Abstract: Objective To investigate the clinical characteristics of nodal marginal zone lymphoma (NMZL). Methods Data of 14 NMZL patients diagnosed by lymph nodes histological examination were retrospectively analyzed. Results Among the 14 patients, 13(92.9%) had abnormal complete blood counts(CBC). Leukocytosis [white blood cells(WBC)≥10.0×109/L] was observed in 6 patients (42.9%); leukocytopenia (WBC<4.0×109/L) in 4 patients(28.6%); absolute lymphocyte counts>5.0×109/L in 7 patients (50%);hemoglobin concentration<120g/L in 10 patients(71.4%), thrombocytopenia(platelet<100×109/L) in 6 patients(42.9%); cytopenia in more than 2 lineages in 4 patients (28.6%);pancytopenia in 2 cases(14.3%). Ten patients(71.4%) had bone marrow involvement. Among them, monoclonal B cells with a light chain restriction was found in 5 patients (50%). Percentage of lymphocytes in peripheral blood correlated positively with percentage of NMZL cells in bone marrow(r=0.811,P=0.008). Hyperimmunoglobulinemia was found in all patients (12/12, 100%). Among them, serum monoclonal paraprotein was found in 10 patients(10/12, 83.3%). At least one auto-antibody positivity was found in 8 out of 11 patients (72.7%). All of 14 patients had Ann Arbor stage Ⅲ or Ⅳ. Immunochemotherapy was administered in 10 patients. Rituximab was given as a single agent to 1 patient. Chemotherapy was given to 2 patients, and interferon alpha was given to 1 patient. Complete response was achieved in 8 patients (57.1%), and partial response in 3 patients (21.4%). Median duration of follow-up was 23 months. Accumulated 2-year overall survival (OS) rate and 2-year progression-free survival (PFS) rate were 84.6% and 71.4%, respectively. Estimated median OS and PFS were 90 and 39 months, respectively. Conclusion NMZL patients at stage Ⅲ/Ⅳ have high a incidence of CBC abnormality, bone marrow involvement and autoimmunologic abnormality. Peripheral blood lymphocytosis might indicate NMZL bone marrow involvement.

Abstract:OBJECTIVE:To investigate the clinicopathologic features of primary nodal marginal zone B-cell lymphoma (NMZL).METHODS:Hematoxylin-Eosin staining and immunohistochemistry were used to evaluate the histological and immunophenotypic characteristics of lymph node (LN) tissue in 22 NMZL cases. Additionally, interphase fluorescence in-situ hybridization (FISH) was carried out to detect the presence of t(11;18) (q21;q21)/API2-MALT1 and/or t(14;18)(q32;q21)/IGH-MALT1 in 9 cases.RESULTS:The median age of the 22 patients was 62 (16 - 77) ys. The male-to-female ratio was 1.2:1. All patients exhibited asymptomatic lymphadenopathy with the cervical region as the most often site to be involved (n = 11), followed by axillary (n = 9), inguinal (n = 7), submandibular (n = 6), mediastinal (n = 4), supraclavicular (n = 2) and retroperitoneal lymph nodes (n = 1). The Ann Arbor stages were I/II in 13 (59%) cases and III/IV in 9 (41%). Immunohistochemical study showed a consistently strong expression of CD20 and an absence in the expression of CD3ε, CD10, CD21, CD23, CyclinD1 and BCL6 by the tumor cells in all the cases. Frequency of expression of CD5 and BCL2 were 39% (7/18) and 30% (3/14) respectively. Among the 9 cases performed with FISH, 2 cases harbored t(14;18)and another 1 case positive for t(11;18) and t(14;18). Complete follow-up data were available for 13 cases. The follow-up time was 6 to 44 months. 3 of them died. 3-year cumulative survival rate was 67%.CONCLUSIONS:NMZL patients are often elderly, which mainly present with multiple lymphadenopathy, rare involvement of extranodal organ and early stage. The diagnosis must be based on a combination of clinicopathologic features, especially those patients detected t(11;18) and/or t(14;18).

