Abstract:Abstract Background: Breast cancer (BC) occurs in 1 of 8 women, often requiring debilitating surgery, chemotherapy or radiation for long term survival. Histologic and molecular biomarkers are used to classify BC according to defined subtypes which dictate the choice of targeted therapy or of non-targeted cytotoxic therapy. Despite high initial response rates, relapses are common for more aggressive tumors, and choosing the right therapy for each patient remains challenging. In vitro 3D BC models maintain biologic features that more closely resemble clinical disease than 2D models. However, many 3D models do not contain multiple cell types, are maintained in static culture conditions and rely on immortalized cell lines previously propagated in 2D culture conditions. To address these issues, we developed long term, 3D heterotypic BC microtumors, which recapitulate the dynamic interaction between stromal and epithelial components, retain subtype-specific biomarkers and demonstrate clinically-relevant drug response. We further demonstrated the value of developing non-lytic, label-free in situ analysis to monitor morphology and function of complex 3D microtumors over time. Materials & Methods: Er+, Her2+ or triple negative (TNBC) cell lines (MCF7, SKBR3, MDA-MB-231) or patient derived xenograft (PDX) cells were embedded with human mammary fibroblasts and adipose cells within a hydrogel encapsulated by a silk fibroin scaffold. Microtumors were maintained at least 4 weeks under perfusion flow utilizing the 3DKUBE™ and were characterized for cell morphology and phenotype (IHC), proliferation (PrestoBlue and PicoGreen), gene expression (qRT-PCR), redox ratio (multiphoton microscopy), and biomarker secretion (xMAP® multiplex immunoassay). Drug response profiling (DRP) was performed with tamoxifen, lapatinib and cisplatin. Results: 3D microtumors successfully recapitulated the morphology of primary BC predicted by molecular subtype and gene expression. Perfusion promoted cell proliferation and impacted redox ratio, gene expression, and biomarker secretion in comparison to static culture. Relative redox ratios of 3D microtumors were significantly different from those of cell lines in 2D (p<0.05). Perfusion, 3D conditions, Her2+ and TNBCs were independently associated with increased biomarker secretion, and both cell line and PDX microtumors had unique secretome signatures. PDX microtumors more accurately predicted drug response. Conclusions: Long-term, 3D heterotypic breast microtumors have unique metabolic and secretome signatures which are different than cells in 2D, and the microtumor morphology, metabolism and drug response can be monitored non-destructively in situ. Our ultimate goal is to develop these microtumors using primary human breast tumors for real time drug response profiling in the preclinical, co-clinical and clinical settings to improve outcomes for women with breast cancer. A novel patient-derived organoids-based xenografts model for preclinical drug response testing in patients with colorectal liver metastases

Abstract:Cancer-related mortality in patients with colorectal cancer (CRC) is predominantly caused by development of colorectal liver metastases (CLMs). How to screen the sensitive chemotherapy and targeted therapy is the key element to improve the prognosis of CLMs patients. The study aims to develop patient-derived organoids-based xenografted liver metastases (PDOX-LM) model of CRC, to recapitulate the clinical drug response. We transplanted human CRC primary tumor derived organoids in murine spleen to obtain xenografted liver metastases in murine liver. Immunohistochemistry (IHC) staining, whole-exome and RNA sequencing, and drug response testing were utilized to identify the homogeneity in biological and genetic characteristics, and drug response between the PDOX-LM models and donor liver metastases. We successfully established PDOX-LM models from patients with CLMs. IHC staining showed that positive expression of CEA, Ki67, VEGF, FGFR2 in donor liver metastases were also well preserved in matched xenografted liver metastases. Whole-exon sequencing and transcriptome analysis showed that both xenografted and donor liver metastases were highly concordant in somatic variants (≥ 0.90 frequency of concordance) and co-expression of driver genes (Pearson’s correlation coefficient reach up to 0.99, P = 0.001). Furthermore, drug response testing showed that the PDOX-LM models can closely recapitulated the clinical response to mFOLFOX6 regiments. This PDOX-LM model provides a more convenient and informative platform for preclinical testing of individual tumors by retaining the histologic and genetic features of donor liver metastases. This technology holds great promise to predict treatment sensitivity for patients with CLMs undergoing chemotherapy.

