Abstract:Background: The optimal surgical strategy for early-stage non-small cell lung cancer (NSCLC) with visceral pleural invasion (VPI) remains unclear. Due to limited prospective comparative data for these surgical modalities, the objective of the current study was to compare the long-term survival outcomes of sublobectomy (Sub) versus lobectomy (Lob) for NSCLC with a tumor size <= 2 cm and VPI. Methods: Patients with early-stage NSCLC characterized by VPI diagnosed between 2004 and 2013 were identified from the Surveillance, Epidemiology, and End Results (SEER) program. The baseline demographic and cancer characteristics, treatment information as well as survival outcome data were extracted from the SEER database, and confounders were balanced by propensity score matching (PSM) and inverse probability of treatment weighting (IPTW) analyses. Lung disease-specific survival (DSS) and overall survival (OS) rates were compared with Cox proportional hazards (PH) regression models based on the unmatched cohort, the propensity-based matched cohort, and the IPTW cohort. Results: Of the 1,386 patients enrolled, 1,000 (72.15%) and 386 (27.85%) underwent lobectomy and sublobectomy, respectively. The 5-year DSS rate was 78.64% for the lobectomy group and 59.47% for the sublobectomy group. Cox regression models demonstrated that the operation type (Sub vs. Lob) was an independent prognostic factor for early-stage NSCLC with VPI based on the three different cohorts. Patients who underwent lobectomy showed better long-term DSS and OS rates than those treated with sublobectomy after PSM [DSS: hazard ratio (II R) 0.689, 95% confidence interval (CI): 0.490-0.968, P=0.032; OS: HR 0.723, 95% CI: 0.549-0.953, P=0.021]. The IPTW analysis yielded similar results. Conclusions: Lobectomy showed superior long-term survival compared with sublobectomy in patients with early-stage NSCLC with a tumor size <= 2 cm and VPI.

Abstract:BACKGROUND:Recurrence of early gastric cancer (EGC) after curative resection is rare, and the types of EGC that may recur have not been well studied. We attempted to create a system for predicting recurrence of EGC after R0 resection.METHODS:From January 1987 to April 2005, 2,923 patients with EGC who underwent curative resection were retrospectively studied. Of them, 79 patients (2.7%) experienced recurrence. Logistic regression was performed to identify independent risk factors for overall recurrence and early recurrence (recurred within 24 months after resection) of EGC. A nomogram was developed on the basis of a Cox regression.RESULTS:Median time to recurrence was 20.5 months, and early recurrence accounted for 60.7% of instances. Presence of lymph node metastasis and elevated gross type were independent risk factors for overall recurrence; patients with both identified risk factors had a higher recurrence rate than average level (17.5% vs. 2.7%, P < 0.001). Meanwhile, male gender, elevated gross type, and presence of lymph node metastasis were significantly associated with early recurrence, and in patients with all of the aforementioned identified risk factors, the early recurrence rate was higher (12.2% vs. 1.6%, P < 0.001). A nomogram for predicting the disease-free survival after operation was constructed. Its c-index was 0.79 and it appeared to be accurate.CONCLUSIONS:Recurrence of EGC after curative resection can be predicted by using common clinical characteristics. Patients at high risk of overall and early recurrence could be identified; individual disease-free survival was predictable by the internally validated nomogram.
Sublobar Resection Is Associated With Better Perioperative Outcomes In Elderly Patients With Clinical Stage I Non-Small Cell Lung Cancer: A Multicenter Retrospective Cohort Study

