Abstract:Accurate force control plays a crucial role but still faces many challenges in functional electrical stimulation (FES) based neuroprosthesis applications. This study proposes an surface electromyography (sEMG) driven solution for FES sequential parameter estimation including its amplitude and pulsewidth (PW). The algorithm forms the mapping relationship from voluntary sEMG to the two FES parameters via the correlated muscle forces as an intermediate variable. Six able-bodied subjects are recruited to optimize and validate the proposed method based on the triangular and random grip force. The estimated amplitude and PW are evaluated respectively by stimulating the ipsilateral and contralateral muscle of the sEMG recording points. The accuracy of muscle force reconstruction achieve by the method are investigated by comparing the FES-induced grip force and the originally recorded voluntary grip force (VGF), in terms of the correlation index ($R$). The experimental results reveals excellent performance of the method on inducing functional muscle contraction closely matched the VGF ($R>0.9$), regardless of the estimated amplitudes or PW. Furthermore, the performance differences between the estimated PW and amplitude are not significant ($p>0.05$). These results not only demonstrate the feasibility of the proposed proportional FES control method, but also confirms its practical potential for muscle force control and strengthening in neuroprosthesis applications.

Abstract:Surface functional electrical stimulation (FES) is a well-recognized technique for motor function rehabilitation of some paralyzed patients. Finding the proper positions of the electrodes on the forearm for selective control of finger movements is a challenge for FES applications. This work developed a FES system towards hand rehabilitation based on customised high-density electrodes in order to achieve optimal selective stimulation. The proposed system is composed of a customised multi-channel electronic stimulator, an electrode array of 32 electrode pads with each electrode pad size of 1.2 cm $times1.5$ cm, and motion feedback sensors. An auto-calibration algorithm was developed to determine the optimal stimulation parameters and electrode pads automatically. The algorithm was based on the prior knowledge gained through manual stimulation. The FES system was tested on eight healthy subjects. The results indicated that the auto-calibration algorithm could obtain the optimal electrode pads for the activation of at least three fingers for six subjects in an isolated manner, and the time required for optimal electrode pads and stimulation parameters detection was significantly reduced, demonstrating the potential for finger movement control in hand rehabilitation.

Real-Time Closed-Loop Functional Electrical Stimulation Control of Muscle Activation with Evoked Electromyography Feedback for Spinal Cord Injured Patients

Abstract:Functional electrical stimulation (FES) is a neuroprosthetic technique to help restore motor function of spinal cord-injured (SCI) patients. Through delivery of electrical pulses to muscles of motor-impaired subjects, FES is able to artificially induce their muscle contractions. Evoked electromyography (eEMG) is used to record such FES-induced electrical muscle activity and presents a form of [Formula: see text]-wave. In order to monitor electrical muscle activity under stimulation and ensure safe stimulation configurations, closed-loop FES control with eEMG feedback is needed to be developed for SCI patients who lose their voluntary muscle contraction ability. This work proposes a closed-loop FES system for real-time control of muscle activation on the triceps surae and tibialis muscle groups through online modulating pulse width (PW) of electrical stimulus. Subject-specific time-variant muscle responses under FES are explicitly reflected by muscle excitation model, which is described by Hammerstein system with its input and output being, respectively, PW and eEMG. Model predictive control is adopted to compute the PW based on muscle excitation model which can online update its parameters. Four muscle activation patterns are provided as desired control references to validate the proposed closed-loop FES control paradigm. Real-time experimental results on three able-bodied subjects and five SCI patients in clinical environment show promising performances of tracking the aforementioned reference muscle activation patterns based on the proposed closed-loop FES control scheme.
Hybrid Cooperative Control of Functional Electrical Stimulation and Robot Assistance for Upper Extremity Rehabilitation

