Abstract:PurposeHypofractionated radiotherapy of prostate cancer is gaining clinical acceptance given its potential increase in therapeutic ratio and evidence for noninferiority and lack of added late toxicities compared to conventional fractionation. However, concerns have been raised that smaller number of fractions might lead to larger dosimetric influence by interfractional motion. We aim to compare the effect of these variations on hypofractionated pencil beam scanning (PBS) proton therapy and volumetric modulated arc therapy (VMAT) for localized prostate cancer.MethodsWeekly CT images were acquired for 6 patients participating in a randomized clinical trial. PBS plans featuring bilateral (BL) and a combination of lateral and anterior-oblique beams (AOL), and VMAT plans were created. All patients were treated to a conventional 79.2Gy total dose in 44 fractions. For this study, hypofractionated dose to the prostate gland was 51.6Gy in 12 fractions or 36.25Gy in 5 fractions, and 32.8, and 23.1Gy to proximal seminal vesicles, respectively. Patients were simulated with endorectal balloons to aid gland immobilization. Three fiducial markers were implanted for setup guidance. All plans were recomputed on the weekly CT images after aligning with the simulation CT. The entire set of 9 CT images was used for dose recalculation for 12-fraction and only 5 used for the 5-fraction case. Adaptive range adjustments were applied to anterior-oblique beams assuming clinical availability of invivo range verification. Fractional doses were summed using deformable dose accumulation to approximate the delivered dose. Biologically equivalent dose to 2Gy(EQD2) was calculated assuming / of 1.5Gy for prostate and 3Gy for bladder and rectum.ResultsThe median delivered prostate D-98 was 0.13/0.14/0.13Gy(EQD2) smaller than planned for PBS-BL, 0.13/0.27/0.17Gy(EQD2) for PBS-AOL and 0.59/0.66/0.59Gy(EQD2) for VMAT, for 44/12/5 fractions, respectively. The largest D-98 reduction was 1.5 and 3.5Gy(EQD2) for CTV1 and CTV2, respectively. Target dose degradation was comparable for all fractionation schemes within each modality. The maximum increase in rectum D-2 was 0.98Gy(EQD2) for a 5-fraction PBS case.ConclusionsThe robustness of PBS and VMAT were comparable for all patients for the studied fractionations. The delivered target dose generally remained within clinical tolerance and the deviations were relatively minor for both fractionation schemes. The delivered OAR dose stayed in compliance with the RTOG hypofractionation constraints for all cases.

Abstract:Introduction: Early cancer detection combined with new treatment technologies has resulted in higher numbers of long‐term cancer survivors. The risk of radiation‐induced secondary cancers to tissues away from the PTV is a growing concern in particular for pediatric patients. The focus of this project is to use whole‐body pediatric phantoms in Monte Carlo dose calculations in order to determine the effective dose from secondary radiation in patients undergoing proton treatment. Methods: Age and gender specific pediatric phantoms have been implemented into the Geant4 Monte Carlo package for organ dose calculations. A proton therapy treatment plan for a pediatric head and neck tumor case was chosen to address the significance of age dependent phantoms for radiation protection calculations. To mimic radiation therapy treatment, the setup of the phantom position was based on field parameters (based on a full treatment head model), including gantry angle, couch angle, and iso‐center position. We distinguish between secondary radiation from the treatment head and secondary radiation generated within the patient. Results: Results using an adult phantom as well as phantoms of a 4‐year old female and an 11‐year old male were analyzed. Organ doses and radiation and tissue weighting factors were used to calculate the effective dose. For proton treatments with double‐scattering system, range modulator and aperture, a significant number of secondary neutrons are generated in the treatment head. More important, differences between phantoms (age dependent) were found with respect to dose to specific organs and relative importance of neutrons generated in the patient versus neutrons from the treatment head. Conclusion: We present results of doses to various anatomic sites in the human body for whole‐body phantoms. The magnitude of secondary dose in organs/tissues depends on the distance from the PTV. For the first time, the significance of age‐dependent phantoms for secondary dose calculations was studied.