Abstract:Purpose: Marginal zone lymphoma (MZL) is an uncommon non-Hodgkin lymphoma with malignant cells that exhibit a consistent dependency on B-cell receptor signaling. We evaluated the efficacy and safety of zanubrutinib, a next-generation selective Bruton tyrosine kinase inhibitor, in patients with relapsed/ refractory (R/R) MZL. Patients and Methods: Patients with R/R MZL were enrolled in the phase II MAGNOLIA (BGB-3111-214) study. The primary endpoint was overall response rate (ORR) as determined by an independent review committee (IRC) based on the Lugano 2014 classification. Results: Sixty-eight patients were enrolled. After a median follow-up of 15.7 months (range, 1.6 to 21.9 months), the IRC-assessed ORR was 68.2% and complete response (CR) was 25.8%. The ORR by investigator assessment was 74.2%, and the CR rate was 25.8%. The median duration of response (DOR) and median progression-free survival (PFS) by independent review was not reached. The IRC-assessed DOR rate at 12 months was 93.0%, and IRC-assessed PFS rate was 82.5% at both 12 and 15 months. Treatment was well tolerated with the majority of adverse events (AE) being grade 1 or 2. The most common AEs were diarrhea (22.1%), contusion (20.6%), and constipation (14.7%). Atrial fibrillation/flutter was reported in 2 patients; 1 patient had grade 3 hypertension. No patient experienced major hemorrhage. In total, 4 patients discontinued treatment due to AEs, none of which were considered treatment-related by the investigators. Conclusions: Zanubrutinib demonstrated high ORR and CR rate with durable disease control and a favorable safety profile in patients with R/R MZL.

Abstract:Topic: 18. Indolent and mantle-cell non-Hodgkin lymphoma - Clinical Background: Zanubrutinib (BGB-3111) is a potent next-generation Bruton tyrosine kinase inhibitor approved in various countries for the treatment of R/R MZL based on the primary analysis results of the MAGNOLIA study (NCT03846427). Aims: To present the final analysis of MAGNOLIA at a median follow-up of 28 months (mo). Methods: MAGNOLIA is a phase 2, multicenter, single-arm study of adult pts requiring systemic treatment for R/R MZL with ≥1 prior line of therapy including ≥1 CD20-directed regimen. All pts received zanubrutinib 160 mg twice daily until disease progression or unacceptable toxicity. Primary endpoint was overall response rate (ORR) by independent review committee (IRC) in accordance with the Lugano classification. Secondary endpoints included ORR by investigator assessment, duration of response (DOR), progression-free survival (PFS), overall survival (OS), and safety. Efficacy was assessed by positron emission tomography (PET)-based Lugano criteria for pts with IRC-confirmed fluorodeoxyglucose (FDG)-avid disease at baseline; non-avid pts were assessed by computed tomography (CT)-based criteria. A sensitivity analysis using only CT-based criteria was also performed. Results: As of May 4, 2022, 68 pts were enrolled and treated. Median age was 70 years (range 37-95). MZL subtypes included extranodal (mucosa-associated lymphoid tissue) in 38.2%, nodal in 38.2%, splenic in 17.6%, and unknown in 5.9% of pts. Most (89.7%) pts received prior chemoimmunotherapy. Sixty-one (89.7%) pts had IRC-assessed FDG-avid disease. After a median follow-up of 28.0 mo (range 1.6-32.9) and a median treatment duration of 24.2 mo (range 0.9-32.9), 66 pts were evaluable for efficacy (Table). IRC-assessed ORR (complete response [CR] + partial response [PR]) was 68.2% (CR 25.8%). ORR was 64.0%, 76.0%, 66.7%, and 50.0% in extranodal, nodal, splenic, and unknown subtypes, respectively. CR rate was 40.0% in extranodal, 20.0% in nodal, 8.3% in splenic, and 25.0% in unknown subtypes. Median DOR, PFS, and OS were not reached. At the 2-year landmark by independent review, >70.0% of pts were alive or progression-free. Sensitivity analysis using only CT-based criteria (n=66) by IRC assessment showed an ORR of 66.7% (CR 24.2%). Median DOR and median PFS were not reached. At study completion, 31 (45.6%) pts deriving benefit rolled over to a long-term extension (LTE) study (NCT04170283); 24 (35.3%) pts discontinued owing to disease progression (investigator assessed); 5 (7.4%) due to adverse events (AEs), 2 (2.9%) required prohibited medications, and 1 (1.5%) withdrew consent. Most common treatment-emergent AEs in >20% of pts were bruising (23.5%), and diarrhea (22.1%). Neutropenia (8.8%) and COVID-19 pneumonia (5.9%) were the most common grade ≥3 AEs. Five (7.4%) pts died due to unrelated AEs: COVID-19 pneumonia (n=2), acute myeloid leukemia (n=1, prior alkylating agent exposure), myocardial infarction (n=1, preexisting coronary artery disease), and septic encephalopathy (n=1, pt in CR). Hypertension occurred in 3 (4.4%) pts, atrial fibrillation and atrial flutter in 1 (1.5%) pt each; none led to treatment withdrawal. One (1.5%) pt experienced grade 3 gastrointestinal hemorrhage while receiving rivaroxaban for pulmonary embolism; pt fully recovered and rolled over to the LTE study. Abstract:When this topic, nonspecific resistance to infectious disease, first came under the author's consideration it seemed quite possible to group many factors affecting disease resistance into the nonspecific category. At first thought, penicillin, botulinus toxin, and climatic differences might be considered such agents. Penicillin affects several species of bacteria in several hosts. Botulinus toxin sickens several hosts. Climate seemingly changes many diseases. Further thought showed that the problem was much more complicated if the actual operation of any of these agents was traced to the mechanism through which it acted in causing resistance or susceptibility. It was true that the action of each of these agents spread over a number of species but in these species the vulnerable point was generally found in some common physicochemical reaction extending throughout the group. This common method of action would be expected if consideration is given to how the disease process could arise. In evolutionary times differentiation took place, making different members of the then living group capable of fitting into different climatic or other conditions. The initiation of these differentiations probably took origin in specific mutations, which control specific mechanisms in the developmental pattern not only of the individual with the inheritance but also in progeny to come. As a result, rivers of like physicochemical reactions flowed out to what ultimately became different species. Similarly pathogens developed and expanded through the incorporation of some change in their organization which gave them the ability to invade a specific host. Again the process was comparable to gene mutation. Ever-present selection refined the processes by which the organism became pathogenic on those species which had the specific characteristic to interact specifically with the newly acquired property.

