Abstract:Careau et al. (5) recently published a paper entitled “Energy compensation and adiposity in humans“ in the journal Current Biology. They evaluated measurements of total energy expenditure (TEE), basal energy expenditure (BEE) and activity energy expenditure (AEE) saved in a large data collection. Based on an overall negative correlation between BEE and AEE in a large group of subjects varying in sex, age and body mass they suggest that intense physical activity generally reduces BEE. There was “controlling for sex, age, and body composition“. But surprisingly the authors do not present values per kg body mass despite comparing subjects with a large variation in this quantity. Data published in former publications by one of the coauthors allow calculating daily kcal/kg from means. This indicates that low BEE accompanied by high AEE was caused by a smaller body mass in physically active subjects.

Abstract:BACKGROUND: The true causes of the obesity epidemic are not well understood and there are few longitudinal population-based data published examining this issue. The objective of this analysis was to examine trends in occupational physical activity during the past 5 decades and explore how these trends relate to concurrent changes in body weight in the U.S.METHODOLOGY/PRINCIPAL FINDINGS: Analysis of energy expenditure for occupations in U.S. private industry since 1960 using data from the U.S. Bureau of Labor Statistics. Mean body weight was derived from the U.S. National Health and Nutrition Examination Surveys (NHANES). In the early 1960's almost half the jobs in private industry in the U.S. required at least moderate intensity physical activity whereas now less than 20% demand this level of energy expenditure. Since 1960 the estimated mean daily energy expenditure due to work related physical activity has dropped by more than 100 calories in both women and men. Energy balance model predicted weights based on change in occupation-related daily energy expenditure since 1960 for each NHANES examination period closely matched the actual change in weight for 40-50 year old men and women. For example from 1960-62 to 2003-06 we estimated that the occupation-related daily energy expenditure decreased by 142 calories in men. Given a baseline weight of 76.9 kg in 1960-02, we estimated that a 142 calories reduction would result in an increase in mean weight to 89.7 kg, which closely matched the mean NHANES weight of 91.8 kg in 2003-06. The results were similar for women.CONCLUSION: Over the last 50 years in the U.S. we estimate that daily occupation-related energy expenditure has decreased by more than 100 calories, and this reduction in energy expenditure accounts for a significant portion of the increase in mean U.S. body weights for women and men.

Abstract:An interleaved half‐bridge converter is presented for high input voltage application. The features of the proposed converter are zero voltage switching (ZVS) turn‐on for all active switches, ripple current reduction at output side, load current sharing and load voltage regulation. Two half‐bridge converters connected in series and two split capacitors are used to limit the voltage stress of each power switch at one‐half of input DC bus voltage. Thus, active switches with low voltage stress can be used at high input voltage application. On the other hand, the output sides of two half‐bridge converters are connected in parallel to share the load current and reduce the current stresses of the secondary windings and the rectifier diodes. Since two half‐bridge converters are operated with interleaved pulse‐width modulation (PWM), the output ripple current can partially cancel each other such that the resultant ripple current at output side is reduced and the size of output inductors can be reduced. In each half‐bridge converter, asymmetrical PWM scheme is used to regulate the output voltage. Based on the resonant behavior by the output capacitance of MOSFETs and the leakage inductance (or external inductance) of transformers, active switches can be turned on at ZVS during the transition interval. Thus, the switching losses of power MOSFETs are reduced. The proposed converter can be applied for high input voltage applications such as three‐phase 380‐V utility system. Finally, experiments based on a laboratory prototype with 960‐W rated power are provided to demonstrate the performance of proposed converter. Copyright © 2012 John Wiley & Sons, Ltd.

Abstract:Obesity interventions can result in weight loss, but accurate prediction of the bodyweight time course requires properly accounting for dynamic energy imbalances. In this report, we describe a mathematical modelling approach to adult human metabolism that simulates energy expenditure adaptations during weight loss. We also present a web-based simulator for prediction of weight change dynamics. We show that the bodyweight response to a change of energy intake is slow, with half times of about 1 year. Furthermore, adults with greater adiposity have a larger expected weight loss for the same change of energy intake, and to reach their steady-state weight will take longer than it would for those with less initial body fat. Using a population-averaged model, we calculated the energy-balance dynamics corresponding to the development of the US adult obesity epidemic. A small persistent average daily energy imbalance gap between intake and expenditure of about 30 kJ per day underlies the observed average weight gain. However, energy intake must have risen to keep pace with increased expenditure associated with increased weight. The average increase of energy intake needed to sustain the increased weight (the maintenance energy gap) has amounted to about 0·9 MJ per day and quantifies the public health challenge to reverse the obesity epidemic.

