Abstract:Objective: To study the histopathological staging of atrophic lesions of the gastric mucosa. Methods: Histology and immunohistochemistry were used to closely examine 2144 specimens of atrophic gastric mucosa that were taken from endoscopic biopsies. Results: When the gastric mucosa epithelium is affected by infection, chemical stimulation, immune factors, genetic factors, and other factors, it may cause an atrophy of gastric mucosa epithelium and a decrease in the number of glands, intestinal metaplasia, hyperplasia of smooth muscle fibers, and atrophy of stem cells in the proliferative zone. In this study, we characterized the above lesions as atrophic lesions of the gastric mucosa. Based on the morphological and histological characteristics of the lesion, as well as the law of cell proliferation and transformation during its occurrence and development, we propose five stages. We also noted the onset age, gender correlation, and histopathological characteristics of each stage of gastric mucosal atrophies. Conclusion: Understanding the pathological staging of gastric mucosal atrophy is essential for treating patients correctly and keeping track of changes in malignant cells. It is also very important in preventing the initiation of gastric cancer or from getting worse.

Abstract:Chronic gastritis is very common in Chinese population,primarily as a result of widespread infection with Helicobacter pylori,leading to intestinal metaplasia,atrophy,dysplasia and malignancy if not treated effectively.A growing body of evidence derived from recent clinicopathologic research suggests that incomplete intestinal metaplasia and multifocal atrophy are associated with a greater potential for malignant progression.These premalignant lesions usually start at the incisure-antrum,and spread,mainly along the lesser curvature,to the body,cardia and fundus,together with parietal cell atrophy,decreased serum level of pepsinogenⅠ,and increased level of gastrin-17.As the loss of functional gastric glands intensifies and expands,dysplasia in epithelial cells emerges,which may progress to malignancy.Effective treatment of chronic gastritis to prevent development or alleviate progression of intestinal metaplasia and atrophy holds the key to defend against malignant progression.Therefore,it is essential that meticulous examination of gastric mucosa at gastroscopy and comprehensive histopathologic evaluation of biopsy tissues should be carried out in every patient for an accurate diagnosis and staging of the lesion to be served as the foundation for effective therapy of chronic gastritis and prevention of gastric malignancy.
Morphological and Pathologic Changes of Experimental Chronic Atrophic Gastritis (CAG) and the Regulating Mechanism of Protein Expression in Rats

Abstract:Objective: To study the pathologic change and molecular regulation in cell proliferation and apoptosis of gastric mucosa in rats with chronic atrophic gastritis (CAG), and evaluate the possible mechanisms. Methods: Rats were administered with 60% alcohol or 2% salicylate sodium, 20 mmol/L deoxycholate sodium and 0.1% ammonia water to establish chronic atrophic gastritis (CAG) models. The gastric specimens were prepared for microscopic view with hematoxylin and eosin (H-E) and alcian blue (A-B) stain. The number of infiltrated inflammatory cells, the thickness of the mucosa gland layer (μm) and the number of gastric glands were calculated. The damage of barrier in mucosa with erosion or ulceration, and the thickness of mucin were examined by scanned electron microscope (SEM). The levels of PGE2, EGF (epiderminal growth factor) and gastrin in the serum were measured with radioimmunoassay or ELISA method. The immunohistochemistry method was used to observe the number of G cells, the expression of protein of EGFR (EGF receptor), C-erbB-2, p53, p16 and bel-2 in gastric tissue. Results: Under SEM observation, the gastric mucosa was diffused erosion or ulceration and the thickness of mucin was decreased. Compared with normal rats, the grade of inflammatory cell infiltration in CAG rats was elevated, whereas the thickness and number of gastric gland were significantly lower (P<0.05). Compared with normal level of (0.61±0.28) μg/L, EGF in CAG (2.24±0.83) μg/L was significantly higher (P<0.05). The levels of PGE2 and gastrin in serum were significantly lower in CAG rats than that in normal rats (P<0.05). Immunohistochemistry detection showed that the number of G cell in antrum was lower in CAG group (P<0.05). Immuno-stain showed EGFR protein expression in the basal and bilateral membrane, and the cytoplasma in atrophic gastric gland, while negative expression was observed in normal gastric epithelial cells. Positive staining of p53 and p16 protein was localized in the nucleus of epithelial cells. The former was higher positively expressed in atrophic gland, while the later was higher positively stained in normal gastric tissue. bel-2 protein was positively stained in the cytoplasma in atrophic gastric gland, while very weakly stained in normal gastric tissue. Conclusion: The pathological findings in gastric gland accorded with the Houston diagnostic criteria of antrum-predominant CAG. CAG in rats was related with the damage of barrier in gastric mucosa and the misbalance of cell proliferation and apoptosis. There was high protein expression of oncogene, while inhibitor of suppressor gene in CAG rats indicated high trend of carcinogenesis.
Gastric Mucin Phenotype Indicates Aggressive Biological Behaviour in Early Differentiated Gastric Adenocarcinomas Following Endoscopic Treatment