Abstract:Objective: Hybrid systems that integrate Functional Electrical Stimulation (FES) and robotic assistance have been proposed in neurorehabilitation to enhance therapeutic benefits. This study focuses on designing a cooperative controller capable of distributing the required torque for movement between robotic actuation and FES, thereby eliminating the need for time-consuming calibration procedures. Methods: The control schema comprises three main blocks: a motion generation block that defines the desired trajectory, a motor control block including both a weight compensation feedforward and a feedback impedance controller, and an FES control block, based on trial-by-trial Iterative Learning Control (ILC), that adjusts the stimulation intensity according to a predefined stimulation waveform. The feedforward motor assistance can be dynamically regulated using an allocation factor. Experiments involving 12 healthy volunteers were conducted using a one-degree-of-freedom elbow testbed. Results: The experimental results showcased the successful integration of Functional Electrical Stimulation (FES) with robotic actuation, ensuring precise trajectory tracking with a Root Mean Square Error (RMSE) below 7°. Notably, allocating more torque to FES led to a 51 % reduction in motor torque. In conditions where FES operated alone, there was poorer tracking performance with an RMSE of 24° and an early onset of muscle fatigue, as evidenced by a reduced number of achieved repetitions. Furthermore, the hybrid approach enabled 100 fatigue-free elbow flexion repetitions, underscoring the effectiveness of cooperative FES-motor control in extending the benefits of FES-induced exercises. Significance: This study proposes a flexible approach which can be extended to a multi-degree-of-freedom hybrid system. Furthermore, it underscores the significance of employing a straightforward and adaptable methodology with a rapid calibration procedure, making it easily transferable to clinical applications.

Abstract:Functional Electrical Stimulation is a promising approach to treat patients by stimulating the peripheral nerves and their corresponding motor neurons using electrical current. This technique helps maintain muscle mass and promote blood flow in the absence of a functioning nervous system. The goal of this work is to control muscle contractions from FES via three different algorithms and assess the most appropriate controller providing effective stimulation of the muscle. An open-loop system and a closed-loop system with three types of model-free feedback controllers were assessed for tracking control of skeletal muscle contractions: a Proportional-Integral (PI) controller, a Model Reference Adaptive Control algorithm, and an Adaptive Augmented PI system. Furthermore, a mathematical model of a muscle-mass-spring system was implemented in simulation to test the open-loop case and closed-loop controllers. These simulations were carried out and then validated through experiments ex vivo. The experiments included muscle contractions following four distinct trajectories: a step, sine, ramp, and square wave. Overall, the closed-loop controllers followed the stimulation trajectories set for all the simulated and tested muscles. When comparing the experimental outcomes of each controller, we concluded that the Adaptive Augmented PI algorithm provided the best closed-loop performance for speed of convergence and disturbance rejection.

Improving reaching with functional electrical stimulation by incorporating stiffness modulation

Abstract:Objective.Intracortical recordings have now been combined with functional electrical stimulation (FES) of arm/hand muscles to demonstrate restoration of upper-limb function after spinal cord injury. However, for each desired limb position decoded from the brain, there are multiple combinations of muscle stimulation levels that can produce that position. The objective of this simulation study is to explore how modulating the amount of coactivation of antagonist muscles during FES can impact reaching performance and energy usage. Stiffening the limb by cocontracting antagonist muscles makes the limb more resistant to perturbation. Minimizing cocontraction saves energy and reduces fatigue.Approach.Prior demonstrations of reaching via FES used a fixed empirically-derived lookup table for each joint that defined the muscle stimulation levels that would position the limb at the desired joint angle decoded from the brain at each timestep. This study expands on that previous work by using simulations to: (a) test the feasibility of controlling arm reaching using asuiteof lookup tables with varying levels of cocontraction instead of a single fixed lookup table for each joint, (b) optimize a simple function for automatically switching between these different cocontraction tables using only the desired kinematic information already being decoded from the brain, and (c) compare energy savings and movement performance when using the optimized function to automatically modulate cocontraction during reaching versus using the best fixed level of cocontraction.Main results.Our data suggests energy usage and/or movement performance can be significantly improved by dynamically modulating limb stiffness using our multi-table method and a simple function that determines cocontraction level based on decoded endpoint speed and its derivative.Significance.By demonstrating how modulating cocontraction can reduce energy usage while maintaining or even improving movement performance, this study makes brain-controlled FES a more viable option for restoration of reaching after paralysis.
Revolutionizing motor dysfunction treatment: A novel closed-loop electrical stimulator guided by multiple motor tasks with predictive control

