Abstract:Introduction Cardiac surgery patients often experience significant pain after median sternotomy. The transversus thoracis muscle plane (TTP) block is a newly developed, single-shot nerve block technique that provides analgesia for the anterior chest wall. In this double-blind pilot study, we assessed the feasibility of performing this novel block as an analgesic adjunct. Methods All patients aged 18–90 undergoing elective cardiac surgery were randomized to the block or standard care control group on admission to the intensive care unit after surgery. Under ultrasound guidance, patients in the block group received the TTP block with 20 mL of either 0.3% or 0.5% ropivacaine bilaterally, based on weight. The control group did not receive any injections. All blocks were performed by a single anesthesiologist, and data collection was performed by blinded assessors. The primary feasibility outcomes were rate of recruitment, adherence, and adverse events. The rate of recruitment was defined as the ratio of patients giving informed consent to the number of eligible patients who were approached to participate. Secondary outcomes included 12-hour and 24-hour Numeric Rating Scale (NRS) pain scores, 24-hour hydromorphone and acetaminophen requirements, time to extubation, time to first opioid administration, and patient satisfaction (on a yes/no questionnaire) at 24 hours. Results Twenty patients were approached for this study and 19 were enrolled. Eight patients received the intended intervention in each group. The recruitment rate was 95% of all approached eligible patients, and the adherence rate to treatment group was 94%. There were no block-related adverse events. The mean (SD) NRS pain scores at rest were 3.3 (3.2) in the block group vs 5.6 (3.2) in the control group at 12 hours. At 24 hours, the pain scores were 4.1 (3.9) vs 4.1 (3.3) in the block and control group, respectively. The mean (SD) 24-hour hydromorphone administration was 1.9 (1.1) mg in the block group vs 1.8 (0.9) mg in the control group. Discussion The TTP block is a novel pain management strategy poststernotomy. The results reveal a high patient recruitment, adherence, and satisfaction rate, and provide some preliminary data supporting safety. Trial registration number NCT03128346 .

Comparison of ultrasound-guided vs.anatomical landmark-guided axillary brachial plexus block

Abstract:Objective To explore the clinical effect and safety of ultrasound-guided and anatomical landmark-guided axillary brachial plexus block.Methods Total 68 patients scheduled for upper limb surgery were randomly divided into ultrasound-guided group and anatomical landmark-guided group with 34 cases in each group.The same local anesthetics with1% lidocaine hydrochloride and 0.375% ropivacaine hydrochloride were performed in both groups.The anesthesia finish,onset of anesthesia,dosage of anesthetics and side effect were analyzed.The anesthetic effect was assessed using visual analogue scale(VAS).T-test and Chi-square test were employed to analyze the results.Results In the ultrasoundguided group,the operating time was(3.11 ± 1.32) min and VAS was(2.86 ± 0.52);in the anatomical landmarkguided group,the operation time was(5.91 ± 2.24) min and VAS was(4.11 ± 0.82) %.The difference in the operation time and the anesthesia effect was statistical significant between two groups(t = 6.282,7.511,P < 0.01).There was one case of Honer syndrome in the ultrasound-guided group,while there were 6 cases of Honer syndrome,3 cases of local anesthetic toxicity reaction,5 cases of hematoma,and 1 case of pneumothorax;the difference in the incidence of honer syndrome and hematoma was statistically significant(χ2= 5.983,5.402,P < 0.05).Conclusion As compared to the anatomical landmark-guided axillary brachial plexus block,ultrasound-guided brachial plexus block can achieve good anesthetic effect,and reduce the adverse reactions.

