Abstract:Introduction of clinical high-field MRI has raised interest in the use of clinical imaging--the efficacy of which has not yet been fully established--in daily practice. A high signal-to-noise ratio and profound susceptibility effects can improve the spatial resolution and image contrast of clinical imaging, whereas the heating effects of the radio frequency tend to prolong acquisition time. As well, inhomogeneities in the static or local magnetic fields can have a negative effect on image quality. The T(1) prolongation may affect T(1) contrast yet improve the enhancement effect of gadolinium chelate and the inflow effect of MR angiography. High-contrast imaging, such as the short inversion-time inversion recovery technique, can provide excellent intra- and extracerebral contrast comparable to microscopic or macroscopic specimens. High-field systems can also be applied to microscopic imaging. High-field MRI is expected to have an increased clinical impact in the near future. Technological advances tailored to high-field systems, as well as the accumulation of scientific evidence, will be necessary to establish its predominance over conventional MRI.
Effects of Group Exercise Intervention on Quality of Life and Physical Parameters in Patients with Childhood Cancer: A Systematic Review

Abstract:Background: Although the survival rates of childhood cancer are increasing, children diagnosed as having cancer experience psychological and physical problems and a declining quality of life (QOL). Methods: A systematic review of PubMed databases was conducted up to September 2023 to identify studies reporting the effects of group exercise intervention in children with cancer. The inclusion criteria were pre-specified, including children aged ≤19 years old who received group exercise intervention and interventional studies written in English. Studies involving non-exercise intervention or non-group intervention were excluded. Results: Five studies were included in the present review. In three studies, QOL and physical parameters were improved after group exercise intervention, and in two studies, only physical parameters were improved. Improvements in QOL were achieved through psychosocial variables, improved scores of subscales of pain and hurt, nausea, and procedure-related anxiety, and reduced cancer-related fatigue. All studies had high numbers of participants who completed the intervention. However, all studies showed a high risk of bias regarding the selection of the reported results, and most studies showed a high risk of bias regarding deviations from the intended intervention and outcome measurement. Conclusion: The reviewed studies showed that group exercise intervention for children with cancer could improve their QOL and/or physical parameters.

Abstract:Background Cancer‐related fatigue (CRF) emerges as a common symptom in pediatric cancer patients during treatment. Exercise interventions are increasingly being used as CRF interventions to improve CRF in children with cancer. Aim The objective of this meta‐analysis was to synthesize the best available evidence concerning the effectiveness of exercise interventions for cancer‐related fatigue in children with cancer. Methods Six databases were extensively searched from inception to December 2023 to identify relevant randomized controlled trials. The risk of bias and methodological quality were assessed using the Cochrane appraisal tool. Pooled effects were calculated using a random‐effects model. Heterogeneity was assessed using the I2 test. Results Eight trials (n = 465) were finally included. Abstract:PurposeThe purpose of this systematic review with meta-analysis was to evaluate the safety, feasibility and effectiveness of exercise in the palliative care phase for people with advanced cancer.MethodsElectronic databases were searched for exercise randomised controlled trials involving individuals with incurable cancer that were published prior to April 14, 2021. Meta-analyses were performed to evaluate the effects of exercise on health outcomes. Subgroup effects for exercise mode, supervision, intervention duration and cancer diagnosis were assessed.ResultsTwenty-two trials involving interventions ranging between 2 weeks and 6 months were included. Interventions comprised of aerobic (n = 3), resistance (n = 4), mixed-mode (n = 14) and other exercise (n = 1) modalities. Cancer types consisted of lung (n = 6), breast (n = 3), prostate (n = 2), multiple myeloma (n = 1) and mixed cancer types (n = 10). Meta-analysis of 20 RCTs involving 1840 participants showed no difference in the risk of a grade 2–4 adverse event between exercise and usual care (n = 110 adverse events (exercise: n = 66 events; usual care: n = 44 events), RD = − 0.01 (91% CI = − 0.01, 0.02); p = 0.24). Overall median recruitment, retention and adherence rates were 56%, 80% and 69%, respectively. Meta-analysis of health outcomes showed effects in favour of exercise for quality of life, fatigue, aerobic fitness and lower-body strength (SMD range = 0.27–0.48, all p < 0.05).ConclusionsParticipants who engaged in exercise experienced an increase in quality of life, fitness and strength and a decrease in fatigue.Implications for Cancer SurvivorsPhysical activity programs were found to be safe and feasible for people with advanced cancer in the palliative care phase.

