Abstract:Splenic marginal zone lymphoma (SMZL) is a rare, predominantly indolent B-cell lymphoma constituting fewer than 2% of lymphoid neoplasms. However, around 30% of patients have a shorter survival despite currently available treatments and the prognosis is especially poor for the 5-15% of cases that transform to a large cell lymphoma. Mounting evidence suggests that the molecular pathogenesis of SMZL is critically shaped by microenvironmental triggering and cell-intrinsic aberrations. Immunogenetic investigations have revealed biases in the immunoglobulin gene repertoire, indicating a role of antigen selection. Furthermore, cytogenetic studies have identified recurrent chromosomal abnormalities such as deletion of the long arm of chromosome 7, though specific disease-associated genes remain elusive. Our knowledge of SMZL's mutational landscape, based on a limited number of cases, has identified recurring mutations in KLF2, NOTCH2, and TP53, as well as genes clustering within vital B-cell differentiation pathways. These mutations can be clustered within patient subgroups with different patterns of chromosomal lesions, immunogenetic features, transcriptional signatures, immune microenvironments, and clinical outcomes. Regarding SMZL epigenetics, initial DNA methylation profiling has unveiled epigenetically distinct patient subgroups, including one characterized by elevated expression of Polycomb repressor complex 2 (PRC2) components. Furthermore, it has also demonstrated that patients with evidence of high historical cell division, inferred from methylation data, exhibit inferior treatment-free survival. This review provides an overview of our current understanding of SMZL's molecular basis and its implications for patient outcomes. Additionally, it addresses existing knowledge gaps, proposes future research directions, and discusses how a comprehensive molecular understanding of the disease will lead to improved management and treatment choices for patients.

Abstract:Abstract Splenic marginal zone B-cell lymphoma (SMZL) is a heterogeneous clinico-biological entity. The clinical course is variable, multiple genes are mutated with no unifying mechanism, and essential regulatory pathways and surrounding microenvironments are diverse. We sought to clarify the heterogeneity of SMZL by resolving different subgroups and their underlying genomic abnormalities, pathway signatures, and microenvironment compositions to uncover biomarkers and therapeutic vulnerabilities. We studied 303 SMZL spleen samples collected through the IELSG46 multicenter international study (NCT02945319) by using a multiplatform approach. We carried out genetic and phenotypic analyses, defined self-organized signatures, validated the findings in independent primary tumor metadata and determined correlations with outcome data. We identified 2 prominent genetic clusters in SMZL, termed NNK (58% of cases, harboring NF-κB, NOTCH, and KLF2 modules) and DMT (32% of cases, with DNA-damage response, MAPK, and TLR modules). Genetic aberrations in multiple genes as well as cytogenetic and immunogenetic features distinguished NNK- from DMT-SMZLs. These genetic clusters not only have distinct underpinning biology, as judged by differences in gene-expression signatures, but also different outcomes, with inferior survival in NNK-SMZLs. Digital cytometry and in situ profiling segregated 2 basic types of SMZL immune microenvironments termed immune-suppressive SMZL (50% of cases, associated with inflammatory cells and immune checkpoint activation) and immune-silent SMZL (50% of cases, associated with an immune-excluded phenotype) with distinct mutational and clinical connotations. In summary, we propose a nosology of SMZL that can implement its classification and also aid in the development of rationally targeted treatments.

Clonal B-Cell Lymphocytosis with Mariginal-Zone Features: Comparison with Overt Splenic Marginal-Zone Lymphomas in 77 Patients from a Monocentric Series