Abstract:Background: Sublobar resection has emerged as an alternative to lobectomy for management of early-stage non-small cell lung cancer (NSCLC). However, controversy remains as to whether it is adequate for elderly patients. The present study aimed to comparatively study the perioperative outcomes and overall survival of sublobar resection vs. lobectomy for management of elderly patients (>= 65 years) with clinical stage I NSCLC.Methods: This is a multicenter retrospective cohort study. Clinical stage I NSCLC patients who underwent lobar or sublobar resection (segmentectomy and wedge resection) at the Department of Thoracic Surgery of 10 tertiary hospitals between January 2014 and September 2017 were retrospectively reviewed from the national collaborative prospective lung cancer database (LinkDoc Technology Co, Ltd., Beijing, China). Clinical data on demographic and tumor characteristics, surgical details were collected. Perioperative outcomes and overall survival were analyzed by using propensity score matching to adjust for selection bias. Subgroup analysis was further carried out to explore the potential sources of heterogeneity.Results: Among the 1,579 eligible patients, 1,164 (73.7%) underwent lobectomy and 415 (26.3%) underwent sublobar resection (106 segmentectomy and 309 wedge resection). Sublobar resection was more frequently performed in patients who were elder, had more comorbidities and smaller, left-sided adenocarcinoma (P<0.001). Propensity-matched analysis showed significant association of sublobar resection with less blood loss, shorter operation time, chest drainage and hospital stay, while with less lymph node removal when compared with lobectomy (P<0.001). Short term survival analysis showed comparable results even after adjusted in the matched analysis. Similar results were obtained when limiting patients to those aged >75 years, at pathologic stage I, and those who smoking or undergoing video-assisted thoracoscopic surgery (VATS) or segmentectomy and lobectomy.Conclusions: Sublobar resection was associated with significantly better perioperative outcomes without compromising short term survival in elderly patients with clinical stage I NSCLC. However, the importance of patient selection and management process, as well as accurate lymph node staging must be acknowledged when making the surgical decision.

Abstract:e20534 Background: Even though anatomical lobectomy with dissection of systematic mediastinal lymph nodes has been the standard treatment for the patients with non-small cell lung cancer (NSCLC), the optimal modes of lymphadenectomies in the patients with sublobar resection are still controversial. The purposes of the study are to evaluate effectiveness and safety of lymphadenectomies and explore the clinical characteristics and survivals in the patients with sublobar resection. Methods: The NSCLC patients ( > = 18 years) with sublobar resection and systematic/selective lymphadenectomies from 2014 to 2017 were evaluated based on the LinkDoc database. Systematic lymphadenectomies was defined as ≥ 5 lymph node stations resected and selective lymphadenectomies as < 5. The overall survivals (OS) were measured by the Kaplan-Meier method and the Log-Rank test. Propensity score matching (PSM) method was implemented to reduce selection bias. Results: A total of 3902 NSCLC subjects were recruited including 35.7% (64.3%) of patients with systematic (selective) lymphadenectomy. Of all, 28.8% of patients were performed with segment resection, 67.6% with wedge resection, and 3.6% with mixed resections. In the total population, no difference in OS rates was observed between the groups with systematic and selective lymphadenectomies regardless of PSM. Further subgroup analysis showed that the OS of the patients with systematic lymphadenectomy was superior to that with selective lymphadenectomy in the group of patients with adenocarcinoma (HR of systematics vs selective lymphadenectomies = 0.49, p < 0.05) and in the group of tumor size ≥1& < 3cm (HR of systematic vs selective lymphadenectomies = 0.33, p < 0.05). No significant difference was observed in the subpopulations categorized by age and N staging. Conclusions: As the additional evidence-based data, this real-world study presented clinical evidences on the indications of systematic/selective lymphadenectomies and clinical characteristics of the Chinese NSCLC patients with sublobar resection and lymphadenectomies. Prognostic Impact of Lymphadenectomy on Outcomes of Sublobar Resection for Stage IA Non–small Cell Lung Cancer ≤2 Cm

