Abstract:Pancreatic cancer is a highly malignant tumor and surgical resection is the only curative treatment option. In order to improve the prognosis of patients with pancreatic cancer, it is very important to diagnose and evaluate pancreatic cancer accurately and early. Imaging examinations play an important role in tumor detection, staging and surgical resectability evaluation, which can provide reliable evidences for diagnosis and treatment of pancreatic cancer. At present, the imaging techniques commonly used for diagnosis of pancreatic cancer include conventional ultrasound, endoscopic ultrasonography, PET-CT, multi-detector row CT (MDCT), magnetic resonance imaging(MRI),in which MDCT and MRI are the most widely used. In this article, the application and research progress of imaging in accurate diagnosis and evaluation of pancreatic cancer are reviewed.
The diagnosis of pancreatic injury with CT scan

Abstract:Objective To discuss the manifestation of CT and improve the accuracy of diagnosis of pancreas injury.Methods The clinical materials of 10 cases' CT scan and contrast scan of PI were analyzed.Results There were 2 cases of intra-pancreas hematoma,7 cases of confused or local swelling,8 cases of omental sac hydrops,10 cases of peri-pancreas hyprops,5 cases of parenchyma laceration with low echo in plan scan.The contrast scan showed ther were 3 cases head,5 cases neck and 2 cases body and tail injury.The coincidence of diagnosis was 80% in pancreas duct combined with other organ injury.Conclusion The pancreas laceration might have characteristics on CT with lamina contrast scan but limited at duct of pancreas.

Abstract:Background: Acute pancreatitis (AP) is characterized by acute onset, rapid development, and poor prognosis. Timely diagnosis and identification of the cause are the key to formulating the clinical program and improving the prognosis. There were several studies on this topic but the results varied. This study systematically evaluated and analyzed reports on the comparison of magnetic resonance imaging (MRI) and computed tomography (CT) for the diagnosis of AP in recent years, providing evidence for clinical diagnosis and treatment.Methods: The databases of PubMed, Web of Science, The Cochrane Library, China National Knowledge Infrastructure (CNKI), and Wanfang Data were searched for literature on MRI and CT in the diagnosis of AP. After evaluating the articles and extracting the data, the software RevMan 5.4 and Stata 16.0 were used for meta-analysis.Results: A total of 9 articles were included in the selection, with a total of 566 patients having undergone diagnosis. Meta-analysis showed that for MRI, the diagnostic sensitivity was 92%, 95% confidence interval (CI): 85% to 96%; specificity was 74%, 95% CI: 50% to 89%; positive likelihood ratio was 3.5, 95% CI: 1.6 to 8.0; negative likelihood ratio was 0.11, 95% CI: 0.05 to 0.24; diagnostic odds ratio (DOR) was 32, 95% CI: 7 to 136; and the area under the curve (AUC) value was 0.93, 95% CI: 0.90 to 0.95. For CT, the diagnostic sensitivity was 73%, 95% CI: 55% to 85%; specificity was 64%, 95% CI: 42% to 82%; positive likelihood ratio was 2.0, 95% CI: 1.1 to 3.6; negative likelihood ratio was 0.43, 95% CI: 0.24 to 0.76; DOR was 5, 95% CI: 2 to 14; and the AUC value was 0.74, 95% CI: 0.70 to 0.78. The AUC value of MRI was significantly greater than CT (Z=3.684, P=0.023).Discussion: In the diagnosis of AP, MRI is more sensitive, specific, and accurate than CT, and can be used as the first choice for the diagnosis of AP.