Abstract:Objective:Observing the anaesthesia quality of supraclavicular and infraclavicular brachial plexus block by ultrasound-guided,we evaluated the value of the brachial plexus block by mean of high-resolution sonography in order to explore the new approach of peripheral nerve block.Methods:One hundred and twenty patients scheduled for elective surgery of forearm and hand or Arteriovenous fistulization of forearm were randomly divided into two groups,i.e.,supraclavicular(SCB) group and infraclavicular(ICB) group,with 60 subjects in each group.The performance time was recorded.The onset time and the block execution time of axillary nerve,median cutaneous nerve of arm,lateral antebrachial cutaneous nerve,median antebrachial cutaneous nerve,ulnar nerve,median nerve,and radial nerve were measured respectively,and then the block quality was evaluated.Results:High-resolution ultrasound could successfully guided brachial plexus block and clearly displayed the local anesthetic distributing around never.The performance times were(4.5±3.2) min in SCB group and(3.9±1.6) min in ICB group(P0.05).The onset times of median antebrachial cutaneous,ulnar nerves and radial nervers in SCB group were(8.3±3.1) min,(7.1±2.8) min and(3.8±4.1) min,and these were(4.1±2.0) min,(3.6±1.7) min,and(8.5±1.8) min in the ICB.The onset times of the antebrachial cutaneous and ulnar nerves in the ICB group were significantly longer than these in SCB group(P0.05),and the radial nerve was shorter(P0.05).The complete success rates of block in SCB and ICB group were respectively 86% and 80%,respectively(P0.05).The incidence of complication for Horner syndrome in the SCB group was 16%,but there was no complication in ICB group.Conclusion:High-resolution sonography can clearly display the brachial plexus.Supraclavicular and infraclavicular brachial plexus block by ultrasound-guided are feasible technique in regional anaesthesia.Ultrasound-guided infracla vicular brachial plexus block is much safer and is deserved to apply in clinic practice.

Abstract:Background and Aims Intertransverse process (ITP) blocks are applied on the posterior side of the thoracic paravertebral space. The modality is described as being a paravertebral block by proxy, possibly providing a similar analgesic effect as the thoracic paravertebral block. However, systematic evidence on anaesthetised dermatomes and the extent of cutaneous sensory loss following ITP blocks is sparse. This study aims to test the single‐ versus the multiple‐injection ITP block. The primary outcome is the number of anaesthetised thoracic dermatomes for each block type. Methods Twelve healthy male volunteers will participate in this randomised, procedure‐related, double‐blinded, non‐inferiority crossover trial after informed consent. Blinded participants will receive either a unilateral single‐injection ITP block with 21 mL ropivacaine 7.5 mg/mL including two sham blocks or a unilateral multiple‐injection ITP block with 3 × 7 mL ropivacaine 7.5 mg/mL on study Day 1, and the other modality on study Day 2. Block applicants will be blinded from outcome assessment and vice versa. Following block application sensory test by mechanical pinprick and temperature discrimination will be performed. Anterior truncal thermography will be measured three times after block application to compare skin temperature in the mid‐clavicular line between the blocked and the contralateral non‐blocked hemithorax. In addition, blood pressure changes are measured three times non‐invasively. Discussion The current study will provide substantial knowledge regarding the cutaneous sensory loss of the ITP block. Furthermore, the study might provide insight regarding the possible clinical usage of thermography as a reliable instrument for measuring nerve block efficacy.

Abstract:Introduction The intertransverse process block is increasingly used to ameliorate postoperative pain following a plethora of surgical procedures involving the thoracic wall. Nevertheless, the optimal approach and cutaneous extent of the sensory block are currently unknown. We aimed to further describe the intertransverse process block, single injection versus multiple injection, and we hypothesized that the single-injection intertransverse process block is a non-inferior technique. Methods Twelve healthy male volunteers were cross-over randomized to receive either single-injection intertransverse process block with 21 mL ropivacaine 7.5 mg/mL, including two sham injections, at the thoracic level T4/T5 or multiple-injection intertransverse process block with three injections of 7 mL ropivacaine 7.5 mg/mL at the thoracic levels T2/T3, T4/T5 and T6/T7 at the first visit. At the second visit, the other technique was applied on the contralateral hemithorax. A non-inferiority margin of 1.5 anesthetized thoracic dermatomes was chosen. Results The mean difference (95% CI) in the number of anesthetized thoracic dermatomes was 0.82 (−0.41 to 2.05) p non-inf <0.01 indicating non-inferiority favoring the single-injection technique. Both techniques anesthetized the ipsilateral thoracic wall and demonstrated contralateral cutaneous involvement to a variable extent. The multiple-injection intertransverse process block anesthetized a significantly larger cutaneous area on the posterior hemithorax and decreased mean arterial pressure at 30 and 60 min postblock application. Thoracic thermography showed no intermodality temperature differences yet compared with baseline temperatures both techniques showed significant differences. Conclusions Single-injection intertransverse process block is non-inferior to multiple injection in terms of anesthetized thoracic dermatomes. Both techniques generally anesthetize the hemithoracic wall to a variable extent. EU clinical trials register 2022-501312-34-01.

