Abstract:BACKGROUND:Currently available tools for assessing high cardiovascular risk (HCR) often require measurements not available in resource-limited settings in low- and middle-income countries (LMIC). There is a need to assess HCR using a pragmatic evidence-based approach. OBJECTIVES:This study sought to report the prevalence of HCR in 10 LMIC areas in Africa, Asia, and South America and to investigate the profiles and correlates of HCR. METHODS:Cross-sectional analysis using data from the National Heart, Lung, and Blood Institute-UnitedHealth Group Centers of Excellence. HCR was defined as history of heart disease/heart attack, history of stroke, older age (≥50 years for men and ≥60 for women) with history of diabetes, or older age with systolic blood pressure ≥160 mm Hg. Prevalence estimates were standardized to the World Health Organization's World Standard Population. RESULTS:A total of 37,067 subjects ages ≥35 years were included; 53.7% were women and mean age was 53.5 ± 12.1 years. The overall age-standardized prevalence of HCR was 15.4% (95% confidence interval: 15.0% to 15.7%), ranging from 8.3% (India, Bangalore) to 23.4% (Bangladesh). Among men, the prevalence was 1.7% for the younger age group (35 to 49 years) and 29.1% for the older group (≥50); among women, 3.8% for the younger group (35 to 59 years) and 40.7% for the older group (≥60). Among the older group, measured systolic blood pressure ≥160 mm Hg (with or without other conditions) was the most common criterion for having HCR, followed by diabetes. The proportion of having met more than 1 criterion was nearly 20%. Age, education, and body mass index were significantly associated with HCR. Cross-site differences existed and were attenuated after adjusting for age, sex, education, smoking, and body mass index. CONCLUSIONS:The prevalence of HCR in 10 LMIC areas was generally high. This study provides a starting point to define targeted populations that may benefit from interventions combining both primary and secondary prevention strategies.
The Impact of Perception Bias for Cardiovascular Disease Risk on Physical Activity and Dietary Habits

Abstract:Objective: Cardiovascular disease (CVD) remains a significant public health challenge in China. Accurate perception of individual CVD risk is crucial for timely intervention and preventive strategies. This study aimed to determine the alignment between CVD risk perception levels and objectively calculated CVD risk levels, then investigate the disparity in physical activity and healthy diet habits among distinct CVD risk perception categories. Methods: From March to August 2022, a cross-sectional survey was conducted in Zhejiang Province using convenience sampling. Participants aged between 20 and 80 years, without prior diagnosis of CVD were included. CVD risk perception was evaluated with the Chinese version of the Attitude and Beliefs about Cardiovascular Disease Risk Perception Questionnaire, while objective CVD risk was assessed through the Prediction for Atherosclerotic Cardiovascular Disease Risk (China-PAR) model. Participants’ demographic information, self-reported physical activity, and healthy diet score were also collected. Results: A total of 739 participants were included in the final analysis. Less than a third of participants (29.2%) accurately perceived their CVD risk, while 64.5% over-perceived it and 6.2% under-perceived it. Notably, half of the individuals (50.0%) with high CVD risk under-perceived their actual risk. Compared to the accurate perception group, individuals aged 60–80 years old (OR = 6.569), currently drinking (OR = 3.059), and with hypertension (OR = 2.352) were more likely to under-perceive their CVD risk. Meanwhile, participants aged 40-<60 years old (OR = 2.462) and employed (OR = 2.352) were more likely to over-perceive their risk. The proportion of individuals engaging in physical activity was lowest in the under-perception group, although the difference among the three groups was not statistically significant (χ2 = 2.556, P = 0.278). In addition, the proportion of individuals practicing healthy diet habits was also lowest in the under-perception group, and a significant statistical difference was observed among the three groups (χ2 = 10.310, P = 0.006). Conclusion: Only a small proportion of participants accurately perceived their CVD risk, especially among those with high actual CVD risk. Individuals in the under-perceived CVD risk group exhibited the lowest rates of physical activity engagement and healthy diet adherence. Healthcare professionals should prioritize implementing personalized CVD risk communication strategies tailored to specific subgroups to enhance the accuracy of risk perception.