Abstract:Diagnosis of non-CLL B Cell Chronic Lymphoproliferative Disorders (BCLPD), defined by the detection of peripheral blood (PB) B-Cell clone with flow cytometry (FC) Matutesscore≤3, is not an infrequent event in clinical daily practice. Only a portion of these cases may be actually classified according to the 2016 revision of WHO classification of lymphoid neoplasms, as in these cases a non-bone marrow (BM) tissue biopsy is rarely available due to "liquid-only disease" (BM, PB, splenomegaly) or difficult accessible sites. The two most likely diagnosis of BCLPD according to WHO 2016 are splenic marginal-zone lymphoma (SMZL) and monoclonal B Cell lymphocytosis (MBL) with non-CLL phenotype (CD19+, CD20+, SIg+, CD5-), sometimes reported as MZL-like, defined by monotypic clonal B lymphocytes (CBL) <5 x 109/l. A recent study pointed out a similar outcome regardless CBL value exceeding or not this arbitrary cut-off in strictly defined cases (absence of cytopenias, splenomegaly and adenopathy) of clonal B cell lymphocytosis with marginal-zone features (CBL-MZ) and confirmed the really indolent behaviour of this entity (Xochelli, Blood 2014). On these bases we decided to retrospectively collect clinical, immunophenotypic, molecular features and outcome of all consecutive patients (pts) with CD5- CBL-MZ owing to the Department of Hematology of University of Insubria (Varese, Italy) from 2011 to 2019 and to compare them with overt SMZL cases. Excluding 5 SMZL pts who were splenectomized, all cases were diagnosed by means of non-BM tissue biopsy, namely BM histology and PB/BM FC, FISH and molecular studies, when appropriate, and complete laboratory and imaging staging procedures (CT scan or US). The primary objective of the study was time to progression (TTP) for CBL-MZ group. Secondary objectives were overall survival (OS), progression-free survival (PFS) for treated pts and evaluation of early progression <24 months (Early POD) (Luminari, Blood 2019). Overall we collected complete data of 33 pts with CD5- CBL-MZ, and 44 with SMZL (Table 1). Focusing on CBL-MZ, FC immunophenotypic findings showed a B-Cell population with Matutes score 0 to 2 in all cases, uniform expression of B Cell antigens, namely CD19, CD20 strong, FMC7 (93%), SIg (intermediate/bright in 71%), while CD10 and CD5 were consistently negative. CD23 was expressed in 18%, CD38 in 9% and CD49b in 95%. BM sinusoidal infiltration pattern was detected in 50% of cases. TP53 mutation/deletion was detected in 4/16 pts (25%) while trisomy 12 and del13q in 1 pt each. Twenty pts presented with <5000/mmc CBL (MBL-MZ) and 13 with ≥5000/mmc CBL. With a median follow-up of 1.8 years, 6 pts (18%) progressed (4 in SMZL, 1 NMZL and 1 SLL; Fig 2) with a median TTP of 1.7 yrs (range 0.3- 12.3), while the remaining 27 remained completely stable. Among 4 pts treated, 2 received Rituximab-based regimen, both achieving CR and 2 with Chlorambucil both experiencing progression. Overall, 5 pts died (2 for progressive lymphoma), with an estimated 5-yrs of 80%. Concerning prognostic factors, after adjusting by competing risk method, at univariate analysis age >60 yrs, CBL >5000/mmc and albumin 60 yrs, Hb <11 g/dl, ECOG PS ≥2 and albumin 60 yrs and CBL>5000/mmc retained prognostic significance for TTP (p<0.0001), while ECOG PS ≥2 and albumin <3.5 g/dl remained independently associated with inferior OS (p<0.0001). Considering the SMZL group, 34 pts underwent systemic treatment (rituximab-based in 32, including R-Bendamustine in 17). With a median follow-up time of 2.8 yrs, 9 pts progressed (4 of whom within 24 months from treatment initiation), with a median PFS 2.9 yrs, and 8 died (7 due to progression, including 3 DLBCL transformations) with an estimated 5-yrs OS of 79%. Notably, Early POD SMZL pts exhibited a significant increased risk of death (p<0.0001, fig.2), while in CBL-MZ did not (p=0.4). In conclusion, CBL-MZ confirmed to be associated with indolent course and preferential evolution toward SMZL, of which it can be viewed as an initial phase or a precursor entity. In contrast with previous findings, the WHO 16 CBL cut-off defining MBL (5 x 109/l), together with advanced age, retained prognostic relevance for TTP. Finally, we confirmed for the first time in an independent series of SMZL the highly prognostic impact of Early POD24. Abstract:Abstract Background Splenic marginal zone lymphoma (SMZL) is a rare non-Hodgkin lymphoma, and much little is known about its clinical characteristics and management strategies. Here we present a case of SMZL and review relevant literature to provide a better recognition of this disease entity. Case presentation A 49-year-old Chinese female was admitted to our hospital with complaints of abdominal distension and acid reflux. Physical examinations and imaging investigations suggested the presence of splenomegaly. Laboratory workups revealed mildly reduced white blood cell count otherwise was not remarkable. The patient underwent splenectomy. Histological examination combined with immunohistochemical analysis of the resected spleen confirmed the diagnosis of SMZL. The patient recovered uneventfully during follow-ups. Conclusions Due to the rarity and unspecific clinical features, SMZL is extremely challenging to be diagnosed preoperatively. Patients with SMZL are generally associated with favorable prognosis. Combining the staging characteristics of non-Hodgkin’s lymphoma and splenic primary lymphoma may assist in clinical staging management of SMZL.

