Abstract:Purpose: To evaluate prospectively the clinical outcomes of topography-guided laser in situ keratomileusis (LASIK) surgery performed with Contoura Vision using the Wavelight excimer laser and planned with the Phorcides Analytical Engine. Setting: Four clinical practices in the U.S. Design: Prospective single-arm interventional study. Methods: 130 eyes of 65 patients with myopia or myopic astigmatism were enrolled in a prospective study of visual and refractive outcomes after treatment with Contoura Vision using the Wavelight EX500 excimer laser to achieve optimal distance vision. At 3 months postoperatively, uncorrected distance visual acuity (UDVA), manifest refraction, and corrected distance visual acuity (CDVA) were measured and compared with previously published studies. Results: At 3 months postoperatively, 100%, 89%, and 28% of eyes achieved UDVA of 20/20, 20/15, and 20/12.5 or better, respectively. 92% of eyes had postoperative UDVA equal to or better than their preoperative CDVA. Postoperative CDVA was equal to, 1 line better, or 2 lines better than the preoperative CDVA in 53%, 40%, and 6% of eyes, respectively. Only 1 eye lost 1 line of CDVA, and no eyes lost more than 1 line of CDVA. A statistically significant decrease was noted in subjective visual complaints, including glare, halos, difficulty driving at night, reading difficulty, starbursts, fluctuation in vision, and light sensitivity. Conclusions: The Phorcides Analytical Engine can be used to optimize visual outcomes for the correction of myopia and myopic astigmatism.

Abstract:PURPOSE:To compare visual acuity and higher order aberrations before and after myopic LASIK using conventional versus customized ablation. METHODS:This prospective, randomized study included 54 myopic eyes in 27 patients who underwent bilateral LASIK using the NIDEK EC-5000 excimer laser system (NIDEK Technologies, Gamagori, Japan). Customized aspheric treatment zone (CATz) treatment was used in one eye (CATz group) and conventional ablation (conventional group) was used in the fellow eye. Uncorrected visual acuity (UCVA) and higher order aberrations (root-mean-square [RMS] in 4-mm and 6-mm zones) of both groups were observed with the NIDEK OPD-Scan aberrometer before and 3 months after LASIK. Preoperative mean refractive error was similar for both eyes of each patient (P>.05). RESULTS:No statistically significant differences were noted in preoperative higher order aberrations (RMS in 4-mm and 6-mm zones) between groups (P>.05). No statistically significant difference was noted between pre- and postoperative higher order aberrations in the CATz group (P>.05). However, a highly statistically significant increase in higher order aberrations was observed after conventional ablation (P<.001). There was a statistically significant higher increase of higher order aberrations after LASIK in the conventional group than the CATz group (P<.05). Postoperative UCVA with both conventional and customized ablation was not significantly different (P>.05). CONCLUSIONS:LASIK with conventional ablation and CATz ablation resulted in the same UCVA. The increase in higher order aberrations after CATz ablation was less than after conventional ablation.

Abstract:Purpose: To verify whether there exists any difference in higher order aberrations after undergoing myopic LASIK (laser in situ keratomileusis) with conventional ablation and customized ablation in different eyes of the same patient. Methods: This was a prospective randomized study of 54 myopic eyes (27 patients) that underwent LASIK using the Nidek EC-5000 excimer laser system (Nidek, Gamagori, Japan). Topography-guided customized aspheric treatment zone (CATz) was used in the first eye of the patient (study group) and the other eye of the same patient was operated on with conventional ablation (control group). Higher order aberrations [root-mean-square (RMS) in the 5-mm zone] of both groups were observed with the Nidek OPD-Scan aberrometer before and 3 months after LASIK. Preoperative mean refractive error was similar between two eyes of the same patient (t=−0.577, P>0.05). Results: Preoperatively, higher order aberrations (RMS in the 5-mm zone) in the CATz ablation and conventional groups were (0.3600±0.0341) μm and (0.2680±0.1421) μm, respectively. This difference was not statistically significant (t=1.292, P>0.05). Three months after LASIK, higher order aberrations (RMS in 5-mm zone) in the CATz ablation and conventional groups were (0.3627±0.1510) μm and (0.3991±0.1582) μm, respectively. No statistically significant difference was noted between pre-and postoperative higher order aberrations in the CATz group (t=−0.047, P>0.05). However, a statistically significant increase in higher order aberrations was observed after conventional ablation (t=−5.261, P<0.05). A statistically significant difference was noted in the increase of higher order aberrations after LASIK between groups (t=−2.050, P=0.045). Conclusion: LASIK with conventional ablation and topography-guided CATz ablation resulted in the same BSCVA (best spectacle-corrected visual acuity) 3 month after LASIK. Higher order aberrations were increased, but the increase of higher order aberrations after customized ablation treatment was less than that after conventional ablation.

