Abstract:The quest for a solid-state tunable dye laser can be satisfied by sol-gel prepared organic-inorganic hybrids. A photostability study of porous silica-Rhodamine 6G hybrids prepared via a sol-gel method is presented. The dye molecules can be incorporated into the silica matrix by forming weak or covalent bonds (hybrids of classes I and II, respectively). New class II samples and traditional class I materials prepared by the pre-doping method were synthesized. Samples were characterized by photoluminescence measurements to compare the emission properties and the photostability of the samples. The decay of the fluorescence signal as the cumulative excitation energy increases is reported and interpreted by hypothesizing that the dye molecules can be hosted in different surroundings within the porous glass matrix. The reported photoluminescence and photobleaching features indicate the class II samples as good candidates for solid-state dye lasers.
Work-related outcome after acute coronary syndrome: Implications of complex cardiac rehabilitation in occupational medicine

Abstract:Objectives: Coronary heart disease is frequent in the working-age population. Traditional outcomes, such as mortality and hospital readmission, are useful for evaluating prognosis. Fit-for-work is an emerging outcome with clinical as well as socioeconomic significance. We describe the possible benefit of a cardiac rehabilitation (CR) program for return to work (RTW) after acute coronary syndrome (ACS). Material and methods: We evaluated 204 patients with recent ACS. They were divided into 4 groups on the basis of their occupational work load: very light (VL), light (L), moderate (M), and heavy (H). Work-related outcomes were assessed with the Work Performance Scale (WPS) of the Functional Status Questionnaire and as "days missed from work" (DMW) in the previous 4 weeks. The variables considered for outcomes were percent ejection fraction, functional capacity expressed in metabolic equivalents (METs), and participation or non-participation in the CR program (CR+ and CR-). Results: One hundred thirty (66%) patients took part in the CR program. Total WPS scores for CR+ and CR- subgroups were VL group: 18±4 vs. 14±4 (p < 0.001), L group: 18±3 vs. 14±3 (p < 0.0001), M group: 19±3 vs. 16±3 (p < 0.003), and H group: 20±4 vs. 17±3 (p < 0.006). Fewer DMW were reported by the CR+ group. Conclusions: Non-participation in CR was a consistent cause of poorer work-related outcomes. Our findings indicate that CR and occupational counseling play a very important role in worker recovery and subsequent reintegration in the workplace, in particular among clerical workers.

Abstract:OBJECTIVES: Return to work after acute myocardial infarction (AMI), a leading cause of death globally, is a multidimensional process influenced by clinical, psychological, social and occupational factors, the single impact of which, however, is still not well defined. The objective of this study was to investigate these 4 factors on return to work (RTW) within 365 days after AMI in a homogeneous cohort of patients who had undergone an urgent coronary angioplasty.PARTICIPANTS: We studied 102 patients, in employment at the time of AMI (88.24% of men), admitted to the Department of Cardiology of the University-Hospital of Ferrara between March 2015 to December 2016. Demographical and clinical characteristics were obtained from the cardiological records. After completing an interview on social and occupational variables and the Hospital Anxiety and Depression (HADS) questionnaire, patients underwent exercise capacity measurement and spirometry.RESULTS: Of the 102 patients, only 12 (12.76%) held a university degree, 68.63% were employees and 31.37% self-employed. The median number of sick-leave days was 44 (IQR 33-88). At day 30, 78.5% of all subjects had not returned to work, at day 60, 40.8% and at day 365 only 7.3% had not resumed working. At univariate analyses, educational degree (p = 0.026), self-employment status (p = 0.0005), white collar professional category (p = 0.020) and HADS depression score were significant for earlier return to work. The multivariate analysis confirms that having a university degree, being self-employed and presenting a lower value of HADS depression score increase the probability of a quicker return to work.CONCLUSIONS: These findings suggest that the strongest predictors of returning to work within 1 year after discharge for an acute myocardial infarction are related more to socio-occupational than to clinical parameters.

