The notion that behavior is greatly influenced by social context in which people lead their lives is demanding a new approach to health and health system policy development. The key questions they elaborate are: How to make health system aware of these changed health needs and How to make health system more capable to respond to these unmet needs? Authors are discussing two alternatives--Health system reform and health system reorientation advocating the second one as the way to go.

Abstract:Study objectives: Sleep restriction therapy (SRT) is one of the most effective treatments for insomnia. Restriction of time in bed (TIB) is assumed to be the central mechanism through which SRT improves sleep consolidation and reduces insomnia symptoms. This hypothesis has never been directly tested. We designed a randomized, controlled, dismantling trial in order to isolate the role of TIB restriction in driving both clinical and polysomnographic sleep outcomes. Methods: Participants aged 25-55 who met diagnostic criteria for insomnia disorder were block-randomized (1:1) to 4 weeks of SRT or time in bed regularization (TBR), a treatment that involves the prescription of a regular but not reduced TIB. The primary outcome was assessed with the insomnia severity index (ISI) at baseline, 4-, and 12-weeks post-randomization. Secondary outcomes included sleep continuity (assessed via polysomnography, actigraphy, and diary) and quality of life. We performed intention-to-treat analyses using linear mixed models. Results: Fifty-six participants (39 females, mean age = 40.78 ± 9.08) were assigned to SRT (n = 27) or TBR (n = 29). Daily monitoring of sleep via diaries and actigraphy confirmed large group differences in TIB (d range = 1.63-1.98). At 4-weeks post-randomization, the adjusted mean difference for the ISI was -4.49 (d = -1.40) and -4.35 at 12 weeks (d = -1.36), indicating that the SRT group reported reduced insomnia severity relative to TBR. Robust treatment effects in favor of SRT were also found for objective and self-reported sleep continuity variables (d range = 0.40-0.92) and sleep-related quality of life (d = 1.29). Conclusions: For the first time, we demonstrate that TIB restriction is superior to the regularization of TIB on its own. Our results underscore the centrality of the restriction component in reducing insomnia symptoms and consolidating sleep.

Abstract:We developed a cross-over study design with two interventions in randomized order to compare the effects of sleep fragmentation and partial sleep restriction on cardiac autonomic tone. Twenty male subjects (40.6 ± 7.5 years old) underwent overnight polysomnography during 2 weeks, each week containing one undisturbed baseline night, one intervention night (either sleep restriction with 5 h of sleep or sleep fragmentation with awakening every hour) and two undisturbed recovery nights. Parameters of heart rate variability (HRV) were used to assess cardiac autonomic modulation during the nights. Sleep restriction showed significant higher heart rate ( p = 0.018) and lower HRV-pNN50 ( p = 0.012) during sleep stage N1 and lower HRV-SDNN ( p = 0.009) during wakefulness compared to the respective baseline. For HR and SDNN there were recovery effects. There was no significant difference comparing fragmentation night and its baseline. Comparing both intervention nights, sleep restriction had lower HRV high frequency (HF) components in stage N1 ( p = 0.018) and stage N2 ( p = 0.012), lower HRV low frequency (LF) ( p = 0.007) regarding the entire night and lower SDNN ( p = 0.033) during WASO during sleep. Sleep restriction increases sympathetic tone and decreases vagal tone during night causing increased autonomic stress, while fragmented sleep does not affect cardiac autonomic parameters in our sample.