Abstract:Abstract Background To compare the outcomes of myopia and myopic astigmatism corrected with topography-modified refraction laser in situ keratomileusis (TMR-LASIK), wavefront-optimized (WFO) LASIK, and topography-guided (TG) LASIK with a correction target based on the manifest refraction (manifest TG-LASIK). Methods This observational, retrospective cohort study included patients who underwent LASIK using the WaveLight® EX500 excimer laser to correct myopia and myopic astigmatism between August 2016 and July 2017. Patients who underwent TMR-LASIK (85 patients), WFO-LASIK (70 patients), or manifest TG-LASIK (40 patients) were enrolled, and only one eye from each patient was analyzed. All participants underwent measurement of the uncorrected distance visual acuity (UDVA), best-corrected distance visual acuity (BCVA), manifest refraction, vector analysis of astigmatic change, corneal topography, and corneal wavefront analysis at baseline and at every posttreatment visit. Results Three months postoperatively, a UDVA of 0.0 logMAR or better and manifest refraction spherical equivalent (MRSE) within ±0.5 diopters (D) did not differ across the TMR-, WFO-, and manifest TG-LASIK groups. However, the residual cylinder in the TMR group was significantly larger than that in the WFO and manifest TG groups. The magnitude of error in the TMR group measured using astigmatism vector analysis was significantly higher than that in the WFO and manifest TG groups. Conclusions Although these three LASIK platforms achieved the predicted surgical outcomes, TMR-LASIK overcorrected astigmatism and showed a higher residual postoperative astigmatism compared with WFO- and manifest TG-LASIK.

Comparison of Wavefront-Optimized Ablation and Topography-Guided Contoura Ablation With LYRA Protocol in LASIK

Abstract:Purpose: To compare the refractive outcomes and aberration data analysis of wavefront-optimized (WFO) ablation and topography-guided Contoura ablation (TGCA) (Contoura on the WaveLight laser; WaveLight GmbH, Erlangen, Germany) in patients who had laser-assisted in situ keratomileusis (LASIK) for myopia or myopic astigmatism. Methods: In this comparative contralateral eye study, patients who underwent LASIK with TGCA in one eye and with WFO ablation in the fellow eye were analyzed. Aberration measurements and corneal topography were analyzed using the WaveLight Oculyzer II diagnostic device (Alcon Laboratories, Inc., Fort Worth, TX). Total corneal higher order aberrations (HOAs) including vertical and oblique astigmatism (Z2 2, Z2 -2), coma (Z3 1, Z3 -1), trefoil (Z3 3, Z3 -3), spherical aberration, and Q value were analyzed. These measurements were taken preoperatively and 3 months postoperatively. Results: This study comprised 32 patients. There were no significant differences between both procedures according to postoperative uncorrected and corrected distance visual acuity values, refractive errors, and manifest refraction spherical equivalents within ±0.50 diopters (D) of emmetropia (P > .05). The preoperative corneal HOAs and Q values were also similar between the groups (P > .05). At 3 months postoperatively, the vertical and horizontal coma values in the WFO ablation group were statistically significantly higher compared to the TGCA group (P = .013 and .020, respectively). Less stromal tissue was ablated in the TGCA group compared to the WFO ablation group (P < .001). Conclusions: Although WFO ablation and TGCA protocols had statistically similar visual outcomes, the TGCA protocol was associated with a significantly lower induction in vertical and horizontal coma and smaller amount of tissue ablation compared to WFO ablation. Patient-Reported Outcomes and Higher Order Aberrations Following Topography-Guided Femtosecond Laser-Assisted In Situ Keratomileusis

