Abstract:Purpose: To summarize the available literature related to reliability and validity of the Timed Up and Go in typical adults and children, and individuals diagnosed with the following pathologies: Huntington's disease, stroke, multiple sclerosis, Parkinson's disease, spinal cord injury, Down syndrome, or cerebral palsy. Materials and methods: A search was conducted using MeSH terms and keywords through a variety of databases. Data regarding reliability and validity were synthesized. Results: This review included 77 articles. Results were variable depending on the studied population. The Timed Up and Go showed excellent reliability in typical adults, in individuals with cerebral palsy, in individuals with multiple sclerosis, in individuals with Huntington's disease, individuals with a stroke, and individuals with a spinal cord injury. The TUG demonstrated strong concurrent validity for individuals with stroke and spinal cord injury. Predictive validity data was limited. Conclusions: Based on the literature assessed, the Timed Up and Go is clinically applicable and reliable across multiple populations. The Timed Up and Go has a wide variety of clinical use making it a diverse measure that should be considered when choosing an outcome an activity based outcome measure. However, there are some limitations in the validity of the utilization of the Timed Up and Go to some populations due to a lack of data and/or poor choice of comparison outcome measures when assessing validity. Additional research is needed for young to middle aged adults.IMPLICATIONS FOR REHABILITATIONOutcome measures are a vital component of clinical practice across all populations.The Timed Up and Go is a highly studied outcome measure in the geriatric population, but lacks research of its applicability to other populations.This study was able to highlight the clinical utility of the Timed Up and Go in populations that under utilize this outcome measure.

Abstract:Objective To 1) develop and evaluate virtual assessment protocols for the Timed-Up-and-Go (TUG) usual and fast-pace, Chair Rise-5 repetition, and the 1-minute Sit-to-Stand (1MSTS) tests, and 2) examine their feasibility, reliability, and validity. Design This was a prospective cohort study, where participants engaged in up to two virtual assessments and data was collected prospectively for one year. Setting Assessments took place in participants' homes in the community. Toolkits were delivered that included the required equipment and internet-enabled tablet. Participants Participants included a random sample of adults ≥ 65 years old who could walk 10m without assistance from another person and communicate in English. Main outcome measures Falls and healthcare utilization data were collected for one year after initial assessments. Descriptive data were used to examine feasibility; intra-class correlation and Pearson's/Spearman's correlation and AUCs were used to evaluate reliability and validity, respectively. Results Sixty participants (mean age 76.6 ± 5.6; 53% female) completed visit 1 and 52 completed visit 2. Technology issues were relatively common, yet no safety incidents occurred. All the virtual mobility tests demonstrated excellent test-retest (ICCs=0.86 to 0.95) and inter-rater (ICC=0.98 to 0.99) reliability. The tests had weak-to-moderate correlations with measures of physical function (r=-0.14 to -0.43) and physical activity (r=-0.19 to 0.32). Fifty-seven total falls were recorded by 28 participants (46%) over 1 year. The virtual mobility tests did not have adequate accuracy for predicting falls (AUCs = 0.53 to 0.59), specialist (AUCs=0.48-0.60), family doctor (AUCs=0.48-0.59), or rehabilitation professional visits (AUCs=0.65-0.67), or hospitalizations (AUCs=0.56-0.63). Conclusion Results support the feasibility and reliability of virtual administration of the TUG usual and fast-pace, Chair Rise, and 1MSTS. Clinicians and researchers can use the included manuals to conduct these tests virtually. Future research should examine the clinical utility of these tests for remote monitoring and mobility assessment.