Abstract:Objective: In cancer patients, the impairment in muscle function is a frequently observed phenomenon. However, comprehensive evaluation of the effect of exercise training on muscle function in childhood cancer patients (CCPs) is sparse and therefore investigated in the MUCKI trial.Study Design: In the randomized controlled MUCKI trial, CCPs during intensive cancer treatment and aged 4–18 years were recruited. Eligible patients were enrolled soon after diagnosis as long as they were physically and mentally able to participate in exercise testing and training. Patients of the exercise group (n = 16) participated in average 2.7 ± 1.2 times per week in a combined resistance and endurance training with moderate exercise intensity, for a time period of 8.0 ± 2.1 weeks, while patients of the control group (n = 17) received usual care. Leg strength was evaluated as the primary endpoint. Secondary endpoints were 6-min walk performance, arm strength, body composition, fatigue, and health-related quality of life.Results: Comparisons of pre- and post-intervention results were evaluated by baseline and stratification criteria adjusted analysis and showed positive effects for the exercise group regarding leg strength [F(1, 20) = 5.733; p = 0.027*; <math>ηp2</math> = 0.223], walking performance [F(1, 25) = 4.270; p = 0.049*; <math>ηp2</math> = 0.146], fatigue [F(1, 13) = 8.353; p = 0.013*; <math>ηp2</math> = 0.391], self-esteem [F(1, 6) = 6.823; p = 0.040*; <math>ηp2</math> = 0.532], and self-reported strength and endurance capacity [F(1, 6) = 6.273; p = 0.046*; <math>ηp2</math> = 0.511]. No significant differences were found for the other parameters.Conclusion: Within one of the first randomized controlled trials, the present study provides evidence for a positive effect of combined training in CCPs during intensive cancer treatment. Further research is needed to confirm these results and to evaluate their clinical impact.Clinical Trial Registration Number: NCT02612025.