Abstract:Background In 2010, overweight and obesity were estimated to cause 3.4 million deaths, 3.9% of years of life lost, and 3.8% of disability-adjusted life-years (DALYs) worldwide. The rise in obesity has led to widespread calls for regular monitoring of changes in overweight and obesity prevalence in all populations. Comparable, up-to-date information about levels and trends is essential to quantify population health effects and to prompt decision makers to prioritise action. We estimate the global, regional, and national prevalence of overweight and obesity in children and adults during 1980-2013.Methods We systematically identified surveys, reports, and published studies (n = 1769) that included data for height and weight, both through physical measurements and self-reports. We used mixed effects linear regression to correct for bias in self-reports. We obtained data for prevalence of obesity and overweight by age, sex, country, and year (n = 19 244) with a spatiotemporal Gaussian process regression model to estimate prevalence with 95% uncertainty intervals (UIs).Findings Worldwide, the proportion of adults with a body-mass index (BMI) of 25 kg/m(2) or greater increased between 1980 and 2013 from 28.8% (95% UI 28.4-29.3) to 36.9% (36.3-37.4) in men, and from 29.8% (29.3-30.2) to 38.0% (37.5-38.5) in women. Prevalence has increased substantially in children and adolescents in developed countries; 23.8% (22.9-24.7) of boys and 22.6% (21.7-23.6) of girls were overweight or obese in 2013. The prevalence of overweight and obesity has also increased in children and adolescents in developing countries, from 8.1% (7.7-86) to 12.9% (12.3-13.5) in 2013 for boys and from 8.4% (8.1-8.8) to 13.4% (13.0-13.9) in girls. In adults, estimated prevalence of obesity exceeded 50% in men in Tonga and in women in Kuwait, Kiribati, Federated States of Micronesia, Libya, Qatar, Tonga, and Samoa. Since 2006, the increase in adult obesity in developed countries has slowed down.Interpretation Because of the established health risks and substantial increases in prevalence, obesity has become a major global health challenge. Not only is obesity increasing, but no national success stories have been reported in the past 33 years. Energy expenditure of physical activity among adolescents aged 7-14 years

Abstract:Objective To determine energy expenditure(EE) of physical activity among adolescents,and to compare differences of energy metabolism between obese and normal-weight adolescents.Methods Resting and physical activity(PA)(doing homework,walking and jogging) related energy expenditure was measured with Cortex portable gas metabolism system in 61 adolescents aged 7 to 14 years.Results Resting energy expenditure(REE) of normal-weight boys and girls,obese boys and girls were(1.29±0.22) kcal/min,(1.23±0.14) kcal/min(1 kcal=4.18 kJ),(1.56±0.32) kcal/min and(1.26±0.24) kcal/min,respectively.REE was significantly higher in obese boys than in normal-weight boys(P<0.05).PA related EE in obese boys were significantly higher than those in normal-weight boys,while there were no significant differences between obese and normal-weight girls.EE of doing homework was approximately equal to REE,which could be classified as low intensity activity.EE of walking were about 2 to 3 times as high as REE,which could be classified as low intensity activity for normal-weight group but moderate intensity activity for obese group.Comparatively,jogging consumed more energy and could be classified as moderate intensity activity.Conclusion There are differences in EE of same PA between obese and normal-weight adolescents.Reducing homework related sedentary time and more moderate-to-high intensity PA should be encouraged for obesity prevention among adolescents.
Temporal Patterns of Energy Intake and Cognitive Function and Its Decline: a Community-Based Cohort Study in China

