Abstract:Background/Purpose The use of finite element (FE) analysis in implant biomechanics offers many advantages over other approaches in simulating the complexity of clinical situations. The aim of this study was to perform an optimization analysis of dental implants with different thread designs in three types of bone quality. Materials and methods The three-dimensional FE model of a mandibular bone block with a screw-shaped dental implant and superstructure was simulated. In the optimization analysis, the design variables included the thread pitch and the thread depth of the implant. The objective was to minimize the displacement of the implant to the target value. Three FE models with different bone qualities (D2: better bone quality; D3: ordinary bone quality; D4: poor bone quality) were created. Results The FE results showed that the displacement of the implant and the stress of the cortical bone increased, while the Young's modulus of the cancellous bone decreased. In the D2 bone, changing the thread pitch and thread depth had little effect on cortical stress and implant displacement. However, in D3 and D4 bone, increasing thread depth reduced cortical stress by 40 % and implant displacement by at least 9 %. Conclusion Adjusted thread depth for D3 and D4 bone would reduce crestal bone stress and increase implant stability, but only a little alteration on crestal bone stress and implant stability for D2 bone.

Abstract:Variations in the implant thread shape and occlusal load behavior may result in significant changes in the biological and mechanical properties of dental implants and surrounding bone tissue. Most previous studies consider a single implant thread design, an isotropic bone structure, and a static occlusal load. However, the effects of different thread designs, bone material properties, and loading conditions are important concerns in clinical practice. Accordingly, the present study performs Finite Element Analysis (FEA) simulations to investigate the static, quasi-static and dynamic response of the implant and implanted bone material under various thread designs and occlusal loading directions (buccal-lingual, mesiodistal and apical). The simulations focus specifically on the von Mises stress, displacement, shear stress, compressive stress, and tensile stress within the implant and the surrounding bone. The results show that the thread design and occlusal loading rate have a significant effect on the stress distribution and deformation of the implant and bone structure during clinical applications. Overall, the results provide a useful insight into the design of enhanced dental implants for an improved load transfer efficiency and success rate.

Optimal design of thread height and width on an immediately loaded cylinder implant: A finite element analysis

Abstract:In this study, we used a finite element method to evaluate the maximum Von Mises stresses in jaw bones of immediately loaded implant with different thread heights and widths, and the maximum displacements in implant–abutment complex. The implant thread height ranged from 0.20 to 0.60mm, and the thread width ranged from 0.10 to 0.40mm. Compared to those in standard designed implants, the maximum Von Mises stresses in cortical and cancellous bones with axially loaded implants decreased by 18.85% and 47.46%, respectively, and by 16.38% and 63.46%, respectively in buccolingually loaded implants. The maximum displacement of implant–abutment complex loaded axially and buccolingually decreased by 13.78% and 6.97%, respectively. These results indicated that thread height played more important roles in affecting bone stresses and implant–abutment complex stability than thread width. Immediately loaded cylinder implants with thread height exceeding 0.44mm and width ranging from 0.19 to 0.23mm caused the lowest stresses to the type B/2 bone.
Prognostic factors for survival in patients with hepatocellular carcinoma after percutaneous microwave ablation.

Abstract:PURPOSE To determine the long-term survival and prognostic factors in patients with hepatocellular carcinoma treated with percutaneous microwave ablation. MATERIALS AND METHODS Institutional review board approval and informed consent were obtained. A database of cases of hepatocellular carcinoma in 288 patients (259 men, 29 women; mean age, 54.8 years +/- 11.4 [standard deviation]; age range, 25-82 years) with 477 histologically proved lesions who underwent percutaneous microwave coagulation therapy between May 1994 and October 2002 was retrospectively analyzed. Prognostic factors for survival were evaluated by means of univariate and multivariate analyses. RESULTS The mean follow-up period after microwave ablation was 31.41 months +/- 20.43 (range, 5-106 months). The 1-, 2-, 3-, 4-, and 5-year cumulative survival rates among all 288 patients were 93%, 82%, 72%, 63%, and 51%, respectively. Ninety-three patients (32%) died. Local recurrence or new tumors occurred in 100 patients (35%). Age (P = .836), sex (P = .073), preablation serum alpha-fetoprotein level (P = .136), and preablation treatment (P = .256) were not related to prognosis, while tumor number (P = .004), tumor size (P < .001), Child-Pugh classification (P = .003), tumor differentiation (P = .026), and local recurrence or presence of new tumors (P = .004) significantly affected survival at univariate analysis. At multivariate analysis, only tumor size (P < .001), number of nodules (P = .005), and Child-Pugh classification (P = .01) each had a significant effect on survival. CONCLUSION With use of microwave ablation, there is a high probability of long-term survival of patients with a single lesion of 4.0 cm or less in maximum diameter and Child-Pugh class A cirrhosis.

