Long-term Surgical Outcomes of Initial Postoperative Overcorrection in Adults with Intermittent Exotropia

Abstract:Purpose: To evaluate the relationship between initial postoperative overcorrection and long-term surgical success in exotropia patients. Methods: The medical records of 46 patients who underwent surgery for intermittent exotropia after the age of 18 were enrolled. Enrolled patients also had at least 2 years of postoperative follow-up. Based on the initial postoperative deviation at distance measured by prism and the alternating cover test at 1 week, patients were assigned to one of the following groups: group A included patients who demonstrated any esodeviation, while group B included patients who showed orthophoria to exodeviation of 10 prism diopters. The records were analyzed to determine the preoperative deviation with stereoacuity and postoperative deviations with stereoacuity at the follow-up examinations at the following intervals: 1 week; 1, 3, and 6 months; and 1 and 2 years. A comparison between groups for demographic data and preoperative and postoperative angles of deviation was performed using analysis of variance. Results: Of the 46 patients with intermittent exotropia included in this study, 18 (39%) belonged to group A, while 28 (73%) belonged to group B. The postoperative angle of deviation for distant fixation until 2 years of follow-up showed statistically significant differences in each group (p < 0.003 in all comparisons). The amount of exodrift until 2 years in group A (from ?9.7 ± 6.1 to 1.6 ± 3.7) was greater than that in group B (from 2.0 ± 2.7 to 6.8 ± 5.6). The long-term surgical success rate within 2 years of surgery was significantly better in group A than in group B (p = 0.027). The number of patients with intermittent diplopia and the duration of diplopia were greater in group A (n = 8) than in group B (n = 2). Conclusions: Long-term surgical success was achieved in 89% of patients who were initially overcorrected. Overcorrection of an average of 10 prism diopters at the first postoperative week was found to be associated with a more favorable long-term surgical outcome.
Predictors of surgical success in patients with intermittent exotropia

Abstract:Background: In patients with moderate- to large-angle exotropic deviations, surgical correction remains the definitive treatment. The purpose of this study was to identify preoperative factors that correlate with surgical success and to produce a binomial model that predicts success based on preoperative factors. Methods: We performed a retrospective review of patients with intermittent exotropia who underwent uniocular recession-resection surgery. Success was defined as ocular deviation ≤10Δ exotropia and ≤5Δ esotropia at distance at last follow-up (at least 3 months' postoperatively). Preoperative factors such as age at surgery, sex, visual acuity, spherical equivalent, prism fusion range, stereopsis, and ocular deviation were analyzed for correlation with success using binomial logistic regression. Results: A total of 82 patients were identified (average age, 33 years; range, 3.7-81.6 years). Average prismatic deviation was 35Δ at near and 34Δ at distance. Average stereopsis was 167″. The average success rate was 58.5%. Univariate binomial regression revealed that patients with smaller angle of deviation at near (OR = 0.96, P = 0.013) or distance (OR = 0.96, P = 0.005), larger myopic refractive errors in terms of mean spherical equivalent (OR = 0.71, P = 0.022) and spherical equivalent in the more myopic eye (OR = 0.75, P = 0.029) contributed to success. A multivariate regression model was able to predict success with an accuracy of 72% (sensitivity, 81%; specificity, 58%; negative predictive value, 67%; positive predictive value, 74%). Conclusions: A smaller preoperative angle of deviation and larger myopic refractive error correlated with success in uniocular intermittent exotropia surgery in our cohort. Larger deviations and more hyperopia correlated with lower success rates.
Analysis of postoperative exodrift according to surgical methods for intermittent exotropia

Abstract:Purpose: This study aimed to compare the periods for exodrift stabilization and the long-term surgical outcomes among different surgical methods in intermittent exotropia. Methods: The medical records of 350 patients who had undergone intermittent exotropia correcting surgery [unilateral lateral rectus recession-medial rectus resection (R&R, n = 221), bilateral lateral rectus recession (BLR, n = 51) and unilateral lateral rectus recession (ULR, n = 78)] with a postoperative follow-up period of 1.5 years or more were retrospectively reviewed. The deviation angles every 6 months after surgery, periods of exodrift stabilization and surgical outcomes were analysed. The period of postoperative exodrift stabilization was defined as when exodrift was no longer significantly different from that at the next visit (p > 0.05). An alignment of 5 PD (prism diopters) esotropia to 10 PD exotropia at a distance and near fixation was considered surgical success. Results: The mean angle of exodeviation was significantly different among surgical procedures at postoperative 1 month (BLR > ULR > R&R, p < 0.001); however, there were no differences among the procedures at 6 months (p = 0.088). The periods of exodrift stabilization were 6 months after ULR, 1 year after BLR and 4.5 years after R&R. The surgical success and reoperation rates did not show significant differences among procedures at the final follow-up. Conclusion: Patients undergoing R&R showed smaller exodeviation shortly after surgery but required a longer period to stabilize the exodrift. Following ULR, exodeviation was larger in the early postoperative period, but the exodrift was stabilized earlier. Therefore, the long-term surgical outcomes were similar among ULR, BLR and R&R.
Unilateral recession and resection surgery with adjustable suture in adult sensory exotropia: long-term success and exotropic drift