Abstract:Pharmaceutical research requires pre-clinical testing of new therapeutics using both in-vitro and in-vivo models. However, the species specificity of non-human in-vivo models and the inadequate recapitulation of physiological conditions in-vitro are intrinsic weaknesses. Here we show that perfusion is a vital factor for engineered human tissues to recapitulate key aspects of the tumour microenvironment. Organotypic culture and human tumour explants were allowed to grow long-term (14–35 days) and phenotypic features of perfused microtumours compared with those in the static culture. Differentiation status and therapeutic responses were significantly different under perfusion, indicating a distinct biological response of cultures grown under static conditions. Furthermore, heterogeneous co-culture of tumour and endothelial cells demonstrated selective cell-killing under therapeutic perfusion versus episodic delivery. We present a perfused 3D microtumour culture platform that sustains a more physiological tissue state and increased viability for long-term analyses. This system has the potential to tackle the disadvantages inherit of conventional pharmaceutical models and is suitable for precision medicine screening of tumour explants, particularly in hard-to-treat cancer types such as brain cancer which suffer from a lack of clinical samples.

Abstract:Cancer organoids to model therapy response Cancer organoids are miniature, three-dimensional cell culture models that can be made from primary patient tumors and studied in the laboratory. Vlachogiannis et al. asked whether such “tumor-in-a-dish” approaches can be used to predict drug responses in the clinic. They generated a live organoid biobank from patients with metastatic gastrointestinal cancer who had previously been enrolled in phase I or II clinical trials. This allowed the authors to compare organoid drug responses with how the patient actually responded in the clinic. Encouragingly, the organoids had similar molecular profiles to those of the patient tumor, reinforcing their value as a platform for drug screening and development. Science , this issue p. 920

Abstract:Introduction: Most advanced colorectal cancers are aggressive, and there is a lack of effective methods for selecting appropriate anticancer regimens. Patient-derived organoids (PDOs) have emerged as preclinical platforms for modeling clinical responses to cancer therapy. Methods: In this study, we successfully constructed a living biobank with 42 organoids derived from primary and metastatic lesions of metastatic colorectal cancer patients. Tumor tissue was obtained from patients undergoing surgical resection of the primary or metastatic lesion and then used to establish PDOs. Immunohistochemistry (IHC) and drug sensitivity assays were performed to analyze the properties of these organoids. Results: The mCRC organoids were successfully established with an 80% success rate. The PDOs maintained the genetic and phenotypic heterogeneity of their parental tumors. The IC50 values of5-fluorouracil (5-FU), oxaliplatin, and irinotecan (CPT11) were determined for mCRC organoids using drug sensitivity assays. The in vitro chemosensitivity data revealed the potential value of PDOs for clinical applications in predicting chemotherapy response and clinical outcomes in mCRC patients. Discussion: In summary, the PDO model is an effective platform for in vitro assessment of patient-specific drug sensitivity, which can guide personalized treatment decisions for patients with end-stage CRC.