Anatomical classification and clinical application of thoracic paraspinal blocks

Abstract:Various techniques for regional anesthesia and analgesia of the thorax are currently being used in clinical practice. A recent international consensus has anatomically classified paraspinal blocks in the thoracic spinal region into the following four types: paravertebral, retrolaminar, erector spinae plane, and intertransverse process blocks. These blocks have different anatomical targets; thus, the spreading patterns of the injectates differ and can consequently exhibit different neural blockade characteristics. The paravertebral block directly targets the paravertebral space just outside the neuraxial region and has an analgesic efficacy comparable to that of the epidural block; however, there are multiple potential risks associated with this technique. Retrolaminar and erector spinae plane blocks target the erector spinae plane on the vertebral lamina and transverse process, respectively. In anatomical studies, these two blocks showed different injectate spreading patterns to the back muscles and the fascial plane. In cadaveric studies, paravertebral spread was identified, but variable. However, numerous clinical reports have shown paravertebral spread with erector spinae plane blocks. Both techniques have been found to reduce postoperative pain compared to controls; however, the results have been more inconsistent than with the paravertebral block. Finally, the intertransverse process block targets the tissue complex posterior to the superior costotransverse ligament. Anatomical studies have revealed that this block has pathways that are more direct and closer to the paravertebral space than the retrolaminar and erector spinae plane blocks. Cadaveric evaluations have consistently shown promising results; however, further clinical studies using this technique are needed to confirm these anatomical findings.

Abstract:Background and objectives The subcostal transversus abdominis plane block (scTAPB) provides analgesia to the anterior abdominal wall but analgesic spread following a single injection remains modest and variable. The oblique scTAPB, which is performed with a continuous injection during needle progression into the TAP along the oblique subcostal line, may extend the block to the whole anterior abdominal wall but needle progression by hydrodissection may be difficult, explaining why this block is not widely used. This study investigated if two sequential scTAPB injections (2scTAPI) may reach more nerves than a single scTAPB (1scTAPI). Methods After institutional approval and under ultrasound guidance, a single injection of 20 mL of blue dye was performed in 16 cadaveric subjects into the TAP along the costal margin and lateral to the semilunaris line (1scTAPI). On the opposite side, a 20 mL of green solution was split into two injections (2scTAPI), using one injection medial and one lateral to the semilunaris line. Dye spread was assessed via gross anatomical dissection using an indirect method to quantify the involvement of the anterior cutaneous branches of the intercostal nerves with each technique. A nerve was considered stained if the dye was observed in the TAP, reaching the costal margin aligned with the bony extremity of its corresponding rib. Results According to the spread to the costal margin, the estimated median number of consecutive nerves reached by dye after 2scTAPI was 5 (IQR 5–6) vs 3 (IQR 3–4) for 1scTAPI (p=0.00001). Spread after 2scTAPI suggested coverage of nerves originating from T8 to T12 in 85% of the subjects, and from T7 to T12 in 5 out of 17 cases. Spread following the 1scTAPI suggested coverage of nerves from T9 to T11 in most cases. Conclusions In a cadaveric model, a larger spread at the costal margin was obtained after 2scTAPI, suggesting a greater coverage of anterior branches of abdominal wall nerves compared with 1scTAPI. Clinical studies are needed to confirm these results.