Abstract:We included adult patients (age ≥18 years) with sensory exotropia who underwent unilateral lateral rectus recession with adjustable suture and medial rectus resection with at least 2-year follow-up. Preoperative and postoperative data was collected. Either Krimsky or alternate and prism cover testing was used to measure the angle of deviation depending on patients' fixation ability. Glasses with full refractive correction were worn if they can provide better visual acuity. Exodeviation at near fixation was used in statistical analysis. A total of 38 patients (mean age at surgery 32 ± 13 years) were included. Median preoperative exotropia was 45 PD (IQR 40-55, range 25-70) and median initial postoperative deviation was esotropia 8 PD (IQR 0-11, range from exotropia 8 to esotropia 30). Mean follow-up time was 5 ± 2.5 years (range 2-10). Motor success, defined as a deviation within 10 PD of orthotropia, was found in 50% and 42% of patients at 2-year and last follow-up. Initial postoperative deviation and amount of lateral rectus recession were significantly associated with long-term outcome (p = .006 and 0.029). Patients with initial moderate and minimal overcorrection had higher success rates (67% and 62%) than patients with initial orthotropia or undercorrection (10%) (p = .002). Median overall rate of exotropic drift was 8 PD/year (IQR 6-13, range from esotropic drift 8 to exotropic drift 26). Time to failure in nonsuccess group was 24 months (IQR 21-24, range 2-24). In conclusion, unilateral recession and resection surgery with adjustable suture in adult sensory exotropia achieved 50% success at 2 years. Initial postoperative moderate overcorrection results provide the highest chance for long-term success. The majority of patients developed exotropic drift with time and time to failure was 2 years.
Long-term ocular alignment after bilateral lateral rectus recession in children with infantile and intermittent exotropia

Abstract:Purpose: To compare postoperative drift after bilateral lateral rectus recession for infantile exotropia (XT) and for intermittent XT and to compare initial postoperative alignment with long-term motor outcome. Methods: Medical records of all patients with infantile exotropia or intermittent exotropia who had undergone bilateral lateral rectus muscle recession surgery with a follow-up longer than 3 years were reviewed. The pre- and postoperative angles of deviation at distance and at near and postoperative drift at distance were compared. Surgical outcome was categorized as "success" (esotropia <6(Δ) or exotropia <11(Δ)), "recurrence" (>10(Δ) exotropia), or "overcorrection" (>5(Δ) of esotropia). Results: The overall mean postoperative exotropic drift at 3 years was 10.4(Δ) in the infantile XT group and 7.2(Δ) in the intermittent XT group (P = 0.05). Both groups had a low success rate at 3 years: 41% in the infantile XT group and 51% in the intermittent XT group (P = 0.270). For patients with an initial esotropia of 0(Δ) to 10(Δ), the success rate at 3 years was 86% in the infantile XT group (12 of 14) and 65% in the intermittent XT group (28 of 43). Conclusions: Postoperative exotropic drift is clinically similar in patients with intermittent versus infantile exotropia. Esotropia of 0(Δ) to 10(Δ) during the early postoperative period may be associated with the best long-term ocular alignment.
Analysis of Postoperative Outcomes in Patients with Intermittent Exotropia According to Preoperative Change of Angle of Deviation

