Abstract:Acute pancreatitis is a common indication for hospital admission, increasing in incidence, including in children, pregnancy and the elderly. Moderately severe acute pancreatitis with fluid and/or necrotic collections causes substantial morbidity, and severe disease with persistent organ failure causes significant mortality. The diagnosis requires two of upper abdominal pain, amylase/lipase ≥ 3 ×upper limit of normal, and/or cross-sectional imaging findings. Gallstones and ethanol predominate while hypertriglyceridaemia and drugs are notable among many causes. Serum triglycerides, full blood count, renal and liver function tests, glucose, calcium, transabdominal ultrasound, and chest imaging are indicated, with abdominal cross-sectional imaging if there is diagnostic uncertainty. Subsequent imaging is undertaken to detect complications, for example, if C-reactive protein exceeds 150 mg/L, or rarer aetiologies. Pancreatic intracellular calcium overload, mitochondrial impairment, and inflammatory responses are critical in pathogenesis, targeted in current treatment trials, which are crucially important as there is no internationally licenced drug to treat acute pancreatitis and prevent complications. Initial priorities are intravenous fluid resuscitation, analgesia, and enteral nutrition, and when necessary, critical care and organ support, parenteral nutrition, antibiotics, pancreatic exocrine and endocrine replacement therapy; all may have adverse effects. Patients with local complications should be referred to specialist tertiary centres to guide further management, which may include drainage and/or necrosectomy. The impact of acute pancreatitis can be devastating, so prevention or reduction of the risk of recurrence and progression to chronic pancreatitis with an increased risk of pancreas cancer requires proactive management that should be long term for some patients.

Abstract:Goal. To show that despite the unwavering attention of clinicians to the problem of acute pancreatitis, late diagnosis or its errors, inadequate choice of conservative treatment tactics without taking into account the pathogenetic mechanisms of its development continue to be among the reasons for unsatisfactory treatment. Introduction. The article demonstrates the unrelenting attention of clinicians to the problem of acute pancreatitis (AP). It is noted that new instrumental diagnostic methods have been introduced into clinical practice: ultrasound, CT, selective celiacography, etc. Minimally invasive surgical techniques have taken a strong place in diagnosis and treatment. At the same time, it is emphasized that despite the widespread introduction of the latest diagnostic technologies and treatment protocols into clinical practice over the past decades, treatment results have not changed significantly. The reason for unsatisfactory treatment results continues to be late diagnosis or its errors, inadequate choice of conservative therapy and surgical techniques without taking into account the pathogenetic mechanisms of its development. The main problem in improving treatment results continues to be the influence of treatment tactics on the development of pancreatic necrosis of the pancreas (PG), parapancreatic tissue and mediastinal tissue. The authors emphasize that although intraorgan thrombosis of the pancreas in AP has been noted by many researchers, there is no common point of view on their formation. Taking this into account, they conducted studies of AP on various models of acute experimental pancreatitis, biopsy material from operated patients and materials from sectional studies. Abstract:Abstract: A clinical study of twenty patients with pancreatitis, aged between (45-68) years, attending Alzahrawi hospital in Mosul city. Each pancreatitis patient was evaluated clinically and laboratory examinations have been done including: (Serum sodium, potassium, magnesium, calcium, albumin, triglycerides (T.G), glucose, urea, total bilirubin, amylase, lactate dehydrogenase (LDH, Aspartate transamines (AST). The analysis of results showed that the levels of Serum Sodium, Calcium, Albumin, Potassium and Magnesium were significantly decreased in pancreatitis patients when compared with control, while T.G, glucose, urea, total bilirubin, LDH, AST and amylase levels were significantly elevated in pancreatitis patients when compared with control.