Abstract:Objective:To evaluate the feasibility and success rate of in-plane ultrasound-guided paravertebral block using laterally intercostal approach.Methods:In the study,27 patients undergoing elective thoracic surgery were selected to do paravertebral block preoperatively.The fifth intercostal space was scanned by ultrasound probe which was placed along the long axis of the rib and 8 cm lateral to the midline of the spine.The needle was advanced in increments aiming at the space between the internal and innermost intercostal muscles.Once the space between the muscles was achieved,20 mL of 0.5％ (mass fraction) ropivacaine was injected and a catheter was inserted.Whether the tip of catheter was in right place was evaluated by ultrasound image.The block dermatomes of cold sensation were recorded 10,20 and 30 min after the bolus drug was given.Then 0.2％ ropivacaine was infused with 6 mL/h via the catheter by an analgesia pump postoperatively.The block dermatomes of cold sensation and pain score were recorded 1,6,24 and 48 h postoperatively.Results:The first attempt success rate of catheteration was 81.48 ％ (22/27);the tips of catheter were proved in right places after the second or third attempt in 5 patients.The median numbers of the block dermatomes 10,20 and 30 min after the bolus drug was given were 2,3,4;the median numbers of block dermatomes were 5,5,5,4,and of pain score were 1,1,2,2 at 1,6,24,48 h postoperatively;no case of bilateral block,pneumothorax or vessel puncture occurred.Conclusion:Thoracic paravertebral block using laterally intercostal approach is feasible,which has high success rate of block and low rate of complications.
Anatomical evaluation of the superficial parasternal intercostal plane block

Abstract:Background and objectives: Few cadaveric studies have evaluated the dye spread with superficial parasternal intercostal plane (SPIP) blocks. In this study, we examined the dye spread of an ultrasound-guided SPIP block in a human cadaveric model with single and double injection techniques. Methods: Seven single and four double ultrasound-guided SPIP blocks were performed in seven unembalmed human cadavers using an in-plane approach with the transducer oriented parasagitally 1 cm lateral to the sternum. For the single SPIP, 20 mL of 0.166% methylene blue was injected in the second or third intercostal space into the plane between the Pec major muscle and internal intercostal muscles. For the double SPIP, 10 mL of 0.166% methylene blue was injected in the SPIP at one intercostal space with an additional 10 mL injected in the SPIP two intercostal spaces caudally. The extent of dye spread was documented. Results: For all SPIP injections, there was consistent mediolateral spread from the sternum to the mid-clavicular line, with many extending laterally to the anterior axillary line. There was craniocaudal spread to a median of 2 intercostal muscles with a single SPIP and 3 intercostal muscles with a double SPIP. There was a median spread to 1 intercostal nerve for the single SPIP and 1.5 intercostal nerves with the double SPIP. Conclusions: The SPIP block demonstrated limited spread in this cadaver study. A single injection of this block may be of limited value and multiple SPIP injections may be needed to have adequate spread for anterior thoracic procedures.
Modified thoracoabdominal nerve block through perichondrial approach (M-TAPA): an anatomical study to evaluate the spread of dye after a simulated injection in soft embalmed Thiel cadavers

Abstract:Background and objectives: There is still no consensus on the analgesic range and mechanisms of action of modified thoracoabdominal nerve block through perichondrial approach (M-TAPA). This cadaveric study aimed to determine the spread of an injectate following simulated M-TAPA. Methods: Simulated M-TAPA injections (n=8) were administered on both sides of soft embalmed Thiel cadavers with 25 mL of a saline-soluble dye. Anatomic dissection was performed to document staining (deeply, faintly, or not stained) of the anterior cutaneous branches of the thoracoabdominal nerves and determine the extent of the injectate spread of the dye to the intercostal space in the thoracic cage following a simulated M-TAPA. Results: The median (IQR) dermatome of the stained segmental nerve was T10 (T8-T11) and the median (IQR) number of stained segmental nerves was 3 (4-2). The T9, T10 and T11 segmental nerves were stained in 75%, 100% and 62.5% of simulated M-TAPA, respectively. Conversely, the T8 segmental nerve was stained in only 25% of simulated M-TAPA. No injectate spread of dye to the intercostal space in the thoracic cage was observed in eight simulated injections of M-TAPA. Conclusion: Our findings suggest that M-TAPA most likely involves the T9, T10 and T11 segmental nerves and that the local anesthetic may not spread to the intercostal space in the thoracic cage in M-TAPA. Further studies are required to confirm the precise mechanism of action and efficacy of M-TAPA in a large sample of human participants.

