Abstract:Background: Actinic keratoses (AK) may progress into squamous cell carcinoma. Many combination therapies were used to improve the clearance rate. However, there are limited studies on the efficacy and safety of lesion-directed Liquid nitrogen cryotherapy combined with field-directed photodynamic therapy (PDT). Objective: This study is aimed to determine the efficacy and safety of Liquid nitrogen cryotherapy combined with PDT in Chinese patients with actinic keratosis. Methods: A total of 61 patients with AK were selected at the Institute of Dermatology, Chinese Academy of Medical Sciences and Peking Union Medical College between August 2018 and August 2023. They were randomly divided into PDT plus Liquid nitrogen cryotherapy group (LN-PDT) and control PDT group (C-PDT) with 30 patients and 31 patients, respectively. 27 patients in the LN-PDT group and 28 patients in the C-PDT group completed the clinical sessions. Efficacy, adverse effects, and cosmetic outcomes were recorded and compared between the two groups. Results: 3 months after the final treatment, the clearance rates for total AK lesions were 97.4 % (449/461) in the LN-PDT group and 93.4 % (456/488) in the C-PDT group (P < 0.05). For grade I AK lesions, the clearance rates were 99.2 % (234/237) for the LN-PDT group and 98.3 % (237/241) for the C-PDT group (P >0.05). For grade II lesions, the clearance rates were 97.5 % (156/160) for the LN-PDT group and 91.9 % (172/186) for the CPDT group (P < 0.05). For grade III lesions, the clearance rates of the LN-PDT and C-PDT groups were 87.5 % (58/64) and 78.7 % (47/61), respectively (P < 0.05). The two groups had no significant differences in pain, erythema, edema, hyperpigmentation and scarring. Cosmetic outcomes mainly were excellent or good in both groups with no significant difference. Conclusion: Liquid nitrogen cryotherapy combined with PDT showed higher efficacy on grade II and grade III AK lesions than PDT alone. The two groups have similar adverse effects and cosmetic outcomes.

Abstract:We propose a high-speed and power-efficient CMOS-inverter-based driver for a silicon photonics-based segmented Mach-Zehnder modulator (MZM). The proposed driver is equipped with area-efficient three-dimensional inductors, which are suitable for flip-chip assembly with a compact area and for inductive peaking to increase the data rate. We also employ a dual-drive push-pull configuration to drive the MZM with large voltage swing in 65-nm CMOS technology. The simulated power consumption of the proposed CMOS driver is 110 mW with 1.1-/2.2-V dual voltage supplies for an output swing of 4.0 Vppd and 40-Gb/s operation. The active area of the designed CMOS driver chip is only 230 × 280 µm2, including all the inductors. We also designed a segmented MZM with two 500-µm sections, and simulated the extinction ratio of the designed MZM with the driver is 2.4 dB at 40-Gb/s.

Abstract:Background Actinic keratosis (AK) is a precancerous lesion that occurs in areas that are chronically exposed to sunlight and has the potential to develop into invasive cutaneous squamous cell carcinoma (cSCC). We investigated the efficacy of 20% 5-aminolevulinic acid-photodynamic therapy (ALA-PDT) with LED red light for the treatment of AK in Chinese patients by examining changes in dermoscopic features, histopathology and fluorescence after treatment. Methods Twenty-eight patients with fourty-six AK lesions from March 2022 to September 2023 were treated with 20% ALA, and 3 hours later, they were irradiated with LED red light (80-100 mW/cm 2 ) for 20 minutes. A session of 20% ALA-PDT was performed once a week for three consecutive weeks, and the dermoscopic, histopathological, fluorescent and photoaging outcomes were measured one week after the treatment. Results One week after ALA-PDT, complete remission (CR) was reached in 53.6% of patients. The CR of Grade I AK lesions was 100%, that of Grade II lesions was 71.4%, and that of Grade III lesions was 38.1%. There was a significant improvement in the dermoscopic features, epidermal thickness and fluorescence of the AK lesions. The presence of red fluorescence decreased, and there was an association between CR and post-PDT fluorescence intensity. ALA-PDT also exhibited efficacy in treating photoaging, including fine lines, sallowness, mottled pigmentation, erythema, and telangiectasias, and improved the global score for photoaging. There were no serious adverse effects during or after ALA-PDT, and 82.1% of the patients were satisfied with the treatment. Conclusion AK lesions can be safely and effectively treated with 20% ALA-PDT with LED red light, which also alleviates photoaging in Chinese patients, including those with multiple AKs. This study highlights the possibility that fluorescence could be used to diagnose AK with peripheral field cancerization and evaluate the efficacy of ALA-PDT.