Abstract:Purpose: The correlation between the existence of the preoperative condition of ≥10 prism diopters (PD) in patients with basic type of intermittent exotropia (IXT) and postoperative outcomes was analyzed. Methods: The medical records of patients that underwent surgery for IXT were analyzed retrospectively. The analysis was conducted by dividing the patients into a group with change of <10 PD (group 1) and ≥10 PD (group 2) before the time of the surgery. Patients who received at least 6 months of follow-up after surgery were included. The age, sex, angle of deviation and stereoacuity of the patients were studied. Surgical success was defined as exodeviation of <10 PD or esodeviation of <4 PD at the final visit after the surgery. The correlation between clinical factors and surgical success rate was analyzed by using correlative analysis. Results: A total of 129 patients participated in the study. There were 108 (83.7%) and 21 (16.3%) patients in groups 1 and 2, respectively. There were 89 (82.4%) and 17 (80.1%) patients with surgical successes in groups 1 and 2, respectively (p = 0.18). Moreover, 13 (12.0%) patients in group 1 and three (14.3%) patients in group 2 required reoperation, showing no significant difference (p = 0.12). There was no statistically significant correlation between surgical success and preoperative change of angle of deviation <10 PD (odds ratio, 1.78; p = 0.17). Conclusions: Among the patients with basic type of IXT subjected to the analysis, 16.3% had a change of ≥10 PD before surgery, and there was no significant correlation between surgical success and preoperative change of angle of deviation.
Surgical Outcomes of Patients with Diagnostic Preoperative Monocular Occlusion in Intermittent Exotropia

Abstract:We evaluated surgical outcomes of bilateral rectus (BLR) recession in patients with intermittent exotropia who underwent diagnostic monocular occlusion. Records of patients with intermittent exotropia who were preoperatively examined one-hour monocular occlusion and underwent BLR recession were reviewed retrospectively. Patients were classified into two groups based on change in exodeviation angle: responders (≥5 change in prism diopters [PD] after occlusion) or non-responders (<5 PD change after occlusion). BLR recession amount was determined by maximal angle deviation after occlusion tests in both groups. Overall follow-up period was 38.81 ± 24.09 months for non-responders (n = 106) and 38.52 ± 19.87 months for responders (n = 142) (p = 0.766). There was no difference in deviation before monocular occlusion between the two groups. Mean angle of deviation at distance (24.23 ± 6.27 PD) and near fixation (25.46 ± 6.78 PD) increased to distance (29.95 ± 6.43 PD) and near deviation (32.15 ± 6.17 PD) after occlusion in the responder group. At postoperative 1 year, surgical success rate was higher in responders (71.1%) than in non-responders (52.8%) (p = 0.003). Kaplan-Meier survival analysis revealed significantly higher surgical success in responders (p = 0.001, log-rank test). Preoperative diagnostic monocular occlusion in patients with intermittent exotropia can influence surgical outcomes by identifying the latent exodeviation angle.
Factors affecting surgical outcome of intermittent exotropia

Abstract:Purpose: The purpose of this study was to investigate the factors affecting surgical outcome in intermittent exotropia. Designs: This was a retrospective interventional study. Methods: Intermittent exotropic patients who had undergone surgical correction with a postoperative follow-up period of 1 month or more were included in the study. Surgical success was defined as an alignment between 10 prism diopters (PD) of exotropia or 5 PD of esotropia at 1 month. After data collection, data were analyzed in SPSS version 23 software. The main outcome measures were the factors affecting surgical outcome. Results: We included 101 patients, including 52 (51.5%) male and 49 (48.5%) female. Among them, 62 (61.4%) patients achieved surgical success. Undercorrection was the primary reason of surgical failure. Multivariate regression analysis showed that a larger preoperative angle of deviation was associated with unfavorable surgical outcome (P = 0.053, odds ratio [OR] =0.97, 95% confidence interval [CI] = 0.94-1.00), and the presence of postoperative day 1 (POD 1) diplopia correlated significantly with higher surgical success (P = 0.001, OR = 4.54, 95% CI = 1.80-11.43). The presence of POD 1 diplopia was highly associated with POD 1 esotropia (P = 0.005, OR = 7.26, 95% CI = 1.84-28.58). Conclusion: In intermittent exotropia, larger preoperative angle of deviation may predict a lower surgical success rate. Despite a worrisome issue, the presence of diplopia on first POD is associated with immediate postoperative alignment of esotropia and predicts a higher surgical success.
The long-term surgical outcomes of lateral rectus advancement in consecutive esotropia

