Abstract:Radiation dose management of medical staff has become increasingly important. Particularly, based on the statement by the International Commission on Radiological Protection (ICRP) in 2011, a new lower equivalent dose limit for the eye lens is being established in each country. Although many reports have discussed the occupational radiation dose in interventional radiology (IR), few studies have examined the dose during computed tomography (CT) examinations. This study investigated the radiation dose exposure to medical staff present in the CT room during irradiation, with particular focus on the exposure to eye lens. The radiation dose exposure to those who assist patients during head, chest and upper abdomen CT examination was measured in a phantom study. The radiation dose exposure with scattered radiation was never negligible (i.e., high); Hp(3) was the highest in head CT examination, at 0.44 mSv per examination. Furthermore, the shielding effect of radiation protection glasses was large, and radiation protection glasses are useful tools for the medical staff who are involved in CT examinations. The justification and optimisation should be carefully considered in assistant actions.
Evaluation of Radiation Protection Methods for Assistant Staff during CT Imaging in High-energy Trauma: Lens Dosimetry with a Phantom Study

Abstract:Staff are exposed to radiation in the scanning room when assisting with CT scans of patients requiring ventilatory support during procedures. We measured lens doses using a phantom during a high-energy trauma protocol. Dosimetry showed that the unprotected lens received 2.02 mGy on the right and 1.91 mGy on the left, which are not negligible doses. Respective exposures to the right and left lens were 53.6% and 55.1% when wearing 0.07 mm Pb protective glasses with side covers; 53.7% and 64.2% when wearing 0.7 mm Pb glasses without side covers when facing away from the patient couch; and 92.1% and 91.2% using protective shielding in the gantry. Since the face direction may change during assistance with CT imaging, it is desirable that the protective glasses have a shape with a side cover. The protective shielding had a major radiation reduction effect, although it is expensive to acquire, install, and maintain.

Abstract:Given the new recommendations for occupational eye lens doses, various lead glasses have been used to reduce irradiation of interventional radiologists. However, the protection afforded by lead glasses over prescription glasses (thus over-glasses-type eyewear) has not been considered in detail. We used a phantom to compare the protective effects of such eyewear and regular eyewear of 0.07-mm lead-equivalent thickness. The shielding rates behind the eyewear and on the surface of the left eye of an anthropomorphic phantom were calculated. The left eye of the phantom was irradiated at various angles and the shielding effects were evaluated. We measured the radiation dose to the left side of the phantom using RPLDs attached to the left eye and to the surface/back of the left eyewear. Over-glasses-type eyewear afforded good protection against X-rays from the left and below; the average shielding rates on the surface of the left eye ranged from 0.70–0.72. In clinical settings, scattered radiation is incident on physicians' eyes from the left and below, and through any gap in lead glasses. Over-glasses-type eyewear afforded better protection than regular eyewear of the same lead-equivalent thickness at the irradiation angles of concern in clinical settings. Although clinical evaluation is needed, we suggest over-glasses-type Pb eyewear even for physicians who do not wear prescription glasses.

Abstract:Purpose: To study and compare the shielding effect of radiologists’eye lens exposure with lead glasses of different equivalent thickness and size in interventional procedures. Methods: Using the human voxel phantom and MCNPX code, eye lens doses of the radiologists were simulated under different conditions of wearing lead glasses and beam projections, and measurements were performed with the physical model by using eye lens dosimeters to verify simulation results. Results: Simulation results show that the eye lens doses were reduced by a factor from 3 to 9 when wearing a 20 cm 2 sized led glasses with the equivalent thickness ranging from 0.1 to 1.0 mm Pb. The dose reduction factors (DRF) not only depend on the lead equivalent, but also the beam projection of X-rays. However, the increase of DRF was not significant whenever the lead equivalent of glasses was larger than 0.35 mm. Furthermore, the DRF was proportional to the size of glass lens from 6 cm 2 to 30 cm 2 with the same lead equivalent. Conclusion: For more reasonable and effective protection of the eye lens of interventional radiologists, a pair of glasses with a lead equivalent of 0.5 mm and large-sized lens is recommended.