Rare lymphomas in routine practice - Treatment and outcome in marginal zone lymphoma in the prospective German Tumour Registry Lymphatic Neoplasms

Abstract:Owing to its heterogeneity and rarity, management of disseminated marginal zone B-cell lymphoma (MZL) remains largely understudied. We present prospective data on choice of systemic treatment and survival of patients with MZL treated in German routine practice. Of 175 patients with MZL who had been documented in the prospective clinical cohort study Tumour Registry Lymphatic Neoplasms (NCT00889798) collecting data on systemic treatment, 58 were classified as extranodal MZL of mucosa-associated lymphoid tissue (MALT) and 117 as non-MALT MZL. We analyzed the most commonly used first-line and second-line chemo(immuno)therapies between 2009 and 2016 and examined objective response rate (ORR), progression-free survival (PFS), overall survival (OS) and prognostic factors for survival. Compared to patients with MALT MZL, those with non-MALT MZL more often presented with bone marrow involvement (43% vs. 14%), Ann Arbor stage III/IV (72% vs. 57%) and were slightly less often in good general condition (ECOG = 0; 41% vs. 47%). In German routine practice, rituximab-bendamustine for a median of 6 cycles was the most frequently used first-line (76%) and second-line treatment (36%), with no major differences between MZL subtypes. The ORR for patients encompassing any positive response was 81%. For patients with MALT and non-MALT MZL, respectively, 5-years PFS was 69% (95% CI 52%-81%) and 66% (95% CI 56%-75%), 5-years OS 79% (95% CI 65%-89%) and 75% (95% CI 66%-83%). Cox proportional hazards models showed a significantly increased risk of mortality for higher age in all patient groups. Our prospective real world data give valuable insights into the management and outcome of non-selected patients with MZL requiring systemic treatment and can help optimize therapy recommendations.

Abstract:Microencapsulation of garlic oil was performed via complex coacervation using type A or type B gelatin and gum acacia as wall materials. Effect of pH on garlic oil encapsulation yield and loading efficiency was investigated. Results showed that the highest garlic oil encapsulation yield and loading efficiency for type A or B gelatin and gum acacia occurred at pH 4.5 and 3.5, respectively. Scanning electron microscopy (SEM) observation showed that coacervates made from type A or B gelatin-gum acacia were spherical with smooth topography. Both cross-linked type A and B gelatin-gum acacia exhibited controlled release of garlic oil during five hours of incubation in pH 2 pepsin solution. Coacervates produced from type A or B gelatin-gum acacia protected garlic oil from primary and secondary oxidation during 12 days storage at 45°C.