Abstract:Topic: 18. Indolent and mantle-cell non-Hodgkin lymphoma - Clinical Background: Zanubrutinib (BGB-3111) is a potent next-generation Bruton tyrosine kinase inhibitor approved in various countries for the treatment of R/R MZL based on the primary analysis results of the MAGNOLIA study (NCT03846427). Aims: To present the final analysis of MAGNOLIA at a median follow-up of 28 months (mo). Methods: MAGNOLIA is a phase 2, multicenter, single-arm study of adult pts requiring systemic treatment for R/R MZL with ≥1 prior line of therapy including ≥1 CD20-directed regimen. All pts received zanubrutinib 160 mg twice daily until disease progression or unacceptable toxicity. Primary endpoint was overall response rate (ORR) by independent review committee (IRC) in accordance with the Lugano classification. Secondary endpoints included ORR by investigator assessment, duration of response (DOR), progression-free survival (PFS), overall survival (OS), and safety. Efficacy was assessed by positron emission tomography (PET)-based Lugano criteria for pts with IRC-confirmed fluorodeoxyglucose (FDG)-avid disease at baseline; non-avid pts were assessed by computed tomography (CT)-based criteria. A sensitivity analysis using only CT-based criteria was also performed. Results: As of May 4, 2022, 68 pts were enrolled and treated. Median age was 70 years (range 37-95). MZL subtypes included extranodal (mucosa-associated lymphoid tissue) in 38.2%, nodal in 38.2%, splenic in 17.6%, and unknown in 5.9% of pts. Most (89.7%) pts received prior chemoimmunotherapy. Sixty-one (89.7%) pts had IRC-assessed FDG-avid disease. After a median follow-up of 28.0 mo (range 1.6-32.9) and a median treatment duration of 24.2 mo (range 0.9-32.9), 66 pts were evaluable for efficacy (Table). IRC-assessed ORR (complete response [CR] + partial response [PR]) was 68.2% (CR 25.8%). ORR was 64.0%, 76.0%, 66.7%, and 50.0% in extranodal, nodal, splenic, and unknown subtypes, respectively. CR rate was 40.0% in extranodal, 20.0% in nodal, 8.3% in splenic, and 25.0% in unknown subtypes. Median DOR, PFS, and OS were not reached. At the 2-year landmark by independent review, >70.0% of pts were alive or progression-free. Sensitivity analysis using only CT-based criteria (n=66) by IRC assessment showed an ORR of 66.7% (CR 24.2%). Median DOR and median PFS were not reached. At study completion, 31 (45.6%) pts deriving benefit rolled over to a long-term extension (LTE) study (NCT04170283); 24 (35.3%) pts discontinued owing to disease progression (investigator assessed); 5 (7.4%) due to adverse events (AEs), 2 (2.9%) required prohibited medications, and 1 (1.5%) withdrew consent. Most common treatment-emergent AEs in >20% of pts were bruising (23.5%), and diarrhea (22.1%). Neutropenia (8.8%) and COVID-19 pneumonia (5.9%) were the most common grade ≥3 AEs. Five (7.4%) pts died due to unrelated AEs: COVID-19 pneumonia (n=2), acute myeloid leukemia (n=1, prior alkylating agent exposure), myocardial infarction (n=1, preexisting coronary artery disease), and septic encephalopathy (n=1, pt in CR). Hypertension occurred in 3 (4.4%) pts, atrial fibrillation and atrial flutter in 1 (1.5%) pt each; none led to treatment withdrawal. One (1.5%) pt experienced grade 3 gastrointestinal hemorrhage while receiving rivaroxaban for pulmonary embolism; pt fully recovered and rolled over to the LTE study. Summary/Conclusion: With more than 2 years of median study follow-up, zanubrutinib continues to be effective as demonstrated by high response rates and durable disease control, and is generally well tolerated with no new safety signals observed.Keywords: Marginal zone, Tyrosine kinase inhibitor, Clinical data

