Abstract:Anthropogenic emissions of toxic trace elements (TEs) have caused worldwide concern due to their adverse effects on human health and ecosystems. Based on a stochastic simulation of factors' probability distribution, we established a bottom-up model to estimate the amounts of five priority-regulatory TEs released to aquatic environments from industrial processes in China. Total TE emissions in China in 2010 were estimated at approximately 2.27 t of Hg, 310.09 t of As, 318.17 t of Pb, 79.72 t of Cd, and 1040.32 t of Cr. Raw chemicals, smelting, and mining were the leading sources of TE emissions. There are apparent regional differences in TE pollution. TE emissions are much higher in eastern and central China than in the western provinces and are higher in the south than in the north. This spatial distribution was characterized in detail by allocating the emissions to 10 km x 10 km grid cells. Furthermore, the risk control for the overall emission grid was optimized according to each cell's emission and risk rank. The results show that to control 80% of TE emissions from major sources, the number of top-priority control cells would be between 200 and 400, and less than 10% of the total population would be positively affected. Based on TE risk rankings, decreasing the population weighted risk would increase the number of controlled cells by a factor of 0.3-0.5, but the affected population would increase by a factor of 0.8-1.5. In this case, the adverse effects on people's health would be reduced significantly. Finally, an optimized strategy to control TE emissions is proposed in terms of a cost-benefit trade-off. The estimates in this paper can be used to help establish a regional TE inventory and cyclic simulation, and it can also play supporting roles in minimizing TE health risks and maximizing resilience. Abstract:Nano-carrier systems have become an attractive alternative in the formulation of hydrophobic drugs, and the number of scientific and patent publications per year on nanoparticles has experienced exponential growth since 2000. As these drugs are carried by different nano-carriers, their pharmacokinetics (PK) and biodistribution will be greatly affected. Therefore, PK and biodistribution have become important parameters to evaluate the nano-systems, which are commonly performed by HPLC. This study looks at certain issues in extraction and HPLC analysis encountered in conducting biodistribution studies for paclitaxel (PTX) nanocrystals. We report here for the first time that the extraction efficiency of PTX depends on the nano-carriers, i.e., nanocrystals and nanomicelles. A selected solvent had to be added prior to extraction to totally destruct the nano-carrier structure to resolve this difference. It was also found that the extraction recovery of PTX carried by the nanocrystals was organ specific. Therefore, it will be critical to consider this difference when studying the distribution of drugs in nano-carriers. We hope that our findings would highlight certain issues to be aware of when conducting PK and biodistribution studies for nano-drug carriers.
Effect driven prioritization of contaminants in wastewater treatment plants across China: A data mining-based toxicity screening approach

Abstract:A diversity of contaminants of emerging concern (CECs) are present in wastewater effluent, posing potential threats to receiving waters. It is urgent for a holistic assessment of the occurrence and risk of CECs related to wastewater treatment plants (WWTP) on national and regional scales. A data mining-based risk prioritization method was developed to collect the reported contaminants and their respective concentrations in municipal and industrial WWTPs and their receiving waters across China over the past 20 years. A total of 10,781 chemicals were reported in 8336 publications, of which 1037 contaminants were reported with environmental concentrations. While contaminant categories varied across WWTP types (municipal vs. industrial) and regions, pharmaceuticals and cyclic hydrocarbons were the most studied CECs. Contaminant composition in receiving water was closer to that in municipal than industrial WWTPs. Publications on legacy pesticides and polycyclic aromatic hydrocarbons in WWTP decreased recently compared to the past, while pharmaceuticals and perfluorochemicals have received increasing attention, showing a changing concern over time. Detection frequency, concentration, removal efficiency, and toxicity data were integrated for assessing potential risks and prioritizing CECs on national and regional scales using an environmental health prioritization index (EHPi) approach. Among 666 contaminants in municipal WWTP effluent, trichlorfon and perfluorooctanesulfonic acid were with the highest EHPi scores, while 17ɑ-ethinylestradiol and bisphenol A had the highest EHPi scores among 304 contaminants in industrial WWTPs. The prioritized contaminants varied across regions, suggesting a need for tailoring regional measures of wastewater treatment and control.
Screening priority indicator pollutants in full-scale wastewater treatment plants by non-target analysis