Abstract:Temporal pattern of energy intake (TPEI) has been related to multiple cardiometabolic diseases and mortality. However, its association with cognitive function remains poorly understood. We aimed to assess the prospective association of the TPEI with cognitive function and its decline in a community-based cohort study in China. The study included data from 3342 participants aged over 55 years old from the China Health and Nutrition Study (CHNS). Dietary data were collected using 3-day 24-hour dietary recalls at study baseline in 1997 and every 2–4 years thereafter. TPEI was identified using the k-means algorithm. Cognitive function was assessed up to four times using the Telephone Interview for Cognitive Status-modified (TICS-m) from 1997–2006. Linear mixed models were used to estimate the association of TPEI with the level of cognitive function and its decline in subsequent years. We identified six TPEIs: breakfast-dominant, lunch-dominant, dinner-dominant, evenly-distributed, breakfast-skipping, and snack-rich patterns. Compared with the evenly-distributed temporal pattern, other patterns were all related to lower cognitive function scores, with differences being −0.94 (95%CI: −1.37, −0.51, points) for breakfast-dominant pattern, −1.18 (95%CI: −1.67, −0.69, points) for lunch-dominant pattern, −0.97 (95%CI: −1.43, −0.51, points) for dinner-dominant pattern, −1.05 (95%CI: −1.70, −0.40, points) for snack-rich pattern, and −1.32 (95%CI: −1.66, −0.99, points) for breakfast-skipping pattern (P < 0.05). Particularly, the breakfast-skipping pattern was also associated with a significantly faster cognitive decline (−0.40 [−0.48, −0.32] point/year) compared with the slowest cognitive decline (−0.26 [−0.31, −0.21] point/year) in the evenly-distributed pattern. Maintaining an evenly-distributed temporal pattern of energy intake was associated with better cognitive function and slower cognitive decline in later life, underscoring the importance of managing and balancing energy intake across major meals throughout the day. Abstract:The worldwide occurrence of overweight and obesity is steadily increasing, with over 2 billion individuals currently categorized as having excess body weight. This represents roughly 30% of the global population. [ 1 ] Obesity and its associated disorders have emerged as significant health issues on a global scale. [ 2 ] Obesity poses a significant health problem as it greatly raises the likelihood of developing diseases like type 2 diabetes mellitus, fatty liver disease, hypertension, coronary artery disease, dementia, osteoarthritis, obstructive sleep apnea, and various cancers. Consequently, it leads to a decrease in both quality of life and life expectancy. [ 3 ] Obesity not only contributes to morbidity but also imposes significant economic burdens on society due to the chronic diseases associated with it. [ 4 ] The primary contributors to obesity are excessive and unhealthy eating habits, coupled with physical inactivity. [ 5 ] Dietary management is a crucial component of weight control, aiming to achieve a negative energy balance at a fundamental level. [ 6 ] The total daily energy demands consist of 3 components: basal metabolic rate (BMR), energy expended for the thermic effect of meals, and energy due to physical activity. [ 7 ] BMR represents the amount of energy needed on a daily basis to sustain fundamental biological activities. [ 8 ] It represents the primary source of energy expenditure and serves as a crucial factor in estimating daily energy needs. [ 9 , 10 ] A review of the literature suggests that accurately predicting BMR is crucial for determining energy requirements and designing effective weight management strategies, particularly in individuals with severe obesity. [ 11–13 ] BMR fluctuates based on factors such as age, gender, weight, height, and is intricately linked to an individual's body composition. [ 14 ] The measurement of BMR can be conducted using either direct or indirect calorimetric techniques. Direct calorimetry directly quantifies heat output, whereas indirect calorimetry (IC) quantifies oxygen intake and carbon dioxide production, which are subsequently translated into their respective energy equivalents. Additionally, predictive equations can also be used to estimate BMR in adults. [ 10 ]

Non-exercise activity thermogenesis: the crouching tiger hidden dragon of societal weight gain

