Abstract:Regulatory frameworks for artificial intelligence (AI) are needed to mitigate risks while ensuring the ethical, secure, and effective implementation of AI technology in healthcare and population health. In this article, we present a synthesis of 141 binding policies applicable to AI in healthcare and population health in the EU and 10 European countries. The EU AI Act sets the overall regulatory framework for AI, while other legislations set social, health, and human rights standards, address the safety of technologies and the implementation of innovation, and ensure the protection and safe use of data. Regulation specifically pertaining to AI is still nascent and scarce, though a combination of data, technology, innovation, and health and human rights policy has already formed a baseline regulatory framework for AI in health. Future work should explore specific regulatory challenges, especially with respect to AI medical devices, data protection, and data enablement.

Abstract:The healthcare sector is faced with challenges due to a shrinking healthcare workforce and a rise in chronic diseases that are worsening with demographic and epidemiological shifts. Digital health interventions that include artificial intelligence (AI) are being identified as some of the potential solutions to these challenges. The ultimate aim of these AI systems is to improve the patient's health outcomes and satisfaction, the overall population's health, and the well-being of healthcare professionals. The applications of AI in healthcare services are vast and are expected to assist, automate, and augment several healthcare services. Like any other emerging innovation, AI in healthcare also comes with its own risks and requires regulatory controls. A review of the literature was undertaken to study the existing regulatory landscape for AI in the healthcare services sector in developed nations. In the global regulatory landscape, most of the regulations for AI revolve around Software as a Medical Device (SaMD) and are regulated under digital health products. However, it is necessary to note that the current regulations may not suffice as AI-based technologies are capable of working autonomously, adapting their algorithms, and improving their performance over time based on the new real-world data that they have encountered. Hence, a global regulatory convergence for AI in healthcare, similar to the voluntary AI code of conduct that is being developed by the US-EU Trade and Technology Council, would be beneficial to all nations, be it developing or developed.

Abstract:Abstract This paper explores the status of Artificial Intelligence (AI) for healthcare research in Africa. The aim was to use bibliometric and thematic analysis methods to determine the publication counts, leading authors, top journals and publishers, most active institutions and countries, most cited institutions, funding bodies, top subject areas, co-occurrence of keywords and co-authorship. Bibliographic data were collected on April 9 2022, through the Lens database, based on the critical areas of authorship studies, such as authorship pattern, number of authors, etc. The findings showed that several channels were used to disseminate the publications, including articles, conference papers, reviews, and others. Publications on computer science topped the list of documented subject categories. The Annals of Tropical Medicine and Public Health is the top journal, where articles on AI have been published. One of the top nations that published AI research was the United Kingdom. With 143 publications, Harvard University was the higher education institution that produced the most in terms of affiliation. It was discovered that the Medical Research Council was one of the funding organizations that supported research, resulting in the publication of articles in AI. By summarizing the current research themes and trends, this work serves as a valuable resource for researchers, practitioners, and funding organizations interested in Artificial intelligence for healthcare research in Africa.

Abstract:Artificial Intelligence (AI) has shown remarkable potential to revolutionise healthcare by enhancing diagnostics, improving treatment outcomes, and streamlining administrative processes. In the global regulatory landscape, several countries are working on regulating AI in healthcare. There are five key regulatory issues that need to be addressed: (i) data security and protection—measures to cover the "digital health footprints" left unknowingly by patients when they access AI in health services; (ii) data quality—availability of safe and secure data and more open database sources for AI, algorithms, and datasets to ensure equity and prevent demographic bias; (iii) validation of algorithms—mapping of the explainability and causability of the AI system; (iv) accountability—whether this lies with the healthcare professional, healthcare organisation, or the personified AI algorithm; (v) ethics and equitable access—whether fundamental rights of people are met in an ethical manner. Policymakers may need to consider the entire life cycle of AI in healthcare services and the databases that were used for the training of the AI system, along with requirements for their risk assessments to be publicly accessible for effective regulatory oversight. AI services that enhance their functionality over time need to undergo repeated algorithmic impact assessment and must also demonstrate real-time performance. Harmonising regulatory frameworks at the international level would help to resolve cross-border issues of AI in healthcare services.