Abstract:Background: Approximately 75% of the mortality and morbidity in low and middle-income countries (LMIC) are from non-communicable diseases (NCDs), with cardiovascular diseases (CVD) accounting for nearly 50% of this burden. This is driven, in part, by suboptimal control of hypertension and diabetes. Yet before designing culturally-appropriate self-management interventions to decrease this burden, we must first understand the reasons why hypertension and diabetes is poorly controlled. Methods: Project SEHAT is a cluster randomized controlled trial investigating the effect of a community health worker-led program to control hypertension, diabetes and smoking in a semi urban community in West Bengal, India. Seventy- eight participants with hypertension and/or diabetes between 35-70 years of age were enrolled and stratified on gender, medication adherence level, and whether they had seen a physician. Focus group discussions were conducted to examine behavioral factors affecting hypertension and diabetes self-management. All focus groups were digitally recorded and wo coders independently coded the data. Qualitative description was used to analyze the data. Results: Most patients and their family members were concerned about their hypertension or diabetes. However, only 26% (6/23) of interviewed patients recognized that they were at increased risk for a CVD event. A majority (72% - 23/32) claimed they were unable to adhere to their medications, often because of cost. Participants did not engage in healthy lifestyle behaviors (i.e., diet and exercise) and were unaware of these interventions. Approximately, 80% (40/50) visited modern physician while 10% (5/50) visited informal health practitioners, usually for financial reasons. Discussion: There is a gap in knowledge about how to prevent CVD, even as most patients expressed concern about it. Poor insight into the disease process, belief in traditional practices, and competing financial needs were significant barriers to adopting healthy lifestyle behaviors. Intervention models should consider the existing level of knowledge and work with local traditions in order to improve self-management among those with hypertension and diabetes in this community.

Abstract:Background: In Sub-Saharan Africa (SSA), the prevalence of hypertension is increasing due to many factors like rapid population growth, globalization, stress, and urbanization. We aimed to characterize the perceptions of cardiovascular disease (CVD) risk among individuals with hypertension living in Nigeria and identify barriers and facilitators to optimal hypertension management. Methods: This cross-sectional survey study was conducted at a large teaching hospital in Lagos, Nigeria. We used a convenient sample of males and females, aged 18 or older, with a diagnosis of hypertension who presented for outpatient visits in the cardiology, nephrology, or family medicine clinics between November 1 and 30, 2020. A semiquantitative approach was utilized with a survey consisting of closed and open-ended questionnaires focused on patient knowledge, perceptions of CVD risk, and barriers and facilitators of behavioral modifications to reduce CVD risk. Results: There were 256 subjects, and 62% were female. The mean age was 58.3 years (standard deviation (SD) = 12.6). The mean duration of the hypertension diagnosis was 10.1 years. Most participants were quite knowledgeable about hypertension; however, we observed some knowledge gaps, including a belief that too much "worrying or overthinking" was a major cause of hypertension and that an absence of symptoms indicated that hypertension was under control. Barriers to hypertension management include age, discomfort or pain, and lack of time as barriers to exercise. Tasteless meals and having to cook for multiple household members were barriers to decreasing salt intake. Cost and difficulty obtaining medications were barriers to medication adherence. Primary facilitators were family support or encouragement and incorporating lifestyle modifications into daily routines. Conclusion: We identified knowledge gaps about hypertension and CVD among our study population. These gaps enable opportunities to develop targeted interventions by healthcare providers, healthcare systems, and local governments. Our findings also help in the promotion of community-based interventions that address barriers to hypertension control and promote community and family involvement in hypertension management in these settings.