Abstract:Acute pancreatitis (AP) is a potentially life-threatening inflammatory disease of the pancreas, with clinical management determined by the severity of the disease. Diagnosis, severity prediction, and prognosis assessment of AP typically involve the use of imaging technologies, such as computed tomography, magnetic resonance imaging, and ultrasound, and scoring systems, including Ranson, Acute Physiology and Chronic Health Evaluation II, and Bedside Index for Severity in AP scores. Computed tomography is considered the gold standard imaging modality for AP due to its high sensitivity and specificity, while magnetic resonance imaging and ultrasound can provide additional information on biliary obstruction and vascular complications. Scoring systems utilize clinical and laboratory parameters to classify AP patients into mild, moderate, or severe categories, guiding treatment decisions, such as intensive care unit admission, early enteral feeding, and antibiotic use. Despite the central role of imaging technologies and scoring systems in AP management, these methods have limitations in terms of accuracy, reproducibility, practicality and economics. Recent advancements of artificial intelligence (AI) provide new opportunities to enhance their performance by analyzing vast amounts of clinical and imaging data. AI algorithms can analyze large amounts of clinical and imaging data, identify scoring system patterns, and predict the clinical course of disease. AI-based models have shown promising results in predicting the severity and mortality of AP, but further validation and standardization are required before widespread clinical application. In addition, understanding the correlation between these three technologies will aid in developing new methods that can accurately, sensitively, and specifically be used in the diagnosis, severity prediction, and prognosis assessment of AP through complementary advantages.

Abstract:Rationale: Acute pancreatitis is an inflammatory disorder of the pancreas, and its correct diagnosis is an area of interest for clinicians. In accordance with the revised Atlanta classification, acute pancreatitis can be diagnosed if at least 2 of the following 3 criteria are fulfilled: abdominal pain; serum lipase (or amylase) activity at least 3 times the upper limit of normal; or characteristic findings of acute pancreatitis on contrast-enhanced computed tomography (CT) or, less often, magnetic resonance imaging or transabdominal ultrasonography. Diagnostic imaging is essential in patients with no or slight enzyme elevation. If enzymes are normal in cases with abdominal distension, there is clinical doubt about the diagnosis of acute pancreatitis, so an early CT scan should be obtained and other life-threatening disorders excluded. Patient concerns: A 50-year-old male presented with a 1-day history of abdominal bloating and distension. On physical examination, abdominal bulging and mild epigastric tenderness were detected. Laboratory evaluation showed normal amylase and lipase. There was no abnormality on abdominal ultrasound or CT of the abdomen and pelvis. On the fourth day of admission, CT of the abdomen and pelvis showed a hypodense lesion in the pancreas surrounded by a moderate amount of peripancreatic fluid. Diagnoses: In accordance with the revised Atlanta classification, acute pancreatitis was diagnosed, based on the presence of abdominal pain, and the results of the CT scan of the abdomen and pelvis. Interventions: The patient was treated with fasting, gastrointestinal decompression bowel rest, intravenous rehydration, and somatostatin. Outcomes: After 2 days of treatment, his abdominal distension was significantly relieved, and the patient was discharged on the seventh day of admission. At the 3-month follow-up, the patient had no recurrence of pancreatitis. Lessons: This case of abdominal distension could not be explained by common causes, such as ascites, bowel edema, hematoma, bowel distension, or ileus, which led us to suspect pancreatitis.

Abstract:Acute pancreatitis (AP) is an inflammatory disease that can progress to severe acute pancreatitis (SAP), which increases the risk of death. AP is characterized by inappropriate activation of trypsinogen, infiltration of inflammatory cells, and destruction of secretory cells. Other contributing factors may include calcium (Ca<sup>2+</sup>) overload, mitochondrial dysfunction, impaired autophagy, and endoplasmic reticulum (ER) stress. In addition, exosomes are also associated with pathophysiological processes of many human diseases and may play a biological role in AP. However, the pathogenic mechanism has not been fully elucidated and needs to be further explored to inform treatment. Recently, the treatment guidelines have changed; minimally invasive therapy is advocated more as the core multidisciplinary participation and "step-up" approach. The surgical procedures have gradually changed from open surgery to minimally invasive surgery that primarily includes percutaneous catheter drainage (PCD), endoscopy, small incision surgery, and video-assisted surgery. The current guidelines for the management of AP have been updated and revised in many aspects. The type of fluid to be used, the timing, volume, and speed of administration for fluid resuscitation has been controversial. In addition, the timing and role of nutritional support and prophylactic antibiotic therapy, as well as the timing of the surgical or endoscopic intervention, and the management of complications still have many uncertainties that could negatively impact the prognosis and patients' quality of life. Consequently, to inform clinicians about optimal treatment, we aimed to review recent advances in the understanding of the pathogenesis of AP and its diagnosis and management.