Abstract:BackgroundShortened rapid eye movement (REM) sleep latency and increased REM sleep amount are presumed biological markers of depression. These sleep alterations are also observable in several animal models of depression as well as during the rebound sleep after selective REM sleep deprivation (RD). Furthermore, REM sleep fragmentation is typically associated with stress procedures and anxiety. The selective serotonin reuptake inhibitor (SSRI) antidepressants reduce REM sleep time and increase REM latency after acute dosing in normal condition and even during REM rebound following RD. However, their therapeutic outcome evolves only after weeks of treatment, and the effects of chronic treatment in REM-deprived animals have not been studied yet.ResultsChronic escitalopram- (10 mg/kg/day, osmotic minipump for 24 days) or vehicle-treated rats were subjected to a 3-day-long RD on day 21 using the flower pot procedure or kept in home cage. On day 24, fronto-parietal electroencephalogram, electromyogram and motility were recorded in the first 2 h of the passive phase. The observed sleep patterns were characterized applying standard sleep metrics, by modelling the transitions between sleep phases using Markov chains and by spectral analysis.Based on Markov chain analysis, chronic escitalopram treatment attenuated the REM sleep fragmentation [accelerated transition rates between REM and non-REM (NREM) stages, decreased REM sleep residence time between two transitions] during the rebound sleep. Additionally, the antidepressant avoided the frequent awakenings during the first 30 min of recovery period. The spectral analysis showed that the SSRI prevented the RD-caused elevation in theta (5-9 hz) power during slow-wave sleep. Conversely, based on the aggregate sleep metrics, escitalopram had only moderate effects and it did not significantly attenuate the REM rebound after RD.ConclusionIn conclusion, chronic SSRI treatment is capable of reducing several effects on sleep which might be the consequence of the sub-chronic stress caused by the flower pot method. These data might support the antidepressant activity of SSRIs, and may allude that investigating the rebound period following the flower pot protocol could be useful to detect antidepressant drug response. Markov analysis is a suitable method to study the sleep pattern.

Abstract:The overlap of classical resonances has been particularly successful in predicting ionization thresholds for hydrogen atoms in strong microwave fields, although not for low scaled frequencies. In this paper, we explicitly relate quantum mechanically computed matrix elements to the classical Fourier amplitudes used in the overlap criterion. The resulting twist to this demonstration of classical-quantum correspondence is that quantum matrix elements can be used to improve predictions of classical-resonance overlap for low scaled frequencies. Conversely, the classical amplitudes are shown to contain delicate cancellations in the quantum matrix elements. In the course of this demonstration, we also introduce a compact numerical method for evaluating high-order multiphoton matrix elements.

Abstract:Summary Older adults with poor sleep tend to show a discrepancy between objective and self‐reported sleep parameters, which can trigger a vicious cycle that worsens their sleep complaints. Cognitive‐behavioural therapy can reduce this discrepancy, but alternative behavioural therapies remain untested. The present exploratory study aimed to investigate the effects of mindfulness‐based therapy for insomnia (MBTI) on reducing sleep discrepancies in comparison with a sleep hygiene, education, and exercise programme (SHEEP). Older adults were randomly allocated into the mindfulness‐based therapy for insomnia group (n = 55) or the sleep hygiene, education, and exercise programme group (n = 58). Subjective and objective sleep parameters were measured using sleep diaries, polysomnography (PSG), and actigraphy. Sleep discrepancies were calculated using the Bland–Altman method for sleep onset latency (SOL) and wake after sleep onset (WASO). Additionally, correlations between the change in sleep discrepancies and the change in subjective sleep quality and trait mindfulness were measured within each group. Sleep onset latency discrepancy measured by polysomnography and actigraphy decreased significantly after the MBTI and SHEEP interventions. In contrast, there was no significant change in wake after sleep onset discrepancy in either group. The change in sleep onset latency discrepancy was correlated with the change in insomnia symptoms and objectively measured trait mindfulness. Mindfulness‐based therapy for insomnia was effective in reducing sleep onset latency discrepancies and improving sleep perception in older adults with sleep disturbances, which in turn drove an improvement in sleep quality and insomnia symptoms. Increases in trait mindfulness may have been an important mechanism in improving sleep perception in the mindfulness‐based therapy for insomnia group.

