Abstract:Magnesium (Mg) or its alloys are widely tested as potential orthopedic implants, particularly as biodegradable alloys for fracture fixation due to their mechanical properties are close to those of bone. Currently, available Mg or its alloys are confronted with challenges in passing regulatory biosafety tests prior to clinical trials due to its fast degradation and associated degradation products. The degradation of Mg is accompanied by the release of Mg ions, the rise of pH and osmolality in surrounding environments. According to the standard of ISO 10993 Part 13, the pH value shall be appropriate to the site of intended use maintaining in an appropriate range. Approaches to overcome these challenges include the selection of adequate alloying elements, proper surface treatment techniques and control of the degradation rate of Mg or its alloys developed as orthopedic implants. To date, Mg or its alloy-based bone implants have not yet been widely used in clinical applications as medical implants. This review critically summarized published methods to improve the corrosion resistance of Mg and its alloys. The current progress on in vitro cytotoxicity and in vivo biocompatibility properties of these metals was also reviewed. This review aimed to provide a reference for further research and development (R&D) of biodegradable Mg and its alloys with regard to the evaluation of their corrosion process and biocompatibility and facilitation of their translation to clinical applications.

In Vivo Degradation Behavior of Magnesium Alloy for Bone Implants with Improving Biological Activity, Mechanical Properties, and Corrosion Resistance

Abstract:This study aimed to establish a surface modification technology for ZK60 magnesium alloy implants that can degrade uniformly over time and promote bone healing. It proposes a special micro-arc oxidation (MAO) treatment on ZK60 alloy that enables the composite electrolytes to create a coating with better corrosion resistance and solve the problems of uneven and excessive degradation. A magnesium alloy bone screw made in this way was able to promote the bone healing reaction after implantation in rabbits. Additionally, it was found that the MAO-treated samples could be sustained in simulated body-fluid solution, exhibiting excellent corrosion resistance and electrochemical stability. The Ca ions deposited in the MAO coating were not cytotoxic and were beneficial in enhancing bone healing after implantation.

Abstract:Proceedings of the Institution of Mechanical Engineers, Part L: Journal of Materials: Design and Applications, Ahead of Print. Magnesium (Mg) based materials show great promise as temporary implant applications owing to their biocompatibility and biodegradability. These characteristics remove the risk of subsequent surgery to extract the implant once the process of bone tissue healing is finished. Additionally, its density and elastic modulus are near to those of natural bone, thereby reducing the stress-shielding effect. Mg is mostly recognized for its osteoconductive abilities, which implies that it encourages the generation of fresh bone tissue. It also has antimicrobial properties, which lower the possibility of infections leading to implant failure. Moreover, the rapid bio-corrosion of pure Mg in the presence of physiological fluids is a serious concern. The implant's mechanical integrity deteriorates as a result of this corrosion before the surrounding tissue has completely recovered. To address these issues, this review focused on approaches, including alloying, the creation of composites, and surface coating, which can increase their biomechanical and bio-corrosion properties. In vitro analysis of biomechanical and bio-corrosion characteristics of newly manufactured Mg-based implant material is presented in this article. In addition to this application, a list of approved devices made from Mg-based material is highlighted. Furthermore, the present challenges and prospects for future research are also discussed.

Abstract:Medical application materials must meet multiple requirements, and the designed implant must mimic the bone structure in shape and support the formation of bone tissue (osteogenesis). Magnesium (Mg) alloys, as a "smart" biodegradable material and as "the green engineering material in the twenty-first century", have become an outstanding bone implant material due to their natural degradability, smart biocompatibility, and desirable mechanical properties. Magnesium is recognised as the next generation of orthopaedic appliances and bioresorbable scaffolds. At the same time, improving the mechanical properties and corrosion resistance of magnesium alloys is an urgent challenge to promote the application of magnesium alloys. Nevertheless, the excessively quick deterioration rate generally results in premature mechanical integrity disintegration and local hydrogen build-up, resulting in restricted clinical bone restoration applicability. The condition of Mg bone implants is thoroughly examined in this study. The relevant approaches to boost the corrosion resistance, including purification, alloying treatment, surface coating, and Mg-based metal matrix composite, are comprehensively revealed. These characteristics are reviewed to assess the progress of contemporary Mg-based biocomposites and alloys for biomedical applications. The fabricating techniques for Mg bone implants also are thoroughly investigated. Notably, laser-based additive manufacturing fabricates customised forms and complicated porous structures based on its distinctive additive manufacturing conception. Because of its high laser energy density and strong controllability, it is capable of fast heating and cooling, allowing it to modify the microstructure and performance. This review paper aims to provide more insight on the present challenges and continued research on Mg bone implants, highlighting some of the most important characteristics, challenges, and strategies for improving Mg bone implants.
Surface Engineering of Biodegradable Magnesium Alloys for Enhanced Orthopedic Implants.