Abstract:Cancer global burden accounts for 19.3 million new cases and 10 million deaths in 2020. Despite advances on the understanding of disease progression and efforts to find successful therapeutic strategies, most drugs fail in clinical stages. Lack of models with clinical mimicry recapitulating the in vivo pathophysiology and the organ microenvironment where the tumor forms is a key driver reducing the success rate of anti-cancer drugs. Animal models remain the gold standard for preclinical research but fail to recapitulate the complex human tumor microenvironment, a critical component determining tumor development, progression, and response to treatments. Organ-on-a-chip models allow to control the environment and organization of cells in an in vivo-like arrangement by culturing multiple cell types to better replicate human conditions, and by implementing different biochemical and physical stimuli, such as fluid shear stress, chemical/nutrient gradients, and perfusion/flow to recapitulate the human vascular system physiology. We leverage our 3D bioprinting technologies to develop complex vascularized organ-on-a-chip models aimed to recapitulate the tumor and liver niche. Both organ-systems are interconnected by our h-VIOSTM platform, allowing dynamic perfusion for 21 days and enabling the simultaneous assessment of anti-tumor therapeutic efficacy and drug-induced liver injury. Our hydrogels feature a complex perfusable vasculature that interfaces with an interstitial chamber where tissues of interest are grown. The vasculature allows for continued and controlled delivery of nutrients, oxygen, and small molecule drugs. Dextran diffusion studies revealed diffusion coefficients ranging from 43.2 μm2/s (4kDa) to 8 μm2/s (70kDa). Vascular architectures are lined with endothelial cells forming a functional barrier expressing the junctional markers ZO-1 and PECAM-1. Moreover, we can modulate the permeability of our vasculature architecture, allowing the extravasation of immune and cancer cells. PBMC-derived T cells and Jurkat cells undergo transendothelial migration in response to a CXCL12 gradient, highlighting the applicability of our model for immunotherapeutic efficacy studies. The tumor interstitial chamber incorporates co-culture of cancer spheroids or tumoroids, fibroblasts, endothelial and immune cells. Colon carcinoma spheroids effectively integrate into the tumor niche with no growth arrest. The liver model consists of an interstitial chamber with a lobular microstructure supporting the monoculture of primary human hepatocytes, and sustaining their viability and function for 9 days as demonstrated by high albumin production, urea synthesis and CYP activity. Our organ-on-a-chip models are fully customizable and anticipated to contribute to advance personalized medicine and improve clinical translation rates. Abstract:The tumor microenvironment plays a critical role in tumor initiation, progression, metastasis, and drug resistance. However, models recapitulating the complex 3D structure, heterogeneous cell environment, and cell–cell interactions found in vivo are lacking. Herein, we report on a gravitational microfluidic platform (GMP) retrofitted with MEMS sensors, which is integrated with 3D nanofiber scaffold-aided tumoroid culture. The results showed that this GMP for tumoroid growth mimics the tumor microenvironment more precisely than static culture models of colon cancer, including higher drug resistance, enhanced cancer stem cell properties, and increased secretion of pro-tumor cytokines. In addition, the GMP includes integrated surface acoustic wave-based biosensing to monitor cell growth and pH changes to assess drug efficacy. Thus, this simple-to-use perfused GMP tumoroid culture system for in vitro and ex vivo studies may accelerate the drug development process and be a tool in personalized cancer treatment.

Abstract:A living biobank of patient‐derived organoid (PDO) is derived from primary colorectal cancer and paired liver metastatic lesions, which captures intra‐ and interpatient molecular fingerprint heterogeneity. PDO chemosensitivity measured in vitro may be used as a predictive tool for clinical chemotherapeutic efficacy, and guide the formulation of precise treatment strategies for colorectal cancer patients with liver metastases. There is no effective method to predict chemotherapy response and postoperative prognosis of colorectal cancer liver metastasis (CRLM) patients. Patient‐derived organoid (PDO) has become an important preclinical model. Herein, a living biobank with 50 CRLM organoids derived from primary tumors and paired liver metastatic lesions is successfully constructed. CRLM PDOs from the multiomics levels (histopathology, genome, transcriptome and single‐cell sequencing) are comprehensively analyzed and confirmed that this organoid platform for CRLM could capture intra‐ and interpatient heterogeneity. The chemosensitivity data in vitro reveal the potential value of clinical application for PDOs to predict chemotherapy response (FOLFOX or FOLFIRI) and clinical prognosis of CRLM patients. Taken together, CRLM PDOs can be utilized to deliver a potential application for personalized medicine.