Abstract:Healthcare in Africa is a complex issue influenced by many factors including poverty, lack of infrastructure, and inadequate funding. However, Artificial intelligence (AI) applied to healthcare, has the potential to transform healthcare in Africa by improving the accuracy and efficiency of diagnosis, enabling earlier detection of diseases, and supporting the delivery of personalized medicine. This paper reviews the current state of how AI Algorithms can be used to improve diagnostics, treatment, and disease monitoring, as well as how AI can be used to improve access to healthcare in Africa as a low-resource setting and discusses some of the critical challenges and opportunities for its adoption. As such, there is a need for a well-coordinated effort by the governments, private sector, healthcare providers, and international organizations to create sustainable AI solutions that meet the unique needs of the African healthcare system.

Abstract:Background: The global health system remains determined to leverage on every workable opportunity, including artificial intelligence (AI) to provide care that is consistent with patients' needs. Unfortunately, while AI models generally return high accuracy within the trials in which they are trained, their ability to predict and recommend the best course of care for prospective patients is left to chance. Purpose: This review maps evidence between January 1, 2010 to December 31, 2023, on the perceived threats posed by the usage of AI tools in healthcare on patients' rights and safety. Methods: We deployed the guidelines of Tricco et al. to conduct a comprehensive search of current literature from Nature, PubMed, Scopus, ScienceDirect, Dimensions AI, Web of Science, Ebsco Host, ProQuest, JStore, Semantic Scholar, Taylor & Francis, Emeralds, World Health Organisation, and Google Scholar. In all, 80 peer reviewed articles qualified and were included in this study. Results: We report that there is a real chance of unpredictable errors, inadequate policy and regulatory regime in the use of AI technologies in healthcare. Moreover, medical paternalism, increased healthcare cost and disparities in insurance coverage, data security and privacy concerns, and bias and discriminatory services are imminent in the use of AI tools in healthcare. Conclusions: Our findings have some critical implications for achieving the Sustainable Development Goals (SDGs) 3.8, 11.7, and 16. We recommend that national governments should lead in the roll-out of AI tools in their healthcare systems. Also, other key actors in the healthcare industry should contribute to developing policies on the use of AI in healthcare systems.

Abstract:This study aims to analyze the adoption of Artificial Intelligence (AI) in South and Southeast Asia and Africa, focusing on how AI technologies shape the digital economies in these regions. The objective is to assess AI's role in key sectors such as energy, manufacturing, healthcare, and finance, and examine its impact on policy formulation, public service delivery, and governance. The paper employs a comparative literature review methodology. Data is gathered from secondary sources, including academic papers and reports, to analyze AI infrastructure, policy frameworks, sectoral impacts, and barriers to adoption. The study identifies similarities and differences in AI integration across the regions. The study reveals that South and Southeast Asia have made greater advancements in AI adoption, driven by stronger infrastructure and government policies, while Africa faces challenges due to infrastructure deficits and limited AI expertise. However, both regions show potential for leveraging AI to drive economic growth, particularly in underserved sectors like agriculture and financial inclusion. Additionally, the research highlights the critical role of AI governance, with a strong need for both regions to develop robust regulatory frameworks to ensure responsible and ethical AI implementation. The study is limited by its reliance on secondary data, which restricts deeper insight into on-the-ground challenges. Future research should incorporate field studies and interviews with AI practitioners for a more comprehensive understanding of AI’s impact. This paper contributes to the limited comparative research on AI adoption between South and Southeast Asia and Africa. It offers insights for policymakers and industry leaders on how AI can be harnessed for inclusive economic growth and transformation in these regions.