Abstract:Functional electrical stimulation (FES) has been demonstrated as a viable method for addressing motor dysfunction in individuals affected by stroke, spinal cord injury, and other etiologies. By eliciting muscle contractions to facilitate joint movements, FES plays a crucial role in fostering the restoration of motor function compromised nervous system. In response to the challenge of muscle fatigue associated with conventional FES protocols, a novel biofeedback electrical stimulator incorporating multi-motor tasks and predictive control algorithms has been developed to enable adaptive modulation of stimulation parameters. The study initially establishes a Hammerstein model for the stimulated muscle group, representing a time-varying relationship between the stimulation pulse width and the root mean square (RMS) of the surface electromyography (sEMG). An online parameter identification algorithm utilizing recursive least squares is employed to estimate the time-varying parameters of the Hammerstein model. Predictive control is then implemented through feedback corrections based on the comparison between predicted and actual outputs, guided by an optimization objective function. The integration of predictive control and roll optimization enables closed-loop control of muscle stimulation. The motor training tasks of elbow flexion and extension, wrist flexion and extension, and five-finger grasping were selected for experimental validation. The results indicate that the model parameters were accurately identified, with a RMS error of 3.83 % between actual and predicted values. Furthermore, the predictive control algorithm, based on the motor tasks, effectively adjusted the stimulus parameters to ensure that the stimulated muscle groups can achieve the desired sEMG characteristic trajectory. The biofeedback electrical stimulator that was developed has the potential to assist patients experiencing motor dysfunction in achieving the appropriate joint movements. This research provides a foundation for a novel intelligent electrical stimulation model.

Abstract:Recent dynamical models, based on the seminal work of V. Hill, allow to predict the muscular response to functional electrostimulation (FES), in the isometric and non-isometric cases. The physical controls are modeled as Dirac pulses and lead to a sampled-data control system, sampling corresponding to times of the stimulation, where the output is the muscular force response. Such a dynamics is suitable to compute optimized controls aiming to produce a constant force or force strengthening, but is complex for real time applications. The objective of this article is to construct a finite dimensional approximation of this response to provide fast optimizing schemes, in particular for the design of a smart electrostimulator for muscularreinforcement or rehabilitation. It is an on-going industrial project based on force-fatigue models, validated by <a class="link-external link-http" href="http://experiments.Moreover" rel="external noopener nofollow">this http URL</a> it opens the road to application of optimal control to track a reference trajectory in the joint angular variable to produce movement in the non-isometric models.

A Co-driven Functional Electrical Stimulation Control Strategy by Dynamic Surface Electromyography and Joint Angle

Abstract:Functional electrical stimulation (FES) is widely used in neurorehabilitation to improve patients' motion ability. It has been verified to promote neural remodeling and relearning, during which FES has to produce an accurate movement to obtain a good efficacy. Therefore, many studies have focused on the relationship between FES parameters and the generated movements. However, most of the relationships have been established in static contractions, which leads to an unsatisfactory result when applied to dynamic conditions. Therefore, this study proposed a FES control strategy based on the surface electromyography (sEMG) and kinematic information during dynamic contractions. The pulse width (PW) of FES was determined by a direct transfer function (DTF) with sEMG features and joint angles as the input. The DTF was established by combing the polynomial transfer functions of sEMG and joint torque and the polynomial transfer functions of joint torque and FES. Moreover, the PW of two FES channels was set based on the muscle synergy ratio obtained through sEMG. A total of six healthy right-handed subjects were recruited in this experiment to verify the validity of the strategy. The PW of FES applied to the left arm was evaluated based on the sEMG of the right extensor carpi radialis (ECR) and the right wrist angle. The coefficient of determination ( R 2 ) and the normalized root mean square error (NRMSE) of FES-included and voluntary wrist angles and torques were used to verify the performance of the strategy. The result showed that this study achieved a high accuracy ( R 2 = 0.965 and NRMSE = 0.047) of joint angle and a good accuracy ( R 2 = 0.701 and NRMSE = 0.241) of joint torque reproduction during dynamic movements. Moreover, the DTF in real-time FES system also had a nice performance of joint angle fitting ( R 2 = 0.940 and NRMSE = 0.071) and joint torque fitting ( R 2 = 0.607 and NRMSE = 0.303). It is concluded that the proposed strategy is able to generate proper FES parameters based on sEMG and kinematic information for dynamic movement reproduction and can be used in a real-time FES system combined with bilateral movements for better rehabilitation.