Abstract:Purpose: To evaluate the long-term surgical efficacy of lateral rectus advancement (LRadv) in patients with consecutive esotropia (CET). Methods: The medical records of 30 patients who developed CET after bilateral lateral rectus (BLR) recession for exotropia (XT) between 2012 and 2020 were reviewed. The characteristics of patients during their XT surgery were summarized. Among them, 15 patients who underwent LRadv as CET treatment with at least a 1-year follow-up were included to evaluate the long-term efficacy of this surgical approach. The main outcomes were the pre- and post-operative angle of deviation after LRadv. Surgical success was defined as the postoperative deviation within 10 prism diopters (PD), and reoperation was not needed. All data were expressed as median and interquartile ranges. Results: The median follow-up was 34 months after LRadv surgery. The median postoperative deviation ranged from 28 to 1 PD at 1 year (P < 0.05) and to 5 PD at the final follow-up (P < 0.05). The deviation at each follow-up time showed no statistically significant difference (P > 0.05). The final surgical success was reduced compared to 1 day and 1 year postoperatively (60% VS 100% and 66.7%, respectively). Undercorrection and overcorrection both occurred at the final follow-up. Conclusion: Although the immediate surgical outcome of LRadv was satisfactory, the success rate reduced with time, which suggests long-term observation is necessary to detect and timely provide appropriate interventions for overcorrection or undercorrection.
Intermittent exotropia: relation between age and surgical outcome: a change-point analysis

Abstract:Purpose: To study the relationship between age and response to surgery in patients with intermittent exotropia and to identify change points in response to surgery. Methods: A retrospective analysis was conducted on 311 patients with intermittent exotropia who had bilateral lateral rectus recession using standard tables with minimum follow-up of 6 months. Data were analyzed using the change-point analysis software to identify cutoff points. A prospective pilot study was then performed on 171 consecutive patients with intermittent exotropia with the same clinical characteristics, in whom amount of recession was modified according to the identified cutoff points. In angles with two change points, 1-mm recession was reduced from patients younger than the lower change point and 1.5-mm recession was added to those older than the upper change point. In angles with one change point, 1.5-mm recession was added to those older than the change point. Satisfactory alignment was defined as esophoria/tropia ≤ 5Δ to exophoria/tropia ≤ 8Δ. Results: There was a negative correlation (P<0.01) between response to surgery and age at surgery for all angles. In younger patients (<7 years) in whom surgical dose was reduced, there was no significant change in success rate (77%), compared with those who had surgery using standard tables (75%). In older patients (>12 years) in whom surgical dose was increased, there was a statistically significant increase in success rate (80% vs 41%). Conclusions: Modifying the surgical dose according to age can improve the success in patients with intermittent exotropia.
Outcomes for Intermittent Exotropia Using Three Common Surgical Approaches

Abstract:Purpose: To compare a single surgeon's surgical outcomes for treating intermittent exotropia using bilateral lateral rectus recession (BLR), unilateral lateral rectus recession and medial rectus resection (RR), and unilateral lateral rectus recession and medial rectus plication (RP). Methods: A retrospective review of all surgeries for basic intermittent exotropia between 2015 and 2023 was performed. Only patients with initial correction using BLR, RR, or RP were included. Exclusion criteria included age older than 18 years, vertical deviation, any nonrefractive ocular diagnoses, prior ocular surgery, and inadequate follow-up. Results: There were 460 patients identified; 123 met inclusion criteria with 54 in the BLR group, 41 in the RR group, and 28 in the RP group. The average preoperative distance alignment (and standard error) values for the BLR, RR, and RP groups were 25.07 (7.35), 22.44 (5.95), and 23.84 (6.42) prism diopters (PD), respectively. At 1 year, the postoperative distance alignment values for the BLR, RR, and RP groups were 8.72 (7.89), 7.46 (6.31), and 12.83 (6.82) PD, respectively ( P = .03). A subanalysis found a significant difference between the BLR and RP ( P = .02) and RR and RP ( P = .02) groups. There was no difference between the BLR and RR groups ( P = .57). Conclusions: This study of three surgical approaches for intermittent exotropia found RP had a significantly larger angle of exodeviation compared to BLR and RR at 1 year of follow-up. Comparative Study of Bilateral Versus Unilateral Strabismus Surgery in the Management of Lateral Incomitance in Intermittent Exotropia

