Abstract:Routine systemic therapy for bullous pemphigoid (BP) has been challenged due to the inevitably adverse effects. According to the successful applications of dupilumab in BP cases reported, therefore, we investigate the real-life efficacy and safety of dupilumab combined with low-dose oral steroid for BP. A cohort of BP patients who received either dupilumab plus low-dose methylprednisolone (dupilumab group) or merely methylprednisolone (control group) was retrospectively reviewed. The time to disease control was investigated. Additionally, the control dose and cumulative dosage of steroids, Bullous Pemphigoid Disease Area Index (BPDAI) scores, pruritus scores, and adverse events were assessed. A total of 40 patients, with 20 in each group, were retrospectively studied. The time to disease control was shorter in the dupilumab group than the control group (14 days vs. 19 days, p = 0.043). When the disease was controlled, the control dose and cumulative dosage of methylprednisolone in the dupilumab group were substantially lower than those of the control (24.6 mg vs. 48.8 mg, 376.8 mg vs. 985.6 mg, both p < 0.01). Compared with the control, the percentage change from baseline in BPDAI scores and pruritus scores were both significantly reduced, and the adverse events were also less frequent in the dupilumab group. The combination therapy of dupilumab plus low-dose methylprednisolone exhibits superior efficacy and safety in comparison with the current first-line systemic therapy for BP.
Efficacy and safety of dupilumab in the treatment of 21 cases of bullous pemphigoid: a retrospective study

Abstract:Objective:To retrospectively analyze the efficacy and safety of dupilumab in the treatment of bullous pemphigoid (BP) .Methods:Clinical data were collected from BP patients who received injections of dupilumab at an initial dose of 600 mg followed by an every-2-week regimen at a dose of 300 mg (the frequency of injections could be increased if necessary) in Department of Dermatology, Peking University First Hospital from October 2020 to October 2021, and their clinical manifestations and changes in laboratory indices were analyzed.Results:A total of 21 BP patients treated with dupilumab were included in this study. Nineteen (90.5%) patients achieved complete or marked disease control after 2-week treatment with dupilumab; 12 patients were followed up for 16 weeks, and all maintained complete disease control at 16 weeks. All patients had a bullous pemphigoid disease area index (BPDAI) score of 122.5 ± 51.1 points at baseline, which decreased to 30.6 ± 27.4 points after 2-week treatment with dupilumab ( t = 8.53, P < 0.001) , and continued to decrease to 12.7 ± 9.1 points after 4-week treatment ( t = 9.73, P < 0.001) . Pruritus was markedly relieved in all the 21 patients within 4-week treatment with dupilumab. Among 10 patients with elevated eosinophil counts at baseline, the eosinophil counts markedly decreased in 9 after treatment. The serum IgE level was elevated in 7 patients at baseline, which markedly decreased in 6 after treatment. Viral conjunctivitis occurred in 1 (4.8%) patient, and no adverse reactions were observed in other patients. Conclusion:Dupilumab is effective in the control of BP and relief of pruritus, with a favorable safety profile.
Long-term efficacy and safety of dupilumab for severe bullous pemphigoid: A prospective cohort study

Abstract:Background: Bullous pemphigoid (BP) is a common subepidermal bullous disease. Dupilumab is a novel treatment for BP. However, its long-term efficacy and safety have not been demonstrated in prospective studies. Objective: Evaluate the long-term efficacy and safety of dupilumab in treating severe BP. Methods: Patients were divided into two groups: the methylprednisolone monotherapy group (M), and the methylprednisolone and dupilumab combination therapy group (D + M). This study consisted of two stages. The first stage focused on the initial treatment phase, where the early efficacy and safety was evaluated. The study then entered the 12-month maintenance treatment stage, where we assessed recurrence in both groups. Additionally, we evaluated the rate of healing of skin lesions, glucocorticoids burden and length of hospital stay and various laboratory test indicators. Results: After four weeks of treatment, the Bullous Pemphigoid Disease Area Index (BPDAI) and pruritus Numerical Rating Scale scores of the D + M group decreased significantly more than those of the M group. The median BPDAI at week 4 was 0 (range: 0.0-3.0) in the D + M group and 10.0 (5.0-12.0) in the M group (P < 0.001). Patients treated with dupilumab experienced a faster cessation of new blisters, quicker glucocorticoid reduction, shorter healing times, and shorter hospital stays (P < 0.001). Additionally, after two weeks of treatment, the levels of eosinophils and immunoglobulin E also decreased (P < 0.001). Follow-up studies further demonstrated that dupilumab monotherapy was associated with a lower recurrence rate. Notably, no serious adverse effects were observed in the study. Conclusions: Our study provides evidence for the efficacy of dupilumab in the treatment of BP based on prospective studies. Additionally, our findings suggest that dupilumab can be considered a reliable single-agent maintenance treatment due to its good safety profile and lower relapse.
The efficacy and safety of dupilumab combined with methylprednisolone in the treatment of bullous pemphigoid in China