Abstract:Purpose: To examine patient-reported outcomes and higher order aberrations following topography-guided laser-assisted in situ keratomileusis (LASIK). Methods: This was a prospective, nonrandomized observational study at a single academic center. Sixty eyes from 30 patients underwent bilateral topography-guided femtosecond LASIK for correction of myopia using the VisuMax 500 femtosecond laser (Zeiss; Oberkochen, Germany) and Allegretto Wave Eye-Q 400 Hz Excimer Laser (Alcon/Wavelight; Erlangen, Germany) with the Contoura topography system (Alcon; Geneva, Switzerland) for topography-modified refraction. The main outcomes of this study were higher order aberrations (HOAs) and results from the Patient-Reported Outcomes with LASIK questionnaire. Results: There was a small, significant increase in HOA root mean square, spherical aberration, and coma at 1, 3, 6, and 12 months following topography-guided LASIK (all P < 0.05), but no change in trefoil. In addition, self-reported worry related to vision (P < 0.001) and ability to perform activities (P < 0.001) significantly improved after surgery. The prevalence of double images, glare, halos, or starbursts decreased from 73% preoperatively to 56%, and no participants reported "very" or "extremely" bothersome visual symptoms after 12 months. Dry eye symptoms per Ocular Surface Disease Index score decreased significantly at 6 (P = 0.01) and 12 (P = 0.002) months after surgery. There was a 100% satisfaction rate with visual outcomes and duration of time to improvement in vision following the procedure. Conclusions: Although there was an increase in HOAs following topography-guided LASIK, there were significant improvements in the presence of double images, glare, halos, and starbursts and vision-related quality-of-life metrics. Overall satisfaction rates in this study were high.
Comparison of Corneal Topography after Topography-Guided FS-LASIK and Wavefront-Optimized FS-LASIK

Abstract:Objective To compare the outcomes of topography-guided LASIK and wavefront-optimized LASIK in eyes of patients undergoing femtosecond laser LASIK (FS-LASIK) for myopia with astigmatism.Methods In this prospective clinical study,44 eyes of 22 patients underwent topography-guided LASIK (topography group) and 43 eyes of 22 patients underwent wavefront-optimized LASIK (wavefront group).The topography treatment was based on data generated by a networked diagnostic device:Wavelight Topolyzer.Visual acuity,refractive error,the index of corneal surface variance (ISV),corneal wavefront aberrations and ablation decentration were evaluated 6 months postoperatively.Data were analyzed using independent t test.Results Six months postoperatively,there were no significant differences in UCVA or BCVA (t=-1.553,-1.570,P＞0.05) between the groups.No eyes lost a line of BCVA in either group.There were no statistically significant differences in postoperative average sphere,cylinder refraction or spherical equivalent (SE) between the groups.The postoperative value of ISV increased compared to baseline for both groups (t=5.216,7.870,P＜0.05).The mean postoperative ISV in the topography group (39.9±13.0) was significantly lower than that in the wavefront group (44.9±12.2) (t=-2.296,P＜0.05).The postoperative mean decentration in the topography group (0.24± 0.29 mm) was less than that in the wavefront group (0.29±±0.31 mm),but the difference was not statistically significant (t=-1.005,P＞0.05).There were significant differences in postoperative coma aberrations (Z31) [-0.166 ±0.357 m vs.-0.362±±0.353 m,t=2.325,P＜0.05] and spherical aberrations (Z40) [0.390± 0.263 μm vs.0.486±±0.164 μm,t=-2.096,P＜0.05] between the two groups.Conclusion Both topography-guided ablation and wavefront-optimized ablation provide good visual acuity and refractive results,but the former treatment results in a better index of corneal surface variance and induces fewer corneal aberrations.

Abstract:There are two morphologically distinct types of glial cells (i.e., ensheathing cells and astrocytes) in the nerve fiber layer (NFL) of the adult mammalian olfactory bulb. Ensheathing cells provide ensheathment for olfactory axons, whereas astrocytes occupy the interfascicular spaces of the olfactory NFL. During embryonic development, however, only one type of glial cell is found in this layer of the olfactory bulb, namely, the ensheathing cell. Even though ensheathing cells take up residence within the CNS, they are actually derived from the olfactory placode. Far less is known about the developmental origin of interfascicular astrocytes, which arise either from the glial progenitor cells that give rise to ensheathing cells or from astrocyte precursor cells that migrate into the NFL from deeper layers of the bulb primordium. In the present study, enriched populations of ensheathing cells were grown in vitro in media known to promote the growth and differentiation of astrocytes to determine whether ensheathing cell progenitors could differentiate into astrocytes. These media failed to induce the appearance of astrocytes in the ensheathing cell cultures. It was concluded that the astrocytes of the NFL most likely arise from progenitor cells that migrate into this layer from deeper parts of the developing bulb. © 1993 Wiley‐Liss, Inc.

