Abstract:Abstract Objective To examine physical activity (PA) of post-percutaneous coronary intervention (PCI) patients and explore the demographic, clinical, and social psychological characteristics associated with PA levels. Methods A total of 246 post-PCI patients from the Peking University Third Hospital in Beijing, China, were included in this crosssectional study through convenience sampling. Data were collected from a self-reported questionnaire. PA was categorized into low, moderate, or high levels. The ordinal multinomial logistic regression model was used to estimate the relationship among demographic, medical, and psychosocial characteristics. Results The overall prevalence of low, moderate, and high PA was 20%, 70%, and 10%, respectively. For the domain-specific PA patterns, most participants took part in leisure-time PA (84.5%); walking was the most common PA. Increased motivation and selfefficacy, lower monthly income, and unemployment were predictors of high PA. Conclusions PA levels in post-PCI patients was not optimal, and leisure-time PA had the highest participation rate. Analyses of influencing factors can provide medical staff and health workers information to focus on high-risk groups and introduce more tailored interventions. Future studies can explore more regions, and ecological models can be introduced to study more influencing factors.

Persistence of coronary risk factor status in participants 12 to 18 months after percutaneous coronary intervention

Abstract:Background: Percutaneous coronary intervention (PCI) is a widely performed revascularization technique for coronary heart disease; however, there is limited research investigating the risk factor status of patients 1 year after the procedure. Objective: This cross-sectional study was conducted to investigate the self-reported risk factor status by patients who had undergone a PCI at a major teaching hospital in Sydney, Australia. Subjects: : Two hundred seventy participants who underwent PCI between April 2003 and March 2004 and who met the inclusion criteria were followed up 1 year after the PCI. Methods: After obtaining informed consent, a follow-up self-administered questionnaire was mailed to participants. Information was collected relating to the following coronary risk factors: smoking, and physical activity status, blood pressure and cholesterol levels, body mass index, depression, anxiety, and stress levels. Results: Two hundred two participants (75%) returned a completed questionnaire. Approximately one third of participants had at least two modifiable risk factors. The most common cardiovascular risk factors identified were physical inactivity, increased body mass index, high blood pressure, and high cholesterol. Approximately half the women (46%) and a quarter of the men had at least two modifiable risk factors. Only a minority (11%) of the participants continued to smoke at 1-year follow up. Participating in physical activity for a total time of 150 minutes or more per week was reported by only 42% of the participants. Depression and anxiety were present in 25% and stress in 17% of the participants. A third of the participants (n = 64) erroneously believed that they had no heart problems. Conclusions: The findings reveal inadequate management of modifiable risk factors among post-PCI participants 12 to 18 months after revascularization, which highlights a need for tailored secondary prevention interventions to address factors contributing to cardiovascular risk. The evidence obtained from this study will inform the development of an intervention to address cardiovascular risk factor modification.