Abstract:Purpose: To evaluate and compare bilateral asymmetric lateral rectus recession versus unilateral resection-recession surgery in the management of lateral incomitance in intermittent exotropia. Methods: A prospective randomized interventional comparative study was conducted consisting of 80 patients with intermittent exotropia (older than 7 years) having significant lateral incomitance. They were equally divided into two groups by a sealed envelope system. The bilateral group underwent bilateral asymmetric lateral rectus recession and the unilateral group underwent unilateral lateral rectus recession and medial rectus resection based on post-patch deviation. Parameters assessed were change in horizontal deviation, change in lateral incomitance, binocularity, motility limitation, and complications, if any. Surgical outcome was considered successful if the primary deviation was within ±8 prism diopters (PD) and lateral incomitance was less than 5 PD. Results: The mean lateral incomitance preoperatively and postoperatively was 8.3 ± 1.6 and 2.8 ± 2.4 PD in the bilateral group and 8.9 ± 1.4 and 3.7 ± 2.5 PD in the unilateral group, respectively. There was no statistically significant difference in the postoperative lateral incomitance between the two groups (P = .25), but a statistically significant difference was observed between preoperative and postoperative lateral incomitance in each group (P < .0001). Conclusions: Both procedures are equally efficacious in achieving acceptable ocular alignment and improving significant lateral incomitance in patients with intermittent exotropia with deviations between 15 and 35 PD without causing significant motility limitation. Abstract:Although there were many prior studies about exotropia, few focused on large-angle intermittent exotropia. The goal of this study was to evaluate single-stage surgical outcomes for large-angle intermittent exotropia and analyze risk factors that may affect the success of surgery. Records from intermittent exotropia patients with exodeviations >60 prism diopters(PD) who were surgically treated at the Zhongshan Ophthalmic Center, of Sun Yat-Sen University were reviewed. Included within this review were data on, pre- and post-operative ocular motility, primary alignment, binocular vision and complications. Patients with exodeviations ≤70PD received two-muscle surgery, while those with exodeviations >70PD were subjected to a three-muscle procedure. A total of 40 records were reviewed. The mean exodeviation was 73±9PD at distance and 75±26PD at near. There were 25 patients received two-muscle surgery and 15 the three-muscle procedure. Orthophoria (deviation within 8PD) was obtained in 77.5% of these patients and the ratios of surgical under-correction and over-correction were 15% and 7.5% respectively. However, when combining ocular alignment with binocular vision as the success criteria, success rates decreased to 30%. No statistically significant differences in success rates were obtained between the two- and three-muscle surgery groups. Seven subjects experienced an abduction deficit during the initial postoperative stages, but eventually showed a full recovery. One patient required a second surgery for overcorrection. No statistically significant risk factors for poor outcome were revealed. Our data showed that single-stage two- and three-muscle surgeries for large-angle intermittent exotropia are effective in achieving a favorable outcome.
Long-term Outcomes of Bilateral Lateral Rectus Recession Using a Modified Normogram for Targeting Small Overcorrection in Intermittent Exotropia

Abstract:Purpose: To evaluate the surgical outcomes of a modified normogram for bilateral lateral rectus recession (BLR) in the surgical management of intermittent exotropia, with a focus on achieving small overcorrection. Methods: A retrospective medical chart review of 242 patients who underwent surgery for intermittent exotropia between October 2014 and June 2020 was performed. Our modified normogram reducing amount of recession targeted for small postoperative overcorrection within 5 prism diopters (PD). Patients were observed for at least 3 years. Esodeviation was denoted by negative numbers and positive values indicated exodeviation. A satisfactory outcome was defined as if distant angle of deviation ranged between ≤10 PD of exotropia and ≤5 PD of esotropia. Recurrence was defined as an exodeviation of >10 PD at distance. Overcorrection was defined as an esodeviation of >5 PD at distance. Results: The mean preoperative deviation angles were 29.32±3.92 PD at distance and 29.88±4.41 PD at near. At postoperative day 1, the mean deviation angles at distance and near were -6.22±1.44 PD and -6.22±1.49 PD, respectively. Overcorrection within 6 PD of esodeviation was seen in 168 patients (69.42%), undercorrection with exophoria was noted in 11 patients (4.55%), and overcorrection by more than 6 PD of esodeviation was seen in 63 patients (26.03%) at postoperative day 1. The long-term success rate at postoperative 3 years was 77.27% (187/242), with a recurrence rate of 19.83% (48/242) and an overcorrection rate of 2.89% (7/242). A positive correlation between the angle of deviation at postoperative day 1 and 3 years was observed (r= 0.22, p=0.001). The final success rate at the final examination after a mean follow-up of 38.60±7.99 months was 83.06%. Conclusion: Our modified normogram for BLR in intermittent exotropia surgery resulted in successful surgical outcomes with small overcorrection, reducing the risk of consecutive esotropia due to excessive overcorrection.
Long-Term Surgical Outcomes of Augmented Bilateral Lateral Rectus Recession in Children With Intermittent Exotropia

