Abstract:Background: This study aimed to explore the impact of occupational activities involving extensive finger movement on the muscular characteristics of the forearms. In particular, the flexor digitorum (FD) muscular thickness and strength asymmetry between right and left hand were compared between musicians and non-musicians. Methods: Ultrasonography was employed to measure the thickness of the FD in each hand, while a validated custom-made device was used to assess the strength of the flexor and extensor digitorum (ED). Initially, muscle differences were estimated by computing the asymmetry index between dominant and non-dominant hands. To assess potential occupational disparities, comparisons of the asymmetry index were conducted between 25 right-handed instrumental musicians and 25 right-handed non-musicians. Results: Musicians exhibited lower asymmetry between dominant and non-dominant hands in both FD thickness and ED strength when compared to non-musicians. This effect was particularly pronounced in musicians playing instruments that extensively use the left-hand fingers (e.g., violinists). Conclusions: Occupational activities, such as playing a musical instrument, can alter forearm muscle mass and strength distribution between dominant and non-dominant hands. This underscores the importance of considering occupational parameters in clinical or experimental interventions and musculoskeletal assessments.
Characteristics, Relationships, and Differences in Muscle Activity and Impact Load Attenuation During Tennis Forehand Stroke with Different Grips

Abstract:In forehand strokes with different grips in tennis, the forearm muscle activities, the distribution and attenuation of the impact loads, and the effects of the muscles on the impact load attenuation exhibited different characteristics. This study aimed to explore these characteristics by analyzing electromyography (EMG) and acceleration data, and comparing the differences between the Eastern and Western grips. Fourteen level II or above tennis players (ten males, aged 22.4 ± 3.6 years; four females, aged 19.8 ± 2.0 years) were recruited and instructed to perform forehand strokes using the Eastern and Western grips, respectively. The EMG of eight forearm muscles and the acceleration data at the ulnar and radial sides of the wrist and elbow were collected. The root mean square (RMS), the peaks of the impact load, the amplitude of impact load attenuation (AC), and the jerk value (Jerk) were calculated. The cross-correlation coefficients and time delays of EMG–EMG, EMG–AC, and EMG–jerk were obtained using the cross-correlation method. The results showed that in the Eastern grip group (group E), the RMS of the flexor carpi ulnaris (FCU) was significantly greater than that in the Western grip group (group W). In group E, the peaks of impact load, AC, and Jerk on the Y axis of the wrist ulnar side were all significantly higher than those in group W. The activity of the extensor digitorum commonis (EDC) had significantly different effects on the amplitude and rate of impact load attenuation at specific locations in different grips, especially at the elbow (p < 0.05). The conclusion indicated that the FCU exhibited higher levels of EMG activity in the Eastern grip. This grip responded to greater impact loads with more substantial and rapid attenuation on the wrist ulnar side. Furthermore, the EDC appeared to contribute more to the amplitude of impact load attenuation in the Western grip and to have a more significant influence on the rate of impact load attenuation in the Eastern grip, especially at the elbow. These results suggest that tennis players and coaches should pay more attention to improving the strength of the EDC and FCU, which can improve sports performance and comfort, as well as prevent sports injuries.

Abstract:Pianists, who attend to the integral relationship of their particular musculoskeletal characteristics to the piano technique at hand, discover an efficient path to technical advancement and, consequently, to injury prevention. Thus, a study of pianist's hand biomechanics in relation to different piano techniques is highly relevant, as hand features may influence various techniques in different ways. This study addressed relationships between pianists' hand biomechanics and the performance of a scale in thirds, as a part of an ongoing series of studies examining relationships between hand biomechanics and performance data of primary techniques. The biomechanics of hand length and width, finger length, hand span, hand and arm weights, and ulnar deviation at the wrist were compared with tempo, articulation, and dynamic voicing (tone balance between two notes of the thirds). Pearson correlation analysis showed a positive association between ulnar deviation and tempo; the other biomechanical features showed no relationships with any of the performance criteria. Qualitative cross-sectional observation of individual profiles showed that experienced pianists perform with a higher degree of synchrony in two-note descent while pianists with organ training background play with a lesser degree of synchrony. All biomechanical features were closely related among one another with one exception: wrist ulnar deviation was not associated with any other biomechanical features; rather, data suggest possible negative associations. This study underscores the importance of wrist mobility in piano skills development. Further research using a complete set of prototype piano techniques and multiple-level pianist-subjects could provide substantive biomechanical information that may be used to develop efficient pedagogy and prevention strategies for playing-related injuries as well as rehabilitation.