Abstract:Objective To investigate the clinical value and impact of cardiac rehabilitation programs applied to patients with acute myocardial infarction(AMI) early after percutaneous coronary intervention(PCI).Methods The enrolled participants were 90 cases of AMI who received PCI in Shaoxing People′s Hospital from January 2014 to May 2016.They were randomized into traditional routine group,traditional strengthening group and new optimization group with 30 cases in each.The cardiac rehabilitation program for traditional routine group was developed based on the rehabilitation program applied to patients one week after emergency PCI included in 2006 Chinese Cardiac Rehabilitation Programs after PCI,mainly consisting of simple forms of exercise such like walking.Both the traditional strengthening group and new optimization group received the cardiac rehabilitation program developed based on the rehabilitation program applied to patients after selective PCI included in 2006 Chinese Cardiac Rehabilitation Programs after PCI.In addition,the new optimization group received the early-applied,personalized and accurate cardiac rehabilitation program developed in accordance with the latest concepts in cardiac rehabilitation received by Avans University,UMC St RAdboud,Queen Elizabeth Hospital and other hospitals in Hong Kong.The traditional strengthening group and new optimization group had greater basal activities and total activities than the traditional routine group.The intervention period for all the groups was six months.Color Doppler echocardiography was performed in the groups immediately after PCI(before the cardiac rehabilitation intervention) and six months after the starting of cardiac rehabilitation program(after the cardiac rehabilitation intervention),respectively for measuring the left ventricular ejection fraction(LVEF),left ventricular end-systolic volume(LVESV),left ventricular end-diastolic volume(LVEDV) and wall motion score index(WMSI).Levels of serum tumor necrosis factor-alpha(TNF-α),nitric oxide(NO),endothelin 1(ET-1) and soluble vascular cell adhesion molecule 1(sVCAM-1) were measured before and after the cardiac rehabilitation intervention.The incidences of malignant arrhythmia,angina pectoris,heart failure and sudden death during the cardiac rehabilitation were observed.Results No significant differences in LVEF,LVESV,LVEDV,WMSI,TNF-α,NO,ET-1 and sVCAM-1 levels were found among the three groups before the cardiac rehabilitation(P>0.05).After the cardiac rehabilitation intervention,the WMSI was lower in the traditional strengthening group than that in the routine traditional group,LVEF was higher in the new optimization group than that in the routine traditional group and traditional strengthening group,WMSI was lower in the new optimization group than that in the routine traditional group and traditional strengthening group(P<0.05).LVEF was higher after the cardiac rehabilitation intervention than that before the cardiac rehabilitation intervention in the three groups(P<0.05).After the cardiac rehabilitation intervention,the levels of TNF-α,ET-1 and sVCAM-1 were lower in the traditional strengthening group than those in the routine traditional group,the level of NO was higher in the traditional strengthening group than that in the routine traditional group,the levels of TNF-α,ET-1 and sVCAM-1 were lower in the new optimization group than those in the routine traditional group and traditional strengthening group,the level of NO was higher in the new optimization group than that in the routine traditional group and traditional strengthening group(P<0.05).The levels of TNF-α,ET-1 and sVCAM-1 were lower,but the level of NO was higher after the cardiac rehabilitation intervention than that before the cardiac rehabilitation intervention in the three groups(P<0.05).During the cardiac rehabilitation intervention,the incidence of malignant arrhythmia differed significantly among the three groups(P<0.05);three groups had similar incidences of angina pectoris and heart failure(P>0.05);no sudden death occurred in all groups.Conclusion Cardiac rehabilitation training applied to patients with AMI early after PCI can obviously improve the cardiac function and prognosis.The humanitarian cardiac rehabilitation program based on personal conditions used in the new optimization group can achieve the optimal effect of rehabilitation,which can also protect the cardiac function to the utmost and improve the prognosis tangibly.

Abstract:Objectives: To describe the current state of exercise capacity as well as to identify its predictors in patients with coronary artery disease (CAD) following percutaneous coronary intervention (PCI) or coronary artery bypass graft (CABG) in the mainland of China. Methods: A retrospective study design was employed. We evaluated 230 CAD patients following PCI or CABG in a cardiac rehabilitation center from January 2019 to October 2019. The patients were referred to undergo incremental cardiopulmonary exercise testing with a cycle ergometer. The Zung Self-Rating Anxiety Scale and the Zung Self-Rating Depression Scale were used to evaluate patients' mental health. Statistical analysis was performed using the chi-square test, Fisher's exact test, t-test, Mann-Whitney U test, and binary logistic regression. Results: Among the 230 patients, 223 patients demonstrated reduced exercise capacity. Resutlts of the logistic regression analysis showed that anxiety (OR = 1.13, 95% CI 1.01-1.32, P = 0.029) was an independent risk factor for reduced exercise capacity in patients following the PCI or CABG. Conclusions: Exercise capacity of Chinese CAD patients after PCI or CABG was relatively poor. Alleviating symptoms of anxiety and making exercise prescriptions according to the results of the cardiopulmonary exercise test should be considered during the intervention to improve CAD patients' exercise capacity.

Abstract:In an elderly hypertensive patient, both the threshold of therapy initiation and the objective are the same as in younger patients. The objective should, however, not be forcefully aspired after, since even a smaller lowering of blood pressure will provide prognostic benefit. Hypertension in an elderly person is treated on the basis of values obtained in an elevated posture or, preferably, at home. Too low blood pressure, possibly causing falling of the patient, should be avoided. The medication is selected individually, and combination drug therapy is often required. Treatment of all other risk factors is also important.

