2082556 Characteristics of Ultrasonographic Breast Images of Ductal Carcinoma in Situ (DCIS) with Abnormalities of the Ducts/BC-02 Study Report

Abstract:ObjectivesThe Japan Association of Breast and Thyroid Sonology (JABTS) classifies ultrasonographic (US) breast images into those with masses and non-mass abnormalities, and non-mass abnormalities are classified into 5 subtypes. The abnormalities of the ducts is one of them. We investigated the rate of DCIS with abnormalities of the ducts and characteristics of US images in a retrospective, multicenter, observational study (JABTS BC-02 study).MethodsDCIS cases were collected from 15 institutions in Japan. The patients were examined between Jan. 2008 and Dec. 2012. The images were judged following the guidelines by JABTS at the institutions and then re-judged by experts of the central image judgment committee.ResultsThe abnormalities of the ducts were noted in 78 (11%) of 705 US images of DCIS. The distribution of duct abnormalities was focal or segmental in all cases. Duct dilatation was noted in 50 (64.1%), a duct with internal echoes in 70 (89.7%), duct wall thickening in 1, and irregularity of the duct caliber in 5 (overlapping included). The duct with internal echoes was classified as solid in 66, echogenic foci in 2, and solid+echogenic foci in 2. The shape of the solid part was sharply protruding in 1, broad based or irregular in 38, and difficult to judge in 29. The solid part contained echogenic foci in 31, no echogenic focus in 36, and was difficult to judge in 1. The B-mode category based on BI-RADS was C3 in 8, C4 in 68, and C5 in 2. Color Doppler and elastography were performed in 70 and 11 cases, respectively. Combining these, the category was changed in 3, and the category rose in all these cases: C3→4 in 2 and C4→5 in 1.ConclusionsDCIS with abnormalities of the ducts accounted for 11% of all cases. The B-mode category was C4 or higher in 90%, but it rose to 92.3% with elastography and color Doppler . In this study, the distribution is focal or segmental in all cases, a duct with internal echoes is noted in 90% and solid echo is detected in 97.1% of these. Of the cases with a solid echo, 89.7% were continuous/multiple. The shape of the solid part was broad-based or irregular in many cases .DCIS should be considered when these findings are observed.