Abstract:Splenic marginal zone lymphoma (SMZL) is an indolent small B-cell lymphoma originating from the splenic white pulp, which account for <2% of non-Hodgkin lymphoma. [ 1 ] Owing to the paucity of this disease entity, the feature of SMZL has only been characterized partly. The pathogenetic process is chronic and insidious and part of patients with SMZL even have no special clinical symptoms at diagnosis. Despite it originates from the spleen and the splenic hilar lymph nodes are susceptible to involve, peripheral blood and bone marrow are generally involved when confirming the diagnosis of SMZL. Subsequently, peripheral lymph nodes and extra-lymph node organs, liver for instance, can be infiltrated with the disease progressing. [ 2 ] Meanwhile, the patients with SMZL frequently combine with autoimmune disease such as autoimmune hemolytic anemia, Sjogren syndrome and immune thrombocytopenia. [ 3 ] In spite of its indolence, approximately 10% of individuals with SMZL probably convert into aggressive diffuse large B-cell lymphoma. [ 4 ] Furthermore, about one third of patients exist plasmocytic differentiation and secrete monoclonal paraproteins including IgM predominantly. Nevertheless, very few reports about the cases of SMZL with plasmocytic differentiation, especially accompanying with monoclonal IgG, are available at present. We currently report a case of SMZL with monoclonal IgG and meanwhile conduct literature review in order to improve the understanding of the diagnosis, differential diagnosis and treatment of this uncommon entity.

Clinical Features and Survival Outcome of Aggressive Splenic Lymphoma: a Retrospective Study

Abstract:Abstract Purpose The present study aims to explore the clinical features, imaging manifestations, pathologic characteristics, treatment, and prognosis of aggressive splenic lymphoma (ASL) patients.Methods From January 2001 to March 2021, patients who were histologically proven splenic lymphoma were enrolled in this study. The survival curves of overall survival (OS) and progressive-free survival (PFS) were calculated by the Kaplan–Meier survival analysis and log-rank test. Univariate and multivariate Cox regression analyses were performed to estimate the potential prognostic factors.Results Sixty-nine patients who were histologically proven ASL in this study. The median follow-up time was 76.3 months (95%CI, 60.194-92.406). Patients with ASL underwent the splenectomy had better PFS and OS than that of patients who did not (OS 61% vs 35%, P=0.084; PFS 57% vs 25%, P=0.061), yet the difference was not remarkably significant. In addition, splenic lymphomas of T-cell origin (OS HR, 3.398[95%CI, 1.327-8.704], P=0.011; PFS HR, 2.776[95%CI, 1.107-6.959], P=0.029) and present with B symptoms (OS HR, 2.703[95%CI, 1.219-5.994], P=0.014; PFS HR, 2.890[95%CI, 1.348-6.194], P=0.006) were independent risk factors correlated with OS and PFS. After establishing a risk-stratified model in combination with cell of origin (COO) and B symptom to divide patients into three risk subgroups: 36 were in low-risk group, 28 were in intermediate-risk group, and 5 were in high-risk group. The differences between each group were statistically significant (OS: χ2=41.000, P<0.001; PFS: χ2=47.739, P<0.001).Conclusion Splenic T-cell lymphoma patients with B symptom at initial diagnosis had the worst outcome among aggressive splenic lymphoma.