Abstract:Abstract Introduction Time-restricted eating (TRE) is an emerging dietary intervention for weight loss. Sleep can be influenced by timing of food intake. Nevertheless, the impact of TRE on sleep patterns remains under-explored. The Time-Restricted Intake of Meals (TRIM) study was a parallel-arm, controlled feeding trial in 41 adults with obesity and prediabetes or untreated diabetes, randomized to TRE (8am-6pm) or usual eating pattern (UEP; 8am-12am) for 12 weeks, where the primary outcome was weight change. We hypothesized that TRE would cause changes in sleep/wake patterns. Methods This was a post-hoc analysis of TRIM using wrist actigraphy data obtained at baseline and week-12. We applied an objective process to determine sleep parameters after sleep estimation (Cole-Kripke) from actigraphy data. We analyzed 6 days/nights of recordings for each participant at each timepoint. Outcomes were sleep duration defined by mean total sleep time per 24-hour period (TST), sleep timing measured by mid-sleep time, sleep onset/offset, and sleep continuity. We used paired t-tests or Wilcoxon signed rank tests to compare data between baseline and week-12 within intervention arms and Mann-Whitney U tests or Wilcoxon signed rank tests to compare changes between intervention arms. Results 38 participants (20 UEP; 18 TRE) with adequate actigraphy data (mean age 60 years, 92% female, and 92% Black) were analyzed. Baseline TST (mean□SD) was 394□90 minutes and 386□81 minutes for UEP and TRE, respectively. Week-12 TST was 397□92 minutes and 444□95 minutes for UEP and TRE, respectively. Compared to UEP, TRE increased TST by 55 minutes (p=0.03). TRE shifted mid-sleep time to 44 minutes earlier, from 3:24am to 2:40am (p=0.01), while UEP maintained the same mid-sleep time at 3:15am. Sleep onset shifted from a median of 12:22am to 11:52pm in TRE (p=0.03) while it remained stable in UEP (p=0.97). There were no differences in sleep offset and sleep continuity within and between intervention arms. Conclusion Sleep behaviors were different by treatment arm with increased total sleep time and earlier sleep onset in TRE participants. Abstract:Summary Dissemination of digital cognitive behavioural therapy is a promising approach for treating insomnia in the broad population. Current evidence supports the effectiveness of the digital format, but clinical findings are often limited by the choice of control group and lack of in‐depth therapeutic measures. This study was designed to investigate the specific effects of digital cognitive behavioural therapy in comparison to a self‐monitoring application. Participants meeting criteria for insomnia were randomly allocated (1:1) to 8 weeks of digital cognitive behavioural therapy or 8 weeks of digital sleep monitoring (control application). The primary outcome, insomnia severity, was assessed at baseline, 8‐ and 16‐weeks post‐randomisation. Secondary outcomes included the assessment of sleep via application‐integrated sleep diaries and actigraphy. Linear‐mixed models were fitted to assess between‐group differences. Fifty‐six participants (48 females, mean age: M = 45.55 ± 13.70 years) were randomised to either digital cognitive behavioural therapy (n = 29) or digital sleep monitoring (n = 27). At 8‐ and 16‐weeks post‐randomisation, large treatment effects (d = 0.87–1.08) indicated robust reductions (−3.70 and −2.97, respectively; p ≤ 0.003) in insomnia severity in the digital cognitive behavioural therapy arm, relative to digital sleep monitoring. Treatment effects in favour of digital cognitive behavioural therapy were also found for self‐reported and actigraphy‐derived sleep continuity variables, indicating that sleep improved throughout the 8‐week intervention period. Our study reinforces the role of digital cognitive behavioural therapy in achieving clinical improvements for patients with insomnia, affirming previous findings and supporting the specific effects of cognitive behavioural therapy.

Abstract:Objectives: To evaluate brain metabolites in objective insomnia subtypes defined from polysomnography (PSG): insomnia with short sleep duration (I-SSD) and insomnia with normal sleep duration (I-NSD), relative to good sleeping controls (GSCs).Methods: PSG empirically grouped insomnia patients into I-SSD (n = 12: mean [SD] total sleep time [TST] = 294.7 minutes [30.5]) or I-NSD (n = 19: TST = 394.4 minutes [34.9]). 1H magnetic resonance spectroscopy (MRS) acquired in the left occipital cortex (LOCC), left prefrontal cortex, and anterior cingulate cortex was used to determine levels of creatine, aspartate, glutamate, and glutamine (referenced to water). Glutathione, glycerophosphocholine, lactate, myoinositol, and N-acetylaspartate measurements were also obtained. Sixteen GSCs were included for comparison. Multivariate analysis of variance was used to evaluate differences in creatine, aspartate, glutamate, and glutamine.Results: Aspartate and glutamine concentrations were reduced in the LOCC in I-SSD compared with I-NSD (both p < .05, d = .80-.99). Creatine displayed a nonsignificant mean reduction in I-SSD compared with I-NSD (p = .05, d = .58). Glutamine was reduced in I-SSD compared with controls (p < .05, d = .93). There were no differences in metabolites between all (I-SSD and I-NSD) insomnia patients and controls. The effect of sleep continuity disruption on multimodal emotion processing and regulation: a laboratory‐based, randomised, controlled experiment in good sleepers