Abstract:Rationale: Acute pancreatitis is an inflammatory disorder of the pancreas, and its correct diagnosis is an area of interest for clinicians. In accordance with the revised Atlanta classification, acute pancreatitis can be diagnosed if at least 2 of the following 3 criteria are fulfilled: abdominal pain; serum lipase (or amylase) activity at least 3 times the upper limit of normal; or characteristic findings of acute pancreatitis on contrast-enhanced computed tomography (CT) or, less often, magnetic resonance imaging or transabdominal ultrasonography. Diagnostic imaging is essential in patients with no or slight enzyme elevation. If enzymes are normal in cases with abdominal distension, there is clinical doubt about the diagnosis of acute pancreatitis, so an early CT scan should be obtained and other life-threatening disorders excluded. Patient concerns: A 50-year-old male presented with a 1-day history of abdominal bloating and distension. On physical examination, abdominal bulging and mild epigastric tenderness were detected. Laboratory evaluation showed normal amylase and lipase. There was no abnormality on abdominal ultrasound or CT of the abdomen and pelvis. On the fourth day of admission, CT of the abdomen and pelvis showed a hypodense lesion in the pancreas surrounded by a moderate amount of peripancreatic fluid. Diagnoses: In accordance with the revised Atlanta classification, acute pancreatitis was diagnosed, based on the presence of abdominal pain, and the results of the CT scan of the abdomen and pelvis. Interventions: The patient was treated with fasting, gastrointestinal decompression bowel rest, intravenous rehydration, and somatostatin. Outcomes: After 2 days of treatment, his abdominal distension was significantly relieved, and the patient was discharged on the seventh day of admission. At the 3-month follow-up, the patient had no recurrence of pancreatitis. Lessons: This case of abdominal distension could not be explained by common causes, such as ascites, bowel edema, hematoma, bowel distension, or ileus, which led us to suspect pancreatitis.

Abstract:The diagnosis of acute pancreatitis is based on the association of acute upper abdominal pain and hyper lipasemia. The CT scan confirms the diagnosis and assess the severity of the pancreatitis, as do several bioclinical indexes. Alcohol and biliary lithiasis, seeked by ultrasound, are the two main causes of acute pancreatitis. Most of acute pancreatitis are oedematous and resolve spontaneously without sequella. Severe necrotizing pancreatitis lead to death in 10 to 20% of cases, because of multiple organ failure or intra-abdominal complications of necrosis. Treatment of severe acute pancreatitis involves intensivists, radiologists and surgeons. Surgery is restricted to drainage of super-infected necrosis and treatment of causative biliary lithiasis.

Abstract:BACKGROUND AND OBJECTIVE: The Atlanta classification of acute pancreatitis enabled standardised reporting of research and aided communication between clinicians. Deficiencies identified and improved understanding of the disease make a revision necessary.METHODS: A web-based consultation was undertaken in 2007 to ensure wide participation of pancreatologists. After an initial meeting, the Working Group sent a draft document to 11 national and international pancreatic associations. This working draft was forwarded to all members. Revisions were made in response to comments, and the web-based consultation was repeated three times. The final consensus was reviewed, and only statements based on published evidence were retained.RESULTS: The revised classification of acute pancreatitis identified two phases of the disease: early and late. Severity is classified as mild, moderate or severe. Mild acute pancreatitis, the most common form, has no organ failure, local or systemic complications and usually resolves in the first week. Moderately severe acute pancreatitis is defined by the presence of transient organ failure, local complications or exacerbation of co-morbid disease. Severe acute pancreatitis is defined by persistent organ failure, that is, organ failure >48 h. Local complications are peripancreatic fluid collections, pancreatic and peripancreatic necrosis (sterile or infected), pseudocyst and walled-off necrosis (sterile or infected). We present a standardised template for reporting CT images.CONCLUSIONS: This international, web-based consensus provides clear definitions to classify acute pancreatitis using easily identified clinical and radiologic criteria. The wide consultation among pancreatologists to reach this consensus should encourage widespread adoption.

Abstract:Objective To review the diagnoses and treatments of acute pancreatitis(AP),especially severe AP.Methods A retrospective analysis was performed on etiology,incentives,laboratory test results,treatments and prognosis in 80 mild AP patients and 22 severe admitted to digestive department of our hospital in 2007.Results The first etiological factor of groups mild AP and severe was bile-derived,accounting for 62.5%(50/80),59.1%(13/22),respectively.In laboratory test,the ratio of C-reactive protein >150 mg/L(53.3%) except Ranson indicators was significantly higher in severe AP group than in mild(7.7%).In treatment,severe AP patients needed supports of such minimally invasive treatment as more aggressive medical treatment,endoscopic retrograde cholangiopancreatography(ERCP),abdominal ultrasound-guided puncture and drainage,enteral nutrition.No patients died except that 1 died of multiple organ failure,only 2 severe patients were transferred to surgical department to perform open surgery after conservative treatment.Conclusion Detection of C-reactive protein>150 mg/L helps determine the severity of AP.Severe AP patients should given ERCP,abdominal ultrasound-guided puncture and drainage,enteral nutrition and ICU support,which can minimize the rate of open suregery and mortality.