Abstract:This study examined wrist motions of instrumentalists who have never experienced a cumulative traumatic disorder. Eighteen student musicians ranging from 18 to 29 years of age, with more than 9 years of playing experience participated in this study using the WristSystem (Greenleaf Medical System). The wrist motions were measured while participants played their instrument for five minutes. The results indicated a different pattern of wrist movements for each instrumentalist. Pianists demonstrated more ulnar deviation for both hands and more flexion for the left hand than any of the other instrumentalists. The average wrist range of motion for all four movements was under 15 for violinists and violists. The right hand of the cellist had more flexion than any other instrumentalists. For both hands, flutists had more wrist extension than any other instrumentalists. This study provided information about wrist motions of musicians as one of the many factors that contribute to cumulative traumatic disorders. However, further research is needed to identify causal factors, and to promote prevention of the injury.

Abstract:BackgroundProduction of a variety of finger-key touches in the piano is essential for expressive musical performance. However, it remains unknown how expert pianists control multi-joint finger and arm movements for manipulating the touch. The present study investigated differences in kinematics and kinetics of the upper-limb movements while expert pianists were depressing a key with two different touches: pressed and struck. The former starts key-depression with the finger-tip contacting the key, whereas the latter involves preparatory arm-lift before striking the key. To determine the effect of individual muscular torque (MUS) as well as non-muscular torques on joint acceleration, we performed a series of inverse and forward dynamics computations.ResultsThe pressed touch showed smaller elbow extension velocity, and larger shoulder and finger flexion velocities during key-depression compared with the struck touch. The former touch also showed smaller elbow extension acceleration directly attributed to the shoulder MUS. In contrast, the shoulder flexion acceleration induced by elbow and wrist MUS was greater for the pressed touch than the struck touch. Towards the goal of producing the target finger-key contact dynamics, the pressed and struck touches effectively took advantage of the distal-to-proximal and proximal-to-distal inter-segmental dynamics, respectively. Furthermore, a psychoacoustic experiment confirmed that a tone elicited by the pressed touch was perceived softer than that by the struck touch.ConclusionThe present findings suggest that manipulation of tone timbre depends on control of inter-segmental dynamics in piano keystroke.

Abstract:Task-specific dystonia is characterized by excessive muscle contractions producing abnormal postures. These structural and functional changes are known to be a result of maladaptive neuroplasticity to prolonged, intense, repetitive movement. While some factors of neuropathology of focal dystonia are known, the etiology of task-induced focal dystonia and effective treatments are unknown. We examined hand biomechanics, neurophysiology, and music performance outcomes in a 53-year-old male pianist with dystonic extension of the left third digit while playing the piano. We aimed to gain information that can help develop a holistic rehabilitation strategy. The pianist led a life with constricted work patterns leaving little time to recharge or relax. He had large hands with a hypermobile structure. Music Instrument Digital Interface (MIDI) data showed the lack of fine digital touch control required for skilled music playing. Functional magnetic resonance imaging (fMRI) demonstrated enhanced activity in contralateral primary sensorimotor cortex, supplementary motor area, and parietal-occipital regions during simulated left-hand scale playing. Intramuscular needle EMG analysis demonstrated hyperactivation of the left extensor digitorum communis (EDC) with a distinct periodic oscillatory pattern during and persisting after playing, which was not as prominent in the unaffected extensor indicis proprius (EIP), thus demonstrating that pianist’s focal dystonia was associated with increased activity in the contralateral motor cortex area and a distinct oscillatory EMG activity. Video recording of retraining sessions using a vibrotactile sensor showed progress. A complete recovery of the affected finger deficit remains elusive after the 8-week retraining.

Medical Problems of Performing Artists

Abstract:Several biomechanical studies have been done with regard to the fingertip, hand, wrist, and upper arm in pianists. However, these studies have generally focused either on one isolated part of the anatomy, or on one specific motion. It is notable that none of the studies conducted to date examines the execution of a technical skill at the piano, such as a scale or arpeggio, in terms of the cooperative work done by all involved parts of the anatomy. Drawing upon established quantitative data, this study took a qualitative approach in describing and analyzing the execution of selected piano technical tasks. The procedure developed for conducting a biomechanical analysis can be readily applied in both medical and pedagogical venues with the goal of preventing injury.