Abstract:Nodal marginal zone lymphoma (NMZL) is a B-cell lymphoma that shares morphologic and immunophenotypic features with extranodal and splenic marginal zone lymphomas but lacks extranodal or splenic involvement at presentation. NMZL occurs mostly in adults with no sex predilection, at advanced stage (III or IV), with frequent relapses and a high incidence of tumoral genetic abnormalities including trisomies 3 and 18 and gain of 7q. Pediatric NMZL, however, is a rare but distinct variant of NMZL with characteristic features including male predominance, asymptomatic and localized (stage I) disease, low relapse rates with excellent outcomes, and a lower incidence of essentially similar genetic aberrations compared to adult NMZL. Here we describe a unique case of childhood NMZL with unusual clinicopathologic features for the pediatric variant including generalized lymphadenopathy, high-stage disease with persistence after therapy, unusual immunophenotype (CD5, CD23, and BCL6 positive), and unique chromosomal abnormalities including monosomy 20 and add(10)(p11.2).

Abstract:Abstract Splenic marginal zone B-cell lymphoma (SMZL) is a heterogeneous clinico-biological entity. The clinical course is variable, multiple genes are mutated with no unifying mechanism, and essential regulatory pathways and surrounding microenvironments are diverse. We sought to clarify the heterogeneity of SMZL by resolving different subgroups and their underlying genomic abnormalities, pathway signatures, and microenvironment compositions to uncover biomarkers and therapeutic vulnerabilities. We studied 303 SMZL spleen samples collected through the IELSG46 multicenter international study (NCT02945319) by using a multiplatform approach. We carried out genetic and phenotypic analyses, defined self-organized signatures, validated the findings in independent primary tumor metadata and determined correlations with outcome data. We identified 2 prominent genetic clusters in SMZL, termed NNK (58% of cases, harboring NF-κB, NOTCH, and KLF2 modules) and DMT (32% of cases, with DNA-damage response, MAPK, and TLR modules). Genetic aberrations in multiple genes as well as cytogenetic and immunogenetic features distinguished NNK- from DMT-SMZLs. These genetic clusters not only have distinct underpinning biology, as judged by differences in gene-expression signatures, but also different outcomes, with inferior survival in NNK-SMZLs. Digital cytometry and in situ profiling segregated 2 basic types of SMZL immune microenvironments termed immune-suppressive SMZL (50% of cases, associated with inflammatory cells and immune checkpoint activation) and immune-silent SMZL (50% of cases, associated with an immune-excluded phenotype) with distinct mutational and clinical connotations. In summary, we propose a nosology of SMZL that can implement its classification and also aid in the development of rationally targeted treatments.