Abstract:What sets AI systems and AI-powered medical robots apart from all other forms of advanced medical technology is their ability to operate at least to some degree autonomously from the human health care practitioner and to use machine-learning to generate new, often unforeseen, analysis and predictions. This poses challenges under the current framework of laws, regulations, and ethical guidelines applicable to health care in South Africa. The article outlines these challenges and sets out guiding principles for a normative framework to regulate the use of AI in health care. The article examines three key areas for legal reform in relation to AI in health care. First, it proposes that the regulatory framework for the oversight of software as a medical device needs to be updated to develop frameworks for adequately regulating the use of such new technologies. Secondly, it argues that the present HPCSA guidelines for health care practitioners in South Africa adopt an unduly restrictive approach centred in the outmoded semantics of telemedicine. This may discourage technological innovation that could improve access to health care for all, and as such the guidelines are inconsistent with the national digital health strategy. Thirdly, it examines the common law principles of fault-based liability for medical negligence, which could prove inadequate to provide patients and users of new technologies with redress for harm where fault cannot clearly be attributed to the healthcare practitioner. It argues that consideration should be given to developing a statutory scheme for strict liability, together with mandatory insurance, and appropriate reform of product liability pertaining to technology developers and manufacturers. These legal reforms should not be undertaken without also developing a coherent, human-rights centred policy framework for the ethical use of AI, robotics, and related technologies in health care in South Africa.

Abstract:Artificial Intelligence (AI) is being adopted across the world and promises a new revolution in healthcare. While AI-enabled medical devices in North America dominate 42.3% of the global market, the use of AI-enabled medical devices in other countries is still a story waiting to be unfolded. We aim to delve deeper into global regulatory approaches towards AI use in healthcare, with a focus on how common themes are emerging globally. We compare these themes to the World Health Organization's (WHO) regulatory considerations and principles on ethical use of AI for healthcare applications. Our work seeks to take a global perspective on AI policy by analyzing 14 legal jurisdictions including countries representative of various regions in the world (North America, South America, South East Asia, Middle East, Africa, Australia, and the Asia-Pacific). Our eventual goal is to foster a global conversation on the ethical use of AI in healthcare and the regulations that will guide it. We propose solutions to promote international harmonization of AI regulations and examine the requirements for regulating generative AI, using China and Singapore as examples of countries with well-developed policies in this area.

Abstract:There is an ongoing debate on balancing the benefits and risks of artificial intelligence (AI) as AI is becoming critical to improving healthcare delivery and patient outcomes. Such improvements are essential in resource-constrained settings where millions lack access to adequate healthcare services, such as in Africa. AI in such a context can potentially improve the effectiveness, efficiency, and accessibility of healthcare services. Nevertheless, the development and use of AI-driven healthcare systems raise numerous ethical, legal, and socio-economic issues. Justice is a major concern in AI that has implications for amplifying social inequities. This paper discusses these implications and related justice concepts such as solidarity, Common Good, sustainability, AI bias, and fairness. For Africa to effectively benefit from AI, these principles should align with the local context while balancing the risks. Compared to mainstream ethical debates on justice, this perspective offers context-specific considerations for equitable healthcare AI development in Africa.

Abstract:Abstract Background Artificial Intelligence (AI) platforms, increasingly deployed in public health, utilize robust data systems as a critical component for health emergency preparedness. Yet, Africa faces numerous challenges in the availability, analyses, and use of data to inform health decision-making. Countries have limited access to their population data. Those with access, struggle to utilize these data for program improvements. Owing to the rapid growth of mobile phone ownership and use in the region, Africa is poised to leverage AI technologies to increase the adoption, access and use of data for health. To discuss and propose solutions for responsible development and adoption of innovations like AI in Africa, a virtual workshop was organized from the 21st to 24th June, 2021. This report highlights critical policy dimensions of strengthening digital health ecosystems by high-level policymakers, technical experts, academia, public and private sector partners. Method The four days’ workshop focused on nine sessions, with each session focusing on three themes. Discussions during the sessions concentrated on public and private sectors, the academia and multilateral organizations’ deployment of AI. These discussions expanded participants’ understanding of AI, the opportunities and challenges that exist during adoption, including the future of AI for health in the African region. Approximately 250 participants attended the workshop, including countries representatives from ministries of Health, Information and Technology, Developmental Organizations, Private Sector, Academia and Research Institutions among others. Results The workshop resolved that governments and relevant stakeholders should collaborate to ensure that AI and digital health receive critical attention. Government ownership and leadership were identified as critical for sustainable financing and effective scale-up of AI-enabled applications in Africa. Thus, government is to ensure that key recommendations from the workshop are implemented to improve health sector development in Africa. Conclusions The AI workshop was a good forum to deliberate important issues regarding AI for health in the African context. It was concluded that there is a need to focus on vital priorities in deploying AI in Africa: Data protection, privacy and sharing protocols; training and creating platforms for researchers; funding and business models; developing frameworks for assessing and implementing AI; organizing forums and conferences on AI; and instituting regulations, governance and ethical guidelines for AI. There is a need to adopt a health systems approach in planning for AI to reduce inefficiencies, redundancies while increasing effectiveness in the use of AI. Thus, robust collaborations and partnerships among governments and various stakeholders were identified as key.
Nteasee: A mixed methods study of expert and general population perspectives on deploying AI for health in African countries