Abstract:Introduction: Knowledge of the risk of developing cardiovascular diseases (CVD) in the population is an important risk management strategy for the prevention of this disease. This is especially true for India, which has resource-restrained settings with an increased risk in a younger population for the development of the disease. An important modifiable risk factor for CVD is hypertension, with its influence on the development of CVD. Methods: The data from the first wave of the Longitudinal Ageing Study in India (LASI) was used to calculate the 10-year CVD Risk Score among older adults ≥45 years using a WHO (2019) non-laboratory- based chart for South Asia. Univariate analysis was done using Pearson's chi-square test, and multivariable analysis using ordinal logistic regression. Categories of CVD risk score were considered as dependent variable. Socio-demographic variables, regular exercise, history of diabetes and hyperlipidaemia were considered as the independent variables. Relationship between CVD Risk score and hypertensives and self-reported hypertensives were presented using restricted cubic splines. Result: Two-thirds (68.8%) of the population had a 10-year CVD risk of <10, and 2.8% had a risk of ≥20%. The self-reported hypertensives were distributed linearly in restricted cubic splines, with a more scattered distribution in higher scores, while actual hypertensives showed a sigmoid pattern. Urban residents (OR-0.88), being unmarried (OR-0.86), being in the richer (OR-0.94) and richest (OR-0.86) monthly per capita expenditure (MPCE) quintile and exercising regularly (OR-0.68) decreased the odds of being in a higher CVD risk score. Less than primary schooling (1.21) and diabetics (1.69) had higher odds for a higher CVD risk score. Conclusion: In this population, two-thirds had <10% risk for the development of CVD. The study shows a higher risk among rural, poor, and those with a lower education and lower CVD risk for those undertaking physical activity. The sigmoid pattern in actual hypertensives highlights the need for early detection. Even those with undiagnosed hypertension but with a higher BP had a similar risk for disease development, thus highlighting the need for an early detection of hypertension.

Abstract:Background of the Study: Cardiovascular Disease (CVD) is affecting millions of people in both developed and developing countries. Although the rate of death attributable to the disease has declined in developed countries in the past several decades, it is still the leading cause of death and extorts a heavy social and economic toll globally. In low-middle income countries, the prevalence of CVD has increased dramatically. By 2020, the disease is forecasted to be the major cause of morbidity and mortality in most developing nations. The Global Burden of Disease study estimate of age-standardized CVD death rate of 272 per 100000 populations in India is higher than the global average of 235 per 100000 populations. CAD was estimated to account for around 15%–20% and 6%–9% of all deaths in India and the US. In addition to mortality, CAD is also responsible for morbidity and loss of quality of life Materials and Methods: Qualitative cross-sectional study design with a descriptive research approach was adopted for the present study. The study was conducted in D. Y. Patil Hospital and Terna Multi-Speciality Hospital and Research Centre, Nerul, Navi Mumbai with a sample size of 75. A Non-probability purposive sampling technique was used. In this study samples were patients who underwent Percutaneous revascularization for coronary artery disease. Data was collected using an Interview technique. The data was tabulated and analysed in terms of objectives of the study, using descriptive and inferential statistics. Results: The study results show that the majority of the respondents belonged to the age group 46 - 65 years with a frequency of 27 (36%) and were married 57 (76%). Representation of male respondents was 44 (59%) and the rest of the respondents belonged to female gender. They were educated up to high school and employed (service staff); their monthly income was <30,000. They were currently diagnosed as STEMI 22 (29.33%), NSTEMI 17 (22.66%) and CAD/IHD 36 (48%). All the respondents had undergone PCI for CAD. Majority of them presented with the selected risk factor variables mentioned in the data collection tool. The respondents had knowledge about the classical symptoms of heart attack. However, they were unable to identify the other associative symptoms of heart attack. 97.33% of the respondents verbalized the correct response that is sudden pain and heaviness on the chest (n=73) which reciprocates to and sudden pain at the back of chest bone with pain moving towards the left or both arms (n=18) which reciprocates to 24%. However, few respondents have identified that heart attack is a sudden weakness of the upper arm and lower limb on one side of the body (n=26) which reciprocates to 35% and this option remains incorrect. Although from this (n=8) which reciprocates to 10% of the respondents have also managed to verbalize the right response which suggests that they may not have complete knowledge about heart attack symptoms. There was a significant difference between the number of people who were confident and those who were not confident, also there was a significant difference between confidence to some extent and versus confident (that is P<0.05) about the items on the lifestyle modifying factors. The results indicate that fewer patients were confident about the lifestyle modifying factors. Also, the respondents showed positive correlation (< +1.0) and negative correlation (<-1.0) with risk factor variables. The study findings revealed that, patient did not verbalize complete knowledge and confidence regarding secondary lifestyle modification through the statistical analysis. There is a significant difference between the number of people who were confident versus those who were not confident and there was a significant difference between confidence to some extent versus confidence about the items on the lifestyle modifying factors. Also, there is a significant difference between risk factors and all the lifestyle modifying variables. Conclusion: There were substantial disparities in the confidence levels associated with lifestyle modification and recognition/response to heart attack. These gaps need to be studied further and disseminated to improve awareness in terms of health education in the population which will eventually increase their level of confidence.

