Abstract:BACKGROUND: Current surgical treatment modalities for breast cancer include breast conserving surgery, mastectomy alone and mastectomy with breast reconstruction. There are recognized benefits of breast conservation and breast reconstruction over mastectomy but there are few studies assessing this area in Australia. The aim of the present study was to compare the various surgical strategies for breast cancer treatment in terms of quality of life, cosmesis and patient satisfaction.METHODS: A chart analysis was conducted of all patients who underwent Breast Cancer Reconstruction at the Royal Adelaide Hospital Breast Unit between 1990 and 2002. Patients were then traced and asked to take part in an interview. Mastectomy and breast conservation patients who attended outpatient clinic for follow up were also approached. All three groups were interviewed and self-assessment quality of life questionnaires (Functional Assessment of Cancer Therapy-Breast, body image) were administered. The breast conservation and reconstruction groups also underwent assessment of satisfaction and cosmesis.RESULTS: A total of 78 mastectomy, 109 breast conservation and 123 breast reconstruction patients were interviewed. Quality of life assessment was similar between the three groups but the breast conservation and reconstruction patients' body image scores were superior to the mastectomy group. Patient satisfaction was higher in the reconstruction group than the breast conservation group of patients, while cosmesis was similar.CONCLUSION: While little difference was seen on quality of life assessment, body image is improved with the use of breast conservation and reconstruction. The high satisfaction and cosmesis scores in the breast reconstruction group are an indication of the superior results that can be achieved with breast reconstruction.

Abstract:Background: Sexist media tropes objectify the female body, and nonconformists to the ideal beauty standards are isolated as dissenters. Mastectomy causes major changes to a woman’s body, and due to the widespread consumption of body-objectifying media, women undergoing mastectomy without reconstruction are vulnerable to body-dissatisfaction, which may lead to mental health issues. Objectives: This study aims to explore the influence of relationship authenticity, social support from spouse, family and friends in reducing body consciousness and enhancing self-esteem for improved psycho-oncological prognosis in mastectomy patients. Method: Participants (N=30) recruited to the study, were involved in a heterosexual marital relationship, with a monthly per head income of roughly 2000 INR and had undergone mastectomy without reconstruction, in admitted observation or out-patient follow-up observation, ranging from a period of 6-12 months post-surgery. Result: Statistical analyses revealed significant association in body consciousness, relationship authenticity and self-esteem in mastectomy patients. The study also found lower self-objectification and higher self-esteem among those who were successfully rehabilitated to their family setting in comparison to patients immediately after surgery. Conclusion: Perceived support from family and body consciousness are predictive of self-esteem in the sample studied.

Coping in Post-Mastectomy Breast Cancer Survivors and Need for Intervention: Systematic Review

Abstract:Background: Breast cancer is the most prominent cancer type to affect women. Surgical treatment of invasive breast cancers involves mastectomy. Due to mastectomy, women are subjected to social, emotional, and cultural problems which need to be addressed. Objective: The objective of the study is to understand how women cope with body image–related issues, trauma, anxiety, and depression post-mastectomy. Design: A systematic literature review was conducted for understanding the coping in post-mastectomy patients. The methods for identifying the studies were based on Preferred Reporting Items for Systematic reviews and Meta-analysis (PRISMA) guidelines. Databases: Medline/PubMed, PsycInfo, and Cochrane databases were used for searching relevant articles. A final of 19 studies were analyzed for the work. Methods: Search strings such as “coping strategies and post mastectomy,” “body image coping and post mastectomy” and “anxiety coping and post mastectomy” were used for identification of references from databases. Eligibility criteria were used for finalizing the references. Results: Analysis of the 19 studies has clearly shown that women who undergo mastectomy suffer from anxiety, stress, and trauma. This study has observed that women have problems with their body image post-mastectomy along with bouts of depression. Self-coping has been observed in relatively few studies. Psychological interventions before surgery have been observed to be a better coping strategy. In most of the studies, women opted for breast reconstruction to overcome the trauma associated with mastectomy. Conclusion: Mastectomy has a severe impact on women’s appearance and psychology. Breast reconstruction and acceptance have played an important role in coping among these women. However, breast reconstruction is not accepted by many women due to a multitude of factors. Thus, it is essential to have proper intervention programs in place to ensure women can cope with this situation and can lead healthy lives. Abstract:Breast cancer is one of the most common cancers among women globally as well in India. Breast cancer patients may suffer from psychological morbidity. Psychological problems like depression, anxiety, may affect the quality of life of breast cancer patients. To study the psychiatric morbidity in diagnosed advanced breast cancer patients. A cross sectional study was done in the department of Psychiatry, over a period of one and half years from January 2014 to June 2015 at Osmania Medical College Allied Hospitals (M.N.J Institute of Oncology & Regional Cancer Centre and Osmania General Hospital), Hyderabad, Telangana. Breast cancer patient cases and controls were studied by the General health questionnaire -28 (GHQ-28) Mini International Neuropsychiatric Interview- plus (mini- plus). This difference in the proportion of psychiatric disorders was statistically significant. By using MINI plus, 26.7% of breast cancer patients reported psychiatric illness and this percentage was 10% in controls. This difference was not statistically significant at p value of 0.05 (Chi square value 2.78, p value 0.18). The most common psychiatric morbidities were Dysthymia, and MDD seen in 3 (10%) women each. Adjustment disorder unspecified was found in 2 (6.7%) women. There are statistically significant differences between cases and controls and these differences are circumscribed to domains of recall and language and orientation. The mean total score obtained by cases is 28.60 with a standard deviation of 1.354 and that obtained by the control group is 29.70 with a standard deviation of 0.535. The difference between the two groups is highly significant (p<0.001). The study brings out the need for identifying psychiatric problems at proper time, referral of breast cancer patients to Psychiatrists and points out the need for establishing psychiatric units in Cancer Institutes.