Abstract:PURPOSE:To compare clinical outcomes between topography-guided customized ablation treatment (TCAT) and wavefront-optimized (WFO) laser in situ keratomileusis (LASIK) in fellow eyes of myopic patients. METHODS:This prospective randomized contralateral study included 432 eyes of 216 myopic patients who underwent LASIK. TCAT was randomly performed in one eye (TCAT group) and WFO LASIK in the fellow eye (WFO group). The WaveLight FS200 femtosecond laser (Alcon Laboratories, Inc., Fort Worth, TX) was used to create the flap and the EX500 excimer laser was used for photoablation. The Sirius combined corneal topographer and tomographer system (CSO, Florence, Italy) was used to measure the corneal aberrations. Refractive and visual outcomes and corneal aberrations were compared between the two groups. RESULTS:At 1 month postoperatively, the uncorrected distance visual acuity (UDVA) was 20/20 or better in 89.4% of eyes in the TCAT group and 93.5% of eyes in the WFO group (P < .05). The UDVA became similar at postoperative 6 months (P > .05). Postoperative corrected distance visual acuity and residual manifest refractive spherical equivalent were similar between the groups (P > .05). The postoperative residual refractive astigmatism in the TCAT group was greater than that in the WFO group (P < .05). The optical path difference and root mean square of higher order aberrations and coma were significantly lower in eyes in the TCAT group at postoperative 1 and 6 months (P < .05). CONCLUSIONS:TCAT in virgin eyes induced less corneal optical path difference, fewer higher order aberrations, and less coma than WFO LASIK. However, TCAT was not as accurate as WFO, especially in astigmatism correction. A better compensation method for TCAT is warranted. [J Refract Surg. 2019;35(9):575-582.].

Abstract:Purpose: To compare outcomes in wavefront-guided LASIK performed with iris recognition software versus Without iris recognition software in different eyes Of the same patient. Methods: A randomised, prospective study of 104 myopic eyes of 52 patients undergoing LASIK surgery with the MEL80 excimer laser system was performed. Iris recognition software was used in one eye of each patient (Study group) and not used in the Other eye (Control group). Higher order aberrations (HOAs), contrast sensitivity, uncorrected vision (UCV), visual acuity (VA) and corneal topography were measured and recorded pre-operatively and at one month and three months post-operatively for each eye. Results: The mean post-operative sphere and cylinder between groups was similar, however the post-operative angles of error (AE) by refraction were significantly smaller in the study group compared to the control group both in arithmetic and absolute means (p = 0,03, p = 0.01). The mean logMAR UCV was significantly better in the study group than in the control group at one month (p = 0.01). The mean logMAR VA was significantly better in the Study group than in control group at both one and three months (p = 0.01, p = 0.03). In addition, mean trefoil, total third-order aberration, total fourth-order aberration and the total scotopic root-mean-square (RMS) HOAs were significantly less in the Study group than those in the control group at the third (p = 0.01, p = 0.05, p = 0.04, p = 0.02). By three months, the contrast: sensitivity had recovered in both groups but the Study group performed better at 2.6, 4.2 and 6.6 cpd (cycles per degree) than the control group (p = 0.01, p < 0.01, p = 0.01). Conclusions: LASIK performed with iris recognition results in better VA, lower mean higher-order aberrations, lower refractive post-operative angles of error and better contrast sensitivity at three months post-operatively than LASIK performed without iris recognition.
Clinical results of topography-guided laser-assisted in situ keratomileusis using the anterior corneal astigmatism axis and manifest refractive astigmatism axis