Abstract:A BSTRACT Aim: Diabetes and hypertension are major risk factors of cardiovascular disease, which is known to be the leading cause of global mortality in the world today. Studies have shown that the prevalence of these risk factors is on the rise, with the burden of diabetes alone increasing by 80% in the last two decades. Complications of diabetes and hypertension result in huge public health challenges for the country and catastrophic medical expenditures for families among the urban poor. Our study aims to estimate the prevalence of diabetes, hypertension, and other cardiovascular risk factors among adults in an urban underprivileged community of Bengaluru city. Objectives and Methods: A cross-sectional study was conducted over a period of 6 months where 2245 individuals aged 30 or older were interviewed using a structured interviewer-administered questionnaire used to capture sociodemographic details that assessed modifiable risk factors for diabetes and hypertension. Inclusion criteria for diabetes were considered if the random blood sugar reading was ≥200 mg/dL, whereas a diagnosis of hypertension was taken into consideration if the systolic blood pressure reading was ≥140 mmHg and/or diastolic blood pressure was ≥90 mmHg. Results: Among the 2245 participants that took part in the study, 15.5% were diabetics and 17.2% were hypertensive. There was a strong association of diabetes among consumers of alcohol, with more than one-third having a high prevalence of the disease (odds ratio (OR): 2.09, 95% confidence interval (95% CI): 1.1–3.9). More than half the population were consumers of junk food; the prevalence of diabetes in this group was 1.35 times higher than that in their counterparts (OR: 1.35, 95% CI: 1.0–1.8). A significant association of diabetes was also seen among those identified with central obesity (OR: 1.83, 95% CI: 1.4–2.5). One-third of the population who consumed alcohol were found to be diagnosed with hypertension (OR: 3.08, 95% CI: 1.6–5.9), and one-fifth of individuals who were regular consumers of junk food had a higher prevalence of hypertension (OR: 1.41, 95% CI: 1.1–1.8). A higher prevalence of hypertension was also seen among individuals with central obesity or a body mass index (BMI) of >30 (OR: 1.59, 95% CI: 1.2–2.1; OR: 1.92, 95% CI: 1.4–2.6). Conclusion: The findings from our study conducted in an urban underprivileged area of Bengaluru city shed light on the significant associations between diabetes and hypertension and various demographic and lifestyle factors. Specifically, male gender and lower educational status were found to have a significant association with diabetes, whereas being unmarried and having a high BMI status were strongly linked to hypertension. In addition, the study revealed that elderly individuals, alcohol consumers, junk food eaters, and those with central obesity demonstrated an increased risk for both diabetes and hypertension. By identifying these risk factors, targeted interventions can be developed to address the unique challenges faced by this vulnerable section of society. Strategies can be designed to raise awareness, encourage healthier lifestyle choices, and improve access to healthcare services to effectively prevent and manage diabetes and hypertension in this community.
Interleukin-10 promoter polymorphisms and expression in Thai children with juvenile systemic lupus erythematosus

Abstract:Interleukin (IL)-10 expression is regulated by its promoter and correlated with the activity of adult-onset lupus (systemic lupus erythematosus (SLE)). As the pathogenesis of adult-onset SLE may differ from SLE with the age at onset <18 years old (juvenile SLE or JSLE), we evaluated polymorphisms at positions −1082A/G, −819T/C and −592A/C of the IL-10 promoter and serum IL-10 levels in 71 patients with JSLE. Disease activity was determined by the SLE disease activity index (SLEDAI). Active SLE was defined by SLEDAI ≥6 and inactive SLE was defined by SLEDAI equal to zero. The mean age was 14.5 ± 2.8 years. Nephritis occurred in 57 patients. In JSLE patients, −592 CC and −819 CC were identified with a higher frequency than in controls with the odds ratio (OR) of 2.75 (95% confidence interval (CI) 1.11–6.81, p = 0.04). GCC increased the susceptibility to nephritis in patients with JSLE (OR 2.16, 95% CI 1.07–4.35, p = 0.03). Serum IL-10 levels were significantly higher in 20 JSLE patients with active disease than in 27 patients with inactive disease and in 15 healthy children (p < 0.001). In conclusion, IL-10 expression was upregulated in active JSLE. The −819 CC and −592 CC genotypes increased the susceptibility to JSLE and GCC increased the susceptibility to nephritis.