Abstract:Background The distribution of mucin phenotypes and their relationship with clinicopathological features in early differentiated gastric adenocarcinomas in a Chinese cohort are unknown. We aimed to investigate mucin phenotypes and analyse the relationship between mucin phenotypes and clinicopathological features, especially biological behaviours, in early differentiated gastric adenocarcinomas from endoscopic specimens in a Chinese cohort. Methods Immunohistochemical staining of CD10, MUC2, MUC5AC, and MUC6 was performed in 257 tissue samples from patients with early differentiated gastric adenocarcinomas. The tumour location, gross type, tumour size, histological type, depth of invasion, lymphovascular invasion, mucosal background and other clinicopathological parameters were evaluated. The relationship between mucin phenotypes and clinicopathological features was analysed with the chi-square test. Results The incidences of gastric, gastrointestinal, intestinal and null phenotypes were 21 %, 56 %, 20 and 3 %, respectively. The mucin phenotypes were related to histology classification ( P < 0.05). The proportion of the gastric phenotype became greater during the transition from differentiated to undifferentiated ( P < 0.05). Complete intestinal metaplasia was higher in the gastric and intestinal phenotypes than in the gastrointestinal phenotype ( P < 0.05). Tumours with poorly differentiated adenocarcinoma were mainly of the gastric phenotype, which was significantly higher than that of purely differentiated tubular adenocarcinoma ( P < 0.05), and the depth of invasion in the mixed type was deeper ( P < 0.05). Neither recurrence nor metastasis was detected. Conclusions The mucin phenotype of early-differentiated gastric adenocarcinoma has clinical implications, and the gastric phenotype has aggressive biological behaviour in early differentiated gastric cancers, especially in those with poorly differentiated adenocarcinoma or papillary adenocarcinoma components.

Abstract:The rapid development of highly accurate imaging techniques, has led to the discovery of asymptomatic lesions within the pituitary gland that constitute a significant challenge for clinicians and neurosurgeons. These lesions found incidentally are termed incidentalomas and can be observed in approximately 10% of MRI studies performed among healthy subjects. The natural course of these lesions has not been clearly defined. Although some of these masses do not change in size overtime and some may even shrink spontaneously, others have the potential risk to increase in volume and cause symptoms associated with mass effect or hormonal hypersecretion. This review describes the current therapeutic and diagnostic consensus in incidentaloma management, and places special emphasis on hormonal and imaging factors suggestive of high risk tumors.

Abstract:Chronic atrophic gastritis (CAG) is a complex syndrome in which long-term chronic inflammatory stimulation causes gland atrophy in the gastric mucosa, reducing the stomach's ability to secrete gastric juice and pepsin, and interfering with its normal physiological function. Multiple pathogenic factors contribute to CAG incidence, the most common being Helicobacter pylori infection and the immune reactions resulting from gastric autoimmunity. Furthermore, CAG has a broad spectrum of clinical manifestations, including gastroenterology and extra-intestinal symptoms and signs, such as hematology, neurology, and oncology. Therefore, the initial CAG evaluation should involve the examination of clinical and serological indicators, as well as diagnosis confirmation via gastroscopy and histopathology if necessary. Depending on the severity and scope of atrophy affecting the gastric mucosa, a histologic staging system (Operative Link for Gastritis Assessment or Operative Link on Gastritis intestinal metaplasia) could also be employed. Moreover, chronic gastritis has a higher risk of progressing to gastric cancer (GC). In this regard, early diagnosis, treatment, and regular testing could reduce the risk of GC in CAG patients. However, the optimal interval for endoscopic monitoring in CAG patients remains uncertain, and it should ideally be tailored based on individual risk evaluations and shared decision-making processes. Although there have been many reports on CAG, the precise etiology and histopathological features of the disease, as well as the diagnosis of CAG patients, are yet to be fully elucidated. Consequently, this review offers a detailed account of CAG, including its key clinical aspects, aiming to enhance the overall understanding of the disease.

