Abstract:Aims To explore decision control preferences and decisional conflicts and to analyse their association among the surrogate decision makers in the intensive care unit. Design The study carried out a cross-sectional survey among the surrogates. Methods The participants were 115 surrogate decision makers of critical patients, from August to September 2019. A Chi-squared test and logistic regression were used to assess decision control preferences and decisional conflicts, and Spearman's rank correlation coefficient was employed to examine their association. Results Of the 115 surrogate decision makers, 51.3% preferred a collaborative role, and 63.48% were somewhat unsure about making decisions. Logistic regression analysis identified decision control preferences was associated with surrogates' age, education level, and personality traits, while decisional conflicts was associated with surrogates' age, education level, character, medical expense burden, and Acute Physiology and Chronic Health Evaluation-II score. Cohen's kappa statistics showed a bad concordance of decision-making expectations and actuality, with kappa values of 0.158 (p < .05). Wherein surrogates who experienced discordance between their preferred and actual roles, have relatively higher decisional conflicts. Conclusion This study identified individual differences of surrogate decision makers in decision control preferences and decisional conflicts. These results imply that incorporation of the individual decision preferences and communication styles into care plans is an important first step to develop high quality decision support. Impact This research is a contribution to the limited study on decision control preferences and decisional conflicts among surrogate decision makers of critically ill patients. Moreover based on the investigation of understanding the status and related factors of decision preferences and decisional conflicts set the stage for developing effective decision support interventions.
Family members' perceptions of surrogate decision-making in the intensive care unit: A systematic review

Abstract:Background A better understanding of the perceptions of family members in making surrogate decisions for loved ones during intensive care is needed to inform the development of targeted supportive interventions. Objective To examine and synthesize qualitative data on family members' perceptions of surrogate decision-making in the intensive care unit. Design We conducted a systematic review and qualitative data synthesis. Eligible studies contained family members' quotes about surrogate decision-making experiences and perceptions in adult intensive care units, published in English or Chinese, in a peer-reviewed journal up to February 2022. Data sources included Embase, PubMed, ISI Web of Science, PsychINFO, CINAHL, Biomedical Literature Service System, China National Knowledge Infrastructure (CNKI), Wanfang Data, and VIP Journal. Methods The searches yielded 5974 identified articles, of which 23 studies were included. At least two different reviewers independently assessed the study quality and extracted data into a Microsoft Excel spreadsheet. A thematic synthesis was performed by classifying all text units into one of the broad themes and subsequently analyzed to inductively develop the first-, second-, and third-order themes. Six family members with experience in intensive care unit surrogate decision-making contributed to the analysis. Results The qualitative data synthesis resulted in five major themes. The following key new insights into family members' perceptions of surrogate decision-making in the intensive care unit were obtained: in individual systems, family members suffered from emotional distress and psychological stress; different cognitive styles emerged; some family members reshaped a new order of life in the disruption; in family systems, the family as a whole was closely connected with each other; and in medical systems families perceived asymmetry in relationships with clinicians, many factors influencing trust, the necessity for role-specific mediators and issues with operations and environments not being sufficiently humanized. Conclusion This qualitative synthesis showed that individuals' emotions and cognition underwent complex processes during surrogate decision-making. The family as a whole, with disparate functional states, also faced different processes and outcomes under the crisis situation. At a broader level, the decision-making process reflected society's perceptions of the medical system. Future studies should use these insights to further explore and optimize the many aspects of surrogate decision support measures for families of critically ill patients and include the measurement of outcomes after interventions at multiple layers of the individual, family, and medical systems.Registration number: The protocol was prospectively published on International Prospective Register of Systematic Reviews (PROSPERO)—CRD42022316687. The Surrogate Decision Making Participation Experience of Severe Acute Pancreatitis Patient in ICU: A Phenomenological Research in China