Abstract:Objective To observe the distribution of local anesthetics by means of CT and analyze the effects of interscalene brachial plexus block.Methods Sixty patients scheduled for upper extremity operations were randomized to two groups(n=30 in each group): group NPAR(non-paresthesia),and group PNS(peripheral nerve stimulator).The extent of sensory and motor blockade of each innervated region was assessed by scale scores,the anesthetic effects of surgical field was observed and recorded as well.CT scan was performed from C4 to T3 to reconstruct,at sagittal and coronal planes,the interscalene injected with anesthetics(8 cases in each group,randomly),and to measure the range of the distribution of local anesthetics.Results The diffusion of local anesthetic was confined in brachial plexus sheath,the sheath was intact at interscalene level,the septa was seen in other place.There was no significant difference in the range of the distribution of local anesthetics in superior or median trunks of brachial plexus between the two groups,but the range in inferior trunks was significantly greater in group PNS than in group NPAR(P<0.05).There was no significant difference in sensory or motor scale scores of axillary nerve,musculocutaneous nerve,median nerve and musculospiral nerve.The sensory scale scores of cutaneous antebrachii medialis nerve and cubital nerve and the motor scores of cubital nerve were significantly higher in group NPAR than in group PNS(P<0.01).Conclusion The brachial plexus sheath is a multicompartmented structure,the location and length of injection affects directly the effects of the brachial plexus block,in ulnar operations PNS is more effective.
Comparison between ultrasound-guided multi-injection intertransverse process and thoracic paravertebral blocks for major breast cancer surgery: a randomized non-inferiority trial

Abstract:Background This study investigated whether a novel multi-injection intertransverse process block could provide non-inferior analgesia and recovery quality following major breast cancer surgery compared with the multi-injection thoracic paravertebral block. Methods Eighty-eight females who underwent mastectomy plus sentinel or axillary lymph node dissection were randomized to receive either intertransverse process block or thoracic paravertebral block, both performed at T2–6 with 5 mL of 0.5% ropivacaine per level. The primary outcome was the worst resting pain score (11-point Numerical Rating Scale) within 30 min in the recovery room. The secondary outcome was recovery quality (15-item quality of recovery scale) 24 hours after surgery, which was tested following a gatekeeping procedure. Results The worst resting pain scores were 0 (0, 1) in the intertransverse process block group vs 0.5 (0, 2) in the thoracic paravertebral block group, with a median difference of 0 (95% CI 0 to 0); the upper 95% CI limit was lower than the prespecified non-inferiority margin of 1 point (non-inferiority p<0.001). Aggregate scores of recovery quality at 24 hours postoperatively were 137.5 (126.5, 142.8) and 137.5 (127.8, 145.0) for the intertransverse process and thoracic paravertebral block groups, respectively, with a median difference of −1 (95% CI −6 to 3); the lower 95% CI limit was larger than the prespecified non-inferiority margin of −8 (non-inferiority p=0.006). Conclusions Compared with a multi-injection thoracic paravertebral block, the multi-injection intertransverse process block provided non-inferior analgesia within 30 min in the recovery room and recovery quality at 24 hours following major breast cancer surgery in females. Abstract:In mice, three Gli genes are thought to mediate sonic hedgehog (Shh) signaling collectively. Mis-expression studies and analysis of null mutants for each gene have indicated that the Gli proteins have different functions. In particular, Gli1 appears to be a constitutive activator, and Gli2 and Gli3 have repressor functions. To determine the precise functional differences between Gli1 and Gli2, we have expressed Gli1 in place of Gli2 from the endogenous Gli2 locus in mice. Strikingly, a low level of Gli1 can rescue all the Shh signaling defects in Gli2 mutants; however, only in the presence of a wild-type Shh gene. These studies demonstrate that only the activator function of Gli2 is actually required, and indicates that in specific situations, Shh can modulate the ability of Gli1 to activate target genes. Furthermore, expression of both copies of Gli1 in place of Gli2 does not disrupt spinal cord patterning, but does result in new gain-of-function defects that lead to lethality. We show that the defects are enhanced when Gli3 function is reduced, demonstrating that an important difference between Gli1 and Gli2 is the ability of Gli1 to antagonize Gli3 function.
Comparison of postoperative analgesia by thoracoscopic-guided thoracic paravertebral block and thoracoscopic-guided intercostal nerve block in uniportal video-asssited thoracic surgery: a prospective randomized controlled trial