Abstract:Acute pancreatitis is characterized by acute chemical injury of the pancreatic parenchyma and peripancreatic tissue. The increased frequency of death in acute pancreatitis is directly correlated with the degree and progress of pancreatic necrosis. Moreover, the occurrence of some local complications in acute pancreatitis, such as pancreatic hemorrhage, peripancreatic abscess or large pseudocyst, and pseudoaneurysm, could influence the choice of treatment for these patients. Magnetic resonance imaging (MRI) can be used to help evaluate the presence and degree of pancreatic necrosis, and is crucial for identifying complications of acute pancreatitis and predicting prognosis. The purpose of this article is to describe MRI techniques for acute pancreatitis, to review the spectrum of pancreatic and peripancreatic patterns, as well as to survey various complications secondary to acute pancreatitis on MRI. The role of MRI in the initial evaluation and staging of acute pancreatitis is emphasized.

Abstract:Acute pancreatitis (AP) is caused by acute inflammation of the pancreas and adjacent tissue and is a common source of abdominal pain. The current CT and MRI evaluation of AP is mostly based on morphologic features. Recent advances in image acquisition and analysis offer the opportunity to go beyond morphologic features. Advanced MR techniques such as diffusion-weighted imaging, as well as T1 and T2 mapping, can potentially quantify signal changes reflective of underlying tissue abnormalities. Advanced analytic techniques such as radiomics and artificial neural networks (ANNs) offer the promise of uncovering imaging biomarkers that can provide additional classification and prognostic information. The purpose of this article is to review recent advances in imaging acquisition and analytic techniques in the evaluation of AP.

Abstract:Objective To review the diagnoses and treatments of acute pancreatitis(AP),especially severe AP.Methods A retrospective analysis was performed on etiology,incentives,laboratory test results,treatments and prognosis in 80 mild AP patients and 22 severe admitted to digestive department of our hospital in 2007.Results The first etiological factor of groups mild AP and severe was bile-derived,accounting for 62.5%(50/80),59.1%(13/22),respectively.In laboratory test,the ratio of C-reactive protein >150 mg/L(53.3%) except Ranson indicators was significantly higher in severe AP group than in mild(7.7%).In treatment,severe AP patients needed supports of such minimally invasive treatment as more aggressive medical treatment,endoscopic retrograde cholangiopancreatography(ERCP),abdominal ultrasound-guided puncture and drainage,enteral nutrition.No patients died except that 1 died of multiple organ failure,only 2 severe patients were transferred to surgical department to perform open surgery after conservative treatment.Conclusion Detection of C-reactive protein>150 mg/L helps determine the severity of AP.Severe AP patients should given ERCP,abdominal ultrasound-guided puncture and drainage,enteral nutrition and ICU support,which can minimize the rate of open suregery and mortality.
Complications of Acute Pancreatitis: MRI Findings of Peripancreatic Vascular Disorders

Abstract:Objective To discuss MRI features of acute pancreatitis(AP) coexisting with various peripancreatic vascular disorders.Materials and Methods 122 cases with AP were selected in this retrospective study at our hospital between June 2009 and September 2010.Patient informed consent was taken.MR imaging was performed with a 1.5T MR scanner included gradient echo T1 weighted imaging,fast recovery fast spin echo T2 weighted imaging,single shot fast spin echo T2 weighted imaging,MRCP and dynamic contrast enhanced MR imaging.A spectrum of vascular abnormalities included:(1) artery involvement/artery vasculitis;(2) pseudoaneurysm;(3) vein vasculitis,phlebothrombosis/vein occlusion;(4) pancreatic portal hypertension/portal sponge like changes were noted.Results In our study,the prevalence of AP coexisting with peripancreatic vascular disorders was 17.2%(21/122).In 21 AP patients coexisting with peripancreatic vascular disorders,16/21 patients occurred artery involvement/artery vasculitis(the splenic artery,common hepatic artery,superior mesenteric artery and celiac trunk was respectively 16/16,13/16,12/16,and 7/16 involved).The splenic pseudoaneurysm was determined in 1/21 on MRI.The vein vasculitis was determined in 19/21(the splenic vein,portal vein,and superior mesenteric vein was respectively 19/19,13/19,and 9/19 involved).The phlebothrombosis/vein occlusion occurred in 2/21(one case was splenic vein partial occlusion,the other was widely vein occlusion in portal vein,splenic vein,and superior mesenteric vein).2/21 patients were identified as pancreatic portal hypertension/portal sponge like changes.Conclusion AP may coexist with various peripancreatic vascular disorders.There are some specific MRI features for various peripancreatic vascular disorders.