Timed Up and Go (TUG) Reference Values and Predictive Cutoffs for Fall Risk and Disability in Singaporean Community-Dwelling Adults: Yishun Cross-Sectional Study and Singapore Longitudinal Aging Study

Abstract:<h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Objectives</h3><p>The "timed up and go" (TUG) test is a simple and widely used test of overall functional mobility. There is a paucity of TUG normative data among Asian individuals who differ in habitual gait speed and fall risk from Caucasians. The objectives of this study were to determine TUG reference values and optimum cutoffs predicting prevalent and incident disability for community-dwelling adults.</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Design</h3><p>One cross-sectional (Study 1–Yishun Study) and one longitudinal (Study 2–Singapore Longitudinal Aging Study) study in Singapore.</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Setting and Participants</h3><p>Study 1 comprised 538 nondisabled, community-dwelling adults aged between 21 and 90 years. Study 2 comprised 1356 community-dwelling older adults aged ≥55 years followed for 3 years.</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Methods</h3><p>Study 1 collected TUG reference values and assessed physiological fall risk (PFR) using the Physiological Profile Assessment (PPA). Study 2 assessed association of TUG with disability with the Barthel Index and the Lawton scale at baseline and follow-up.</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Results</h3><p>From Study 1, mean TUG time for individuals aged 60 to 74 years was 9.80 seconds, shorter than values reported for Caucasians of 12.30 seconds. It was significantly associated with high PFR (odds ratio [OR] 1.14, 95% confidence interval [CI] 1.03–1.27), 74.0% agreement, Cohen's kappa = 0.314 (95% CI 0.238–0.390); area under the curve = 0.85 (95% CI 0.80–0.90). A TUG cutoff of 10.2 seconds discriminated high PFR from low PFR with 84.4% sensitivity and 72.6% specificity. In Study 2, the threshold for observing significantly increased risk of disability was ≥9.45 seconds for prevalent disability (OR 2.98, 95% CI 1.41–6.78), functional decline (OR 2.68, 95% CI 1.33–5.80), and incidental disability (OR 2.25, 95% CI 1.08–4.97).</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Conclusions and Implications</h3><p>TUG reference values and cutoff predicting disability for community-dwelling older adults in Singapore are consistent with Asian data and lower than for Caucasians. TUG could be used to guide development and evaluation of risk screening of adverse health outcomes across the life span in Singapore.</p>

Quantitative falls risk assessment in elderly people: results from a clinical study with distance based timed up-and-go test recordings

Abstract:Abstract Objective : A third of people over 65 years experiences at least one fall a year. The Timed Up-and-Go (TUG) test is commonly used to assess gait and balance and to evaluate an individual’s risk of falling. Approach : We conducted a clinical study with 46 older participants for evaluating the fall risk assessment capabilities of an ultra-sound based TUG test device. The fall protocols over a period of one year were used to classify participants as fallers and non-fallers. For frailty evaluation, state-of-the-art questionnaires were used. Fall recordings were compared to six TUG test measurements that were recorded in fallers and non-fallers. Main results : TUG test data were available for 39 participants (36 f, age 84.2 ± 8.2, BMI 26.0 ± 5.1). Twenty-three participants did fall at least once within the fall screening period. We fitted two different regression and probability models into a region of interest of the distance over time curve as derived from the TUG device. We found that the coefficient of determination for Gaussian bell-shaped curves ( p < 0.05, AUC = 0.71) and linear regression lines ( p < 0.02, AUC = 0.74) significantly separated fallers from non-fallers. Subtasks of the TUG test like the sit-up time showed near significance ( p < 0.07, AUC = 0.67). Significance : We found that specific features calculated from the TUG distance over time curve were significantly different between fallers and non-fallers in our study population. Automatic recording and analysis of TUG measurements could, therefore, reduce time of measurements and improve precision as compared to other methods currently being used in the assessments of fall risk.