Abstract:We access the safety and efficacy of methylprednisolone combined with dupilumab in treating the bullous pemphigoid. 27 patients were enrolled, of which 9 received dupilumab in addition to methylprednisolone (dupilumab group, D group), while the other 18 patients were administered methylprednisolone alone (traditional group, T group). The median time to stop the formation of the new blister was 5.5 days (3.5-11.75 days) and 10 days (9-15 days) in the D group and the T group, respectively (p = 0.032). Additionally, the median time of complete healing reached was 21 days (16.25-31 days) and 29 days (25-50 days) in the D group and the T group, separately (p = 0.042). The median amount of cumulative methylprednisolone at the time of disease control was 240 mg (140-580 mg) and 460 mg (400-840 mg) in the D group and the T group, respectively (p = 0.031). The total amount of the methylprednisolone used at the time of complete healing reached was 792 mg (597-1,488.5 mg) in the D group while that was 1,370 mg (1,000-2,570 mg) in the T group (p = 0.028). No adverse event associated with dupilumab was recorded. Methylprednisolone in combination with dupilumab appeared superior to methylprednisolone alone in control of disease progression and the methylprednisolone-sparing effect.
Dupilumab effectively and rapidly treats bullous pemphigoid by inhibiting the activities of multiple cell types

Abstract:Background: Bullous pemphigoid (BP) is an autoimmune skin-blistering disease. Systemic corticosteroids remain the first line treatment for moderate-to-severe BP with the potential for severe adverse events. Dupilumab has emerged as an alternative option for BP patients. Objective: We evaluated the efficiency and safety of dupilumab on BP treatment and explored a mode of drug action in depth. Methods and results: A multicenter retrospective cohort included 20 BP patients who received dupilumab with or without systemic corticosteroid in dupilumab group, and 20 matched BP patients who received corticosteroid alone in conventional group. Serum samples were collected from 20 patients (10 from dupilumab group and 10 from conventional group) at baseline and week 4. Compared to systemic corticosteroid alone, dupilumab with or without systemic corticosteroid was similarly efficacious in clinical remission at week4 (complete remission plus partial remission: 100%) and week24 (complete remission plus partial remission:100%), but allowing significant decreases in the cumulative doses of corticosteroids with reducing the incidence of adverse events. However, dupilumab did not decrease BP180 antibody despite an obvious clinical improvement. Comparative plasma proteomic analysis performed before and after treatment in 3 BP patients from dupilumab group revealed that drug use was associated with 30 differentially expressed proteins, including 26 down-regulated and 4 up-regulated proteins. The former consisted of immune related proteins involved in T/B cell interactions (inducible T-cell co-stimulator ligand, ICOSL) and in the activation of eosinophils (PRG2), mast cells (S100A12), and complement (CR2). TARC and ICOSL levels correlated with BP severity in patients who received either dupilumab or conventional treatment. Conclusion: Dupilumab has similar efficacy in treating BP as conventional drugs, by inhibiting the activities of many types of immune cells and complement, and regulating the interactions between T and B cells.
A Systematic Evaluation of Dupilumab for Bullous Pemphigoid Treatment

Abstract:This paper systematically reviews the current articles regarding the use of dupilumab in the treatment of bullous pemphigoid (BP) to evaluate its safety and efficacy. PubMed, Embase, Cochrane Library, and Web of Science databases were searched for publications on dupilumab for BP from inception to 10 March, 2023. A total of 26 studies were included for systematic review. The primary outcome was clinical remission, and the secondary outcomes were recurrence and adverse events. Among 96 patients, 71.8% (n = 69/96) received systemic or topical steroids, immunosuppressants, immunomodulators, intravenous immunoglobulins, antihistamines, plasmapheresis, rituximab, and omalizumab, but none of them were successful. After dupilumab treatment, 66.7% (n = 64/96) of patients achieved complete remission, 25.0% (n = 24/96) had partial remission, 5.2% (n = 5/96) showed no remission, and no patients experienced deterioration. In addition, 1.0% (n = 1/96) and 2.0% (n = 2/96) patients stopped using dupilumab due to adverse reactions and cost, respectively. The average remission time was 4.5 months. 46.2% (n = 25/96) of the patients were followed up with a median follow-up of 8 months and only 2 patients relapsed at 8 and 7 months, respectively. Adverse event was 16.9% (n = 12/71), of which transient eosinophilia was the most common. This study indicates that the dupilumab is a promising treatment for BP with high clinical benefit associated with low recurrence rate, adverse event rate, and mortality. However, a large-scale randomized controlled trial is needed to further confirm the safety and efficacy of dupilumab in patients with BP treatment.