Abstract:Introduction Breast cancer is currently the first cancer diagnosed among women. The impact of its local treatments such as surgery and radiation and its systemic treatments such as chemotherapy, hormone-suppressant therapies and targeted therapies on women's bodies anatomically and physiologically and on their body image and body esteem are the major factors altering their sexual health and the quality of their spousal relationships. Objective We aim to evaluate the qualitative impact of breast cancer on its survivors' body-image. Methods We ran a prospective qualitative study using semi-structured interviews until saturation of findings. The patients included were women diagnosed, treated for breast cancer, and followed up in our institution. All participants were sexually active and no older than 55. The study enrolled 20 participants over a 6-month period (February 2022-July 2022). Results All participants had a history of breast surgery either lumpectomy or mastectomy (70%) with (one case) or without reconstructive surgery. All patients reported a history of radiation and chemotherapy. 60% had received hormone therapy with medically induced menopause. The majority of participants explained how their bodies had changed not only physically and anatomically on the outside but also on the inside especially in relation to the adverse effects of chemotherapy and hormone therapy. They also reported body emotional changes. This required an adaptation to their new body for them and their partners and entourage. The main themes emerging from their discourse were: physical and emotional adaptation to breast cancer per se and its local and systematic treatments, fear of others judgment and comments on their physical changes and loss or gain of self-esteem. Breast reconstruction was also an important theme it the participants verbatim. Conclusions The words and themes that characterized the participants' speeches highlighted the impact of cancer diagnosis and oncological therapies on breast cancer survivors. Being part of a psychological intervention focused on self-compassion and body acceptance helps breast cancer survivors develop a better perception and acceptance of their new bodies. Disclosure No.

Abstract:In parallel with the rapid rise in breast cancer incidence, there is also a noticeable rise in the number of patients who experience persistent negative body image cognition after breast cancer surgery. This study aimed to explore the differences in illness perception, social support, and body image cognition among breast cancer patients with different surgical methods, and the correlation, regression, and mediation among the three variables. The Brief Illness Perception Questionnaire (BIPQ), the Social Support Rating Scale (SSRS) and the Body Image Cognition after Breast Cancer Questionnaire (BIBCQ) were used to evaluate breast cancer patients’ illness perception, social support and body image cognition. Data analyses were performed by descriptive statistics, independent samples t -test, analysis of variance (ANOVA), Pearson correlation, and linear regression. The mediation was explored by the PROCESS V3.3. The study found that breast cancer patients undergoing radical mastectomy (RM) and modified radical mastectomy (MRM) demonstrated more negative illness perception, body image cognition, and lower social support compared with the patients receiving nipple-sparing mastectomy (NSM; p < 0.05). The subscale cognitive representation (CR) of BIPQ was strongly positively correlated with BIBCQ ( p < 0.05). Illness perception positively predicted body image cognition ( p < 0.01), while social support negatively predicted body image cognition. Social support partially mediated illness perception and body image cognition, exhibiting a positive role in post-operative body image cognition.