Abstract:Purpose: To evaluate the clinical results of primary topography-guided femtosecond laser-assisted in situ keratomileusis (TG-FS-LASIK) using the Pentacam-measured anterior corneal astigmatism axis (ACA) or manifest refractive astigmatism axis (MRA). Methods: In this prospective cohort study, all eyes were treated with primary TG-FS-LASIK using the manifest cylinder. Thirty-two right eyes were treated using ACA with axis disparity > 5° in the experimental group, and 32 right eyes were treated using MRA with axis disparity ≤ 5° in the control group. Visual, refractive outcomes, and corneal higher-order aberrations were evaluated. Vector analysis of astigmatism was performed using Alpins method with the ASSORT software. Results: The mean logMAR UDVA in the experimental group was - 0.12 ± 0.06, - 0.05 ± 0.08, and - 0.08 ± 0.08 at 1-week, 1-month, and 3-month follow-up, whereas - 0.05 ± 0.06, - 0.12 ± 0.06, and - 0.14 ± 0.06 in the control group (p = 0.017, p < 0.001, and p = 0.003). At 3-month follow-up, 79% eyes achieved a UDVA of 20/16 or better, 9% gained one line of corrected distance visual acuity, the mean manifest cylinder was - 0.375 ± 0.254 D, 84% showed a manifest cylinder within ± 0.50 D, the mean magnitude of difference vector was 0.41 ± 0.35 D, the mean absolute angle of error (AE) was 7.36 ± 6.21°, and 41% exhibited an arithmetic AE within ± 5° in the experimental group, whereas 94%, 19%, - 0.203 ± 0.148 D, 100%, 0.21 ± 0.15 D, 3.39 ± 3.68°, and 69% in the control group (all p < 0.05). The postoperative and preoperative corneal spherical aberrations were comparable in both groups (p > 0.05). Conclusion: Primary topography-guided FS-LASIK using Pentacam-measured anterior corneal astigmatism axis exhibited slightly inferior clinical results to that using the manifest refractive astigmatism axis. Both methods achieved comparable postoperative and preoperative corneal spherical aberrations.

Abstract:Background: To analyze the clinical outcomes after topography-guided femtosecond laser-assisted laser in situ keratomileusis (FS-LASIK) with Phorcides Analytic Engine (PAE) algorithm or Custom-Q FS-LASIK for myopia with nonastigmatic eyes. Methods: In this retrospective study, a total of 90 eyes with myopia without manifest astigmatism (82 patients) were included. All surgeries were performed by topography-guided FS-LASIK planned with a PAE algorithm (42 eyes) or Custom-Q system (48 eyes). Refractive, visual outcomes and corneal aberrations were compared between the two groups. Results: At 6 months postoperatively, the postoperative uncorrected distance visual acuity (UDVA) was 20/20 or better in 42 eyes (100%) in the PAE compared with 44 eyes (92%) in Custom-Q (P = .120). The postoperative UDVA of 20/16 or better was measured in 92% of eyes in the PAE group and 81% of eyes in the Custom Q group (P = .320). Postoperative corrected distance visual acuity, manifest refractive spherical equivalent and refractive astigmatism were similar between the two groups (P > .05). The postoperative optical path difference (OPD) and Strehl ratio (SR) were significantly better in the PAE group compared with the Custom Q group. Conclusions: Topography-guided FS-LASIK with PAE algorithm or Custom Q demonstrated similar refractive efficacy and predictability. PAE for the patients with zero manifest astigmatism demonstrated better results in correcting corneal aberrations.

Abstract:Purpose: The purpose of this retrospective study was to evaluate and analyze visual outcomes by recording pre and postoperative trefoil, coma and refractive astigmatism in wavefront optimized myopic LASIK. Methods: In this retrospective case review 200 eyes (one hundred patients) that had undergone myopic (with corresponding astigmatism) wavefront-optimized LASIK using the FS200 femtosecond and EX500 excimer lasers (Alcon/Wavelight, Erlagen, Germany) were evaluated. The 12 months post-operative UDVA and CDVA, low (myopia and/or astigmatism) along with high order aberration C6 to C9 changes were compared to the pre-operative values. Pre-operative topography data were available and used to generate for this study hypothetical treatment data (low and high order aberrations) if Topography-Guided (TG) with TMR cylinder amount and axis adjustment was used instead of the actual WFO. Results: Mean values at 12 months: UDVA of 20/22 and CDVA of 20/20. The postoperative refractive error in Diopters was -0.20±0.46 sphere and – 0.45±0.27 cylinder. The average absolute value for the high order aberrations studied were pre-op: C6: 0.10±0.12, C7: 0.19±0.16, C8: 0.15±0.12, C9: 0.09±0.09 μm and respectively post-op, C6: 0.11±0.10, C7: 0.46±0.38, C8: 0.34±0.30, C9: 0.11±0.13 μm. If topography-guided customization with TMR was originally employed an addition mean -0.36D of astigmatism would have been attempted. Conclusion: Wavefront optimized ablations do not address HOA, pre-existing trefoil (C6, C9) in this group essentially did not change while coma (C7 and C8) increased despite the essential achievement of emmetropia. In theory topography-guided customization with TMR may had offered improved C7 and -C8 outcomes, along with superior cylindrical correction.
12-Month Visual and Refractive Outcomes of Topography-guided Femtosecond Laser-Assisted LASIK for Myopia and Myopic Astigmatism

