Abstract:Background: The stricture-formation rate following ureteroscopy ranges from 0.5 to 5% and might amount to 24% in patients with impacted ureteral stones. The pathogenesis of ureteral stricture formation is not yet fully understood. It is likely that the patient and stone characteristics, as well as intervention factors, play a role in this process. In this systematic review, we aimed to determine the potential factors responsible for ureteral stricture formation in patients having impacted ureteral stones. Methods: Following the Preferred Reporting Items for Systematic Reviews and Meta-Analysis (PRISMA) criteria, we conducted systematic online research through PubMed and Web of Science without a time restriction, applying the keywords “ureteral stone”, “ureteral calculus”, “impacted stone”, “ureteral stenosis”, “ureteroscopic lithotripsy”, “impacted calculus”, and “ureteral strictures” singly or in combination. Results: After eliminating non-eligible studies, we identified five articles on ureteral stricture formation following treatment of impacted ureteral stones. Ureteral perforation and/or mucosal damage appeared as key predictors of ureteral stricture following retrograde ureteroscopy (URS) for impacted ureteral stones. Besides ureteral perforation stone size, embedded stone fragments into the ureter during lithotripsy, failed URS, degree of hydronephrosis, nephrostomy tube or double-J stent (DJS)/ureter catheter insertion were also suggested factors leading to ureteral strictures. Conclusion: Ureteral perforation during surgery might be considered the main risk factor for ureteral stricture formation following retrograde ureteroscopic stone removal for impacted ureteral stones.

Abstract:Ureteral stricture formation after ureteroscopic lithotripsy is a late complication that can lead to hydronephrosis and a subsequent risk of renal deterioration. The specific incidence is unknown, and the mechanism of stricture formation has not been completely explained. In this review, we summarize the current evidence regarding the incidence of this condition and discuss its pathogenesis. We then list preventive strategies to reduce the morbidity of ureteral strictures.

Abstract:OBJECTIVE To describe the relationship among different tumor nodules in multiple leiomyomas of uterus. METHODS Genomic DNA was extracted from fresh tissue samples, digested through incubation with methylation-sensitive Hha I or Hpa II, and amplified via PCR for androgen receptor (AR) and phosphoglycerate kinase (PGK) genes. The length polymorphism on AR gene was demonstrated by denaturing polyacrylamide gel electrophoresis and silver staining, the PGK gene products were treated with Bst XI and resolved on agarose gels. RESULTS 112 cases of leiomyomas and one case of leiomyosarcoma were examined, 89% showing the length polymorphism for AR gene and 30% carrying the polymorphic Bst XI site at PGK locus. Loss of X-chromosome inactivation mosaicism was observed in all the 321 tumor nodules examined from 77 cases, reflecting their clonal cellular composition. The relationship between different nodules was evaluated by their X-chromosome inactivation patterns in the 295 tumor nodules taken from 57 multiple leiomyomas. Similar inactivated alleles were found in all nodules in 30, in most nodules in 7 cases, similar to a multi-nodular leiomyosarcoma, while 20 cases showed near-random distribution of the inactivated alleles in different nodules, indicating their multicentric origins. No relevance was found between this difference and any histopathological parameters including number of mitotic figures and occurrence of bizarre nuclei and degeneration. In addition, an identical mutation and loss of heterozygosity were found at the AR locus in two apparently discrete tumor nodules in one case, providing further evidence for the unicellular origin of these lesions. CONCLUSIONS The multi-nodular leiomyomas may be classified into multicentric, unicentric types, as well as a mixed type. It remains to be clarified whether different nodules in the unicentric cases originate from a parent tumor by migration or by spreading.
Ureteral stricture after ureteroscopy for stones: A prospective study for the incidence and risk factors

Abstract:Context: A ureteral stricture is a serious complication of ureteroscopy (URS) that was reported in the literature in highly variable rates from 0.2% to 24%. Aims: Our aims are to estimate the incidence and to detect the risk factors of ureteral stricture after URS. Settings and design: This is a prospective, case-series study. Materials and methods: During the period from May 2015 to August 2016, 251 adult patients underwent 263 URS for the treatment of 304 ureteral stones. Postoperative regular follow-up was done for 12 months by ultrasound. Computed tomography urography and diuretic renogram were performed for the cases developed hydronephrosis to confirm and detect the level of the stricture. Statistical analysis: IBM SPSS Statistics for Windows, Version 19.0, Armonk, NY: IBM Corp. used for data analysis. Chi-square and Fisher's exact tests were used to compare between qualitative variables. Mann-Whitney test was used to compare between two quantitative variables in case of nonparametric data. Multiple logistic regression analysis was done to measure the risk factors. P value was considered statistically significant when <0.05. Results: The mean age was 43.5 years (standard deviation [SD]: ±13.6), and the mean body mass index was 28.39 (±3.96). The mean total stone burden was 12.8 mm (SD: ±5.9). Bilateral URS was performed in 12 cases. The mean operative time was 54.8 min (SD: ±22.68). Initial and final stone-free rates were 83.3% and 100%, respectively. The overall complications rate was 28.1%. Stricture occurred in four cases (1.5%). Conclusions: In our experience, the incidence of post-URS ureteral stricture is low. The impacted stone is the most common cause of URS complications and hence stricture formation.