Abstract:Abstract Introduction Dental implants are common treatment modality for tooth loss which leads to unaesthetic appearance and may also cause deterioration of mastication and speech. The aim of implant therapy in dentistry is to restore tissue contour, function, comfort, aesthetic, and speech. Dental implant role is to transfer the mechanical force created during chewing to the supporting osseous tissues within the mandible and maxilla. The importance of biomechanical factors such as the bone-implant interface, implant thread design, the length and diameter of implants, type of loading, the quality and quantity of surrounding bone have been strained by various authors. The selection of implant thread design plays an important role in the outcome of the treatment. This study was done to evaluate the influence of different thread designs on stress distribution of osseointegrated implant using three-dimensional (3D) finite-element analysis. Materials and Methods Three implants with different thread designs, namely V-thread, buttress, and reverse buttress thread designs were considered and dimensions were standardized. The site considered was the mandibular molar region with cortical and cancellous bone assuming to be isotropic and homogeneous. The implant modeling was done with the ANSYS 18.1 software. Axial load (100N) and buccolingual load (50N) were applied. The stresses were calculated as Von Mises stress criterion. Results Minimum von mises Stress concentration was seen for tapered implant body with reverse buttress thread design under axial load 100N and tapered implant body with V-thread under buccolingual load of 50N at cortical bone which signifies bone preservation. Stress levels were observed maximum at implant and minimum at the cancellous bone. Conclusion Hence, within the limitations of this study the results obtained can be applied clinically for appropriate selection of implant thread design for a predictable success of implant therapy.

Abstract:Dental implants have emerged as an important treatment in modern dentistry for repairing lost teeth, providing patients with enhanced oral function, aesthetics, and overall quality of life. An optimal dental implant minimizes peak stress values at the interface between the implant and the bone while optimizing anchoring strength in the human jawbone under specified standard loads. Implant design controls stress concentration at the bone-implant interface, which influences the biological reaction of the bone. This paper models an optimal thread design to minimize stress distribution surrounding the jawbone. Cylindrical dental implants with micro-threading and square threading improve anchoring and reduce screw loosening, thereby optimizing performance. Biomaterials used in the implant also play a major role. Ni-Ti shape memory alloys (SMAs) have unique characteristics such as biocompatibility, excellent wear and corrosion resistance, and functional features like superelasticity and the shape memory effect, making them suitable for a wide range of biomedical applications. Dental implant design has focused on modifying the shape, size, material, and surface topography to meet market demands. Consequently, the modeling of a dental implant with an improved threading type in the implant body has been accomplished with precise dimensions using SolidWorks 3D CAD software. COMSOL Multiphysics, based on finite element analysis, is then used to analyze the biomechanical properties by importing an STL file from SolidWorks. Several biomechanical factors affect the effectiveness of dental implants, including the nature of loading, the properties of the material used, the shape and geometry of the implants, the quantity and quality of bone around the implants, the surgical technique, and the degree of rapid integration of the implant surface with the jawbone.

Abstract:BACKGROUND: Many factors affluence the stability of dental implant, the implant-bone interface, implant-abutment interface, as well as the thread structure of implant, play a key role in long term reliability. OBJECTIVE: To analyze static force and fatigue between thread structure of dental implant and contact surface, and to explore the potentially fatigue fracture of dental implant. METHODS: A three-dimensional model of dental implant and alveolar bone were established by using CAD software CATIA and Ansys Workbench, respectively. Then the static force and fatigue between dental implant and alveolar bone were analyzed after meshing the model, defining attribute, connections, and loading. RESULTS: A two-segment abutment implant system was constructed using CAD software CATIA. The implant system was characterized by platform transfer, hexagonal location, and taper seal, which represented structure of implants. The size of implant system was designed according to actual sizes and the assemblage was based on standard of mechanical design. Stress concentrated in screw and interface of dentalim plant and fatigue safety factor of the region was lower. These are potentially fatigue fracture of dental implant.