Mycophenolic Acid (Cellcept and Myofortic) Induced Injury of the Upper GI Tract

Abstract:Background Mycophenolic acid (MPA) is an immunosuppressant drug commonly used in patients undergoing solid organ transplant. Although its pattern of inducing injury in the colon is well-known and features prominent crypt apoptosis that mimics graft-versus-host-disease, the injury pattern in the upper gastrointestinal (GI) tract is less extensively documented. We studied the pattern of upper GI tract injury in symptomatic patients taking MPA. Design Twenty solid organ transplant patients who were taking MPA and had concurrent upper GI tract biopsies were identified on a laboratory information system search. From these 20 patients, 17 duodenal, 18 gastric, and 7 esophageal biopsies were examined. All patients were symptomatic. Apoptosis and patterns of chronic and active injuries were assessed on standard hematoxylin and eosin, periodic acid-Schiff/Alcian blue, and Diff-Quik stained slides. To measure the significance of apoptosis, we standardized the apoptotic counts in normal biopsies using duodenal, gastric, and esophageal biopsies from 45 normal cases and performed statistical analysis. For the purposes of this study, we regarded apoptotic counts higher than the mean plus 2 SDs as significant. Thus the cut-off values for apoptosis were ≥2 apoptotic bodies/100 crypts for duodenum, ≥3/100 glands for stomach, and ≥2/10 high-power field for esophagus. Results GI-related symptoms and abnormalities manifested between 1 month and 10 years posttransplant and included diarrhea (55%); nausea (45%); abdominal pain (35%); vomiting (25%); GI bleeding (15%); dysphagia (10%); dyspepsia, anemia, and hematemesis (5% for each). Most (14 out of 17, 82%) duodenal biopsies showed apoptotic counts of ≥2/100 crypts, 28% (5 out of 18) of gastric biopsies showed apoptotic counts of ≥3/100 glands, and 57% (4 out of 7) of esophageal biopsies showed apoptotic counts of ≥2/10 high-power field. Four gastric biopsies showed a previously undescribed injury pattern of parietal cells resembling the ballooning degeneration. Additional pathologic findings included: chronic peptic duodenitis (6 out of 17, 35%), active duodenitis (1 out of 17, 6%), and celiac-like features (2 out of 17, 12%) in the duodenum; chemical gastropathy (3 out of 18, 17%), active chronic gastritis without Helicobacter pylori (2 out of 18, 11%), and erosion (1/18,6%) in the stomach; reactive epithelial change (3 out of 7, 43%), active esophagitis (3 out of 7, 43%), ulceration (2 out of 7, 29%), and erosion (1 out of 7, 14%) in the esophagus. Serum MPA levels were available in 7 patients, 6 of whom had abnormal duodenal apoptotic counts. On follow-up, available for 16 patients, symptoms improved in all the patients whose dose was decreased or whose medication was withdrawn (10 out of 10) and symptoms persisted in all the patients whose dose was not altered (6 out of 6). Follow-up biopsies after reduction of the medication dose were available for 1 patient and showed substantial reduction in apoptosis. In contrast, follow-up biopsies from 1 patient whose dosage was not altered showed persistent abnormal apoptotic counts in the duodenum. Conclusions As noted by others (Parfitt JR, Jayakumar S, Driman DK. Am J Surg Pathol. 2008; 32:1367–1372), mycophenolate mofetil-associated injury of the upper GI tract, like that in the colon, is characterized by prominent apoptosis similar to that of mild or grade I graft-versus-host-disease injury. We offer apoptotic count guidelines, which we hope will facilitate recognition of mycophenolate mofetil-associated injury in the upper GI tract.

Abstract:Purpose: To evaluate the effectiveness of therapeutic physical exercise (TPE) interventions on the physical functioning, psychosocial well-being, and quality of life (QoL) of children undergoing treatment for cancer. Method: Systematic review: databases were searched in April 2023. Selection criteria: children (<18 years old) undergoing treatment for cancer or a malignant neoplasm, randomized controlled trial design, utilization of TPE, and including physical and psychosocial outcomes. Internal validity was measured with Physiotherapy Evidence Database scale. Results: Seven randomized controlled trials were included. Most studies showed that strength, fatigue, and QoL improved after the intervention. Cardiorespiratory capacity through 6-minute walk test and physical activity levels were better in the experimental groups. No changes were noted in other variables. Conclusions: This review supports the importance of a TPE program during cancer treatment, with the aim of maintaining physical capacities and counteracting physical inactivity.

Abstract:Background: Cancer and its treatment can cause serious health issues that impair physical and mental well-being in children and adolescents. Exercise may be a valid strategy for managing some symptoms, including fatigue. In the light of our experience, we provide further justification for including exercise as part of routine childhood cancer care. Methods: Forty-four children and adolescents who had solid cancers not contraindicating their movement were invited to join an in-hospital 6-week supervised exercise program, and asked afterwards to complete validated quality of life and fatigue scales. The program consisted of personalized workout sessions of aerobic, resistance and flexibility exercises. The results obtained on the scales were compared between 21 patients who engaged in the exercise program (GYM group) and 23 who refused (No-GYM group), examining the different dimensions of health-related quality of life (physical, emotional, cognitive, social) and fatigue (general, sleep/rest, cognitive) in the two groups. Results: Being diagnosed with cancer initially prompted all but one of the respondents to drop-out of previous routine exercise or sports although their continuation had not been contraindicated. After 6 weeks of exercise, the GYM group's scores for quality of life and fatigue showed a statistically significant better perceived emotional functioning, and a trend toward a better social functioning than in the No-GYM group. Conclusion: We suggest that exercise improves the satisfaction of children and adolescents with cancer with their physical, mental and social functioning. We would emphasize the potential benefits of general practitioners discussing and recommending exercise for their young patients with cancer.