Abstract:HDACs (histone deacetylases) are enzymes that remove the acetyl moiety from N‐∍‐acetylated lysine residues in histones and non‐histone proteins. In recent years, it has turned out that HDACs themselves are also subject to post‐translational modification. Such structural alterations can determine the stability, localization, activity and protein—protein interactions of HDACs. This subsequently affects the modification of their substrates and the co‐ordination of cellular signalling networks. Intriguingly, physiologically relevant non‐histone proteins are increasingly found to be deacetylated by HDACs, and aberrant deacetylase activity contributes to several severe human diseases. Targeting the catalytic activity of these enzymes and their post‐translational modifications are therefore attractive targets for therapeutical intervention strategies. To achieve this ambitious goal, details on the molecular mechanisms regulating post‐translational modifications of HDACs are required. This review summarizes aspects of the current knowledge on the biological role and enzymology of the phosphorylation, acetylation, ubiquitylation and sumoylation of HDACs.
[Proliferative Changes of Human Gastric Mucosa Cells in Different Pathological Lesions and Their Clinical Significance].

Abstract:OBJECTIVE:To investigate the variation of cell proliferation and growth in different pathological lesions of gastric mucosa and to assess the possible roles of expression of epidermal growth factor receptor (EGFR), transforming growth factor beta receptor type I and type II (TGF(beta)RI, TGF(beta)RII).METHODS:The proliferating cell nuclear antigen (PCNA), EGFR, TGF(beta)RIand TGF(beta)RII were studied in chronic superficial gastritis (CSG, n = 30), chronic atrophic gastritis (CAG, n = 26), intestinal metaplasia (IM,n = 40), Dysplasia (DYS, n = 22), early gastric cancer (EGC, n = 22), advanced gastric cancer (AGC, n = 26) by immunohistochemical methods and their relations with carcinogenesis were analyzed.RESULTS:(1) In different gastric mucosa lesions (CSG, CAG, IM, DYS, EGC, AGC), there were significantly different expression of PCNA (chi2 = 91.06, P < 0.0001), EGFR (chi2 = 52.82, P < 0.0001), TGF(beta)RI (chi2 = 15.93, P = 0.007) and TGF(beta)RII (chi2 = 40.48, P < 0.0001), PCNA and EGFR were increased, TGF(beta)RI and TGF(beta)RII were decreased. (2) In DYS stage, PCNALI (40.00 +/- 16.34) was higher than in CSG (16.63 +/- 10.52), CAG (16.92 +/- 8.50) and IM (23.25 +/- 18.64), but lower than EGC (53.09 +/- 13.51) and AGC (57.54 +/- 16.88) (P < 0.0001); (3) EGFR expression in IM (55.0%) and DYS (72.7%) were higher than in CSG (10.0%) and CAG (3.8%) (P < 0.0001), but no different with EGC (59.1%) and AGC (73.1%). (4) TGF(beta)RI expression in EGC (50.0%) and AGC (30.8%) were lower than in CSG (73.3%) (P = 0.007). (5) TGF(beta)RII expression in AGC (26.9%) was lower than in CSG (83.3%), CAG (82.8%), IM (65.0%), DYS (54.5%) and EGC (45.5%) significantly (P < 0.0001). (6) The expression of EGFR had positive correlation with PCNA, TGF(beta)RI and TGF(beta)RII had negative correlation with PCNA respectively, TGF(beta)RI and TGF(beta)RII had positive correlation.CONCLUSIONS:DYS is the key link in the change of cell proliferation during gastric carcinogenesis; The increase of EGFR and the decrease of TGF(beta)R may play important roles in promoting gastric carcinogenesis by affecting gastric cell proliferation.
Histopathological staging and differential diagnosis of marginal zone lymphoma of gastric mucosa-associated lymphoid tissue