Abstract:Purpose The treatment of brain tumors in pregnant patients poses challenges, as the out‐of‐field dose exposure to the fetus can potentially be harmful. A pregnant patient with prior radiation treatment was presented with a brain tumor at our clinic. This work reports on our pre‐treatment study that compared fetal dose exposure between intensity‐modulated proton therapy (IMPT) using pencil beam scanning (PBS) and conventional photon 3D conformal radiation therapy (3DCRT) and volumetric‐modulated arc therapy (VMAT), and the subsequent pregnant patient's radiation treatment. Materials and methods Pre‐treatment measurements of clinical plans, 3DCRT, VMAT, and IMPT, were conducted on a phantom. Measurements were performed using a device capable of neutron detections, closely following AAPM guidelines, TG158. For photon measurements, fetus shielding was utilized. On patient treatment days, which was determined to be proton treatment, shielding was used only during daily imaging for patient setup. Additionally, an in vivo measurement was conducted on the patient. Results Measurements showed that IMPT delivered the lowest fetal dose, considering both photon and neutron out‐of‐field doses to the fetus, even when shielding was implemented for photon measurements. Additionally, the proton plans demonstrated superior treatment for the mother, a reirradiation case. Conclusion The patient was treated with proton therapy, and the baby was subsequently delivered at full term with no complications. This case study supports previous clinical findings and advocates for the expanded use of proton therapy in this patient population.

Abstract:Purpose: Increasing concern that brainstem toxicity incidence after proton radiation therapy might be higher than with photons led to a 2014 University of Florida (UF) landmark paper identifying its risk factors and proposing more conservative dose constraints. We evaluated how practice patterns changed among the Pediatric Proton/Photon Consortium Registry (PPCR). Material and methods: This prospective multicenter cohort study gathered data from patients under the age of 22 years enrolled on the PPCR, treated between 2002 and 2019 for primary posterior fossa brain tumors. After standardizing brainstem contours, we garnered dosimetry data and correlated those meeting the 2014 proton-specific brainstem constraint guidelines by treatment era, histology, and extent of surgical resection. Results: A total of 467 patients with evaluable proton radiation therapy plans were reviewed. Median age was 7.1 years (range: <1-21.9), 63.0% (n = 296) were men, 76.0% (n = 357) were White, and predominant histology was medulloblastoma (55.0%, n = 256), followed by ependymoma (27.0%, n = 125). Extent of resection was mainly gross total resection (GTR) (67.0%, n = 312), followed by subtotal resection (STR) or biopsy (20.0%, n = 92), and near total resection (NTR) (9.2%, n = 43). The UF brainstem constraint metrics most often exceeded were the goal D50% of 52.4 gray relative biological equivalents (43.3%, n = 202) and maximal D50% of 54 gray relative biological equivalents (12.6%, n = 59). The compliance rate increased after the new guidelines (2002-2014: 64.0% vs 2015-2019: 74.6%, P = .02), except for ependymoma (46.3% pre- vs 50.0% post-guidelines, P = .86), presenting lower compliance (48.8%) in comparison to medulloblastoma/ primitive neuroectodermal tumors/pineoblastoma (77.7%), glioma (89.1%), and atypical teratoid/rhabdoid tumors (90.9%) (P < .001). Degree of surgical resection did not affect compliance rates (GTR/NTR 71.0% vs STR/biopsy 72.8%, P = .45), even within the ependymoma subset (GTR/NTR 50.5% vs STR/biopsy 38.1%, P = .82). Conclusion: Since the publication of the UF guidelines, the pediatric proton community has implemented more conservative brainstem constraints in all patients except those with ependymoma, irrespective of residual disease after surgery. Future work will evaluate if this change in practice is associated with decreased rates of brainstem toxicity.

Abstract:Soluble cytokine receptors naturally arise from genes encoding membrane‐bound receptors or are direct derivatives of the receptors themselves. There is mounting evidence that soluble receptors play important roles in human disease states. In many cases, soluble receptors appear to play an integral part in the dynamic interaction between ligands and their membrane‐bound receptors in maintaining and restoring health after a pathological insult but, in some instances, dysregulated expression of soluble receptors can contribute to disease pathology. Nonetheless, an appreciation of the biological actions of soluble receptors, particularly as cytokine inhibitors, has led to their therapeutic use in human diseases. Although early clinical trials of soluble receptors have had unexpected toxicities, their application in medicine continues to advance and it is likely that soluble receptors will join hormones, cytokines, and growth factors as established biological therapies. Assessment of secondary cancer risks within non-target organs during proton therapy for lung cancer: A Monte Carlo study