Abstract:The pancreas is a glandular organ that is responsible for the proper functioning of the digestive and endocrine systems, and therefore, it affects the condition of the entire body. Consequently, it is important to effectively diagnose and treat diseases of this organ. According to clinicians, pancreatitis—a common disease affecting the pancreas—is one of the most complicated and demanding diseases of the abdomen. The classification of pancreatitis is based on clinical, morphologic, and histologic criteria. Medical doctors distinguish, inter alia, acute pancreatitis (AP), the most common causes of which are gallstone migration and alcohol abuse. Effective diagnostic methods and the correct assessment of the severity of acute pancreatitis determine the selection of an appropriate treatment strategy and the prediction of the clinical course of the disease, thus preventing life-threatening complications and organ dysfunction or failure. This review collects and organizes recommendations and guidelines for the management of patients suffering from acute pancreatitis.

Abstract:Acute pancreatitis is a gastrointestinal emergency where diagnosis is based on typical symptoms, increased serum lipase concentration, and abdominal imaging. Local complications and organ failure in severe acute pancreatitis regularly necessitate treatment in the intensive care unit and are associated with increased mortality rates. Only optimal interdisciplinary treatment can improve the prognosis of patients with severe acute pancreatitis. This article gives guidance on the initial diagnostic and etiological examinations as well as on the evaluation of organ failure and the severity assessment according to common classification systems. Furthermore, the endoscopic management of biliary pancreatitis and infected necrosis is discussed and the basics of targeted volume therapy, nutrition, and indications for antibiotic treatment are reviewed.

Abstract:BACKGROUND: Chronic pancreatitis (CP) does not have diagnostic or prognostic biomarkers. CP is the end stage of a progressive inflammatory syndrome that is diagnosed at late stages by morphologic features. To diagnose earlier stages of the disease, a new mechanistic definition was established based on identifying underlying pathogenic processes and biomarker evidence of disease activity and stage. Although multiple risk factors are known, the corresponding biomarkers needed to make a highly accurate diagnosis of earlier disease stages have not been established. The goal of this study is to systematically analyze the literature to identify the most likely candidates for development into biomarkers of CP.METHODS: We conducted a systematic review of candidate analytes from easily accessible biological fluids and identified 67 studies that compared CP to nonpancreatic-disease controls. We then ranked candidate biomarkers for sensitivity and specificity by area under the receiver operator curves (AUROCs).RESULTS: Five biomarkers had a large effect size (an AUROC > 0.96), whereas 30 biomarkers had a moderate effect size (an AUROC between 0.96 and 0.83) for distinguishing CP cases from controls or other diseases. However, the studies reviewed had marked variability in design, enrollment criteria, and biospecimen sample handling and collection.CONCLUSIONS: Several biomarkers have the potential for evaluation in prospective cohort studies and should be correlated with risk factors, clinical features, imaging studies and outcomes. The Consortium for the Study of Chronic Pancreatitis, Diabetes and Pancreas Cancer provides recommendations for avoiding design biases and heterogeneity in sample collection and handling in future studies.