Abstract:Wastewater treatment plants (WWTPs) are the main sources of emerging contaminants (ECs) in aquatic environment. However, the standards for limiting emerging pollutants in effluent are extremely lacking. We investigated the occurrence and removal of emerging pollutants in 16 WWTPs in China using non-target analysis. 568 substances screened out were divided into 9 kinds including 167 pharmaceuticals, 113 natural substances, 85 pesticides, 86 endogenous substances, 64 chemical raw materials, 14 personal care products, 17 food additives, 6 hormones and 16 others. And they were divided into 5 fates. Pesticides and pharmaceutical compounds seemed to be the most notable categories, the kinds detected in each sample is the largest compared with other compounds. Besides, the average removal rate of pesticides and pharmaceuticals in all WWTPs were the lowest, at 9.54% and 23.77%, respectively. Priority pollutants were screened by considering distribution of pollutants with different fates. Pollutants with the same fate especially "consistent" in different WWTPs had attracted attention. 4 potential priority pollutants including metoprolol, carbamazepine, 10, 11-dihydro-10, 11-dihydroxycarbamazepine and irbesartan were proposed. And it was found that the 4 compounds, "consistent suspects" and "consistent non-targets" had similar rankings of removal rate in 16 WWTPs, which can reflect the performance of different WWTPs.
Identification of Priority Pharmaceuticals in the Water Environment of China.

Abstract:In recent years, increasing attention has been paid to the trace-level contamination of pharmaceuticals in the water environment all over the world. Considering a large number of pharmaceuticals used, it is crucial to establish a priority list of pharmaceuticals that should be monitored and/or treated first. In the present study, we developed a ranking system based on the pharmaceutical consumption, removal performance in the wastewater treatment plants (WWTPs) and potential ecological effects, and applied to the situation of China. 39 pharmaceuticals, which had available consumption data and also been reported previously in the WWTPs of China, were selected as candidate pharmaceuticals. Among them, seventeen pharmaceuticals were considered as priority pharmaceuticals, out of which, erythromycin, diclofenac acid and ibuprofen, had the high priority. Compared with other literatures, we found that some pharmaceuticals given concerns to globally should also be included in the priority list in China; while some pharmaceuticals, not mentioned in other literatures, such as cefalexin, ketoconazole, should be also given prior consideration in China. Among all the therapeutic classes, antibiotics, which were grossly abused in China, contributed the most to the priority pharmaceuticals. However, priority antibiotics accounted for only 32% of candidate antibiotics, while 71% and 100% of the candidate anti-inflammatory and antilipidemic respectively were identified as the priority pharmaceuticals, indicating that antibiotics might be overanxiously considered in the previous studies on their behaviors in the WWTPs of China.

Abstract:Purpose We analyzed and compared the distributions of 13 target pharmaceuticals in different water samples from the Hangzhou metropolitan area and Linan County, Southeast China. Methods Sampling was conducted in five hospitals, two wastewater treatment plants (WWTPs), and Qiantang River. Samples were concentrated by solid-phase extraction and PPCP concentrations were determined by UPLC-MS/MS. Results and discussion Trimethoprim, erythromycin A dihydrate, norfloxacin, ofloxacin, diclofenac sodium, and atenolol were the most frequently detected pharmaceuticals in hospital effluents. Most of the pharmaceutical concentrations in hospital effluents were higher than those in the WWTP influents. Although both WWTPs adopt the anaerobic–aerobic–anoxic treatment process, the removal rates for pharmaceuticals, such as trimethoprim and diclofenac sodium, were completely different. Meanwhile, erythromycin A dihydrate, ofloxacin, penicillin-G, cephalexin, cefazolin, ibuprofen, and diclofenac sodium were detected in Qiantang River. Conclusions These results indicate that hospitals are more concentrated sources of pharmaceuticals than WWTPs, and the WWTPs are not the only route of entry of pharmaceuticals into aquatic environments in these two regions.
Priority Analysis for Controlling Endocrine Disrupting Chemicals in Municipal Wastewater Treatment Plants of China