Abstract:Health psychology sheds light on the process of returning to work after sick leave and subsequent medical rehabilitation. A 15-month longitudinal study with N = 201 orthopedic rehabilitation patients is reported. It examined whether and how both physiological health (e.g. body mass index and oxygen reabsorption) and psychological/social-cognitive factors (e.g. self-efficacy and social support) are related to returning to work. It was found that social-cognitive and physiological variables, such as oxygen consumption at anaerobic threshold, are important for returning to work. While physical activity was significantly correlated with social-cognitive variables, it was not significantly correlated directly with returning to work. Results suggest that self-efficacy and oxygen consumption at anaerobic threshold should be improved during medical rehabilitation.

Abstract:The stability of axisymmetric plasmas confined by a closed poloidal magnetic field is investigated using magnetohydrodynamic equations with anisotropic resistivity and sound waves retained. It is shown that when the system is axially and up–down symmetric and the plasma beta=(plasma pressure/magnetic pressure) is finite, a resistive instability with a growth rate proportional to the cube root of the resistivity exists at the ideal stability boundary for up–down antisymmetric modes. Both the ideal and resistive stability of a Z-pinch equilibrium and the point dipole equilibrium of Krasheninnikov, Catto, and Hazeltine [Phys. Rev. Lett. 82, 2689 (1999)] are studied in detail. For a Z pinch, ideal instabilities are found to always dominate over resistive instabilities. For the point dipole, ideal up–down antisymmetric modes are always stable, and the only resistive instabilities permitted have a growth rate proportional to the resistivity times the square of the azimuthal mode number.
Psychosocial predictors for cardiac rehabilitation participation for patients with coronary artery disease

Abstract:Aims: Cardiac rehabilitation (CR) is a class Ia recommendation by international guidelines. However, the participation remains disappointingly low. Multiple barriers were already identified. Nevertheless, the relationship with psychosocial factors remains unsettled. The aim of this study is to analyse the role of psychosocial factors in CR participation. Methods and results: A prospective study on 165 patients with a percutaneous coronary intervention. The main outcome was participation in a centre-based CR programme. The independent relationship between participation and optimism, type D personality or distressed personality, health literacy, depression, social support and self-efficacy. A total number of 165 patients were included in the study of which 81 (49.1%) participated in a centre-based CR programme. Participants were predominantly male. Solely, patients participating in CR were significantly younger (P = 0.005). Participants of CR had significantly higher levels of social support especially from their significant other (P = 0.020) and family (P = 0.050). Furthermore, the health literacy score was significantly higher in the participants group (P = 0.030). A multiple logistic regression demonstrated that higher age, higher social support, not working, being obese and being able to drive a car were significant statistical predictors for CR participation. Conclusions: This study is one of the first to demonstrate the role of social support and health literacy in the participation of CR. Other well-known barriers for CR participation such as age, transport and scheduling issues were identified as barriers in this study.