Abstract:This prospective study evaluated the effect of an individualized, comprehensive, home-based cardiac rehabilitation program combining exercise training with risk factor modification and psychosocial counseling on risk factors, psychological well-being, functional capacity, and work resumption in 99 post-percutaneous coronary interventions (PCI) patients randomized to control (standard care plus telephone follow-up, n=49) or intervention (individualized, comprehensive, home-based cardiac rehabilitation, n=50) groups. Data were collected at time 1 (T(1)) during hospital admission, time 2 (T(2)) approximately 2 months post-PCI, and time 3 (T(3)) approximately 12 months post-PCI. Results suggest that the allocation to an individualized, comprehensive, home-based cardiac rehabilitation program provided more advantageous outcomes. At both follow-ups, the intervention group showed within-group improvement in serum cholesterol levels (P<0.02; P<0.01) and exercise participation (P<0.001; P<0.001) with differences in exercise participation favoring the intervention group (P<0.01) at T(2). Repeated measures ANOVA showed significant improvements over time in body mass index (BMI) (P<0.01), psychological well-being (P<0.001), and functional capacity (P<0.001) for both groups. More patients in the intervention group had returned to work at T(2) (P<0.001) and did so more quickly (P<0.01). These findings suggest that an individualized, comprehensive, home-based cardiac rehabilitation program improves risk factor profiles and work resumption patterns for patients following PCI.

Abstract:A 22-year-old Caucasian male was referred for evaluation of familial polyposis. During a barium contrast study of his colon, hypertrophy of the pedicles on the right side at T10-T12 and Li was noted. The patient denied symptoms related to his back. Physical and neumologic examinations were normal. There was no evidence of hypentrophy of any soft tissues. Laboratory studies including serum calcium, phosphate, and alkaline phosphatase were within normal limits. Complete spine films and skull films were negative, except for the might-sided hypertrophied pedicles in the lower thomacic and upper lumbar spine (fig. 1). The contralateral pedicles were normal or the same size as the pedicIes above and below the involved area. Serial laminograms in the antemopostemior and lateral projections demonstrated no evidence of a neural arch defect. The right pedicles measured from 0.6 cm to 1 cm larger in all diameters than the left pedicles (fig. 2). Associated hypertrophy of the lamina on the right and narrowing of the neural canal on the right were present (fig. 2C). A total body sw Tc pynophoSphate bone scan and pinhole collimated views over the lower thomacic to upper lumbar spine did not reveal abnormal uptake or discrepancy between the hypertmophied and contralateral normal pedicles.
Exploring the status and associated factors of the readiness for return-to-work in young and middle-aged stroke patients

Abstract:Stroke increasingly affects individuals of working age. An accurate assessment of Readiness for Return-to-Work (RRTW) can help determine the optimal timing for RRTW and facilitate an early reintegration into society. This study investigates the current state of RRTW and the influencing factors among young and middle-aged stroke patients in China. A sample of young and middle-aged stroke patients hospitalized in a tertiary hospital in Henan Province between December 2021 and May 2022 were included in this study. A general information questionnaire and the Readiness for RRTW scale, the Social Support Rate Scale, the Stroke Self-Efficacy Scale, and the Fatigue Severity Scale were administered to the patients. Of the 203 patients successfully surveyed, 60 (29.6%) were in the pre-contemplation stage, 35 (17.2%) in the contemplation stage, 81 (39.9%) in the prepared for action-self-evaluative stage, and 27 (13.3%) in the prepared for action- behavior stage. Logistic regression analysis identified education level, monthly income, time to start rehabilitation therapy, social support, stroke self-efficacy, and fatigue severity as key factors affecting RRTW scale readiness in young and middle-aged stroke patients. The readiness of young and middle-aged stroke patients to Return-to-Work needs to be increased further. Healthcare professionals should consider the influencing factors of RRTW and design targeted intervention programs to facilitate a successful Return-to-Work and normal life.