Abstract:Abstract Objectives The purpose of this study was to comprehensively assess the heterogeneity of procedures in the diagnostic guidelines for acute pancreatitis and to identify gaps limiting knowledge in diagnosing this disease. Methods A systematic search of a number of databases was performed to determine the guidelines for the diagnosis of acute pancreatitis in patients with severe pancreatitis. The guidelines for the diagnosis of severe pancreatitis were evaluated by AGREE II. The Measurement Scale of Rate of Agreement (MSRA) was used to assess the guidelines (2015–2020) and extract evidence supporting these recommendations for analysis. Results Seven diagnostic guidelines for acute pancreatitis were included. Only the 2019 WSES Guidelines for the Management of Severe Acute Pancreatitis and the Japanese Guidelines for the Management of Acute Pancreatitis: Japanese Guidelines 2015 had a total score of more than 60%, which is worthy of clinical recommendation. The average scores of the Scope and Purpose domain and the Clarity and Expression domain were the highest at 71.62% and 75.59%, respectively, while the average score of the Applicability area was the lowest at 16.67%. The included guidelines were further analyzed to determine the heterogeneity of the diagnosis of acute pancreatitis. The main reasons for the heterogeneity were the citation of low-quality evidence, the presence of far too many indicators for the classification of acute pancreatitis, unclear depictions of the standard, and poorly comprehensive recommendations for the diagnosis of the aetiology in the primary diagnosis of acute pancreatitis, the severity classification, the aetiological diagnosis, and the diagnosis of comorbidities. Conclusions The quality of different diagnostic guidelines for severe pancreatitis is uneven. The recommendations are largely based on low-quality evidence, and the guidelines still have much room for improvement to reach a high level of quality. The diagnostic procedures for acute pancreatitis vary widely in different guidelines. There are large differences between them, and resolving the abovementioned reasons would be a very wise choice for guideline developers to revise and upgrade the guidelines in the future.

Abstract:Background:Acute inflammation of the pancreas, acute pancreatitis (AP) shows an increasing incidence in the world as well as in Algeria. However, lethality and mortality are decreasing, mainly due to recommendations from Atlanta, the International Pancreatology Association and the American Pancreatology Association. The purpose of this work was to analyze the management of AP in the general surgery department of the UHC of Sidi Bel Abbes. Method: In a retrospective study, 100 acute pancreatitis cases admitted to general surgery between January 2017 and December 2019 were analyzed. The abdominal scanner was systematic at the admission. The rate of lipasemia, when greater than 3 times the normal one, allowed, in association with the evocative pain, to make the diagnosis. The severity of AP was assessed using the Balthazar score. An MRI was requested when the scanner was inconclusive, to look for an etiology and / or possible complications. 79 (79%) patients received broad-spectrum antibiotic therapy and diet was systematic. Results: We have transmitted the recommendations of scholar societies to the various specialists and the management of AP is currently more unified and more effective. Scanner is no longer systematic in the early phase. Antibiotic therapy is no longer administered without biological evidence. Enteral nutrition is now possible. Conclusion: We were able to reach the goal of reducing hospitalization and scanning requests. A further analysis of 100 other PA files is planned to confirm the obtained results.

Abstract:The Papanicolaou Society of Cytopathology has developed a set of guidelines for pancreatobiliary cytology including indications for endoscopic ultrasound (EUS)-guided fine needle aspiration (FNA) biopsy, techniques of EUS-FNA, terminology and nomenclature of pancreatobiliary disease, ancillary testing, and postbiopsy treatment and management. All documents are based on the expertise of the authors, a review of the literature, discussions of the draft document at several national and international meetings over an 18-month period and synthesis of online comments of the draft document on the Papanicolaou Society of Cytopathology web site (www.papsociety.org). This document selectively presents the results of these discussions and focuses on a proposed standardized terminology scheme for pancreatobiliary specimens that correlate cytological diagnosis with biological behavior and increasingly conservative patient management of surveillance only. The proposed terminology scheme recommends a six-tiered system: Nondiagnostic, Negative, Atypical, Neoplastic (benign or other), Suspicious and Positive. Unique to this scheme is the "Neoplastic" category separated into "benign" (serous cystadenoma), or "Other" (premalignant mucinous cysts, neuroendocrine tumors, and solid-pseudopapillary neoplasms). The positive or malignant category is reserved for high-grade, aggressive malignancies including ductal adenocarcinoma, acinar cell carcinoma, poorly differentiated neuroendocrine carcinomas, pancreatoblastoma, lymphoma, and metastases. Interpretation categories do not have to be used. Some pathology laboratory information systems require an interpretation category, which places the cytological diagnosis into a general category. This proposed scheme provides terminology that standardizes the category of the various diseases of the pancreas, some of which are difficult to diagnose specifically by cytology. In addition, this terminology scheme attempts to provide maximum flexibility for patient management, which has become increasingly conservative for some neoplasms.