Abstract:Closed-loop neuroprostheses show promise in restoring motion in individuals with neurological conditions. However, conventional activation strategies based on functional electrical stimulation (FES) fail to accurately modulate muscle force and exhibit rapid fatigue because of their unphysiological recruitment mechanism. Here, we present a closed-loop control framework that leverages physiological force modulation under functional optogenetic stimulation (FOS) to enable high-fidelity muscle control for extended periods of time (>60 minutes) in vivo. We first uncovered the force modulation characteristic of FOS, showing more physiological recruitment and significantly higher modulation ranges (>320%) compared with FES. Second, we developed a neuromuscular model that accurately describes the highly nonlinear dynamics of optogenetically stimulated muscle. Third, on the basis of the optogenetic model, we demonstrated real-time control of muscle force with improved performance and fatigue resistance compared with FES. This work lays the foundation for fatigue-resistant neuroprostheses and optogenetically controlled biohybrid robots with high-fidelity force modulation.

Abstract:Functional electrical stimulation (FES) has been proven to be a reliable rehabilitation technique that increases muscle strength, reduces spasms, and enhances neuroplasticity in the long term. However, the available electrical stimulation systems on the market produce stimulation signals with no personalized voltage–current amplitudes, which could lead to muscle fatigue or incomplete enforced therapeutic motion. This work proposes an FES system aided by machine learning strategies that could adjust the stimulating signal based on electromyography (EMG) information. The regulation of the stimulated signal according to the patient's therapeutic requirements is proposed. The EMG signals were classified using Long Short-Term Memory (LSTM) and a least-squares boosting ensemble model with an accuracy of 91.87% and 84.7%, respectively, when a set of 1200 signals from six different patients were used. The classification outcomes were used as input to a second regression machine learning algorithm that produced the adjusted electrostimulation signal required by the user according to their own electrophysiological conditions. The output of the second network served as input to a digitally processed electrostimulator that generated the necessary signal to be injected into the extremity to be treated. The results were evaluated in both simulated and robotized human hand scenarios. These evaluations demonstrated a two percent error when replicating the required movement enforced by the collected EMG information.

Abstract:Human movement disorders or paralysis lead to the loss of control of muscle activation and thus motor control. Functional Electrical Stimulation (FES) is an established and safe technique for contracting muscles by stimulating the skin above a muscle to induce its contraction. However, an open challenge remains on how to restore motor abilities to human limbs through FES, as the problem of controlling the stimulation is unclear. We are taking a robotics perspective on this problem, by developing robot learning algorithms that control the ultimate humanoid robot, the human body, through electrical muscle stimulation. Human muscles are not trivial to control as actuators due to their force production being non-stationary as a result of fatigue and other internal state changes, in contrast to robot actuators which are well-understood and stationary over broad operation ranges. We present our Deep Reinforcement Learning approach to the control of human muscles with FES, using a recurrent neural network for dynamic state representation, to overcome the unobserved elements of the behaviour of human muscles under external stimulation. We demonstrate our technique both in neuromuscular simulations but also experimentally on a human. Our results show that our controller can learn to manipulate human muscles, applying appropriate levels of stimulation to achieve the given tasks while compensating for advancing muscle fatigue which arises throughout the tasks. Additionally, our technique can learn quickly enough to be implemented in real-world human-in-the-loop settings.