Abstract:Ecological risk quotients and estradiol equivalent concentrations of 32 endocrine disrupting chemicals (EDCs) in 4 categories were calculated and analyzed using reported concentrations in secondary effluents from municipal wastewater treatment plants in China, considering both extreme scenario for maximum concentration and moderate scenario for median concentration. These EDCs include industrial chemicals, pesticides, natural estrogens, and pharmaceuticals. In extreme scenario, 12 chemicals of ecological risk and 9 chemicals of estrogenicity were identified. While in moderate scenario, 6 chemicals of ecological risk and 5 chemicals of estrogenicity were identified. After further scoring and ranking, 4 priority EDCs have been suggested for control in municipal wastewater treatment plants in China, including 17a-ethynylestradiol, estrone, nonylphenol, and bisphenol A.
Emerging Organic Contaminants in Chinese Surface Water: Identification of Priority Pollutants

Abstract:The occurrence and impacts of emerging organic contaminants (EOCs) in the aquatic environment have gained widespread attention over the past two decades. Due to large number of potential contaminants, monitoring campaigns, treatment plants, and proposed regulations should preferentially focus on specific pollutants with the highest potential for ecological and human health effects. In the present study, a multi-criteria screening approach based on hazard and exposure potentials was developed for prioritization of 405 unregulated EOCs already present in Chinese surface water. Hazard potential, exposure potential, and risk quotients for ecological and human health effects were quantitatively analyzed and used to screen contaminants. The hazard potential was defined by contaminant persistence, bioaccumulation, ecotoxicity, and human health effects; similarly, the exposure potential was a function of contaminant concentration and detection frequency. In total, 123 compounds passed the preselection process, which involved a priority index equal to the normalized hazard potential multiplied by the normalized exposure potential. Based on the prioritization scheme, 11 compounds were identified as top-priority, and 37 chemicals were defined as high-priority. The results obtained by the priority index were compared with four other prioritization schemes based on exposure potential, hazard potential, or risk quotients for ecological effects or human health. The priority index effectively captured and integrated the results from the more simplistic prioritization schemes. Based on identified data gaps, four uncertainty categories were classified to recommend: ① regular monitoring, derivation of environmental quality standards, and development of control strategies, ② increased monitoring, ③ fortified hazard assessment, and ④ increased efforts to collect occurrence and toxicity data. Overall, 20 pollutants were recommended as priority EOCs. The prioritized list of contaminants provides the necessary information for authoritative regulations to monitor, control, evaluate, and manage the risks of environmentally-relevant EOCs in Chinese surface water.
Which type of pollutants need to be controlled with priority in wastewater treatment plants: Traditional or emerging pollutants?

Abstract:Although wastewater treatment plants (WWTPs) can purify wastewater, they also discharge numerous contaminants into the environment through effluent discharge and sludge disposal. The occurrence, emission flux, and risk assessment of traditional pollutants (e.g., heavy metals [HMs]), and emerging pollutants (e.g., perfluoroalkyl substances [PFASs] and pharmaceutical and personal care products [PPCPs]) in WWTP emissions are of important concern. The present study analyzed 17 PFASs, 25 PPCPs, and 8 HMs in influent, effluent, and excess sludge from six WWTPs along the Yanghe River, North China. Samples were collected during four sampling campaigns from November 2016 to July 2017. The mean concentrations of PFASs and PPCPs in influent were 46.4 ng L−1 and 6.57 μg L−1, respectively; while those in effluent were 38.5 ng L−1 and 2.14 μg L−1, respectively. The highest concentrations of HMs was detected of Zn in influent (2,866 μg L−1) and effluent (3,960 μg L−1). According to the concentration composition, short-chain PFASs, fluoroquinolones (FQs), and Zn were the predominant components in both influents and effluents. The mean PFAS and PPCP concentrations in excess sludge were 5.95 ng g−1 and 3.74 μg g−1 dry weight (dw). Zn was the most abundant HMs in excess sludge with the concentration range of 156–14,271 μg g−1 dw. The compositions of PFASs, PPCPs and HMs differed between wastewater and excess sludge. The estimated emission flux of these pollutants was ordered as HMs > PPCPs > PFASs through effluent discharge and sludge disposal. Sludge disposal discharged more PPCPs and HMs into the environment than effluent discharge, which was contrary for PFASs. Relative risk of each pollutant is calculated by comparing the mean effluent concentration with the median effective concentration. Algae and fish were selected as recipient organisms to calculate the relative risk of 23 selected pollutants towards aquatic organisms. The highest-risk pollutant was Zn on both algae and fish, while perfluorobutane sulfonate (PFBS) and atenolol (ATE) posed the lowest risk. In general, HMs (regarded as traditional pollutants) presented higher risks in effluent, followed by the emerging pollutants (PPCPs and PFASs). Therefore, control of traditional pollutants should be prioritized in WWTPs in this region. This study presents an overall assessment of the current status of traditional and emerging pollutants in WWTPs and provides useful information for upgrading wastewater treatment processes.