Abstract:Non-exercise activity thermogenesis (NEAT) is the energy expenditure of all physical activities other than volitional sporting-like exercise. NEAT includes all the activities that render us vibrant, unique, and independent beings such as working, playing, and dancing. Because people of the same weight have markedly variable activity levels, it is not surprising that NEAT varies substantially between people by up to 2000 kcal per day. Evidence suggests that low NEAT may occur in obesity but in a very specific fashion. Obese individuals appear to exhibit an innate tendency to be seated for 2.5 hours per day more than sedentary lean counterparts. If obese individuals were to adopt the lean "NEAT-o-type," they could potentially expend an additional 350 kcal per day. Obesity was rare a century ago and the human genotype has not changed over that time. Thus, the obesity epidemic may reflect the emergence of a chair-enticing environment to which those with an innate tendency to sit, did so, and became obese. To reverse obesity, we need to develop individual strategies to promote standing and ambulating time by 2.5 hours per day and also re-engineer our work, school, and home environments to render active living the option of choice.
Lower core body temperature and greater body fat are components of a human thrifty phenotype

Abstract:Background/Objectives:In small studies, a thrifty human phenotype, defined by a greater 24-hour energy expenditure (EE) decrease with fasting, is associated with less weight loss during caloric restriction. In rodents, models of diet-induced obesity often have a phenotype including a reduced EE and decreased core body temperature. We assessed whether a thrifty human phenotype associates with differences in core body temperature or body composition.Subjects/Methods:Data for this cross-sectional analysis were obtained from 77 individuals participating in one of two normal physiology studies while housed on our clinical research unit. Twenty-four-hour EE using a whole-room indirect calorimeter and 24-h core body temperature were measured during 24 h each of fasting and 200% overfeeding with a diet consisting of 50% carbohydrates, 20% protein and 30% fat. Body composition was measured by dual X-ray absorptiometry. To account for the effects of body size on EE, changes in EE were expressed as a percentage change from 24-hour EE (%EE) during energy balance.Results:A greater %EE decrease with fasting correlated with a smaller %EE increase with overfeeding (r=0.27, P=0.02). The %EE decrease with fasting was associated with both fat mass and abdominal fat mass, even after accounting for covariates (β=−0.16 (95% CI: −0.26, −0.06) %EE per kg fat mass, P=0.003; β=−0.0004 (−0.0007, −0.00004) %EE kg−1 abdominal fat mass, P=0.03). In men, a greater %EE decrease in response to fasting was associated with a lower 24- h core body temperature, even after adjusting for covariates (β=1.43 (0.72, 2.15) %EE per 0.1 °C, P=0.0003).Conclusions:Thrifty individuals, as defined by a larger EE decrease with fasting, were more likely to have greater overall and abdominal adiposity as well as lower core body temperature consistent with a more efficient metabolism.
Physical Activity and Sedentary Behavior in Metabolically Healthy versus Unhealthy Obese and Non-Obese Individuals – The Maastricht Study

Abstract:BACKGROUND: Both obesity and the metabolic syndrome are associated with increased risk of cardiovascular diseases and type 2 diabetes. Although both frequently occur together in the same individual, obesity and the metabolic syndrome can also develop independently from each other. The (patho)physiology of "metabolically healthy obese" (i.e. obese without metabolic syndrome) and "metabolically unhealthy non-obese" phenotypes (i.e. non-obese with metabolic syndrome) is not fully understood, but physical activity and sedentary behavior may play a role.OBJECTIVE: To examine objectively measured physical activity and sedentary behavior across four groups: I) "metabolically healthy obese" (MHO); II) "metabolically unhealthy obese" (MUO); III)"metabolically healthy non-obese" (MHNO); and IV) "metabolically unhealthy non-obese" (MUNO).METHODS: Data were available from 2,449 men and women aged 40-75 years who participated in The Maastricht Study from 2010 to 2013. Participants were classified into the four groups according to obesity (BMI≥30kg/m2) and metabolic syndrome (ATPIII definition). Daily activity was measured for 7 days with the activPAL physical activity monitor and classified as time spent sitting, standing, and stepping.RESULTS: In our study population, 562 individuals were obese. 19.4% of the obese individuals and 72.7% of the non-obese individuals was metabolically healthy. After adjustments for age, sex, educational level, smoking, alcohol use, waking time, T2DM, history of CVD and mobility limitation, MHO (n = 107) spent, per day, more time stepping (118.2 versus 105.2 min; p<0.01) and less time sedentary (563.5 versus 593.0 min., p = 0.02) than MUO (n = 440). In parallel, MHNO (n = 1384) spent more time stepping (125.0 versus 115.4 min; p<0.01) and less time sedentary (553.3 versus 576.6 min., p<0.01) than MUNO (n = 518).CONCLUSION: Overall, the metabolically healthy groups were less sedentary and more physically active than the metabolically unhealthy groups. Therefore, physical activity and sedentary time may partly explain the presence of the metabolic syndrome in obese as well as non-obese individuals.