Abstract:In CT examinations, the usefulness of protective glasses for reducing lens exposure to assistants has been reported. The present study aimed to compare the dose reduction effect for assistants with lead-acrylic shields and protective glasses (0.07 mmPb, 0.5 mmPb) during CT examination. The air dose distribution in a CT examination room with and without a lead-acrylic shield was compared. It was found that the amount of scattered radiation was significantly reduced by installing a lead-acrylic shield at the CT gantry aperture. Moreover, the reduction rate of air kerma at the assistant's lens was higher using the lead acrylic shield than with the protective glasses—95.7% during head holding and 76.1% during assisted ventilation.

Abstract:According to International Commission of Radiological Protection (ICRP), the equivalent dose limit for the eye lens for occupational exposure is recommended to be 20 mSv/y, averaged over 5 years, with no single year above 50 mSv. Some studies reported the measurement of assistant's lens exposure in diagnostic computed tomography (CT) examinations, but further investigation is still required in the association between the lens dose for assistants and various dose parameters. Therefore, we measured the assistant's lens exposure using small optically stimulated luminescence dosimeters. The type of occupation, type of assistance, total scan time, total mAs, total scan length, and dose-length product (DLP) were recorded and analyzed in association with air kerma at the lens position. The assistance was classified into four types: "assisted ventilation," "head holding," "body holding," and "raising patient's arm." The air kerma of lens position was not significantly different for each assistance type (p < 0.05, Kruskal-Wallis test). Further, the lens doses for assistants correlated with DLP, but with various strengths of correlation with the assistance type and were influenced by the distance from the CT gantry. In conclusion, lens dose during assistance and DLP demonstrated the strongest correlation. "Raising patient's arm" and "head holding" exhibited stronger correlations, which required less table movement during the CT scan than "assisted ventilation" and "body holding."

Abstract:Purpose:To study how eyeglass design features and postures of the interventional radiologist affect the radiation dose to the lens of the eye.Methods:A mesh‐based deformable phantom, consisting of an ultra‐fine eye model, was used to simulate postures of a radiologist in fluoroscopically guided interventional procedure (facing the patient, 45 degree to the left, and 45 degree to the right). Various eyewear design features were studied, including the shape, lead‐equivalent thickness, and separation from the face. The MCNPX Monte Carlo code was used to simulate the X‐ray source used for the transcatheter arterial chemoembolization procedure (The X‐ray tube is located 35 cm from the ground, emitting X‐rays toward to the ceiling; Field size is 40cm X 40cm; X‐ray tube voltage is 90 kVp). Experiments were also performed using dosimeter placed on a physical phantom behind eyeglasses.Results:Without protective eyewear, the radiologist's eye lens can receive an annual dose equivalent of about 80 mSv. When wearing a pair of lead eyeglasses with lead‐equivalent of 0.5‐mm Pb, the annual dose equivalent of the eye lens is reduced to 31.47 mSv, but both exceed the new ICRP limit of 20 mSv. A face shield with a lead‐equivalent of 0.125‐mm Pb in the shape of a semi‐cylinder (13cm in radius and 20‐cm in height) would further reduce the exposure to the lens of the eye. Examination of postures and eyeglass features reveal surprising information, including that the glass‐to‐eye separation also plays an important role in the dose to the eye lens from scattered X‐ray from underneath and the side. Results are in general agreement with measurements.Conclusion:There is an urgent need to further understand the relationship between the radiation environment and the radiologist's eyewear and posture in order to provide necessary protection to the interventional radiologists under newly reduced dose limits.
Assessment of the health impact of occupational risk in Africa: current situation and methodological issues.

Abstract:This paper presents information from the current monitoring systems in Africa, mainly Southern Africa, for occupational illness and injury and discusses the quality of the reported data in estimating the health impact of occupational risk. The paper presents and discusses the current profile of reported injury and fatalities for those countries for which data are available, in particular for the countries of the Southern African Development Community. These data indicate that the reported annual injury rates for wage workers in the Southern African Development Community region range widely from 0.35 to 49.42 injuries per 1,000 workers, and reported occupational fatality in the region ranges from 0.85 to 21.6 fatalities per 100,000 workers. Despite wide variability in reported rates (probably caused by variability in coverage and accuracy of reporting systems), transport, agriculture, mining and, to a lesser extent, construction consistently make up about three-quarters of all fatalities, with vehicle- or transport-related causes accounting for high proportions of fatal accidents. The paper identifies and discusses major sources and direction of bias and error in the reported data and suggests approaches for a better assessment of the health impact of occupational illness, injury, and mortality in African countries.

