Abstract:Transfemoral amputees demand a mechatronic lower limb prosthesis as technical substitute for restoring their gait functions. Prosthetic knee is the key component of a transfemoral prosthesis. The performance of the prosthetic knee determines the walking ability of the transfemoral amputee. This study proposed a novel microprocessor-controlled prosthetic knee with hydraulic damper and evaluated the performance of the prosthetic knee by function simulation and evaluation platform. The prosthetic knee with electrical-controlled hydraulic cylinder that could modulate knee flexion and extension damping properties independently and continuously by single motor was designed. Gait phase identification system based on knee angle sensor, inertial measurement units mounted on thigh connector and shank and force transducer embedded in shank was proposed. Gait phase identification and damping control strategy were determined by typical gait events during walking. Speed adaption and gait symmetry tests were conducted with a customized gait simulator to evaluate the performance of the proposed microprocessor-controlled prosthetic knee. The angle trajectories of the prosthetic knee were similar under a range of walking speeds. While the symmetry index values indicated that the stance phase was more asymmetry than swing phase, the peak swing flexion knee angles were consistently controlled between 60-70 degrees under different speeds. The knee angle symmetry was observed in different speeds during swing phase. It is suggested that the proposed microprocessor-controlled prosthetic knee could meet the fundamental demands of walking with smooth angular transition across different walking speeds.

Abstract:Abstract As the essential technology of human-robotics interactive wearable devices, the robotic knee prosthesis can provide above-knee amputations with functional knee compensations to realize their physical and psychological social regression. With the development of mechanical and mechatronic science and technology, the fully active knee prosthesis that can provide subjects with actuating torques has demonstrated a better wearing performance in slope walking and stair ascent when compared with the passive and the semi-active ones. Additionally, with intelligent human-robotics control strategies and algorithms, the wearing effect of the knee prosthesis has been greatly enhanced in terms of stance stability and swing mobility. Therefore, to help readers to obtain an overview of recent progress in robotic knee prosthesis, this paper systematically categorized knee prostheses according to their integrated functions and introduced related research in the past ten years (2010–2020) regarding (1) mechanical design, including uniaxial, four-bar, and multi-bar knee structures, (2) actuating technology, including rigid and elastic actuation, and (3) control method, including mode identification, motion prediction, and automatic control. Quantitative and qualitative analysis and comparison of robotic knee prosthesis-related techniques are conducted. The development trends are concluded as follows: (1) bionic and lightweight structures with better mechanical performance, (2) bionic elastic actuation with energy-saving effect, (3) artificial intelligence-based bionic prosthetic control. Besides, challenges and innovative insights of customized lightweight bionic knee joint structure, highly efficient compact bionic actuation, and personalized daily multi-mode gait adaptation are also discussed in-depth to facilitate the future development of the robotic knee prosthesis.

Abstract:Background: With the in-depth development of assistive treatment devices, the application of artificial knee joints in the rehabilitation of amputees is becoming increasingly mature. The length of residual limbs and muscle strength of patients have individual differences, and the current artificial knee joint lacks certain adaptability in the personalized rehabilitation of patients. Purpose: In order to deeply analyze the impact of different types of artificial knee joints on the walking function of unilateral thigh amputees, improve the performance of artificial knee joints, and enhance the rehabilitation effect of patients, this article combines image processing technology to conduct in-depth research on the walking gait analysis of different artificial knee joints of unilateral thigh amputees. Methods: This article divides patients into two groups: the experimental group consists of patients with single leg amputation, and the control group consists of patients with different prostheses. An image processing system is constructed using universal video and computer hardware, and relevant technologies are used to recognize and track landmarks; Furthermore, image processing technology was used to analyze the gait of different groups of patients. Finally, by analyzing the different psychological reactions of amputees, corresponding treatment plans were developed. Results: Different prostheses worn by amputees have brought varying degrees of convenience to life to a certain extent. The walking stability of wearing hydraulic single axis prosthetic joints is only 79%, and the gait elegance is relatively low. The walking stability of wearing intelligent artificial joints is as high as 96%. Elegant gait is basically in good condition. Conclusion: Image processing technology helps doctors and rehabilitation practitioners better understand the gait characteristics and rehabilitation progress of patients wearing different artificial knee joints, providing objective basis for personalized rehabilitation of patients.