Abstract:Purpose: To report 12-month visual and refractive outcomes following topography-guided femtosecond laser-assisted laser in situ keratomileusis (LASIK) for myopia and compound myopic astigmatism correction. Methods: This prospective, single-center observational study was conducted in an outpatient clinical practice at the Stanford University Byers Eye Institute in Palo Alto, California. Uncorrected (UDVA) and corrected (CDVA) distance visual acuity, 5% and 25% contrast sensitivity CDVA, and manifest refraction following topography-guided femtosecond laser-assisted LASIK were assessed. Refractive measurements were used to perform a vector analysis. Results: Sixty eyes of 30 patients (mean age: 32.8 ± 7.0 years; range: 23 to 52 years) undergoing topography-guided LASIK for the correction of myopia and compound myopic astigmatism were analyzed. Mean postoperative UDVA was -0.09 ± 0.10 logarithm of the minimum angle of resolution (logMAR) at 12 months. Mean preoperative CDVA was -0.09 ± 0.09 and -0.13 ± 0.08 logMAR at postoperative 12 months. At 12 months, 26.9% of eyes had gained one or more lines of postoperative UDVA compared to baseline CDVA. Mean pre-operative 5% contrast sensitivity CDVA was 0.68 ± 0.07 and 0.64 ± 0.12 logMAR at 12 months (P = .014) following LASIK. Conclusions: Topography-guided LASIK for myopia and myopic astigmatism correction provided excellent visual and refractive outcomes that were predictable, precise, and stable up to 12 months postoperatively. Retrospective Analysis of the Post-Operative Changes in Higher-Order Aberrations: A Comparison of the WaveLight EX500 to the VISX S4 Laser in Refractive Surgery

Abstract:Introduction: In an attempt to maximize treatment outcomes, refractive surgery techniques are being directed toward customized ablations to correct not only lower-order aberrations but also higher-order aberrations specific to the individual eye. Measurement of the entirety of ocular aberrations is the most definitive means to establish the true effect of refractive surgery on image quality and visual performance. Whether or not there is a statistically significant difference in induced higher-order corneal aberrations between the VISX Star S4 (Abbott Medical Optics, Santa Ana, California) and the WaveLight EX500 (Alcon, Fort Worth, Texas) lasers was examined. Methods: A retrospective analysis was performed to investigate the difference in root-mean-square (RMS) value of the higher-order corneal aberrations postoperatively between two currently available laser platforms, the VISX Star S4 and the WaveLight EX500 lasers. The RMS is a compilation of higher-order corneal aberrations. Data from 240 total eyes of active duty military or Department of Defense beneficiaries who completed photorefractive keratectomy (PRK) or laser in situ keratomileusis (LASIK) refractive surgery at the Wilford Hall Ambulatory Surgical Center Joint Warfighter Refractive Surgery Center were examined. Using SPSS statistics software (IBM Corp., Armonk, New York), the mean changes in RMS values between the two lasers and refractive surgery procedures were determined. A Student t test was performed to compare the RMS of the higher-order aberrations of the subjects' corneas from the lasers being studied. A regression analysis was performed to adjust for preoperative spherical equivalent. The study and a waiver of informed consent have been approved by the Clinical Research Division of the 59th Medical Wing Institutional Review Board (Protocol Number: 20150093H). Results: The mean change in RMS value for PRK using the VISX laser was 0.00122, with a standard deviation of 0.02583. The mean change in RMS value for PRK using the WaveLight EX500 laser was 0.004323, with a standard deviation of 0.02916. The mean change in RMS value for LASIK using the VISX laser was 0.00841, with a standard deviation of 0.03011. The mean change in RMS value for LASIK using the WaveLight EX500 laser was 0.0174, with a standard deviation of 0.02417. When comparing the two lasers for PRK and LASIK procedures, the p values were 0.431 and 0.295, respectively. Conclusion: The results of this study suggest no statistically significant difference concerning induced higher-order aberrations between the two laser platforms for either LASIK or PRK. Overall, the VISX laser did have consistently lower induced higher-order aberrations postoperatively, but this did not reach statistical significance. It is likely the statistical significance of this study was hindered by the power, given the relatively small sample size. Additional limitations of the study include its design, being a retrospective analysis, and the generalizability of the study, as the Department of Defense population may be significantly different from the typical refractive surgery population in terms of overall health and preoperative refractive error. Further investigation of visual outcomes between the two laser platforms should be investigated before determining superiority in terms of visual image and quality postoperatively.
Analysis of correlation between corneal topographical data and visual performance