Abstract:Threaded implants have been shown to play an important role in increasing mechanical osseointegration. The aim of this study was to determine bone stress distribution when using different types of implant thread pitches and designs. Five 3D finite element models were constructed to simulate bone stresses induced in implant bodies with two types of thread form: triangular (“Tri” prefix) and trapezoidal (“Trap” prefix). The former had thread pitches of 0.8, 1.2, and 1.6 mm, while the latter had thread pitches of 1.2 and 1.6 mm. A biting load of 143 N was applied vertically and obliquely to the occlusal central fossa of the crown. The main effects of each level of the three factors investigated (loading type, pitch, and thread form) in terms of the stress value were computed for all models. Results indicated that the loading type was the main factor of influence on the peak compressive stress of the alveolar bone. Optimal thread pitch was 1.2 mm for a triangular-thread implant, and a trapezoidal-threaded implant with thread pitch of 1.6 mm had the lowest stress value among trapezoidal-threaded implants. This study concluded that each thread form has its unique optimal thread pitch with regard to lower concentration of bone stress. Clinically, this study suggests that in biomechanical consideration, thread pitch exceeding 0.8 mm is more appropriate for a screwed implant. For clinical cases that require greater bone-implant interface, trapezoidal-threaded implants with thread pitch of 1.6 mm provide greater primary stability and lower concentration of bone stress under different loading directions.

Abstract:Dental implants' usage life and strength are critical factors for implant patients. This paper examines the optimization of dental implant threads by modifying the C-Tech implant system model to ascertain thread design's impact on micromotion through finite element analysis (FEA). The fundamental measurements of the redesigned C-Tech implant system are established by dynamic (FEA). Six implant parameters are chosen as the control factors to be advanced. Experimental simulations are built using a uniform design (UD) method. The dynamic FEA tool ANSYS/LS-DYNA is utilized for each experimental simulation to identify the maximal micromotion in the modified C-Tech implant system. The optimum design model is acquired by minimizing the micromotion by applying the Kriging interpolation (KGI) and genetic algorithm (GA). The improved design has a micromotion of 12.19 μm, as opposed to the original design's micromotion of 38.11 μm. The improvement rate is 68.02%. Finally, the following innovative design is to add a secondary thread to the implant body. After conducting simulations, the micromotion is reduced to 4.72 μm. Further, it shows a 61.28% improvement compared with the optimization design version and an 87.62% improvement compared with the primary implants.

Abstract:Implant design developed considerably with the advancement of restorative dentistry. Examining the stress distribution in the cancellous and cortical bones around custom-made implants with different thread-profile models is the study’s objective. The newly designed implants were made with a diameter and length of 4.5 mm and 11.5 mm. The implants were designed the same, but had different thread profiles. Model A is designed with a standard V-shape thread design, and it was compared with the remaining three dental implants (models B, C, and D) having different customized thread-profile designs. The biomechanical characteristics of the four implant models were compared with the use of biomechanical profiling to predict the mechanical performance of various dental-screw models, including the influence of physiological factors. The stress distribution in the D4 bone area of implants with different thread-profile designs under a vertical load of 100 N at 0° and an oblique load of 223.6 N at 25° was examined using ANSYS Workbench. The trabecular and cortical bones comprise the structure of the D4 bone area. Deformation and stress (von Mises) findings were found for the dental implants and bone. While implant models C and D showed less stress distribution in the cortical and cancellous bone, they nonetheless produced outcomes superior to those of the conventional model A underloading. According to the findings, the unique dental implant design lessens the stress concentration in the cortical bone’s neck area. The suggested model C increases the implant’s stability in that region by distributing a low stress over the D4 bone.