Abstract:The purpose of this study was to explore the histopathological staging and differential diagnosis of marginal zone lymphoma in gastric mucosa-associated lymphoid tissue (MALT lymphoma). We performed detailed histomorphology and immunohistochemistry investigations as well as genetic testing on endoscopic biopsy and endoscopic mucosal resection specimens from 18 patients with gastric MALT lymphoma. We found that gastric MALT lymphoma typically begins as a small, isolated area outside the lymphoid follicular mantle zone or proliferates in a multifocal, patchy manner, gradually spreads to the interfollicular zone, forming diffuse proliferation, invades the gastric mucosal glands, and infiltrates or proliferates into the center of peripheral reactive lymphoid follicles. Abnormally proliferating lymphocytes invade the surrounding lymphoid follicles, resulting in damage, atrophy, and disappearance of their normal follicles as well as of the gastric mucosa glands, forming diffuse proliferation. Redifferentiation and proliferation lead to the transformation of lymphocytes; that is, MALT transitions into highly invasive lymphoma. Based on our findings in this study, we propose the following five stages in the process of development and progression of gastric MALT lymphoma: the stage of cell proliferation outside the lymphoid follicular mantle zone; the stage of heterogeneous proliferative lymphoepithelial lesion; the stage of reactive lymphoid follicular implantation; the stage of lymphoid follicular clonal proliferation; and the stage of MALT transforming into highly invasive lymphoma. We examined the differential diagnosis of histopathological features at each stage. The clinicopathological staging of gastric MALT lymphoma can help clinicians provide accurate treatment and track malignant cell transformation, thus playing a significant role in controlling its development and progression.

Morphological characteristics of background pathological processes in the mucous membrane adjacent to the tumor in gastric cancer of the intestinal type

Abstract:Introduction. Many fundamental and practical problems associated with pre-tumor changes in the stomach have not been sufficiently studied, which indicates the relevance of their study both at the stages of carcinoma development and in the mucous membrane adjacent to the tumor. The study aimed to detect the incidence, severity, and interconnections of background pathological processes in the gastric mucosa at different distances from the tumor node in gastric carcinomas of the intestinal type. Materials and methods. The surgical material of 43 patients with gastric carcinomas of the intestinal type after gastrectomy and resection of the stomach (626 tissue samples) from the tumor node and adjacent gastric mucosa at a distance from 1 cm to the resection lines was investigated. We assessed the frequency of occurrence, severity, and correlation relationships of the background pathological processes, i.e.,epithelial hyperplasia, intestinal metaplasia, and inflammatory and atrophic changes. Results. In the mucous membrane adjacent to the tumor, we revealed epithelial hyperplasia, intestinal metaplasia, and inflammatory and atrophic changes. There was a significant decrease in the number of cases of hyperplasia and intestinal metaplasia of the epithelium at the distance from the tumor. Moreover, we detected a decrease in the severity of all studied pathological processes. Positive moderate correlations were revealed between the parameters characterizing pathological processes at different distances from the tumor. Conclusion. The incidence and severity of background pathological processes, i.e., epithelial hyperplasia, intestinal metaplasia, and inflammatory and atrophic changes, detected in the mucous membrane in intestine type of gastric carcinoma decrease with distance from the tumor node. The results of the correlation analysis indicate the possibility of a functional relationship of such processes as epithelial hyperplasia, intestinal metaplasia and atrophy, as well as their important role in gastric carcinogenesis. The detection of these changes in the material obtained from the areas of the resection lines indicates the need for further study of their clinical significance. Clinicopathological Analysis of Primary Gastric (Gastrointestinal)-Type Mucoglandular Lesion of Endometrium