Abstract:Objective: To investigate the prognostic impact of lymph node (LN) dissection on sublobar resection for stage IA non-small cell lung cancer (NSCLC) tumors <= 2 cm. Methods: All patients who underwent sublobar resection for stage IA NSCLC tumors <= 2 cm were identified from the Surveillance, Epidemiology and End Results database. Patients were classified into no-LN dissection and LN dissection groups, and the latter was subclassified by the extent of LN dissection (1-3 regional LNs or >= 4 regional LNs). These groups were compared in terms of lung cancer-specific survival (LCSS) and overall survival (OS) rates. Propensity score-matched comparative analysis and a Cox regression model were used. Results: A total of 3269 patients met our criteria, including 1459 (44.6%) who underwent no LN dissection, 891 (27.3%) who underwent dissection of 1 to 3 regional LNs, 919 (28.1%) who underwent dissection of >= 4 regional LNs. Compared with no LN dissection, LN dissection was associated with more favorable LCSS and OS rates in patients who underwent sublobar resection for stage IA tumors <= 2 cm. More extensive regional LN dissection (>= 4 regional LNs) was associated with better LCSS and OS rates compared with less extensive regional LN dissection (1 to 3 regional LNs). A multivariable analysis of our patient population revealed independent associations of no LN dissection and less extensive regional LN dissection with poorer LCSS and OS rates compared with overall LN dissection and more extensive regional LN dissection, respectively. Conclusions: This propensity score-matched analysis reveals an association of LN dissection, particularly more extensive regional LN dissection (>= 4 regional LNs), with better survival rates in patients who undergo sublobar resection for stage IA NSCLC tumors <= 2 cm.
Procedure-Specific Risk Prediction for Recurrence in Patients Undergoing Lobectomy or Sublobar Resection for Small (≤2 Cm) Lung Adenocarcinoma: an International Cohort Analysis.

Abstract:INTRODUCTION:This work was performed to develop and validate procedure-specific risk prediction for recurrence following resection for early-stage lung adenocarcinoma (ADC) and investigate risk prediction utility in identifying patients who may benefit from adjuvant chemotherapy (ACT).METHODS:In patients who underwent resection for small (≤2 cm) lung ADC (lobectomy, 557; sublobar resection, 352), an association between clinicopathologic variables and risk of recurrence was assessed by a competing risks approach. Procedure-specific risk prediction was developed based on multivariable regression for recurrence. External validation was conducted using cohorts (N = 708) from Japan, Taiwan, and Germany. The accuracy of risk prediction was measured using a concordance index. We applied the lobectomy risk prediction approach to a propensity score-matched cohort of patients with stage II-III disease (n = 316, after matching) with or without ACT and compared lung cancer-specific survival between groups among low- or high-risk scores.RESULTS:Micropapillary pattern, solid pattern, lymphovascular invasion, and necrosis were involved in the risk prediction following lobectomy, and micropapillary pattern, spread through air spaces, lymphovascular invasion, and necrosis following sublobar resection. Both internal and external validation showed good discrimination (concordance index in lobectomy and sublobar resection: internal, 0.77 and 0.75, respectively; and external, 0.73 and 0.79, respectively). In the stage II-III propensity score-matched cohort, among high-risk patients, ACT significantly reduced the risk of lung cancer-specific death (subhazard ratio 0.43, p = 0.001), but not among low-risk patients.CONCLUSIONS:Procedure-specific risk prediction for patients with resected small lung ADC can be used to better prognosticate and stratify patients for further interventions.
Postoperative CT surveillance in the evaluation of local recurrence after sub-lobar resection of neoplastic lesions of the lung

Abstract:Objective: As indications for sub-lobar resections increase, it will become more important to identify risk factors for postsurgical recurrence. We investigated retrospectively the association between local recurrence after sub-lobar resection of neoplastic lung lesions and pre- and post-operative CT imaging and pathologic features. Materials and methods: We reviewed retrospectively neoplastic lung lesions with postoperative chest CT surveillance of sub-lobar resections in 2006-2016. We defined "suspicious" findings as nodularity ≥3 mm or soft tissue thickening ≥4 mm along the suture line and/or progression and explored their association with local recurrence. Primary lung cancer stage, tumoral invasion of lymphatics, visceral pleura or large vessels, bronchial and vascular margin distance were also assessed. Results: Our study group included 45 cases of sub-lobar resection took for either primary (n = 37) or metastatic (n = 8) lung tumors. Local recurrence was observed in 16 of those patients. New nodularity ≥3 mm or soft tissue thickening ≥4 mm along the suture line on surveillance CT was significantly associated with local recurrence (p = 0.037). Additionally, solid nodule (p = 0.005), age at surgery ≤60 years (p = 0.006), two or more sites of invasion (p < 0.0001) and poor histologic differentiation (p = 0.0001) were also significantly associated with local tumor recurrence. Of 16 patients with surveillance post-surgical PET-CT, 15 had elevated FDG uptake. Conclusion: The postoperative changes along the suture line should follow a predictable time course demonstrating a pattern of stability, thinning or resolution on CT surveillance. New or increasing postoperative nodularity ≥3 mm or soft tissue thickening ≥4 mm along the suture line requires close diagnostic work-up. Surgical pathology characteristics added prognostic value on postoperative recurrence surveillance.
Sublobar Resection for Pulmonary Aspergilloma: A Safe Alternative to Lobectomy