Abstract:Proton therapy is a rapidly progressing modality with a significant impact on lung cancer treatment. However, there are concerns about the subsequent effects of secondary radiation in out-of-field organs. Thus, the present study aimed to evaluate the risk of subsequent secondary cancers within non-target organs during proton therapy for lung cancer. A Monte Carlo model of the International Commission on Radiological Protection (ICRP) 110 male phantom was employed to calculate the absorbed dose associated with secondary photons and neutrons within out-of-field organs for different tumor locations. The risk of induced secondary cancers was then estimated using the Biological Effects of Ionizing Radiation Committee (BEIR) VII and National Council on Radiation Protection and Measurements (NCRP) 116 risk models. Organs close to the tumor, such as the heart, esophagus, thymus, and liver, received the highest equivalent doses. The calculated equivalent doses increased as the tumor depth increased from 4-8 cm to 12-16 cm. The contribution of neutrons to the total equivalent dose was dominant (up to 90%) in most of the organs studied. The calculated risks of secondary cancers were higher in the liver and esophagus compared with other organs when using the BEIR risk model. The maximum risk value was obtained for the left lung when the NCRP 116 risk model was used. Furthermore, the estimated risks of secondary malignancies increased with the tumor depth using both risk models. The calculated risks of radiation-induced secondary cancers were relatively lower than the baseline cancer risks. However, extra attention is warranted to minimize subsequent secondary cancers after proton therapy for lung cancer.
The influence of daily imaging and target margin reduction on secondary cancer risk in image-guided and adaptive proton therapy

Abstract:Objective:Image-guided and adaptive proton therapy rely on daily CBCT or CT imaging, which increases radiation dose and radiation-induced cancer risk. Online adaptation however also reduces setup uncertainty, and the additional risk might be compensated by reducing the setup robustness margin. This work developed a framework to investigate how much this robustness margin should be reduced to offset the secondary cancer risk from additional imaging dose and applied it to proton therapy for head-and-neck cancer. Approach:For five patients with head-and-neck cancer, voxel-wise CT and CBCT imaging doses were estimated with Monte Carlo radiation transport simulations, calibrated with air and PMMA phantom measurements. The total dose of several image-guided and adaptive treatments protocols was calculated by summing the planning CT dose, daily and weekly CBCT or CT dose, and therapy dose, robustly optimized with setup margins between 0 and 4 mm. These were compared to a reference protocol with 4 mm setup margin without daily imaging. All plans further used 3% range robustness. Organ-wise excess absolute risk (EAR) of cancer was calculated with three models to determine at which setup margin the total EAR of image-guided and adaptive treatment protocols was equal to the total EAR of the reference. Results:The difference between the simulated and measured CT and CBCT doses was within 10%. Using the Monte Carlo models, we found that a 1 mm setup margin reduction was sufficient for most patients, treatment protocols, and cancer risk models to compensate the additional risk \imposed by daily and weekly imaging. For some protocols, even a smaller reduction sufficed, depending on the imaging frequency and type. The risk reduction by reducing the margin was mainly due to reducing the risk for carcinomas in the brain and, for some patients, the oral cavity. Significance: Our framework allows to compare an increased imaging dose with the reduced treatment dose from margin reductions in terms of radiation-induced cancer risk. It is extendable to different treatment sites, modalities, and imaging protocols, in clinic-specific or even patient-specific assessments.

Abstract:Purpose: With the average age of radiation therapy patients decreasing due to early cancer detection and a larger proportion of patients being cured and thus at risk for treatment complications, there has to be a growing concern for the risk of developing cancer due to radiation treatments, in particular for pediatric and young patients. Age and gender specific whole‐body phantoms have been implemented in Geant4 in order to determine the dose from secondary radiation in patients undergoing protontreatment.Method and Materials: In a systematic study, we have used three methods to calculate the neutronorgan equivalent dose to 14 organs distal to the target volume for six circular fields of varying diameters and range/modulation width combinations and for two patient fields that were previously used to treat two pediatric patients at the F. H. Burr Proton Therapy Center of Massachusetts General Hospital for brain lesions. Results using an adult phantom as well as phantoms of a 9‐month old male, a 4‐year old female, an 8‐year old female, an 11‐year old male and a 14‐year old male were analyzed. Results: The relative contribution to organdose from secondary radiation from the treatment head and secondary radiation generated within the patient was evaluated. We found that the organ equivalent dose has a marked dependence on the phantom age, the distance between the organ and the PTV as well as on field characteristics. Overall, the organ equivalent doses were below 3 mSv per treatment Gy. Conclusions: The organ equivalent neutrondose for protontreatments of brain lesions in organs distal to the target was calculated for six age‐dependent phantoms. We found that the dose decreases with patient age and also depends on field size, beam energy and modulation. Work supported by NIH/NCI RO1 CA 116743.
Clinical outcomes and late endocrine, neurocognitive, and visual profiles of proton radiation for pediatric low-grade gliomas