Abstract:Introduction Marginal Zone Lymphoma (MZL) includes three subtypes of indolent lymphoma: splenic MZL, extranodal MZL of mucosa-associated lymphoid tissue (MALT) and nodal MZL. The diagnosis of MZL is often made by exclusion of other lymphoma subtypes based on phenotype. One of the markers that are often involved in this discernment is CD5. The CD5 is not expressed in most of the cases of MZL, probably independently of the subtype, while it is usually expressed in different diseases such as Mantle Cell Lymphoma and Small Lymphocytic Lymphoma/Chronic Lymphocytic Leukemia. A subgroup of MZL, however, unequivocally expresses CD5, and very little is known about the characteristics and outcomes of these patients compared to their CD5 negative counterparts. Methods From our pathology database, we collected all the reports of indolent lymphoma containing the words "marginal zone" and/or "MALT" diagnosed or reviewed at MSKCC. We reviewed the text of the selected pathology reports to identify all cases that were unequivocally MZL and reported positivity or negativity of the CD5 marker in the lymphoma cells. From the electronic medical records, we collected all the relevant clinical information for all CD5+ MZL cases, and for a subset of CD5- MZL cases with a 1:2 match for age at diagnosis and sex. The survival and baseline characteristics analyses only included patients that were followed by our lymphoma group. We report differences between groups with the chi-squared test. Overall survival (OS) was estimated using the cumulative incidence method, and time to treatment (TTT) was estimated using the subdistribution function to adjust for semi-competing risk of death on treatment initiation. A modified proportional hazards model was used to compare the subdistribution hazard between groups, and a standard Cox proportional hazards model was used to compare the hazard of death between groups. Results From 03/1998 to 09/2019, 64 patients were diagnosed with CD5+ MZL and followed by the Lymphoma Service at MSKCC. The control group of CD5- MZL included 137 patients that matched the CD5+ positive patients for age at diagnosis and sex. 20.3% of the CD5+ vs 14.6% of the CD5- MZL were nodal (p=0.30); Extra-nodal (EN) localizations included: GI tract - other than stomach (5.9% CD5+, 9.4% CD5-), stomach (17.7% CD5+, 17.1% CD5-), skin (7.8% CD5+, 12.0% CD5-), lung (9.8% CD5+, 12.0% CD5-), eye (9.8% CD5+, 7.7% CD5-), spleen (25.5% CD5+, 18% CD5-), multiple sites (13.7% CD5+, 10.3% CD5-), other single EN sites (9.8% CD5+, 13.7% CD5-). IPI score was low in 52% CD5+ vs 61% CD5-; low intermediate in 28% CD5+ vs 19% CD5-; high intermediate in 21% CD5+ vs 13% CD5-; no CD5+ had high IPI, while 7% of CD5- did (chi-square p=0.04). Transformation to DLBCL was similar in the 2 groups with 7% events in CD5+ and 4% in CD5- (p=0.42). Bone marrow involvement seemed to be more prevalent in the CD5+ MZL (67.5% vs 47.2% of the CD5-; p=0.04). IGHV was mutated in 80% of the CD5+ and 64% of the CD5- (p=0.24). OS analysis was performed first considering death by any cause: median OS was not reached for CD5+ MZL (95% C.I. 8.8 years - NE) and it was 27 years for CD5- (95% C.I. 14.3 years - NE). No difference was observed between CD5+ vs CD5- (p=0.9, Figure 1A). We also calculated MZL related survival, including deaths related to transformation to high grade lymphoma. No difference was observed between CD5+ vs CD5-, (p=0.3, Figure 1B). Median time to 1st treatment (including topical treatment and radiation therapy) was 1.4 years (95% C.I. 0.4 - 2.3) for CD5+ and 0.4 years (95% C.I. 0.3 - 0.9). No difference was observed between CD5+ vs CD5- (p=0.2, Figure 1C). Median time to first systemic treatment was 4.3 years (95% C.I. 1.6 - 12.9) for CD5+ and 7 years (95% C.I. 3.1 - 15.4) for CD5- MZL. No difference was observed between CD5+ vs CD5- (p=0.6, Figure 1D). Conclusions In this retrospective case-control analysis assessing the differences between CD5+ and CD5- MZL in terms of clinical presentation, survival, transformation occurrence and time to first topical or systemic treatment, we demonstrate that the two subgroups might have some differences in terms of bone marrow involvement but probably no difference in terms of outcome. Our sample size is small, and larger studies on a population with more events might clarify, with multivariate analysis, if these differences are real. The genomic and molecular landscape of splenic marginal zone lymphoma, biological and clinical implications

Abstract:Splenic marginal zone lymphoma (SMZL) is a rare, predominantly indolent B-cell lymphoma constituting fewer than 2% of lymphoid neoplasms. However, around 30% of patients have a shorter survival despite currently available treatments and the prognosis is especially poor for the 5-15% of cases that transform to a large cell lymphoma. Mounting evidence suggests that the molecular pathogenesis of SMZL is critically shaped by microenvironmental triggering and cell-intrinsic aberrations. Immunogenetic investigations have revealed biases in the immunoglobulin gene repertoire, indicating a role of antigen selection. Furthermore, cytogenetic studies have identified recurrent chromosomal abnormalities such as deletion of the long arm of chromosome 7, though specific disease-associated genes remain elusive. Our knowledge of SMZL's mutational landscape, based on a limited number of cases, has identified recurring mutations in KLF2, NOTCH2, and TP53, as well as genes clustering within vital B-cell differentiation pathways. These mutations can be clustered within patient subgroups with different patterns of chromosomal lesions, immunogenetic features, transcriptional signatures, immune microenvironments, and clinical outcomes. Regarding SMZL epigenetics, initial DNA methylation profiling has unveiled epigenetically distinct patient subgroups, including one characterized by elevated expression of Polycomb repressor complex 2 (PRC2) components. Furthermore, it has also demonstrated that patients with evidence of high historical cell division, inferred from methylation data, exhibit inferior treatment-free survival. This review provides an overview of our current understanding of SMZL's molecular basis and its implications for patient outcomes. Additionally, it addresses existing knowledge gaps, proposes future research directions, and discusses how a comprehensive molecular understanding of the disease will lead to improved management and treatment choices for patients.
Clinicopathologic Features of CD23-Positive Nodal Marginal Zone Lymphomas

