose-averaged linear energy transfer (LETd ) and RBE-weighted dose (DRBE ) distributions was conducted for three clinical cases with suspected treatment-related toxicities following intracranial proton therapy. Alternative treatment strategies aiming to reduce toxicity risks are also presented. Methods: The clinical single-field optimized (SFO) plans were recalculated for 81 error scenarios with a Monte Carlo dose engine. The fractionation DRBE was 1.8 Gy (RBE) in 28 or 30 fractions assuming a constant RBE of 1.1. Two LETd - and α/β-dependent variable RBE models were used for evaluation, including a sensitivity analysis of the α/β parameter. Resulting distributions of DRBE and LETd were analyzed together with normal tissue complication probabilities (NTCPs). Subsequently, four multi-field optimized (MFO) plans, with an additional beam and/or objectives penalizing protons stopping in OARs, were created to investigate the potential reduction of LETd , DRBE , and NTCP. Results: The two variable RBE models agreed well and predicted average RBE values around 1.3 in the toxicity volumes, resulting in an increased near-maximum DRBE of 7-11 Gy (RBE) compared to RBE = 1.1 in the nominal scenario. The corresponding NTCP estimates increased from 0.8%, 0.0%, and 3.7% (RBE = 1.1) to 15.5%, 1.8%, and 45.7% (Wedenberg RBE model) for the three patients, respectively. The MFO plans generally allowed for LETd , DRBE , and NTCP reductions in OARs, without compromising the target dose. Compared to the clinical SFO plans, the maximum reduction in the near-maximum LETd was 56%, 63%, and 72% in the OAR exhibiting the toxicity for the three patients, respectively. Conclusions: Although a direct causality between RBE and toxicity cannot be established here, high LETd and DRBE correlated spatially with the observed toxicities, whereas setup and range uncertainties had a minor impact. Individual factors, which might affect the patient-specific radiosensitivity, were however not included in these calculations. The MFO plans using both an additional beam and proton track-end objectives allowed the largest reductions in LETd , DRBE , and NTCP, and might be future tools for similar cases.
Brain-Specific Relative Biological Effectiveness of Protons Based on Long-term Outcome of Patients With Nasopharyngeal Carcinoma

Abstract:Purpose: Uncertainties in relative biological effectiveness (RBE) constitute a major pitfall of the use of protons in clinics. An RBE value of 1.1, which is based on cell culture and animal models, is currently used in clinical proton planning. The purpose of this study was to determine RBE for temporal lobe radiographic changes using long-term follow-up data from patients with nasopharyngeal carcinoma. Methods and materials: Five hundred sixty-six patients with newly diagnosed nasopharyngeal carcinoma received double-scattering proton therapy or intensity modulated radiation therapy at our institutions. The 2 treatment cohorts were well matched. Proton dose distributions were simulated using Monte Carlo and compared with those obtained from the proton clinical treatment planning system. Late treatment effect was defined as development of enhancement of temporal lobe on T1-weighted magnetic resonance imaging, with or without accompanying clinical symptoms. The tolerance dose was calculated with receiving operator characteristic analysis and the Youden index. Tolerance curves, expressed as a cumulative dose-volume histogram, were generated using the cutoff points. Results: With a median follow-up period >5 years for both cohorts, 10% of proton patients and 4% of patients undergoing intensity modulated radiation therapy developed temporal lobe enhancement in unilateral temporal lobe. There was no significant difference in dose distributions between the Monte Carlo method and treatment planning system. The tolerance dose-volume levels were V10 (26.1%), V20 (21.9%), V30 (14.0%), V40 (7.7%), V50 (4.8%), and V60 (3.3%) for proton therapy (P < .03). Comparison of the two tolerance curves revealed that tolerance doses of proton treatments were lower than that of photon treatments at all dose levels. The dose tolerance at D1% was 58.56 Gy for protons and 69.07 Gy for photons. The RBE for temporal lobe enhancement from proton treatments were calculated to be 1.18. Conclusions: Using long-term clinical outcome of patients with nasopharyngeal carcinoma, our data suggest that the RBE for temporal lobe enhancement is 1.18 at D1%. A prospective study in a large cohort would be necessary to confirm these findings.
Radiation exposure of the glandular mammary tissue in women patients with mediastinal Hodgkin lymphoma treated with protons

Abstract:Purpose: Secondary breast cancer is a frequent late adverse event of mediastinal Hodgkin lymphoma radiotherapy. Secondary breast cancers overwhelmingly correspond to ductal carcinoma and develop from the glandular mammary tissue. In addition, during childhood, radiation overexposure of the glandular tissue may lead to a late breast hypotrophy at adult age. The aim of this study was to evaluate the radiation exposure to the glandular tissue in patients treated for mediastinal Hodgkin lymphoma with intensity-modulated proton therapy, in order to evaluate the potential dosimetric usefulness of its delineation for breast sparing. Materials and methods: Sixteen consecutive intermediate-risk mediastinal female patients with Hodgkin lymphoma treated with consolidation radiation with deep inspiration breath hold intensity-modulated proton therapy to the total dose of 30Gy were included. Breasts were delineated according to the European Society for Radiotherapy and Oncology guidelines for treatment optimization ("clinical organ at risk"). The glandular tissue ("glandular organ at risk") was retrospectively contoured on the initial simulation CT scans based on Hounsfield unit (HU) values, using a range between -80HU and 500HU. Results: The mean and maximum doses delivered to the glandular organ at risk were significantly lower than the mean and maximum doses delivered to the clinical organ at risk, but were statistically correlated. Glandular organ at risk volumes were significantly smaller. Conclusion: Optimizing the treatment plans on the clinical breast contours will systematically lead to overestimation of the dose received to the glandular tissue and, consequently, to an indistinct and involuntary improved glandular tissue sparing. As such, our findings do not support the consideration of the glandular tissue as an additional organ at risk when planning intensity-modulated proton therapy for mediastinal Hodgkin lymphoma in female patients.