Abstract:Magnesium (Mg) alloys have been promised for biomedical implants in orthopedic field, however, the fast corrosion rate and mode challenge their clinical application. To push Mg alloys materials into practice, a composite coating with biodegradable and high compatible components to improve anticorrosion property of an Mg alloy (i.e., AZ31) is designed and fabricated. The inner layer is micro-nano structured Mg(OH)2 through hydrothermal treatment. Then stearic acid (SA) is introduced to modify Mg(OH)2 for better reducing the gap below a surface-degradation polymer layer of poly(1,3-trimethylene carbonate). Benefited by the SA modification effect, this sandwiched coating avoids corrosive medium penetration via enhancing the adhesion strength at the interface between outer and inner layers. Both in vitro and in vivo tests indicate that the composite coating modified AZ31 perform a better anticorrosion behavior and biocompatibility compared to bare AZ31. Strikingly, a 1.7-fold improvement in volume of newly formed bone is observed surrounding the composite coating modified implant after 12 week implantation. The sandwiched biocompatible coating strategy paves a hopeful way for future translational application of Mg alloys orthopedic materials in clinics.

Abstract:Magnesium (Mg) based biodegradable materials are a new generation orthopedic implant materials that are intended to possess same mechanical properties as that of bone. Mg alloys are considered as promising substitutes to permanent implants due to their biodegradability in the physiological environment. However, rapid corrosion rate is one of the major constraints of using Mg alloys in clinical applications in spite of their excellent biocompatibility. Approaches to overcome the limitations include the selection of adequate alloying elements, proper surface treatment, surface modification with coating to control the degradation rate. This review focuses on current advances on surface engineering of Mg based biomaterials for biomedical applications. The review begins with a description of corrosion mechanism of Mg alloy, the requirement for appropriate surface functionalization/coatings, their structure-property-performance relationship, and suitability for biomedical applications. The control of physico-chemical properties such as wettability, surface morphology, surface chemistry, and surface functional groups of the coating tailored by various approaches forms the pivotal part of the review. Chemical surface treatment offers initial protection from corrosion and inorganic coating like hydroxyapatite (HA) improves the biocompatibility of the substrate. Considering the demand of ideal implant materials, multilayer hybrid coatings on Mg alloy in combination with chemical pretreatment or inorganic HA coating, and protein-based polymer coating could be a promising technique to improve corrosion resistance and promote biocompatibility of Mg-based alloys.

Abstract:Magnesium (Mg) alloys possess comparable physical and mechanical properties to bone, making them an outstanding candidate of implant materials for bone fracture treatment. In addition to the excellent biocompatibility, and bioactivity, the engagement of Mg alloys is key for a number of biological functionalities in the human body. The unique biodegradation nature of Mg alloy implants implies that it may not require a secondary removal procedure when the expected supporting tasks accomplish, as they may simply and safely “disappear” over time. Nonetheless, the demonstrated drawback of potentially rapid degradation, is an issue that must be addressed appropriately for Mg implants and is consequently given unique attention in this review article. Herein, the critical criteria and the state-of-the-art strategies for controlling the degradation process of Mg alloys are reported. Furthermore, future developments of biodegradable Mg and its alloys systems with satisfactory specifications for clinical trials and deployment, are discussed. This review aims to provide information to materials scientists and clinical practitioners in the context of developing practical biodegradable Mg alloys.
Corrosion and bone healing of Mg-Y-Zn-Zr-Ca alloy implants: Comparative in vivo study in a non-immobilized rat femoral fracture model