Abstract:Bullous pemphigoid is often difficult to treat with the limited therapies available. Here, we describe clinical outcomes among 30 adults with bullous pemphigoid patients treated with dupilumab. We performed a multicenter, retrospective case series between March 2020 to August 2022. Patients received a loading dose of dupilumab 600 mg, followed by 300 mg maintenance dose with varying administration frequency tailored to individual patient response. All patients experienced at least some improvement in blister formation and pruritus, with 23 (76.7%) of patients demonstrating either complete clearance of blistering or marked response. Complete clearance of pruritus or marked response was noted in 25 (83.3%) of patients. Eight patients were effectively maintained solely on dupilumab. One (3.3%) patient reported an injection site reaction. Thirty patients represent a small sample, however, to our knowledge, this is the second largest group of BP treated with dupilumab. Furthermore, we provide an understandable framework for clinicians outside of academics to follow and assess treatment responses in their BP patients treated with dupilumab. Dupilumab should be considered as a therapeutic option in patients with bullous pemphigoid given its ability to induce sustained blistering and pruritus response in both typical and refractory cases while maintaining a favorable safety profile. Abstract:Abstract Through an investigation of the Shenzhen Collective Consultation Ordinance and the Guangdong Regulations on the Democratic Management of Enterprises, this article demonstrates how transnational capital in China deploys its associational power alongside its structural economic power to lobby and pressure the national and local governments to advance its own interests. In addition, building upon the ideas of Hall and Soskice about the varieties of capitalism, the authors have developed the concept of “varieties of transnational capital” to account for the differing positions of overseas business associations regarding the two laws. Abstract:Introduction: Dupilumab has emerged as a promising treatment option for bullous pemphigoid (BP). Rapid identification of responders could avoid the need of additional immunosuppressive treatments that are associated with increased morbidity and mortality. Methods: To investigate the course of itch as an early indicator of treatment response, data of 12 BP patients treated with dupilumab at the University Hospital of Zurich were retrospectively evaluated. Disease severity was assessed by bullous pemphigoid disease area index (BPDAI) and pruritus by a numeric rating scale (NRS, 0-10) at baseline, day 1, 3, 14, month 1, 2 and last follow-up. Results: 8/12 patients (67%) had complete, and 4/12 patients (33%) had partial response during dupilumab treatment. Notably, a highly significant reduction of pruritus (p < 0.0001) was observed already on day 1 with further improvement at later time points. Moreover, fast relief of itch could predict treatment response with a significant correlation to clinical response on day 14 (Spearman correlation R 0.70, p-value 0.025), with a positive but non-significant trend on day 3 (R 0.63, p-value 0.091). Additionally, 92% (11/12 patients) were on dupilumab monotherapy at last follow-up without any concomitant systemic or topical treatment for BP. Conclusions: The rapid and significant decline in BP-associated pruritus observed with dupilumab correlated significantly with disease remission. Early evaluation of pruritus response could change how bullous pemphigoid is treated in the future and avoid additional immunosuppressive treatment in BP.

Abstract:Bullous pemphigoid is an autoimmune blistering disease affecting the dermo-epidermal junction, most commonly seen in older patients. First-line treatment includes systemic, topical corticosteroids and/or steroid-sparing immunosuppressants. Treatment with these medications may be limited by their safety profile. Dupilumab is a humanized monoclonal antibody targeting interleukin-4 and interleukin-13 cytokines currently indicated for moderate-to-severe atopic dermatitis, severe asthma, chronic rhinosinusitis with nasal polyposis, and moderate-to-severe prurigo nodularis. We report a case of a patient with recalcitrant bullous pemphigoid effectively treated with dupilumab.