Abstract:Silver nanoparticle-embedded poly(vinyl alcohol) films are fabricated through a simple in situ process. The nanocomposite films are a few hundred nanometers thick with silver concentrations below 10% and the nanoparticles 5-10 nm in diameter. These films are shown to exhibit appreciable microwave absorption in the 8-12 GHz range; the return and insertion losses are found to be sensitive to the nanoparticle content.

Abstract:AbstractPurposeTo investigate the effects of exercise interventions on the physical fitness, fatigue, and quality of life (QOL) in adults with hematological malignancy (HM) who did not receive hematopoietic stem cell transplantation (HSCT).MethodsWe performed a systematic review with meta-analysis and qualitative evidence synthesis. We systematically searched 10 electronic databases including Medline, Allied and Complementary Medicine Database, Cochrane Central Register of Controlled Trials, Embase, CINAHL complete, SPORTDiscus, Web of Science, China Wan Fang Database, China National Knowledge Internet, and China Science and Technology Journal Database from their inception up to March 2021. The Cochrane guideline was followed, and a PRISMA checklist for reporting systematic reviews was completed.ResultsFifteen studies were included in this systematic review, of which 11 were included in the meta-analysis. There was a significant increase in the physical fitness between the exercise group and the control group, in terms of the 6-min walk test and peak oxygen consumption. There was a significant change in the peak oxygen consumption from the baseline to the endpoint in the exercise group. In the between-group analyses, there were no significant differences in the effect of exercise on the grip strength, fatigue, and QOL; however, in the within-group analysis, QOL was significantly increased in the combination exercise group.ConclusionBoth aerobic exercise and combination exercise appear safe and feasible and can improve physical fitness in adult patients with HM who have not received HSCT. The combination exercise may have potential benefits for their QOL.

Potential Benefits from Physical Exercise in Advanced Cancer Patients Undergoing Systemic Therapy? A Narrative Review of the Randomized Clinical Trials

Abstract:Design. The purpose of this review is the analysis of the literature concerning the effects of physical exercise in cancer patients undergoing medical oncologic treatment. Papers were retrieved from the scrutiny of 15 reviews/meta-analyses published in the last 2 years, which, however, pooled different populations of patients (surgical and medical patients, receiving or not an oncologic therapy, harboring a cancer, or being survivors). Results. We reviewed the data of 35 RCTs on the use of physical exercise in cancer patients, distinguishing well-nourished from malnourished patients. The conclusions of our study are the following: No major difference between well-nourished and malnourished patients as regards compliance/adherence with physical exercise and outcomes. Compliance with physical exercise was reported in about 70% of the studies. Compared with a control group receiving the usual care, in patients who practiced physical exercise, a benefit in some parameters of physical function and quality of life and lean body mass (LBM) was reported in 61%, 47%, and 12%, respectively, of the studies in non-malnourished patients, and in 50%, 100%, and 36%, respectively, of the studies in malnourished patients. The benefit in LBM was more frequently reported in weight-losing patients. There was no strict association among the results of different outcomes (muscle function vs. quality of life vs. LBM). There are still some ill-defined issues, including the optimal physical regimen (with some authors favoring high-intensity interval training and resistance) and the place of exercising (patients usually preferring home exercises, which, however, have been proved less efficacious).