Abstract:Background Patients with severe acute pancreatitis mental state，tissues and organs function severely affected, patients themselves unable to effective participation in decision making, they have to rely on their family members to participate in clinical decision making, when the patient was admitted in ICU. However, it is unclear what factors influence the surrogate involved in decision making during anxiety, depression, post-traumatic stress disorder and other adverse psychological reactions.To describe the experiences of ICU patient’s family members with severe acute pancreatitis during decision making process. Methods Husserl’s phenomenology was applied in this study. This study was conducted in a critical care unit of a China tertiary hospital. They used Semi-structured face-to-face interviews with 12 surrogates of people with severe acute pancreatitis patient to explore the influencing factors of decision participation. This study followed the Colaizzi’s phenomenological method guided. Results/Finding Our sample consisted of 12 participants. Data analysis revealed four main themes: (i) ICU family surrogate cognitive self-role, (ii) ICU family surrogate consider of uncertainty, (iii) ICU family surrogate Communication and consulting, (iv) Decision-making attitude of ICU surrogate. Conclusion A framework of 4 themes of decisional involvement of family members of patients with severe acute pancreatitis in the ICU was constructed, and the interrelationship of the 4 themes was revealed. Psychological and somatic experience of the family as an outward expression of participation in decision-making, self-ability cognition is the internal cause of decision-making participation, consulting experience and considering uncertain factors are the external cause of decision-making participation. It is very important for ICU professionals to consider family members' decision-making participation. The suggestion that the family members of ICU patients participate in the intervention plan was initially formed ICU professionals should respect the values of patients' family members, to improve the disease awareness of the patient's family, to improve the support of the medical care system, and to improve Communication and emotional support skills for health care workers.

Abstract:Background Surrogate decision–makers (SDMs) who make decisions for critically ill patients often face great challenges. Although shared decision‐making has been advocated, its implementation has been less than satisfactory. Aim To explore the experience of the surrogate decision‐making process and determine how SDMs understand the values and preferences of critically ill patients. Study Design A constructivist qualitative study was used. The data were collected using semistructured interviews with four questions conducted between January and February 2023. The audio recordings were transcribed verbatim and analysed via thematic analysis. Fourteen SDMs (all family members) of 10 critically ill patients were recruited according to the inclusion criteria. Results We identified eight sub‐themes that were classified into three themes: (a) the unbalanced act: no respite for the surrogates, conflict between rationality and emotion, and sacrifice in surrogate decision‐making; (b) unclear values and preferences: ignored values, intentional silence and substituted values; and (c) unsatisfying surrogate decisions: decision regret and decreased surrogate decision‐making self‐efficacy. Conclusion These findings underscore the urgent need for enhanced communication strategies and support systems to better assist SDMs in navigating their roles. Effective interventions could help clarify patients' values and preferences, thereby improving the decision‐making process and outcomes for both SDMs and patients. Relevance to Clinical Practice Misalignment between patient values and those of SDMs can potentially degrade the quality of decision‐making. Clinicians should be skilled in stimulating SDMs to think about patient values and preferences.

Decision-Making Processes in Surrogates of Cancer Patients in a Taiwan Intensive Care Unit

Abstract:Background: Few studies in Asian countries have explored the emotional entanglements and conflicts that surrogates often experience during the medical decision-making process. This study was to explore decision-making processes in surrogates of cancer patients in a Taiwan intensive care unit (ICU). This qualitative study surveyed a purposive sample of surrogates (n = 8; average age, 48 years) of cancer patients in the ICU of a medical center in Taiwan. A phenomenological methodology was used, and a purposive sample of surrogates of cancer patients were recruited and interviewed during the first three days of the ICU stay. Results: Based on the interview results, four themes were generalized through text progression: (1) Use love to resist: internal angst. This theme was related to the reflexive self -blame, the feelings of inner conflict, and the reluctance to make healthcare decisions, which surrogates experienced when they perceived suffering by the patient. (2) Allow an angel to spread love among us: memories and emotional entanglements. Memories of the patient caused the surrogate to experience emotional entanglements ranging from happiness to sadness and from cheerfulness to anger. (3) Dilemmas of love: anxiety about ICU visitor restrictions. The confined space and restricted visiting hours of the ICU limited the ability of surrogates to provide emotional support and to share their emotions with the patient. (4) Suffocating love: entanglement in decision-making. Emotional entanglements among family members with different opinions on medical care and their struggles to influence decision-making often prevented surrogates from thinking logically. Conclusions: Expression of emotions by ICU surrogates is often restrained and implicit, particularly in Asian populations. These results can help health professionals understand the psychological shock and inner conflict experienced by surrogates and provide a useful reference for improving their communications with surrogates.

