Abstract:An interleaved half‐bridge converter is presented for high input voltage application. The features of the proposed converter are zero voltage switching (ZVS) turn‐on for all active switches, ripple current reduction at output side, load current sharing and load voltage regulation. Two half‐bridge converters connected in series and two split capacitors are used to limit the voltage stress of each power switch at one‐half of input DC bus voltage. Thus, active switches with low voltage stress can be used at high input voltage application. On the other hand, the output sides of two half‐bridge converters are connected in parallel to share the load current and reduce the current stresses of the secondary windings and the rectifier diodes. Since two half‐bridge converters are operated with interleaved pulse‐width modulation (PWM), the output ripple current can partially cancel each other such that the resultant ripple current at output side is reduced and the size of output inductors can be reduced. In each half‐bridge converter, asymmetrical PWM scheme is used to regulate the output voltage. Based on the resonant behavior by the output capacitance of MOSFETs and the leakage inductance (or external inductance) of transformers, active switches can be turned on at ZVS during the transition interval. Thus, the switching losses of power MOSFETs are reduced. The proposed converter can be applied for high input voltage applications such as three‐phase 380‐V utility system. Finally, experiments based on a laboratory prototype with 960‐W rated power are provided to demonstrate the performance of proposed converter. Copyright © 2012 John Wiley & Sons, Ltd.
Total energy expenditure measured using doubly labeled water in adults with major chronic diseases: A systematic review

Abstract:Background: Energy requirement assessment is a cornerstone for nutrition practice. The extent to which total energy expenditure (TEE; indicator of energy requirements) has been measured in adults with chronic diseases has not been explored. Objective: This systematic review aimed to: 1) Characterize evidence on TEE among individuals with chronic diseases, and 2) Describe TEE across chronic diseases and compared to controls without a chronic disease. Methods: A literature search using terms related to doubly labeled water and TEE was conducted in PubMed, MEDLINE, Web of Science, and Embase. Eligible articles included those that measured TEE using doubly labeled water in adults with a major chronic disease. Methodological quality was determined using the Academy of Nutrition and Dietetics Quality Criteria Checklist. Sample size-weighted TEE was calculated in each chronic disease subgroup. Results: Fifty studies were included, of which 15 had a control group. Median sample size was 20 participants, and approximately half of studies were published over 10 years ago. Thirty-five (70%) studies reported resting energy expenditure, and about half (k=26) reported physical activity level. Methodological quality was 'neutral' (k=25) or 'positive' (k=23) for most studies. TEE among individual studies ranged from 934 to 3274 kcal/day. Mean weighted TEE was lowest among gastrointestinal (1786 kcal/day) and neurological (2104 kcal/day) subgroups and highest among cancer (2903 kcal/day), endocrine (2661 kcal/day), and autoimmune (2625 kcal/day) subgroups. Excluding one article in cancer survivors resulted in a low TEE in the cancer subgroup (2112 kcal/day). Most studies with a control group reported no differences in TEE between controls and patients; however, only one study was powered for between-group comparisons. Conclusion: Energy requirements vary across chronic diseases, although there is insufficient evidence to suggest that TEE is different than controls. Further research is needed to inform energy requirement recommendations that consider chronic disease.