Semi-structured, face-to-face or telephone in-depth interviews were conducted at the cardiology unit of a tertiary referral hospital in Yunnan Province, China from March 2022 to January 2023. Results: Seven categories with some subcategories were constructed from the data, categorized into three domains. Firstly, individual factors include (i) Personal coping with healthy lifestyle requirements (tried but failed; I can do it), (ii) individual perception and feeling toward disease (knowing about the disease; belief of cure; fears of relapse), and (iii) personal benefits (improved health; meaning of life). Secondly, social factors include (i) social facilitators (family resources; healthcare support), (ii) social barriers (inconvenient medical care service; conflicting information). Finally, cultural factors include (i) way of living (dietary habits; key roles of yan (cigarette) and jiu (alcohol) in Chinese society), (ii) way of thinking (fatalism and Confucian familism). Conclusion: The determinants of health-related behaviors of patients after PCI are multifaceted and dynamic. Different interventions should be formulated to promote patients' adherence to health behaviors. Moreover, priority should be given to the impact of traditional Chinese philosophy on the health behaviors of patients after PCI, and the health promotion program for these patients should be culturally sensitive. In addition, future research should further explore the determinants of health behaviors among diverse ethnic minorities after PCI, which has not been fully inquired in this study. Self-management in patients with coronary heart disease after stent implantation at the long-term stage: a cross-sectional study

Abstract:BACKGROUND: The self-management of risk reduction in coronary heart disease (CHD) plays an important role in mediating health outcomes following percutaneous coronary intervention (PCI), but there is a lack of research on self-management status in the long-term stage after PCI in Chinese patients with CHD. Hence, this study investigated the self-management status in the long-term stage (>2 years) after PCI in patients with CHD. The results could provide a reference for the development of targeted interventions.METHODS: This cross-sectional study administered a questionnaire survey on self-management in patients with CHD who underwent PCI (convenience sampling) and had been discharged from our medical center for >2 years, excluding the patients with severe hepatic or renal dysfunction or tumor. Data about cardiovascular risk factors, including body mass index (BMI), total cholesterol (TC), low-density lipoprotein cholesterol (LDL-C), and self-management status, were collected. The Coronary Artery Disease Self-Management Scale (CSMS) was used to assess patient self-management.RESULTS: The total CSMS score was 69.5±11.0, suggesting that the patients' self-management level was average. The scores for life management, emotion management, and disease management were 87.4±9.7, 77.6±7.7, and 57.5±11.0, respectively. Significant differences were noted in the patients' self-management scores according to occupation, education level, residence, and sex (all P<0.05). The self-management ability of farmers, primary school-educated, and male participants was relatively low. Moreover, cardiovascular risk factors were positively correlated with the emotion and disease management scores.CONCLUSIONS: CHD patients' self-management status in the long-term stage after PCI was moderate or poor. Medical staff should provide targeted guidance and education (for example, effective guidance on smoking cessation, first aid training, repeated disease education, and use of electronic devices to improve medication compliance) to improve the self-management level for the secondary prevention of CHD.

Predicting return to work after acute myocardial infarction: Socio-occupational factors overcome clinical conditions

Abstract:OBJECTIVES: Return to work after acute myocardial infarction (AMI), a leading cause of death globally, is a multidimensional process influenced by clinical, psychological, social and occupational factors, the single impact of which, however, is still not well defined. The objective of this study was to investigate these 4 factors on return to work (RTW) within 365 days after AMI in a homogeneous cohort of patients who had undergone an urgent coronary angioplasty.PARTICIPANTS: We studied 102 patients, in employment at the time of AMI (88.24% of men), admitted to the Department of Cardiology of the University-Hospital of Ferrara between March 2015 to December 2016. Demographical and clinical characteristics were obtained from the cardiological records. After completing an interview on social and occupational variables and the Hospital Anxiety and Depression (HADS) questionnaire, patients underwent exercise capacity measurement and spirometry.RESULTS: Of the 102 patients, only 12 (12.76%) held a university degree, 68.63% were employees and 31.37% self-employed. The median number of sick-leave days was 44 (IQR 33-88). At day 30, 78.5% of all subjects had not returned to work, at day 60, 40.8% and at day 365 only 7.3% had not resumed working. At univariate analyses, educational degree (p = 0.026), self-employment status (p = 0.0005), white collar professional category (p = 0.020) and HADS depression score were significant for earlier return to work. The multivariate analysis confirms that having a university degree, being self-employed and presenting a lower value of HADS depression score increase the probability of a quicker return to work.CONCLUSIONS: These findings suggest that the strongest predictors of returning to work within 1 year after discharge for an acute myocardial infarction are related more to socio-occupational than to clinical parameters.