Abstract:In the early diagnostic workup of acute pancreatitis (AP), the role of contrast-enhanced CT is to establish the diagnosis in uncertain cases, assess severity, and detect potential complications like necrosis, fluid collections, bleeding or portal vein thrombosis. The value of texture analysis/radiomics of medical images has rapidly increased during the past decade, and the main focus has been on oncological imaging and tumor classification. Previous studies assessed the value of radiomics for differentiating between malignancies and inflammatory diseases of the pancreas as well as for prediction of AP severity. The aim of our study was to evaluate an automatic machine learning model for AP detection using radiomics analysis. Patients with abdominal pain and contrast-enhanced CT of the abdomen in an emergency setting were retrospectively included in this single-center study. The pancreas was automatically segmented using TotalSegmentator and radiomics features were extracted using PyRadiomics. We performed unsupervised hierarchical clustering and applied the random-forest based Boruta model to select the most important radiomics features. Important features and lipase levels were included in a logistic regression model with AP as the dependent variable. The model was established in a training cohort using fivefold cross-validation and applied to the test cohort (80/20 split). From a total of 1012 patients, 137 patients with AP and 138 patients without AP were included in the final study cohort. Feature selection confirmed 28 important features (mainly shape and first-order features) for the differentiation between AP and controls. The logistic regression model showed excellent diagnostic accuracy of radiomics features for the detection of AP, with an area under the curve (AUC) of 0.932. Using lipase levels only, an AUC of 0.946 was observed. Using both radiomics features and lipase levels, we showed an excellent AUC of 0.933 for the detection of AP. Automated segmentation of the pancreas and consecutive radiomics analysis almost achieved the high diagnostic accuracy of lipase levels, a well-established predictor of AP, and might be considered an additional diagnostic tool in unclear cases. This study provides scientific evidence that automated image analysis of the pancreas achieves comparable diagnostic accuracy to lipase levels and might therefore be used in the future in the rapidly growing era of AI-based image analysis.

Abstract:This paper reviews the values of MRI and CT in the diagnosis and treatment of acute pancreatitis in recent years and presents the image-based alterations in the revised Atlanta classification of acute pancreatitis(2008),including further standardization of terms concerning acute pancreatitis defined according to imaging findings,so as to evaluate the roles of imaging tools in the diagnosis and treatment of this disease.It is thought that imaging technology is of great clinical significance for the diagnosis and treatment of acute pancreatitis and standardization of related terms may facilitate the communication in the research and clinical practice regarding this disease.
Together We Stand, Divided We Fall: A Multidisciplinary Approach in Complicated Acute Pancreatitis

Abstract:Acute pancreatitis (AP) is an inflammatory condition with a mild course in most patients, but 20–30% evolve to single or multiple organ dysfunction and pancreatic/peripancreatic necrosis, with potentially infected collections. In the first weeks of disease, a systemic inflammatory syndrome (SIRS) dominates the clinical setting, and early management decisions in this precocious phase can change the course of the disease. Imaging is crucial in the diagnosis, and since the adoption of the revised Atlanta classification, four different types of pancreatic/peripancreatic collections have been defined. The management of the complicated forms of AP has been defined by several treatment guidelines, and the main indication for intervention is local infection, preferably in walled-off necrosis. Open surgery necrosectomy is associated with a very high rate of morbimortality, giving a place to different multidisciplinary methodologies, emphasizing drainage and necrosectomy techniques in a “step-up” approach starting from mini-invasive endoscopic drainage and moving, if needed, to progressively more invasive techniques, including interventional radiology and mini-invasive surgery. With the advent of several new technologies in the specialties involved, the complicated AP cases which need drainage and necrosectomy benefit from a new era of multidisciplinary cooperation, permitting higher efficacy with lower levels of morbimortality and reducing hospital stay and costs.