[Effects of mastectomy on dimensions of psychological and psychosocial experience and behavior of affected women]

Abstract:The present study deals with the issue of how mastectomy will impact selected dimensions of psychological and psychosocial experience and behaviour in the women concerned. The study had comprised 21 women, mean age 55. 73 years, with a breast cancer diagnosis given between 1 and 21 years ago. Data collection was carried out using a 17-item questionnaire standardized for this purpose. Factor analyses produced four clearly interpretable factors. Study findings suggest that breast cancer surgery will not inevitably result in impaired sexual behaviour and experience in the affected women, whereas it is sure to impact, to varying degrees, on the factors and dimensions of "body image and self-image", "psychological and psychosomatic complaints" as well as anticipated "performance capacity and social relationships". Intercorrelations among questionnaire items are set out. It has further been found that the variables "time of breast removal" and "life age" had exercised only negligible influence on the women's questionnaire response, and that the psychosocial support experienced post-surgery was reported to have primarily come from the circle of family and friends, and less so been extended by self-help groups or medial staff. Concluding, the need for integrated rehabilitative measures is pointed out.

Abstract:Background: Breast cancer in India it varies from as low as 5 per 100,000 female populations per year in rural areas to 30 per 100,000 female populations per year in urban areas. Medical treatments of surgery, chemotherapy etc. for cancer usually precipitates with serious adverse effects affecting overall quality of life in physical, psychological and social aspects. Objective of the Study: To evaluate health related quality of life in patients who had been followed up after the breast cancer surgery for chemotherapy at our hospital. Methods: Quality of life was documented using European Organization of Research and Treatment for Cancer Quality of life Questionnaire-Cancer 30 (EORTC QOL-C30) and European Organization of Research and Treatment for Cancer Quality of life Questionnaire-Breast 23 (EORTC QOL-BR23). Results: After analysis of EORTC QOL-C30 questionnaire, participants had maximum affection of cognitive, social and physical functions and reported high levels of pain, fatigue and insomnia. Domains of EORTC QOL-BR23, the such as worry about self body image, sexual dysfunction, upset by hair loss, systemic therapy side effects, breast and arm symptom were reported by maximum participants. Various domains of quality of life were maximally affected in patients of breast cancer surgery undergoing chemotherapy. Health care professionals in the field of Oncology should address the above mentioned domains of quality of life in Indian patients with breast cancer surgery undergoing chemotherapy. Conclusion: Comprehensive cancer management plan should include psychological counseling, emotional support, physiotherapy care for pain and other musculoskeletal problems etc. along with medical treatment of patients with breast cancer.

Immediate Psychological Implications of Risk-Reducing Mastectomies in Women With Increased Risk of Breast Cancer: A Comparative Study

Abstract:Background: Risk-reducing mastectomy is recommended for high-risk patients but may have significant psychological consequences. This study aimed to determine the differences in anxiety, depressive symptomatology, body image and quality of life in women with an increased risk of breast cancer immediately before and after undergoing risk-reducing mastectomy. Methods: Eighty-eight women with an increased risk of breast cancer due to BRCA1/2 mutations or a previous cancer diagnosis participated in this study. Instruments used were the Hospital Anxiety and Depression Scale, Body Image Scale and the European Organization for Research and Treatment of Cancer Quality-of-Life Questionnaire Core 30 and Breast 23, administered 15-30 days before and after surgery. Results: Following surgery, there was an immediate and significant worsening in anxiety, depressive symptomatology and body image. There was a significant deterioration in global, physical, role, and social functioning, as well as in body image and sexual enjoyment scales. Additionally, there were increases in fatigue, nausea and vomiting, constipation, dyspnoea, insomnia, appetite loss, perceived financial difficulties, pain, systemic therapy side effects, and breast and arm symptoms. However, there was an improvement in future perspective. These changes occurred independently of whether participants had a cancer diagnosis or BRCA1/2 mutation. Conclusion: Risk-reducing mastectomies have immediate psychological consequences. While these procedures improve future health perspective, they increase anxiety and depressive symptomatology and decrease body image and quality of life, regardless of cancer diagnosis or BRCA1/2 mutation. These findings highlight the psychological consequences of such surgical procedures, emphasizing the need for comprehensive psychological interventions both before and after surgery.