Design trends in actuated lower-limb prosthetic systems: a narrative review

Abstract:Introduction: Actuated lower limb prostheses, including powered (active) and semi-active (quasi-passive) joints, are endowed with controllable power and/or impedance, which can be advantageous to limb impairment individuals by improving locomotion mechanics and reducing the overall metabolic cost of ambulation. However, an increasing number of commercial and research-focused options have made navigating this field a daunting task for users, researchers, clinicians, and professionals. Areas covered: The present paper provides an overview of the latest trends and developments in the field of actuated lower-limb prostheses and corresponding technologies. Following a gentle summary of essential gait features, we introduce and compare various actuated prosthetic solutions in academia and the market designed to provide assistance at different levels of impairments. Correspondingly, we offer insights into the latest developments of sockets and suspension systems, before finally discussing the established and emerging trends in surgical approaches aimed at improving prosthetic experience through enhanced physical and neural interfaces. Expert opinion: The ongoing challenges and future research opportunities in the field are summarized for exploring potential avenues for development of next generation of actuated lower limb prostheses. In our opinions, a closer multidisciplinary integration can be found in the field of actuated lower-limb prostheses in the future.

Abstract:The four-bar linkage prosthetic knee has attracted widespread attention in the study of lower limb prosthesis because it shows a better bionic feature and a higher locomotive safety than the uniaxial joint prosthetic knee. Based on a real four-bar linkage prosthetic knee, this paper mainly studies the strongly nonlinear effects, e.g. the foot-ground interaction force and the unilateral constraint force of knee joint, on the gait of the lower limb prosthesis. For this purpose, firstly, the Kelvin-Voigt contact model is adopted to represent the effect of foot-ground contact force and the unilateral constraint force of the knee joint. The Coulomb model is employed to describe the effect of foot-ground friction force. Then, the Lagrange equations of the first kind are applied to model the dynamics of the prosthesis. Based on this model, the measured hip joint motion of an able-bodied testee is used as the driven signal and the gait characteristics analysis is conducted numerically. The numerical results reveal that if the stiffness of the hydraulic cylinder, which supports the motion of the prosthetic knee joint, is small, the strongly nonlinear effects may lead to the remarkable subharmonic response, which further results in the so-called gait inconformity. Further research shows that the subharmonic response can be avoided by lifting the hip joint, which provides a new insight into the compensatory mechanism such as lifting the hip for the amputee walking from the view of mechanics. In order to evaluate the consistence of the gaits between the prosthesis and the able-bodied testee, this paper further defines the correlation coefficient and analyzes the effects of the hydraulic cylinder's stiffness and damping on this coefficient. The results show that the correlation coefficient of the gaits can be better than 0.9 with proper stiffness and damping design. This discovery provides a solid foundation for further optimization of the four-bar linkage prosthesis.

Abstract:Powered knee prostheses promise to improve the mobility of transfemoral amputees by imitating the biomechanics of the missing knee joint. Unfortunately, the heavy weight and short battery life severely limit the application of powered prostheses. Here, we present a lightweight powered knee prosthesis, which employs a series elastic actuator (SEA) integrating a high torque density external rotor motor, a ball screw and a slider-crank mechanism. A genetic algorithm (GA) is used to optimize the slider-crank mechanism's key parameters and improve the prosthesis's compactness. Combined with the hardware, a hybrid controller is proposed, which comprises impedance control in the stance phase with position control during the pre-swing and swing phases. The controller was applied to the powered prosthesis, and its capability to provide level walking functionality was evaluated on four above-knee amputees. The data obtained from these experimental trials indicated that the prosthesis could replicate the key biomechanical functions of a biological knee in the sagittal plane while the weight was lighter than most powered knee prostheses. In addition, the vertical ground reaction force (GRF) was quantitatively analyzed to validate that the powered knee prosthesis could improve wearing comfort with early-stance knee flexion.

Abstract:ABSTRACT This paper introduces a novel computational framework for evaluating above‐knee prostheses, addressing a major challenge in gait deviation studies: distinguishing between prosthesis‐specific and patient‐specific contributions to gait deviations. This innovative approach utilizes three separate computational models to quantify the changes in gait dynamics necessary to achieve a set of ideal gait kinematics across different prosthesis designs. The pilot study presented here employs a simple two‐dimensional swing‐phase model to conceptually demonstrate how the outcomes of this three‐model framework can assess the extent to which prosthesis design impacts a user's ability to replicate the dynamics of able‐bodied gait. Furthermore, this framework offers potential for optimizing passive prosthetic devices for individual patients, thereby reducing the need for real‐life experiments, clinic visits, and overcoming rehabilitation challenges.