Abstract:Background Aminolevulinic acid photodynamic therapy (ALA‐PDT) is an effective treatment for multiple actinic keratosis (AK). However, PDT‐induced pain often discontinues the therapy to reduce its efficacy, limiting its application. If modified painless PDT schedule with shorter photosensitizer dressing and higher dose illumination could achieve good efficacy in AK, it is still unknown. Objectives To explore the efficacy and pain tolerance of the modified painless PDT (M‐PDT) in facial multiple AK. Methods A split‐face controlled clinical study including 14 patients with facial multiple AK was conducted. The patients received conventional PDT (C‐PDT) on the left and M‐PDT in the contralateral area. The left area (C‐PDT) was illuminated by a red light‐emitting diode light (144 J/cm2) after applying the 10% ALA cream for 3 h; the other had illumination for a total light dose of 288 J/cm2 after applying the 10% ALA cream for 0.5 h. The primary endpoint was the lesion clearance rate at 1‐month postthree sessions of PDT. Secondary endpoints included pain scores, the incidence of adverse events during treatment, and cosmetic outcomes. Results At 1 month following three treatments, the total lesion clearance rate was comparable between M‐PDT and C‐PDT (91.6% vs. 89.0%). Abstract:To the Editor: Actinic keratosis (AK) is a precancerous skin condition caused by chronic exposure to ultraviolet radiation. It mainly occurs on sun-exposed areas like the head and face, leading to cosmetic concerns and an increased risk of progressing to primary squamous cell carcinoma (with a 0.60% risk at 1 year and 2.57% risk at 4 years). [ 1 ] Photodynamic therapy (PDT) is a targeted, non-invasive therapy that has been approved in Europe for the treatment of AK, squamous cell carcinoma in situ , and basal cell carcinomas. 5-Aminolaevulinic acid (ALA)-based PDT, which utilizes the photosensitizer ALA, has shown high cure rates, low recurrence rates, and favorable cosmetic outcomes as a first-line treatment for AK. [ 1 ] However, due to variations in skin color, ethnicity, and lifestyle habits, the incidence of AK is lower among Asians compared to Caucasians. [ 1 ] There are limited clinical studies on PDT for AK in Asia—particularly lacking large-scale multicenter trials. This knowledge gap hinders the establishment of treatment standards specifically tailored for Asian patients with AK undergoing PDT. Nevertheless, as the aging population increases in Asia, the incidence of AK demand for medical intervention among Asians is also rising accordingly. We conducted this multicenter study aiming to comprehensively investigate the efficacy and safety of ALA-PDT in Chinese patients with AK.

Abstract:Background Primary endpoints of clinical studies investigating treatments for actinic keratosis (AK) are mainly based on clinical evaluation, but a recent study showed that in AK, clinical classification according to Olsen and the extent of keratinocyte atypia do not necessarily correlate. The influence of the epidermal extent of atypia on treatment efficacy is usually not investigated and therefore remains largely unknown. Objective To evaluate whether the extent of keratinocyte atypia influences efficacy of photodynamic therapy (PDT) when treating AK. Methods We performed a post-hoc analysis of histological (keratinocyte intraepithelial neoplasia (KIN)), and clinical (Olsen) data of biopsied lesions of three pivotal studies evaluating PDT using 10 % aminolevulinic acid (ALA) gel or vehicle and narrow- or broad-spectrum red light lamps. Results Overall, 514 biopsied lesions were considered. Clearance rates after red light PDT with 10 % ALA gel were comparable for KIN I-III (88.2 %, 92.0 % and 87.9 %) and Olsen I-II lesions for any given lamp type. Generally, clearance rates were higher using narrow- compared to broad-spectrum lamps. For both lamp types, the variation in clearance rates from KIN I-III was low. Clearance was lower with vehicle. Limitations Varying lesion numbers in the subgroups and a remaining risk of bias due to the biopsies are potential limitations. Conclusion Our results suggest that red light PDT with 10 % ALA gel is an effective treatment option for AK regardless of the extent of keratinocyte atypia.