Abstract:Objectives: In this study, we investigated the physical activity status and impact factors in patients after percutaneous coronary intervention in China. Methods: This cross-sectional survey was conducted from January to October 2019. A total of 260 participants were recruited in the cardiovascular department who had successfully undergone percutaneous coronary intervention after one year for physical re-examination at 2 hospitals in Henan Province. They were investigated using 5 questionnaires – the International Physical Activity Questionnaire-Long Form, the Exercise Self-efficacy Questionnaire, the Multi-dimensional Fatigue Scale, the Social Support Rating Scale, and the Hospital Anxiety and Depression scale. Results: After percutaneous coronary intervention, most patients were at low and moderate intensity physical activity and 60.8% of patients did not meet the recommended amount of guidelines. Through the analysis of the influencing factors of physical activity in patients with coronary heart disease stent implantation, the status of physical activity was affected by the NYHA classification, LVEF, exercise self-efficacy, fatigue, and social support. Conclusions: The level of physical activity of patients after percutaneous coronary intervention needs to be improved further. Abstract:In the smooth-muscle cells from guinea-pig ileum a highly developed sarcoplasmic reticulum has been observed immediately underneath the plasma membrane in close relationship to the caveolae intracellulares. It is suggested that sarcoplasmic reticulum in smooth muscle may play a similar role as in skeletal muscle, constituting the main intracellular store for calcium.

Abstract:People with coronary heart disease (CHD) often require prolonged absences from work to convalesce after acute disease events like myocardial infarctions (MI) or revascularisation procedures such as coronary artery bypass grafting (CABG) or percutaneous coronary intervention (PCI). Reduced functional capacity and anxiety due to CHD may further delay or prevent return to work. To assess the effects of person‐ and work‐directed interventions aimed at enhancing return to work in patients with coronary heart disease compared to usual care or no intervention. We searched the databases CENTRAL, MEDLINE, Embase, PsycINFO, NIOSHTIC, NIOSHTIC‐2, HSELINE, CISDOC, and LILACS through 11 October 2018. We also searched the US National Library of Medicine registry, clinicaltrials.gov, to identify ongoing studies. We included randomised controlled trials (RCTs) examining return to work among people with CHD who were provided either an intervention or usual care. Selected studies included only people treated for MI or who had undergone either a CABG or PCI. At least 80% of the study population should have been working prior to the CHD and not at the time of the trial, or study authors had to have considered a return‐to‐work subgroup. We included studies in all languages. Two review authors independently selected the studies and consulted a third review author to resolve disagreements. Two review authors extracted data and independently assessed the risk of bias. We conducted meta‐analyses of rates of return to work and time until return to work. We considered the secondary outcomes, health‐related quality of life and adverse events among studies where at least 80% of study participants were eligible to return to work. We found 39 RCTs (including one cluster‐ and four three‐armed RCTs). We included the return‐to‐work results of 34 studies in the meta‐analyses. Person‐directed, psychological counselling versus usual care We included 11 studies considering return to work following psychological interventions among a subgroup of 615 participants in the meta‐analysis. Most interventions used some form of counselling to address participants' disease‐related anxieties and provided information on the causes and course of CHD to dispel misconceptions. We do not know if these interventions increase return to work up to six months (risk ratio (RR) 1.08, 95% confidence interval (CI) 0.84 to 1.40; six studies; very low‐certainty evidence) or at six to 12 months (RR 1.24, 95% CI 0.95 to 1.63; seven studies; very low‐certainty evidence). We also do not know if psychological interventions shorten the time until return to work. Psychological interventions may have little or no effect on the proportion of participants working between one and five years (RR 1.09, 95% CI 0.88 to 1.34; three studies; low‐certainty evidence). Person‐directed, work‐directed counselling versus usual care Four studies examined work‐directed counselling. These counselling interventions included advising patients when to return to work based on treadmill testing or extended counselling to include co‐workers' fears and misconceptions regarding CHD. Work‐directed counselling may result in little to no difference in the mean difference (MD) in days until return to work (MD −7.52 days, 95% CI −20.07 to 5.03 days; four studies; low‐certainty evidence). Work‐directed counselling probably results in little to no difference in cardiac deaths (RR 1.00, 95% CI 0.19 to 5.39; two studies; moderate‐certainty evidence). Person‐directed, physical conditioning interventions versus usual care Nine studies examined the impact of exercise programmes. Compared to usual care, we do not know if physical interventions increase return to work up to six months (RR 1.17, 95% CI 0.97 to 1.41; four studies; very low‐certainty evidence). Physical conditioning interventions may result in little to no difference in return‐to‐work rates at six to 12 months (RR 1.09, 95% CI 0.99 to 1.20; five studies; low‐certainty evidence), and may also result in little to no difference on the rates of patients working after one year (RR 1.04, 95% CI 0.82 to 1.30; two studies; low‐certainty evidence). Physical conditioning interventions may result in little to no difference in the time needed to return to work (MD −7.86 days, 95% CI −29.46 to 13.74 days; four studies; low‐certainty evidence). Physical conditioning interventions probably do not increase cardiac death rates (RR 1.00, 95% CI 0.35 to 2.80; two studies; moderate‐certainty evidence). Person‐directed, combined interventions versus usual care We included 13 studies considering return to work following combined interventions in the meta‐analysis. Quality of life after percutaneous coronary intervention in the elderly with acute coronary syndrome