Abstract:Medical staffs like radiological technologists, doctors, and nurses are at an increased risk of exposure to radiation while assisting the patient in a position or monitor contrast medium injection during computed tomography (CT). However, methods to protect medical staff from radiation exposure and protocols for using radiological protection equipment have not been standardized and differ among hospitals. In this study, the distribution of scattered X-rays in a CT room was measured by placing electronic personal dosimeters in locations where medical staff stands beside the CT scanner gantry while assisting the patient and the exposure dose was measured. Moreover, we evaluated non-uniform exposure and revealed effective techniques to reduce the exposure dose to medical staff during CT. The dose of the scattered X-rays was the lowest at the gantry and at the examination table during both head and abdominal CT. The dose was the highest at the trunk of the upper body of the operator corresponding to a height of 130 cm during head CT and at the head corresponding to a height of 150 cm during abdominal CT. The maximum dose to the crystalline lens was approximately 600 μSv during head CT. We found that the use of volumetric CT scanning and X-ray protective goggles, and face direction toward the gantry reduced the exposure dose, particularly to the crystalline lens, for which lower equivalent dose during CT scan has been recently recommended in the International Commission on Radiological Protection Publication 118.
Influences of Operator Head Posture and Protective Eyewear on Eye Lens Doses in Interventional Radiology: A Monte Carlo Study

Abstract:Purpose: To quantify the effects of operator head posture and different types of protective eyewear on the eye lens dose to operators in interventional radiology (IR). Methods: A deformable computational human phantom, Rensselaer Polytechnic Institute (RPI) Adult Male, consisting of a high-resolution eye model, was used to simulate a radiologist who is performing an interventional radiology procedure. The radiologist phantom was deformed to a set of different head postures. Three different protective eyewear models were incorporated into the posture-deformed radiologist phantom. The eye lens dose of the radiologist was calculated using the Monte Carlo code, MCNP. Effects of the radiologist's head posture and different types of protective eyewear on eye lens doses were studied. The relationship between efficacy of protective eyewear and the radiologist's head posture was investigated. Effects of other parameters on efficacy of protective eyewear were also studied, including the angular position of the radiologist, the gap between the eyewear and the face of the radiologist, and the lead equivalent thickness. Results: The dose to both lenses decreased by 80% as the head posture moved from looking downward to looking upward. Sports wrap glasses were found to reduce doses further than the other two studied models. The efficacy of eyewear was found to be related to radiologist's head posture as well. When the radiologist was looking up, the protective eyewear almost provided no protection to both lenses. Other factors such as the face-to-eyewear distance and the lead equivalent thickness were also found to have an impact on the efficacy of protective eyewear. The dose reduction factor (DRF), defined as the ratio of the dose to the lens without protection to that with protection, decreased from 4.25 to 1.07 as the face-to-eyewear distance increased. The DRF almost doubled when the lead equivalent thickness increased from 0.07 to 0.35 mm. However, further increase in lead equivalent thickness showed little improvement in dose reduction. Conclusion: The radiologist's head posture has a significant influence on the eye lens dose in IR. Sports wrap protective eyewear which conforms to the curve of the face is essential for the radiation protection of the eye lens. However, the radiologist's head posture and other exposure parameters should be considered when evaluating the protection of the radiologist's eyes. Abstract:The ICRP 2011 Seoul Statement recommended a reduction in the dose limit for lens exposure to 100 mSv for 5 y and 50 mSv for 1 y. Based on this recommendation, the dose limit for lens exposure was lowered in Japan with the revision of the Ionization Regulations, which took effect in April 2021. In the present study, lens doses were measured during fluoroscopic procedures performed in four departments (Urology, Pediatrics, Gastroenterology, and Orthopedics). Lens doses were measured without protective eyewear for 6 mo (pre-intervention) and then with protective eyewear for the next 6 mo (post-intervention). Monthly doses were collected and lens doses before and after the use of protective eyewear were calculated as the lens dose per unit time. The use of protective eyewear reduced the lens dose per unit time by approximately two thirds. In all departments, the lens dose was slightly lower after than before the intervention. A significant difference was observed in lens doses between the pre- and post-intervention periods in the Urology department. The present results demonstrated the effectiveness of protective eyewear in daily practice. Therefore, the use of protective eyewear is recommended during fluoroscopic procedures.