Abstract:People with coronary heart disease (CHD) often require prolonged absences from work to convalesce after acute disease events like myocardial infarctions (MI) or revascularisation procedures such as coronary artery bypass grafting (CABG) or percutaneous coronary intervention (PCI). Reduced functional capacity and anxiety due to CHD may further delay or prevent return to work. To assess the effects of person‐ and work‐directed interventions aimed at enhancing return to work in patients with coronary heart disease compared to usual care or no intervention. We searched the databases CENTRAL, MEDLINE, Embase, PsycINFO, NIOSHTIC, NIOSHTIC‐2, HSELINE, CISDOC, and LILACS through 11 October 2018. We also searched the US National Library of Medicine registry, clinicaltrials.gov, to identify ongoing studies. We included randomised controlled trials (RCTs) examining return to work among people with CHD who were provided either an intervention or usual care. Selected studies included only people treated for MI or who had undergone either a CABG or PCI. At least 80% of the study population should have been working prior to the CHD and not at the time of the trial, or study authors had to have considered a return‐to‐work subgroup. We included studies in all languages. Two review authors independently selected the studies and consulted a third review author to resolve disagreements. Two review authors extracted data and independently assessed the risk of bias. We conducted meta‐analyses of rates of return to work and time until return to work. We considered the secondary outcomes, health‐related quality of life and adverse events among studies where at least 80% of study participants were eligible to return to work. We found 39 RCTs (including one cluster‐ and four three‐armed RCTs). We included the return‐to‐work results of 34 studies in the meta‐analyses. Person‐directed, psychological counselling versus usual care We included 11 studies considering return to work following psychological interventions among a subgroup of 615 participants in the meta‐analysis. Most interventions used some form of counselling to address participants' disease‐related anxieties and provided information on the causes and course of CHD to dispel misconceptions. We do not know if these interventions increase return to work up to six months (risk ratio (RR) 1.08, 95% confidence interval (CI) 0.84 to 1.40; six studies; very low‐certainty evidence) or at six to 12 months (RR 1.24, 95% CI 0.95 to 1.63; seven studies; very low‐certainty evidence). We also do not know if psychological interventions shorten the time until return to work. Psychological interventions may have little or no effect on the proportion of participants working between one and five years (RR 1.09, 95% CI 0.88 to 1.34; three studies; low‐certainty evidence). Person‐directed, work‐directed counselling versus usual care Four studies examined work‐directed counselling. These counselling interventions included advising patients when to return to work based on treadmill testing or extended counselling to include co‐workers' fears and misconceptions regarding CHD. Work‐directed counselling may result in little to no difference in the mean difference (MD) in days until return to work (MD −7.52 days, 95% CI −20.07 to 5.03 days; four studies; low‐certainty evidence). Work‐directed counselling probably results in little to no difference in cardiac deaths (RR 1.00, 95% CI 0.19 to 5.39; two studies; moderate‐certainty evidence). Person‐directed, physical conditioning interventions versus usual care Nine studies examined the impact of exercise programmes. Compared to usual care, we do not know if physical interventions increase return to work up to six months (RR 1.17, 95% CI 0.97 to 1.41; four studies; very low‐certainty evidence). Physical conditioning interventions may result in little to no difference in return‐to‐work rates at six to 12 months (RR 1.09, 95% CI 0.99 to 1.20; five studies; low‐certainty evidence), and may also result in little to no difference on the rates of patients working after one year (RR 1.04, 95% CI 0.82 to 1.30; two studies; low‐certainty evidence). Physical conditioning interventions may result in little to no difference in the time needed to return to work (MD −7.86 days, 95% CI −29.46 to 13.74 days; four studies; low‐certainty evidence). Physical conditioning interventions probably do not increase cardiac death rates (RR 1.00, 95% CI 0.35 to 2.80; two studies; moderate‐certainty evidence). Performance Measures for Short-Term Cardiac Rehabilitation in Patients of Working Age: Results of the Prospective Observational Multicenter Registry OutCaRe

Abstract:Objective: To determine immediate performance measures for short-term, multicomponent cardiac rehabilitation (CR) in clinical routine in patients of working age, taking into account cardiovascular risk factors, physical performance, social medicine, and subjective health parameters and to explore the underlying dimensionality. Design: Prospective observational multicenter register study in 12 rehabilitation centers throughout Germany. Setting: Comprehensive 3-week CR. Participants: Patients (N=1586) ≤65 years of age (mean 53.8±7.3y, 77.1% men) in CR (May 2017-May 2018). Interventions: Not applicable. Main outcome measures: Feasibility, defined by data availability for ≥85% of patients (CR admission and discharge), and modifiability based on pre-post comparison (statistical significance, with P value<.01; standardized effect size≥.35; change by ≥5% points in categorical variables). In addition, latent factors were identified using an exploratory factor analysis (EFA). Results: Based on feasibility and modifiability criteria, smoking behavior, lifestyle change behavior, blood pressure, endurance training load, depression in Patient Health Questionnaire-9 (PHQ-9), the 5-item World Health Organization Well-Being Index (WHO-5), physical and mental health and pain scale of the indicators of rehabilitation status-24 (IRES-24), and self-assessed health prognosis proved to be suitable performance measures. As a result of the EFA, 2 solid factors were identified: (1) subjective mental health including PHQ-9, WHO-5, mental health (IRES-24), mental quality of life, and anxiety and (2) physical health including physical quality of life, physical health and pain scale of IRES-24, and self-assessed occupational prognosis. A third factor represents the blood pressure. Conclusions: We provide a small set of performance measures, that are essentially based on 3 latent factors (subjective mental health, physical health, blood pressure). These performance measures can represent immediate success of comprehensive CR and be applied easily in clinical practice.