Abstract:Background: Biomechanics of an implant-supported prosthesis play a key role in the success or failure of rehabilitation of missing teeth. This study aimed to analyze biomechanical factors, such as an implant design and prosthesis design for a single implant-supported prosthesis. An in-vitro strain gauge analysis and finite element analysis were performed to assess different implant thread shapes and prosthesis retention modes for their strain-producing property in the peri-implant region of bone. Methodology: Four study models were prepared. Two models were fitted with Bio Horizon Tapered-Pro implants having predominant buttress-shaped threads (BT) and then two models were fitted with Grande Morse Neo Dent implants having trapezoid-shaped threads (TT). Each design was used with two types of retention modes for prostheses, BP-C and TT-C for cement-retained prostheses and similarly BT-S and TT-S for screw-retained prostheses. The strain gauges were bonded to the models and connected to a strain meter. Using an opposing porcelain fused to a metal prosthesis, a combined (axial and non-axial) load of 50–300 N at a strain rate of 0.95 mm/s was applied stepwise to each prosthesis. The strain values were recorded, and the collected data were organized and analyzed using SPSS version 22. For the finite element analysis, four 3-D models were designed. The bone, dental implants, and prostheses for each group were designed using Solid Works. A static, linear simulation was conducted in Ansys software. Results and discussion: The strain values recorded were all less than 3000μɛ and within the physiological loading zone as per Frost’s theory. Statistically significant differences were found between all groups with p-values <0.05, suggesting that changes in implant design led to differences in peri-implant bone strains. At the maximum loading of 300 N, i.e., at the mean biting force of an individual adult, the maximum strain value of 1812 με was recorded for group TT-C. At the minimum loading, all strain values were less than 500 με except for group TT-C for which 518 με was recorded. The peri-implant bone next to the implant’s crest showed maximum strain, which means that this site is more subjected to the effects of overloading than any other part. The von Mises stress was seen concentrated at the implant neck. Conclusion: TT-C implant-supported prostheses give a high strain profile. In comparison, the BT-C implant-supported prostheses give a low strain profile at mean biting forces.

Abstract:The origin of the open areas of white sand campinas of the lower Rio Negro is discussed. Campinas are compared with the denser campina forests or caatingas of the upper Rio Negro. It is concluded that the campinas are areas of white sand forest cleared by Indians. Evidences from pottery shards and radiocarbon dating of charcoal deposited in the soil demonstrate that the campinas were occupied by Indians of the Guarita subculture around 800 A.D. The open campina areas show a gradual succession to more closed type of campina to campina forest, the climax on the white sand. The lack of a large number of endemic species also indicates a recent origin of the open areas. Recolonization after Indian clearing is extremely slow on white sand because of limiting factors such as nutrients and excessive drainage in the dry season.
Influence of Thread Shapes of Custom-Made Root-Analogue Implants on Stress Distribution of Peri-Implant Bone: A Three-Dimensional Finite Element Analysis

Abstract:OBJECTIVE:To explore the effects from the thread shapes of custom-made root-analogue implant (RAI) on distributions of von Mises stress around the peri-implant bone.METHODS:Five one-stage RAI three-dimensional finite element (FE) models with different thread shapes (V-shaped design, square design, buttress design, reverse buttress design and none thread design) and congruent bone were created through reverse engineering technology. The data of the five models were imported into the FE analysis software to calculate. A force of 100 N was applied parallelly and of 45° to the implant axis respectively. Analysis was performed to evaluate the von Mises stress distributions at the peri-implant regions with the help of the Ansys 16 software.RESULTS:The von Mises stresses distributed mostly at the implant cervical regions and the tip ends of the threads on the cortical bone under oblique loading, while on the cancellous bone, the stresses concentrated mostly on the implant lateral cervical regions, the tip ends of the threads and the apical regions. When under vertical loading, the von Mises stresses distributed mostly at the implant cervical regions on the cortical bone while at the tip ends of the threads and the lateral apical regions on the cancellous bone. The von Mises stresses were better distributed on the thread groups under both kinds of loadings compared with no thread design. But there was no obvious difference among the different thread groups. The concentrations of the von Mises stresses on the cancellous bone in the thread groups were mostly at the tip ends of the threads while less in the apical area. The von Mises stresses were better distributed on the cancellous bone on the other three thread designs than on square design.CONCLUSION:Thread designs are advocated for the reason that adding thread designs to the RAI standard design will have a positive effect on stress distributions at the peri-implant regions and it will reduce the concentrations of von Mises stresses on the cortical bone. From the standpoint of the stress distribution, V-shaped design, buttress design and reverse buttress design are more suitable for RAI than square design. There is no difference of the distributions of the von Mises stresses in the RAI between different thread designs.
Model-based biomechanical dental implant optimization in bone-implant system