Abstract:Abstract Background Cardiovascular disease (CVD) is the leading cause of mortality in India. Social and behavioural factors are strongly interrelated in the prevention and control of CVD. The ability to make lifestyle changes to control hypertension and diabetes (major risk factors for CVD) is determined by factors such as education, gender, caste, poverty, and urbanicity. This study aimed to improve our understanding of the inter-relationship of social and behavioural factors in the management of elevated serum glucose and high blood pressure and co-morbid mental health conditions. Methods A population-based catchment area cross sectional survey was conducted in Kerala, India. Data were collected from residents aged over 30 years ( n = 997) using standardized tools and clinical measures. We performed latent class analysis incrementally to extract homogeneous latent classes of individuals based on their responses to social and behavioural risk factors in the survey. Using structural equation models, we assessed the mediating effect of depression and anxiety, and social or behavioural risk factors, on management of high blood pressure and raised serum glucose levels. Results The prevalence of high blood pressure and blood glucose in the sample was 33 and 26% respectively. Latent class analysis found three clusters of risk factors. One had a predominance of behavioural characteristics, another of social risk factors and the third was a low risk group. Age, female sex, and marital status had an effect on high blood pressure and high glucose, though were mediated by mental health, social and behavioural risk factors. Conclusions Interventions to improve the management of risk factors for CVD need to address social risk factors and be sensitive to the needs of population sub-groups that may require additional support to access health services. An integration of social and health services may be required to achieve this.

Abstract:Purpose: To assess the awareness of diabetic retinopathy among Type 2 diabetes mellitus from central India. Method: A cross-sectional study involving 350 patients with type 2 diabetes mellitus was conducted between August 2022 and January 2023. Participants completed a 21-point questionnaire on diabetic retinopathy awareness and knowledge. Results were expressed in frequencies and percentages. Results: 54.3% (190 participants) had awareness about diabetic retinopathy. Education significantly influenced awareness, with 72% of educated participants showing higher awareness. 66% of urban residents were more aware than 47.1% of rural residents. No significant gender difference was observed. Younger participants demonstrated better awareness (63.9%) than older individuals (17.3%). Conclusion: About 54.2% of the participants were aware of Diabetic Retinopathy (DR). Younger patients, patients from urban areas and the group with a higher level of education were found to be more aware of DR. Effective health education and regular eye screenings are essential to improve awareness about DR, aiding in its prevention and better disease management among individuals with type 2 diabetes.

While younger CAD patients tend to experience lower quality of life, their risk perception profile is less well-characterized.Methods:The Young Heart Study enrolled 60 patients ≤59 years old, with coronary artery disease noted on cardiac catheterization or CT coronary angiogram and at least two uncontrolled risk factors: BMI>25 kg/m2, A1C >7% in patients with diabetes, and >5.6% in those without, systolic blood pressure >130 mmHg, LDL>70 mg/dL or active smoking. Perception of cardiovascular (CV) risk and health was collected via structured questionnaires.Results:Among the 60 patients in the study, the median age (Q1-Q3) was 51 (45-55), 70% were male, 46.7% were white. Participants had hypertension (76.7%), hyperlipidemia (91.7%), diabetes (36.7%), prediabetes (28.3%), obstructive CAD (88.3%), myocardial infarction history (38.3%), family history of premature ASCVD (48.3%) and self-reported depression or anxiety (11.7%). Health perception related questions were answered by 46 of these patients. Gender-specific waist circumference goals were not met in 54.6% and metabolic syndrome was noted in 60%. Most (91.7%) had a BMI greater than 25. Participants rated their CV risk as below average (21.7%), average (30.4%), and above average (47.8%) risk of peers of same age and sex. Only 8.3% identified a need for change to their medication regimen, despite most having biomarkers above targets (52.5% for LDL, 53.3% for BP, and 68.1% for A1c). Diet and physical activities were identified as areas for improvement by 83.3%, and weight by 72.9% of patients. Among smokers, all felt their smoking status could be improved. Most (68.1%) reported they were confident they could control their CV risk, while 21.3% answered neutral and 10.7% disagreed.Conclusion:In this above-average/high-risk population, we detected a gap between the clinician's and patient's CV risk perception. Exploring patient health perception in the clinic may identify areas for education and counseling. Future studies are needed to assess the impact of various interventions on perceived CV risk and on the adoption of cardio-protective behaviors.