Abstract:Background Surrogate preparedness for medical decision‐making is an important part of care planning. This study examined preparedness and engagement among historically marginalized surrogates. Methods Surrogates were included if they were named medical decision‐makers by patients ≥55 years at a San Francisco safety‐net and Veterans Affairs hospital. We assessed preparedness for medical decision‐making by asking if surrogates had been formally asked to be the medical decision‐maker, if patients had discussed medical wishes with surrogates, and if the surrogate role and these medical wishes had been documented. We assessed surrogate confidence and readiness using a modified Surrogate ACP Engagement Survey. We used Wilcoxon rank‐sum tests to measure the association of engagement scores with surrogate characteristics. Results Of 422 surrogates, their mean age was 53 years (SD ±14.5), 73% were from minoritized groups, 38% were Spanish‐speaking, and 15% had limited health literacy. For preparedness outcomes, 13% of surrogates were not formally asked to play this role, 46% reported the patient had not discussed end‐of‐life medical wishes, and 51% reported there had been no formal documentation of the surrogate role. Communication Quality Predicts Psychological Well-Being and Satisfaction in Family Surrogates of Hospitalized Older Adults: An Observational Study

Abstract:Background: Many hospitalized older adults require family surrogates to make decisions, but surrogates may perceive that the quality of medical decisions is low and may have poor psychological outcomes after the patient's hospitalization. Objective: To determine the relationship between communication quality and high-quality medical decisions, psychological well-being, and satisfaction for surrogates of hospitalized older adults. Design: Observational study at three hospitals in a Midwest metropolitan area. Participants: Hospitalized older adults (65+ years) admitted to medicine and medical intensive care units who were unable to make medical decisions, and their family surrogates. Among 799 eligible dyads, 364 (45.6%) completed the study. Main measures: Communication was assessed during hospitalization using the information and emotional support subscales of the Family Inpatient Communication Survey. Decision quality was assessed with the Decisional Conflict Scale. Outcomes assessed at baseline and 4-6 weeks post-discharge included anxiety (Generalized Anxiety Disorder-7), depression (Patient Health Questionnaire-9), post-traumatic stress (Impact of Event Scale-Revised), and satisfaction (Hospital Consumer Assessment of Healthcare Providers and Systems). Key results: The mean patient age was 81.9 years (SD 8.32); 62% were women, and 28% African American. Among surrogates, 67% were adult children. Six to eight weeks post-discharge, 22.6% of surrogates reported anxiety (11.3% moderate-severe anxiety); 29% reported depression, (14.0% moderate-severe), and 14.6% had high levels of post-traumatic stress. Emotional support was associated with lower odds of anxiety (adjusted odds ratio [AOR] = 0.65, 95% CI 0.50, 0.85) and depression (AOR = 0.80, 95% CI 0.65, 0.99) at follow-up. In multivariable linear regression, emotional support was associated with lower post-traumatic stress (β = -0.30, p = 0.003) and higher decision quality (β = -0.44, p < 0.0001). Information was associated with higher post-traumatic stress (β = 0.23, p = 0.022) but also higher satisfaction (β = 0.61, p < 0.001). Conclusions: Emotional support of hospital surrogates is consistently associated with better psychological outcomes and decision quality, suggesting an opportunity to improve decision making and well-being.

Abstract:Despite the growing need for surrogate decision‐making for older adults, little is known about how surrogates make decisions and whether advance directives would change decision‐making. We conducted a nationally representative experimental survey that cross‐randomized cognitive impairment, gender, and characteristics of advance care planning among hospitalized older adults through a series of vignettes. Our study yielded three main findings: first, respondents were much less likely to recommend life‐sustaining treatments for patients with dementia, especially after personal exposure. Second, respondents were more likely to ignore patient preferences for life‐extending treatment when the patient had dementia, and choose unwanted life‐extending treatments for patients without dementia. Third, in scenarios where the patient's wishes were unclear, respondents were more likely to choose treatments that matched their own preferences. These findings underscore the need for improved communication and decision‐making processes for patients with cognitive impairment and highlight the importance of choosing a surrogate decision‐maker with similar treatment preferences.