Abstract:Photodynamic therapy (PDT) with photosensitization using 5-aminolevulinic acid (ALA) [including a nanoemulsion (BF-200 ALA)] is approved in the USA for the treatment of actinic keratoses (AKs); another derivative, methyl aminolevulinate, is not approved in the USA but is used in Europe. For AK treatment, the photosensitizer may be applied to individual AK lesions or, depending on treatment regimen, to broader areas of sun-damaged skin to manage field cancerization, although not all products are approved for field treatment. ALA-PDT and photosensitizers have also been used off-label for the treatment of nonmelanoma skin cancers, primarily basal cell carcinomas (BCCs) and cutaneous squamous cell carcinomas (cSCC). Advantages of PDT include potentially improved cosmesis and patient satisfaction; disadvantages include pain and duration of treatment. Alternative illumination approaches, including intense pulsed light as well as pulsed-dye lasers, have also been used successfully. Pretreating the affected tissue or warming during incubation can help to increase photosensitizer absorption and improve therapeutic efficacy. Combinations of multiple treatments are also under exploration. Reducing incubation time between photosensitizer application and illumination may significantly reduce pain scores without affecting treatment efficacy. Substituting daylight PDT for a conventional illumination source can also reduce pain without compromising efficacy. The objective of this narrative review is to describe current and ongoing research in the use of topical photosensitizers and modified light delivery regimens to achieve improved therapeutic outcomes with less toxicity in patients with AK, cSCC, BCC, and field cancerization.

Treatment of actinic keratosis: a systematic review

Abstract:Treatment of actinic keratoses (AKs) can help lower the risk of eventual skin cancer and address field pre-cancerization. This review compares the different therapeutic options for actinic keratosis. Databases used include Medline, EMBASE, Web of Science and the Cochrane Library from inception to December 2019. Randomized control trials that were related to any approved or recognized treatment for actinic keratosis were included. 1186 studies were found, of which 80 with 6748 patients were included in the analysis. A network meta-analysis was not possible due to interstudy heterogeneity. The greatest degree of improvement was seen with photodynamic therapy (PDT) used adjunctively with other modalities, but this was not significantly different compared to other treatments. PDT, cryotherapy, imiquimod, ingenol mebutate (IMB), 5-fluorouracil (5-FU), trichloroacetic acid (TCA), and ablative fractional laser (AFXL), were all non-inferior to one another in terms of percent clearance of AKs, but the lowest rates of clearance were seen with diclofenac sodium. When results were substratified by body site, 5-FU, combination PDT and combination 5-FU with calcipotriol were the most beneficial for AKs on the head and neck, although they often caused the highest proportion of initial side effects. Absence of randomized control trials for surgical treatments and non-ablative laser limits comparison of these treatments to other modalities. Limitations include the lack of standardized outcome reporting limited the comparability of results across trials. The results of this analysis do not account for individual patient risk or cumulative risk for development of skin cancer. At present, PDT, cryotherapy, imiquimod, IMB, 5-FU, TCA, AFXL, and combination treatments are similarly efficacious in reducing AKs in immunocompetent patients.Registration: N/A.

Abstract:Background Photodynamic therapy (PDT) with 5-aminolevulinic acid (ALA) is a reliable treatment for actinic keratosis (AK), but its effect needs to be enhanced in thick lesions. Plum-blossom needle is a traditional Chinese cost-effective instrument for enhancing the transdermal delivery of ALA. However, whether it could improve the efficacy of AK treatment has not yet been investigated. Objective To compare the efficacy and safety of plum-blossom needle–assisted PDT in facial AK in the Chinese population. Methods In this multicenter, prospective study, a total of 142 patients with AKs (grades I–III) were randomized into the plum-blossom needle–assisted PDT group (P-PDT) and control PDT group (C-PDT). In the P-PDT group, each AK lesion was tapped vertically by a plum-blossom needle before the application of 10% ALA cream. In the C-PDT group, each lesion was only wiped with regular saline before ALA cream incubation. Then, 3 hours later, all the lesions were irradiated with light-emitting diode (LED) at a wavelength of 630 nm. PDT was performed once every 2 weeks until all lesion patients achieved complete remission or completed six sessions. The efficacy (lesion response) and safety (pain scale and adverse events) in both groups were evaluated before each treatment and at every follow-up visit at 3-month intervals until 12 months. Results In the P-PDT and C-PDT groups, the clearance rates for all AK lesions after the first treatment were 57.9% and 48.0%, respectively ( P < 0.05). For grade I AK lesions, the clearance rates were 56.5% and 50.4%, respectively ( P = 0.34). For grade II AK lesions, the clearance rates were 58.0% and 48.9%, respectively ( P = 0.1). For grade III AK lesions, the clearance rates were 59.0% and 44.2%, respectively ( P < 0.05). Moreover, grade III AK lesions in the P-PDT group required fewer treatment sessions ( P < 0.05). There was no significant difference in the pain score between the two groups ( P = 0.752). Conclusion Plum-blossom needle tapping may enhance the efficacy of ALA-PDT by facilitating ALA delivery in the treatment of AK.