Abstract:Purpose: To study correlation among corneal asphericity, higher-order aberrations and visual performance for eyes of virgin myopia and postoperative laser in situ keratomileusis (LASIK). Methods: There were 320 candidates 590 eyes for LASlK treatment included in this study. The mean preoperative spherical equivalence was -4.35 +/- 1.51 D (-1.25 to -9.75), with astigmatism less than 2.5 D. Corneal topography maps and contrast sensitivity were measured and analyzed for every eye before and one year after LASIK for the analysis of corneal asphericity and wavefront aberrations. Results: Preoperatively, only 4th and 6th order aberration had significant correlation with corneal asphericity and apical radius of curvature (p < 0.001). Postoperatively, all 3th to 6th order aberrations had statistically significant correlation with corneal asphericity (p < 0.01), but only 4th and 6th order aberration had significant correlation with apical radius of curvature (p < 0.05). The asymmetrical aberration like coma had significant correlation with vertical offset of pupil center (p < 0.01). Preoperatively, corneal aberrations had no significant correlation with visual acuity and area under the log contrast sensitivity (AULCSF) (P > 0.05). Postoperatively, corneal aberrations still didn't have significant correlation with visual acuity (P > 0.05), but had significantly negative correlation with AULCSF (P < 0.01). Corneal asphericity had no significant correlation with AULCSF before and after the treatment (P > 0.05). Conclusions: Corneal aberrations had different correlation with corneal profile and visual performance for eyes of virgin myopia and postoperative LASIK, which may be due to changed corneal profile and limitation of metrics of corneal aberrations.

Abstract:INTRODUCTION: Refractive surgery procedures, transepithelial photorefractive keratectomy (T-PRK) and femtosecond laser in situ keratomileusis (Fs-LASIK) are regarded as safe and efficacious methods for correcting myopia and myopic astigmatism. These two methods do not have much differences in results when treating spherical myopia, while some differences does exist in treatment of myopic astigmatism. Vector analysis presents powerful tool to show the real differences between these two methods regarding higher order ocular aberrations and central corneal thickness of treated eyes.AIM: The aim of the study is to investigate changes in higher order ocular aberrations (HOAs) and central corneal thickness (CCT) following treatment of myopia and myopic astigmatism above -5.00DS and up to -2.00DC after either T-PRK or Fs-LASIK.METHODS: Patients (30 eyes per group) underwent T-PRK (group I) or Fs-LASIK (group II) procedure using Schwind Amaris 750S laser. HOAs (3mm&5mm pupil) and CCT were measured objectively at pre-, 1,3 & 6 months postop in each case.RESULTS: Key results at 6 months were: i) mean values of trefoil (5mm pupil) were 0.092μm (sd,0.055,95% CI 0.072 to 0.112) & 0.126μm (sd,0.078,95% CI 0.098 to 0.154) in group I, and 0.088μm (sd,0.058,95% CI 0.067 to 0.109) & 0.064μm (sd,0.034,95% CI 0.052 to 0.076) in group II (P=0.001 at 6 months); ii) Changes in CCT (ΔCCT) and best spherical equivalent correction (ΔBSE) was significant in group II (ΔCCT=-26.55[ΔBSE]-14.06,R=0.486,P=0.006) but not in group I (p=0.034).CONCLUSIONS: After T-PRK trefoil is worse than Fs-LASIK. The predictability of corneal changes is better following Fs-LASIK. .