Abstract:Temporal pattern of energy intake (TPEI) has been related to multiple cardiometabolic diseases and mortality. However, its association with cognitive function remains poorly understood. We aimed to assess the prospective association of the TPEI with cognitive function and its decline in a community-based cohort study in China. The study included data from 3342 participants aged over 55 years old from the China Health and Nutrition Study (CHNS). Dietary data were collected using 3-day 24-hour dietary recalls at study baseline in 1997 and every 2–4 years thereafter. TPEI was identified using the k-means algorithm. Cognitive function was assessed up to four times using the Telephone Interview for Cognitive Status-modified (TICS-m) from 1997–2006. Linear mixed models were used to estimate the association of TPEI with the level of cognitive function and its decline in subsequent years. We identified six TPEIs: breakfast-dominant, lunch-dominant, dinner-dominant, evenly-distributed, breakfast-skipping, and snack-rich patterns. Compared with the evenly-distributed temporal pattern, other patterns were all related to lower cognitive function scores, with differences being −0.94 (95%CI: −1.37, −0.51, points) for breakfast-dominant pattern, −1.18 (95%CI: −1.67, −0.69, points) for lunch-dominant pattern, −0.97 (95%CI: −1.43, −0.51, points) for dinner-dominant pattern, −1.05 (95%CI: −1.70, −0.40, points) for snack-rich pattern, and −1.32 (95%CI: −1.66, −0.99, points) for breakfast-skipping pattern (P < 0.05). Particularly, the breakfast-skipping pattern was also associated with a significantly faster cognitive decline (−0.40 [−0.48, −0.32] point/year) compared with the slowest cognitive decline (−0.26 [−0.31, −0.21] point/year) in the evenly-distributed pattern. Maintaining an evenly-distributed temporal pattern of energy intake was associated with better cognitive function and slower cognitive decline in later life, underscoring the importance of managing and balancing energy intake across major meals throughout the day. Abstract:Objective: To validate a diet history (DH).Design: Energy intake (EI) estimated by a diet history (DH) was validated against total energy expenditure (TEE) measured by doubly labeled water (DLW) (n=12) used as reference, by heart rate monitoring (HR) and by an activity diary (AD).Setting: Department of Geriatric Medicine, Göteborg University, Vasa Hospital, Gothenburg, Sweden.Subjects: 20 healthy free-living elderly subjects (73 years) from the gerontological and geriatric population studies, Gothenburg, Sweden (H70).Results: Mean value for EI was 9.02 (s.d. 2.30), for TEE by HR was 9.66 (s.d. 2.25) MJ/d, and for TEE by AD was 9.40 (s.d. 2.08) MJ/d. In the 12 individuals measured with DLW, EI was 8.62 (s.d. 2.06), TEE by DLW was 9.90 (s.d. 1.43) MJ/d, TEE by HR was 8.94 (s.d. 1.96) MJ/d, and TEE by AD was 9.24 (s.d. 2.15) MJ/d. Mean difference between DH and DLW was 1.28 (s.d. 2.17) MJ (NS) and the DH/DLW ratio was 0.88. Four subjects were identified as under-reporters and one as an over-reporter.Conclusion: The DH slightly underestimated EI compared to the HR, but was in concordance with the AD. Compared to DLW, DH underestimated EI by 12%. On group level, the DH method gave comparable values to HR and AD. The DH was not valid for ranking of individuals. Universal Relation for Life-span Energy Consumption in Living Organisms: Insights for the origin of ageing

Abstract:Metabolic energy consumption has long been thought to play a major role in the aging process ({\it 1}). Across species, a gram of tissue on average expends about the same amount of energy during life-span ({\it 2}). Energy restriction has also been shown that increases maximum life-span ({\it 3}) and retards age-associated changes ({\it 4}). However, there are significant exceptions to a universal energy consumption during life-span, mainly coming from the inter-class comparison ({\it 5, 6}). Here we present a unique relation for life-span energy consumption, valid for $\sim$300 species representing all classes of living organisms, from unicellular ones to the largest mammals. The relation has an average scatter of only 0.3 dex, with 95\% ($\rm 2-\sigma$) of the organisms having departures less than a factor of $\pi$ from the relation, despite the $\sim$20 orders of magnitude difference in body mass, reducing any possible inter-class variation in the relation to only a geometrical factor. This result can be interpreted as supporting evidence for the existence of an approximately constant total number $\rm N_r \sim 10^8$ of respiration cycles per lifetime for all organisms, effectively predetermining the extension of life by the basic energetics of respiration, being an incentive for future studies that investigate the relation of such constant $\rm N_r$ cycles per lifetime with the production rates of free radicals and oxidants, which may give definite constraints on the origin of ageing.