Abstract:Photodynamic therapy (PDT) is safe and effective in the treatment of patients with actinic keratosis (AK). The aim of the study was to assess the efficacy, tolerability and cosmetic outcome of topical PDT in the treatment of AKs with three forms of photosensitizers: 5-Aminolevulinic acid hydrochloride (ALA-HCl), 5-Aminolevulinate methyl ester hydrochloride (MAL-HCl) and 5-Aminolevulinate phosphate (ALA-P). The formulations were applied onto selected scalp/face areas. Fluorescence was assessed with a FotoFinder Dermoscope 800 attachment. Skin areas were irradiated with Red Beam Pro+, Model APRO (MedLight GmbH, Herford, Germany). Applied treatments were assessed during the PDT as well as 7 days and 12 weeks after its completion. Ninety-four percent of patients rated obtained cosmetic effect excellent. The efficacy of applied PSs did not differ significantly. However, pain intensity during the PDT procedure was significantly lower in the area treated with ALA-P (5.8 on average) in comparison to the areas treated with ALA-HCl or MAL-HCl (7.0 on average on 0–10 scale). Obtained results show that ALA-P may undergo more selective accumulation than ALA-HCl and MAL-HCl. Our promising results suggest that PDT with the use of ALA-P in AK treatment may be an advantageous alternative to the already used ALA-HCl and MAL-HCl.

Abstract:Objectives: The study examines the changes in dermoscopic features of actinic keratosis (AK) after photodynamic therapy, and delineates the association between AK dermoscopic and histopathological findings. Methods: A total of 21 patients (23 lesions) with pathologically confirmed actinic keratosis who received 5-aminolaevulinic acid (ALA) photodynamic therapy (ALA-PDT) were enrolled. The numbers of PDT treatments were: 1, n = 1; 2, n = 2; 3, n = 10; 4, n = 6; 5, n = 2; 6, n = 2). The dermoscopic features before and after the PDT were compared. Results: There were statistically significant decreases in the positive rates of dermoscopic features including scales (P < 0.001), follicular plugs with whitish halo (P = 0.013), and red pseudonetwork (P = 0.022) among patients treated with ALA-PDT. Dermoscopic feature was significantly associated with pathological grade (P < 0.001). Histopathological hyperkeratosis was significantly associated with dermoscopic red pseudonetwork (P = 0.034) and wavy vessel (P = 0.005). Parakeratosis was associated with wavy vessels (P = 0.001). For vascular hyperplasia in dermal papillae, the significant correlates included scales (P = 0.011), follicular plugs with whitish halo (P = 0.011), red pseudonetwork (P < 0.001); coiled vessels (P = 0.003) and rosette sign (P = 0.004). Wavy vessels was the only feature correlating keratosis pilaris (P = 0.003). Conclusions: The findings of the present study support dermoscopy as having potential to be useful for diagnosing and monitoring of actinic keratosis.
Effective treatment of actinic keratosis on the hands with red light photodynamic therapy using BF-200 ALA

Abstract:Background Acral actinic keratosis (AK) lesions are considered difficult to treat, and published data for photodynamic therapy (PDT) on these lesions is limited. Thus, we evaluated sustained efficacy, safety, and satisfaction after PDT for AK on the hands. Methods We analysed subgroup data for treatment on the hands from a randomised, double-blind, intra-individual phase III study. All participants previously underwent up to two field-directed red light PDTs with 10% 5-aminolevulinic acid nanoemulsion gel (BF-200 ALA). Assessments included pain during PDT, clearance and recurrence rates, and satisfaction. Results 24 participants treated on the hands were included; 21 participants were analysed. Complete clearance rates with BF-200 ALA were 90.9% (lesion-based) and 76.2% (per participant's side), both markedly higher than with vehicle. The lesion recurrence rate with BF-200 ALA was 29.0%. Adverse events reflected the mode of action. Mean pain intensities were 4.8 ± 3.8 (BF-200 ALA) and 0.8 ± 2.1 (vehicle) on an 11-point numeric rating scale. Most participants (81.0%) rated their satisfaction with BF-200 ALA as very good or good. Conclusion This subgroup analysis indicates that PDT with BF-200 ALA provides a suitable treatment for AK lesions on the hands.