Extracting and Classifying Spatial Muscle Activation Patterns in Forearm Flexor Muscles Using High-Density Electromyogram Recordings

Abstract:The human hand is capable of producing versatile yet precise movements largely owing to the complex neuromuscular systems that control our finger movement. This study seeks to quantify the spatial activation patterns of the forearm flexor muscles during individualized finger flexions. High-density (HD) surface electromyogram (sEMG) signals of forearm flexor muscles were obtained, and individual motor units were decomposed from the sEMG. Both macro-level spatial patterns of EMG activity and micro-level motor unit distributions were used to systematically characterize the forearm flexor activation patterns. Different features capturing the spatial patterns were extracted, and the unique patterns of forearm flexor activation were then quantified using pattern recognition approaches. We found that the forearm flexor spatial activation during the ring finger flexion was mostly distinct from other fingers, whereas the activation patterns of the middle finger were least distinguishable. However, all the different activation patterns can still be classified in high accuracy (94–100%) using pattern recognition. Our findings indicate that the partial overlapping of neural activation can limit accurate identification of specific finger movement based on limited recordings and sEMG features, and that HD sEMG recordings capturing detailed spatial activation patterns at both macro- and micro-levels are needed.
Training Wrist Extensor Function and Detecting Unwanted Movement Strategies in an EMG-controlled Visuomotor Task.

Abstract:Stroke patients often suffer from severe upper limb paresis. Rehabilitation treatment typically targets motor impairments as early as possible, however, muscular contractions, particularly in the wrist and fingers, are often too weak to produce overt movements, making the initial phase of rehabilitation training difficult. Here we propose a new training tool whereby electromyographic (EMG) activity is measured in the wrist extensors and serves as a proxy of voluntary corticomotor drive. We used the Myo armband to develop a proportional EMG controller which allowed volunteers to perform a simple visuomotor task by modulating wrist extensor activity. In this preliminary study six healthy participants practiced the task for one session (144 trials), which resulted in a significant reduction of the average trial time required to move and hold a cursor in different target zones (p < 0.001, ANOVA), indicating skill learning. Additionally, we implemented an EMG based classifier to distinguish between the desired movement strategy and unwanted alternatives. Validation of the classifier indicated that accuracy for detecting rest, wrist extension and unwanted strategies was 92.5 + 6.9% (M + SD) across all participants. When performing the motor task the classification algorithm flagged 4.3 + 3.5% of the trials as 'unwanted strategies', even in healthy subjects. We also report initial feedback from a survey submitted to two chronic stroke patients to inquire about feasibility and acceptance of the general setup by patients.
The plyometric activity as a conditioning to enhance strength and precision of the finger movements in pianists

Abstract:Stability of timing and force production in repetitive movements characterizes skillful motor behaviors such as surgery and playing musical instruments. However, even trained individuals such as musicians undergo further extensive training for the improvement of these skills. Previous studies that investigated the lower extremity movements such as jumping and sprinting demonstrated enhancement of the maximum force and rate of force development immediately after the plyometric exercises. However, it remains unknown whether the plyometric exercises enhance the stability of timing and force production of the dexterous finger movements in trained individuals. Here we address this issue by examining the effects of plyometric exercise specialized for finger movements on piano performance. We compared the training-related changes in the piano-key motion and several physiological features of the finger muscles (e.g., electromyography, rate of force development, and muscle temperature) by well-trained pianists. The conditioning demonstrated a decrease of the variation in timing and velocity of successive keystrokes, along with a concomitant increase in the rate of force development of the four fingers, but not the thumb, although there was no change in the finger muscular activities through the activity. By contrast, such a conditioning effect was not evident following a conventional repetitive piano practice. In addition, a significant increase in the forearm muscle temperature was observed specifically through performing the plyometric exercise with the fingers, implying its association with improved performance. These results indicate effectiveness of the plyometric exercises for improvement of strength, precision, and physiological efficiency of the finger movements even in expert pianists, which implicates that ways of practicing play a key role in enhancing experts' expertise.

Abstract:Grip strength loss in extended and flexed wrist postures has been explained by reduced force-generating capacities of extrinsic finger flexor resulting from non-optimal length, owing to the force-length relationship. Recent works suggested that other muscles, especially wrist extensors, participate in this grip strength loss. The objective of this study was to clarify the role of the force-length relationship in finger force production. 18 participants performed maximal isometric finger force production during pinch grip (Pinch) and four-finger pressing (Press) tasks in four different wrist postures (extended, flexed, neutral, spontaneous). The maximum finger force (MFF), finger and wrist joint angles, as well as activation of four muscles were determined using dynamometry, motion capture, and electromyography. The force and length of the four muscles were estimated from joint angles and muscle activation using a musculoskeletal model. MFF decreased for flexed wrist during Pinch but remained stable across wrist postures during Press. The results suggested that the loss of pinch grip force in deviated wrist posture is partially related to force-length relationship of finger extensors. In opposition, MFF during Press was not influenced by the modulation of muscle capacities but was probably first limited by mechanical and neural factors related to finger interdependence