Abstract:Objective: The purpose of this study is to conduct a systematic review to assess the effects of different forms of resistance exercises (resistance exercise, resistance exercise combined with aerobic exercise, and resistance exercise combined with other exercises) on physical fitness, quality of life (QOL), and fatigue of patients with cancer. Methods: We conducted a systematic review using the Cochrane Handbook for Systematic Reviews of Interventions guidelines. We searched PubMed, Web of Science, and Scopus databases for the studies from the establishment of the database to September 2023, including randomized controlled trials and clinical trials that evaluated the effects of different resistance exercise on physical fitness, QOL, and fatigue in all patients with cancer. Two reviewers independently assessed the quality of all the included studies using the Cochrane Handbook for Systematic Reviews of Interventions and MINORS scale. We divided the intervention into three types: resistance exercise, resistance exercise combined with aerobic exercise, and resistance exercise combined with other exercises. Results: In total, 48 studies (3,843 participants) met the inclusion criteria. The three exercise intervention forms have significant effects on physical fitness and QOL, but the improvement effect on fatigue is not clear. A total of 34 studies reported significant and beneficial effects of resistance exercise on physical fitness across all types of cancer. There were 28 studies that reported significant or borderline improvement effects of resistance on QOL, and only 10 studies reported significant effects of resistance exercise interventions on fatigue improvement in patients with cancer. Conclusions: Resistance exercise, resistance exercise combined with aerobic exercise, and resistance exercise combined with other exercises all have a positive effect on improving fitness and QOL in patients with cancer. Resistance exercise has an advantage in improving muscle strength, while combined resistance exercise has an advantage in improving QOL; however, there are no consistent findings in improving fatigue, although low-intensity resistance exercise is effective. Basic Need Status and Health-Promoting Self-Care Behavior in Adults

Abstract:Health-promoting self-care behavior emphasizing positive lifestyle practices may improve the health and quality of life of adults. One variable that may influence health-related decisions is the status of basic needs as described by Maslow. The purpose of this study was to investigate the relationships among basic need satisfaction, health-promoting self-care behavior, and selected demographic variables in a sample of community-dwelling adults. A convenience sample of 84 community-dwelling adults was recruited to complete the Basic Need Satisfaction Inventory, the Health-Promoting Lifestyle Profile II, and demographic information. Results of the study indicated that self-actualization, physical, and love/belonging need satisfaction accounted for 64% of the variance in health-promoting self-care behavior. The findings of this study are consistent with Maslow’s theory of human motivation and suggest that persons who are more fulfilled and content with themselves and their lives, have physical need satisfaction, and have positive connections with others may be able to make better decisions regarding positive health-promoting self-care behaviors.
The effectiveness of non-pharmacological interventions on fatigue in children and adolescents with cancer: a systematic review

Abstract:Background: Fatigue is one of the most distressing and prevalent symptoms reported by paediatric oncology patients. With the increase in cancer survival rate, how to control children's cancer-related fatigue during treatments or the coexisting period with the disease to further increase their quality of life has become the focus of medical teams who provide care to children with cancer. Objectives: The objective of this systematic review was to synthesise the best available evidence concerning the effectiveness of non-pharmacological interventions on fatigue in children and adolescents with cancer. Search strategy: The search strategy aimed to find studies published between 1960 and 2010, in either the English or Chinese languages. Reference lists of studies that met the inclusion criteria were also searched for additional studies. Types of studies: This review included randomised controlled trials and quasi-experimental studies that examined the effectiveness of non-pharmacological interventions for fatigue in children and adolescents with cancer. Types of participants: Children and adolescents aged between one to 18 years old, with cancer either during or after the chemotherapy. The types of cancer included were acute lymphoblastic leukaemia, acute myeloid leukaemia, lymphoma and solid tumours. Types of interventions: Non-pharmacological interventions for fatigue in children and adolescents with cancer including: activity enhancement (exercise, physical activity), psychosocial interventions, cognitive behavioural therapy, stress management, relaxation, nutrition consultation, massage and educational interventions. The intervention descriptions included the length, frequency, setting and intervention providers. Types of outcome measures: The outcome measures considered fatigue scores assessment. Types of setting: The review considered studies conducted either in a hospital or a community setting. Data collection and synthesis: The data were extracted and synthesised using the standardised data extraction tool and synthesis function offered by the Joanna Briggs Institute Meta-Analysis of Statistics Assessment and Review Instrument. Results: The review included six studies - five English-language papers and one Chinese-language paper. Meta-analysis did not show a statistically significant impact on the effectiveness of exercise interventions in reducing overall fatigue in children and adolescents with cancer. Exercise interventions did lead to a statistically significant improvement (p = 0.01) in general fatigue (i.e. tiredness, physical weakness) (effect size = -0.76; 95% Confidence Interval -1.35 to -0.17). Conclusions: The results of this systematic review show that exercise interventions can effectively reduce the level of general fatigue of children aged six to 18 years. No strong conclusions can be made for the use of massage therapy or health education measures, as there was only one article for each of these interventions. Implications for practice: The review provides an evidenced-based guide to future priorities for clinical practice. Exercise intervention could reduce the level of general fatigue of children aged six to 18 years. In particular, exercise interventions for fatigue are feasible and safe. Implications for research: There is still a lack of rigorous research on this specific topic. Further research requires more rigorous study design and reporting of methodological issues, such as randomised controlled trials using concealment of allocation. Other non-pharmacological interventions for this problem should also be examined, including cognitive behavioural therapy, stress management, relaxation, support groups and massage therapy.
Therapeutic exercise interventions in pediatric survivors of brain cancer and other solid tumors: A scoping review