Abstract:Background. This study was performed to evaluate the effectiveness of sublobar resection for the treatment of pulmonary aspergilloma compared with lobectomy.Methods. Patients with pulmonary aspergilloma who underwent lobectomy or sublobar resection in our department between March 2007 and December 2015 were retrospectively reviewed. Data were collected for patient demographic characteristics, medical history, preoperative investigations, perioperative findings, postoperative conditions, and recurrence status. Propensity-matched comparative analyses were performed to adjust for potential differences of patients' baseline characteristics between the groups.Results. A total of 96 patients underwent lobectomy, 46 patients underwent attempted sublobar resection. The median follow-up time is 53 months. No recurrence was found in either group. Three patients (3.1%) in the lobectomy group required reoperation for bleeding. The patients who underwent sublobar resection had less underlying lung disease (p = 0.031), smaller lesions (p = 0.033), and were more likely to have been treated with video-assisted thoracic surgery (p < 0.001). These differences were eliminated by propensity score matching (46 pairs were successfully matched). Comparative analyses in matched groups demonstrate that there was no marked difference in the volume and duration of chest drainage or the length of postoperative hospital stay. However, the patients with sublobar resection had shorter operation time (p = 0.004), less blood loss (p = 0.042), and less postoperative complication (p = 0.048).Conclusions. Sublobar resection performed for small simple pulmonary aspergilloma and selected complex pulmonary aspergilloma has a low recurrence rate and confers perioperative advantages compared with lobectomy. Comparison of survivals between sublobar resection and lobar resection for patients with clinical stage I non-small cell lung cancer and interstitial lung disease: a propensity score matching analysis

Abstract:Background: Patients with early-stage lung cancer and interstitial lung disease have a poorer prognosis than those without interstitial lung disease. This study aimed to compare the long-term outcomes of lobar and sublobar resections in these patients. Methods: We retrospectively analyzed 138 consecutive patients with clinical stage I non-small cell lung cancer and interstitial lung disease who underwent surgical treatment at two institutions between January 2010 and December 2020. Propensity score matching analysis was performed to adjust for baseline characteristics. Results: Thirty-six patients underwent sublobar resection and 102 underwent lobar resection. The median follow-up was 45.7 months. In all patients, 5-year overall survival (OS) rates were 33.2% and 73.2%, and 5-year recurrence-free survival (RFS) rates were 24.2% and 60.1% in the sublobar and lobar resection groups, respectively (p < 0.01, <0.01). Death due to lung cancer and locoregional recurrence were significantly more frequent in the sublobar resection group than in the lobar resection group (p = 0.034, <0.01, respectively). On propensity score matching analysis, the 5-year OS rates of the 19 matched pairs were 46.3% and 73.2%, and the RFS rates were 31.6% and 67.6% in the sublobar and lobar resection groups, respectively (p = 0.036, <0.01). The Cox proportional hazards model demonstrated a significant association between lobar resection and improved survival (p = 0.047). Conclusion: The patients in the lobar resection group had better survival rates than those in the sublobar resection group. In terms of long-term prognosis, deliberately limited surgery may not be necessary for patients who tolerate lobectomy.
Risk Factors for Recurrence after Complete Resection of Pathological Stage N2 Non-Small Cell Lung Cancer