Abstract:PURPOSE:To investigate lens dose reduction with organ based tube current modulation (TCM) using the Monte Carlo method.METHODS:To calculate lens dose with organ based TCM, 36 pairs of X-ray sources with bowtie filters were placed around the patient head using a projection angle interval of 10° for one rotation of Computed Tomography (CT). Each projection was simulated respectively. Both voxelized and stylized eye models and Chinese reference male phantoms were used in the simulation, and tube voltages 80, 100, 120 and 140 kVp were used.RESULTS:Dose differences between two eye models were less than 20%, but large variations were observed among dose results from different projections of all tube voltages investigated. Dose results from 0° (AP) directions were 60 times greater than those from 180° (PA) directions, which enables organ based TCM reduce lens doses by more than 47%.CONCLUSIONS:Organ based TCM may be used to reduce lens doses. Stylized eye models are more anatomically realistic compared with voxelized eye models and are more reliable for dose evaluation.
[Development of the Radiation Protective Curtain for the Dose Reduction of Caregivers during Computed Tomography Examinations]

Abstract:Purpose: During computed tomography (CT) examinations, it may be necessary to assist the patient to maintain an appropriate body position. However, there is little protective equipment available for caregivers who approach the gantry. This study aimed to evaluate the effectiveness of novel radiation protective curtains in reducing radiation exposure to caregivers while assisting patients, especially during CT examinations of the head. Method: The absorbed dose in air around the gantry during CT examinations of the head was measured using glass dosimeters. The measurement points from the center of the gantry were 40 to 120 cm in the front, 0 to 100 cm for each side in the right and left, and 60 to 180 cm from the floor. Measurements were performed at each 20-cm interval, and all points were accumulated 10 times. The absorbed dose in air in a CT room was compared with and without the protective curtains. Next, we assumed the height of the caregiver to be 170 cm, and measured the points for the crystalline lens, chest, and abdomen. Also, using the protective glasses and the protective apron, we measured the absorbed dose in air for the caregivers behind the protective curtains. Result: The absorbed exposure dose in air toward the crystalline lenses, chest, and abdomen was reduced more than 90% by using the protective curtains and more than 95% by using the protective apron and protective glasses in addition to them. Conclusion: This study showed the usefulness of protective curtains in reducing the absorbed exposure dose in air to caregivers.

Abstract:Abstract This paper summarizes the dose to the eye lens of workers of Memorial Sloan Kettering Cancer Center, a high-volume US oncologic and associated diseases facility. The doses presented in this report were collected from personal dosemeter readings using optically stimulated luminescence badges to estimate Hp(3). Doses were collected for 5950 clinical and research workers between January 2012 and December 2017. The median eye lens dose for all monitored workers was 0.23 mSv y−1. Workers performing, or supporting, fluoroscopy procedures received the highest unprotected eye lens dose of all workers with a median eye dose of 10 mSv. The use of leaded glasses by this group reduced the actual doses to the lens. Nurses and technicians involved in positron emission tomography injections received median eye lens dose of 1.2 mSv.

Abstract:Occupational radiation exposure to the eye lens of medical staff during endoscopic retrograde cholangiopancreatography (ERCP) should be kept low so as not to exceed annual dose limits. Dose should be low to avoid tissue reactions and minimizing stochastic effects. It is known that the head and neck of the staff are exposed to more scattered radiation in an over-couch tube system than in a C-arm system (under-couch tube). However, this is only true when radiation-shielding curtains are not used. This study aimed to compare the protection radiation to the occupationally exposed worker between a lead curtain mounted on a C-arm system and an ERCP-specific lead curtain mounted on an over-couch tube system. A phantom study simulating a typical setting for ERCP procedures was conducted, and the scattered radiation dose at four staff positions were measured. It was found that scattered radiation doses were higher in the C-arm with a lead curtain than in the over-couch tube with an ERCP-specific lead curtain at all positions measured in this study. It was concluded that the over-couch tube system with an ERCP-specific lead curtain would reduce the staff eye dose by less than one-third compared to the C-arm system with a lead curtain. For the C-arm system, it is necessary to consider more effective radiation protection measures for the upper body of the staff, such as a ceiling-suspended lead screen or another novel shielding that do not interfere with procedures.