Abstract:Surrogate decision makers (SDMs) are challenged by difficult decisions at the end of life. This becomes more complex in young adult patients when parents are frequently the SDMs. This age group (18 to 39 years old) commonly lacks advanced directives to provide guidance which results in increased moral distress during end of life decisions. Multiple factors help guide medical decision making throughout a patient’s disease course and at the end of life. These include personal patient factors and SDM factors. It has been identified that spiritual and community group support is a powerful, but inadequately used resource for these discussions. It can improve patient-SDM-provider communications, decrease psycho-social distress, and avoid unnecessary interventions at the end of life.

Abstract:Intense emotional distress and impaired information processing have been implicated in reducing a surrogate decision maker's ability to formulate informed health care decisions for a critically ill patient. The heightened intensity of negative emotions, mental effort, and impaired judgment is consistent with the manifestation of decision fatigue. The aim of this article is to describe the validity and reliability of the Decision Fatigue Scale (DFS) among surrogate decision makers of the critically ill. A convenience sample of 101 surrogate decision makers were administered the DFS and a battery of psychosocial instruments at two time points. The DFS was specified as a unidimensional measure with adequate internal consistency (Cronbach's αs = .87, .90) and stability reliability. Discriminant validity was established with measures of emotion regulation, anxiety, and depressive symptoms. The DFS is the first subjective measure of decision fatigue for surrogate decision makers of the critically ill that demonstrates satisfactory psychometric properties.

Assessment of the Shear Bond Strength between Nanofilled Composite Bonded to Glass-ionomer Cement Using Self-etch Adhesive with Different pHs and Total-Etch Adhesive

Abstract:Statement of the Problem In the sandwich technique, the undesirable bond between the composite resin and glass-ionomer cement (GIc) is one of the most important factors which lead to the failure of restoration. Total-etch and self-etch adhesives may improve the bond strength based on their pH. Purpose The purpose of this study was to evaluate the shear bond strength between the nanofilled composite resin and GIc using different adhesives. Materials and Method In this experimental study, 40 specimens (6×6mm) in 4 groups (n=10) were prepared in acrylic mold. Each specimen contained conventional GI ChemFil Superior with a height of 3mm, bonded to Z350 composite resin with a height measured 3mm. In order to bond the composite to the GI, the following adhesives were used, respectively: A: mild Clearfil SE Bond self-etch (pH=2), B: intermediate OptiBond self-etch (pH=1.4), C: strong Adper Prompt L-Pop (pH=1), and D: Adper Single Bond 2 total-etch (pH=7.2). The shear bond strength was measured by using universal testing machine with a crosshead speed of 1mm/min. One-way ANOVA and Tukey’s test were used to analyze the data (p< 0.05). Results The shear bond strength in group A was significantly higher than group B (p= 0.002), C (p< 0.001), and D (p< 0.001). Moreover, the shear bond strength of groups A and B (self-etch) was significantly different from group D (total-etch) (p< 0.001); and C (self-etch) with D (p= 0.024). Conclusion The results of this study showed that applying the mild self-etch adhesive between the composite and the GIc results in stronger shear bond strength compared to intermediate and strong self-etch adhesives. Moreover, the self-etch adhesive increased the shear bond strength between composite resin and GIc more significantly than total-etch adhesive.