Abstract:The possible role of well-assessed radiological parameters in the prediction of ureteral stricture formation in cases with impacted obstructive ureteral calculi has been evaluated. 46 adult patients with or without ureteral stricture formation after ureteroscopic stone management were included. In addition to stone size and some certain radiological parameters including ureteral wall thickness ( UWT ) of the involved ureter at the impacted stone site was also measured and noted on computed tomography (CT) images. Parameters were evaluated in two subgroups of cases, namely: Group 1: patients in whom a ureteral stricture formed after endoscopic stone removal and Group 2: patients normal ureteral anatomy without any stricture formation. The possible relationship between the UWT values and degree of hydronephrosis (HN) with subsequent stricture formation was comparatively evaluated. All of the stones were proximal ureteral calculi in both groups. Both the degree of HN and proximal ureteral diameter (PUD) parenchymal was higher in cases with stricture formation. In addition, mean parenchymal thickness was lower and mean values of UWT measurements at the stone site were 3.70 ± 0.97 mm and 2.17 ± 0.26 mm in Groups 1 and 2, respectively. A cutoff value 2.49 mm for UWT was found to be highly predictive for stricture formation. UWT value calculated at the obstructing stone site was found to be predictive enough for the likelihood of ureteral stricture formation with high sensitivity and specificity . This evaluation along with some other radiological parameters may enable the urologists to follow such cases on this aspect with necessary measures taken.

Abstract:Abstract Background Ureteroscopic lithotripsy is a common treatment for middle and lower ureteral stones. Although effective and minimally invasive, this technique might cause significant complications, including postoperative ureteral strictures, which occur in 1–4% of patients after ordinary ureteroscopic lithotripsy and in 7.8–24% when lithotripsy is performed for impacted stones. The main purpose of the management of ureteral strictures is to improve hydronephrosis and protect renal function. However, factors that influence the success rate of the endoscopic management of US (e.g., the cause and length of stricture, the duration of hydronephrosis, surgical management technique, and the number of placed ureteral stents) are controversial. The aim of this study was to investigate factors determining the outcomes of endoscopic management for stone-related ureteral stricture. Methods This multi-center case series study was performed at one of the highest-volume centers and the affiliated institutions in Japan. Data of patients who underwent endoscopic surgery for ureteral stricture from January 2016 to March 2019 were retrospectively analyzed. Laser incision and/or balloon dilation were performed for management; single or double stents were placed at the end of the surgery. Treatment success was defined as improvement in hydronephrosis status. Results Nineteen patients were treated for stone-related ureteral stricture. Hydronephrosis successfully improved in 12 patients (63.2%). All seven patients with failed endoscopic management had ureteroscopic lithotripsy-related stricture, whereas 3/12 (25.0%) and 7/12 (58.3%) patients with ureteroscopic lithotripsy-related stricture and impacted stone-related stricture, respectively, underwent successful endoscopic treatment (P = 0.004). Stricture length > 15 mm was observed in five patients (71.4%) with failed management, two patients (16.6%) with successful management (P = 0.046), 7/10 patients (70.0%) with ureteroscopic lithotripsy-related stricture, and 1/7 patients (14.3%) with impacted stone-related stricture (P = 0.049). Among patients with ureteroscopic lithotripsy-related stricture and impacted stone-related stricture, both laser incision and balloon dilation were performed in 5/10 (50.0%) and 2/7 (28.6%) patients (P = 0.874) while double stents were placed in 5/10 (50.0%) and 5/7 (57.1%) patients (P = 0.874), respectively. Conclusions Ureteroscopic lithotripsy as a cause and stricture length > 15 mm could strongly affect the success rate of endoscopic management of ureteral stricture. In such cases, reconstructive management should be considered in the early stages.