Abstract:Patient-derived organoids (PDOs) are improving our understanding of cancer heterogeneity and personalized medicine. In particular, patients' intrinsic and acquired treatment resistance is being evaluated with PDOs, which has become an essential factor in developing new anticancer drugs and designing successful clinical trials. However, there are obstacles to cancer heterogeneity and patient-specific cancer therapy due to lack of understanding of tumoral diversity resulting from the tumor microenvironment (TME). Complex direct and indirect interactions between various types of TME cells and the resulting tumoral diversity are challenges that must be addressed for customized cancer therapy. Standard cancer therapy targets tumor cells without considering possible damage on the tumor microenvironment that could impair therapy response. Therefore, the construction and analysis of a platform for co-culturing cancer organoids with other cell types that make up the TME over a long period of time is necessary. We generated and performed characterization of blood vessel organoid (BVO). The BVO expressed endothelial marker CD31 and pericyte marker PDGFRβ. We attempted to co-culture with patient-derived lung cancer organoid (LCO) on Matrigel by dissociating the constructed BVO, and it was confirmed that BVO and LCO were interconnected after 10 days. This new platform was treated with Gefitinib and Poziotinib, and after 5 days of drug treatment, the IC50 value increased compared to LCO-only culture. Additionally, flow cytometry analysis confirmed that co-culturing with BVO during drug treatment resulted in a lower percentage of dead LCO cells compared to LCO-only culture. Image-based analysis also verified that co-culturing with blood vessels enhances the viability of LCOs when treated with drugs. Taken together, the co-culture of cancer organoids and BVOs present a platform that closely mimics the in vivo TME compared to the existing cancer organoid culture method. This innovative platform demonstrates higher drug resistance than existing approaches, offering a novel method for drug screening. Abstract:The effective Hamiltonian formalism is extended to vectorial electromagnetic waves in order to describe statistical properties of the field in reverberation chambers. The latter are commonly used in electromagnetic compatibility tests. As a first step, the distribution of wave intensities in chaotic systems with varying opening in the weak coupling limit for scalar quantum waves is derived by means of random matrix theory. In this limit the only parameters are the modal overlap and the number of open channels. Using the extended effective Hamiltonian, we describe the intensity statistics of the vectorial electromagnetic eigenmodes of lossy reverberation chambers. Finally, the typical quantity of interest in such chambers, namely, the distribution of the electromagnetic response, is discussed. By determining the distribution of the phase rigidity, describing the coupling to the environment, using random matrix numerical data, we find good agreement between the theoretical prediction and numerical calculations of the response.

Abstract:BACKGROUND/AIM: Cancer mortality has decreased due to the contribution of extensive research on cancer treatment, including chemotherapy, radiation, and immunotherapy. However, histopathologically similar tumors originating from the same organ are treated with identical or similar chemotherapeutic regimens regardless of patient characteristics or cancer subtypes. The aim of this study was to evaluate the utility of organoids in predicting responses to chemotherapeutic agents.PATIENTS AND METHODS: This study retrospectively reviewed patient-derived organoids (PDOs) from 10 colorectal cancer patients to compare chemotherapy responses. Drug sensitivities for 5-fluorouracil (5-FU), cisplatin, oxaliplatin, and irinotecan were compared using GI50 (concentration that inhibits cancer cell growth by 50%).RESULTS: When organoids were treated with 5-FU, GI50 was the lowest compared to the other three chemotherapeutic agents (cisplatin, oxaliplatin, and irinotecan). The responsiveness to chemotherapeutic agents differed depending on specific patient characteristics including age, tumor location, stage, and gross type. The response of the patients' organoids to chemotherapeutic agents was consistent with the response to chemotherapy actually performed in those patients with cancer recurrence after surgery.CONCLUSION: PDOs may be useful as a preclinical model in predicting chemotherapy responses in cancer patients.

Abstract:153 Background: Conventional monolayer cell cultures and xenograft models, while useful and economical in early drug discovery, cannot predict clinical efficacy. Further, preclinical screening assays that rely on differential metabolic activity between separate control and treated wells are incapable of capturing phenotypic response and could overstate efficacy for cells with high rates of proliferation. Consequently, over 95% of anticancer agents that show efficacy in preclinical studies, fail in clinical trials. Recently, patient-derived organoid (PDO) models have been utilized in developing platforms to predict clinical efficacy of preclinical formulations. If successful, such predictive ex vivo technologies could revolutionize cancer treatment by reducing cost and time-to-market for new, more effective therapeutics. Methods: Bioink for 3D printing BOTs was prepared with HT-29 cells, an established NCI-60 human colorectal adenocarcinoma cell line with known sensitivity to proteosome and survivin inhibitors. Bioink was deposited layer-by-layer on multiple substrates, in various geometrical configurations, and cured in stages to allow cells and matrix to self-assemble with limited degrees of freedom. BOTs were screened 24h and 48h after printing with proteosome inhibitor Bortezomib and survivin inhibitor YM-155. BOTs were evaluated 48h and 72h after treatment using immunofluorescence live/dead assay. Morphological phenotypic changes resulting from treatment were also recorded. Results: Proteasome and survivin inhibitors have been reported to inhibit proliferation and induce cell death in colorectal cancer cells. A dose dependent response was observed for both agents in our novel BOT HTS thereby validating the platform. In addition, characteristic self-assembly of HT-29 cells was observed to be disrupted at effective doses and at certain concentrations below the effective dose. Traditional ATP assays are incapable of recording such phenotypic modulation. Further, a higher proliferation profile was observed in untreated BOTs suggesting that use of independent control wells in traditional assays could overstate efficacy of treatment. Conclusions: Functional high-throughput ex vivo DRP technologies have the potential to transform cancer treatment – from bench to bedside – along the drug discovery to market roadmap for much needed novel anticancer agents.