Abstract:Medical Decision Making, Ahead of Print. ObjectiveTo longitudinally examine caregiver-reported treatment decision-making roles and to investigate the associations of these roles with caregiver burden, caregiving esteem, caregiver anxiety, and depression.Methods281 caregivers of patients with stage IV solid cancers were recruited from outpatient clinics in Singapore. Caregivers were eligible if they were aged ≥21 y, primary informal caregiver, and involved in treatment decision making. We used 3 y of longitudinal data. The decision-making roles were grouped into 4 categories: no family involvement, patient/physician-led, joint, and family-led/alone decision making. Mixed-effects linear regressions were used to assess associations between decision-making roles and caregiver outcomes.ResultsOn average, 72% of caregivers reported family involvement in decision making. Compared with baseline, a higher proportion of caregivers at the 36-mo follow-up reported no family involvement (from 23% to 34%, P = 0.05) and patient/physician-led decision making (from 22% to 34%, P = 0.02), while a lower proportion reported family-led/alone (from 19% to 7%, P = 0.01) decision making. Compared with family-led/alone decision making, caregivers reporting no family involvement reported lower impact on finances (−0.15 [−0.28, −0.01], P = 0.03) while caregivers who reported patient/physician-led decision making reported lower impact on schedule and health (−0.12 [−0.20, −0.03], P = 0.01), and finances (−0.15 [−0.28, −0.03], P = 0.02), and lower anxiety (−0.69 [−1.17, −0.22], P < 0.01), and depressive symptoms (−0.69 [−1.12, −0.26], P < 0.01). Caregivers who reported joint decision making reported higher caregiving esteem (0.07 [0.01, 0.14], P = 0.02).ConclusionsHigher family involvement in decision making was associated with higher caregiver burden and psychological distress. Help should be given so that family caregivers can support patient decision-making in a meaningful way.HighlightsCompared with baseline, a lower proportion of caregivers at the 36-mo follow-up reported family-led or family-alone decision making, and a higher proportion of caregivers reported patient-led or physician-led decision making.Higher levels of family involvement in decision making were associated with burdens on caregivers' daily life, health, and finances and increased caregiver anxiety and depression.Support must be given so that family members can contribute to making medical decisions in a rewarding manner.

Expanding the paradigm of the physician's role in surrogate decision-making: an empirically derived framework

Abstract:Background: Little is known about what role physicians take in the decision-making process about life support in intensive care units. Objective: To determine how responsibility is balanced between physicians and surrogates for life support decisions and to empirically develop a framework to describe different models of physician involvement. Design: Multi-centered study of audio-taped clinician-family conferences with a derivation and validation cohort. Setting: Intensive care units of four hospitals in Seattle, Washington, in 2000 to 2002 and two hospitals in San Francisco, California, in 2006 to 2008. Participants: Four hundred fourteen clinicians and 495 surrogates who were involved in 162 life support decisions. Results: In the derivation cohort (n = 63 decisions), no clinician inquired about surrogates' preferred role in decision-making. Physicians took one of four distinct roles: 1) informative role (7 of 63) in which the physician provided information about the patient's medical condition, prognosis, and treatment options but did not elicit information about the patient's values, engage in deliberations, or provide a recommendation about whether to continue life support; 2) facilitative role (23 of 63), in which the physician refrained from providing a recommendation but actively guided the surrogate through a process of clarifying the patients' values and applying those values to the decision; 3) collaborative role (32 of 63), in which the physician shared in deliberations with the family and provided a recommendation; and 4) directive role (1 of 63), in which the physician assumed all responsibility for, and informed the family of, the decision. In 10 out of 20 conferences in which surrogates requested a recommendation, the physician refused to provide one. The validation cohort revealed a similar frequency of use of the four roles, and frequent refusal by physicians to provide treatment recommendations. Conclusions: There is considerable variability in the roles physicians take in decision-making about life support with surrogates but little negotiation of desired roles. We present an empirically derived framework that provides a more comprehensive view of physicians' possible roles.