Abstract:Breast cancer (BC) is a highly prevalent malignant tumor in women, with a high incidence in China, and the annual incidence shows a rising trend, posing a serious threat to life and health in female. [ 1 ] The diagnosis of BC can trigger great impact on the psychology of female patients, and even some patients may suffer from serious depression due to the disease. [ 2 ] It has been reported that young BC patients have a lot of uncertainty about the disease in the early stage of diagnosis, such as the progression of the disease, treatment and prognosis, and cannot evaluate the value of the event, let alone predict the outcome of the disease. [ 3 ] The uncertainty of the disease can lead to reduced coping ability of patients, resulting in depression, anxiety and other negative mentality. [ 4 ] As BC is located in the breast, the body shape of patients alters greatly after surgical treatment, leading to a lack of body confidence of patients. [ 5 ] Therefore, the primary problem to solve the psychological disorders of patients is to help them understand and accept their own disease, and can successfully face the disease, and enhance the courage of patients to fight against the disease. Psychosocial effect of mastectomy versus conservative surgery in patients with early breast cancer

Abstract:Purpose: To compare the quality-of-life (QoL) and psychosocial changes in a group of patients with early breast cancer who underwent conservative surgery (BCS) or modified radical mastectomy(MRM). Methods: Self-administered questionnaire assessing body image perception, social habits, sexual attraction and self-consciousness with relatives/friends, was randomly assigned to 125 patients (61 BCS, 64 MRM; aged 53 +/- 8 and 50 + 9 years, respectively, p = NS). Results: MRM patients reported a significantly higher frequency of changes in body image perception and other related social behaviour such as avoiding going to the beach or using low-cut clothes, and reticence with friends. Conversely, no differences were found regarding sexuality, denial of the disease by the husband/partner, or concealing the disease from family members. Also, no significant differences were found between patients above and below the age of 50 years, for all variables studied after adjustment for surgical procedure. Conclusions: Modified radical mastectomy has a negative effect on body image perception and in social behaviour patterns of patients and with a concomitant decrease in QoL. The sexuality of the patient is not significantly affected.

Abstract:Background: Despite robust evidence confirming equivalent survival rates and better cosmetic outcomes with breast-conserving surgery (BCS) and radiotherapy compared to mastectomy, the rates of mastectomy among women with early breast cancer have not declined significantly in Sri Lanka. This study explores views on the surgical treatment of breast cancer among Northern Sri Lankan women who were eligible for BCS but underwent mastectomy. Methods: An exploratory descriptive qualitative study was carried out among 15 women who underwent mastectomy for early breast cancer. Patients who were referred to the Tellippalai Trail Cancer Hospital for adjuvant therapy after mastectomy and matched the study criteria were recruited. Data were collected through in-depth semi-structured interviews, which were transcribed in Tamil, translated into English, coded using QDA Miner Lite software, and analysed thematically. Results: Nine out of 15 participants were either not aware of breast-conserving surgery (BCS) as a treatment option or their eligibility for BCS at the time of mastectomy. According to participant narratives, the treating team had recommended mastectomy to most participants. While many opted for mastectomy believing that it was associated with lower rates of recurrence and spread compared to BCS, these beliefs were frequently reinforced by the treating team. The pros and cons of the two approaches had not been discussed before surgery with most participants. In the absence of information, family and friends weighed in on the decision to opt for mastectomy, ultimately resulting in feelings of loss and regret in most instances. Conclusion: Most participants were not aware that they were eligible for BCS. These information gaps need to be urgently addressed for women to make informed decisions about their health.

Abstract:A BSTRACT Background: Breast cancer patients report difficulties with concentration, multi-tasking, and memory. Cognitive dysfunction can impact the quality of life by affecting activities of daily living, treatment compliance, interpersonal relationships, and profession. Aims: The objective of this study was to evaluate and compare the cognitive functions and psychological complications in breast cancer patients with a control population. Settings and Design: This study was conducted in the outpatient department of Bharath Hospital and Institute of Oncology, Mysore, where the cases were recruited. The study participants were 110 female breast cancer patients and 100 noncancer healthy females as controls. Materials and Methods: Demographic details of the participants were collected through a questionnaire. Clinical data were obtained from clinical records. Tools used were Montreal Cognitive Assessment (MoCA), Hamilton Depression Rating Scale (HAM-D), and Hamilton Anxiety Rating Scale (HAM-A) to evaluate cognitive functions, depression, and anxiety, respectively. Statistical Analysis: The various findings were analyzed using mean, frequency, Pearson’s correlation, and two-sample t -test. Results: Mild cognitive impairment was observed in 88% of the cancer patients, and more than 95% were under severe anxiety and depression. There is a highly significant difference in all three tests (MoCA Test, HAM-D test, HAM-A test) with a P < 0.001. Conclusion: Breast cancer patients show statistically significant cognitive deficits as compared to noncancer individuals.