Abstract:Acute pancreatitis is a common indication for hospital admission, increasing in incidence, including in children, pregnancy and the elderly. Moderately severe acute pancreatitis with fluid and/or necrotic collections causes substantial morbidity, and severe disease with persistent organ failure causes significant mortality. The diagnosis requires two of upper abdominal pain, amylase/lipase ≥ 3 ×upper limit of normal, and/or cross-sectional imaging findings. Gallstones and ethanol predominate while hypertriglyceridaemia and drugs are notable among many causes. Serum triglycerides, full blood count, renal and liver function tests, glucose, calcium, transabdominal ultrasound, and chest imaging are indicated, with abdominal cross-sectional imaging if there is diagnostic uncertainty. Subsequent imaging is undertaken to detect complications, for example, if C-reactive protein exceeds 150 mg/L, or rarer aetiologies. Pancreatic intracellular calcium overload, mitochondrial impairment, and inflammatory responses are critical in pathogenesis, targeted in current treatment trials, which are crucially important as there is no internationally licenced drug to treat acute pancreatitis and prevent complications. Initial priorities are intravenous fluid resuscitation, analgesia, and enteral nutrition, and when necessary, critical care and organ support, parenteral nutrition, antibiotics, pancreatic exocrine and endocrine replacement therapy; all may have adverse effects. Patients with local complications should be referred to specialist tertiary centres to guide further management, which may include drainage and/or necrosectomy. The impact of acute pancreatitis can be devastating, so prevention or reduction of the risk of recurrence and progression to chronic pancreatitis with an increased risk of pancreas cancer requires proactive management that should be long term for some patients.

Abstract:Objectives To investigate the clinical charactistics of acute pancreatitis(AP),and to summarize the experiences of treatment,improve the diagnostic level and therapeutic effect.Method In this retrospective clinical trial,450 patients with acute pancreatitis admitted to our hospital from January 2000 to December 2004 were enrolled for analysis the etiology,clinical symptoms,diagnostic crieria,complications,therapeutic regimens and prognosis.Results In the 450 patients,366 had mild acute pancreatitis(MAP)(81.3%),84 had severe acute pancreatitis(SAP)(18.7%).As for the etiology,biliary tract diseases accounted for 62.7%,alcoholic-induced 15.3%,hyperlipidemia 12.4%,other factors 12.9%,unknown causes 18% cases.54 cases(12%)were relapsing acute pancreatitis,among which 57.4% suffered from biliary tract diseases.The abdominal pain played a major role in the clinical symptoms(97.3%).82.4% of the patients had serum amylase 3 times higher than the upper normal limit.Around 95.3% had cute pancreatitis confirmed by CT scan.The main local complications of SAP were pancreatic pseudocyst(19.0%).Organ dysfunction/failure was presented in 67.9% of the severe cases.The rate of multiple organ dysfunction(MODS)in SAP was 26.2%,and that of fulminate pancreatitis was 25%.96.7% of patients received antibiotics,and 98.9% received H2RA or PPI.Parenteral nutrition(PN)was given in 88% of the patients,and early enteral nutrition(EN)was performed in 45.2% of the severe cases.14 severe cases underwent continuous veno-venous hemofiltration(CVVH).In order to relieve bile duct obstruction,18 cases underwent endoscopic sphincterotomy(EST)and(or) endoscopic nosal biliary drainage(ENBD).7 severe cases(8.3%)received surgical intervention.Conclusions Biliary tract diseases,alcohol and hyperlipidemia are main etiologic factors of acute pancreatitis.The main causes of AP relapsing may be biliary tract diseases.Serum amylase and CT scan are reliable diagnostic methods for acute pancreatitis.Abdominal pain is the chief syndrome of AP.The complications of SAP are excessive and complicated,which should be attracted considerable attention and treated promptly.It is safe and efficient to perform EN in SAP patients.CVVH is an effective therapeutic option for SAP.The treatment of EST/ENBD for AP patients suffering from bile duct obstruction is safe and effective.Conservative treatment should be an initial choise for SAP.Operation should be performed if there is a specific indication for early surgical intervention,or for conversion to operation.
Effectiveness of self-help smoking cessation guides for firefighters.