Surrogate Decision-Making After Stroke in a Community Setting: The OASIS Project

Abstract:Stroke, Ahead of Print. Background:Patients with severe stroke often rely on surrogate decision-makers for life-sustaining treatment decisions. We investigated ethnic differences between Mexican Americans (MAs) and non-Hispanic White (NHW) individuals in surrogate reports of physician quality of communication and shared decision-making from the OASIS study (Outcomes Among Surrogate Decision Makers in Stroke) project.Methods:Patients had ischemic stroke or intracerebral hemorrhage in Nueces County, TX. Surrogates self-identified as being involved in decisions about do-not-resuscitate orders, brain surgery, ventilator, feeding tube, or hospice/comfort care. Surrogate reports of physician quality of communication (scale score, range from 0 to 10) and shared decision-making (CollaboRATE scale score, binary score 1 versus 0) were compared by ethnicity with linear or logistic regression using generalized estimating equations, adjusted for prespecified demographics, clinical factors, and confounders.Results:Between April 2016 and September 2020, 320 surrogates for 257 patients with stroke enrolled (MA, 158; NHW, 85; and other, 14). Overall quality of communication score was better among surrogates of MA patients than NHW individuals after adjustment for demographics, stroke severity, and patient survival though the ethnic difference was attenuated (β, 0.47 [95% CI, −0.17 to 1.12];P=0.15) after adjustment for trust in the medical profession and frequency of personal prayer. High CollaboRATE scale scores were more common among surrogates of MA patients than NHW individuals (unadjusted odds ratio, 1.75 [95% CI, 1.04–2.95]). This association persisted after adjustment for demographic and clinical factors though there was an interaction between patient age and ethnicity (P=0.04), suggesting that this difference was primarily in older patients.Conclusions:Surrogate decision-makers of MA patients generally reported better outcomes on validated measures of quality of communication and shared decision-making than NHW individuals. Further study of outcomes among diverse populations of stroke surrogate decision-makers may help to identify sources of strength and resiliency that may be broadly applicable.

Mothers' responses during the child's stem cell transplantation: pilot study

Abstract:Purpose: Examine the relationships between the mothers' anxiety and depressive symptomatology and resources (problem-solving orientation and style, coping, and social support) during their child's stem cell transplantation (SCT). Method: A prospective correlation design was used. A convenience sample of 23 mothers completed State-Anxiety Inventory, Center of Epidemiologic Studies Depression Scale, Social Problem-Solving Inventory-Revised, and Stress Support Scale at the child's admission and 10 days after the stem cells infusion. Findings: Significant relationships were found between mothers' negative problem-solving orientation and emotional responses, coping and depressive symptomatology, and between social support and emotional responses but in an inverse direction. Conclusion: Health care professionals need to increase their understanding of mothers' stress responses during the SCT. These responses may affect the mother/child interactions and the mother's ability to provide childcare. A psychosocial intervention that includes these resources may assist the mothers during their child's SCT.
Prehospital Providers' Perspectives about Online Medical Direction in Emergency End-of-Life Decision-Making

Abstract:Background: End-of-life treatment decisions present special challenges for prehospital emergency providers. Paramedics regularly make value-laden choices that transcend technical judgment and professional skill, affecting the type of care, how and to whom it is provided. Changes in prehospital emergency care over the last decade have created new moral challenges for prehospital emergency providers; these changes have also accentuated the need for paramedics to make rapid and reasoned ethical judgments. Objective: The purpose of the study was to explore the decision-making process that occurs when prehospital emergency teams respond to an end-of-life call with a focus on how state authorized documents such as a Non-Hospital Do Not Resuscitate (NHDNR) or Medical/Physician's Orders for Life-Sustaining Treatment (MOLST/POLST) or lack thereof inform decision-making. This paper presents the specific circumstances that informed the need for intervention from Online Medical Direction (OLMD) framed in the perspectives and words of the prehospital providers seeking that assistance. Methods: This study involved in-depth in-person interviews with 50 providers to elicit participants' experiences in their own words using a semi-structured interview instrument. Interviews were audio recorded and transcribed with permission. Results: Five themes emerged that illuminated how and when OLMD was involved in emergency end-of-life decisions: Termination of Resuscitation (TOR); Family Revoked DNR; Missing Documents; No Documents and No CPR; and Unusual Situations. Participants illustrated how the decision to terminate efforts was best-supported when it was made by collaboration between the on-scene provider and OLMD. Participants described ethical dilemmas when families asked them to initiate CPR in the presence of DNR orders and cognitive dissonance when CPR has been initiated but a valid DNR/MOLST is subsequently located. Conclusions: The study findings demonstrate the invaluable contribution of OLMD for complex end-of-life care decisions by prehospital providers, especially when there are difficult legal, ethical, and logistical questions. OLMD provides far more than technical support.