Abstract:To investigate the intra- and inter-rater reliability of the face‐to‐face assessment and tele‐assessment of performance-based tests in older adults. The results of this study suggest that the Timed-Up and Go, Five Times Sit-to-Stand Test, and 4-Meter Walk Test (4MWT) seem to be reliable tools of tele-assessment in older adults. These practical assessment tools can be used in home settings for older adults as tele-health interventions. This study aimed to assess Timed-Up and Go (TUG), Five Times Sit-to-Stand Test (5xSST), and 4-Meter Walk Test (4MWT) intra- and inter-rater reliability between face-to-face and tele-assessment tools in older adults. A total of 54 older adults (aged ≥ 65 years) were included in the study. Measurements (TUG, 5xSST, and 4MWT) were carried out face-to-face and tele-assessment. The intraclass correlation coefficients (ICCs), and Bland–Altman plots was used determine intra- and inter-rater reliability. The mean age of the participants, 53% of whom were female, was 69.59 years. Inter-rater reliability between face-to-face and tele-assessment was as following: excellent for TUG (ICC = 0.931) and 5STS (ICC = 0.908), and good for 4MWT (ICC = 0.803). In addition, intra-rater reliability between the two tele-assessments was excellent for all tests. The ICC values for TUG, 5STS, and 4MWT were 0.973, 0.948, and 0.934, respectively. To conclude, the TUG, 5xSST, and 4MWT seem to be reliable tools of tele-assessment in older adults. These practical assessment tools can be used in home settings for older adults as tele-health interventions.

Use of Dual-Task Timed-Up-and-Go Tests for Predicting Falls in Physically Active, Community-Dwelling Older Adults-A Prospective Study

Abstract:This prospective study aimed to determine which specific mobility tests were the most accurate for predicting falls in physically active older adults living in the community. Seventy-nine physically active older adults who met the American College of Sports Medicine physical activity guidelines volunteered. Participants were assessed and followed up for 12 months. Mobility assessments included the 30-s sit-to-stand test, five times sit-to-stand test, single-task timed-up-and-go test (TUG), motor dual-task TUG (Mot-TUG), and cognitive dual-task TUG (Cog-TUG). Mot-TUG and Cog-TUG performances were moderately correlated with number of falls (r = .359, p < .01 and r = .372, p < .01, respectively). When Mot-TUG, Cog-TUG, or Age were included as fall predictors, discrimination scores represented by the area under the receiver operating characteristic curve (AUC) were AUC (Mot-TUG) = 0.843 (p < .01), AUC (Cog-TUG) = 0.856 (p < .01), and AUC (Age) = 0.734 (p < .05). The cutoff point for Cog-TUG was 10.98 s, with test sensitivity of 1.00 and specificity of 0.66. Fall predictors for different populations may be based on different test methods. Here, the dual-task TUG test more accurately predicted falls in older adults who met American College of Sports Medicine's physical activity guidelines.

Abstract:OBJECTIVE: To determine normative values and identify contributing factors for physical performance tests in older, Thai, community-dwelling adults.DESIGN: Nationwide cross-sectional study.SETTING: Thai older community-dwelling adults.PARTICIPANTS: Thai older community-dwelling adults aged ≥60 years who had no major health problems (N=1430) between March 2021 and August 2022.INTERVENTIONS: Not applicable.MAIN OUTCOME MEASURES: Normative values for the timed Up and Go (TUG) test, gait speed test, and 5-times sit-to-stand (5TSTS) test were determined for sex and age groups. Multivariable quantile regression analysis was employed to evaluate the participants, considering factors that may influence physical performance, such as height, and Charlson comorbidity index (CCI).RESULTS: The study included 1430 eligible participants. Their mean age was 68.4±5.8 years, and 58.5% were women. Men demonstrated superior physical performance in the medians (p50) of the TUG (10.0 s vs 11.0 s), gait speed (0.98 m/s vs 0.91 m/s), and 5TSTS (14.0 s vs 16.1 s) tests compared with women. These differences were consistently observed across age groups. Moreover, age, sex, and height were significantly associated with poor physical performance.CONCLUSION: This study observed variations in the normative values of TUG, gait speed, and 5TSTS tests among different age groups of older, Thai, community-dwelling adults. Additionally, our findings identified age, sex, and height as significant contributing factors to physical performance in this population.