Abstract:A 22-year-old Caucasian male was referred for evaluation of familial polyposis. During a barium contrast study of his colon, hypertrophy of the pedicles on the right side at T10-T12 and Li was noted. The patient denied symptoms related to his back. Physical and neumologic examinations were normal. There was no evidence of hypentrophy of any soft tissues. Laboratory studies including serum calcium, phosphate, and alkaline phosphatase were within normal limits. Complete spine films and skull films were negative, except for the might-sided hypertrophied pedicles in the lower thomacic and upper lumbar spine (fig. 1). The contralateral pedicles were normal or the same size as the pedicIes above and below the involved area. Serial laminograms in the antemopostemior and lateral projections demonstrated no evidence of a neural arch defect. The right pedicles measured from 0.6 cm to 1 cm larger in all diameters than the left pedicles (fig. 2). Associated hypertrophy of the lamina on the right and narrowing of the neural canal on the right were present (fig. 2C). A total body sw Tc pynophoSphate bone scan and pinhole collimated views over the lower thomacic to upper lumbar spine did not reveal abnormal uptake or discrepancy between the hypertmophied and contralateral normal pedicles.

Abstract:Background: Low health-related quality of life (HRQoL) is associated with adverse cardiovascular outcomes in coronary heart disease (CHD) patients. Cardiac rehabilitation (CR) improves HRQoL; however, evidence on long-term HRQoL changes after CR, and their predictors, is missing. Methods: A total of 153 patients with complete HRQoL data in the short-form (SF)-36 Health Survey at CR entry, discharge and follow-up were included. Using linear mixed-effects regression models for repeated time measurements, we examined predictors of follow-up HRQoL, including age and clinical characteristics. Results: Both physical (t = −5.66, p < 0.001) and mental (t = −2.06, p = 0.040) HRQoL improved significantly from CR entry to discharge, with improvements remaining stable over a mean follow-up of four years (range 2.4–6.1). Better functional capacity (6MWT) at CR entry predicted better physical HRQoL (t = 5.50, p < 0.001) and, with a trend, better mental HRQoL (t = 1.92, p = 0.056) at follow-up. A psychiatric diagnosis at CR entry predicted better mental HRQoL at follow-up (t = 3.85, p < 0.001). Conclusions: Improvements in HRQoL during CR remain stable during long-term follow-up. Levels of functional capacity appear to be relevant to both physical and mental HRQoL at follow-up.

Abstract:Chronic inflammatory diseases are a major health problem with global dimension. Particularly, the incidence of allergic diseases has been increased tremendously within the last decades. This world-wide trend clearly indicates the demand for new approaches in the investigation of early allergy development. Recent studies underlined the basic postulate of the hygiene hypothesis that early exposure to microbial stimuli plays a crucial role in the prevention of chronic inflammatory conditions in adulthood. There is ample evidence that, both, exogenous microbes and endogenous microbial communities, the human microbiota, shape the developing immune system and might be involved in prevention of pathologic pro-inflammatory trails. According to the Barker hypothesis, epidemiological studies pointed to transmaternal transmission from the mother to the offspring already in prenatal life. Experimental data from murine models support these findings. This state of the art review provides an overview on the current literature and presents new experimental concepts that point out to future application in the prevention of allergic diseases.

Abstract:Abstract Funding Acknowledgements Type of funding sources: None. Introduction Cardiac Rehabilitation Programs (CRP) still do not reach all patients with this indication. In the context of COVID-19 pandemic, the new home-CRP model has been introduced. Purpose To evaluate the results in terms of improvement of psychosocial outcomes and changes in employment situation with home-CRP. Methods Cross-Sectional Observational study that included patients of both sexes, ≥ 18 years, after acute coronary syndrome or after coronary revascularization, included in home CRP. Information was collected from the computerized clinical history of the patients (diseased vessels, degree and type of revascularization, risk stratification). Data was collected at the beginning and in the end after 2 months of the home CRP: smoking habit, weight, abdominal perimeter, anxiety/ depression (measured by HADS: Hospital Anxiety and Depression scale) and employment status. Results A total of 173 patients has been studied. Mean age was 51.6 (SD: 7.94), 15.6% women. Indication for home CRP was non ST elevation myocardial infarction (NSTEMI) in 45.7%, 31.8% had STEMI, 17.9% after elective coronary interventions ,3.5% after cardiac arrest, and 1.2% corresponded to other causes. Patients were at low risk in 50%, 48% moderate and 2% high risk. With 1, 2, or 3 vessels disease in 38.1%, 28.3%, and 33.5% respectively. Coronary revascularization was obtained by percutaneous interventions in 80.9%, by surgery in 14.5% and 4.6% of patients received only medical treatment. At baseline, 33.5% obese and 55% smoked at index admission. After two months of home PRHC we obtained a reduction of 85.5% in smoking (p<0.001), a loss of 4.6 kg of weight in obese people (p=0.125), reduction of anxiety (p= 0.0012) and depression (p=0.0013) amongst those who had a HADS scale higher than 10, and improvement in employment status with return to work in 61,2% (p<0.001). Conclusions The present study indicates that home- cardiac rehabilitation programs reduce anxiety, depression and improve return to work after acute coronary syndrome.