Abstract:BACKGROUND: World Health Organization/International Society of Hypertension (WHO/ISH) charts have been employed to predict the risk of cardiovascular outcome in heterogeneous settings. The aim of this research is to assess the prevalence of Cardiovascular Disease (CVD) risk factors and to estimate the cardiovascular risk among adults aged >40 years, utilizing the risk charts alone, and by the addition of other parameters.METHODS: A cross-sectional study was performed in two of the villages availing health services of a medical college. Overall 570 subjects completed the assessment. The desired information was obtained using a pre-tested questionnaire and participants were also subjected to anthropometric measurements and laboratory investigations. The WHO/ISH risk prediction charts for the South-East Asian region was used to assess the cardiovascular risk among the study participants.RESULTS: The study covered 570 adults aged above 40 years. The mean age of the subjects was 54.2 (±11.1) years and 53.3% subjects were women. Seventeen percent of the participants had moderate to high risk for the occurrence of cardiovascular events by using WHO/ISH risk prediction charts. In addition, CVD risk factors like smoking, alcohol, low High-Density Lipoprotein (HDL) cholesterol were found in 32%, 53%, 56.3%, and 61.5% study participants, respectively.CONCLUSION: Categorizing people as low (<10%)/moderate (10%-20%)/high (>20%) risk is one of the crucial steps to mitigate the magnitude of cardiovascular fatal/non-fatal outcome. This cross-sectional study indicates that there is a high burden of CVD risk in the rural Pondicherry as assessed by WHO/ISH risk prediction charts. Use of WHO/ISH charts is easy and inexpensive screening tool in predicting the cardiovascular event.

Abstract:Background: The single greatest killer in both females and males globally is cardiovascular disease. Coronary artery disease, commonly known as ischemic heart disease, is described as “impairment of heart function induced by obstructive alterations in the coronary circulation to the heart, caused by inadequate blood flow to the heart relative to its needs.” There are 4.5 million persons in India who suffer from coronary artery disease. Objective: In this study planned to determine the patient’s perception regarding triple vessel disease and its management and to associate the demographic variable with the patient’s perception regarding triple vessel disease and its management. Methodology: It is a descriptive research design used in this research study conducted on patients are having triple vessel disease. The purposive sampling technique is used to select samples for analysis. This research study included 45 patients of a selected hospital in Wardha district. Patients must select according to requirements for inclusion and exclusion. For this study, patients who had triple vessel disease would mainly take. Expected Results: The patients are having good perception or may be having bad perception. Conclusion: The conclusion will be drawn from the results. The patient’s will be having good or bad perception regarding triple vessel disease and its management.
Cardiovascular disease risk perception among community-dwelling adults in southwest Nigeria: A mixed-method study