Abstract:Purpose of review: To present the latest evidence related to the predictors of urinary tract infections (UTIs) and urosepsis after ureteroscopy (URS) for stone disease. Recent findings: Our review suggests that almost half of all post-URS complications are related to infectious complications although reported rates of urosepsis were low. The use of antibiotic prophylaxis, treatment of pre-operative UTI, and low procedural time seem to reduce this risk. However, the risk is higher in patients with higher Charlson comorbidity index, elderly patients, female gender, long duration of pre-procedural indwelling ureteric stents and patients with a neurogenic bladder and with high BMI. Infectious complications following ureteroscopy can be a source of morbidity and potential mortality. Although majority of these are minor, efforts must be taken to minimise them especially in high-risk patients. This includes the use of prophylactic antibiotics, limiting stent dwell and procedural time, prompt identification and treatment of UTI and urosepsis, and careful planning in patients with large stone burden and multiple comorbidities.

Abstract:Performing port scans through Tor is a way to hide the source's IP address from the target. Researchers hoping to source their own scans benefit from a means of scanning that helps them to anonymize themselves from targets that may potentially retaliate as the result of being scanned. Though effective in providing anonymization during scanning, it is not scalable to the point of scanning the entire IPv4 Address space on multiple ports, as scans take considerably longer to execute through Tor. This paper specifically explores using a third-party data source to target specific areas of interest in the IPv4 range and then scanning those areas anonymously with parallelized scanners as an effective way to anonymously collect internet scan data. The results demonstrate the feasibility of this approach.

Abstract:Objective: To investigate the influencing factors of ureteroenteric strictures (UES) in patients undergoing laparoscopic radical cystectomy plus urinary diversion (UD). Method: A total of 412 patients who underwent UD after radical prostatectomy from January 2008 to December 2016 were retrospectively included in this study. Age, gender, body mass index (BMI), diversion type, time to diagnosis of UES, duration of ureteral stent, postoperative complications, including urinary tract infections, ureteroenteric leakage and UES were collected. Kaplan-Meier curves were used to describe time to developing UES. Prognostic factors of UES were analyzed using COX proportional hazard regression model. Result: Median follow-up time was 37 (IQR 17-120) months. A total of 59 patients (70 sides) developed UES, including 34 cases on the left side, 14 cases on the right side and 11 cases on both sides, following UD after radical cystectomy. The median time to diagnosis of UES was 7 (IQR 4-11) months. The total incidence of UES was 14.3%. The incidence of UES was 10.9%, 13.3% and 14.1% at 1, 3 and 5 years after UD, respectively. Cox proportional hazard regression model analysis demonstrated that BMI≥25kg·m(-2) (P=0.008), ureteroenteric leakage (P=0.001) and urinary tract infections (P=0.037) were the independent risk factors associated with UES following UD after radical cystectomy. Conclusion: The incidence rate of UES following UD after radical cystectomy was relatively high, which occurs more common on the left side. Obese patients, combined with ureteroenteric leakage, urinary tract infection after UD, are more likely to develop into UES.

Abstract:Background During ureteroscopy, severe ureter straightness or contortion may occur before the stone is passed. Purpose To identify clinical factors associated with distal ureteral status below the ureteral calculi in patients before surgery. Material and Methods From October 2016 to March 2017, 101 patients with ureteral calculi who underwent ureteroscopic lithotripsy were reviewed. Patients who lacked clinical data and underwent preoperative indwelling ureteral stent placement were excluded. Univariate and multivariate analyses were performed to determine the clinical factors associated with intraoperative findings. Results A total of 101 patients were enrolled in the study (mean age = 54 years; mean stone size = 7.9 +/- 4.5 mm). Overall, 25 of the 101 patients (24.7%) were diagnosed with poor distal ureteral status defined as intraoperative ureterostenosis or contortion resulting in a ureteroscope being unable to pass during the initial attempt. Univariate analysis showed significant differences in renal parenchyma thickness, ureteral wall thickening on imaging, and stone location (all, P < 0.05) with and without poor distal ureteral status. On multivariable analysis, renal parenchyma thickness (adjusted odds ratio [aOR] 0.288; 95% confidence interval [CI] 0.099-0.838; P = 0.022) and ureteral wall thickening on imaging (aOR 6.114; 95% CI 2.015-18.548; P = 0.001) independently predicted poor distal ureteral status. However, only renal parenchyma thickness was associated with severe ureter straightness or tortuosity that resulted in conversion. Conclusion In conclusion, renal parenchyma thickness and ureteral wall thickening on imaging were associated with poor distal ureteral status. Therefore, patients with these predictive factors should undergo more intensive preparation before surgery.