Abstract:Background: Adequate research is not reported so far to underline the influence of commonly used polycentric knee joints on gait performance of subjects with trans-femoral amputation. Objective: The intent of this investigation is to analyze prosthetic gait of unilateral traumatic trans-femoral amputees with polycentric four-bar linkage knee and compare it with normal subjects for evaluating any asymmetry in gait performance. Methods: Objective three-dimensional gait analysis of 15 subjects [mean (age): 36.4 (10.7) years] were performed in gait lab through force plate and optoelectronic devices to measure temporal-spatial parameters, kinematic and kinetic performances. Gait patterns of amputees were compared with those of 15 individuals with normal gait to analyze distinct functionalities of existing polycentric knee. Results: Asymmetry in gait was observed between amputees and normal subjects for all variables concerned ([Formula: see text]). Amputee gait was with significantly lesser velocity, cadence with shorter step and stride length. There was significantly less hip, knee and pelvic motions, however, pelvic obliquity and rotation did not show significant difference from the normal subjects. The vertical component of the ground reaction force differed significantly between prosthetic and intact limb [49.7 (8.5)% and 90.4 (7.4)% body weight] and also from normal subjects [107.5 (2.4)% body weight] during stance ([Formula: see text]). Interpretation and Conclusion: This difference may be attributed to nonproportionate loading of limbs and mechanical adaptations for counteracting deficiencies of prosthetic side. This study will help to explain gait asymmetry in trans-femoral amputees and to identify underlying mechanisms to enhance the quality of the existing design of prosthetic knee through optimizing design parameters and utilizing appropriate materials.

Abstract:This article presents a robotic transfemoral prosthesis that consists of an electrohydraulic knee and a motor-driven ankle. The electrohydraulic knee includes a controllable hydraulic damper to provide stance control and an electrical actuator to provide swing assistance. The motor-driven ankle provides damping adaptation for different terrains. The embedded finite-state-based controller enables the knee-ankle prosthesis to facilitate the amputee's ambulation on five different terrains. The proposed knee is lightweight and compact, with low electrical energy consumption. Experimental results demonstrate the potential advantages of the proposed transfemoral prosthesis compared with the hydraulic prosthesis daily used by the subject.

Abstract:Most existing transtibial prostheses are energetically passive. Their ankle joints are either rigid or rotatable in a limited range, and their feet are single-segment structures without toe joints. Amputees using these passive prostheses exhibit nonsymmetrical gait patterns, consume more metabolic energy, and walk at lower speeds compared with able-bodied individuals. In this paper, we design and construct a powered transtibial prosthesis with stiffness adaptable ankle and toe joints, which are driven by adapted series-elastic actuators, to improve the walking performance of the amputees. Mechanical models of both joints are built to help analyze joints' capabilities of adjusting stiffness. In actual control of the prosthesis, we utilize a linearized trajectory control method to adjust the stiffness of both joints. To evaluate the performance of the proposed prosthesis, experiments are carried out on an amputee with a unilateral transtibial amputation. Experimental results indicate that both ankle and toe angles of the proposed prosthesis are close to those of the sound limb, and the vertical ground reaction force of the prosthetic side is similar to that of the sound side. Compared with a commercial passive prosthesis, the proposed prosthesis can help the amputee obtain more natural and symmetrical walking gaits.

Abstract:Accurate and rapid detection of gait phases is of utmost importance in achieving optimal performance of powered lower-limb prostheses and exoskeletons. With the increasing versatility and complexity of these robotic systems, there is a growing need to enhance the performance of gait detection algorithms. The development of reliable and functional gait detection algorithms holds the potential to enhance precision, stability, and safety in prosthetic devices and other rehabilitation technologies. In this systematic review, we delve into the extensive body of research and development in the domain of gait event detection methods, with a specific focus on their application to prosthetic devices. Our review critically assesses various proposed methods, aiming to identify the most effective approaches for gait phase classification in lower limb robotic systems. Through a comprehensive comparative analysis, we highlight the strengths and weaknesses of different algorithms, shedding light on their performance characteristics, applicability, and potential for further improvements. This comprehensive review was conducted by screening two databases, namely IEEE and Scopus. The search was limited to 204 papers published from 2010 to 2023. A total of 6 papers that focused on Heuristic, Thresholding, and Amplitude Zero Crossing involved techniques were identified and included in the review. 33.3% of implemented Algorithms used kinematic parameters such as joint angles, joint linear and angular velocity, and joint angular acceleration. This study purely focuses on threshold-based algorithms and thus paper focusing on other gait phase detection methods were excluded.