Abstract:Background: Thoracoscopic-guided thoracic paravertebral nerve block (TG-TPVB) and thoracoscopic-guided intercostal nerve block (TG-INB) are two postoperative analgesia technology for thoracic surgery. This study aims to compared the analgesic effect of TG-TPVB and TG-INB after uniportal video-asssited thoracic surgery (UniVATS). Methods: Fifty-eight patients were randomly allocated to the TG-TPVB group and the TG-INB group. The surgical time of nerve block, the visual analog scale (VAS) scores, the consumption of sufentanil and the number of patient-controlled intravenous analgesic (PCIA) presses within 24 h after surgery, the incidence of adverse reactions were compared between the two groups. Results: The VAS scores were significantly lower during rest and coughing at 2, 6, 12, and 24 h in the TG-TPVB group than in the TG-INB group (P < 0.05). The consumption of sufentanil and the number of PCIA presses within 24 h after surgery were significantly lower in the TG-TPVB group than in the TG-INB group (P < 0.001).The surgical time of nerve block was significantly shorter in the TG-TPVB group than in the TG-INB group (P < 0.001). The incidence of bleeding at the puncture point was lower in the TG-TPVB group than that in the TG-INB group (P < 0.05). Conclusion: TG-TPVB demonstrated superior acute pain relieve after uniVATS, shorter surgical time and non-inferior adverse effects than TG-INB.

Abstract:Background: Pectoralis I and II (Pecs I/Pecs II) blocks are modern regional anesthetic techniques performed in combination to anesthetize the nerves involved in breast surgery and axillary node dissection. Pecs II spread and clinical efficacy is thought to be independent of whether injection occurs between pectoralis minor and serratus anterior or deep to serratus anterior. Injecting deep to serratus anterior onto the rib may be technically easier; however, our clinical experience suggests that this approach may be less effective for axillary dissection. We undertook a cadaveric study to evaluate a subserratus plane approach for use in breast and axillary surgery. Methods: Ultrasound-guided blocks using methylene blue dye were performed on 4 Genelyn-embalmed cadavers to assess and compare dye spread after a conventional Pecs II and a subserratus plane block at the third rib. Results: Conventional Pecs II injection demonstrated staining of the intercostobrachial nerve, third intercostal nerve, thoracodorsal nerve, long thoracic nerve, medial pectoral, and lateral pectoral nerve. The subserratus plane produced significantly less axillary spread, incomplete staining of the medial pectoral, and very minimal staining of the lateral pectoral nerve. Dye spread was limited to the lateral cutaneous branches of the intercostal nerves in both injections. Conclusions: In our cadaveric study, injecting deep to serratus plane produced significantly less axillary spread. For breast surgery excluding the axilla, both techniques may be effective; however, for axillary dissection, the conventional Pecs II is likely to produce superior analgesia and additionally may help achieve complete coverage of the deeper pectoral nerve branches.
Effect of viral infection of Fc receptors and immunologically nonspecific receptors of blood polymorphonuclear leukocytes: an in vitro model.

Abstract:I describe the effect on phagocytic activity of the migration of bovine blood polymorphonuclear leukocytes (PMNL) through collagen and a monolayer of PK-15 cells in vitro infected with rotaviruses (strain OSU, RFC-17, SA-11), a picornavirus (strain SVDV), bovine herpesvirus (BHV-1) and equine herpesvirus (EHV-1). The PMNL were examined before and after their migration as follows: (1) for EA rosette formation with sensitized sheep erythrocytes, (2) for phagocytic activity mediated through Fc receptors (FcRs), (3) for phagocytic activity mediated through immunologically nonspecific receptors (INsRs). Random migration of PMNL through the infected monolayer to RPMI-1640 medium containing or lacking chemotactic agents (zymosan activated serum--Act. serum) and/or containing control agents (lipopolysaccharide--LPS or formyl-L-methionyl-L-leucyl-L-phenylalanine--fMLP) decreased the percentage of rosettes forming PMNL and the phagocytosis mediated by FcRs and INsRs. The results obtained suggest that the viruses used in this study independently of their biological properties exerted a suppressive action on the phagocytic activity mediated by FcRs and INsRs.