Definition of benign ureteroenteric anastomotic strictures in ileal conduits after radical cystectomy – Experience from a single center and previously published literature

Abstract:Objective To evaluate a cohort of patients diagnosed with benign ureteroenteric stricture (UES) after radical cystectomy with ileal conduits using a strict predefined definition of strictures. Additionally, we want to illustrate the UES debut, regarding symptoms and clinical findings. UES is a well-known long-term complication after radical cystectomy, affecting up to 20% of all patients. In the literature, different incidence rates are reported. However, these are based on various definitions of strictures. Methods We used strict predefined criteria to evaluate UES incidence including symptoms, timing, diagnostic methods, treatment, and outcome in all patients who underwent radical cystectomy with an ileal conduit between 2012 and 2018 at a single high-volume center. Results Of a total of 693 patients who underwent radical cystectomy with ileal conduit, we found 109 patients with 135 UES in total, corresponding to 15.7% of patients (CI: 13.2-18.6) and 10% of all included ureteroenteric anastomosis (CI: 8.5-11.6) after radical cystectomy. Median follow-up was 24 months (IQR: 12 – 31), and postoperatively UES was diagnosed after a median of 6 months (IQR: 3 – 16). A total of 56% was diagnosed with elevated creatinine. Every UES underwent a median of two (IQR: 1-2) treatment attempts and 122 UES were treated successfully. Conclusion Benign UES is a significant cause of morbidity following radical cystectomy. Our findings contribute to the knowledge of timing, incidence, and recommended treatment of strictures. We argue the importance of establishing a clear gold standard when defining UES to ensure accurate reporting in future research.

Prevalence and predictors of stone passage after double J stenting for symptomatic ureteral stones: a cross-sectional, real-life study

Abstract:PurposeTo evaluate the rate of and predictors of stone passage (SP) after urgent retrograde stenting for symptomatic ureteral stones.MethodsWe retrospectively analysed data from 249 consecutive patients presenting to the emergency department for symptomatic ureteral stones and treated with retrograde stenting. Demographic, clinical and laboratory characteristics were collected. Stones parameters were collected before stenting and SP was evaluated at 1 month with computerized tomography. Descriptive statistics and logistic regression models tested the association between predictors and SP.ResultsOverall, median (IQR) age and stone diameter were 56 (45–68) years and 7.1 (4.4–9.8) mm, respectively. Stones were located in the proximal, mid and distal ureter in 102 (41.0%), 48 (19.3%) and 99 (39.8%) cases. SP was observed in 65 (26.2%) individuals. Stone diameter (3.2 vs. 7.7 mm, p < 0.001) and stone density (416 vs. 741, p < 0.001) were lower and a higher rate of distal stones (76.9% vs. 26.7%, p < 0.001) was found in the SP group compared to that with persistent stones. Multivariable logistic regression analysis showed that distal ureteral stone location (OR 7.9, p < 0.01) and lower HU (OR 0.9, p < 0.01) were associated with SP, after accounting for stone volume. Patients with a distal stone of 500 HU had a 75% probability of SP.ConclusionStone passage occurred in 26% of patients with indwelling stent due to symptomatic ureteral stones. Lower stone density and distal stone location were independent predictors of stone passage. Patients with these criteria should be managed with follow-up imaging and stent removal instead of ureteroscopy.

Abstract:Abstract The susceptibility of 1870 Streptococcus pyogenes and 1595 Streptococcus pneumoniae to macrolides and lincosamides has been monitored from 1993 to 2004 in Central Italy. Among S. pyogenes, 30.2% were erythromycin resistant; 18.5% were also resistant to josamycin and clindamycin (MLS phenotype). After an increasing erythromycin resistance rate in 1993-1997 (maximum 53.16%), a definite decrease was observed since 2001 with resistance rates always less than 30%. Thirty six percent of pneumococcal isolates were erythromycin-resistant, with minor temporal fluctuations; the MLS phenotype was the most prevalent overall (32.6%) and in individual years. S. pneumoniae strains were also tested for susceptibility to b-lactams and other antimicrobial agents: 11.2% were penicillin non-susceptible, with a gradually increasing prevalence after 2001 (maximum rate 17.3% in 2004), 31.15% were resistant to tetracycline, 4.9% to chloramphenicol, 0.74% to rifampin. All pneumococcal isolates were susceptible to teicoplanin and 99.9% to ceftriaxone and ofloxacin.