Abstract:Background: As breast cancer incidence rises among younger women, there is a knowledge gap regarding the emotional, physical, and social effects of mastectomy, specifically in a crisis-affected country such as Syria. This study aimed to explore these effects on young women with breast cancer in Syria, taking into consideration the cultural significance of a woman's breast as part of her feminine identity. Methods: A qualitative design, using semi-structured in-depth interviews with 10 young women with breast cancer who underwent mastectomy, was conducted between June to December 2022. Results: Thematic analysis was used to analyze the data, and five main themes were identified: (1) psychological and emotional well-being (altered self-esteem and femininity, impact on sexual life and relationships, psychological distress associated with mastectomy, mirror trauma and the need for psychological care); (2) body image and breast reconstruction (the dilemma over reconstruction decision, body image and clothing and lack of access to prosthetic information/services); (3) social and interpersonal factors (lack of marriage choices and society's view and stigma); (4) coping mechanisms with mastectomy effects (family support; faith in god almighty; comparing their situation to others and use of prosthetics) and (5) physical health and functioning (physical effects on mobility and function). Conclusion: Mastectomy has significant physical, emotional, and social consequences on young women with breast cancer, particularly in crisis-affected Syria where access to breast reconstruction is limited. It is crucial for healthcare professionals to understand these impacts, to raise awareness, encourage early detection, and promote less aggressive treatments to improve women's quality of life.

Abstract:Abstract PurposeTo explore the dynamic changes in Quality of Life (QoL), anxiety/depression status, and body image (BI) of women who received different types of breast cancer (BC) surgery within an 8-years follow-up period.MethodsWomen with major BC surgeries were invited to complete the World Health Organization Quality of Life–Brief (WHOQOL-BREF), the European quality of life five dimensions questionnaire (EQ-5D), and a body image scale within 8 years of surgery. Kernel smoothing methods were applied to describe dynamic changes in QoL, anxiety/depression, and BI at different time points. Linear mixed effects models were constructed to identify the interaction between time, different types of surgery, and the determinants of QoL in these patients.ResultsA total of 1,803 women who had undergone a mastectomy, a modified radical mastectomy (MRM), and breast reconstruction (BR) were included. The BR group exhibited a high QoL score of WHOQOL one to five years after surgery with some fluctuations. The MRM group had comparatively stable, low QoL scores of WHOQOL items and less depressed/anxious. BR group generally showed fluctuated, lower scores of BI two years after operation, but they exhibited more anxiety/depression status after five years. Medical comorbidities, the status of anxiety/depression, and BI were the major factors influencing all domains and items of the WHOQOL BREF.ConclusionWhile MRM may decrease the likelihood of depression in patients with BC and BR would significantly improve their QoL in the first 5 years. We recommend that these findings should be considered and discussed in the patient participatory decision-making for breast surgery.

Abstract:Aggressive individuals are at increased risk of poor health, early mortality, mental health problems, and decreased life satisfaction. However, recent research suggests that trait aggression is not a unitary phenomenon. To date, most personality research on the topic has focused on direct aggression. Individuals high in trait direct aggression tend to respond to provocation with immediate anger and retaliation. By contrast, those high in trait displaced aggression tend to respond to provocations with initial restraint, yet eventually “take out” their anger on innocent individuals. Trait displaced aggression is a multifaceted personality dimension consisting of angry rumination, revenge planning, and aggression against undeserving targets. The present research was the first to investigate the effects of trait displaced aggression on physical and mental health-related variables. Specifically, more than 800 Internet respondents completed measures of direct and displaced aggression, physical health, life satisfaction, stress, and risky behaviour. Trait displaced aggression predicted negative physical and mental health outcomes as well as decreased life satisfaction. Structural equation modeling tested a process model whereby these effects occur. Results showed that perceived stress and avoidant provocation-related coping strategies mediated the relationship between trait displaced aggression and self-reported physical health symptoms while perceived stress mediated the effect of trait displaced aggression on life satisfaction.

Abstract:Psychological factors and quality of life influence the breast cancer patient (Khan et al., 2010). The aim of this retrospective study is to evaluate the breast cancer patients’ self-esteem, perceived social support, and quality of life. Sixty patients were taken in this, whose age was between twenty to sixty years. A self-esteem scale, social support questionnaire, and EORTC quality of life questionnaires were used to assess breast cancer patients. All statistical analyzes were completed via SPSS 16.0. The majority of the patients (93%) had high self-esteem and social support (90%). There was a significant association was found between self-esteem and social support (.001). Global health status and functional scores were high. Patients had increased complaints of fatigue, systemic therapy side-effect, and arm symptoms. Participants had suffered increased financial difficulties. It was observed that the sexual activity and sexual enjoyment of the participants had decreased. Breast cancer patients’ self-esteem and social support were high. The overall quality of life in breast cancer patients appeared to be good. The participants had felt more tired, difficulty lifting their arms. Therefore, Higher Self- esteem and social support were strongly associated with a good overall quality of life.