An appraisal of the role of laparoscopy: past, present, and future.

Abstract:OBJECTIVE To review the role of laparoscopy in the management of chronic pelvic pain (CPP). METHOD A literature search was conducted to obtain a clear perspective on the role of laparoscopy for CPP. RESULTS Laparoscopy has been widely used as a diagnostic tool in CPP, but often falls short in defining the type and activity of ectopic endometrial-like tissue. Because pelvic endometriosis includes a wide range of lesions, histological confirmation varies greatly. The evaluation of peritoneal endometriosis at laparoscopy can be misleading. In one study 43% of the patients with minimal or mild endometriosis were found at second-look laparoscopy to be free of lesions. Finally, laparoscopy is not without major complications and the risk of undiagnosed bowel injury is likely to be underestimated. CONCLUSION The diagnosis of endometriosis can no longer be limited to the visual inspection of the pelvis but requires a wider range of investigations to assess the reproductive system and the role of endometriotic lesions and adhesions in CPP.
A synthetical methodology for identifying priority pollutants in reclaimed water based on meta-analysis

Abstract:Wastewater reclamation and reuse is an increasing global project, while the reclamation treatment on wastewater does not completely remove all pollutants in water. The residual pollutants in reclaimed water would cause potential risk on human health and ecosystem safety during the long-term use. It is impossible to analyze and control all pollutants one by one in practice, therefore, identification and control of priority pollutants will be efficient strategy to ensure the safe use of reclaimed water. An integrated three-step methodology for identifying priority pollutants in reclaimed water was proposed in this study. First, a comprehensive literature survey on the occurrence of pollutants in reclaimed water was conducted, and a dataset DPR for pollutants occurrence in reclaimed water was established, containing 1,113 pollutants. Second, 611 chemicals that had been recommended as hazardous pollutants for various water bodies in previous literatures were summarized, and a dataset DHP for hazardous pollutants in water was obtained. Third, meta-analysis on these two datasets (DPR and DHP) was performed, a new dataset DHPR for hazardous pollutants in reclaimed water was established, including 265 candidates. Finally, 59 substances out of dataset DHPR were identified as priority pollutants for reclaimed water based on their recommendation frequency. It is expected that this synthetical methodology will provide powerful support for scientific evaluating and managing water pollution and ensuring safe use of reclaimed water.