Abstract:Summary Previous research shows that experimental sleep deprivation alters emotion processing, suggesting a potential mechanism linking sleep disruption to mental ill‐health. Extending previous work, we experimentally disrupted sleep continuity in good sleepers and assessed next‐day emotion processing and regulation using tasks with established sensitivity to depression. In a laboratory‐based study, 51 good sleepers (37 female; mean [SD] age 24 [3.63] years), were randomised to 1 night of uninterrupted sleep (n = 24) or sleep continuity disruption (n = 27). We assessed emotion perception, attention, and memory the following day. Participants also completed an emotion regulation task and measures of self‐reported affect, anxiety, sleepiness, overnight declarative memory consolidation, and psychomotor vigilance. Confirming the effects of the manipulation, sleep continuity disruption led to a marked decrease in polysomnography‐defined total sleep time (229.98 versus 434.57 min), increased wake‐time after sleep onset (260.66 versus 23.84 min), and increased sleepiness (d = 0.81). Sleep continuity disruption led to increased anxiety (d = 0.68), decreased positive affect (d = −0.62), reduced overnight declarative memory consolidation (d = −1.08), and reduced psychomotor vigilance (longer reaction times [d = 0.64] and more lapses [d = 0.74]), relative to control. However, contrary to our hypotheses, experimental sleep disruption had no effect on perception of, or bias for, emotional facial expressions, emotional memory for words, or emotion regulation following worry induction. In conclusion, 1 night of sleep continuity disruption had no appreciable effect on objective measures of emotion processing or emotion regulation in response to worry induction, despite clear effects on memory consolidation, vigilance, and self‐reported affect and anxiety.

Abstract:Background/Objective: Some older adults with insomnia experience sleep discrepancy, often characterized by greater subjective sleep difficulties and shorter subjective sleep duration than the estimates derived from objective measures. The present study examined whether a brief behavioral therapy for insomnia (BBTi) is efficacious for reducing sleep discrepancy in older adults.Methods: This study is a secondary analysis of a randomized controlled trial of BBTi for community dwelling older adults with chronic insomnia (N = 62). Thirty-two participants received BBTi, delivered in four individual face-to-face sessions. Thirty received the self-monitoring control (SMC). They all completed daily sleep diaries and wore an actigraph from baseline to posttreatment, and for 2 weeks at 3-month follow-up. Sleep discrepancy was calculated by subtracting diary from actigraphy estimates of sleep onset latency (SOL), wake after sleep onset (WASO), and total sleep time (TST). Mixed modeling was used to analyze data. SOL discrepancy decreased significantly in BBTi participants compared to SMC participants. The decreases in SOL discrepancy were explained by changes in diary-assessed SOL and subjective sleep quality but not changes in actigraphy-assessed SOL. Although WASO discrepancy and TST discrepancy decreased from baseline to posttreatment and follow-up, the Time by Group interaction effects were not significant indicating that BBTi participants did not experience greater reductions in WASO discrepancy and TST discrepancy than SMC participants. In conclusion, BBTi is efficacious for reducing SOL discrepancy in older adults with chronic insomnia.