Abstract:Purpose: To compare the long-term surgical outcomes of augmented bilateral lateral rectus (LR) recession with original surgery. Design: Retrospective, nonrandomized clinical study. Methods: A total of 447 children with ≤35 prism diopters (PD) of basic and divergence excess-type intermittent exotropia, who underwent original bilateral LR recession based on the largest angle measured at distance and near, or augmentation surgery with the surgical dosage augmented by 1.0-1.5 mm more than the original formula were included. Patients were observed for at least 2 years. Success rates, cumulative probabilities of success, factors related to recurrence, and overcorrection were evaluated. Results: At a mean follow-up of 4.0 years, 48 of 101 patients (48%) undergoing original surgery maintained successful alignment within 10 PD of exophoria/tropia and 5 PD of esophoria/tropia; 49 (49%) had recurrence, and 3 (3%) had overcorrection. After augmented surgery, 203 of 346 patients (59%) were successfully aligned, 129 (37%) had recurrence, and 14 (4%) had overcorrection. Augmented surgery showed higher long-term successful alignment rates (P= .047) and lower recurrence rates compared to original surgery (P = .042) and the overcorrection rate was similar between the 2 groups (P= .774). Patients with divergence excess type showed higher cumulative success rates compared to that of the basic type (P = .010) after augmented surgery. Conclusion: Augmented bilateral LR recession resulted in more successful alignment and lower recurrence without higher overcorrection compared to the original surgery for the children with intermittent exotropia. Augmentation of the original table should be considered when planning bilateral LR recession, especially in patients with divergence excess-type exotropia.

Abstract:We compare the surgical outcomes of intermittent exotropia of the basic, pseudo-divergence excess (pseudo-DE) and true divergence excess (true DE) types. A study was performed with 342 patients who had undergone surgery for intermittent exotropia of the basic, pseudo-DE or true DE type with a postoperative follow-up period of 6 months or more. The main outcome measures were postoperative angles of deviation at distance and near, and surgical success rates. Surgical success was defined as alignment between exodeviation of 10 PD and esodeviation of 5 PD at distance and near. Additionally, survival curves of recurrence were analyzed by the Kaplan-Meier method. The postoperative angles of deviation at both distance and near in pseudo-DE type were significantly smaller than those in basic type at the final examination (p = 0.003, <0.001). The final surgical success rate in pseudo-DE (70.2%) was better than in basic (46.3%) or true DE (28.6%) (p = 0.003, 0.01). Reoperation for recurrent exotropia was performed in 27% of the basic, 17% of the pseudo-DE, and 35.7% of the true DE cases. According to a survival analysis for recurrence, patients with pseudo-DE showed lower incidence of recurrence than did patients with basic and true DE (p = 0.003, 0.02). In conclusion, the patients with intermittent exotropia of the pseudo-DE type showed better surgical outcomes than those with the basic or true DE type. Pseudo-DE also showed a lower recurrence rate than did the other 2 groups.

Abstract:Introduction: Intermittent exotropia (IXT) is commonly seen in areas with more sunlight, females and Asians. Surgical alignment is recommended to improve binocular function, but a high recurrence rate has been reported. This study aimed to assess the stability of ocular alignment and factors contributing to exodrift post-surgery for IXT over 6-month follow-up. Methods: We retrospectively reviewed the medical records of patients with IXT who underwent unilateral recess resect procedures and attended follow-up examinations for at least six months post-surgery. A significant exodrift was defined as exodrift of more than 10PD at distance and near. Ocular deviation, binocularity, and stereopsis at one and six-month post-operative visits were studied to assess the incidence of exodrift post-surgery and the probable factors causing such drift. Results: Of the 50 patients studied, 26 (52%) were males, 42 (84%) were <20 years. The median age at surgery was 10 years (range 6-14), and the median preoperative angle of deviation was 37 PD. Majority of patients (62%) had exodrift within six months of surgery, and about a quarter of the study population had significant exodrift >10 PD. No clinical factors were found to contribute significantly to exodrift in this study.16 (61.5%) patients attained near and distance BSV post-surgery (p-value: <0.001). Six (12%) and 10 (20%) patients with subnormal near and distance stereopsis, respectively, attained normal stereopsis post-surgery. Conclusion: Over half of the patients operated for IXT had some Exodrift, and one-fourth had significant exodrift within six months post-surgery. Despite improved BSV, many patients failed to attain normal stereopsis with just motor alignment.