Abstract:Biodegradable magnesium (Mg) alloys exhibit improved mechanical properties compared to degradable polymers while degrading in vivo circumventing the complications of permanent metals, obviating the need for surgical removal. This study investigated the safety and efficacy of Mg-Y-Zn-Zr-Ca (WZ42) alloy compared to non-degradable Ti6Al4V over a 14-week follow-up implanted as pins to fix a full osteotomy in rat femurs and as wires wrapped around the outside of the femurs as a cerclage. We used a fully load bearing model allowing implants to intentionally experience realistic loads without immobilization. To assess systemic toxicity, blood cell count and serum biochemical tests were performed. Livers and kidneys were harvested to observe any accumulation of alloying elements. Hard and soft tissues adjacent to the fracture site were also histologically examined. Degradation behavior and bone morphology were determined using micro-computed tomography scans. Corrosion occurred gradually, with degradation seen after two weeks of implantation with points of high stress observed near the fracture site ultimately resulting in WZ42 alloy pin fracture. At 14 weeks however, normal bone healing was observed in femurs fixed with the WZ42 alloy confirmed by the presence of osteoid, osteoblast activity, and new bone formation. Blood testing exhibited no significant changes arising from the WZ42 alloy compared to the two control groups. No recognizable differences in the morphology and more importantly, no accumulation of Mg, Zn, and Ca in the kidney and liver of rats were observed. These load bearing model results collectively taken, thus demonstrate the feasibility for use of the Mg-Y-Zn-Zr-Ca alloy for long bone fracture fixation applications.

Micro-alloying of Zn and Ca in Vacuum Induction Casted Bioresorbable Mg System: Perspectives on Corrosion Resistance, Cytocompatibility, and Inflammatory Response

Abstract:There is an increasing interest in biodegradable materials, such as magnesium, for orthopedic implants. This is driven by their potential to address challenges like stress shielding and the need for secondary removal surgery. In this study, biodegradable magnesium alloys were produced using the Vacuum Induction Casting technique. The impact of micro-alloying Zn and Ca in Mg-xZn-0.2Ca (x= 0.1, 0.2, 0.3, and 0.4 wt%) alloys on corrosion resistance, cytocompatibility, and early-stage inflammatory response was investigated. XRD and SEM-EDS analysis confirmed the presence of Ca2Mg6Zn3 secondary phases in all alloys. The Mg-0.3Zn-0.2Ca alloy exhibited the lowest corrosion rate and an elastic modulus of 33.7 GPa, resembling that of natural bone. Electrochemical measurements indicated a correlation between grain size and secondary phase volume fraction in explaining corrosion behaviour. In vitro degradation in simulated body fluid (SBF) for 21 days showed hydroxyapatite formation on alloy surfaces, aligning with electrochemical studies. In vitro cytotoxicity tests demonstrated the cytocompatibility of all alloys, with Mg-0.3Zn-0.2Ca having the highest cell viability over a 6-day cell culture. Investigation into the inflammatory response with RAW-Blue macrophages revealed the anti-inflammatory properties of Mg-0.3Zn-0.2Ca alloys. Micro-alloying with 0.3 wt% Zn and 0.2 wt% Ca enhanced mechanical properties, corrosion resistance, cytocompatibility, and immunomodulatory properties. This positions the Mg-0.3Zn-0.2Ca alloy as a promising biodegradable implant for bone fixation applications.

Abstract:Biocompatible and biodegradable magnesium (Mg) and its alloys possess excellent potential as orthopedic implant biomaterials, but they corrode too quickly inside the human body to maintain such functions as supporting and fixation. In this study, we have successfully prepared a composite biomedical material with Mg as the substrate and a multilayered coating on the metal, which contains a chemical conversion layer of fluoride or phosphate, an adhesion layer, and a layer of biodegradable polylactic acid (PLA). The recorded weight loss and the release of ionic magnesium (Mg2+) in the simulated body fluid (SBF) demonstrate that the corrosion rate of the protected Mg has been reduced to 1/10 that of the unprotected bare Mg. The corrosion rate of the protected Mg can be adjusted with either the chemical conversion type or the thickness of the PLA layer. The bare and the protected Mg has been manufactured as an implant for bone defect repair of Sprague-Dawley rats. The X-ray and micro-CT images show an improved anticorrosion of the protected Mg in vivo, and the histological analysis indicates that the Mg2+ ions released at an appropriate rate are beneficial for bone growth. The preliminary tests prove that the biodegradable Mg has been effectively protected by the innovative multilayered coating, and the implant has met the required biological functions such as biocompatibility and promotion of bone growth.
Short-Term In Vivo Response to Anodized Magnesium Alloy as a Biodegradable Material for Bone Fracture Fixation Devices