Abstract:Background: Acute pancreatitis (AP) is an inflammatory disease with rapid onset and rapid progression. In severe cases, it can cause systemic inflammatory response syndrome (SIRS), multiple organ failure (POF) and death. In order to explore the effective index of early diagnosis of AP, we have detected the levels of serum lipase (LPS), serum amylase (S-AMY) and C-reactive protein (CRP). Meanwhile, cost evaluation was performed on these biochemical parameters for AP patients. Methods: In this study, 212 patients were admitted with acute abdominal pain, divided into AP group (n=107) and non-AP group (n=105), including cholelithiasis (n=8), intestinal obstruction (n=34), acute cholangitis (n=14), acute cholecystitis (n=20), acute appendicitis (n=20) and pancreatic cancer (n=9). Demographic data and laboratory examination results were collected, the sensitivity and specificity of LPS, S-AMY and CRP in the diagnosis of AP were calculated respectively, as well as the costs of single or multiple tests were evaluated. Results: The serum level of S-AMY and LPS in AP group were significantly higher than those in nonAP group (P<0.01), The serum level of CRP in AP group was higher than that in non-AP group (P<0.05). When the critical value of the normal reference value is three times as the critical value, the sensitivity and specificity of the LPS in diagnosing AP were 91.6% and 90.5%, S-AMY was 70.1% and 91.4%. And CRP was 59.8% and 56.2%, respectively. Compared with the trend of serum LPS expression, the expression trend of S-AMY was positively correlated (r=0.81), while there was no correlation with the trend of CRP expression. Conclusions: The ability of LPS to diagnose AP is significantly better than that of LPS + CRP, as well as the combined detection of LPS + CRP + S-AMY, which can be used as the first choice for laboratory examination of AP.

Abstract:Background: Previous data from this unit suggest that postnatal growth retardation (PGR) is inevitable in preterm infants. However, the study was performed in a single level III neonatal intensive care unit and applicability of the findings to other level III or level I–II special care baby units was uncertain. Objectives: To examine postnatal hospital growth and to compare growth outcome in preterm infants discharged from four level III tertiary care units and 10 level I–II special care baby units in the former Northern Region of the United Kingdom. Subjects/methods: Preterm infants (⩽ 32 weeks gestation; ⩽ 1500 g) surviving to discharge were studied. Infants were weighed at birth and discharge. Body weight was converted into a z score using the British Foundation Growth Standards. To ascertain the degree of PGR, the z score at birth was subtracted from the z score at discharge. Data were evaluated using a combination of split plot (level III v I–II = main factor; individual centre = subfactor) and stepwise regression analyses. Results were considered significant at p < 0.05. Results: A total of 659 (level III, n = 335; level I–II, n = 324) infants were admitted over a 24 month period (January 1998–December 1999). No differences were detected in birth characteristics, CRIB score (a measure of illness in the first 24 hours of life), length of hospital stay, weight gain, weight at discharge, or degree of PGR between infants discharged from level III and level I–II units. Significant variation was noted in length of hospital stay (∼35%; p < 0.001), weight gain (∼33%; p < 0.001), weight at discharge (∼20%; p < 0.001), and degree of PGR (∼200%; p < 0.001) between the level III units. Even greater variability was noted in the duration of hospital stay (∼40%; p < 0.001), weight gain (∼60%; p < 0.001), weight at discharge (∼40%, p < 0.001), and degree of PGR (∼300%, p < 0.001) between the level I–II units. Conclusions: These data stress the variable but universal nature of PGR in preterm infants discharged from level III and I–II neonatal intensive care units and raise important questions about nutritional support of these infants before and after hospital discharge.
Early detection of biliary pancreatitis

Abstract:Biochemical tests (serum glutamic pyruvic transaminase, serum glutamic oxaloacetic transaminase, alkaline phosphatase, gammaglutamyltranspeptidase, bilirubin, and serum amylase) were performed upon admission in 84 patients with suspected (36) or proven (48) acute pancreatitis at the time of the first episode of acute abdominal pain suspected clinically as acute pancreatitis. These parameters all increased significantly more in patients with gallstone pancreatitis. Among them, the SGPT was the most discriminant test between biliary and nonbiliary pancreatitis. The positive predictive value of SGPT was 92%, when the cutoff point was chosen at twice the upper limit of normal. In patients with increased SGPT, a SGOT-SGPT ratio less than 1 is the rule (88%) for those with gallstone pancreatitis. This enzymatic determination allowed us to select more accurately the patients suitable for morphological procedures to confirm the biliary origin of the pancreatitis.