Abstract:Introduction: Endocrine-disrupting chemicals (EDCs) in effluent and residual sludge from wastewater treatment plants (WWTPs) pose significant environmental and human health risks due to their persistence, bioaccumulation, and difficulty in detection and degradation. This study investigates the environmental exposure and risks associated with EDCs in effluent and sludge from four WWTPs: Tangxi River (TXH), Zipeng Mountain (ZPS), Lianxi (LX), and Wang Xiaoying (WXY). Methods: Environmental exposure indexes of EDCs were assessed in the effluent and sludge of the four WWTPs across four seasons (spring, summer, autumn, and winter) from October 2017 to October 2018. Detection rates of various pollutants, their seasonal and spatial characteristics, and removal rates were analyzed. Positive matrix factorization (PMF) was used for source analysis under influent data, and an ecological risk assessment was conducted using the risk quotient (RQ) method. Results: The study found 4-n-nonylphenol (NP) and di-2-ethylhexyl phthalate (DEHP) had 100% detection rates in the effluent of all four WWTPs, while only DEHP showed a 100% detection rate in the sludge. Bisphenol A (BPA) exhibited the highest concentration in the TXH effluent during autumn. Benzo(a)pyrene (B(a)P) was detected only in the sludge during spring and summer and in the effluent of TXH and WXY. PMF source analysis indicated industrial wastewater discharge as the primary source of pollutants. Ecological risk assessment revealed a high RQ for estriol (E3) in TXH effluent during autumn, and DEHP presented a potential carcinogenic risk through drinking water. Discussion: The findings highlight significant seasonal and spatial variations in EDC concentrations and removal rates across the WWTPs. The persistent presence of DEHP and the high-risk levels of E3 in specific seasons underscore the need for improved treatment processes and stricter industrial discharge regulations to mitigate EDC-related risks. Further research is recommended to explore advanced detection and degradation techniques for EDCs in WWTPs.

Distribution characters and ecological risk assessment of typical PPCPs in sewage treatment plant and its receiving water

Abstract:In recent years, pharmaceuticals and personal care products（PPCPs） in the environment have received increasing attention for their possible continuing threats to ecosystems and human health. The occurrence and concentration changes of 19 typical PPCPs in a domestic sewage treatment plant and its receiving water in Nanjing were investigated. Risk quotients（RQs） were used to evaluate the ecological risk of target PPCPs, and the overall ecological risk was assessed by acute toxicity and estradiol equivalents. The results showed that diclofenac(DCF, 351.42 ng·L -1 ) and carbamazepine(CBZ, 258.3 ng·L -1 ) were the main contributors to PPCP pollutions in effluent. The most hazardous PPCPs in influent of the sewage treatment plant were estriol（E3）, DCF, and clarithromycin（CLA）, while in effluent, were CLA and roxithromycin（ROX）. Urban sewage treatment system had a good removal on most targeted PPCPs. The ecological risks of the targeted PPCPs except for triclosan（TCS） were reduced to different degrees after secondary biochemical treatment. For instance, the ecological risk of ibuprofen（IBU） was reduced from low risk to no risk, CBZ from medium risk to low risk or no risk, CLA from high risk to no risk in winter, and DCF from high risk to medium risk in summer. The acute toxicity of effluent is nontoxic, and the estrogen effect was decreased by 33% ～ 49.8% after sewage treatment, showing that the sewage treatment system could effectively control acute toxicity and estrogenic effects. In addition, ecological purification process played a positive role in alleviating the ecological risk, but the reduction in estrogen effects is limited.

Abstract:The effluents of wastewater treatment plants (WWTPs) are the important sources for pollutants. These pollutants may entry into surface water, groundwater, and soil by recharging and irrigation then pose risk to human beings. Using reports from the past 20 years ( n = 58), we conducted this review of organic contaminants (OCs) in the effluent of Chinese WWTPs. All the studies were during 2005 to 2017 and more than two papers were conducted in every year. Nineteen provinces (20 cities) were covered and most of the studies were located in the eastern part of China, especially the most developed and highest density of population areas. Two hundred eighty-four OCs were contained. E antibiotics, endocrine-disrupting chemicals (EDCs), and pharmaceuticals were the most commonly studied groups of OCs, and ofloxacin, norfloxacin, and sulfamethoxazole were the three most commonly reported OCs. The highest concentration appeared for bisphenol A (BPA). The relative standard deviations (RSDs) of concentrations of the most commonly studied compounds were high. In the future, more studies should pay attention on national analysis and more pollutants.