Abstract:Biodegradable materials based on magnesium alloys have a huge potential for bone fracture fixation devices due to their adequate mechanical properties and biocompatibility. However, their fast degradation and the consequent liberation of hydrogen gas at the initial stages of implantation is the major limitation for their use. In this study, the AZ91D magnesium alloy was surface treated by an environment-friendly, nontoxic, and low-cost anodizing process and the early in vivo response was studied in a rat transcortical model. Adequate maturation of woven bone around implants-detected at day 7 post implantation-to lamellar bone was observed from day 15. Lamellar bone after 15 and 30 days of implantation presented similar volume, mineralization pattern, mineral to protein content, and estimated bone maturity between anodized AZ91D and polylactic acid (control) implants. Histology observation showed neither release of hydrogen bubbles in the region closed to the anodized AZ91D implant nor systemic effects on liver, kidney, and spleen. Thus, anodizing of AZ91D in the conditions stated here induced an adequate short-term in vivo response, which postulates their use as potential biodegradable fracture fixation devices for bone healing.
Biodegradable Magnesium Bone Implants Coated with a Novel Bioceramic Nanocomposite

Abstract:Magnesium (Mg) alloys are being investigated as a biodegradable metallic biomaterial because of their mechanical property profile, which is similar to the human bone. However, implants based on Mg alloys are corroded quickly in the body before the bone fracture is fully healed. Therefore, we aimed to reduce the corrosion rate of Mg using a double protective layer. We used a magnesium-aluminum-zinc alloy (AZ91) and treated its surface with micro-arc oxidation (MAO) technique to first form an intermediate layer. Next, a bioceramic nanocomposite composed of diopside, bredigite, and fluoridated hydroxyapatite (FHA) was coated on the surface of MAO treated AZ91 using the electrophoretic deposition (EPD) technique. Our in vivo results showed a significant enhancement in the bioactivity of the nanocomposite coated AZ91 implant compared to the uncoated control implant. Implantation of the uncoated AZ91 caused a significant release of hydrogen bubbles around the implant, which was reduced when the nanocomposite coated implants were used. Using histology, this reduction in the corrosion rate of the coated implants resulted in an improved new bone formation and reduced inflammation in the interface of the implants and the surrounding tissue. Hence, our strategy using a MAO/EPD of a bioceramic nanocomposite coating (i.e., diopside-bredigite-FHA) can significantly reduce the corrosion rate and improve the bioactivity of the biodegradable AZ91 Mg implant.

Abstract:Magnesium alloys have shown prospective applications as a new biodegradable metal within bone. To garantee the longterm biocompatibility, a Mg-Zn-Ca alloy, composing of essential elements for human, was prepared and its feasibility for orthopedic applications was investigated. The in vitro and in vivo corrosion of Mg-Zn-Ca alloy as well as the biocompatibility were studied. The in vitro corrosion tests in five kinds of physiological solutions showed that the corrosion rates and corrosion morphologies of the alloy were strongly influenced by the solution used. The addition of serum in Hank’s and MEM significantly slowed down the corrosion rate and improved the corrosion uniformity of the alloy. The corrosion rate decreased with increasing serum concentration. The alloy showed the slowest corrosion rate as well as homogeneous corrosion morphology in MEM+10%FBS. Both the indirect and direct cell experiments indicated good cytocompatibility of the extruded Mg-Zn-Ca alloy. In vivo , we observed a gradual degradation process from the surface of extruded Mg-Zn-Ca alloy and only 40% in volume of implant was left after 4 weeks implantation in medullary cavities of mice. The micro-CT and histological analyses revealed its good biocompatibility with peri-implant new bone formation and increasing cortical bone thickness with increasing implantation period. This study showed that the extruded Mg-Zn-Ca alloy provided sufficient biocompatibility for orthopedic application, though the in vivo corrosion rate should be further reduced for clinical use.