Abstract:Background: Colorectal cancer (CRC) is a heterogeneous disease; current research relies on cancer cell lines and animal cancer models, which may not precisely imitate inner human tumors and guide clinical medicine. The purpose of our study was to explore and further improve the process of producing three-dimensional (3D) organoid model and impel the development of personalized therapy. Methods: We subcutaneously injected surgically resected CRC tissues from a patient into BALB/c-nu mice to build patient-derived xenografts (PDXs). Isolated cells from PDXs at appropriate tumor size were mingled with Matrigel, and then seeded in ultra-low attachment 96-well plates at four cell densities (500, 1000, 2000, and 4000 single cells/well). Cells were cultured with advanced Dulbecco's Modified Eagle Medium/F12 medium additional with various factors added to maintain tumor's biological traits and growth activity. The growth curves of the four cell densities were measured after 24 h of culture until 25 days. We evaluated the effects of four chemotherapeutic agents on organoid model by the CellTiter-Glo® Luminescent Cell Viability Assay. Hematoxylin and eosin (H and E) staining of 3D organoids was performed and compared with patient and CRC PDX tissues. Furthermore, immunohistochemistry was performed, in which the organoids were stained with the proliferation marker, Ki-67. During the experimental process, a phase-contrast microscope was used. Results: Phenotype experimental results showed that 3D organoids were tightly packed together and grew robustly over time. All four densities of cells formed organoids while that composed of 2000 cells/well provided an adequate cultivation system and grew approximately 8-fold at the 25th day. The chemosensitivity of the four conventional drugs was [s]-10-hydroxycamptothecin > mitomycin C > adriamycin > paclitaxel, which can guide clinical treatment. Histological features of CRC patient's tumor tissues and mice tumor xenograft tissues were highly similar, with high-column-like epithelium and extracellular matrix. H and E-stained sections showed heterogeneous cell populations harbored in cancer organoids and were histologically similar to tumor tissues. The proliferation index was only 8.33% within spheroids, which exhibited confined proliferative cells that might be cancer stem cells. Conclusions: We successfully constructed a CRC organoid model that grew robustly over 25 days and demonstrated that 2000 cells/well in 96-well plate was a prime seeding density for cells to form organoids. The results confirmed that organoid model can be used for agent screening and personalized medicine.