Abstract:BACKGROUND:Tumor recurrence is the most common cause of treatment failure, especially after complete resection of pathological stage N2 non-small cell lung cancer (NSCLC). In this study, we investigated the clinicopathological characteristics in order to identify independent risk factors for postoperative recurrence.METHODS:Between January 2001 and December 2013, 96 patients who underwent surgical resection for pathological N2 NSCLC were retrospectively reviewed. Recurrence-free survival (RFS) was calculated by the Kaplan-Meier method to explore risk factors, while the Cox proportional hazard model was used to assess independent predictors.RESULTS:The median and five-year RFS rates were 15 months and 27.4%, respectively. Univariate analysis showed a significantly poorer prognosis for non-regional N2 metastasis, more than three metastatic N2 lymph nodes, multiple N2 station, and multiple N2 zone involvement. Multivariate analysis demonstrated that non-regional N2 metastasis (hazard ratio [HR] 1.857, 95% confidence interval [CI] 1.061-3.249, P = 0.030) and more than three metastatic N2 lymph nodes (HR 2.555, 95% CI 1.164-5.606, P = 0.019) were independent risk factors for RFS. Additionally, the incidence of non-regional N2 metastasis was higher in patients with a primary tumor in the left lower (57.1%) or right lower lobe (48.1%), followed by left upper (31.8%), right middle (14.3%) and right upper lobe (7.7%).CONCLUSION:The combination of the distribution and number of metastatic N2 lymph nodes provides a more accurate prediction for N2 NSCLC regarding recurrence. Non-regional N2 metastasis could occur with a primary tumor in any lobe, but occurs more frequently in the lower lobe.

Abstract:BACKGROUND Lobectomy is a standard surgical procedure for peripherally located early-stage non-small cell lung cancers (NSCLC) measuring 2–4 cm. However, it is unclear whether sublobar resections, such as wedge resection and segmentectomy, are effective in treating tumors with driver mutations in epidermal growth factor receptor (EGFR). METHODS We analyzed the clinicopathological findings and surgical outcomes of 1,395 patients with radiologically solid-dominant NSCLC measuring 2–4 cm, without clinical lymph node involvement, who underwent complete resection between 2010 and 2020. The patients, who underwent sublobar resections (n = 231) or lobectomy (n = 1164), were categorized based on their EGFR mutation status and the surgical procedures performed. The follow-up was conducted for a median of 45.3 months. RESULTS The five-year overall survival (OS) rates after sublobar resections (n = 39) were comparable to those after lobectomy (n = 359) in patients with EGFR mutation-positive tumors (80.5% [95% confidence interval (CI), 51.3%–93.2%] vs. 88.8% [95% CI, 84.1%–92.1%], P = 0.16). Multivariable Cox regression analysis of OS revealed that the surgical procedure was an independent prognostic predictor in the entire cohort (hazard ratio (HR):0.6 (0.4–1.0), P = 0.028), while it was not an independent prognostic predictor in patients with EGFR mutated tumors (HR:0.6 (0.2–1.7), P = 0.32). CONCLUSIONS Sublobar resection with a secure surgical margin could be a viable option for appropriately selected patients with peripheral early-stage NSCLC tumors measuring 2-4 cm and harboring EGFR mutations, as it provides comparable OS to that of lobectomy.