Abstract:The rising prevalence of cardiovascular diseases (CVD) remains a global concern. In Nigeria, the current prevalence of CVD was 76.11% with its attendance burden. The CVD risk perception of individuals is a precursor to the desired lifestyle modification necessary for CVD prevention and management. This study assessed the CVD risk perception and sociodemographic determinants among rural and urban dwellers in southwest Nigeria. The study employed a convergent parallel mixed-methods design involving concurrent data collection. The participants' CVD risk perception was obtained using the Perception of Risk of Heart Disease Scale (quantitative data) and a validated focus group discussion (FGD) guide (qualitative data). Quantitative analysis was completed using descriptive statistics, Phi, Cramer's V, and multivariate linear regression, while the FGD was thematically analysed. The quantitative study involved 1,493 participants (62.4% women) with a mean age of 46.90±15.65 years, while the FGD involved 53 participants (52.8% women) with a mean age of 50.10±13.5 years. Over a quarter (28%) of the participants had a poor CVD risk perception; the mean score was 44.40±8.07. Rural residents had a significantly poorer CVD risk perception than their urban counterparts (Mean difference = -3.16, p <0.001). Having tertiary education (β = 0.100, p < 0.001), living in urban areas (β = 0.174, p <0.001), and living in Lagos (β = 0.074, p = 0.013) and in Oyo, other than Ogun state (β = -0.156, p <0.001) significantly predicted having a good perception of CVD risk. FGD produced three themes: knowledge about CVD, CVD risk factors, and CVD prevention. Participants had a fair understanding of the causes and prevention of CVD. Yet, a substantial portion underestimated their own risk of developing CVD, particularly rural dwellers and people with lower education. More public health education is required to improve the CVD risk perception in southwestern Nigeria.

Abstract:Hypertension is an important health problem in India. The emergence of hypertension and other cardiovascular diseases are strongly related to various risk factors. Knowledge about hypertension and related risk factors is often stressed on their utility in prevention and management of the disease. Still, there is a poor understanding about associated behavioural risk factors of hypertension and importance of knowledge in adopting health-promoting behaviours and controlling hypertension among rural areas of India. This study aimed at assessing the association of behavioural risk factors with hypertension knowledge and hypertension among rural population. The present study focused on a south-western state of India from which a taluk with one of the lowest socioeconomic ratings was selected. A total of 263 participants were selected by using a multistage random sampling technique. Data were collected by in-person interview using behavioural risk factors questionnaire, hypertension knowledge questionnaire, and physical measurement. Data were analysed using descriptive statistics, chi square, Pearson correlation and binary logistic regression. Findings revealed that there is no significant relationship between risk factors index and knowledge of hypertension. It was also observed that factors such as smoking (OR = 0.29; CI: 090–0.961), fruit and vegetable consumption (OR = 1.32; CI: 1.01–1.74), body mass index (OR = 1.85; CI: 1.21–2.84), and age group (OR = 1.55; CI: 1.14–2.11) were significantly associated with the odds of hypertension. The factors such as smokeless tobacco use, alcohol consumption, physical activity, gender, education, and occupation were not associated with the odds of hypertension. Future research should focus on bringing down the associated risk factors to prevent and control hypertension.

Abstract:BACKGROUND: Knowledge and attitude are significant factors impinging on whether individuals seek healthcare service. This flows on to impact public health knowledge of prevalence of diseases, and in turn, the practice of preventive medicine. As part of the international research collaboration agenda for Prediabetes and Cardiovascular Complications Study, a preliminary survey of one of the Ndokwa communities of Nigeria has been carried out.AIM: This study was to understand the baseline knowledge, attitudes and practices of a rural community in regards to cardiovascular diseases, and behavior toward risk management.MATERIALS AND METHODS: Seventy-four volunteer participants were recruited, after public lectures, through secondary school and churches in the community. The survey was done using questionnaire. The knowledge component comprised questions about educational and personal health opinion. The attitude and practice components comprised questions about exercises and visiting healthcare facilities. Occupational backgrounds were also asked.RESULTS: It is observed that majority of the community dwellers have (1) completed at least secondary education, (2) never attended a health check-up; and (3) do not engage in physical activity in the context of exercise. Twenty of the participants indicated not being in good health, of which only 35% have attended medical check-up for their ailment. Many of those who are yet to seek healthcare service cite affordability as their reason. With specific regards to diabetes and cardiovascular risk, over 71% of the survey participants are yet to do any blood sugar and/or lipid profile tests.CONCLUSION: This preliminary survey indicates that although the majority of respondents have secondary education and therefore are relatively literate, there is a gap between their knowledge of ill-health versus attitude and practice toward prevention; especially cardiovascular and diabetes diseases.