Abstract:Objective:To investigate the clinical and pathological features of primary gastric (gastrointestinal)-type mucoglandular lesions of the endometrium.Methods:Eight cases of primary gastric (gastrointestinal)-type mucoglandular lesions of endometrium diagnosed between 2014 to 2022 were retrieved from pathology archives of the Obstetrics and Gynecology Hospital Affiliated to Fudan University, Shanghai, China. The clinical history, pathological sections and follow-ups were analyzed.Results:The eight patients ranged in age from 35 to 67 years, with an average age of 55.5 years. Seven patients were examined for high-risk human papillary virus (HPV) before operation. Only one of them was positive for high-risk HPV52. No cervical mucinous lesions were found in any of the patients. Two cases were invasive gastric (gastrointestinal)-type adenocarcinoma, 2 cases were benign gastric (gastrointestinal)-type mucinous metaplasia, and the other 4 cases were atypical gastric (gastrointestinal)-type mucinous gland hyperplasia. Microscopically, tumor cells showed mucous epithelium with gastrointestinal differentiation. Immunophenotyping showed that MUC6 was diffusely or focally positive in 5 cases, CK20 and CDX2 were positive in 3 cases. And p16 was negative or focally positive in 5 cases and strongly positive in 1 case. ER was expressed in both benign and atypical lesions, and weakly positive or negative in the invasive adenocarcinoma. p53 showed mutant expression in one case and wild-type expression in the rest. HPV in situ hybridization was negative.Conclusions:Primary gastric (gastrointestinal)-type mucoglandular lesions of the endometrium show various forms of gastrointestinal differentiation, which are high-risk HPV independent. Morphology combined with immunohistochemistry is helpful for the diagnosis, which can only be made on exclusion of cervical gastrointestinal glandular lesion, gastrointestinal metastatic carcinoma and the mucinous subtype of endometrioid carcinoma.

Abstract:A parallel controlled clinical trial was carried out to investigate the effect on blood pressure (BP) of replacing normal salt with mineral salt in seasonings. After a 2-wk run-in period, 21 subjects (10 men and 11 women; age, 66.0+/-7.6 yr) were given mineral salt in seasonings instead of normal salt [mineral salt (MS) group], while 20 subjects (10 men and 10 women; age, 65.9+/-7.4 yr) continued to receive normal salt in seasonings [regular salt (RS) group] for the next 5 wk in the experimental (E) period. In the MS group, systolic (S) and diastolic (D) BP decreased significantly from 134.7+/-17.2/77.2+/-9.7 at baseline (week 0) to 127.3+/-12.0/73.5+/-8.9 mmHg at the end of the E period (week 5), and the reductions in both SBP and DBP were larger in hypertensive subjects than in normotensive subjects in the MS group. The serum sodium and chloride concentrations decreased significantly, while the serum potassium and HDL-cholesterol concentrations increased significantly at week 5. The 24-h urinary sodium excretion decreased, and the 24-h potassium and magnesium excretions increased significantly from week 0 to week 5 in the MS group. In contrast, SBP, DBP, serum chemistry, and urinary electrolyte excretion did not change significantly in the RS group. These findings suggest that the excessive salt content and insufficient potassium and magnesium content of the present Japanese diet could be easily and safely corrected by replacing seasonings prepared with regular salt with those prepared with mineral salt.
Distinction of differentiated type early gastric carcinoma with gastric type mucin expression

Abstract:Background: Intestinal and diffuse gastric carcinomas differ in morphology and growth behavior. Differentiated type gastric carcinoma (DGC), which corresponds roughly with the intestinal type of Lauren, can demonstrate phenotypic properties associated with mucin expression and brush border. However, their clinical significance is controversial. A classification based on mucin phenotype and brush border was performed to determine the clinicopathologic diversity of DGCs in their early stage. Methods: A total of 120 specimens from 116 DGC patients with definite submucosal invasion were evaluated both macroscopically and histologically. All sections were examined immunohistochemically with human gastric mucin, Muc-2, and CD10 and with mucin histochemically with paradoxical concanavalin A staining and high iron Diamine-Alcian Blue. They were classified into gastric type (G-type), intestinal type (I-type), mixed gastric and intestinal type (M-type), or null type (N-type) phenotypes. The immunoreactivity of E-cadherin and beta-catenin also was investigated to determine the correlation between mucin phenotype and clinicopathologic factors. Results: The G-type phenotype was found to be in contrast to I-type: G-type was an independent factor associated with lymph node metastasis. Significant correlations were observed between the G-type phenotype and the complex type carcinoma found that was histologically: lymphatic invasion, lymph node metastasis, and the abnormal expression of E-cadherin. A significant difference in the proportion of mucin phenotypes between papillary type and tubular type carcinoma was observed. G-type was found to be the predominant phenotype in papillary carcinoma in contrast to tubular carcinoma. Conclusions: The G-type mucin phenotype and papillary adenocarcinoma should be distinguished from other types of DGCs because of their increased malignant potential in the incipient phase of invasion and metastasis. The significance of G-type and papillary adenocarcinoma should be reflected in the treatment of patients with early stage DGCs, including endoscopic mucosal resection.
Studies of the Pathologic Change of Gastric Mucosa and Regulating Factors of Cell Proliferation in Rats with Experimental Chronic Atrophic Gastritis