Abstract:The problem of modelling stresses incurred at the finger joints is critical to the design of durable joint replacements in the hand. The goal of this study was to characterise the forces and stresses at the finger and thumb joints occurring during activities such as typing at a keyboard, playing piano, gripping a pen, carrying a weight and opening a jar. The metacarpal and proximal phalanx were modelled using a COMSOL-based finite element analysis. Analysis of these activities indicates that joint forces in excess of 100 N may be common at the metacarpophalangeal joint (MCP) due to carrying objects such as groceries or while opening jars. The model predicted that stresses in excess of 2 MPa, similar to stresses at the hip, occur at the MCP with the properties of cancellous bone playing a significant role in the magnitude and distribution of stress.

Abstract:This study aimed at investigating the relationship between the trunk and upper limb muscle coordination and mass of the tennis racket during forehand drive. A total of 15 male tennis players performed seven series of ten crosscourt forehand drives, both with their personal racket and six rackets with increased mass ranging from 6 to 16% (step = 2%) of their personal racket mass. The electromyographic (EMG) activity was recorded from nine trunk and upper limb muscles. The onset before impact and EMGrms values of the bursts were individually calculated. Results showed that the ball speed and the muscle activation temporal sequences were similar, whatever the increase in racket mass. Interestingly, in all participants, the activation level of the pectoralis major, latissimus dorsi and biceps brachii decreased when the racket mass increased, while the variations in the anterior deltoid activation level were correlated to the individual personal racket mass. These findings strongly suggest that the study of muscle activity during tennis practice should be considered as a complementary technique to determine a better adequacy of the racket characteristics to those of the player.

Abstract:ABSTRACT Playing a musical instrument necessitates stability in the hands, neck, shoulders, and trunk, involving both isotonic and isometric movements, muscular endurance, and various other physical demands. Musicians, especially those engaged in professional or educational pursuits, often dedicate several hours each day to practicing and performing. Despite the substantial physical challenges associated with musical performance, the realm of performing arts medicine lacks comprehensive research and practical methodologies tailored to adequately address the conditioning requirements of musicians. Musculoskeletal discomfort is prevalent among professional musicians, predominantly affecting the upper body. Instrumental players encounter issues such as musculoskeletal overuse, nerve entrapment, focal dystonia, and other playing-related musculoskeletal disorders, which can persist for extended periods. Consequently, this article aims to present a specialized strength and conditioning regimen tailored for instrumental musicians participating in large music ensembles, specifically symphony orchestras and wind bands. The guide will identify the mechanics inherent to playing these instruments and the prevalent injuries associated with them. Using this information, a strength and conditioning protocol suitable for the application within a symphony orchestra and wind band music classrooms will be presented.

Common Wrist-Extensor Tendon and Pectoralis Muscle Stiffness in Healthy Recreational Tennis Players

Abstract:Context: Imbalances in upper-extremity soft tissue stiffness may play a role in the development of shoulder and elbow musculoskeletal injuries in tennis players. Ultrasound shear wave elastography provides quantifiable and specific data regarding muscle stiffness. The purpose of this study was to compare tendon and muscle stiffness in healthy tennis players to nontennis players. Design: Cross-sectional study. Methods: The shear wave modulus, measured in kilopascals, was obtained for the dominant pectoralis major, pectoralis minor, and common wrist-extensor tendon using 2-dimensional shear wave elastography ultrasound imaging (GE Logiq S8, L9 linear transducer). Independent t test was run to compare age, body mass index, and the activity index score between both groups. Within-day intrarater reliability was assessed using a within-examiner intraclass correlation coefficients (ICC [3, 1]) with 95% confidence intervals. A multivariate general linear model was run to compare the mean differences between the tennis and nontennis players for each of the soft tissues. Results: Twenty-six individuals (13 tennis players and 13 nontennis players) were recruited. Within-day ICCs were very good (ICC > .78 for the pectoralis musculature) and excellent (ICC > .94 for the common wrist extensor). Common extensor tendon stiffness was significantly higher in tennis players compared to nontennis players (mean difference = 114.8 [61.8], confidence interval, -22.8 to 252.5 kPa for the dominant arm [P = .039]). Mean pectoralis major and minor stiffness differences were not significant (P > .214). Conclusions: Common wrist-extensor stiffness in healthy recreational tennis players is higher than those who do not play tennis. Therefore, clinicians may need to facilitate a greater soft tissue stiffness response with resistance training when rehabilitating recreational tennis players as compared to those not playing tennis. Additional normative data on a larger sample of recreational tennis players should be collected.