Abstract:Abstract Objectives: To identify and describe the clinicopathologic features characterizing a set of patients with nodal marginal zone lymphoma (N-MZL) positive for expression of CD23 and to contrast findings against those of a cohort of patients with CD23-negative N-MZL. Methods: Cases of N-MZL diagnosed over a 20-year interval were identified from the medical record and allocated to two cohorts based on immunophenotypic expression of CD23. Clinical, morphologic, immunophenotypic and cytogenetic data were retrospectively reviewed. Results: Twenty-six cases with histological and immunophenotypic findings typical of N-MZL were identified, 12 of which expressed CD23 by flow cytometry or immunohistochemistry. In the CD23-positive N-MZL cohort, only 2/12 (17%) of patients demonstrated multicompartment lymphadenopathy and another 3/12 (25%) demonstrated bone marrow involvement, with an Ann Arbor disease stage IV. In the CD23-negative cohort, 8/14 (57%) patients presented with multicompartment lymphadenopathy and an additional 6/14 (43%) demonstrated bone marrow involvement, with an Ann Arbor disease stage IV. The average follow-up period for all patients (CD23-positive and CD23-negative cohorts) was 6.7 years (range: 1-17; IQR: 10). In the CD23-positive disease cohort, treatment and follow-up data were recorded for 10/12 (83%) patients. Three patients were followed without intervention, one of which transformed to an aggressive lymphoma. This patient was treated with a series of rituximab-based chemotherapies and remains well two years post stem cell transplant. One patient was surgically managed with adjuvant radiation and remains well with no evidence of disease. Six others were treated with various rituximab-based chemotherapy regimens. Two died secondary to sequalae related to progression of their disease. The others remain alive and well with no evidence of active disease. In the CD23-negative disease cohort, treatment and follow-up data were recorded for 13/14 (93%) patients. For seven patients in this cohort followed by observation only, five were alive and well at last follow-up. Two were deceased due to unrelated causes. For those otherwise treated, there was only a single fatality due to progression of disease. Comparing the two cohorts, those with CD23-positive disease were significantly more likely to present with a lower Ann Arbor stage (p=0.0155) and FLIPI score (p=0.0086). However, neither 5-year (p=0.6908) nor overall survival (p=0.5956) were significantly different between the two cohort medians. Conclusions: CD23 expression in our cohort of N-MZL patients correlated with greater likelihood of single versus multiple compartment lymphadenopathy, restriction of lymphadenopathy to the head and neck regions, lower Ann Arbor stage, and lower FLIPI score. The data here, while derived from a limited cohort, demonstrates that CD23 correlates with a lower-grade clinical assessment at time of disease presentation but is associated with a more rapid clinical decline given a lack of evidence for survival benefits in the long term.

Validation and application of a radioimmunoassay for ovine growth hormone in the diagnosis of acromegaly in cats

Abstract:The validity of an ovine growth hormone (ogh) assay for the detection of feline growth hormone (fgh) was demonstrated by the parallel displacement of radiolabelled ogh by standard concentrations of ogh and serial dilutions of pooled fgh-rich serum. The minimum detectable limit of the assay was 1·67 μg/l. The mean (sd) basal fasting fgh level in 19 non-acromegalic, non-diabetic cats aged two to 16 years was 4·01 (1·38) μg/l (range 1·87 to 6·33); 19 acromegalic cats had significantly higher fgh levels (range 8·45 to 33·2 μg/l). There were no significant differences in the fgh levels measured when aprotinin was added to the samples or when plain serum and serum gel separation tubes were used for blood collection, but the fgh levels were significantly higher when the samples were collected into edta. There were also no significant differences between the concentrations of fgh measured in samples in which the separation of the serum and storage had been delayed by 24 hours, or in samples that had been stored for up to four weeks at −20°C.
Transformation in Marginal Zone Lymphoma: Results from a Prospective Cohort and a Meta-Analysis of the Literature