Abstract:Objective: Ischemic heart disease is mostly seen among all Cardiovascular diseases and is responsible for more than 80% of cardiovascular disease deaths. This study has been developed to design and to validate a Knowledge, attitude and practice (KAP) Questionnaire in order to assess the knowledge, attitude and practice of ischemic heart disease patients towards their medications. Study Design: Prospective observational study Methods: The study was conducted for duration of six months among 336 patients. The study subjects taken were above 18 years of age, who was diagnosed with Coronary Artery Disease by a Consultant Cardiologist in Cardiology Department. Practice questionnaires and all the details were explained by the investigator before filling the KAP forms. The data was tabulated and statistically analysed by SPSS software. Results: 336 patients including both males and females were enrolled in the study. Majority of the patient were having good knowledge about the disease conditions that is 50% of the patients were aware about the ischemic heart disease. 326 (97%) of the 336 research participants were aware of the signs and symptoms of ischemic heart disease. Majority of the patient have an attitude towards the adverse conditions related to the disease i.e. 91.4% were aware of consulting a physician when they came across a side effect. 85.45% of the study subjects think that non-compliance to medication can lead to worsening of their condition. 98.2% of the study subjects are willing to take the treatment recommended by their doctor. Considering the practice, 81.5% the study subjects were ready to follow all the instructions given by the doctor. Conclusion: Findings from this study provide a better understanding of “knowledge, attitude and practice” in Ischemic heart disease patients towards their medications. Estimating the knowledge, attitude and practices of the community has sufficiently provided crucial baseline information for the implementation of primary and secondary preventive programs.

Abstract:Abstract Background/Aims Good knowledge, Attitude, and Practice (KAP) of diabetes influence its control and complications. We examined the KAP of diabetes in patients with sight-threatening diabetic retinopathy (STDR) and non-sight-threatening diabetic retinopathy (NSTDR) attending two different referral hospitals in India. Methods 400 consecutive patients (mean age = 58.5 years ± 10.3) with diabetic retinopathy attending retina referral clinics in Chennai (private) and Darjeeling (public) were recruited. A validated questionnaire on diabetic KAP was administered in English or the local language. Data were analysed using an established scalar-scoring method in which a score of 1 was assigned to the correct answer/healthy lifestyle and 0 to an incorrect answer/unhealthy lifestyle/practice. Clinical data included fasting blood sugar, blood pressure, retinopathy, and visual acuity. Retinopathy was graded as STDR/NSTDR from retinal images using Early Treatment of Diabetic Retinopathy Study criteria. Results Usable data from 383 participants (95.8%) were analysed. Of these, 83 (21.7%) had STDR, and 300 (78.3%) had NSTDR. The NSTDR group reported a significantly lower total KAP score (mean rank = 183.4) compared to the STDR group (mean rank = 233.1), z = -3.0, p < 0.001. A significantly greater percentage in the NSTDR group reported to being unaware that diabetes could affect eyes, did not know about possible treatment for DR, and checked their blood sugar less frequently than once a month. Conclusion Patients who had not developed STDR had poorer KAP about diabetes and diabetes-related eye diseases. This is an important issue to address as the risk of their progressing to STDR is high unless appropriate steps to improve their knowledge/awareness and lifestyle practice are introduced early.

Abstract:The increasing prevalence rate of diabetes mellitus (DM) and the associated long-term complications warrant a need to improve awareness of DM-related complications in the Indian population. Our questionnaire-based pan-India study (April 2021-March 2022) aims to capture the observations of healthcare practitioners (HCPs) on the prevailing level of knowledge and awareness regarding diabetes among their patients. We refer to this as the 90:90:90 program. It aims to achieve 90% awareness, 90% screening and detection of diabetes and prediabetes, and 90% achievement of effective treatment and control. A structured questionnaire was circulated to 1800 HCPs using Google Forms (Google, Mountain View, CA) and Zoom poll questions (Zoom Video Communications, Inc., San Jose, CA) during 125 symposiums. About half (48.6%) of the HCPs observe that less than 40% of their patients are aware of the risk factors of diabetes, and less than 60% of the patients were aware of its cardiovascular complications. About 92-95% of the HCPs recommend screening for diabetes in adults over 30 years of age and suggest the inclusion of a blood glucose estimate as a fifth vital to be tested during doctor visits. Less than 40% of patients fail to achieve the treatment goal, possibly due to lack of adherence, access to medicines, and financial constraints. Therefore, spreading awareness of DM complications and early screening for DM among adults (>30 years) could help achieve better management and treatment outcomes.