Abstract:Abstract Actinic keratoses (AKs) are considered precursor lesions of cutaneous squamous cell carcinoma, with a reported conversion rate of 3–16%. Photodynamic therapy (PDT) is a widely approved therapy for AKs. Conventional PDT (c-PDT) using red light is limited by pain and a small treatment field. Daylight PDT (d-PDT), an approved alternative, is limited by geographical location and weather conditions. Simulated daylight PDT (sd-PDT) with light-emitting diode (LED) light sources is now commercially available due to robust evidence that these devices are effective, but the devices are limited by cost (∼€30 000). We hypothesized that sd-PDT with a 4000 K white-light LED floodlight, costing €961, is equally as effective and well tolerated as d-PDT in the treatment of AKs across treatment fields of up to 55 cm2 on the scalp. Twenty-two men with a mean age of 74.5 years (SD 7.0) and multiple scalp AKs were enrolled in this prospective, randomized, single-blinded study, which employed a split-scalp design to compare the effectiveness and adverse events of d-PDT and sd-PDT. Block randomization was used to determine which side of the scalp would be treated with d-PDT. Each patient received four treatments in total, two d-PDT and two sd-PDT, each a week apart. The AKs were counted, graded, mapped and photographed at baseline, at blinded 1- and 3-month reviews and at unblinded 6-, 9- and 12-month reviews. The primary endpoint was mean lesion-specific clearance, defined as the mean number and percentage of cleared AKs per treatment field at 3 months. At baseline, the mean (SD) number of AKs per field was 44 (17.4) for the d-PDT group and 45.5 (17.8) for the sd-PDT group (P = 0.4). The mean lesion-specific clearance (percentage reduction) in AKs per field was 35.3 (71.5%) for d-PDT and 35.0 (75.1%) for sd-PDT at 1 month (P > 0.99), 33.6 (66.4%) for d-PDT and 30.3 (61.6%) for sd-PDT at 3 months (P = 0.6). Both d-PDT and sd-PDT were effective, with a significant reduction in AKs from baseline at 1 and 3 months (P < 0.01). Pain was reported by 15 patients with d-PDT and 12 with sd-PDT (median maximum score out of 100, 3.5 vs. 4.5; P = 0.6). Erythema was observed in 17 after d-PDT and 15 after sd-PDT. On a scale of 0 (intolerable) to 10 (very tolerable), patients rated d-PDT as 10 and sd-PDT as 9.8 (P = 0.08). We conclude that sd-PDT with a 4000 K white-light LED floodlight is equally as effective and well tolerated as d-PDT and is an affordable treatment option not limited by pain, small treatment field, geographical location or weather conditions.

Abstract:BACKGROUND: Daylight photodynamic therapy (DL-PDT) has become one of the most effective treatments for the resolution of actinic keratosis (AK) of Olsen grade 1 and 2. Generally, PDT it is carried out in a clinic setting, which involves the patient's and their caregivers commuting to the hospital as well as a significant use of resources to carry it out within the clinic setting.OBJECTIVES: To determine the efficacy and safety of a home-based treatment of AK with DL-PDT with the BF-200 ALA gel compared to a clinic-based setting.METHODS: The study was performed as a randomized, single-center, non-inferiority clinical trial with two parallel groups. 9 patients received one clinic-based DL-PDT (group 1) and 11 patients received one session of home-based DL-PDT (group 2). The primary endpoints were the mean AK clearance per patient and the total AK lesion clearance rate 12 weeks after treatment. The secondary endpoints were the number of remaining AKs and new AKs appearing in the treatment field 12 weeks after one PDT session. The pain during and 24 h after PDT as well as the local skin reactions were also assessed.RESULTS: The overall reduction of AK lesions per patient was similar in both groups with one PDT session. An overall AK clearance per patient of 10 ± 4.33 for group 1 versus 9.73 ± 2.9 for group 2 without statistically significant differences (p = 0.868). Regarding the clearance rate, although it was slightly higher in group 2 (71.58 ± 22.51 vs 82.1 ± 11.13), the analysis did not show statistically significant differences. The mild pain recorded during the treatment course and the mild local skin reactions were similar in both groups. Patient satisfaction was high for both groups without statistically significant differences.CONCLUSION: Self-performed home-based DL-PDT with BF-200 ALA gel is as effective as the one performed in a clinic-based setting, with a comparable safety profile, high levels of patient satisfaction and with advantages for the patients and their caregivers that can enhance patient´s adherence to the treatment.