Abstract:This prospective study evaluated the effect of an individualized, comprehensive, home-based cardiac rehabilitation program combining exercise training with risk factor modification and psychosocial counseling on risk factors, psychological well-being, functional capacity, and work resumption in 99 post-percutaneous coronary interventions (PCI) patients randomized to control (standard care plus telephone follow-up, n=49) or intervention (individualized, comprehensive, home-based cardiac rehabilitation, n=50) groups. Data were collected at time 1 (T(1)) during hospital admission, time 2 (T(2)) approximately 2 months post-PCI, and time 3 (T(3)) approximately 12 months post-PCI. Results suggest that the allocation to an individualized, comprehensive, home-based cardiac rehabilitation program provided more advantageous outcomes. At both follow-ups, the intervention group showed within-group improvement in serum cholesterol levels (P<0.02; P<0.01) and exercise participation (P<0.001; P<0.001) with differences in exercise participation favoring the intervention group (P<0.01) at T(2). Repeated measures ANOVA showed significant improvements over time in body mass index (BMI) (P<0.01), psychological well-being (P<0.001), and functional capacity (P<0.001) for both groups. More patients in the intervention group had returned to work at T(2) (P<0.001) and did so more quickly (P<0.01). These findings suggest that an individualized, comprehensive, home-based cardiac rehabilitation program improves risk factor profiles and work resumption patterns for patients following PCI.

Associations Between Psychological Factors and Adherence to Health Behaviors After Percutaneous Coronary Intervention: The Role of Cardiac Rehabilitation

Abstract:Abstract Background Cardiac rehabilitation (CR) participation after percutaneous coronary intervention (PCI) for coronary heart disease lowers the disease burden and risk of recurrent cardiac events. Examining psychological factors may improve post-PCI health behavior adherence. Purpose To determine whether psychological factors are associated with post-PCI health behavior adherence, and the role of CR participation. Methods Data from 1,682 patients (22.1% female, Mage = 64.0, SDage = 10.5 years) from the THORESCI cohort were included. Adjusted mixed models were used to examine associations between psychological factors and the 1-year course of health behaviors, using interactions to test for moderation by CR participation. Results Psychological factors were associated with the trajectories of adherence to medical advice, exercise, and diet. The strongest association found was between optimism and the trajectory of dietary adherence (B: = −0.09, p = .026). Patients with high optimism levels had a worse trajectory of dietary adherence compared to patients with low to middle optimism levels. Participation in CR buffered the associations of high anxiety, pessimism, and low to middle resilience, but strengthened the associations of high stress in the past year with the probability of smoking. Conclusions Psychological factors are associated with post-PCI health behavior adherence, but the pattern of associations is complex. Patients with high levels of anxiety, pessimism, and low to middle resilience levels may disproportionately benefit from CR. Cardiac rehabilitation programs could consider this to improve post-PCI health behavior adherence. Clinical Trials Registration # NCT02621216.