Abstract:Purpose To compare the degree to which four accelerometer metrics—total activity counts per day (TAC per day), steps per day (steps per day), physical activity energy expenditure (PAEE) (kcal·kg−1·d−1), and moderate- to vigorous-intensity physical activity (MVPA) (min·d−1)—were correlated with PAEE measured by doubly labeled water (DLW). Additionally, accelerometer metrics based on vertical axis counts and triaxial counts were compared. Methods This analysis included 684 women and 611 men age 43 to 83 yr. Participants wore the Actigraph GT3X on the hip for 7 d twice during the study and the average of the two measurements was used. Each participant also completed one DLW measurement, with a subset having a repeat. PAEE was estimated by subtracting resting metabolic rate and the thermic effect of food from total daily energy expenditure estimated by DLW. Partial Spearman correlations were used to estimate associations between PAEE and each accelerometer metric. Results Correlations between the accelerometer metrics and DLW-determined PAEE were higher for triaxial counts than vertical axis counts. After adjusting for weight, age, accelerometer wear time, and fat free mass, the correlation between TAC per day based on triaxial counts and DLW-determined PAEE was 0.44 in women and 0.41 in men. Correlations for steps per day and accelerometer-estimated PAEE with DLW-determined PAEE were similar. After adjustment for within-person variation in DLW-determined PAEE, the correlations for TAC per day increased to 0.61 and 0.49, respectively. Correlations between MVPA and DLW-determined PAEE were lower, particularly for modified bouts of ≥10 min. Conclusions Accelerometer measures that represent total activity volume, including TAC per day, steps per day, and PAEE, were more highly correlated with DLW-determined PAEE than MVPA using traditional thresholds and should be considered by researchers seeking to reduce accelerometer data to a single metric.

Abstract:OBJECTIVE: Malnutrition and obesity are complex burdensome challenges in pediatric chronic kidney disease (CKD) management that can adversely affect growth, disease progression, wellbeing, and response to treatment. Total energy expenditure (TEE) and energy requirements in children are essential for growth outcomes but are poorly defined, leaving clinical practice varied and insecure. The aims of this study were to explore a practical approach to guide prescribed nutritional interventions, using measurements of TEE, physical activity energy expenditure (PAEE), and their relationship to kidney function.DESIGN AND METHODS: In a cross-sectional prospective age-matched and sex-matched controlled study, 18 children with CKD (6-17 years, mean stage 3) and 20 healthy, age-matched, and gender-matched controls were studied. TEE and PAEE were measured using basal metabolic rate (BMR), activity diaries and doubly labeled water (healthy subjects). Results were related to estimated glomerular filtration rate (eGFR). The main outcome measure was TEE measured by different methods (factorial, doubly labeled water, and a novel device).RESULTS: Total energy expenditure and PAEE with or without adjustments for age, gender, weight, and height did not differ between the groups and was not related to eGFR. TEE ranged from 1927 ± 91 to 2330 ± 73 kcal/d; 95 ± 5 to 109 ± 5% estimated average requirement (EAR), physical activity level (PAL) 1.52 ± 0.01 to 1.71 ± 0.17, and PAEE 24 to 34% EAR. Comparisons between DLW and alternative methods in healthy children did not differ significantly, except for 2 (factorial methods and a fixed PAL; and the novel device).CONCLUSION: In clinical practice, structured approaches using supportive evidence (weight, height, BMI sds), predictive BMR or TEE values and simple questions on activity, are sufficient for most children with CKD as a starting energy prescription.

Abstract:Obesity interventions can result in weight loss, but accurate prediction of the bodyweight time course requires properly accounting for dynamic energy imbalances. In this report, we describe a mathematical modelling approach to adult human metabolism that simulates energy expenditure adaptations during weight loss. We also present a web-based simulator for prediction of weight change dynamics. We show that the bodyweight response to a change of energy intake is slow, with half times of about 1 year. Furthermore, adults with greater adiposity have a larger expected weight loss for the same change of energy intake, and to reach their steady-state weight will take longer than it would for those with less initial body fat. Using a population-averaged model, we calculated the energy-balance dynamics corresponding to the development of the US adult obesity epidemic. A small persistent average daily energy imbalance gap between intake and expenditure of about 30 kJ per day underlies the observed average weight gain. However, energy intake must have risen to keep pace with increased expenditure associated with increased weight. The average increase of energy intake needed to sustain the increased weight (the maintenance energy gap) has amounted to about 0·9 MJ per day and quantifies the public health challenge to reverse the obesity epidemic.