Abstract:Most smokers would prefer to quit smoking on their own. However, among specific blue-collar occupations it is not known whether self-help techniques will be effective or even used to help smokers quit smoking. In this study we evaluated the effectiveness of a self-help smoking cessation guide designed specifically for firefighters (FF) and compared it with the American Lung Association (ALA) guide designed for the general public. We hypothesized that a shorter guide (FF), written in the jargon of the fire service, and formatted as a workbook would be more effective. One hundred five firefighters randomly received either the ALA or FF guide after having an annual physical examination. Twelve percent of the ALA group and 11% of the FF group quit smoking after the physical examination but before receiving the guides. We found no relationship between the specific guide that the firefighters received and subsequent smoking status. Few firefighters reported filling out pages of the ALA guide (11.9%) or the FF guide (17.8%). For smokers who have not reached the active stage of quitting smoking, guides may not be a factor in quitting. Screening smokers to determine their "stage of change" at the physical examination may increase the effectiveness of self-help guides.
Diagnostic value of hematological parameters in acute pancreatitis

Abstract:Background: Acute pancreatitis (AP) is an inflammatory disease with rapid onset and rapid progression. In severe cases, it can cause systemic inflammatory response syndrome (SIRS), multiple organ failure (POF) and death. In order to explore the effective index of early diagnosis of AP, we have detected the levels of serum lipase (LPS), serum amylase (S-AMY) and C-reactive protein (CRP). Meanwhile, cost evaluation was performed on these biochemical parameters for AP patients. Methods: In this study, 212 patients were admitted with acute abdominal pain, divided into AP group (n=107) and non-AP group (n=105), including cholelithiasis (n=8), intestinal obstruction (n=34), acute cholangitis (n=14), acute cholecystitis (n=20), acute appendicitis (n=20) and pancreatic cancer (n=9). Demographic data and laboratory examination results were collected, the sensitivity and specificity of LPS, S-AMY and CRP in the diagnosis of AP were calculated respectively, as well as the costs of single or multiple tests were evaluated. Results: The serum level of S-AMY and LPS in AP group were significantly higher than those in nonAP group (P<0.01), The serum level of CRP in AP group was higher than that in non-AP group (P<0.05). When the critical value of the normal reference value is three times as the critical value, the sensitivity and specificity of the LPS in diagnosing AP were 91.6% and 90.5%, S-AMY was 70.1% and 91.4%. And CRP was 59.8% and 56.2%, respectively. Compared with the trend of serum LPS expression, the expression trend of S-AMY was positively correlated (r=0.81), while there was no correlation with the trend of CRP expression. Conclusions: The ability of LPS to diagnose AP is significantly better than that of LPS + CRP, as well as the combined detection of LPS + CRP + S-AMY, which can be used as the first choice for laboratory examination of AP.

Abstract:Background: Previous data from this unit suggest that postnatal growth retardation (PGR) is inevitable in preterm infants. However, the study was performed in a single level III neonatal intensive care unit and applicability of the findings to other level III or level I–II special care baby units was uncertain. Objectives: To examine postnatal hospital growth and to compare growth outcome in preterm infants discharged from four level III tertiary care units and 10 level I–II special care baby units in the former Northern Region of the United Kingdom. Subjects/methods: Preterm infants (⩽ 32 weeks gestation; ⩽ 1500 g) surviving to discharge were studied. Infants were weighed at birth and discharge. Body weight was converted into a z score using the British Foundation Growth Standards. To ascertain the degree of PGR, the z score at birth was subtracted from the z score at discharge. Data were evaluated using a combination of split plot (level III v I–II = main factor; individual centre = subfactor) and stepwise regression analyses. Results were considered significant at p < 0.05. Results: A total of 659 (level III, n = 335; level I–II, n = 324) infants were admitted over a 24 month period (January 1998–December 1999). No differences were detected in birth characteristics, CRIB score (a measure of illness in the first 24 hours of life), length of hospital stay, weight gain, weight at discharge, or degree of PGR between infants discharged from level III and level I–II units. Significant variation was noted in length of hospital stay (∼35%; p < 0.001), weight gain (∼33%; p < 0.001), weight at discharge (∼20%; p < 0.001), and degree of PGR (∼200%; p < 0.001) between the level III units. Even greater variability was noted in the duration of hospital stay (∼40%; p < 0.001), weight gain (∼60%; p < 0.001), weight at discharge (∼40%, p < 0.001), and degree of PGR (∼300%, p < 0.001) between the level I–II units. Conclusions: These data stress the variable but universal nature of PGR in preterm infants discharged from level III and I–II neonatal intensive care units and raise important questions about nutritional support of these infants before and after hospital discharge.