Abstract:The tumor microenvironment (TME) is well recognized for its role in tumor progression, chemoresistance, and recurrence. The TME consists of various components, such as extracellular matrix (ECM), chemical and mechanical cues, as well as stromal cells and their interactions. As the most abundant cells in tumor stroma, cancer-associated fibroblasts (CAFs) have drawn a lot of attention. It has been reported that CAFs not only directly interact with cancer cells via surface proteins and cytokines, but also influence their behaviors by remodeling the ECM. However, due to their heterogenous characteristics and diverse functions, the influence of tumor-CAF interactions on drug response is not fully understood. Here, we present a coculture model for investigating CAF effects on patient-derived colorectal tumor organoids (CTOs) growth, differentiation, and drug response. It consists of normal colon fibroblasts, CCD18-Co, or patient-derived CAFs seeded with CTOs in a mixed hydrogel containing 90% basement membrane extract (BME) and 10% collagen type I. Media conditions were optimized (1:1 mix of CTO culture media plus reduced media excluding growth factors and inhibitors) to achieve similar organoid sizes in the monoculture and coculture conditions at the onset of drug testing. To support higher throughput investigations, our workflow incorporates a liquid handler for automated CTO seeding, media and drug exchange, and an automated microscope with a robotic arm for multi-plate imaging. Briefly, single cell CTOs and fibroblasts were seeded at a 1:1 ratio (3000 total cells/well) in 96-well plates. After 4 days in culture, anti-cancer drugs were administrated in reduced media to the samples. Brightfield images were acquired on days 0, 3, and 5 after drug administration. The collected images were analyzed with a neural network (NN) to dynamically monitor the growth and viability of individual organoids. The CellTiter-Glo assay was performed on day 5 to quantify the total CTO cell viability. The normalized drug response (NDR) was calculated from the NN processed images to determine the drug response. An immunostaining panel, which includes protein markers such as CK20, Ki67, α-SMA, Vimentin, and FAP, was used to evaluate the fibroblast phenotypes and CTO differentiation. Additionally, the cultures were subjected to fluorescent lifetime imaging microscopy (FLIM), a label-free imaging technique, to measure the metabolic adaptations of each cell population. Preliminary studies revealed a CAF-dependent increase in CTO growth and glycolytic signatures in the co-culture setting; however, minimal effects on drug response to common chemotherapy agents such as 5-fluorouracil and irinotecan were observed. In the future, we will use this integrated pipeline as a screening method to identify new treatment strategies aimed at targeting the stromal microenvironment in colorectal cancer. Abstract:Background. Organoids have been widely accepted as 3D tumor models for patient-specific drug response. However, research involving non-small cell lung cancer (NSCLC) organoids is hampered by the lack of appropriate platforms. Although dome cultures show promising results, the relatively large size variability of patient-derived organoids (PDOs) hinders their application in drug response studies. Here, we report a microfluidic chip for maintaining high uniformity and requiring miniscule sample volume for PDO drug screening. Methods. The microfluidic chip includes a bottom microwell layer and a top microchannel layer. Each chip has 5 channels for parallel treatment conditions, with 30 U-shaped microwells (400μm dia x 250μm deep) per channel. PDOs were loaded from side channels and trapped by filters adjacent to microwells. NSCLC PDOs F231/F671 (NCI PDMR) were cultured and treated by KRASG12C inhibitor adagrasib (0-2000nM, 72h). PDO drug response was assessed via viability using fluorescent dyes. Results. On-chip PDO size after one-week incubation exhibited the coefficient of variation (CV) of 28% for F231 and 36% for F671, which was notably lower than measurements obtained using Matrigel dome (126%, n=3) and low attachment plate (80%, n=4). The F231/F671 showed viability >81% after 1wk (n=20). On-chip cytotoxicity results showed that both KRASG12C PDOs exhibited sensitivity to adagrasib, matching the clinical response, with IC50 values of 830 nM (F231) and 1324nM (F671). When cultured with fibroblast (WI-38) supernatant, the cytotoxic effect was mitigated due to the fibroblast-induced resistance, with IC50 increasing ≥2 ×. No statistical difference was observed for the fibroblast effect in the open-format microwells and our microfluidic chip (69.6±16.9% to 80.2±9.0% vs. 69.0±12.2% to 82.2±4.1%, respectively, following 500nM adagrasib for 72h), further underscoring the reliability of our chip for PDOs' drug response studies. Application of continuous perfusion at 70nL/min yielded no statistical difference in viability from static conditions. Conclusions. Our microfluidic chip offers uniform PDO growth with perfusion allowing drug screening in almost real time. The on-chip response to adagrasib was clinically relevant. Resistance due to tumor microenvironment (TME) can be readily assessed using fibroblast supernatant, and could be expanded to other TME components in future studies to investigate their impact on treatment resistance. Abstract:Determinations of cerebrospinal fluid (CSF) albumin, IgG, albumin blood brain barrier (BBB) permeability and local IgG synthesis indexes in CNS were carried out on 85 patients with various neck, shoulder and upper extremity pain syndromes. CSF was obtained by lumbar puncture in 29, and by lateral neck puncture in 56 of the patients. The patients were classified into 3 different groups according to varying severity of degenerative changes, or cavitation verified by myelography. CSF protein patterns in these patients were compared with lumbar CSF findings in 18 patients with multiple sclerosis. CSF protein changes in patients with abnormal myelographic findings were slight. Protein values were clearly more abnormal in lumbar CSF than in cervical CSF, probably due to a retardation of the CSF flow. Only 3 of 62 patients with a narrowing of the cervical spinal canal had pathological values for IgG synthesis or BBB permeability indexes. On the other hand, 14 of 18 patients with multiple sclerosis had abnormal, high values for the IgG index. Thus the present results suggest that investigation of the CSF protein pattern has value in differential diagnosis between patients with multiple sclerosis and degencrative diseases of the cervical spine.