Abstract:Background: Segmentectomy for small-sized stage Ia non-small cell lung cancer (NSCLC) may be comparable to lobectomy regarding prognosis and local recurrence. However, the clinical results of wedge resection for such patients are still under debate. In this international multicenter study, we retrospectively studied surgical outcomes of sublobar resections for patients with small-sized stage Ia adenocarcinoma to elucidate whether wedge resection is inferior to segmentectomy for such patients. Methods: Between March 2000 and August 2011, 173 patients underwent segmentectomy (group I), and 181 patients underwent wedge resection (group II) at three institutions in Japan and China. The tumor was defined as Ground glass opacity (GGO) type when the proportion of GGO was equal or more than 50% in HRCT, while solid type was defined as the proportion of GGO less than 50%. Clinicopathologic factors, local recurrence rate, and survival were compared. Results: The two groups were similar in sex, comorbidity rate, and composition of Noguchi type. There was no in-hospital death. Postoperative morbidity rate of group I was significantly higher than that of group II (11.0% vs. 2.2%, P=0.016). Local recurrence rates were similar between group I (4.0%) and group II (4.4%), while no patient with GGO type tumors had local recurrence. Overall and lung cancer-specific survivals were of no significant difference between the two groups. Lung cancer-specific survival rates at 10 years were significantly better in patients with GGO type tumors than in those with solid type tumors (100% vs. 76.9%, P<0.001). In multivariate Cox regression analyses of lung cancer-specific survival of all patients, GGO type turned out to be an independent prognostic factor, while extent of resection did not have any influence. Conclusions: Our data suggests that sublobar resection is an acceptable procedure for small lung adenocarcinomas without nodal involvement, and wedge resection may not be inferior to segmentectomy for small GGO type tumors. Our study also demonstrates that GGO type is an independent prognostic factor of disease-free survival for small-sized (diameter <= 2.0 cm) stage Ia lung adenocarcinomas.

Abstract:In order to determine the potential for natural selection to promote genetic specialization to different environments, this study quantified genetic variation for response to conspecific density and to other aspects of the environment that vary spatially. The progeny of a random collection of Salvia lyrata, a perennial herb, were planted into the source field in a range of densities and into several closely neighboring locations. The highest density and particular locations induced significantly greater mortality relative to the remaining densities and locations, indicating major effects of density and spatial location on fitness. Over the duration of the entire study, there was also significant variation among families in mortality. The survival data gave no indication of variation among families in their responses to the range of environments. Conversely, results based on growth and size indicated that different families were favored in different densities and locations, in support of the hypothesis that environment‐dependent selection promotes specialization to different environments in this species. The correlation among families between leaf number in high and low density was small and positive, indicating near‐independence of performance in different densities.

Abstract:Objective To analysis the recurrence pattern after curative resection by thoracoscopic lobectomy of non-small cell lung cancer.Methods All patients who underwent lobectomy for clinical stage Ⅰ lung cancer determined by means of computed tomographic and positron emission tomographic analysis were identified from a prospective database at a single institu-tion.All patients were selected for video-assisted thoracoscopic surgery.Patients’ characteristics, perioperative results, and re-currences were recorded.A logistic regression model was constructed to identify variables influencing the development of recur-rent disease.Results There were 1 073 NSCLC patients who underwent curative thoracoscopic lobectomy between September 2006 and December 2013.969 cases were included according to the inclusion criteria.957 cases were included finally except for 12 cases without any follow-up information.The median follow-up time interval was 27 months(7-94 months) .The overall recurrent rate is 17.6%(168/957), with 46.4%(78/168) distant recurrence, 38.7%(65/168) local recurrence, and 14.9%(25/168) synchronous distant and local recurrence.The most common local recurrent sites were: ipsilateral lung (53.8%), and mediastinal lymph node (36.6%).The most common distant recurrent sites were: contralateral lung (25.4%), bone(25.4%), and brain(17.5%).Univariate analysis showed that age≥60 years, operation period, patholog-ic stage, tumor N stage, tumor T staging, and the degree of pathological differentiation were the factors influencing recurrence and metastasis after operation .Conclusion The recurrent pattern of thoracoscopic lobectomy for treating NSCLC is similar to thoracotomy.Thoracscopic lobectomy is a safe, feasible, and effective procedure for non-small cell lung cancer.The most com-mon local recurrent sites are ipsileateral lung and mediastinal lymph node .The most common distant recurrnent sites are contra-lateral lung, bone and brain.