Can Regular Physical Activity Prevent Obesity ? Part 2 : Empirical cross-sectional evidence of efficacy

Abstract:Objective: The objective of this narrative review is to examine cross-sectional empirical data suggesting the efficacy of adequate daily physical activity in the prevention of obesity. Methods: Information obtained from Ovid/Medline and Google Scholar through to March 2019 was supplemented by a search of the author's extensive personal files. Results: Average skin-fold readings are often as low as 5 mm, in male elite longdistance competitors who spend many hours per week in training, and values for female endurance competitors are also often only a half of those found in the general female population. Physical education (PE) students (particularly men) are somewhat thinner than their peers in other faculties, but this advantage is attenuated in averaged data because a proportion of PE students have a lesser commitment to an active lifestyle, and also some students in other faculties engage in physically demanding inter-collegiate sports. Early occupational studies showed low body fat levels in physically active occupations, and a similar difference can still be observed in elite military groups where high daily energy expenditures are required. Amish and Old-order Mennonite families who eschew modern mechanical devices have very high levels of daily physical activity, and this is reflected in their correspondingly low levels of body fat. Similar findings have been reported for Inuit who still follow a hunter-gatherer lifestyle. Studies validating questionnaires and accelerometers in the general population have show correlations of up to -0.50 between scores on such instruments and estimates of body fat content. Finally, cross-sectional comparisons within circumpolar communities have demonstrated higher levels of aerobic fitness in those populations with the least acculturation to a sedentary "white" lifestyle, and this sub-group of circumpolar residents also has a low body fat content. Conclusions: Conclusions about the impact of vigorous daily physical activity upon body fat content are limited by the possible self-selection of a physically active lifestyle, with the adoption of other favorable health behaviours by active individuals. Nevertheless, all of numerous population comparisons show lower levels of body fat in those groups who maintain an adequate level of habitual physical activity. In some cases, the levels of physical activity adopted have been very high, but there are also examples of populations where a favourable body composition has been associated with the regular performance of volumes of physical activity that are appropriate for average middle-aged adults. Abstract:Background: Energy requirement assessment is a cornerstone for nutrition practice. The extent to which total energy expenditure (TEE; indicator of energy requirements) has been measured in adults with chronic diseases has not been explored. Objective: This systematic review aimed to: 1) Characterize evidence on TEE among individuals with chronic diseases, and 2) Describe TEE across chronic diseases and compared to controls without a chronic disease. Methods: A literature search using terms related to doubly labeled water and TEE was conducted in PubMed, MEDLINE, Web of Science, and Embase. Eligible articles included those that measured TEE using doubly labeled water in adults with a major chronic disease. Methodological quality was determined using the Academy of Nutrition and Dietetics Quality Criteria Checklist. Sample size-weighted TEE was calculated in each chronic disease subgroup. Results: Fifty studies were included, of which 15 had a control group. Median sample size was 20 participants, and approximately half of studies were published over 10 years ago. Thirty-five (70%) studies reported resting energy expenditure, and about half (k=26) reported physical activity level. Methodological quality was 'neutral' (k=25) or 'positive' (k=23) for most studies. TEE among individual studies ranged from 934 to 3274 kcal/day. Mean weighted TEE was lowest among gastrointestinal (1786 kcal/day) and neurological (2104 kcal/day) subgroups and highest among cancer (2903 kcal/day), endocrine (2661 kcal/day), and autoimmune (2625 kcal/day) subgroups. Excluding one article in cancer survivors resulted in a low TEE in the cancer subgroup (2112 kcal/day). Most studies with a control group reported no differences in TEE between controls and patients; however, only one study was powered for between-group comparisons. Conclusion: Energy requirements vary across chronic diseases, although there is insufficient evidence to suggest that TEE is different than controls. Further research is needed to inform energy requirement recommendations that consider chronic disease.