Abstract:The aim of this study was to investigate the clinical and laboratorial characteristics of splenic marginal zone lymphoma (SMZL) with an abnormal complete blood count (CBC). Data of 19 newly diagnosed SMZL patients with abnormal CBC were analyzed retrospectively. Seven patients were diagnosed by using splenic histology, 12 patients who did not undergo splenectomy were diagnosed on the basis of typical clinical presentation and cytologic, immunophenotypic and histologic characteristics of peripheral blood and bone marrow, according to SBLG guidelines. The results showed that leukocytosis (≥ 10.0×10(9)/L) was seen in 5 cases (26.3%); leukocytopenia (< 4.0×10(9)/L) was found in 6 cases (31.6%), hemoglobin concentration less than 120 g/L was found in 14 cases (73.7%) and thrombocytopenia was found in 11 (57.9%) patients. Fourteen (73.7%) patients had cytopenia in one or more lineage. As a specific morphologic character, villous lymphocytes were found in 10 (52.6%) patients. Similar immunophenotype was determined by histology in both bone marrow and spleen. Various histological infiltration patterns including intrasinusoidal pattern were found in bone marrow. Nine out of 16 (56.3%) patients displayed an increase of serum monoclonal immunoglobin. Autoimmune phenomena was found in 12 out of 15 (80.0%) patients. Splenectomy, as the only treatment could not achieve a ≥ 50% improvement of CBC in 4 patients, and then was judged as no response. Splenectomy followed by chemotherapy achieved partial response (PR) in 1 patient. Overall response rate of the therapeutic strategies with Rituximab was 100.0% (11/11). Furthermore, complete response was achieved in 9 out of 11 (81.8%) patients. It is concluded that SMZL with abnormal CBC has a higher incidence of cytopenia, bone marrow involvement and autoimmune phenomena. Therapeutic strategies consisting of Rituximab show a better efficacy.

Abstract:Purpose: Marginal zone lymphoma (MZL) is an uncommon non-Hodgkin lymphoma with malignant cells that exhibit a consistent dependency on B-cell receptor signaling. We evaluated the efficacy and safety of zanubrutinib, a next-generation selective Bruton tyrosine kinase inhibitor, in patients with relapsed/ refractory (R/R) MZL. Patients and Methods: Patients with R/R MZL were enrolled in the phase II MAGNOLIA (BGB-3111-214) study. The primary endpoint was overall response rate (ORR) as determined by an independent review committee (IRC) based on the Lugano 2014 classification. Results: Sixty-eight patients were enrolled. After a median follow-up of 15.7 months (range, 1.6 to 21.9 months), the IRC-assessed ORR was 68.2% and complete response (CR) was 25.8%. The ORR by investigator assessment was 74.2%, and the CR rate was 25.8%. The median duration of response (DOR) and median progression-free survival (PFS) by independent review was not reached. The IRC-assessed DOR rate at 12 months was 93.0%, and IRC-assessed PFS rate was 82.5% at both 12 and 15 months. Treatment was well tolerated with the majority of adverse events (AE) being grade 1 or 2. The most common AEs were diarrhea (22.1%), contusion (20.6%), and constipation (14.7%). Atrial fibrillation/flutter was reported in 2 patients; 1 patient had grade 3 hypertension. No patient experienced major hemorrhage. In total, 4 patients discontinued treatment due to AEs, none of which were considered treatment-related by the investigators. Conclusions: Zanubrutinib demonstrated high ORR and CR rate with durable disease control and a favorable safety profile in patients with R/R MZL.

Abstract:Abstract Background Peripheral T-cell lymphoma(PTCL) is a group of lymphoproliferative tumors originated from post-thymic T cells or mature natural killer (NK) cells. It shows highly aggressive clinical behaviour, resistance to conventional chemotherapy, and a poor prognosis. Its incidence rate in China is 1 to 2 times higher than in western countries. Therefore, optimal strategies for identifying high-risk patients are urgently needed. Materials and Methods We retrospectively studied 347 newly diagnosed PTCL patients from January 2011 to October 2019 and analyzed the relationship between peripheral blood lymphocyte-monocyte ratio (LMR) and platelet-monocyte ratio (PMR) and prognosis. The model of Peripheral Blood Score was established to screen out high-risk patients. Results The receiver operating characteristic (ROC) curve was used to determine the optimal cut-off value based on survival rate. It was found that patients with PTCL with LMR ≤ 1.68 and PMR ≤ 300 had inferior overall survival (OS) and the difference was significant in both low-risk (P<0.001) and medium high-risk (P<0.001) groups of IPI score. In multivariate analysis, LMR ≤ 1.68 (HR=1.751, 95% CI 1.158-2.647, p=0.006), PMR ≤ 300 (HR=1.762, 95% CI 1.201-2.586, p=0.002), stage III-IV (HR=3.276, 95% CI 1.512-7.099, p=0.003), Eastern Cooperative Oncology Group (ECOG) score 3-5 (HR=2.351, 95% CI 1.647-3.356, p<0.001) and extra-nodal invasion more than one site (HR=1.659, 95% CI 1.125-2.445, p=0.039) were independently associated with short survival. LMR and PMR were integrated into "Peripheral Blood Score (PBS)" model. PTCL patients were divided into three risk groups: low-risk group, medium risk group and high-risk group. The 1-year OS was 86%, 55.3% and 22.6%, and the 3-year OS was 43.4%, 20% and 13.1%, respectively. Conclusion Overall, LMR and PMR can be used as early prognostic indicators in PTCL patients. Moreover, we can easily detect the complete blood cell count (CBC), and use PBS model to preliminarily screen and stratify patients. It is simple, convenient and accurate to screen out patients with short lives, and formulate personalized treatment strategies.