Abstract:The estimation of total daily energy expenditure (TEE) or the energy needs of athletes is important for maintaining a proper energy balance during training. This study was conducted to measure the TEE of Japanese middle- and long-distance runners using a heart-rate monitoring method (the HR method), and to ascertain the impact of three key energy expenditure (EE) components occurring within the day: sleep, exercise (training) and inactivity (other daily habitual activities). Additionally, energy balance was evaluated by comparing TEE with energy intake (EI). The mean TEE of six male athletes aged 19-21 y was 4,514+/-739 kcal/d and their mean EI was 3,784+/-91 kcal/d during the training period of 11 d. This negative energy balance led to a significant decline in body weight (p<0.01), indicating that proper care should be taken to balance energy during training. The mean daily EEs during sleep, inactivity, and exercise were 536+/-46 (12.0% of TEE), 1,722+/-145 (38.7%), and 2,255+/-658 (49.3%) kcal/d, respectively. There was a significant correlation between TEE and exercise EE (p<0.01) in the athletes. The TEE of the endurance athletes was higher than expected and significantly affected by exercise EE, indicating that the TEE of athletes, in general, must be carefully evaluated before incorporating nutritional support for them.

Abstract:Abstract Objective To evaluate the associations between adiposity measures (body mass index, waist circumference, and waist-to-height ratio) with decline in glomerular filtration rate (GFR) and with all cause mortality. Design Individual participant data meta-analysis. Setting Cohorts from 40 countries with data collected between 1970 and 2017. Participants Adults in 39 general population cohorts (n=5 459 014), of which 21 (n=594 496) had data on waist circumference; six cohorts with high cardiovascular risk (n=84 417); and 18 cohorts with chronic kidney disease (n=91 607). Main outcome measures GFR decline (estimated GFR decline ≥40%, initiation of kidney replacement therapy or estimated GFR <10 mL/min/1.73 m2) and all cause mortality. Results Over a mean follow-up of eight years, 246 607 (5.6%) individuals in the general population cohorts had GFR decline (18 118 (0.4%) end stage kidney disease events) and 782 329 (14.7%) died. Adjusting for age, sex, race, and current smoking, the hazard ratios for GFR decline comparing body mass indices 30, 35, and 40 with body mass index 25 were 1.18 (95% confidence interval 1.09 to 1.27), 1.69 (1.51 to 1.89), and 2.02 (1.80 to 2.27), respectively. Results were similar in all subgroups of estimated GFR. Associations weakened after adjustment for additional comorbidities, with respective hazard ratios of 1.03 (0.95 to 1.11), 1.28 (1.14 to 1.44), and 1.46 (1.28 to 1.67). The association between body mass index and death was J shaped, with the lowest risk at body mass index of 25. In the cohorts with high cardiovascular risk and chronic kidney disease (mean follow-up of six and four years, respectively), risk associations between higher body mass index and GFR decline were weaker than in the general population, and the association between body mass index and death was also J shaped, with the lowest risk between body mass index 25 and 30. In all cohort types, associations between higher waist circumference and higher waist-to-height ratio with GFR decline were similar to that of body mass index; however, increased risk of death was not associated with lower waist circumference or waist-to-height ratio, as was seen with body mass index. Conclusions Elevated body mass index, waist circumference, and waist-to-height ratio are independent risk factors for GFR decline and death in individuals who have normal or reduced levels of estimated GFR.
Total energy expenditure as assessed by doubly labeled water and all-cause mortality in a cohort of postmenopausal women