Abstract:Background Health-related quality of life (HRQoL) is an important but often neglected outcome measure in acute coronary syndrome (ACS) management. The prevalence of elderly presenting with ACS and undergoing percutaneous coronary intervention (PCI) is rising. We aimed to explore the impact of PCI on health status in elderly ACS patients. Methods We prospectively enrolled 624 patients admitted to our institution with ACS from February 2006 to May 2008. Short Form (SF)-36 health survey was used to assess HRQoL at baseline and 6months. Baseline characteristics and HRQoL were compared for patients treated with PCI within 30days of index ACS admission vs. medical therapy across 3 age groups (<60, 60–79 and ≥80years). Results PCI was performed in 73.6%, 55.7% and 21.3% in patients aged <60, 60–79 and older than 80years, respectively (p<0.01). Elderly patients were more likely to be female (16.9 vs. 35.4 vs. 54.6%, p<0.01) and had more co-morbidities (p<0.01). Older patients were less likely to undergo angiography (84.8 vs. 65.2 vs. 24.8%, p<0.01). Baseline HRQoL decreased with advancing age (p<0.01). However, elderly patients who underwent PCI experienced the most improvement in physical health than younger age groups. PCI was an independent predictor (Odds Ratio=1.79, 95% CI: 1.10–2.92) of better physical health status at 6months. Conclusion Elderly ACS patients who underwent PCI experienced the most improvement in physical health compared to younger patients. Our findings suggest that age per se should not deter against revascularization because of potential benefits in HRQOL.

Abstract:OBJECTIVE:To investigate the smoking cessation rate and its related factors in cardiovascular patients after percutaneous coronary intervention.METHODS:A cross-sectional survey of 1241 outpatients or inpatients fulfilling the inclusion criteria was conducted at 14 hospitals across China.RESULTS:The survey population mainly consisted of middle-aged male patients with an average age of (59.9 ± 10.7) years old. Their median time after surgery was 1.5 years. Among them, 77.4% tried to quit smoking while 55.4% succeed. The elder men were more likely to try to quit smoking and succeed eventually (OR values: 1.02 vs 1.03). The higher number of cigarettes smoked daily, the greater possibility of trying to quit smoking (OR = 1.03), but less likely to quit smoking eventually (OR = 0.97). Furthermore the years of smoking and cessation motivation had impacts on the cessation behaviors as well.CONCLUSION:Smoking cessation was achieved in more than half of the patients at one year after PCI. Age, years of smoking, daily number of cigarettes, concurrent diseases and physician recommendations are the influencing factors of smoking cessation.