Abstract:Beyond qualitative assessment, gait analysis involves the quantitative evaluation of various parameters such as joint kinematics, spatiotemporal metrics, external forces, and muscle activation patterns and forces. Utilizing multibody dynamics-based musculoskeletal (MSK) modeling provides a time and cost-effective non-invasive tool for the prediction of internal joint and muscle forces. Recent advancements in the development of biofidelic MSK models have facilitated their integration into clinical decision-making processes, including quantitative diagnostics, functional assessment of prosthesis and implants, and devising data-driven gait rehabilitation protocols. Through an extensive search and meta-analysis of over 116 studies, this PRISMA-based systematic review provides a comprehensive overview of different existing multibody MSK modeling platforms, including generic templates, methods for personalization to individual subjects, and the solutions used to address statically indeterminate problems. Additionally, it summarizes post-processing techniques and the practical applications of MSK modeling tools. In the field of biomechanics, MSK modeling provides an indispensable tool for simulating and understanding human movement dynamics. However, limitations which remain elusive include the absence of MSK modeling templates based on female anatomy underscores the need for further advancements in this area.

Robotic orthoses for gait rehabilitation: An overview of mechanical design and control strategies

Abstract:The application of robotic devices in providing physiotherapies to post-stroke patients and people suffering from incomplete spinal cord injuries is rapidly expanding. It is crucial to provide valid rehabilitation for people who are experiencing abnormality in their gait performance; therefore, design and development of newer robotic devices for the purpose of facilitating patients' recovery is being actively researched. In order to advance the traditional gait treatment among patients, exoskeletons and orthoses were introduced over the last two decades. This article presents a thorough review of existing robotic gait rehabilitation devices. The latest advancements in the mechanical design, types of control and actuation are also covered. The study comprehends discussions on robotic rehabilitation devices developed both for the training on treadmill and over-ground training. The assist-as-needed strategy for the gait training is particularly emphasized while reviewing various control strategies applied to these robotic devices. This study further reviews experimental investigations and clinical assessments of different control strategies and mechanism designs of robotic gait rehabilitation devices using experimental and clinical trials.

Abstract:DAP-kinase (DAPK) and DAP1 were recently identified as positive mediators of IFN-{gamma}-induced programmed cell death. The inactivation of these genes by antisense RNA expression reduced the susceptibility of HeLa cells to IFN-{gamma}-induced apoptosis. DAP1 is expressed as a single 2.4-kb mRNA that codes for a basic proline-rich 15-kDa protein. DAPK is transcribed into a single 6.3-kb mRNA and codes for a structurally unique 160-kDa calmodulin-dependent serine-threonine kinase that carries eight ankyrin repeats and two putative P-loop consensus sites. Since genes involved in control of cell death can, when dysregulated, behave as oncogenes or growth suppressor genes, DAPK and DAP1 as positive mediators of IFN-{gamma}-induced cell death may be candidate tumor suppressor genes. Thus, knowledge of the chromosome locations of the DAP1 and DAPK genes might be useful in initial assessment of involvement in neoplasia or other diseases. 7 refs., 1 fig.

A Sagittal Planar Ankle-Foot Prosthesis with Powered Plantarflexion and Socket Alignment

Abstract:Powered ankle-foot prostheses can often reduce the energy cost of walking by assisting with push-off. However, focus on providing mechanical work may lead to ignoring or exacerbating common issues with chronic pain, irritation, pressure ulcer development, and eventual osteoarthritis in persons with amputation. This paper presents the design and validation of a novel transtibial prosthesis informed by predictive biomechanical simulations of gait which minimize a combination of user effort and interaction loading from the prosthesis socket. From these findings, the device was designed with a non-biomimetic anterior-posterior translation degree of freedom with a 10 cm range of motion which is primarily position-controlled to change the alignment of the prosthetic foot with the residual limb. The system is both mobile and tethered, with the batteries, actuators, and majority of electronics located in a small backpack. Mechanical loads are transmitted through cables to the prosthesis, minimizing the distal mass carriage required. We measured torque and force sensing accuracy, open loop actuator performance, closed loop torque and position control bandwidth, and torque and position tracking error during walking. The system is capable of producing up to 160 N-m of plantarflexion torque and 394 N of AP translation force with a closed loop control bandwidth of about 7 Hz in both degrees of freedom. Torque tracking during walking was accurate within about 10 N-m but position tracking was substantially affected by phase lag, possibly due to cable slack in the bidirectional mechanism. The prototype was capable of replicating our simulated prosthesis dynamics during gait and offers useful insights into the advantages and the practical considerations of using predictive biomechanical simulation as a design tool for wearable robots.