Abstract:Actinic keratosis (AK), due to its widespread prevalence, as well as the possibility of progression to an invasive form of squamous cell carcinoma, requires treatment regardless of the clinical stage. New imaging techniques, such as in vivo reflectance confocal microscopy (RCM), significantly increase the accuracy of diagnosis and allow noninvasive evaluation of the therapeutic efficacy of the ongoing treatment. Our objective was to evaluate the prevalence of specific (video)dermoscopy and RCM features of pigmented and classical subtypes of AK before and after photodynamic therapy (PDT) treatment. We included patients with facial grade II AKs (25 pigmented, 275 non-pigmented) were included in the study. Skin lesions were evaluated by (video)dermoscopy and RCM at the baseline and three months after PDT. In classic AK, the most frequent dermoscopic findings were fine wavy vessels (96%), scale (92%), microerosions (48%), and "strawberry" pattern (36%), while pigmented AK was characterized mostly by "rhomboidal pattern" (80%), scale (60%), white globules (48%), "jelly sign", and superficial pigmentation (40%). RCM's most characteristic classic AK findings were abnormal honeycomb pattern in the spinous layer, epidermal inflammatory infiltrate, and solar elastosis that were present in 96% of lesions. Pigmented AKs presented mostly with dark central areas of parakeratosis (72%), mottled pigmentation (72%), dermal inflammatory infiltrate (64%), solar elastosis (60%), and abnormal honeycomb pattern in the spinous layer (56%). Dermoscopically, PDT resulted in complete disappearance of the "rhomboidal pattern" in both classical and pigmented AKs, "starburst pattern" and "jelly sign" in classical AKs, and inner gray halo, "rosette sign" and central crust in pigmented AKs. Three months after one PDT session, RCM evaluation showed mostly solar elastosis in both classical and pigmented AK subtypes, epidermal inflammatory infiltrate in classical AKs, and dermal inflammatory infiltrate in pigmented AKs. New noninvasive imaging techniques such as RCM and (video)dermoscopy can help practitioners better visualize the efficacy of the ongoing PDT treatment in either classical or pigmented AK subtypes.

Abstract:Objective To observe the clinical effect and light aging resistant of 5-aminolevulinic acid photodynamic therapy in the treatment of actinic keratosis.Methods 20 cases of actinic keratosis patients in our hospital from 2015 january to 2015 november were as the research object,Aminolevulinic acid 20% solution was prepared,and applied to skin lesions, After 4-6 hours.The lesions were irradiated with red light at an energy density of 80 to 100 J/cm2 for 20 minutes.Totally,3 to 5 sessions of photodynamic therapy were given at a frequency of once a week.Results Follow-up 6 months,there were 18 patients with complete response(90.0%),two patients partial response(10.0%), no failure and recurrence case;the average pigment score was 4.1±1.25 before treatment and 2.8±1.15 after treatment,with an average decrease of 1.3±0.66, the change was statistically signiifcant (P<0.01); the average wrinkle score was 5.8±0.95 before treatment and 4.9 ± 1.02 after treatment, with an average decrease of 0.9±0.55,the change was statistically signiifcant (P<0.01). 95% of patients were satisifed (7/20) and very satisifed (12/20) with the treatment, the average satisfaction score is 3.55 points.Conclusion To be 5-aminolevulinic acid photodynamic therapy for actinic keratosis patients for treatment,efifcacy,adverse reactions are mild,light aging resistance effect,patient satisfaction is high,is worth promoting in clinic.

Abstract:Silicon carbide (SiC) semiconductor devices have received extensive attention with the better performance of the wide band gap material. It is necessary to compare with their silicon (Si) counterparts due to SiC semiconductor devices are new. In this paper, a test platform based on buck converter is constructed to test the switching characteristics of SiC MOSFET, Si CoolMOS and IGBT, the input voltage of which is 400V, the output current of which is 4-10A. Switching waveforms, switching times, switching energy losses, dv/dt, di/dt and reverse recovery characteristic of internal diodes of three devices are presented. Finally, theoretical efficiencies and tested efficiencies of a 2kW dual active bridge (DAB) converter are compared.