Abstract:3D cell culture models have been developed to better mimic the physiological environments that exist in human diseases. As such, these models are advantageous over traditional 2D cultures for screening drug compounds. However, the practicalities of transitioning from 2D to 3D drug treatment studies pose challenges with respect to analysis methods. Patient-derived tumor organoids (PDTOs) possess unique features given their heterogeneity in size, shape, and growth patterns. A detailed assessment of the length scale at which PDTOs should be evaluated (i.e., individual cell or organoid-level analysis) has not been done to our knowledge. Therefore, using dynamic confocal live cell imaging and data analysis methods we examined tumor cell growth rates and drug response behaviors in colorectal cancer (CRC) PDTOs. High-resolution imaging of H2B-GFP-labeled organoids with DRAQ7 vital dye permitted tracking of cellular changes, such as cell birth and death events, in individual organoids. From these same images, we measured morphological features of the 3D objects, including volume, sphericity, and ellipticity. Sphericity and ellipticity were used to evaluate intra- and interpatient tumor organoid heterogeneity. We found a strong correlation between organoid live cell number and volume. Linear growth rate calculations based on volume or live cell counts were used to determine differential responses to therapeutic interventions. We showed that this approach can detect different types of drug effects (cytotoxic vs cytostatic) in PDTO cultures. Overall, our imaging-based quantification workflow results in multiple parameters that can provide patient- and drug-specific information for screening applications.

Abstract:Despite having yielded extensive breakthroughs in cancer research, traditional 2D cell cultures have limitations in studying cancer progression and metastasis and screening therapeutic candidates. 3D systems can allow cells to grow, migrate, and interact with each other and the surrounding matrix, resulting in more realistic constructs. Furthermore, interactions between host tissue and developing tumors influence the susceptibility of tumors to drug treatments. Host-liver colorectal-tumor spheroids composed of primary human hepatocytes, mesenchymal stem cells (MSC) and colon carcinoma HCT116 cells were created in simulated microgravity rotating wall vessel (RWV) bioreactors. The cells were seeded on hyaluronic acid-based microcarriers, loaded with liver-specific growth factors and ECM components. Only in the presence of MSC, large tumor foci rapidly formed inside the spheroids and increased in size steadily over time, while not greatly impacting albumin secretion from hepatocytes. The presence of MSC appeared to drive self-organization and formation of a stroma-like tissue surrounding the tumor foci and hepatocytes. Exposure to a commonly used chemotherapeutic 5-FU showed a dose-dependent cytotoxicity. However, if tumor organoids were allowed to mature in the RWV, they were less sensitive to the drug treatment. These data demonstrate the potential utility of liver tumor organoids for cancer progression and drug response modeling.

Abstract:Optimal treatment of cancer requires diagnostic methods to facilitate therapy choice and prevent ineffective treatments. Direct assessment of therapy response in viable tumor specimens could fill this diagnostic gap. Therefore, we designed a microfluidic platform for assessment of patient treatment response using tumor tissue slices under precisely controlled growth conditions. The optimized Cancer-on-Chip (CoC) platform maintained viability and sustained proliferation of breast and prostate tumor slices for 7 days. No major changes in tissue morphology or gene expression patterns were observed within this time frame, suggesting that the CoC system provides a reliable and effective way to probe intrinsic chemotherapeutic sensitivity of tumors. The customized CoC platform accurately predicted cisplatin and apalutamide treatment response in breast and prostate tumor xenograft models, respectively. The culture period for breast cancer could be extended up to 14 days without major changes in tissue morphology and viability. These culture characteristics enable assessment of treatment outcomes and open possibilities for detailed mechanistic studies.Significance:The Cancer-on-Chip platform with a 6-well plate design incorporating silicon-based microfluidics can enable optimal patient-specific treatment strategies through parallel culture of multiple tumor slices and diagnostic assays using primary tumor material.