Abstract:Background: The association of TEE with all-cause mortality is uncertain, as is the dependence of this association on age. Objectives: To examine the association between TEE and all-cause mortality, and its age interaction, in a Women's Health Initiative (WHI) cohort of postmenopausal United States women (1992-present). Methods: A cohort of 1131 WHI participants having DLW TEE assessment of ∼10.0 y (median) following WHI enrollment with ∼13.7 y (median) of subsequent follow-up, was used to study the EE associations with all-cause mortality. To enhance the comparability of TEE and total EI, key analyses excluded participants having >5% weight change between WHI enrollment and DLW assessment. The influence of participant age on mortality associations was examined, as was the ability of concurrent and earlier weight and height measurements to explain the results. Results: There were 308 deaths following the TEE assessment through 2021. TEE was unrelated to overall mortality (P = 0.83) in this cohort of generally healthy, older (mean 71 y at TEE assessment) United States women. However, this potential association varied with age (P = 0.003). Higher TEE was associated with a higher mortality rate at the age of 60 y and a lower mortality rate at the age of 80 y. Within the weight-stable subset (532 participants, 129 deaths), TEE was weakly positively related to overall mortality (P = 0.08). This association also varied with age (P = 0.03), with mortality HRs (95% CIs) for a 20% increment in TEE of 2.33 (1.24, 4.36) at the age of 60 y, 1.49 (1.10, 2.02) at 70 y of age, and 0.96 (0.66, 1.38) at 80 y of age. This pattern remained, although was somewhat attenuated, following control for baseline weight and weight changes between WHI enrollment and TEE assessment. Conclusions: Higher EE is associated with higher all-cause mortality among younger postmenopausal women, only partially explained by weight and weight change. Abstract:Purpose To assess the capacity of different acceleration metrics from wrist accelerations to estimate total energy expenditure (TEE) and activity energy expenditure (AEE) using doubly labeled water in preschool children. Methods Thirty-nine preschoolers (5.5 ± 0.1 yr) were included. Total energy expenditure was measured using doubly labeled water during 14 d, and AEE was then calculated using a predicted basal metabolic rate. Participants wore a wGT3X-BT accelerometer on their nondominant wrist for ≥5 d. We derived the following metrics from raw accelerations: raw ActiGraph activity counts using the normal filter and the low-frequency extension; and alternate summary metrics, such as the Euclidian norm minus 1g (ENMO), Euclidian norm of the high-pass-filtered accelerations (HFEN), the bandpass-filtered accelerations, the HFEN plus Euclidean norm of low-pass filtered accelerations minus 1g (HFEN+) and the mean amplitude deviation. Results Alternate summary metrics explained a larger proportion of the variance in TEE and AEE than ActiGraph’s activity counts (counts, 7–8 and 25% of TEE and AEE; alternate summary metrics, 13%–16% and 35%–39% of TEE and AEE). Adjustments for body weight and height resulted in an explanation of 51% of AEE by ENMO. All of the metrics adjusted for fat mass and fat-free mass explained up to 84% and 67% of TEE and AEE, respectively. Conclusions ENMO and the other alternate summary metrics explained more of the variance in TEE and AEE than the ActiGraph’s activity counts in 5-yr-old children, suggesting further exploration of these variables in studies on physical activity and energy expenditure in preschoolers. Our results need confirmation in other populations with wider age groups and varying body compositions.