Abstract:Purpose/objective(s): Primary low-grade gliomas are common brain tumors of childhood, many of which require radiation therapy (RT) as definitive treatment. Increased conformality of RT could decrease the incidence and severity of late effects. We report our experience with 32 pediatric patients treated with proton RT. Methods and materials: Thirty-two pediatric patients with low-grade gliomas of the brain or spinal cord were treated with proton RT from 1995 to 2007. Sixteen patients received at least 1 regimen of chemotherapy before definitive RT. The median radiation dose was 52.2 GyRBE (48.6-54 GyRBE). Results: The median age at treatment was 11.0 years (range, 2.7-21.5 years), with a median follow-up time of 7.6 years (range, 3.2-18.2 years). The 6-year and 8-year rates of progression-free survival were 89.7% and 82.8%, respectively, with an 8-year overall survival of 100%. For the subset of patients who received serial neurocognitive testing, there were no significant declines in Full-Scale Intelligence Quotient (P=.80), with a median neurocognitive testing interval of 4.5 years (range, 1.2-8.1 years) from baseline to follow-up, but subgroup analysis indicated some significant decline in neurocognitive outcomes for young children (<7 years) and those with significant dose to the left temporal lobe/hippocampus. The incidence of endocrinopathy correlated with a mean dose of ≥40 GyRBE to the hypothalamus, pituitary, or optic chiasm. Stabilization or improvement of visual acuity was achieved in 83.3% of patients at risk for radiation-induced injury to the optic pathways. Conclusions: This report of late effects in children with low-grade gliomas after proton RT is encouraging. Proton RT appears to be associated with good clinical outcome, especially when the tumor location allows for increased sparing of the left temporal lobe, hippocampus, and hypothalamic-pituitary axis.

Abstract:Abstract Proton minibeam radiation therapy (pMBRT) is a novel dose delivery method based on spatial dose fractionation. pMBRT has been shown to be promising in terms of reduced side effects and superior tumour control in high-grade glioma-bearing rats compared to standard irradiation. These findings, together with the recent optimized implementation of pMBRT in a clinical pencil beam scanning system, have triggered reflection on the possible application to patient treatments. In this context, the present study was designed to conduct a first theoretical investigation of the clinical potential of this technique. For this purpose, a dedicated dose engine was developed and used to evaluate two clinically relevant patient treatment plans (high-grade glioma and meningioma). Treatment plans were compared with standard proton therapy plans assessed by means of a commercial treatment planning system (ECLIPSE-Varian Medical systems) and Monte Carlo simulations. A multislit brass collimator consisting of 0.4 mm wide slits separated by a centre-to-centre distance of 4 or 6 mm was placed between the nozzle and the patient to shape the planar minibeams. For each plan, spread-out Bragg peaks and homogeneous dose distributions (±7% dose variations) can be obtained in target volumes. The Peak-to-Valley Dose Ratios (PVDR) were evaluated between 9.2 and 12.8 at a depth of 20 mm for meningioma and glioma, respectively. Dose volume histograms (DVHs) for target volumes and organs at risk were quantitatively compared, resulting in a slightly better target homogeneity with standard PT than with pMBRT plans, but similar DVHs for deep-seated organs-at-risk and lower average dose for shallow organs. The proposed delivery method evaluated in this work opens the way to an effective treatment for radioresistant tumours and will support the design of future clinical research.

Abstract:Abstract Radiation therapy represents an important approach in the therapeutic management of children and adolescents with malignant tumors and its application with modern techniques – including Proton Beam Therapy (PBT) – is of great interest. In particular, potential radiation-induced injuries and secondary malignancies – also associated to the prolonged life expectancy of patients – are still questions of concern that increase the debate on the usefulness of PBT in pediatric treatments. This paper presents a literary review of current applications of PBT in non-Central Nervous System pediatric tumors (such as retinoblastoma, Hodgkin Lymphoma, Wilms tumor, bone and soft tissues sarcomas). We specifically reported clinical results achieved with PBT and dosimetric comparisons between PBT and the most common photon-therapy techniques. The analysis emphasizes that PBT minimizes radiation doses to healthy growing organs, suggesting for reduced risks of late side-effects and radiation-induced secondary malignancies. Extended follow up and confirms by prospective clinical trials should support the effectiveness and long-term tolerance of PBT in the considered setting.