Abstract:OBJECTIVETo deepen the understanding of splenic marginal zone lymphoma (SMZL) and improve the level of diagnosis and therapy.METHODSA typical case of SMZL, a 61 year old female with lymphocytosis and splenomegaly found fortuitously, was reported. The pathologic, immunologic and genetic features of tumor cells in peripheral blood, bone marrow and spleen were studied with light microscopy, phase contrast microscopy, scanning electron microscopy, immunohistochemical method, flow cytometry, G chromosome banding technique and PCR for studying the pattern of IgH gene rearrangement.RESULTSThe spleen was large with uniform parenchyma and smooth surface. There were multiple small gray-white nodules on sections. Histologically, the neoplastic cells replaced the marginal and mantle zones with complete replacement of germinal centers in the white pulp. The neoplastic cells were predominantly of small to medium size with oval or slightly irregular nuclei. Lymph nodes in the splenic hilum were infiltrated by tumor cells. Immunophenotypic analysis demonstrated that the lymphocytes in the bone marrow expressed CD(20), HLA-DR, CD(45) RA and bcl-2. The monoclonal pattern of IgH gene rearrangement in peripheral blood and bone marrow was found to be the same as that in spleen. After splenectomy, COP chemotherapy and IFNalpha-2a were given and the abnormally increased lymphocytes decreased to normal level. Seven months later the monoclonal rearranged immunoglobulin heavy chain gene pattern changed to polyclonal pattern.CONCLUSIONSplenomegaly, lymphocytosis in peripheral blood and bone marrow without lymph node enlargement and leukocytosis are clinical characters of SMZL. Presence of monoclonal rearranged IgH gene is in favor of the diagnosis. Splenectomy should be done earlier in suspicious patients to avoid malignant transformation.

Abstract:Splenectomy is considered as one of the first-line treatments for symptomatic patients with splenic marginal zone lymphoma (SMZL). Between 1997 and 2012, 100 hepatitis C virus-negative patients with SMZL were treated by splenectomy as first-line treatment. At 6 months, all patients but three recovered from all cytopenias. The median lymphocyte count at 6 months and 1 year was 11.51 × 10(9)/L and 6.9 × 10(9)/L, respectively. Median progression-free survival (PFS) was 8.25 years. The 5-year and 10-year overall survival (OS) rates were 84% and 67%, respectively. Histological transformation occurred in 11% of patients, and was the only parameter significantly associated with a shorter time to progression (p = 0.0001). Significant prognostic factors for OS were age (p = 0.0356) and histological transformation (p = 0.0312). In this large retrospective cohort, we confirmed that splenectomy as first-line treatment in patients with SMZL corrected cytopenias and lymphocytosis within the first year and was associated with a good PFS.

High-Grade Transformation in A Splenic Marginal Zone Lymphoma with A Cerebral Manifestation