Abstract:BACKGROUND Calprotectin is released from neutrophils and monocytes, and increased calprotectin levels in stool may serve as a marker of intestinal inflammation. Intestinal permeability is increased in inflammatory bowel diseases, especially in Crohn disease. We studied the relationship between intestinal permeability and calprotectin concentration in intestinal lavage fluid in patients with known or suspected inflammatory bowel disease (IBD). METHODS Thirty-eight patients were examined; 17 had Crohn disease; 3, ulcerative colitis; and 18, irritable bowel syndrome. Intestinal lavage was performed by means of a nasojejunal tube positioned by gastroduodenoscopy. By means of a peristaltic pump 2 l isotonic polyethylene glycol solution (MW, 3350) containing 50 microCi 51Cr-labelled ethylenediaminetetraacetic acid (EDTA) were administered through the tube over a period of 40 min. The first clear fluid passed per rectum was collected and analysed for calprotectin levels with an enzyme-linked immunosorbent assay method. Urine was collected for 5 h and analysed for gamma radioactivity. 51Cr-EDTA excretion in urine was expressed as percentage of dose administered (that is, intestinal permeability). RESULTS Both intestinal permeability and calprotectin concentration were significantly higher in patients with IBD than in patients with functional conditions. In Crohn disease the values depended on disease activity but not on whether the disease was located in the small or in the large bowel. There was a highly significant correlation between calprotectin concentration in gut lavage fluid and intestinal permeability (r=0.79, P<0.0001). CONCLUSION The significant correlation between calprotectin concentration in gut lavage fluid and intestinal permeability supports the view that increased intestinal permeability in IBD might, at least in part, be a consequence of increased transepithelial migration of neutrophils.

Abstract:Abstract Weight disorders are strikingly prevalent globally and can contribute to a wide array of potentially fatal diseases spanning from type II diabetes to coronary heart disease. These disorders have a common cause: poor calorie balance. Since energy expenditure (EE) (kcal d −1 ) constitutes one half of the calorie balance equation (the other half being food intake), its measurement could be of great value to those suffering from weight disorders. A technique for contact free assessment of EE is presented, which only relies on CO 2 concentration monitoring within a sealed office space, and assessment of carbon dioxide production rate (VCO 2 ). Twenty healthy subjects were tested in a cross-sectional study to evaluate the performance of the aforementioned technique in measuring both resting EE (REE) and exercise EE using the proposed system (the ‘SmartPad’) and a U.S. Food and Drug Administration (FDA) cleared gold standard reference instrument for EE measurement. For VCO 2 and EE measurements, the method showed a correlation slope of 1.00 and 1.03 with regression coefficients of 0.99 and 0.99, respectively, and Bland–Altman plots with a mean bias = −0.232% with respect to the reference instrument. Furthermore, two subjects were also tested as part of a proof-of-concept longitudinal study where EE patterns were simultaneously tracked with body weight, sleep, stress, and step counts using a smartwatch over the course of a month, to determine correlation between the aforementioned parameters and EE. Analysis revealed moderately high correlation coefficients (Pearson’s r ) for stress ( r average = 0.609) and body weight ( r average = 0.597) for the two subjects. The new SmartPad method was demonstrated to be a promising technique for EE measurement under free-living conditions.

Assessment of Total Energy Expenditure and Physical Activity Using Activity Monitors

Abstract:To assess total daily energy expenditure (TDEE) under daily living conditions, the doubly labelled water (DLW) technique is considered the gold standard. This technique is accurate but also costly and requires specific lab equipment and expertise. It also provides an average measure of TDEE over a period of one to two weeks and hence no information on physical activity (PA) patterns is available. To overcome these shortcomings, activity monitors can be used to assess activity patterns and an estimate of TDEE can be made, provided the activity monitor has been previously validated in daily life using DLW. Most activity monitors contain accelerometers, that measure the acceleration of the body and hence represent body movement. By definition, body movement leads to energy expenditure (EE) and hence the two always need to be related. Activity monitors that provide an estimate of EE need to be validated so that the contribution of the sensor output to the prediction of EE is known. Subject characteristics such as body mass, height, gender and age already explain most of the variation of TDEE; the accelerometer should then represent the physical activity component of TDEE and improve the explained variation. Many activity monitors also contain additional sensors measuring other (physiological) output parameters such as heart rate, skin temperature, galvanic skin response or gps positioning. Although promising, so far there is no compelling evidence that these additional sensors improve the prediction of EE, so careful consideration is needed whether or not these are worth the extra cost, and the extra battery power and storage capacity needed. Again, it is important to know the individual contribution of each outcome parameter to the prediction of TDEE. In conclusion, activity monitors are valuable tools in PA research but also in nutritional research when energy balance is studied.