Abstract:Marginal zone lymphoma (MZL) includes extranodal (EMZL), splenic (SMZL), and nodal (NMZL) subtypes. Histological transformation (HT) to large B-cell lymphomas is well documented but with a large variability in published cumulative incidence rates. We report results from the Molecular Epidemiology Resource (MER) cohort for the cumulative incidence of HT (with death as competing risk) and associated risk factors and outcomes. We also conduct a meta-analysis of available studies on the cumulative incidence of HT. From 2002-2015, 529 patients with MZL were enrolled in the MER (69% EMZL, 16% SMZL, 15% NMZL). Ten-year overall survival (OS) from diagnosis was 66%. HT occurred in 21 patients, with 5-year and 10-year cumulative incidence of HT of 2.7% (95% confidence interval [CI] 0.02-0.05) and 3.6% (95%CI 0.02-0.06), respectively. HT was associated with an increased risk of death (subdistribution hazard ratio (HR)=3.95; 95%CI 2.06-7.55). Predictors of HT were ≥2 extranodal sites and MALT-IPI score ≥2. OS was 79% at 5 and 55% at 10 years after HT. Age at HT≥70 years was the only predictor of OS after HT (HR=3.57; 95%CI 1.34-9.48). In meta-analysis of 12 studies (6,161 patients), the 5- and 10-year cumulative incidence of HT across all subtypes were 5% (95%CI 0.05-0.06) and 8% (95%CI 0.07-0.09), respectively. Rates were lower in EMZL (3% and 5%) than in SMZL (7% and 13%) and NMZL (9% and 13%). While HT is relatively uncommon in the first decade after MZL diagnosis, it is associated with an inferior outcome and needs new approaches to prevention and management.

Abstract:Disease overview Marginal zone lymphomas (MZL) are collectively the second most common type of indolent lymphoma. Diagnosis Three subtypes of MZL are recognized: splenic, extranodal, and nodal. The diagnosis is secured following biopsy of an involved nodal or extranodal site demonstrating a clonal B‐cell infiltrate with CD5 and CD10 negative immunophenotype most common. Some cases will features IgM paraprotein, but MYD88 L256P mutations are less frequent than in Waldenstrom macroglobulinemia. Prognostication Several prognostic models have been developed, including the MALT‐IPI and the MZL‐IPI. The latter is broadly applicable across MZL subtypes and incorporates elevated serum LDH, anemia, lymphopenia, thrombocytopenia and nodal or disseminated subtypes as independent predictors of outcome. Treatment We discuss suggested approach to therapy for both early and advanced‐stage disease, with reference to chemo‐immunotherapy, radiotherapy, and emerging treatments in relapsed/refractory disease such as BTK inhibitors.

Abstract:Marginal zone lymphomas (MZLs) are a rare, indolent group of non‐Hodgkin lymphomas with different diagnostic, genetic and clinical features and therapeutic implications. The most common is extranodal MZL of mucosa‐associated lymphoid tissue, followed by splenic MZL and nodal MZL. Patients with MZL generally have good outcomes with long survival rates but frequently have a relapsing/remitting course requiring several lines of therapy. The heterogeneous presentation and relapsing course present the clinician with several diagnostic and therapeutic challenges. This position statement presents evidence‐based recommendations in the setting of Australia and New Zealand.