Abstract:Introduction Insomnia is a prevalent sleep disorder that negatively affects quality of life. Multicomponent cognitive-behavioural therapy (CBT) is the recommended treatment but access remains limited, particularly in primary care. Sleep restriction therapy (SRT) is one of the principal active components of CBT and could be delivered by generalist staff in primary care. The aim of this randomised controlled trial is to establish whether nurse-delivered SRT for insomnia disorder is clinically and cost-effective compared with sleep hygiene advice. Methods and analysis In the HABIT (Health-professional Administered Brief Insomnia Therapy) trial, 588 participants meeting criteria for insomnia disorder will be recruited from primary care in England and randomised (1:1) to either nurse-delivered SRT (plus sleep hygiene booklet) or sleep hygiene booklet on its own. SRT will be delivered over 4 weekly sessions; total therapy time is approximately 1 hour. Outcomes will be collected at baseline, 3, 6 and 12 months post-randomisation. The primary outcome is self-reported insomnia severity using the Insomnia Severity Index at 6 months. Secondary outcomes include health-related and sleep-related quality of life, depressive symptoms, use of prescribed sleep medication, diary and actigraphy-recorded sleep parameters, and work productivity. Analyses will be intention-to-treat. Moderation and mediation analyses will be conducted and a cost-utility analysis and process evaluation will be performed. Ethics and dissemination Ethical approval was granted by the Yorkshire and the Humber - Bradford Leeds Research Ethics Committee (reference: 18/YH/0153). We will publish our primary findings in high-impact, peer-reviewed journals. There will be further outputs in relation to process evaluation and secondary analyses focussed on moderation and mediation. Trial results could make the case for the introduction of nurse-delivered sleep therapy in primary care, increasing access to evidence-based treatment for people with insomnia disorder. In the primary publication, N = 238 participants meeting criteria for the 5th edition of Diagnostic and Statistical Manual of Mental Disorders chronic insomnia disorder were randomly assigned to either 8 weeks of digital cognitive behavioural therapy for insomnia + treatment‐as‐usual, or waitlist + treatment‐as‐usual. To determine the clinical effects of digital cognitive behavioural therapy for insomnia in populations with comorbid anxiety and depression symptoms, this secondary analysis focused on two subgroups: (1) participants with high initial depressive symptoms; and (2) participants with high initial anxiety symptoms. Symptoms of insomnia, depression and anxiety as primary outcome measures were obtained at baseline, 8 weeks post‐randomization and, in the intervention group only, at 6‐ and 12‐months follow‐up. At 8 weeks post‐randomization, the use of digital cognitive behavioural therapy for insomnia in both subgroups was associated with large reductions in insomnia severity in comparison to control (depression subgroup: d = 2.37; anxiety subgroup: d = 2.13). Between‐group treatment effects were also observed for symptoms of depression in the depression subgroup (d = 1.59), and for symptoms of anxiety in the anxiety subgroup (d = 1.28). Within‐group effects were stable over time (d = 0.64–1.63). This secondary analysis shows that digital cognitive behavioural therapy for insomnia reduces insomnia and comorbid symptoms in participants with high initial symptoms of either depression or anxiety with sustained long‐term effects.

Abstract:Animal studies show that insufficient silencing of the locus coeruleus (LC) during REM sleep impairs sleep-related brain plasticity. Restless REM sleep, a characteristic of several psychiatric disorders, likely reflects insufficient LC silencing. We investigated whether endogenous REM sleep interruptions interfere with overnight reorganization of limbic circuits in human volunteers with a wide range of insomnia severity, from no insomnia complaints to fulfilling community-sample criteria for insomnia disorder. We induced a self-conscious emotion during two functional MRI sessions and recorded sleep EEG in between. Amygdala reactivity decreased overnight in proportion to the total duration of consolidated REM sleep. Restless REM sleep, in contrast, impeded overnight amygdala adaptation. Using targeted memory reactivation with odors tagged to the self-conscious emotional stimulus, we could experimentally enhance both the favorable effect of consolidated REM sleep and the unfavorable effect of restless REM sleep. The findings reveal a maladaptive type of sleep, providing a target for interventions in mental disorders characterized by restless REM sleep.