Abstract:This closed‐loop wearable device network, which incorporates intrinsically‐controlled, bilateral coordinated functional electrical stimulation, utilizes electromyographic signals from the rectus femoris and tibialis anterior muscles for activation and modulation of functional electrical stimulation, aims to aid walking at ankle joint angles, alleviates oxyhemoglobin burden, and enhance motor function during clinical interventions for stroke‐induced foot drop patients. Innovative functional electrical stimulation has demonstrated effectiveness in enhancing daily walking and rehabilitating stroke patients with foot drop. However, its lack of precision in stimulating timing, individual adaptivity, and bilateral symmetry, resulted in diminished clinical efficacy. Therefore, a closed‐loop wearable device network of intrinsically controlled functional electrical stimulation (CI‐FES) system is proposed, which utilizes the personal surface myoelectricity, derived from the intrinsic neuro signal, as the switch to activate/deactivate the stimulation on the affected side. Simultaneously, it decodes the myoelectricity signal of the patient's healthy side to adjust the stimulation intensity, forming an intrinsically controlled loop with the inertial measurement units. With CI‐FES assistance, patients' walking ability significantly improved, evidenced by the shift in ankle joint angle mean and variance from 105.53° and 28.84 to 102.81° and 17.71, and the oxyhemoglobin concentration tested by the functional near‐infrared spectroscopy. In long‐term CI‐FES‐assisted clinical testing, the discriminability in machine learning classification between patients and healthy individuals gradually decreased from 100% to 92.5%, suggesting a remarkable recovery tendency, further substantiated by performance on the functional movement scales. The developed CI‐FES system is crucial for contralateral‐hemiplegic stroke recovery, paving the way for future closed‐loop stimulation systems in stroke rehabilitation is anticipated.

Abstract:Introduction: Functional electrical stimulation (FES) is an established method of supporting neurological rehabilitation. However, particularly on the forearm, it still cannot elicit selective muscle activations that form the basis of complex hand movements. Current research approaches in the context of selective muscle activation often attempt to enable targeted stimulation by increasing the number of electrodes and combining them in electrode arrays. In order to determine the best stimulation positions and settings, manual or semi-automated algorithms are used. This approach is limited due to experimental limitations. The supportive use of simulation studies is well-established, but existing simulation models are not suitable for analyses of selective muscle activation due to missing or arbitrarily arranged innervation zones. Methods: This study introduces a new modeling method to design a person-specific digital twin that enables the prediction of muscle activations during FES on the forearm. The designed individual model consists of three parts: an anatomically based 3D volume conductor, a muscle-specific nerve fiber arrangement in various regions of interest (ROIs), and a standard nerve model. All processes were embedded in scripts or macros to enable automated changes to the model and the simulation setup. Results: The experimental evaluation of simulated strength–duration diagrams showed good coincidence. The relative differences of the simulated amplitudes to the mean amplitude of the four experiments were in the same range as the inter-experimental differences, with mean values between 0.005 and 0.045. Based on these results, muscle-specific activation thresholds were determined and integrated into the simulation process. With this modification, simulated force-intensity curves showed good agreement with additionally measured curves. Discussion: The results show that the model is suitable for simulating realistic muscle-specific activations. Since complex hand movements are physiologically composed of individual, selective muscle activations, it can be assumed that the model is also suitable for simulating these movements. Therefore, this study presents a new and very promising approach for developing new applications and products in the context of the rehabilitation of sensorimotor disorders.

Thus we can obtain appropriate internal representations, depending on the given targets. The information mediators were applied to multi-level information processing. In this processing information minimization is related to information maximization through the information mediators. Experimental results on language acquisition problems show that simple and well-interpretable internal representations can be obtained by the information mediators.