Abstract:Epithelial sodium channels (ENaC) are regulated by various intracellular and extracellular factors including divalent cations. We studied the inhibitory effect and mechanism of external Ni(2+) on cloned mouse alpha-beta-gamma ENaC expressed in Xenopus oocytes. Ni(2+) reduced amiloride-sensitive Na(+) currents of the wild type mouse ENaC in a dose-dependent manner. The Ni(2+) block was fast and partially reversible at low concentrations and irreversible at high concentrations. ENaC inhibition by Ni(2+) was accompanied by moderate inward rectification at concentrations higher than 0.1 mm. ENaC currents were also blocked by the histidine-reactive reagent diethyl pyrocarbonate. Pretreatment of the oocytes with the reagent reduced Ni(2+) inhibition of the remaining current. Mutations at alphaHis(282) and gammaHis(239) located within the extracellular loops significantly decreased Ni(2+) inhibition of ENaC currents. The mutation alphaH282D or double mutations alphaH282R/gammaH239R eliminated Ni(2+) block. All mutations at gammaHis(239) eliminated Ni(2+)-induced inward current rectification. Ni(2+) block was significantly enhanced by introduction of a histidine at alphaArg(280). Lowering extracellular pH to 5.5 and 4.4 decreased or eliminated Ni(2+) block. Although alphaH282C-beta-gamma channels were partially inhibited by the sulfhydryl-reactive reagent [2-(trimethylammonium)ethyl] methanethiosulfonate bromide (MTSET), alpha-beta-gamma H239C channels were insensitive to MTSET. From patch clamp studies, Ni(2+) did not affect unitary current but decreased open probability when perfused into the recording pipette. Our results suggest that external Ni(2+) reduces ENaC open probability by binding to a site consisting of alphaHis(282) and gammaHis(239) and that these histidine residues may participate in ENaC gating.

Abstract:Purpose: To determine the effective secondary neutron dose to patients undergoing proton therapy. Goal was to separate the secondary radiation generated from the proton treatment nozzle from the one generated inside the patient. Further, the secondary dose should be calculated for specific organs independently. Method and Materials: GEANT4 based Monte Carlo methods were used to study the neutron induced secondary dose deposited to healthy organs/tissues during proton therapy. A whole‐body patient model, VIP‐Man, was implemented as patient model. Two proton treatment plans for tumor in the lung tumor and paranasal sinus were applied to VIP‐Man. Internal (generated inside the patient body) and external (generated in the treatment nozzle) neutrons were studied separately. The averaged absorbed dose and neutron quality factor were deduced for each organ. The ICRP‐60 tissue weighting factors were used to calculate the whole body effective dose. Results: The magnitude of secondary dose in organs/tissues depends on the distance to the primarily irradiated volume. In general, the averaged secondary dose is at least three orders of magnitudes lower than the planned dose in GTV. The neutron quality factor is calculated as to be ∼6 since the neutron fluence includes a large fraction of thermal and low energy neutrons. Neutrons are mainly generated in the treatment nozzle. Results show that the whole body effective dose for the two proton plans is 0.162 Sv and 0.027 Sv, respectively. The statistic error of the estimated effective dose is better than 0.5% and 2% for internal and external neutrons, respectively. Conclusion: Our VIP‐Man Monte Carlo method is an effective tool for secondary dose calculations in patient geometry. Secondary dose can be calculated organ specific. Our method can be used for any treatment modality and is planned to serve as basis for more quantitative evaluation of secondary cancer risk following radiation therapy.

Abstract:The use of 60Co teletherapy unit for the treatment of unilateral retinoblastoma (Rb) patients is a very common procedure in many developing countries including Tanzania. The aim of this study was to estimate organ-specific absorbed doses from an external beam radiation therapy 60Co unit for unilateral Rb and to assess the risks of the patients developing a secondary primary cancer. The absorbed dose estimations were based on a Monte Carlo method and a set of age-dependent computational male phantoms. The estimated doses were used to calculate the secondary cancer risks in out-of-field organs using the Biological Effects of Ionising Radiation VII risk models. The survival information and baseline cancer risks were based on relevant statistics for the Tanzanian population. The resulting out-of-field organ doses data showed that organs which are close to the target volume, such as the brain, salivary glands and thyroid glands, received the highest absorbed dose from scattered photons during the treatment of Rb. It was also found that the resulting photons dose to specific organs depends on the patient's age. Younger patients are more sensitive to radiation and also received higher dose contributions from the treatment head due to a larger part of the body exposed to the photon radiation. In all sites considered, the overall risks associated with radiation-induced secondary cancer were relatively lower than the baseline risks. Thus, the results in this article can help to provide good estimations of radiation-induced secondary cancer after radiation treatment of unilateral Rb using 60Co teletherapy unit in Tanzania and other developing countries.