Abstract:Background Bullous pemphigoid (BP) is a common subepidermal bullous disorder that lacks adequate treatment alternatives. Dupilumab, an anti-interleukin (IL) 4 receptor α antibody blocking Th2 molecules IL-4 and 13, has been used off-label and shown to be effective in refractory BP cases. Methods BP patients with various disease severities and comorbidities were included in this case series. All patients received dupilumab alone or in combination with immunosuppressants in a real-world setting. Complete remission (CR) was defined as the absence of pruritus symptoms and previous BP eruptions, with only hyperpigmentation patches and without newly occurring lesions for at least 4 weeks. Disease relapse was classified as the appearance of three or more new lesions within 1 month or at least one large urticarial or eczematous lesion that did not resolve within a week. Findings Ten individuals were enrolled in this case series. Pruritus symptoms and BP eruptions improved significantly in nine patients (90%). Seven patients (70%) attained CR, including all mild-to-moderate (100%) cases and three of six (50%) severe BP cases. At the dupilumab monotherapy stage, eosinophilia was observed in two severe cases. One patient out of seven (14.3%) relapsed after 1 year of follow-up after CR. Conclusion Treatment of BP with diverse comorbidities with anti-IL-4 receptor α antibody provides further credentials to a prospective randomized study. More impressive efficacy and safety profiles were observed in patients with mild-to-moderate disease after 1 year of follow-up. Eosinophilia may occur in patients receiving dupilumab monotherapy.

Abstract:Objective: Limited information is available on the use of dupilumab for the treatment of atopic dermatitis (AD) in the Chinese population. Methods: We analyzed laboratory data from a previously published randomized, double-blind phase III trial (NCT03912259) to provide further insight into the safety of dupilumab in Chinese adults with moderate to severe AD. The trial participants received either 300 mg of dupilumab or placebo every 2 weeks for 16 weeks. Hematology, blood chemistry, serum thymus and activation-regulated chemokine (TARC), and total immunoglobulin E (IgE) were evaluated. Results: In total, 82 participants received dupilumab and 83 received placebo. With the exception of eosinophil counts of >0.8 × 109/L, which were found less frequently with dupilumab (9.8%) than with placebo (18.7%), the hematology and blood chemistry values were generally stable in both treatment groups. There were no clinically significant differences between the dupilumab and placebo groups, and no participants developed treatment-emergent abnormalities of potential clinical significance. However, compared with placebo, greater decreases in serum lactate dehydrogenase (mean change, −97.4 vs. −33.5 IU/L), TARC (median percent change, −78.6% vs. −30.8%), and total IgE (median percent change, −53.4% vs. −0.2%) were observed with dupilumab than placebo at week 16. Conclusion: Dupilumab demonstrated a favorable laboratory safety profile in Chinese adults with moderate to severe AD.

Abstract:Previously case reports showed dupilumab may benefit for hand eczema treatment, but relatively comprehensive assessments are lacking. A 45-year-old male with multiple severe vesicles, bullae and pustule on the palmar aspects of both hand and foot diagnosed dyshidrotic eczema by pathology was treated with dupilumab at an initial dose of 600 mg subcutaneously, followed by 300 mg every 2 weeks. The physician's assessment of the patient revealed an excellent response to the treatment with dupilumab; the lesions and symptoms achieved dramatic improvement on the third day, and at 6 weeks, the hands and feet became completely normal without relapse in the past 1.5 years of discontinuation. Systematic literature searches were performed, and 6 case reports, 5 case series, 2 prospective observational studies and 1 retrospective review with a total of 150 patients were identified to describing the evaluation of efficacy and safety of dupilumab treatment for hand and foot eczema. Dupilumab appears to be safe and well tolerated with clinical benefit in recalcitrant hand and foot eczema. Larger randomized controlled trials using validated outcome measures and detailed hand eczema type and population classification are needed before dupilumab can be applied in clinical settings.

Abstract:The purpose of this study was to determine the effects of impaired renal function on the amount of orthodontic tooth movement (OTM) in a rat model of renal insufficiency (RI). The animals were divided into three groups: a control unoperated group in which only OTM was done, a sham-operated group, and an RI group. OTM was done two weeks after performance of the sham operation or the surgery to induce RI. Renal failure was induced by electrocoagulation of one kidney cortex followed by contralateral nephrectomy. A fixed orthodontic appliance consisting of a 5 mm length of nickel-titanium closed-coil spring was ligated between the maxillary right incisor and first molar to deliver an initial mesial force of 60 g. Four weeks after RI induction, the amount of tooth movement was estimated by measuring the space created between the first and second molars on a stone cast. Bone density was measured by optical densitometry on radiographs and serum parathyroid hormone (PTH) was measured with a radioimmunoassay method. The results showed that induction of RI significantly increased both OTM (P < .05) and PTH levels (P < .01), although there was no significant difference in bone density by optical densitometry between the unoperated, sham-operated, and RI groups four weeks after RI induction. The results suggest that an increase of bone-remodeling activities, as evident from a significantly higher level of PTH, may be responsible for increase in tooth movement.
Successful treatment of prurigo nodularis by dupilumab: Report of 24 patients