Abstract:Abstract Introduction “Sleep to forget and sleep to remember” model postulates that the content of emotional memory is strengthened during sleep while the arousal component is attenuated. Experimental sleep deprivation studies in healthy individuals have supported the model, but it is not known if sleep in individuals with insomnia, a condition of chronic poor sleep, provides the same effect on emotional memory. Methods Individuals reporting insomnia and good sleepers (n = 76 and n = 83 respectively; 83% female, mean age of 43.2 years) completed a picture-rating task to assess a) emotional valence ratings pre and post sleep and b) recognition accuracy post-sleep. In addition, an autobiographical memory task was completed during the day to assess a) the number of recalled memories in a 2-minute period for good and bad past events respectively, and b) emotional intensity ratings of the retrieved memories at the time the event occurred and how they feel now. Results Wilcoxon rank-sum test was used to assess group differences. Sleep diary variables on the experimental night showed that individuals with insomnia experienced a significantly more disrupted sleep than good sleepers (SOL: r = 0.537; WASO: r = 0.620; TST: r = 0.669). In the picture-rating task, there was no significant group difference for recognition accuracy (neutral stimuli: r = 0.016; negative stimuli: r = 0.018) or change in emotional valence pre-to-post sleep (neutral stimuli: r = 0.003; negative stimuli: r = 0.008). In the autobiographical memory task, individuals with insomnia remembered significantly fewer good days (r = 0.245) and rated their bad days at the time of its occurrence as significantly worse than good sleepers (r = 0.235). There was no significant group difference in the extent to which emotional intensity faded more for bad days than good days (r = 0.113). Conclusion We have shown that autobiographical memory is altered in insomnia which may have important influence on mental health. Our findings also suggest that overnight memory consolidation for negative stimuli is not affected in insomnia versus good sleepers. Abstract:Uricase (UC) was conjugated with dextran and polyethylene glycol and their biochemical and biopharmaceutical properties were studied. UC-dextran conjugates (UC-D) synthesized by four methods, periodate oxidation, cyanogen bromide, carbodiimide and cyanuric chloride largely retained the UC enzymatic activity depending on the extent to which they modified amino groups. The periodate oxidation method seemed best because it gave a conjugate with high yield and satisfactory activity retention. The conjugate of UC with activated polyethylene glycol (UC-PEG2) was also obtained with high yield but the remaining activity was somewhat lower than those of dextran conjugates at the same modification extent. UC-D and UC-PEG2 showed sustained enzymatic activity in plasma after intravenous injection to rats. The advantage of chemical modification of proteins, especially with dextran, by the periodate oxidation method for preparation of a protein-delivery system was thus suggested.

Abstract:Abstract Introduction Sleep fragmentation by arousals and sleep-disordered breathing (SDB) is known to increase with age and differ by gender, although the magnitude of these effects is largely unknown. Population studies allow large collection of continuous nightly data to explore these effects. Methods Sleep fragmentation index (SFI, arousals/hour) and SDB were analyzed by a commercially available home monitoring device (Sleeptracker-AI Monitor, Fullpower Technologies Inc., California, USA) that passively monitors sleep using piezo-electric sensors. Validated sleep/respiratory parameters were derived from device data. De-identified data were analyzed, following review and exemption of the study (#57681) from Stanford University IRB. SFI was assessed using an estimated arousal index, estimating individual arousal events from passive sensor data, with validation against PSG. Data 8/16/2023-12/15/2023 were reviewed. Individuals with birth year and gender provided and age 20-79 years in 2023 were included in the analytic dataset. Results 117,846 individuals (64,049 men, 48.2±12.6 years; 53,797 women, 48.3±12.6 years) with 10,090,778 recorded nights met the inclusion criteria. For AHI< 5, the SFI mean±SE for women and men were 13.6±0.16 and 12.6±0.13 at 20-24 years, respectively, and increased with age to 15.0±0.26 and 16.0±0.24 for 75-79 years. For AHI< 5, men showed a significantly greater SFI during REM sleep than women across all ages above 24 years, but not during NREM sleep. For AHI< 5, SFI significantly increased with age during NREM sleep (women 11.9±0.15 to 14.6±0.17, men 10.8±0.12 to 15.1±0.26), but for REM sleep increased up to 25-34 years (women 18.8±0.12, men 19.6±0.09), but then steadily decreased with age to 75-79 years (women 16.1±0.27, men 18.1±0.24), and a similar trend was found for AHI≥5. For AHI≥5 compared to AHI< 5, SFI was significantly higher by 6-13 arousals/hour across all ages. Conclusion Use of a noninvasive in-home monitoring device enabling continuous nightly collection/analysis of a large sample of sleep/respiratory data revealed differences in sleep fragmentation across age and by gender. Greater sleep fragmentation was observed for men across age during REM sleep, and fragmentation during REM sleep decreased with age after 34 years, which is unexpected and warrants further study given that overall quantities of NREM and REM sleep decrease with age.