Abstract:Patients who participate in cardiac rehabilitation programs (CRP) experience significant improvements in quality of life, rehospitalization rates, and mortality associated with cardiovascular disease. The potential efficacy of CRP is limited however, by significant program dropout rates and poor patient adherence to prescribed exercise regimens following rehabilitation. Recently, models of motivational readiness for behavior change, such as the Transtheoretical Model, have been applied to understanding the process of exercise adoption and maintenance. Interventions based on this dynamic model of behavior change have produced significant improvement in adherence to exercise in community and worksite populations. This study investigates the applicability of this model to this special population. Sixty-two men and women completed measures of motivational readiness, self-efficacy, and decisional balance for exercise adoption upon entry into a 12-week cardiac rehabilitation program at posttreatment and at a 3-month follow-up. Patients made signi ficant gains during CRP in time spent exercising and self-efficacy, but not in utilization of the cognitive processes or in the perceived benefits of exercising. Exercise maintenance at follow-up was differentially associated with self-efficacy, decisional balance, and use of behavioral processes at posttreatment. Motivation-based models of exercise adoption may provide insights regarding the adoption and maintenance of regular physical activity in cardiac rehabilitation populations.

Effectiveness of Graded Return to Work After Multimodal Rehabilitation in Patients with Mental Disorders: A Propensity Score Analysis

Abstract:Purpose Graded return to work (GRTW) is a strategy aimed at bringing people gradually back to coping with a full workload after an extended period of sick leave. This study aims to determine the effect of GRTW in addition to a multimodal rehabilitation on longer-term work participation in people with chronic mental disorders (CMDs). Methods Patients filled out questionnaires at the start of a multimodal rehabilitation and 15 months later. Balanced groups (GRTW, no GRTW) were formed by propensity score matching based on 27 covariates. The primary outcome measures were the return to work (RTW) status at follow-up and the number of days on sick leave during follow-up. Results From 1062 data sets (GRTW 508, no GRTW 554), 381 pairs were matched (age: 47.8 years; 78% female; 65% affective disorders, 28% neurotic or somatic disorders). At follow-up, 88% of the GRTW group had returned to work compared to only 73% of the controls (RR = 1.22, 1.13-1.31). The mean sick leave duration during the follow-up period was 7.0 weeks in the GRTW group compared to 13.4 weeks in the control group (p < 0.001). Additional explorative analyses showed that these effects were only observed in patients with an unsure or negative subjective RTW prognosis. Conclusions Based on this analysis, GRTW in addition to a multimodal rehabilitation is effective in enhancing successful work participation in people with CMDs. Earlier studies showing larger effects in people with a higher risk of a non-RTW were confirmed.
Expectation, satisfaction, and predictors of dropout in cardiac rehabilitation

Abstract:Background: Despite the evidence of benefit many patients fail to adhere to cardiac rehabilitation (CR). This study aims at describing the rate, reasons, and predictors of dropout from CR, as well as expectations und satisfaction among participants. Methods: Prospective analysis of demographic, medical, and psychosocial data in a large cohort of consecutive participants of outpatient CR. Results: Between 1999 and 2008, 2521 consecutive patients were enrolled in CR and complete data for analysis were available in 2371 patients. Diagnoses were coronary heart disease in 85%, valvular heart disease in 15%, and other in 10%. Mean age was 59.7 ± 11.4 years; 85% were male. Specific expectations towards CR were identified in a subgroup of participants and satisfaction was high in 75% of patients. Early dropout was noted in 305 patients (12.9%); reasons for dropout were cardiac in 39 (1.7%) and non-cardiac in 266 (11.2%). Significant differences between dropouts vs. completers were: peak exercise capacity (116 ± 41 vs. 123 ± 39 Watt, p = 0.003); body mass index (28 ± 7 vs. 27 ± 4 kg/m(2), p = 0.04); prevalence of diabetes (18 vs. 13%, p = 0.011); smoking (32 vs. 16%, p < 0.001); being professionally active (24 vs. 34%, p = 0.007); widowhood (8 vs. 3%, p = 0.04); higher education (37.5 vs. 46.4%, p = 0.003); white collar work (21 vs. 27%, p = 0.012); and scales of a quality-of-life score (p < 0.02). Multivariate analysis revealed low exercise capacity, high body mass index, smoking, diabetes, and widowhood to be significant independent predictors for early dropout. Conclusion: In the present study we demonstrated a low rate of mainly non-cardiac dropout from outpatient CR in motivated and largely satisfied participants. In order to improve adherence, early attention and close supportive counselling is recommended in patients with low exercise capacity and those who are obese, diabetic, smokers, or bereaved of a partner.