Abstract:OBJECTIVE:The aim of our study was to investigate whether sublobar resection is non-inferior to lobar resection in early-stage non-small cell lung cancer less than 2 cm. METHODS:This is a meta-analysis for randomized controlled trials. Databases including PubMed, Web of Science, EMBASE and Cochrane Central Register were searched up to June 3, 2023. The primary outcome was 5-year survival, and the secondary outcomes were 5-year disease-free survival, cancer-related mortality, recurrence rate, postoperative lung function and perioperative events. RESULTS:A total of 5 studies enrolling 2035 patients were included. Sublobar resection was found to be non-inferior to lobar resection concerning the 5-year survival rate, 5-year disease-free survival rate and cancer-related mortality. However, sublobar resection was associated with higher recurrence rate and less reduction of postoperative lung function. CONCLUSIONS:Sublobar resection was non-inferior to lobar resection in terms of survival outcomes and was associated with better postoperative lung function.

Abstract:Background This study aimed to investigate whether sublobar resection (SR) is equivalent to lobectomy for small (<= 2 cm) second primary lung cancer (SPLC). Methods We identified 834 patients with T1aN0M0 SPLC from the Surveillance, Epidemiology, and End Results (SEER) database during 2000-2016. Overall survival (OS) was compared between lobectomy and SR after propensity-score matching. A total of 228 patients with SPLC were identified from three institutions in China as the validation set. Results SR was an independent risk factor for patients with 1 to 2 cm SPLC (SR vs Lob: hazard ratio [HR], 1.593; 95% confidence interval [CI], 1.186-2.141; P = .002) but not for patients with SPLC <= 1 cm (SR vs Lob: HR, 1.206; 95% CI, 0.790-1.841; P = .385). Subgroup analysis on the SEER data indicated that OS favored lobectomy compared with SR for contralateral SPLC <= 2 cm but not for ipsilateral ones (ipsilateral: P = .692; contralateral: P = .030). Our multi-institutional data also revealed that SR was equivalent to lobectomy for patients with <= 2 cm ipsilateral SPLC. Conclusions SR is equivalent to lobectomy for SPLC <= 1 cm but not for SPLC > 1 to 2 cm. SR might be recommended for patients with ipsilateral small SPLC considering the difficulty in reoperations.
Clinical, Imaging, and Pathological-Molecular Characteristics Associated with Stage IA Invasive Lung Adenocarcinoma Recurrence After Sub-lobar Resection

Abstract:Rationale and objectives: This study aimed to investigate the association of clinical, imaging, and pathological-molecular characteristics with the prediction of patient prognosis with stage IA invasive lung adenocarcinoma (ILADC) after sub-lobar resection. Materials and methods: This study assessed 360 patients, including 91 and 269 with and without recurrence 3 years postoperatively, respectively, with stage IA ILADC undergoing preoperative chest computed tomography (CT) scans and subsequent sub-lobar resection at our institution. Their clinical and CT features and histological subtypes and gene mutation status were compared. Binary logistic regression analysis was conducted to identify the independent risk factors for recurrence. An external validation cohort included 113 patients, used to test the model's efficiency. Results: For clinical features, old age, male gender, smokers, and high age-adjusted Charlson comorbidity index (ACCI) were frequently observed in patients with recurrence than those without (all p < 0.05). For CT features, large tumor size, solid-predominant density, spiculation, peripheral fibrosis, type II pleural tag, and pleural adhesion were more common in recurrent patients than non-recurrent ones (all p < 0.05). The regression model revealed old age, large tumor size, solid-predominant density, spiculation, type II pleural tag, and pleural adhesion as independent risk factors for recurrence, with an area under the curve (AUC) of 0.942. The external validation cohort obtained an AUC of 0.958. For phological-molecular features, micropapillary/solid-predominant growth pattern, KRAS, ALK, and NRAS mutation or fusion were more common in the recurrent group, whereas EGFR mutation was more frequent in the non-recurrent group (all p < 0.05). Conclusion: Clinical and CT features help predict the prognosis of patients with stage IA ILADC after sub-lobar resection and decide for individualized treatment. Moreover, patients with different prognosis demonstrated different pathological-molecular features.