Abstract:Few studies have reported on the validity of a sensor-based lower-limb kinematics evaluation during the timed up and go (TUG) test. This study aimed to determine the validity of a wearable gait sensor system for measuring lower-limb kinematics during the TUG test. Ten young healthy participants were enrolled, and lower-limb kinematics during the TUG test were assessed using a wearable gait sensor system and a standard optical motion analysis system. The angular velocities of the hip, knee, and ankle joints in sit-to-stand and turn-to-sit phases were significantly correlated between the two motion analysis systems (R = 0.612–0.937). The peak angles and ranges of motion of hip, knee, and ankle joints in the walking-out and walking-in phases were also correlated in both systems (R = 0.528–0.924). These results indicate that the wearable gait sensor system is useful for evaluating lower-limb kinematics not only during gait, but also during the TUG test.

Abstract:Abstract Objectives The aim of this study was to determine the concurrent validity and reliability of the Osteoarthritis Research Society International recommended performance-based tests delivered by video-based in adults with knee osteoarthritis. Methods Thirty-two participants (aged 40 to 70 years; 15 men) undertook assessments of 4 performance-based tests via both video-based (real-time and recorded) and face-to-face approaches, on the same day. Outcome measures were performance-based test and the number of technical issues encountered. The performance-based tests included the 40-Meter Fast-Paced Walk Test (velocity, m/s), the 30-Second Chair Stand Test (number of repetitions), the Stair Climb Test (time, seconds), and the Timed "Up & Go” Test (TUG; time, seconds). The Bland-Altman limit of agreement measures, standard error of measurement (SEM), coefficient of variation (CV), minimal detectable changes (MDCs), and bias (mean difference) were employed to analyse the concurrent validity between video-based and face-to-face approaches of the performance-based tests. Reliability was measured using intraclass correlation coefficients (ICC), CV, and SEM. Results A high degree of concurrent validity for the TUG (bias = −0.22), the 30-Second Chair Stand Test (bias = −0.22), the Stair Climb Test (bias = −0.31), and the 40-Meter Fast-Paced Walk Test (bias = −0.06) was found. SEM and CV values were within the acceptable level for concurrent validity. There was a high degree of reliability demonstrated for all tests analysed. ICC measures ranged from 0.95 to 1.00 for intrarater reliability, and from 0.95 to 0.99 for interrater reliability. Conclusions Video-based assessment of physical performance tests is a valid and reliable tool for measuring physical function among adults with knee osteoarthritis via Microsoft Teams (Microsoft Corp, Redmond, WA, USA). Impact Video-based assessment is a promising public health tool to measure physical function in adults with knee osteoarthritis.

The Application of 'timed Up and Go' Test in Fall Screening of Elderly People in Shanghai: A Cross-Sectional Study.

Abstract:Background:We aimed to evaluate the predictive value of the 'Timed Up and Go' test (TUGT) for identifying fall risk in community-dwelling elderly.Methods:From Aug 2016 to Feb 2017, cluster sampling was conducted among residents aged over 60 from 15 communities in Songjiang district, Shanghai. Face-to-face questionnaire interviews and TUGT measures were conducted to collect data.Results:6,014 participants were enrolled, with an average age 72.7±7.0 years. 637 (10.6%) elderly people had a fall experience in the past year. TUGT for the non-fall group, one-fall group and recurrent-fall group was 9.02±4.39, 10.00±5.26 and 10.78±4.51 seconds respectively (P<0.001). ROC analysis showed that the TUGT cut-off point for the elderly was 12.5 seconds and AUC was 0.573 for any-fall group and 0.613 for recurrent-fall group respectively. After adjusting for age and gender, the predictive value was not high for any-fall group (AUC=0.614) and recurrent-fall group (AUC=0.648). The TUGT cut-off point for the elderly aged below 65, 65-74, 75-84 and 85 and above was 13.52s, 12.51s, 12.51s, 12.00s, respectively. After adjustment of the confounding factors, the OR values for the risk of fall for the elderly men and women who completed TUGT longer than the cut-off point was 2.404 and 2.075 times higher than those who completed TUGT shorter than the cut-off point, respectively.Conclusion:TUGT with the cut-off score of 12.5s has limited capability in predicting fall risk in community-dwelling elderly.
Empowering a Gait Feature-Rich Timed-Up-and-Go System for Complex Ecological Environments.