Abstract:Hip-knee-ankle-foot prostheses (HKAF) are full lower-limb devices for people with hip amputations that enable individuals to regain their mobility and move freely within their chosen environment. HKAFs typically have high rejection rates among users, as well as gait asymmetry, increased trunk anterior-posterior lean, and increased pelvic tilt. A novel integrated hip-knee (IHK) unit was designed and evaluated to address the limitations of existing solutions. This IHK combines powered hip and microprocessor-controlled knee joints into one structure, with shared electronics, sensors, and batteries. The unit is also adjustable to user leg length and alignment. ISO-10328:2016 standard mechanical proof load testing demonstrated acceptable structural safety and rigidity. Successful functional testing involved three able-bodied participants walking with the IHK in a hip prosthesis simulator. Hip, knee, and pelvic tilt angles were recorded and stride parameters were analyzed from video recordings. Participants were able to walk independently using the IHK and data showed that participants used different walking strategies. Future development of the thigh unit should include completion of a synergistic gait control system, improved battery-holding mechanism, and amputee user testing.

Abstract:In this article, a new hydraulic semi-active knee (HSAK) prosthesis is proposed. Compared with knee prostheses driven by hydraulic-mechanical coupling or electromechanical systems, we novelly combine independent active and passive hydraulic subsystems to solve the incompatibility between low passive friction and high transmission ratio of current semi-active knees. The HSAK not only has the low friction to follow the intentions of users, but also performs adequate torque output. Moreover, the rotary damping valve is meticulously designed to effectively control motion damping. The experimental results demonstrate the HSAK combines the advantages of both passive and active prostheses, including the flexibility of passive prostheses, as well as the stability and the sufficient active torque of active prostheses. The maximum flexion angle in level walking is about 60°, and the peak output torque in stair ascent is greater than 60Nm. Relative to the daily use of prosthetics, the HSAK improves gait symmetry on the affected side and contributes to the amputees better maintain daily activities.

Abstract:Abstract The sialic acid concentration in sera from three ruminant species was quantified and averaged 206, 177, and 207 μmoles per 100ml serum in the cow, sheep, and goat, respectively. A large degree of animal and breed variation was recorded during lactation. The mean level for Holstein, Guernsey, and Brown Swiss was 204, 157, and 241 μ moles of sialic acid per 100ml serum, respectively. Significant amounts of sialic acid were associated with the serum β -lipoproteins.

Abstract:Amputees with lower limb loss need special care during daily life activities to make the movement natural as before amputation. No such work exists covering the main aspects from causes of amputation to the psycho-social impact of the amputees after using the prosthetic device. This review presents for lower limb prosthesis; the study of lower limb amputation, design & development, control strategies & machine learning algorithms, the psycho-social impact of prosthetic users, and design trends in patents. Research articles, review papers, magazines, letters, study reports, surveys, and patents, etc. have been used as sources for this review. Traumatic injuries and different diseases have been found as common causes of amputation. Design & development section illustrates design mechanisms, the categories of passive, active, & semi-active prostheses, an overview of a subset of commercially available prosthetic devices, and 3D printing of the accessories. The control section provides information about control techniques, sensors used, machine learning algorithms, and their key outcomes. Quality of life, phantom limb pain, and psycho-social impact of prosthetic users have been summarized for different countries that are believed to attract the interest of the readers. We have also developed an open-source database "FAKH-50" for patents to emphasize the design trends and advancements in lower limb prostheses from 1970 to 2020. Overall trend analysis determined is in the descending order as the knee (48%) > ankle (28%) > foot (22%) > hip (2%) patents in the current version of our database. The forthcoming section highlights the challenges and prospects of the domain. A mutual observation demands the design of a bio-c-mpatible, lightweight, and economic prosthesis to track the normal human gait by eliminating phantom limb pain. This will empower the amputees to live a quality life in society. This work may be beneficial for researchers, technicians, clinicians, and amputees.