Abstract:Objectives Surgical resection remains the primary treatment for early-stage non-small cell lung cancer (NSCLC), with lobectomy considered the standard approach. However, recent evidence suggests that sublobar resection may be an alternative option for select patients. Materials and Methods A systematic review and meta -analysis were conducted following PRISMA guidelines. Randomized controlled trials (RCTs) and propensity-score matched (PSM) cohort studies comparing lobectomy and sublobar resection in NSCLC patients were included. The primary outcome was overall survival (OS), and secondary outcomes included disease-free survival (DFS), 30-day mortality, and cancer recurrence rates. Individual patient data (IPD) were reconstructed from Kaplan-Meier curves, and one-stage and two-stage meta -analyses were performed. Results A total of 18 studies involving 6,075 NSCLC patients (3,119 undergoing lobectomy, 2,956 undergoing sublobar resection) were included. Lobectomy was associated with significantly better OS compared to sublobar resection (hazard ratio [HR]: 0.78, 95 % confidence interval [CI]: 0.68–0.89, p < 0.001). However, when sublobar resection was further divided into segmentectomy and wedge resection, no significant difference in OS was observed between lobectomy and segmentectomy (HR:0.92, 95 %CI: 0.75–1.14, p = 0.464) whereas lobar resection was associated with better OS compared to wedge resection (HR:0.52, 95 %CI: 0.41–0.67, p < 0.001). DFS outcomes were similar between lobectomy and sublobar resection (HR:0.98, 95 %CI: 0.84–1.14, p = 0.778). Conclusion Lobectomy is associated with better overall survival compared to sublobar resection in NSCLC patients. However, when sublobar resection is subdivided, segmentectomy shows comparable outcomes to lobectomy, while wedge resection is inferior. These findings support the consideration of segmentectomy as the surgical option of choice for Stage IA NSCLC patients.

Abstract:Objective To compare the outcomes between sublobar resection and lobectomy for T1 non-small cell lung cancerNSCLCtumor diameter ≤3 cm.Methods The clinical data of 278 patients with T1 NSCLC undergoing surgery in Department of Thoracic Surgery in China-Japan Friendship Hospital between January 2007 and December 2014 were collected.The mean age of these patients was60.7 ± 10.4 years. Sixty-one patients received sublobar resectionwedge resectionn = 35segmentectomyn = 26 and the other 217 underwent lobectomy. There was 81.7% of adenocarcinoma12.9% of squamous cell carcinoma and 5.4% of cancer of other types. High differentiatedmoderate differentiated and poor differentiated cancer accounted for 8.6%27.0% and 20.5%respectivelyand there were 43.9% cases with undermined differentiation status.In cases of adenocarcinomathere were 4.0% preinvasive lesions7.5% minimally-invasive lesions and 88.5%invasive adenocarcinoma.There were 86.7% cases with T1N0M01.1% cases with T1N1M0 and 12.2% cases with T1N2M0.Results Compared with lobectomy grouppatients were much elderthe duration of operation was much shorterand the percentage of lesion≤2 cm was significantly larger in sublobar resection groupt=0.496P =0.009t =8.082P =0.029χ2 =2.105P =0.002.There was no significant difference in forced expiratory volume in one secondFEV1FEV1 %surgical procedure and incidence of postoperative complications between two groupst =0.065P =0.713t =2.12P =0.085χ2 =0.399P =0.274χ2 =0.438P =0.490.For NSCLC patients with T1N0M0the 5-year survival of sublobar resection group and lobectomy group were 73.9% and 83.5%respectivelywith no significant differenceP =0.883.In sublobar resection groupthe 5-year survival of patients undergoing wedge resection and segmentectomy were 79.4% and 70.6%respectivelywith no significant differenceP =0.979.Multivariate analysis indicated that only age and mediastinal lymph node metastasis N2 were risk factors for poor prognosisHR=1.07P =0.048HR=5.56P =0.011. There was no significant difference in 5-year survival between sublobar resection group and lobectomy groupHR = 1.38P = 0.552.Conclusions For NSCLC patients with T1N0M0though sublobar resection can not totally substitute lobectomyit may become a main surgical procedure for senior patients with poor pulmonary reserve.