Abstract:Identifying fall risk can prevent injuries in the elderly as well as reduce the related financial burden. A balance assessment, Timed Up and Go (TUG), has been widely applied to estimate fall risk. However, the standardized TUG usually excludes complex factors (e.g. slopes and obstacles) falling short of representing challenges in environments that many older adults navigate. Having information on the motor performance in more complex settings can better inform clinicians about an individual's risk of falling. To this end, we present Smart Insole TUG (SITUG), a cost-efficient, real-time and self-assessment system suitable for the complex environmental TUG. Based on the human stride mechanism, our SITUG educes four refined aspects in the gait feature and segments the TUG process by six detailed phases, providing accurate and advanced information for the fall risk estimation. We evaluate the system with four complex environmental TUG. The results show that the SITUG achieves mean accuracy 94.1% in extracting subcomponents within a stride and 93.13% in deriving the stride length based on the verification of estimated walking distance. Moreover, this system can distinguish six TUG phases with the correctness around 90%.

Abstract:The timed up-and-go (TUG) test has been widely accepted as a standard assessment for measuring the basic functional mobility of patients with Parkinson’s disease. Several basic mobility sub-tasks “Sit,” “Sit-to-Stand,” “Walk,” “Turn,” “Walk-Back,” and “Sit-Back” are included in a TUG test. It has been shown that the time costs of these sub-tasks are useful clinical parameters for the assessment of Parkinson’s disease. Several automatic methods have been proposed to segment and time these sub-tasks in a TUG test. However, these methods usually require either well-controlled environments for the TUG video recording or information from special devices, such as wearable inertial sensors, ambient sensors, or depth cameras. In this paper, an automatic TUG sub-task segmentation method using video-based activity classification is proposed and validated in a study with 24 Parkinson’s disease patients. Videos used in this paper are recorded in semi-controlled environments with various backgrounds. The state-of-the-art deep learning-base 2-D human pose estimation technologies are used for feature extraction. A support vector machine and a long short-term memory network are then used for the activity classification and the subtask segmentation. Our method can be used to automatically acquire clinical parameters for the assessment of Parkinson’s disease using TUG videos-only, leading to the possibility of remote monitoring of the patients’ condition.

Abstract:The aim of the study was to investigate the reliability and validity of the timed up & go (TUG) test and the 30-s sit-to-stand (30-s STS) test performed via tele-assessment in ambulatory patients with stroke. Sixty-one patients with chronic stroke were included. For reliability, test-retest and inter-rater reliability were determined. For validity, the correlation coefficient of the values obtained from the face-to-face and tele-assessment procedures of the TUG and 30s-STS tests carried out by the first rater was used. On the first day, the first rater made TUG and 30s-STS tests face-to-face. On the second day, the TUG and 30-s STS were repeated by the first and second raters using the tele-assessment procedure. The tests were applied as tele-assessment 5–7 days later by the first rater to determine the test–retest reliability of the tele-assessment method. ICC for the TUG and 30-s STS tests were 0.998 and 0.992, respectively. The inter-rater agreement between tele-assessments was good for both the TUG test (k = 0.996) and the 30-s STS test (k = 0.981). There was a very strong correlation between tele-assessment and face-to-face assessment of the TUG ( r = 0.97) and the 30-s STS test ( r = 0.94). The TUG and the 30-s STS tests performed via tele-assessment were reliable and valid for assessing functionality and lower extremity capacity of ambulatory patients with stroke.