Abstract:Objective To assess pathologic and molecular regulating changes of cell proliferation of gastric mucosa in rats with chronic atrophic gastritis (CAG) and approach the possible mechanisms of the disease. Methods Rats were administered with 60% alcohol (2% Salicylate Sodium), 20 mmol/L sodium deoxycholate and 0.1% ammonia water to establish CAG model. The pathological change and mucin secretion were evaluated. The level of the serum prostaglandin E (PGE)2 and endothelial growth factor (EGF) were measured with radioimmunoassay. The immunohistochemistry was used to detect the expression of EGF receptor (EGFR), C-erbB2, P53, P16 and Bcl-2. Results Compared with normal rats, the grade of inflammatory cell infiltration in gastric mucosa in CAG rats was elevated, whereas the thickness and number of gastric gland were significant lower. Scanning electromicroscopy showed the cells rupture and injury of barrier of gastric mucosa. The levels of haxosamine in gastric mucosa and PGE2 in serum were lower in CAG rats than in normal rats (P0.05). Compared with normal level of (0.61±0.28) μg/L, EGF in CAG in serum (2.24± 0.83) μg/L was significant higher (P0.05). The immunohistochemistry studies showed that the expression of EGFR and C-erbB2 were 45% and 70% respectively in CAG rats, while no expression in normal rats (P 0.05). The expression rate of P16 was 75% in normal rats,which was 10% higher than that in CAG rats (P0.05). Compared with the rats in normal group, Bcl-2 expression was higher in CAG (10% vs. 50% P0.05). There was no significant difference in expression of P53 between CAG and normal group (30% vs. 10%, P0.05).Conclusion The pathogenesis of CAG in rats was related with the damage of barrier in gastric mucosa and the imbalance of cell proliferation and apoptosis.

Abstract:The article deals with the appraisal of the gastric mucosa in various developmental stages of pyloroduodenal stenosis. The characteristics of the mucosa after selective proximal vagotomy (SPV) and after resection of the stomach are compared. It was found that the normal structure of the gastric mucosa is restored 12 month after SPV. Chronic gastritis with signs of dystrophy as well as intestinal and pseudopyloric metaplasia develops after resection of the stomach.

Abstract:AIM:To evaluate the relationship between gastric lesions and cell proliferation and apoptosis activity. METHODS:A total of 1523 subjects received an endoscopic examination and five biopsies were obtained from standard sites of the stomach.Ex- pression of Ki-67 was assessed by immunohis- tochemistry,and apoptotic cells were detected with the terminal deoxynucleotide transferase mediated dUTP nick end-labeling(TUNEL) method. RESULTS:Among the subjects screened by en- doscopic examination,93.8% had chronic atro- phic gastritis,intestinal metaplasia or dysplasia. The prevalence rates of gastric lesions varied in different sites of the stomach.In the greater curvature of the body,the proportion of normal mucosa or superficial gastritis was 84.56%,but dysplasia was only 0.07%.In the angulus,the proportion of normal mucosa or superficial gas- tritis was reduced to 19.19%,while the propor- tion of dysplasia reached 5.26%.Furthermore, gastric lesions were positively associated with cell proliferation activity,showing a tendency to increase Ki-67 labeling index according to the se- verity of gastric lesions.However,no significant association was seen between gastric lesion and cell apoptosis(P=0.159). CONCLUSION:Advanced gastric lesions,such as intestinal metaplasia and dysplasia,tend to occur along the lesser curvature of stomach, where gastric cancer occurs.Imbalance in cell proliferation and apoptosis can be observed in advanced gastric lesions.