Abstract:Background: Improved survival rates for children with solid tumors presents an ongoing challenge of how to maximize quality of survivorship and effectively manage the short- and long-term complications of disease and treatment. To gain an understanding of the extent and nature of research pertaining to therapeutic exercise interventions and identify knowledge gaps, we conducted a scoping review of exercise training studies conducted in pediatric survivors of brain cancer and other solid tumors. Method: A systematic literature search was performed across four electronic databases. Papers were selected for full-text review if they included participants treated for brain cancer or other solid tumors, with at least 50% of participants aged ≤ 21 years, evaluated an exercise intervention ≥2-weeks in duration, and were published in an English, peer-reviewed journal. We included the following quantitative study designs; randomized controlled trials, non-randomized trials, and single-arm pre-test-post-test. Results: Of the 7,482 citations identified, 17 papers met the inclusion criteria (presenting findings from eleven studies). Two studies were randomized controlled trials, five studies were non-randomized controlled trials, and four studies were a single-arm pre-test post-test design. Average age of participants ranged from 7.3–15.5 years, and time since diagnosis ranged from 3 to 70 months. Five studies included participants with brain tumors exclusively, three studies included other solid tumors, and three studies included a mixed sample (brain and other solid tumors). A wide range of exercise modalities were employed, including cycle ergometry, resistance training, sport, yoga, and active gaming. The length of the exercise program ranged from 3–40 weeks and frequency from 3–11 sessions per week. Exercise session duration ranged from 15–180 min, with most studies reporting 30–90-min sessions. Adherence ranged from 77 to 100%, with none of the studies reporting adverse events. Studies reported improvements in cardiorespiratory fitness, functional strength, physical activity, and quality of life. Conclusions: A small number of mostly low methodological quality studies have examined the effects of therapeutic exercise in pediatric survivors of solid tumors. Although limited, the extant literature supports the feasibility and safety of therapeutic exercise interventions for pediatric survivors of brain cancer and other solid tumors.

Adventure-based training to promote physical activity and reduce fatigue among childhood cancer survivors: A randomized controlled trial

Abstract:Background: Cancer-related fatigue is one of the most distressing symptoms reported by childhood cancer survivors. Despite the body of evidence that regular physical activity helps alleviate cancer-related fatigue, insufficient participation in physical activity is frequently observed among childhood cancer survivors. Objectives: This study examined the effectiveness of an adventure-based training programme in promoting physical activity, reducing fatigue, and enhancing self-efficacy and quality of life among Hong Kong Chinese childhood cancer survivors. Design: A prospective randomised controlled trial. Settings: A paediatric oncology outpatient clinic, a non-governmental organisation, and a non-profit voluntary organisation. Participants: Hong Kong Chinese childhood cancer survivors aged 9-16 years who reported symptoms of fatigue and had not engaged in regular physical exercise in the past 6 months. Methods: The experimental group underwent a 4-day adventure-based training programme. The control group received a placebo intervention. The primary outcome was fatigue at 12 months. Secondary outcomes were physical activity levels, self-efficacy and quality of life at 12 months. Data collection was conducted at baseline, and 6 and 12 months after the intervention began. We performed intention-to-treat analyses. Results: From 6 January, 2014 to 8 June, 2015, we randomly assigned 222 eligible childhood cancer survivors to either an experimental (n = 117) or a control group (n = 105). The experimental group showed statistically significantly lower levels of cancer-related fatigue (P < 0.001), higher levels of self-efficacy (P < 0.001) and physical activity (P < 0.001), and better quality of life (P < 0.01) than the control group at 12 months. Conclusions: This study provides evidence that adventure-based training is effective in promoting physical activity, reducing cancer-related fatigue, and enhancing self-efficacy and quality of life among Hong Kong Chinese childhood cancer survivors. These results may help inform parents and healthcare professionals that regular physical activity is crucial for the physical and psychological wellbeing and quality of life of childhood cancer survivors.
Effects of exercise by type and duration on quality of life in patients with digestive system cancers: A systematic review and network meta-analysis