Abstract:The ageing of the population poses a threat to both public and private health and social systems. In the last 50 years, life expectancy has increased by an average of 20 years, and by the year 2050, life expectancy will exceed 90 years of age. However, quality of life in the last years of life is not guaranteed due to conditions such as functional decline and frailty, ultimately progressing to disability. Thus, the detection of such a condition in time is of utmost importance. This paper presents an ultrasonic sensor-based gait speed measurement device controlled via a mobile interface, which permits patients to self-assess physical performance. The system was developed and validated in an iterative process involving a total of 28 subjects (21 in the first round and 7 in the second one). After first evaluation at Hospital Universitario de Getafe, some technical problems arose whereas usability testing was well evaluated. The second version addressing the identified issues was technically validated at university premises with good and promising results. Future work envisages deployment of the system developed at subjects’ homes to be remotely and unobtrusively monitored.

Camera-Based Short Physical Performance Battery and Timed Up and Go Assessment for Older Adults With Cancer

Abstract:This paper presents an automatic camera-based device to monitor and evaluate the gait speed, standing balance, and 5 times sit-stand (5TSS) tests of the Short Physical Performance Battery (SPPB) and the Timed Up and Go (TUG) test. The proposed design measures and calculates the parameters of the SPPB tests automatically. The SPPB data can be used for physical performance assessment of older patients under cancer treatment. This stand-alone device has a Raspberry Pi (RPi) computer, three cameras, and two DC motors. The left and right cameras are used for gait speed tests. The center camera is used for standing balance, 5TSS, and TUG tests and for angle positioning of the camera platform toward the subject using DC motors by turning the camera left/right and tilting it up/down. The key algorithm for operating the proposed system is developed using Channel and Spatial Reliability Tracking in the cv2 module in Python. Graphical User Interfaces (GUIs) in the RPi are developed to run tests and adjust cameras, controlled remotely via smartphone and its Wi-Fi hotspot. We have tested the implemented camera setup prototype and extracted all SPPB and TUG parameters by conducting several experiments on a human subject population of 8 volunteers (male and female, light and dark complexions) in 69 test runs. The measured data and calculated outputs of the system consist of tests of gait speed (0.041 to 1.92 m/s with average accuracy of >95%), and standing balance, 5TSS, TUG, all with average time accuracy of >97%.
A Novel Environment-Adaptive Timed Up and Go Test System for Fall Risk Assessment With Wearable Inertial Sensors

Abstract:Objective: Falls are often accompanied by huge social costs, and fall risk assessment is essential to protect the elderly from serious injuries and reduce financial burdens. The standard timed up and go (TUG) balance assessment test focuses on the total walking time and scenarios without environmental changes, which is flawed in providing rich information related to falls and evaluating the gait adaptability in response to environmental changes. Therefore, a fall risk assessment system that relies on a variable environment is actually needed. Methods: We have constructed an environment-adaptive TUG (EATUG) test system with three terrain surfaces (levels/obstacles/stairs). One hundred and three elderly from Shenzhen Luohu Hospital is recruited to participate in the experiment. The wearable inertial sensors attached to the two shanks are used to acquire data, and the gait parameters that may be related to falls are extracted and quantified. Results: Most of the parameters have significant differences between the high-risk group and the low-risk group (e.g., peak power, maximum radius, double support, etc., p < 0.001). In addition, the average sensitivity and specificity of fall risk prediction are 85.7% and 92.9%, while the average accuracy is 9.52% higher than the standard TUG test. Conclusion: The EATUG test system can provide richer gait characteristics and fall-related information, which is a good improvement on the drawbacks of the standard TUG test. Significance: The proposed test system is expected to replace the standard TUG test and be used for fall screening of high-risk elderly in the community to reduce the occurrence of falls.