Abstract:Rehabilitation of paralysis caused by a stroke or a spinal cord injury remains a complex and time-consuming task. This work proposes a hybrid exoskeleton approach combining a traditional exoskeleton and functional electrical stimulation (FES) as a promising method in rehabilitation. However, hybrid exoskeletons with a closed-loop FES control strategy are functionally challenging to achieve and have not been reported often. Therefore, this study aimed to investigate a powered lower-limb exoskeleton with a closed-loop FES control for Sit-to-Stand (STS) movements. A body motion capture system was applied to record precise hip and knee trajectories of references for establishing the human model. A closed-loop control strategy with allocation factors is proposed featuring a two-layer cascaded proportional–integral–derivative (PID) controller for both FES and exoskeleton control. Experiments were performed on two participants to examine the feasibility of the hybrid exoskeleton and the closed-loop FES control. Both open- and closed-loop FES control showed the desired performance with a relatively low root-mean-squared error (max 1.3∘ in open-loop and max 4.1∘ in closed-loop) in hip and knee trajectories. Notably, the closed-loop FES control strategy can achieve the same performance with nearly 60% of the electrical power input compared to the open-loop control, which reduced muscle fatigue and improved robustness during the training. This study provides novel insights into body motion capture application and proposes a closed-loop FES control for hybrid exoskeletons.

Abstract:Functional electrical stimulation (FES) is the most commonly used technology for improving motor function in individuals who have spinal cord injury. Despite the wide range of FES applications reported in the literature, few electrical stimulation systems that can generate meaningful functional outcomes are currently available for use outside research laboratories. We tested proportional-integral-derivative, gain scheduling, and sliding mode control closed-loop control algorithms in a simulation of electrically induced knee extension against gravity to uncover some of the reasons why closed-loop control is not being more widely used in real-world FES systems. We also subjected the simulated FES system to muscle fatigue, muscle spasms, and the effects of muscle retraining. All of the controllers exhibited significantly degraded performance when these real-world nonlinear effects were included in the simulation. Moreover, all of the controllers were sensitive to variation in the parameters of the muscle recruitment function, which are subject to change during real-world FES use. We suggest several ways to improve the performance of closed-loop control algorithms for use in FES applications. We believe that closed-loop controllers have an important place in future FES applications, but the performance of these algorithms must be greatly improved before they can be implemented in real-world systems.

Abstract:Communication skills are key to the workforce success of engineering graduates. The Stewart School of Industrial and Systems Engineering (ISyE) Workforce Communication Program at Georgia Tech has successfully incorporated executive panel interaction into its capstone design course to align student skills with executive expectations. The objectives of the panel are to raise student awareness about the importance of communication to workforce success and to gain knowledge about communication skills directly from executives. Executives interact directly with students about workforce communication, career advancement, and the communication skills they consider most critical. The process of assembling and holding a panel is described for potential implementation in other engineering programs.

Abstract:Muscle fatigue limits the effectiveness of FES when applied to regain functional movements in spinal cord injured (SCI) individuals. The stimulation intensity must be manually increased to provide more force output to compensate for the decreasing muscle force due to fatigue. An artificial neural network (ANN) system was designed to compensate for muscle fatigue during functional electrical stimulation (FES) by maintaining a constant joint angle. Surface electromyography signals (EMG) from electrically stimulated muscles were used to determine when to increase the stimulation intensity when the muscle's output started to drop. In two separate experiments on able-bodied subjects seated in hard back chairs, electrical stimulation was continuously applied to fatigue either the biceps (during elbow flexion) or the quadriceps muscle (during leg extension) while recording the surface EMG. An ANN system was created using processed surface EMG as the input, and a discrete fatigue compensation control signal, indicating when to increase the stimulation current, as the output. In order to provide training examples and test the systems' performance, the stimulation current amplitude was manually increased to maintain constant joint angles. Manual stimulation amplitude increases were required upon observing a significant decrease in the joint angle. The goal of the ANN system was to generate fatigue compensation control signals in an attempt to maintain a constant joint angle. On average, the systems could correctly predict 78.5% of the instances at which a stimulation increase was required to maintain the joint angle. The performance of these ANN systems demonstrates the feasibility of using surface EMG feedback in an FES control system.