Abstract:Artificial Intelligence (AI) for health has the potential to significantly change and improve healthcare. However in most African countries, identifying culturally and contextually attuned approaches for deploying these solutions is not well understood. To bridge this gap, we conduct a qualitative study to investigate the best practices, fairness indicators, and potential biases to mitigate when deploying AI for health in African countries, as well as explore opportunities where artificial intelligence could make a positive impact in health. We used a mixed methods approach combining in-depth interviews (IDIs) and surveys. We conduct 1.5-2 hour long IDIs with 50 experts in health, policy, and AI across 17 countries, and through an inductive approach we conduct a qualitative thematic analysis on expert IDI responses. We administer a blinded 30-minute survey with case studies to 672 general population participants across 5 countries in Africa and analyze responses on quantitative scales, statistically comparing responses by country, age, gender, and level of familiarity with AI. We thematically summarize open-ended responses from surveys. Our results find generally positive attitudes, high levels of trust, accompanied by moderate levels of concern among general population participants for AI usage for health in Africa. This contrasts with expert responses, where major themes revolved around trust/mistrust, ethical concerns, and systemic barriers to integration, among others. This work presents the first-of-its-kind qualitative research study of the potential of AI for health in Africa from an algorithmic fairness angle, with perspectives from both experts and the general population. We hope that this work guides policymakers and drives home the need for further research and the inclusion of general population perspectives in decision-making around AI usage.

Abstract:A woman developed what appeared to be typical Graves' disease in 1965 at the age of 45 years. After 9 years of antithyroid drug therapy, she was treated with radioiodine. Ten years later (1985) she developed postablative hypothyroidism. Despite replacement doses of thyroxine that resulted in thyroid hormone levels that were in the hyperthyroid range, TSH levels remained elevated. Initial biochemical studies, including a high alpha-subunit to TSH ratio, suggested a pituitary TSH-secreting tumor, but a CT scan of the sella turcica was normal. In 1994, while undergoing an otolaryngologic examination, the patient was found to have a nasopharyngeal mass lesion, which was ultimately shown histologically and immunohistochemically to be an ectopic pituitary tumor. Resection of the mass restored TSH and alpha-subunit levels to normal. This patient probably represents the first ectopic TSH-secreting pituitary tumor to be reported.

Abstract:Abstract The sharing of health data is an essential component in the provision of healthcare, in medical research, and disease surveillance. Health data sharing is subject to regulatory frameworks that vary across jurisdictions. In Africa, numerous factors complicate the regulation of health data sharing, including technological, motivational, economic, and political barriers, as well as ethical and legal challenges. This comparative study examines the regulation of health data sharing in Africa by comparing and contrasting the legal and policy frameworks of five African countries. The study identifies gaps and inconsistencies in the current regulatory regimes and provides recommendations for improving the regulation of health data sharing in Africa.