Abstract:A variety of chemicals discharged into the aquatic environment by the wastewater treatment plant (WWTP), which is a potential source of hazard to the ecological environment and human health. This study established a novel analytical method for all compounds using non-targeted screening to comprehensively explore the fate and transport of organic compounds from WWTP to aquatic environment. 3967 and 3636 features were detected in WWTP samples and river samples, respectively. Multi-level classification was applied to all identified compounds, and results showed that aliphatics were dominant in both abundance and response, accounting for an average of 35.49 % and 74.10 %, respectively. A total of 88 Emerging Contaminants (ECs), including 22 endocrine disrupting chemicals (EDCs), 12 pharmaceuticals and personal care products (PPCPs), 12 pesticides, 10 volatile organic compounds (VOCs), 5 persistent organic pollutants (POPs) and 27 chemicals with other uses, were identified from all compounds, and their traceability analysis was performed. Furthermore, the contribution rate of organic compounds from WWTP effluent to river was calculated to be 33.60 % by the analysis of source-sink relationship. An in-depth and comprehensive exploration of the fate and transport of all organic compounds will help to provide guidelines for the treatment technologies and achieve the traceability of pollutants.

Abstract:Effluent discharged from urban wastewater treatment plants (WWTPs) is a major source of emerging contaminants (ECs) requiring effective regulation. To this end, we collected discharge datasets of pharmaceuticals (PHACs) and endocrine-disrupting chemicals (EDCs), representing two primary categories of ECs, from Chinese WWTP effluent from 2012 to 2022 to establish an exposure database. Moreover, high-risk ECs' long-term water quality criteria (LWQC) were derived using the species sensitivity distribution (SSD) method. A total of 140 ECs (124 PHACs and 16 EDCs) were identified, with concentrations ranging from N.D. (not detected) to 706 μg/L. Most data were concentrated in coastal regions and Gansu, with high ecological risk observed in Gansu, Hebei, Shandong, Guangdong, and Hong Kong. Using the assessment factor (AF) method, 18 high-risk ECs requiring regulation were identified. However, only three of them, namely carbamazepine, ibuprofen, and bisphenol-A, met the derivation requirements of the SSD method. The LWQC for these three ECs were determined as 96.4, 1010, and 288 ng/L, respectively. Exposure data for carbamazepine and bisphenol-A surpassed their derived LWQC, indicating a need for heightened attention to these contaminants. This study elucidates the occurrence and risks of ECs in Chinese WWTPs and provides theoretical and data foundations for EC management in urban sewage facilities.

Abstract:HIV prevalence is higher in jails than in the community, yet many jails do not conduct HIV testing. Jails in Baltimore, Maryland; Philadelphia, Pennsylvania; and the District of Columbia have implemented innovative rapid HIV testing programs. We have summarized the results of these programs, including the numbers of persons tested, rapid and confirmatory HIV test results, and numbers of persons newly diagnosed with HIV. We have described facilitators and challenges of implementation. These programs confirmed that rapid HIV testing in jails was feasible and identified undiagnosed HIV infection. Challenges included limited space to provide confidential rapid HIV testing and rapid turnover of detainees. Implementation required collaboration between local governments, health agencies, and correctional institutions. These programs serve as models for expanding rapid HIV testing in jails.

The Science of The Total Environment

Abstract:There has been increasing concern regarding the adverse environmental and health effects of organic pollutants. A list of priority control organic pollutants (PCOPs) can provide regulatory frameworks for the use and monitoring of organic compounds in the environment. In this study, 20,010 groundwater samples were collected from 15 "first level" groundwater resource zones in China. Fifty (50) organic compounds were analyzed based on their prevalence, occurrence, and physicochemical properties (persistence, bioaccumulation, and toxicity). Results showed that 16 PCOPs, including 12 pesticides, 3 aromatic hydrocarbons (AHs), and 1 phthalate ester, were recognized. Pesticides and AHs accounted for 75 % and 18.75 % of the high-priority pollutants, respectively. There were significant differences in PCOPs between confined and phreatic groundwater. Higher concentrations of pesticides were mainly detected in phreatic groundwater. PCOPs detected in samples from the 15 groundwater resource zones were mainly pesticides and AHs. The groundwater data indicate that the organic compounds detected in the Yellow River Basin (YRB), Yangtze River Basin (YZB), Liaohe River Basin (LRB), and Songhua River Basin (SRB) are mainly categorized as Q1 (high priority) and Q2 (medium priority) pollutants based on the contaminants ranking system in China. The findings from this study offer a snapshot of the wide distribution of PCOPs in the surveyed regions, and are expected to establishing treatment and prevention measures at both the regional and national levels in China.