Abstract:Background Photodynamic therapy (PDT) is increasingly used for the treatment of basal cell carcinoma (BCC) and squamous cell carcinoma (SCC). However, it is unknown whether photodynamic therapy is more effective than other commonly used treatment modalities for these cancers. Purpose The aim of this study was to determine the relative efficacy and safety of PDT compared with placebo or other interventions for the treatment of skin carcinomas. Methods Searches were performed in PubMed, Web of Science, Embase, and the Cochrane Central Register of Controlled Trials databases. We included randomized controlled trials comparing the PDT with other interventions in adults skin BCC or SCC that reported on lesion response, recurrence, cosmetic appearance, or safety outcomes. Results Seventeen unique randomized controlled trials, representing 22 study arms from 21 publications were included. The included trials included 2,166 participants, comparing methyl aminolevulinic (MAL) PDT (six studies) or aminolevulinic acid (ALA) PDT (two studies). Comparators included placebo, surgery, hexaminolevulinic (HAL) PDT, erbium: yttrium-aluminum-garnet ablative factional laser (YAG-AFL) PDT, fluorouracil, and imiquimod. There were few studies available for each comparison. Mantel-Haenszel fixed effects risk ratios were calculated for response, recurrence, cosmetic outcomes, and adverse events. MAL-PDT had similar response rates to surgery, ALA-PDT, fluorouracil and imiquimod at 3- and 12 months post-intervention. The rate of recurrence was similar, showing few differences at 12 months, but at later time points (24–60 months), fewer lesions recurred with surgery and imiquimod than with PDT. PDT also caused more adverse events and pain than other interventions. However, PDT treatment was more likely to receive a “good” or “excellent” rating for cosmetic appearance than surgery or cryotherapy. Conclusion This systematic review and meta-analysis demonstrates that the choice of treatment modality for BCC or SCC is best chosen in the context of the location and size of the lesion, the socioeconomic circumstances of the patient, as well as the patient’s preferences. We call for more high quality studies to be done, in order to enable more reliable interpretations of the data. Abstract:Photodynamic therapy (PDT) using 5-aminolevulinic acid (5-ALA) is an emerging treatment option in the care of actinic keratosis (AK). A self-adhesive 5-ALA patch was recently developed that allows a precise PDT procedure. Here, we review the current literature and report the findings of our case series that observed the outcomes and safety of 5-ALA patch PDT. Ten patients with a total of 40 AKs were treated with a single session of conventional or daylight PDT using 5-ALA patch at the Department of Dermatology and Venereology, Sapienza University of Rome or at the European Institute of Oncology, Milan, Italy. Complete response was observed in three patients, while partial response was seen in seven patients. Overall tolerability was good or excellent, with local adverse events observed in four patients. This is the first case series reported where the 5-ALA patch was applied using daylight PDT, and its efficacy and tolerability in the treatment of AK were demonstrated. In conclusion, the self-adhesive 5-ALA patch is a convenient application of PDT that provides a well-tolerated and effective treatment option with satisfactory cosmetic outcomes.

Abstract:A 72-year-old woman suffering from multiple actinic keratosis (AK) complicating steroid-induced rosacea received 5-aminolevulinic acid-photodynamic therapy (ALA-PDT) in our outpatient clinic. Both AKs and rosacea achieved remission after one session of PDT. However, an adverse effect of severe acute inflammatory response emerged with lasting hyperpigmentation. We then terminated the following PDT sessions . After skin care and close follow-up for a half year, most symptoms and lesions of AK and rosacea disappeared with mild hyperpigmentation left. ALA-PDT is commonly recommended for multiple AKs based on effectiveness and noninvasiveness, but has controversial efficacy and safety for rosacea. The unusual excessive inflammation in this patient after ALA-PDT may due to skin barrier destruction, vasomotor dysfunction and the immune response by dead Demodex after PDT. This case indicated that carefully evaluation before ALA-PDT is of great importance, especially for those patients with complicated skin situation. For AKs complicating rosacea, modified parameters of ALA-PDT such as less ALA incubation time or reduced light dose should be further studied to achieve the optimal efficacy and safety of ALA-PDT and offer the best benefit.