Abstract:Artificial Intelligence (AI) is revolutionizing various fields, including public health surveillance. In Africa, where health systems frequently encounter challenges such as limited resources, inadequate infrastructure, failed health information systems and a shortage of skilled health professionals, AI offers a transformative opportunity. This paper investigates the applications of AI in public health surveillance across the continent, presenting successful case studies and examining the benefits, opportunities, and challenges of implementing AI technologies in African healthcare settings. Our paper highlights AI's potential to enhance disease monitoring and health outcomes, and support effective public health interventions. The findings presented in the paper demonstrate that AI can significantly improve the accuracy and timeliness of disease detection and prediction, optimize resource allocation, and facilitate targeted public health strategies. Additionally, our paper identified key barriers to the widespread adoption of AI in African public health systems and proposed actionable recommendations to overcome these challenges.

Abstract:The introduction of artificial intelligence (AI) marks the beginning of a revolutionary period for the global economic environments, particularly in the developing economies of Africa. This concept paper explores the various ways in which AI can stimulate economic growth and innovation in developing markets, despite the challenges they face. By examining examples like VetAfrica, we investigate how AI-powered applications are transforming conventional business models and improving access to financial resources. This highlights the potential of AI in overcoming obstacles such as inefficient procedures and restricted availability of capital. Although AI shows potential, its implementation in these areas faces obstacles such as insufficient digital infrastructure, limited data availability, and a lack of necessary skills. There is a strong focus on the need for a balanced integration of AI, which involves aligning technological progress with ethical considerations and economic inclusivity. This paper focuses on clarifying the capabilities of AI in addressing economic disparities, improving productivity, and promoting sustainable development. It also aims to address the challenges associated with digital infrastructure, regulatory frameworks, and workforce transformation. The methodology involves a comprehensive review of relevant theories, literature, and policy documents, complemented by comparative analysis across South Africa, Nigeria, and Mauritius to illustrate transformative strategies in AI adoption. We propose strategic recommendations to effectively and ethically utilize the potential of AI, by advocating for substantial investments in digital infrastructure, education, and legal frameworks. This will enable Africa to fully benefit from the transformative impact of AI on its economic landscape. This discourse seeks to offer valuable insights for policymakers, entrepreneurs, and investors, emphasizing innovative AI applications for business growth and financing, thereby promoting economic empowerment in developing economies.

Abstract:Thirteen cases of pyruvate kinase (PK) deficiency, considered to be heterozygous for different PK mutants because of no consanguinities in their parents, were characterized by the International Committee for Standardization in Haematology (ICSH) recommended methods. These deficiency cases are named PK “Kagoshima,” PK “Kyoto,” PK “Takamatsu,” PK “Abeno,” PK “Kobe,” PK “Marugame,” PK “Hoenzaka,” PK “Osaka,” PK “Motomachi,” PK “Gifu,” PK “Hiroshima” PK “Matsumoto,” and PK “Tama.”

Abstract:Recent achievements in respect of Artificial Intelligence (AI) open up opportunities for new tools to assist medical diagnosis and care delivery. However, the typical process for the development of AI is through repeated cycles of learning and implementation, something that poses challenges to our existing system of regulating medical devices. Product developers face tensions between the benefits of continuous improvement/deployment of algorithms and keeping products unchanged. The latter more easily facilitates collecting evidence for safety assurance processes but sacrifices optimisation of performance and adaptation to user needs gained through learning-implementation cycles. The challenge is how to balance potential benefits with the need to assure their safety. Governance and assurance processes are needed that can accommodate real-time or near-real-time machine learning. Such an approach is of great importance in healthcare and other fields of application. AI has stimulated an intense process of learning as this new technology embeds in application contexts. The process is not only about the application of AI in the real world but also about the institutional arrangements for its safe and dependable deployment, including regulatory experimentation involving new market pathways, monitoring and surveillance, and sandbox schemes. We review the key themes, challenges and potential solutions raised at two stakeholder workshops and highlight recent attempts to adapt the laws for AI-enabled medical devices (AIeMD) with a special focus on the regulatory proposals in the UK and internationally. The UK regulatory trajectory shows signs of alignment with the US thinking, and yet the European Union model is still the most closely aligned framework.