Abstract:Background There is scant evidence regarding the effects of exercise type and duration on quality of life (QoL) in digestive system cancer (DSC) survivors. We aim to investigate the optimal type and duration of exercises to improve QoL for DSC survivors through a systematic review and network meta-analysis. Methods A systematic literature search of PubMed, EMBASE, and Web of Science was performed. Eligibility for study inclusion was limited to studies that were randomized controlled trials involving all kinds of exercise in adult patients with DSCs, and the comparator was in standard care or other types of exercise. The primary outcome was QoL, including general health, physical health, mental health, and role function. Secondary outcomes included cancer-related symptoms such as fatigue, insomnia, depression, anxiety, and duration of hospital stay. The network meta-analyses were performed using a random-effect model. Results The analysis included 32 eligible articles and a total of 2558 participants. Our primary outcome indicated that short-term aerobic exercise significantly enhanced general health (standardized mean differences (SMDs)‚ÄØ=‚ÄØ0.66, 95% credible intervals (CrIs): 0.05 to 1.3, and also contributed to a better mental health (SMD‚ÄØ=‚ÄØ0.37, 95%CrI: -0.06 to 0.81 and role function (SMD‚ÄØ=‚ÄØ0.48, 95%CrI: ‚Äì0.27 to 1.20). Although without significant changes, short-term resistance exercise tends to increase the physical health of patients with DSCs (SMD‚ÄØ=‚ÄØ0.69, 95%CrI: ‚Äì0.07 to 1.50) and effective in alleviating fatigue (SMD‚ÄØ=‚ÄØ‚Äì0.77, 95%CrI: ‚Äì1.50 to 0.01). Short-term aerobic exercise was related to a lower score of insomnia (SMD‚ÄØ=‚ÄØ‚Äì1.20, 95%CrI: ‚Äì2.40 to 0.06), depression (SMD‚ÄØ=‚ÄØ‚Äì0.51, 95%CrI: ‚Äì1.50 to 0.45) and anxiety (SMD‚ÄØ=‚ÄØ‚Äì0.45, 95%CrI: ‚Äì1.30 to 0.34). All types of exercise related to a trend of declined hospital stays (‚Äì0.87 to ‚Äì5 days). Long-term resistance exercise, however, was negatively associated with general health (SMD‚ÄØ=‚ÄØ‚Äì0.33, 95%CrI: ‚Äì1.70 to 1.00), physical health (SMD‚ÄØ=‚ÄØ‚Äì0.19, 95%CrI:‚Äì1.30 to 0.90), and role function (SMD‚ÄØ=‚ÄØ‚Äì1.20, 95%CrI: ‚Äì2.50 to 0.11). Conclusions This study suggests that short-term aerobic exercise, with or without resistance exercise programs, enhances QoL (especially for general health) as well as relieves cancer-related symptoms for DSC survivors, while long-term resistance exercise may have negative effects, and thus should be adopted cautiously. These results provide important evidence for the management of DSCs.