Abstract:Purpose: It has been long known that patients treated with radiation carry a risk of developing second cancers in their lifetimes as a result of their treatments. In addition to hosting a symposium on “Secondary Cancer Risks for Emerging Radiation Treatments,” the AAPM has recently formed Task Group 158 that will provide recommendations on the dosimetry protocols for quantifying scattered radiation dose outside the treatment volume. Measurements are useful in providing a relative comparison of different treatment modalities or beam characteristics. On the other hand, more precise organ dose equivalent values need to be calculated from Monte Carlo simulations and anatomically realistic computational phantoms. This study outlines a computational platform that integrates anatomically‐refined virtual patient models with a detailed model of a medical accelerator head to calculate scattered doses to patients treated with radiation. Method and Materials: A detailed model of a 6‐MV Varian Clinac 2100C was developed in MCNPX. Several virtual patient models have been integrated with the accelerator head model in MCNPX. Various conventional and IMRT treatment plans have been considered. Results: Average organ doses are significantly different than point‐wise measurement values. The differences can be attributed to various sources of scattered radiation that deposits dose in the organ. Large dose gradients will be present throughout organs at short and intermediate distances from the tumor volume, while organs at longer distances will have a more uniform dose distribution. Conclusion: A computational platform to calculate organ dose from scattered radiation during radiation treatments has been demonstrated. Results indicate that average organ/tissue doses are significantly different than point‐wise measurements. Organ doses calculated in this manner will likely provide more accurate organ dose information for epidemiologic studies and for assessing the risk during the treatment planning.
Proton PBS Planning Techniques, Robustness Evaluation, and OAR Sparing for the Whole-Brain Part of Craniospinal Axis Irradiation

Abstract:Proton therapy is a promising modality for craniospinal irradiation (CSI), offering dosimetric advantages over conventional treatments. While significant attention has been paid to spine fields, for the brain fields, only dose reduction to the lens of the eye has been reported. Hence, the objective of this study is to assess the potential gains and feasibility of adopting different treatment planning techniques for the entire brain within the CSI target. To this end, eight previously treated CSI patients underwent retrospective replanning using various techniques: (1) intensity modulated proton therapy (IMPT) optimization, (2) the modification/addition of field directions, and (3) the pre-optimization removal of superficially placed spots. The target coverage robustness was evaluated and dose comparisons for lenses, cochleae, and scalp were conducted, considering potential biological dose increases. The target coverage robustness was maintained across all plans, with minor reductions when superficial spot removal was utilized. Single- and multifield optimization showed comparable target coverage robustness and organ-at-risk sparing. A significant scalp sparing was achieved in adults but only limited in pediatric cases. Superficial spot removal contributed to scalp V30 Gy reduction at the expense of lower coverage robustness in specific cases. Lens sparing benefits from multiple field directions, while cochlear sparing remains impractical. Based on the results, all investigated plan types are deemed clinically adoptable.

Abstract:For the preparation of Gamma Knife (GK) treatments of brain tumours, a head CT examination is necessary when using a new CT-based convolution dose calculation algorithm included in Leksell Gamma Plan software (version 10.1.1). The purpose of this study was to measure the organ doses in paediatric anthropomorphic phantoms during a head CT scan and compare them with the measured GK treatment doses. CT doses were measured with radiophotoluminescence (RPL) dosimeters and compared with results of MC simulations. Measured doses for organs within the CT scan volume were comparable for both phantoms. The highest organ CT dose was evaluated for eyes: 27.6 +/- 0.2 mGy and 27.3 +/- 0.6 mGy for 5 and 10 year old phantom respectively. Doses for organs not included in the scan volume decreased with distance from the scan volume and were on average 3.5 times higher for the 5 year old phantom compared with the 10 year old phantom. Comparison of measured and MC dose values showed good agteement. For the 10 year old phantom, CT doses were 22%, 6%, 5% and 3% of the GK dose for thyroid, lungs, eyes and breasts respectively. Enhancement of the total organ doses for the whole GK treatment due to CT scanning is not negligible but nevertheless, the use of the new convolution algorithm seems reasonable.