Abstract:OBJECTIVETo evaluate the prevalence and the risk factors of the ureteral stricture following renal transplantation and outcomes of surgical managements.METHODSBy a retrospective analysis, we assessed 1 293 patients who underwent renal transplantation between January 2001 and December 2013 at our center. The patients with ureteral stricture had undergone surgical management which included neoureterocystostomy, ureteral anastomosis with the native ureter and endoscopic therapy.RESULTSUreteral stricture following renal transplantation was diagnosed in 17 cases (1.3%) including 6 males and 11 females. The mean age of the patients at the time of surgery was 44 (29 to 64) years. The median interval between ureteral stricture obstruction and kidney transplantation was 4 (1 to 120) months. The ureteral stricture was significantly correlated with multiple renal arteries, donor age and delayed graft function. The effective rate of surgical management was 65.75%.CONCLUSIONThe incidence of ureteral stricture as a urologic complication after renal transplantation is low. The advanced techniques that preserve the ureteric blood supply should be applied. The surgical managements have respective advantages and disadvantages. The neoureterocystostomy shows best outcome, while ureteral anastomosis with the native ureter has poor outcome.
Analysis of Risk Factors of SIRS Following Flexible Ureteroscopy in the Treatment of Renal Stones

Abstract:Objective Analyze the risk factors that affect the development of postoperative systemic inflammatory response syndrome( SIRS) following flexible ureteroscopy( f- URS) in the treatment of renal stones.Patients and methods We retrospectively analyzed the data of f- URS in the treatment of renal stones between October 2011 and February 2013. The patients were divided into two groups as patients developing SIRS and not developing SIRS. Univariate and multivariate statistical analyses were performed to determine factors affecting the development of SIRS. Results The study included 561 patients( 381 male and 180 female),among whom 5. 2%( 29 /561) developed SIRS. In the univariate analysis,there was significant difference between the two groups in bilateral operation( P = 0. 032),history of chronic renal insufficiency( P = 0. 041),kidney anatomic deformity( P = 0. 035),a recent history of stone related fever( P 0. 001),operation history of the same side urinary stones( P = 0. 010),preoperative positive urine culture( P 0. 001),endoscopic signs of infection( P 0. 001),the stone of magnesium ammonium phosphate( P 0. 001). While age,gender,BMI,operative time,stone burden,history of diabetes,proportion of the isolated kidney,preoperative creatinine level,proportion of preoperative double- J placement,proportion of ureteral access sheath placement were not related to the development of SIRS. Multivariate analysis further confirmed that without preoperative placement of double- J( P = 0. 021),a recent history of stone related fever( P 0. 001),and endoscopic signs of infection( P 0. 001) were the high risks of SIRS following f- URS in the treatment of renal stones. Conclusions Without preoperative placement of double- J,a recent history of stone related fever,and endoscopic signs of infection are significant predictors of SIRS following f- URS in the treatment of renal stones. Attention should be paid to ask the related history and improve preoperative examinations,preventing the development of urosepsis.
[Characteristics of Ureteral Stone Position and Dilatation of Ureter in Patients Before Treated with Endourologic Lithotripsy].

Abstract:OBJECTIVE:To investigate the position and the size of ureteral stone in patients before treated with ureteroscopic lithotripsy or percutaneous nephrolithotomy (PCNL), as well as the dilatation of ureter caused by stone, which may be helpful for better understanding of pathogenesis of ureteral stone and improvement of the diagnosis and treatment in clinic.METHODS:A total of 129 cases with ureteral stone for endourologic lithotripsy in Peking University People's Hospital from Aug. 2016 to Mar. 2017 were included for the retrospective review. The CT data of the ureteral stones and the ureteral dilatation were collected, including the position of stone, the transverse and longitudinal diameter of stone, and the transverse diameter of dilated upper ureter and ureteropelvic junction (UPJ). The distribution of ureteral stones and dilatation in different parts of ureter were compared and analyzed.RESULTS:All of the 129 cases of ureteral stone, stones were located at UPJ in 9 cases (7.0%), proximal ureter in 75 (58.0%), ureter crossing external iliac vessel (UEIV) in 6 (4.7%), distal ureter in 26 (20.2%), and ureterovesical junction (UVJ) in 13 (10.1%). The mean transverse diameter of proximal ureteral stones was greater than that of distal ureteral stones [(8.47±2.36) mm vs. (6.74±1.99) mm, P<0.001], as the same as the mean longitudinal diameter [(11.00±4.41) mm vs. (7.50±4.28) mm, P<0.001]. In 114 cases of dilated ureter for stone, the UPJ had a greater transverse diameter compared with the upper ureter [(14.39±6.09) mm vs. (11.45±3.85) mm, P<0.001].CONCLUSION:The most common location of stone is the proximal ureter in patients for endourologic lithotripsy, as the location in UEIV is rare. Both transverse and longitudinal diameters of stone in proximal ureter are greater than those in distal ureter. For dilated ureter, it is more severe in UPJ than in upper ureter. Traditionally, it is accepted that the stones lodge at 3 sites of natural narrowing in ureter, which may be questioned.