Abstract:Objectives To evaluate the acceptability, reliability (inter- and intrarater), and validity (convergent, known groups, and predictive) of virtually administered gait speed tests for community-dwelling older adults. Design A prospective cohort study was performed, tracking health outcomes for a year. Setting and Participants The 3-m gait speed test at usual and fast pace was administered to community-dwelling older adults over Zoom. Method To examine acceptability, participants completed questionnaires regarding telehealth usability and experience. Virtual gait speed tests were administered at baseline and 24 to 72 hours later to evaluate reliability. Self-report mobility measures were used to examine convergent and known-groups validity. Participants' health outcomes were tracked for a year to evaluate predictive validity. Results Sixty participants completed the baseline assessment and 52 completed the second assessment. Participants reported an overall positive experience with the test. Intraclass correlation coefficients for reliability ranged from 0.79 to 0.90. For convergent validity, correlations >0.30 were found predominantly for usual gait speed with self-report mobility measures. Both the usual- and fast-gait speed were able to discriminate between difficulty walking and gait aid use. Usual gait speed was able to predict specialist and family doctor visits and fast gait speed was able to predict rehabilitation specialist visits over 1 year. Conclusions and Implications Our findings demonstrate support for the acceptability, reliability, and validity of virtually administered gait speed tests for community-dwelling older adults. Although future studies are needed to examine the validity of virtual gait speed tests in larger and more diverse samples to improve generalizability of results, clinicians and researchers can virtually administer 3-m gait speed tests with confidence that scores are trustworthy and reflect older adults' mobility.

Automation of the Timed Up and Go Test Using a Doppler Radar System for Gait and Balance Analysis in Elderly People

Abstract:The timed up and go (TUG) test is a simple, valid, and reliable clinical tool that is widely used to assess mobility in elderly people. Several research studies have been conducted to automate the TUG test using wearable sensors or motion-tracking systems. Despite their promising results, the adopted technological systems present inconveniences in terms of acceptability and privacy protection. In this work, we propose to overcome these problems by using a Doppler radar system set into the backrest of a chair in order to automate the TUG test and extract additional information from its phases (i.e., transfer, walk, and turn). We intend to segment its phases and extract spatiotemporal gait parameters automatically. Our methodology is mainly based on a multiresolution analysis of radar signals. We proposed a segmentation technique based on the extraction of limbs oscillations signals through a semisupervised machine learning approach, on the one hand, and the application of the DARC algorithm on the other hand. Once the speed signals of torso and limbs oscillations were detected, we suggested estimating 14 gait parameters. All our approaches were validated by comparing outcomes to those obtained from a reference Vicon system. High correlation coefficients were obtained by comparing the speed signals of the torso (ρ=0.8), the speed signals of limbs oscillations (ρ=0.91), the initial and final indices of TUG phases (ρ=0.95), and the extracted parameters (percentage error < 4.8) obtained after radar signal processing to those obtained from the Vicon system.
Comparison on gait characteristics between controlled and free-living conditions in old adults

Abstract:Gait is an important biomarker of functional conditions and gait characteristics can help us assessing health conditions and managing progression of diseases. Most of the existing research study the gait in controlled condition, such as clinical tests. In this paper, we study the gait characteristics in free-living conditions in old adults and compare them with that in controlled conditions, i.e., Timed Up and Go (TUG) test. 65 subjects (12 patients with mobility impairment and 53 healthy controls) are recruited from elderly nursing institutions. The video data are collected from them in TUG test and free-living conditions and the 9 gait characteristics, including gait speed, are extracted from the data. Two-sample tests and independence test based on copula entropy are conducted on the extracted data to compare the characteristics in two conditions. Comparison results show that gait characteristics, such as gait speed, pace, speed variability, etc., in daily life are different from that of in TUG test. In daily life, people tend to have slow gait speed, smaller pace and speed variability, more frequent stride, and smaller acceleration range than in TUG test. We also found that gait speed, pace, and speed variability have stronger dependence with TUG score in the 3 conditions (TUG, daily life, and both) and that other 5 characteristics have stronger dependence with TUG score in both condition than in each condition. The comparison in this study suggests that TUG and daily life conditions are complementary with each other, and that TUG test can be considered as intervention on the movement state of human.