Abstract:Purpose: The purpose of this study was to identify the important parameters for a model-based design optimization method through comparison of different biomechanical environments and various dental implant models. Materials and Methods: Finite element analysis (FEA) was applied to model a mandibular bone implant system. Essential parameters, such as, cortical bone properties, boundary conditions, and loading directions, were investigated and altered. The effects of altering these parameters in bone-implant FEA models were evaluated based on differences in stresses. Results: The results of this study showed that under different oblique loads, cortical bone properties and constraints the differences in the stress level and distribution between the different implant designs could be clearly identified. Key parameters for a model-based design optimization method were identified effectively by using FEA to compare the stress situation in different implant systems with various biomedical environments. Conclusion: Without knowing which parameters play substantial roles, design optimization of a dental implant system remains a challenge, especially when comparison of different implants are involved and bone quality has to be taken into account. This study shows that an oblique load, excellent cortical bone property, and minimal support constraints are critical parameters affecting the stress distribution in the bone-implant model. Abstract:When one or more teeth are lost for any reason such as accident, caries, and gum disease, they should be replaced with dentures. Researchers discovered that titanium successfully bonds to bone because of the development of osteoblasts on its rough surface. However, the success of dental implants depends on biological, mechanical, and chemical factors. Each of these factors is itself a function of different parameters that, any changes in these factors affect the mechanism of transfering from the prosthesis to the implant and from implant to bone. The role determinant in the success of implant therapy is the function of the geometry and mechanical properties of the implant, jaw and bone-implant joint. Now, due to the increasing use of dental implants, various companies are constantly launching their new products and, as a result, express many benefits of these products, which in most cases are based on their claims. It is not a long-term study, and therefore it is not possible to make an accurate judgment on the designs, macroscopic and microscopic properties, and benefits offered, due to the lack of accurate and practical comparisons between systems. It will not be possible to prove the effectiveness of each of them. Therefore, understanding the design method and its philosophy, along with familiarity with the types of systems available in the market, is a determining factor in the clinical success of dental implant treatment. The presence of threads prevents overload on the surface of the cortical bone and reduces this load by up to 36%. Reducing the thread pitch contributes to the stability of the implant, and increasing the depth of the thread stabilizes patients with poor bone quality. The behavior of the bone and its relation to the implant with regard to applied loads with a maximum inclination angle of 15° have been evaluated by evaluating the tension in the bony areas of the cortex and trabecular bone.

Finite element analysis allows researchers to predict the stress distribution in the bone implant without the risk and cost of implant placement. In this study, the stresses created in the 3A.P.H.5 dental implant's titanium fixture and screw due to the change in abutment angles tolerance have been investigated. The results show that although the fixture and the screw's load and conditions are the same in different cases, the change of the abutment angle and the change in the stress amount also made a difference in the location of maximum stress. The 21-degree abutment puts the fixture in a more critical condition and increases the chance of early plasticization compared to other states. The results also showed that increasing the abutment angle to 24 degrees reduces the stress in the screw, but decreasing the angle to 21 degrees leads to increased screw stress and brings it closer to the fracture.

Abstract:A 52-year-old woman underwent a right frontotemporal craniotomy for microsurgical clip obliteration of a ruptured right dorsal variant ophthalmic segment carotid aneurysm. During the craniotomy, a defect involving the orbital roof was inadvertently created. The patient was noted postoperatively to have fluid egressing from her OD. The fluid was analyzed and based on glucose and chloride levels was determined to be cerebrospinal fluid (CSF). CT scan of the head demonstrated the orbital roof defect created during surgery. After placement of a lumbar drain, fluid egress from the eye significantly decreased, further confirming the suspicion for CSF leak. Patient was found to have a conjunctival defect of the OD, approximately 2.5 cm × 1.5 cm, extending to the fornix from 9 to 12 o’clock. The conjunctival defect and fornix were repaired with an amniotic membrane graft and a temporary tarsorrhaphy with subsequent resolution of CSF egress. The case report is in compliance with the Health Insurance Portability and Accountability Act.