Abstract:Recently, magnesium has been investigated as a promising bioresorbable orthopedic biomaterial. Its mechanical properties are very similar to natural bone, making it appropriate for load-bearing orthopedic fracture repair applications. However, significant hurdles remain regarding the design of practical implants and methods to control degradation and enhance biocompatibility. Although attempts have been made to hinder magnesium's rapid corrosion via alloying and coating, these studies have used solid monoliths. In an effort to reduce the amount of alloy used for implantation in a shape that mimics cortical bone shape, this study used a thin sheet of Mg AZ31 which was rolled into hollow cylindrical scaffolds. The scaffold was coated with different amounts of Ca-P; this implant demonstrated slowed corrosion in simulated body fluid (SBF) as well as enhanced biocompatibility for mesenchymal stem cells (MSC). In vivo implantation of magnesium alloy scaffold adjacent to the rat femur showed significant biointegration with further deposition of complex Mg-Ca phosphates/carbonates typical of natural bone. Finally, the implant was placed in a critical-size ulna defect in live rabbits, which lead to radiographic union and partial restoration of biomechanical strength in the defect. This study demonstrated that a thin sheet of coated Mg alloy that was spirally wrapped wound be a promising orthopedic biomaterial for bone repair.
Microalloying Design of Biodegradable Mg-2Zn-0.05Ca Promises Improved Bone-Implant Applications

Abstract:Mg and its alloys have been comprehensively studied and show huge potential for clinical orthopedic applications. However, balancing the mechanical strength and corrosion resistance of alloys is still a challenge. In light of this, micro-level contents of Zn and Ca were added to pure Mg to fabricate a Mg-2Zn-0.05Ca microalloy to expectedly enhance the mechanical strength and concurrently improve the corrosion resistance. The characteristics of the rolled Mg-2Zn-0.05Ca microalloy were explored using optical microscopy, X-ray diffraction, and tensile tests. The corrosion behavior and mechanical strength loss were explored using electrochemical and immersion tests. The effects of the microalloy extract on the proliferation, adhesion, and osteogenic differentiation of MC3T3-E1 cells were systematically studied. Moreover, implantations were done in femoral condyles of rabbits to study the degradation properties, osteogenic effect, mechanical strength loss, and biosafety of the microalloy. The ultimate tensile strength and yield strength of the rolled microalloy were found to be significantly elevated to 257 ± 2.74 and 237.6 ± 8.29 MPa, respectively. The microalloy showed a stable and gradual strength loss during degradation, both in vivo and in vitro. Concurrently, the microalloy exhibited improved corrosion resistance ability and especially, in vivo, the rolled microalloy exhibited a comparable degradation rate to that of rolled pure Mg within the initial 12 weeks of implantation. Additionally, the microalloy promoted osteogenesis, both in vitro and in vivo, and no short- and long-term toxicities of the microalloy were observed in rabbits. This study suggested that the rolled Mg-2Zn-0.05Ca microalloy effectively balanced the mechanical strength and corrosion resistance and showed potential application as bone implants.

Abstract:The use of magnesium and its alloys as temporary implants has gained interest in the last two decades due to their good mechanical properties and bio-degradability in the in-vivo conditions. However, the issues of higher corrosion rate and stress corrosion cracking persist, which are responsible for the implants' early failure. This review paper focuses on the challenges involved in the use of magnesium-based implants and the advancements in mitigating the corrosion-related issues for in-vivo use of biodegradable magnesium alloy implants. Herein we review the degradation behavior of three groups of magnesium alloys, i.e., aluminum-containing Mg alloy, rare earth element (REE) containing Mg alloy, and aluminum-free Mg alloy in a variety of testing media. We also review various surface modification techniques such as mechanical methods, physical methods, and chemical methods adopted to address the shortcomings of the Mg alloys. Furthermore, recent developments in Mg based bioimplants such as Mg-based open porous scaffolds, nanostructured Mg alloys and Mg based bulk metallic glasses are reviewed. In the end, recent clinical trials of the Mg-based implant were reported in detail.