Clonal B-Cell Lymphocytosis with Mariginal-Zone Features: Comparison with Overt Splenic Marginal-Zone Lymphomas in 77 Patients from a Monocentric Series

Abstract:Diagnosis of non-CLL B Cell Chronic Lymphoproliferative Disorders (BCLPD), defined by the detection of peripheral blood (PB) B-Cell clone with flow cytometry (FC) Matutesscore≤3, is not an infrequent event in clinical daily practice. Only a portion of these cases may be actually classified according to the 2016 revision of WHO classification of lymphoid neoplasms, as in these cases a non-bone marrow (BM) tissue biopsy is rarely available due to "liquid-only disease" (BM, PB, splenomegaly) or difficult accessible sites. The two most likely diagnosis of BCLPD according to WHO 2016 are splenic marginal-zone lymphoma (SMZL) and monoclonal B Cell lymphocytosis (MBL) with non-CLL phenotype (CD19+, CD20+, SIg+, CD5-), sometimes reported as MZL-like, defined by monotypic clonal B lymphocytes (CBL) <5 x 109/l. A recent study pointed out a similar outcome regardless CBL value exceeding or not this arbitrary cut-off in strictly defined cases (absence of cytopenias, splenomegaly and adenopathy) of clonal B cell lymphocytosis with marginal-zone features (CBL-MZ) and confirmed the really indolent behaviour of this entity (Xochelli, Blood 2014). On these bases we decided to retrospectively collect clinical, immunophenotypic, molecular features and outcome of all consecutive patients (pts) with CD5- CBL-MZ owing to the Department of Hematology of University of Insubria (Varese, Italy) from 2011 to 2019 and to compare them with overt SMZL cases. Excluding 5 SMZL pts who were splenectomized, all cases were diagnosed by means of non-BM tissue biopsy, namely BM histology and PB/BM FC, FISH and molecular studies, when appropriate, and complete laboratory and imaging staging procedures (CT scan or US). The primary objective of the study was time to progression (TTP) for CBL-MZ group. Secondary objectives were overall survival (OS), progression-free survival (PFS) for treated pts and evaluation of early progression <24 months (Early POD) (Luminari, Blood 2019). Overall we collected complete data of 33 pts with CD5- CBL-MZ, and 44 with SMZL (Table 1). Focusing on CBL-MZ, FC immunophenotypic findings showed a B-Cell population with Matutes score 0 to 2 in all cases, uniform expression of B Cell antigens, namely CD19, CD20 strong, FMC7 (93%), SIg (intermediate/bright in 71%), while CD10 and CD5 were consistently negative. CD23 was expressed in 18%, CD38 in 9% and CD49b in 95%. BM sinusoidal infiltration pattern was detected in 50% of cases. TP53 mutation/deletion was detected in 4/16 pts (25%) while trisomy 12 and del13q in 1 pt each. Twenty pts presented with <5000/mmc CBL (MBL-MZ) and 13 with ≥5000/mmc CBL. With a median follow-up of 1.8 years, 6 pts (18%) progressed (4 in SMZL, 1 NMZL and 1 SLL; Fig 2) with a median TTP of 1.7 yrs (range 0.3- 12.3), while the remaining 27 remained completely stable. Among 4 pts treated, 2 received Rituximab-based regimen, both achieving CR and 2 with Chlorambucil both experiencing progression. Overall, 5 pts died (2 for progressive lymphoma), with an estimated 5-yrs of 80%. Concerning prognostic factors, after adjusting by competing risk method, at univariate analysis age >60 yrs, CBL >5000/mmc and albumin 60 yrs, Hb <11 g/dl, ECOG PS ≥2 and albumin 60 yrs and CBL>5000/mmc retained prognostic significance for TTP (p<0.0001), while ECOG PS ≥2 and albumin <3.5 g/dl remained independently associated with inferior OS (p<0.0001). Considering the SMZL group, 34 pts underwent systemic treatment (rituximab-based in 32, including R-Bendamustine in 17). With a median follow-up time of 2.8 yrs, 9 pts progressed (4 of whom within 24 months from treatment initiation), with a median PFS 2.9 yrs, and 8 died (7 due to progression, including 3 DLBCL transformations) with an estimated 5-yrs OS of 79%. Notably, Early POD SMZL pts exhibited a significant increased risk of death (p<0.0001, fig.2), while in CBL-MZ did not (p=0.4). In conclusion, CBL-MZ confirmed to be associated with indolent course and preferential evolution toward SMZL, of which it can be viewed as an initial phase or a precursor entity. In contrast with previous findings, the WHO 16 CBL cut-off defining MBL (5 x 109/l), together with advanced age, retained prognostic relevance for TTP. Finally, we confirmed for the first time in an independent series of SMZL the highly prognostic impact of Early POD24.