Abstract:Background : To determine the value and direction of the stress and strain across the different implants placed in the bone when loaded with normal or heavy forces; with either a healthy or compromised anatomical condition is always a controversial topic with quite a bit of ambiguity. Studies suggest more favorable stress distribution in bone with a wider diameter implant and by selecting threaded implant. Ideally 12 mm implant length needs to be maintained in favorable condition but the same rule cannot be applied in compromised anatomical conditions. There is a need for a study to evaluate the favorable stress distribution in bone using different implant dimensions and designs both threaded and nonthreaded, to achieve a better control over loading and stress release. Objectives: A finite element analysis (FEA) study was designed to study ideal stress distribution in cancellous and cortical bone under axial and nonaxial loading using, (1) different implant dimensions and (2) designs. Materials and Methods: A finite element models of eight dental implants with crowns were modeled using Catia, a modeling software with material properties as limitation (FEA elements = 113,388, FEA nodes = 173,212). Results: Magnitude of stresses along the implants and surrounding bone did not increase appreciably when the implant length was changed from 10 to 14 mm. With the increase in implant diameter, the stress levels within the bone reduced in threaded implant and in nonthreaded implant the stress levels increased.Conclusion: The results indicate that, for implants with a wider diameter and in threaded implant with a shorter length, there was favorable distribution of stress and strain pattern during axial and nonaxial loading.

Abstract:<p>The failure of dental implant is frequently occurred as the result of mechanical complications under daily biting forces. The long-term stability and the success of a dental implant are highly depended upon developing an implant system having high fracture resistance under cyclic biting forces. The design of implant has a crucial role in improving the fracture resistance of implant by decreasing stress concentration around the implant system. In the present study, three-dimensional (3D) models of the internally connected Ti-6Al-4V implant systems were developed in different dimensions and the failure analysis of these implants was performed under real biting forces according to ISO 14801 using Finite Element Analysis (FEA). In these models, the design parameters (implant diameter, abutment diameter, taper angle of abutment, implant wall thickness, etc.) were optimized in order to prevent the failure of implants in long-term use. The implant model exhibiting the best biomechanical behavior was determined and the stress value was decreased to 170.34 MPa (von-Mises stress) in this implant under cyclic biting loads. Besides the high fracture resistance of this model, longer service life was predicted and relatively smoother stress transmission was achieved from the implant to the bone tissues under biting forces. The implant stability was also increased by optimizing implant tightening torques. Additionally, the mechanical behavior of this implant model was investigated under different biting conditions.</p>

Abstract:Abstract Temporomandibular joint replacement (TMJR) is a surgical procedure that relies heavily on the biomechanical properties of the implant-bone interface for success. In this study, we investigated the effects of three commonly used implant screw thread designs (buttress, square, and triangle) on the biomechanical performance of the mandibular bone-implant construct, with the aim of improving osseointegration. Using finite element analysis, we simulated the mechanical behavior of the prosthesis and mandible and examined the biomechanics of the temporomandibular joint. We considered five coefficients of friction ranging from 0.1 to 0.5 in our analyses. Our hypothesis was that changing the screw thread shape while keeping the pitch, height, and depth constant could enhance the biomechanical environment at the peri-implant bone. Our results indicate that the square thread design produced the highest stress concentration, while the triangle thread design exhibited the most favorable distribution of stress around the implant. Furthermore, increasing the coefficient of friction led to an increase in stress concentration in the implant and surrounding bone. Our findings offer valuable insights into the biomechanical performance of different screw thread designs in the mandibular bone-implant construct. They highlight the significance of considering screw thread shape and coefficient of friction in TMJR implant design. Future studies should incorporate the viscoelastic properties of bone to improve the accuracy of finite element analysis. This research contributes to the optimization of TMJR implants and ultimately enhances patient outcomes.