Abstract:Magnesium and its alloys are one of the most used materials for bone implants and tissue engineering. They are characterized by numerous advantages such as biodegradability, high biocompatibility and mechanical properties with values close to the human bone. Unfortunately, the implant surface must be adequately tuned, or Mg-based alloys must be alloyed with other chemical elements due to their increased corrosion effect in physiological media. This article reviews the clinical challenges related to bone repair and regeneration, classifying bone defects and presenting some of the most used and modern therapies for bone injuries, such as Ilizarov or Masquelet techniques or stem cell treatments. The implant interface challenges are related to new bone formation and fracture healing, implant degradation and hydrogen release. A detailed analysis of mechanical properties during implant degradation is extensively described based on different literature studies that included in vitro and in vivo tests correlated with material properties' characterization. Mg-based trauma implants such as plates and screws, intramedullary nails, Herbert screws, spine cages, rings for joint treatment and regenerative scaffolds are presented, taking into consideration their manufacturing technology, the implant geometrical dimensions and shape, the type of in vivo or in vitro studies and fracture localization. Modern technologies that modify or adapt the Mg-based implant interfaces are described by presenting the main surface microstructural modifications, physical deposition and chemical conversion coatings. The last part of the article provides some recommendations from a translational perspective, identifies the challenges associated with Mg-based implants and presents some future opportunities. This review outlines the available literature on trauma and regenerative bone implants and describes the main techniques used to control the alloy corrosion rate and the cellular environment of the implant.
In vivo biodegradation and biological properties of a Mg-Zn-Ca amorphous alloy for bone defect repair

Abstract:Amorphous magnesium alloys, characterised by their unique disordered structure, exhibit exceptional mechanical properties and superior corrosion resistance. This study introduced a novel biodegradable Mg-Zn-Ca amorphous alloy. Immersion and electrochemical tests revealed uniform degradation in a simulated body fluid environment. The degradation products predominantly contained zinc and oxygen. After 28 days of immersion, the alloy's structure largely remained intact, and it exhibited a minimal weight loss rate of 11.64 ± 1.85%, indicating its excellent corrosion resistance. In addition, the homogeneous and rapid degradation of the Mg-Zn-Ca amorphous alloy in rats (the residual volume at 12 weeks was 14.94 ± 5.05%, at least 2–3 times of the corrosion rate of immersion in vitro), thus increasing the bone volume/total volume and trabecular number. The new bone and the residual material surface showed good bone integration. Blood and organ tissue tests confirmed the in vivo biological safety. Therefore, Mg-Zn-Ca amorphous alloys have excellent potential as bone-repair materials, especially in load-bearing areas.

Chemiluminescent Analysis of Plasma Antioxidant Capacity in Uremic Patients Undergoing Hemodialysis

Abstract:Objective. Hemodialysis is a common therapeutic strategy for patients with end stage renal failure. During the hemodialytic process, the neutrophils are activated (neutrophil burst) due to the hemoincompatibility induced by hemodialysis. As a result, the activated neutrophils release reactive oxygen species (ROS), such as hydrogen peroxide, singlet oxygen, and hypochlorite, into the bloodstream and cause oxidative damage. Methods. This study investigated the antioxidant alteration of plasma in uremic patients undergoing hemodialysis by chemiluminescent analysis. The antioxidant capacities of plasma in scavenging hydrogen peroxide, singlet oxygen, and hypochlorite were investigated in this experiment. In addition, investigation of the ferric-reducing ability of plasma (FRAP) would be covered in this study as well. Results. This study found that after hemodialysis, the antioxidant capacities of plasma in scavenging hydrogen peroxide, singlet oxygen, and hypochlorite decreases 7.9%, 18.8%, and 18.9%, respectively. Moreover, the FRAP is reduced by 56%. We speculate that the loss of dialyzable solutes (such as uremic solutes and antioxidants with small molecular weight) in plasma resulted in its decrease in antioxidant capacity. Conclusion. We therefore suggest that the supplement of antioxidants with small molecular weight is capable of regaining antioxidant defense in plasma and preventing oxidative damage induced by hemodialysis.