Abstract:The aim was to evaluate the effects of additional exercises during inpatient stays on bone mass in pediatric bone tumor patients. 21 patients were non-randomly allocated either to the exercise group (n = 10) or the control group (n = 11). DXA of the lumbar spine, the non-affected femur and both calcanei was performed after completion of neoadjuvant chemotherapy (baseline), as well as 6 and 12 months after baseline. Bone mineral content (BMC), bone mineral density (BMD) and height-corrected lumbar spine Z-scores were determined. Group changes after 6 and 12 months were compared by covariance analyses. Additionally, daily physical activities (PA) were assessed by means of accelerometry. After adjusting for initial age, height and weight, mean reductions in lumbar spine and femoral BMC were lower in the exercise group (not significant). Effect sizes during the observational period for lumbar spine and femur BMC were generally small (partial η² = 0.03). The exercise group demonstrated substantially higher PA levels in terms of gait cycles per day, per hour and moderate PA (activities above 40 gait cycles per minute). Additional exercises for bone tumor patients are feasible during hospitalization. Though the intervention did not influence BMC, it appeared beneficial regarding PA promotion with respect to volume and intensity.

Abstract:Background: Cancer-related fatigue (CRF) is one of the most common and distressing symptoms in paediatric oncology. Based on previous studies, physical activity interventions are considered to be effective in reducing CRF in adult cancer patients. Aim: The aim of this systematic review is to investigate whether physical activity interventions can reduce CRF in paediatric patients undergoing cancer treatment. Methodology: A systematic literature search was conducted in PubMed and Sport-Discus in October 2021 to identify intervention studies examining the effects of physical activity on CRF in cancer patients ≤ 21 years of age. Their methodological quality was assessed using the JBI Critical Appraisal Tool. Results: A total of 20 studies (seven randomized-controlled, six quasi-experimental and seven single-arm intervention trials) were included in the review. Nine studies reported significant positive effects of physical activity interventions on CRF in group comparison or within groups. Eleven trials reported no significant changes in CRF. Conclusion: Physical activity as a therapeutic intervention in paediatric oncology may have the potential to reduce CRF in childhood cancer patients undergoing cancer treatment. Further high-quality studies with large samples are needed to verify these results and to assess the interdependence of dose and response of physical activity interventions.

Abstract:Integrative Cancer Therapies, Volume 23, Issue , January-December 2024. Introduction: Physical activity, as a promising complementary therapy, has shown considerable potential for reducing chemotherapy-related cardiotoxicity (CTRCT) and enhancing cardiorespiratory function (CRF). This study aimed to systematically assess the effects of physical activity on CTRCT and CRF in various cancer survivors receiving chemotherapy. Methods: A systematic review and meta-analysis was conducted. A literature search was conducted across 8 databases from inception to January 2024 and was limited to the English and Chinese languages. Statistical analysis was conducted using RevMan 5.3 and Stata 17.0 software. Results: Sixteen randomized controlled trials (RCTs) were included in the systematic review and 15 RCTs were included in the meta-analysis. Among various cancer survivors undergoing chemotherapy, physical activity markedly increased absolute oxygen uptake (VO2peak or VO2max; WMD = 292.99, 95% confidence interval [CI]:87.87 to 498.12, P = .005), with significant effects of subgroup analysis at 4 to 10 weeks (P = .02) or over 16 weeks (P < .01), moderate-to-high or high intensity training (both P < .0001), patients with breast cancer (P = .009) and reported CTRCT (P = .007); relative VO2peak or VO2max(WMD = 3.30, 95%CI: 2.02 to 4.58, P < .00001), with significant effects of subgroup analysis at 10 to 16 weeks or over 16 weeks, moderate-to-high or high intensity training, patients with breast cancer, with or without reported CTRCT and exercise during chemotherapy (all P < .01); E/A values (WMD = 0.11, 95%CI:0.03 to 0.18, P = .007) and flow-mediated dilatation (WMD = 2.71, 95%CI:1.49 to 3.94, P .05), and global longitudinal strain (WMD = 0.37, 95%CI: −0.20 to 0.94, P = .20) with non-significant effects being independent of exercise duration and cancer types(both P > .05). Conclusions: Physical activity may be an effective complementary therapy to improve CRF and CTRCT in various cancer survivors, particularly during medium to long duration and moderate-to-high and high intensity exercise with concurrent chemotherapy.