Abstract:Personality comparisons using Cattell's 16PF were made between 137 pilot incumbents, 81 pilot applicants, and the general population norms. No significant differences were found between the scores on the personality factors for the Pilot Incumbents and the Pilot Applicants. Further, the incumbents and applicants who had previous military training versus those who did not had highly similar personalities. However, on nearly every personality factor a significant difference was found between the general population norms and the sample of Pilot Incumbents and Applicants. The Pilot Incumbent/Applicant group scored significantly more intelligent, emotionally stable, and mature in comparison to the general population norms. We believe that it is the high-risk nature of this occupation that leads applicants, wishing to pursue this field, to assess very carefully their own person-job fit and self-select themselves, thus ultimately producing this very distinct “pilot personality profile” described in 1995 by Bartram.
Primary Gastric Mucosa-Associated Lymphoid Tissue Lymphoma in Endoscopic Biopsied Specimens:A Pathological Analysis of 32 Cases

Abstract:Objective: To investigate the pathological characteristics of gastric mucosa associated lymphoid tissue(MALT) lymphoma in endoscopic biopsied specimens.Methods: All the tissue samples were 10% formalin-fixed,paraffin-embedded and stained with hematoxylin-eosin and SP immunohistochemistry.Results: There were variable atypia,diffuse lymphoid cell infiltration with decrease or disappearance of the gland in the biopsied sample.The lymphoepithelial lesions were seen in 12 cases and suspected lymphoepithelial lesions in 7 cases.There were inflammatory necrosis and pressing tissues injury in 15 cases.Conclusion: The main diagnostic characteristics of gastric lymphoma were variable atypia,diffuse lymphoid cell infiltration in the biopsied tissues.Lymphoepithelial lesions were not always observed,but other changes were often seen in the biopsied samples.Multiple deep biopsy,special pathological changes,immunohistochemistry and molecularbiology techniques will help to reach the correct diagnosis of gastric MALT lymphoma.
Pathological Changes in Gastric Polyp Tissues and Its Background Mucosa under Gastroscopy

Abstract:Objective To investigate the relationship between the pathology of gastric polyp tissues and the precancerous and malignant transformations of its background mucosa by endoscopic biopsy. Methods From 2003 to 2008,1825 patients were diagnosed with gastric polyps in our hospital by gastroscopy and pathological examination. We classified the lesions and then analyzed the relationship between pathological changes of gastric polyp tissues and the precancerous and malignant transformations of its background mucosa by endoscopic biopsy. Results In this study,the detection rate of gastric polyps was 4.3% (1825/42 003). The lesions included fundic gland polyps (FGP,62.5%,1141/1825),hyperplasia polyps (HPP,32.2%,587/1825),adenoma (0.8%,14/1825),inflammatory polyps (IP,1.7%,31/1825),and coincident polyps (two types of polyps were found in one case,2.8%,52/1825). The rates of intestinal metaplasia (IM,5.3%,31/587) and dysplasia (6.8%,40/587) in HPP tissue were higher than those in FGP tissues (0.2%,2/1141 and 0.4%,4/1141,P<0.001). The rate of carcinoma in HPP tissue was 0.3% (2/587),no carcinoma was detected in other types of polyp. As shown in pathological analysis of the background mucosa,the rates of IM in the background mucosa of all types of gastric polyps were higher than those in polyp tissues (P<0.05),and the IM in the background mucosa was more severe than that in polyps tissue in FGP,HPP and IP cases (P<0.05). The rates of dysplasia in the background mucosa of FGP and adenoma cases were higher than those in polyps tissues (P=0.000),and the dysplasia in the background mucosa was more severe than that in polyp tissues in FGP and adenoma cases (P=0.000). The rate of malignant transformation in background mucosa (0.5%,7/1355) was higher than that in polyp tissues (0.1%,2/1773). There were three,one,and three cases of malignant transformation of the background mucosa respectively in FGP,HPP and adenoma cases. Conclusions The incidence of gastric polyps is low. In various types of polyps,malignant transformation may be detected in HPP. Whereas,both precancerous lesions and malignant transformation are detectable in background mucosa of different types of polyps. Therefore,it is necessary to carry out endoscopic biopsy of the background mucosa.