Abstract:Studies have shown that there is high radiation exposure to medical staff during computed tomography fluoroscopy (CTF)-guided procedures. This study aims to investigate staff dose reduction techniques considering the CTF gantry positioning in the room and room dimensions in addition to the conventional use of thyroid collars, aprons and eye goggles. A Toshiba Aquilion One 640 slice CT scanner and CTF room were modelled using SimpleGeo. Standing and supine adult mesh phantoms were used to represent the staff and patient. The models were spatially put together on one platform using VOXEL2MCNP. Based on this, MCNPX input files were generated for the studies. CTF gantry and staff positions, and CTF room size were varied for different scenarios. Effective, eye lens and thyroid dose to staff were estimated for each scenario. Characterization of the human type I interferon receptor by ligand blotting

Abstract:The human type I interferon (IFN) receptor was characterized by ligand blotting. In this method, plasmalemma proteins or detergent‐lysed whole‐cell extracts from human Burkitt lymphoma cell lines were separated on polyacrylamide gels and subsequently transferred onto nitrocellulose sheets. Probing the blots with 3 × 10−10 M 125I‐labeled recombinant IFN‐αA (125I‐rIFN‐αA) revealed an IFN‐α‐binding protein with an apparent molecular mass of 95 kDa (p95). Performing the electrophoretic run under reducing conditions completely abrogated the signal on the blot, indicating that the type I IFN receptor contains a'disulfide bond essential for IFN binding. Optimal binding of 125I‐rIFN‐αA occurred at pH 9. The specificity of the binding reaction was established by simultaneously adding an excess of unlabeled IFN during incubation of the blots with 125I‐rIFN‐αA. The addition of either unlabeled IFN‐α or IFN‐β, but not IFN‐γ, abolished the binding of 125I‐rIFN‐αA to p95. 125I‐rIFN‐γ at 1.25 × 10−11 M bound to two proteins distinct from p95 with apparent molecular mass of 92 and 87 kDa, respectively. Saturability of 125I‐rIFN‐αA binding was demonstrated by probing a constant amount of membrane proteins with increasing amounts of 125I‐rIFN‐αA. Scatchard analysis of the binding data yielded an apparent Kd of 5.4 × 10−10 M for the immobilized type I IFN receptor. The expression of p95 on IFN‐α‐resistant and ‐sensitive cells was indistinguishable. We conclude that p95 is the IFN‐α/β receptor and that two proteins (p92 and p87) can specifically bind IFN‐γ. These results indicate that ligand blotting is a versatile method for characterization of unmodified IFN receptors and IFN‐receptor interaction and could also provide a new investigational approach for other cytokine receptor systems.
Modulation of the functions of dengue virus-specific human CD8+ cytotoxic T cell clone by IL-2, IL-7 and IFN gamma.

Abstract:Lymphokines play an important role in immune responses to viruses by modulating functions of T lymphocytes. In the present study, we examined the effects of interleukin-2 (IL-2), interleukin-4 (IL-4), interleukin-6 (IL-6), interleukin-7 (IL-7), and interferon gamma (IFN gamma) on proliferation, cytotoxic activity and lymphokine production of a dengue virus-specific CD8+ human cytotoxic T lymphocyte (CTL) clone. IL-2 and IL-7 induced proliferation of the CD8+ CTL clone in the presence or absence of specific antigen, while IFN gamma suppressed proliferation of the clone. IL-7 and IFN gamma augmented dengue virus-specific cytotoxic activity without inducing non-specific cytotoxic activity, and IL-2 induced non-specific cytotoxic activity. IL-2 induced IFN gamma production by the CD8+ CTL clone. IL-4 and IL-6 did not modulate the functions of the CD8+ CTL clone in these experimental conditions. These results suggest that functions of dengue virus-specific CD8+ CTL are modulated by IL-2, IL-7 and IFN gamma, and that IL-7 is a lymphokine useful to induce growth and to maintain specific cytotoxic activity of CD8+ CTL clones in vitro.