Abstract:Objective This work aimed to parse out the role of changing environments on body composition, total energy expenditure, and physical activity in the Mexican Pima, a population experiencing rapid industrialization. Methods Using doubly labeled water, we compared energy expenditure and physical activity in a longitudinal cohort of Mexican Pima (n = 26; female: 12) in 1995 and 2010. Body mass and composition were assessed by bioimpedance analysis. To determine the effects of environmental factors on body weight independent of age, we compared the 1995 longitudinal cohort with an age‐ and sex‐matched cross‐sectional cohort (n = 26) in 2010. Results Body mass, fat mass, and fat‐free mass all significantly increased between 1995 and 2010. Despite a 13% average increase in body weight, weight‐adjusted total daily energy expenditure decreased significantly. Measured physical activity levels also decreased between 1995 and 2010, after we adjusted for weight. Conclusions Our results suggest that the recent industrialization of the Maycoba region in Sonora, Mexico, has contributed to a decrease in physical activity, in turn contributing to weight gain and metabolic disease among the Mexican Pima.

Modulators of Energy Expenditure Accuracy in Adults with Overweight or Obesity: E-MECHANIC Secondary Analyses

Abstract:Purpose: American College of Sports Medicine (ACSM) metabolic equations are used to estimate energy expenditure (EE) of physical activity and prescribe aerobic exercise to meet EE requirements. Limited evidence supports their accuracy in sedentary adults with overweight or obesity during controlled exercise interventions. The purpose of this study was to compare EE estimated by the ACSM walking equation versus EE measured by indirect calorimetry during a 24-week aerobic exercise intervention, and identify potential modulators for their accuracy. Methods: Data from the exercising groups (8 or 20 kcal·kg body weight-1·week-1) of the E-MECHANIC study were utilized in this ancillary analysis (N = 103). Every 2 weeks for the initial 8 weeks and monthly thereafter, EE was measured via indirect calorimetry during absolute (2 mph, 0% grade) and relative (65-85% VO2peak) workload exercise. Resting metabolic rate, VO2peak, and body composition were assessed at baseline and follow-up. An EE offset factor (EOF) was calculated to express measured EE as a percentage of the estimated EE at each workload (EOF < 100% represents an overestimation of ACSM estimated EE). Results: The accuracy of the equation decreased with increasing exercise workload (0.44%, 9.2%, and 20.3% overestimation at absolute, relative, and maximal workloads, respectively, at baseline) and overestimation of EE was greater after the exercise intervention. Furthermore, race, sex, age, fat mass, and VO2peak were identified as modulators for equation accuracy. Greater overestimation of EE was observed in Black compared to white females, particularly at lower exercise workloads. Conclusions: These findings support future efforts to improve the accuracy of metabolic equations, especially in diverse populations. Researchers should account for exercise efficiency adaptations when using metabolic equations to prescribe exercise precisely.

Abstract:AIM: To determine whether basal metabolic rate (BMR) was causally related to MetS, and to study the role of gender in this relationship.METHODS: Seventy-two Caucasian subjects (43 women, 29 men) had changes in basal metabolic rate (BMR), carbohydrate oxidation rate (COR), fat oxidation rate (FOR) and prevalence of the metabolic syndrome (MetS) assessed in response to weight loss.RESULTS: There was a significant gender×MetS interaction in BMR at the start. Women with MetS had higher adjusted BMR, whilst men with MetS had lower adjusted BMR than their respective counterparts. Weight loss resulted in a significant decrease in fat mass (-5.2±0.31 kg, p=0.001), fat free mass (-2.3±0.27 kg, p=0.001), BMR (-549±58 kJ/d, p=0.001) and a decreased proportion of MetS (22/72, χ(2)=0.005). Subjects who recovered from MetS after weight loss (RMS) had ∼250 kJ/d significantly lower adjusted BMR compared to those who were never MetS (NMS, p=0.046) and those who still had MetS (MetS+, p=0.047). Regression analysis showed that change (Δ) in BMR was best determined by Δglucose×gender interaction (r(2)=23%), ΔFOR (r(2)=20.3%), ΔCOR (r(2)=19.4%) and Δtriglycerides (r(2)=7.8%).CONCLUSIONS: There is a sexual dimorphism of BMR in MetS. Overall, the data support the notion that alterations in BMR may be central to the etiopathogenesis of MetS.