Abstract:Background: Prurigo nodularis (PN) is a chronic pruritic skin disease which is difficult to treat. Current treatment options often lead to limited clinical benefit or severe side effects.Objective: To evaluate the efficacy and safety of dupilumab in the treatment of prurigo nodularis in adults. Method: This study is a retrospective cohort study. Twenty-four adult patients with prurigo nodularis were included and treated with dupilumab. The primary outcomes were the mean reduction in the Investigator's Global Assessment (IGA) score, and pruritus numeric rating scale (p-NRS) score. Outcomes were assessed at baseline, week 4, week16, and week 36.Results: The study included 24 patients, while 9 (37.5%) were male, and the mean(SD) age of the enrolled patients was 49.88 +/- 16.71 years. At the end of treatment, the mean p-NRS score decreased from 7.50 +/- 2.21 to 1.41 +/- 0.91 (P<0.001), sleeplessness numeric rating scale (s-NRS) score declined from 5.33 +/- 3.29 to 0.18 +/- 0.59(P <0.001), and Dermatology Life Quality Index (DLQI) score decreased from 13.32 +/- 4.88 to 0.91 +/- 0.81 (P<0.001). Fourteen (63.6%) patients achieved IGA 0/1 and 21 (95.4%) patients achieved IGA activity 0/1. Among 14 patients who achieved IGA 0/1,10 had an elevated serum IgE level, and patients with a high serum IgE level showed a more remarkable reduction in IGA (r=0.52, P=0.03). Patients with AD responded faster than those without AD (3.76 +/- 1.71 weeks vs 6.40 +/- 1.67 weeks, P=0.01). Adverse events were recorded in 4/24 (16.6%) patients, with conjunctivitis the most frequent.Conclusion: This study demonstrated that dupilumab is effective and safe for prurigo nodularis and could be a potential therapeutic option.
Rituximab, Omalizumab, and Dupilumab Treatment Outcomes in Bullous Pemphigoid: A Systematic Review

Abstract:Background: Bullous pemphigoid (BP) is the most common autoimmune subepidermal bullous disease of the skin. First-line treatment of systemic corticosteroids may cause serious adverse events. Rituximab, omalizumab, and dupilumab should be explored as alternative treatment options to improve outcomes. Objective: To systematically review the rituximab, omalizumab, and dupilumab treatment outcomes in bullous pemphigoid. Methods: A PubMed, Embase, Web of Science, and Cochrane library search were conducted on March 10, 2022. A total of 75 studies were included using Preferred Reporting Items for Systematic Reviews and Meta-analyses guidelines. Results: Use of rituximab (n=122), omalizumab (n=53) and dupilumab (n=36) were reported in 211 patients with BP. Rituximab led to complete remission in 70.5% (n=86/122) and partial remission in 23.8% (n=29/122) of patients within 5.7 months, with a recurrence rate of 20.5% (n=25/122). 9.0% (n=11/122) of patients died and infection (6.6%, n=8/122) was the most common adverse event. Omalizumab led to complete remission in 67.9% (n=36/53) and partial remission in 20.8% (n=11/53) of patients within 6.6 months, with a recurrence rate of 5.7% (n=3/53). 1.9% (n=1/53) of patients died and thrombocytopenia (1.9%, n=1/53) was observed as the most common adverse event. Dupilumab led to complete remission in 66.7% (n=24/36) and partial remission in 19.4% (n=7/36) of patients within 4.5 months of treatment without any reported adverse events, with a recurrence rate of 5.6% (n=2/36). Conclusions: Rituximab, omalizumab, and dupilumab have similar clinical benefits for BP patients. However, rituximab resulted in higher recurrence rates, adverse events, and mortality rates. Abstract:Bullous pemphigoid (BP) is a rare autoimmune blistering condition that predominantly affects the elderly population. Typical treatment regimens target the immune system and inflammatory response. We present a case of BP in a 78-year-old male patient that occurred following the coronavirus disease 2019 (COVID-19) vaccination. This case was refractory to topical steroids and immunosuppressants. However, it responded to treatment with dupilumab, a monoclonal antibody therapy. Dupilumab is classically indicated for the treatment of asthma, eosinophilic esophagitis, atopic dermatitis, and chronic rhinosinusitis with nasal polyposis. We highlight the importance of considering the off-label use of dupilumab and its success in treating BP.