Abstract:BACKGROUND Splenic marginal zone lymphomas (SMZLs) are generally uncommon, indolent lymphomas that typically affect older adults, but the development of the transformation to high-grade lymphoma may occur in a small proportion of patients and represents a rare event with blastic cell infiltration in the lymph nodes and bone marrow. CASE REPORT Here, we present a young adult patient who was diagnosed with a SMZL and developed a high-grade transformation to diffuse large B cell lymphoma (DLBCL) with central nervous system involvement. The patient was a 31-year-old woman whose hematologic medical history began with severe anemia and thrombocytopenia associated with atypical lymphoid infiltrate in the bone marrow and massive splenomegaly. A splenectomy was performed and revealed the SMZL. She was first treated with the CHOP regimen (cyclophosphamide, doxorubicin, vincristine, and prednisone) and subsequently with the R-FMD regimen (rituximab, fludarabine, mitoxantrone, and dexamethasone) because the peripheral blood leukocytes were remarkably increased postoperatively. Six months after the splenectomy, she complained of headaches. A magnetic resonance imaging scan of her brain revealed intracerebral tumorous lesions from which a biopsy was taken. On morphological and immunohistochemical examination, the tumor fulfilled the criteria for a DLBCL. Treatment with pulse-dose intravenous methylprednisolone followed by high-dose methotrexate was promptly initiated, but the patient's condition continued to deteriorate and she died of the disease 13 months after the splenectomy. CONCLUSIONS Although there is a general tendency for SMZL to display low aggressiveness, central nervous system involvement associated with a histological transformation to high-grade lymphoma, as presented here, can occur in advanced stage of the disease.

Abstract:Purpose: Splenic marginal lymphoma (SMZL) is a rare indolent B-cell lymphoma with long survival. The minimal residual disease (MRD) detection in indolent B-cell lymphomas has shown power in predicting survival and even in guiding treatment duration in chronic lymphocytic leukemia. As almost all SMZL patients have bone marrow involvement (BMI) at diagnosis, we prospectively designed the MRD detection during treatment to study its prognostic value in SMZL. Methods: SMZL patients who needed chemotherapy would regularly monitor the MRD status until undetectable MRD by multiparameter flow cytometry (MFC). Patients with MRD under 0.01% were defined as uMRD. Other clinical factors were also analysed as prognostic factors. Results: In total, 71 patients with frontline therapy were enrolled in this study. There were 284 evaluable BM MRD samples. At end of treatment, 55 patients (77.4%) got uMRD, 10 patients had a MRD level of 0.01-1%, and 6 patients had a MRD level of ≥ 1%. The uMRD rate increased at the end of treatment. With a median follow-up of 50 (24-157) months, uMRD patients showed superior outcomes compared with MRD-positive patients. Patients with uMRD had a significant better progression free survival (PFS) compared to those with MRD. PR with positive-MRD after induction therapy was a significantly poor predictor for PFS and overall survival (OS). Multivariate prognostic analysis showed it was a powerful independent prognostic factor for PFS [HR=0.357 (95%CI 0.131-0.972), P=0.044]. Conclusion: uMRD in BM was an independent prognostic factor in SMZL patients, especially for the patients only achieving PR at the end of therapy. Abstract:The splenic marginal zone lymphoma (SMZL) is a relatively rare chronic B lymphoproliferative disease, which primarily manifest increase of peripheral blood lymphocyte count and/or scale, and splenomegaly, while the peripheral superficial lymph nodes are often not swollen. Therefore, the splenectomy are usually needed to confirm the diagnosis, but the majority of patients could not accept such management, resulting in early difficult diagnosis. This study was purposed to explore the more prior way for diagnosis based flow cytometry (FCM). Six patients with suspected diagnosis of SMZL were used as research objects, 10 healthy bone marrow donors and 10 cases of chronic lymphocytic leukemia (CLL), 3 cases of hairy cell leukemia (HCL), 3 cases of lymphatic plasma cell lymphoma/Waldenströ's macroglobulinemia (LPL/WM) were selected as control. The immunophenotype of bone marrow cells were analyzed and compared by FCM using a panel of antibodies including CD45, CD5, CD10, CD19, CD20, CD22, CD23, CD25, CD103, CD11c, CD123, κ,λ, Cyclin D1, and combined with bone marrow cell morphology. The results indicated that 6 cases of suspected SMZL showed a large increase of lymphocytes and splenomegaly. Because absence of peripheral lymphadenopathy, 6 patients did not suffer from lymph node biopsy, only 1 patient underwent diagnostic splenectomy. The immunophenotypes of bone marrow in patients and controls were analyzed by FCM, as a result, except for the healthy donors, varying degrees of abnormal mature B cell clones were found in bone marrow of all patients, and the further differentiation from other B-cell tumors was performed through CD5, CD10 expression and combination with other B-cell phenotype. All 6 cases of SMZL patients expressed CD19(+) and CD20(+), but CD10 expression was negative, 4 patients expressed CD5(-), 2 patients expressed CD5(+). The expressions of CD23, CD38, ZAP-70, CD11c, CD103, CD123, Cyclin D1 were negative. The morphological examination of bone marrow cells showed velutinous abnormal lymphocytes. Combined with clinical characteristics, 6 patients were diagnosed as SMZL, 1 patient suffered from splenectomy because of concurrent hypersplenism, and this postoperative pathologic examination confirmed the patient with SMZL. Ten cases of CLL mainly expressed CD5, CD23; 3 cases of HCL had more typical morphology of "hair like" in addition to CD11c, CD103 and CD123 positive; 3 cases of LPL/WM had significantly increased light chain restriction expression, IgM, plasmacytoid lymphocytes. It is concluded that the FCM immunophenotype analysis can be used as a powerful tools for clinical diagnosis of SMZL.

Abstract:Rituximab® provides high response rates and effective disease palliation in patients with splenic marginal zone lymphoma (SMZL). We conducted a phase II trial in patients with SMZL who were either untreated or were splenectomized but had shown disease progression within 1 year after splenectomy. Treatment consisted of six courses of Rituximab with cyclophosphamide, vincristine, non-pegylated liposomal doxorubicin and prednisone (R-COMP). Fifty-one patients were eligible for the analysis. The overall response rate was 84%. The 6-year progression-free survival and overall survival were 54% and 72%, respectively. Toxicity was substantial (grade≥3 neutropenia: 26%; grade≥3 infections: 8%). Of the 15 deaths, two occurred on treatment (one sepsis and one pneumonia). Six deaths were due to lymphoma progression, four to secondary neoplasia, one to sepsis, one to pneumonia and one to splenectomy complications. R-COMP should be restricted to patients with bulky disease associated with symptoms or to patients with possible histological transformation.

Abstract:When this topic, nonspecific resistance to infectious disease, first came under the author's consideration it seemed quite possible to group many factors affecting disease resistance into the nonspecific category. At first thought, penicillin, botulinus toxin, and climatic differences might be considered such agents. Penicillin affects several species of bacteria in several hosts. Botulinus toxin sickens several hosts. Climate seemingly changes many diseases. Further thought showed that the problem was much more complicated if the actual operation of any of these agents was traced to the mechanism through which it acted in causing resistance or susceptibility. It was true that the action of each of these agents spread over a number of species but in these species the vulnerable point was generally found in some common physicochemical reaction extending throughout the group. This common method of action would be expected if consideration is given to how the disease process could arise. In evolutionary times differentiation took place, making different members of the then living group capable of fitting into different climatic or other conditions. The initiation of these differentiations probably took origin in specific mutations, which control specific mechanisms in the developmental pattern not only of the individual with the inheritance but also in progeny to come. As a result, rivers of like physicochemical reactions flowed out to what ultimately became different species. Similarly pathogens developed and expanded through the incorporation of some change in their organization which gave them the ability to invade a specific host. Again the process was comparable to gene mutation. Ever-present selection refined the processes by which the organism became pathogenic on those species which had the specific characteristic to interact specifically with the newly acquired property.

Abstract:Marginal zone lymphoma (MZL) is the second most common subtype of inert B-cell non-Hodgkin's lymphoma, accounting for 5–15% of non-Hodgkin's lymphoma cases. Patients with MZL have a long survival period, with a median survival of >10 years, and patients treated with a combination of anti-CD20 monoclonal antibody can achieve an overall effective rate of 81%. However, 20% of patients with MZL show relapse or experience disease progression within 2 years, with a median survival of only 3–5 years. Currently, the treatment options for patients with relapsed/refractory (R/R) MZL are limited, underscoring the pressing need for novel therapeutic drugs. The advent of novel anti-CD20 monoclonal antibodies, small molecule kinase inhibitors, immunomodulators, and other therapeutic strategies has ushered in a new era in the treatment of R/R MZL. Our objective is to summarize the existing treatment strategies, including immunotherapy and the emergent targeted therapies, and to evaluate their effectiveness and safety in the management of R/R MZL. By doing so, we aim to provide a clear understanding of the therapeutic landscape for R/R MZL, and to guide future research directions toward improving the prognosis and quality of life for patients afflicted with this challenging disease.
