Abstract:Pedestrian safety remains a significant concern, with the growing number of severe pedestrian crashes resulting in substantial human and economic costs. Previous research into pedestrian crashes has extensively analyzed the influences of weather, lighting, and pedestrian demographics. However, these studies often overlook the critical spatial variables that contribute to pedestrian crashes. Our study aims to explore these overlooked spatial variables by examining the distance pedestrians travel before encountering a severe crash. This approach provides a supplementary perspective in safety analysis, emphasizing the importance of pedestrian movement patterns. The model considers various factors that may influence pedestrian traveled distance before being involved in a severe crash, such as weather conditions, lighting conditions, and pedestrian demographics. Ohio's pedestrian-involved crashes were gathered and analyzed as a case study. The results indicated that 50 % of fatal pedestrian crashes occurred within 0.84 miles of the pedestrians' residences. Moreover, it was shown that factors including lighting condition, pedestrian age, drug toxication, and the location at impact significantly influence the pedestrians traveled distance. These findings provide valuable insights into the spatial distribution of pedestrian crashes and shed light on the factors contributing to their severity. By understanding these relationships, policymakers and urban planners can design targeted interventions such as improving street lighting, implementing traffic calming measures, and developing safety awareness campaigns for specific age groups, to enhance pedestrian safety and reduce the incidence of severe crashes.

Abstract:Obstruction of the upper airway in infants may be associated with continued respiratory efforts without air entering the lungs.1-4 Our clinical experience has led us to suspect that some apnea monitors in current use are unable to differentiate between these ineffective respiratory efforts and true breathing. To document this deficiency of apnea monitors, we examined the response of two types of apnea monitors to respiratory movements during upper airway obstruction. SUBJECTS AND METHODS Five healthy full-term infants were studied during the first three days of life, and two premature infants in stable condition (birth weight, 1.32 kg and 1.30 kg; gestational age, 32 weeks) were studied at 25 days of age when they no longer had apnea associated with bradycardia in the nursery.

Abstract:Pedestrians, particularly susceptible to road traffic crashes, experience varying injury severities influenced by age and time shifts. This research aims to investigate the differences and temporal shifts in factors influencing pedestrian injury severities across different age groups. To achieve this, three random parameters binary logit models with heterogeneity in the means (and variances) were employed. Four years of pedestrian crash data in Hong Kong were utilized in this study. According to United Nations' definitions of the young and elderly, pedestrians were categorized into three groups: young (under 25 years old), middle-aged (25–65 years old), and elderly (over 65 years old). Initial likelihood ratio tests indicated temporal stability in the young group between 2019 and 2021, with further tests confirming age transferability and overall temporal stability after integrating the three years of young data. The partially constrained temporal stability approach was then developed to further capture the temporal stability of individual variables and simplify model results. Model results identified factors impacting pedestrian injury severities, encompassing pedestrian, driver, vehicle, temporal, and light condition characteristics. Some contributing variables exhibit age-transferability or temporal stability, such as controlled crossing, near controlled crossing, inattentive driver and private car. However, the significance of most contributors varies across age groups and years, with certain factors being age-specific or year-specific. Out-of-sample predictions underscore the cumulative likelihood of fatal or severe injuries with advancing age, and the middle-aged models showed the highest level of temporal stability regarding the risk of injury severity compared to the other two age models. Moreover, middle-aged pedestrians in Hong Kong faced the highest risk of fatal or severe injuries during the first year of the COVID-19 lockdown (2020), but the risk significantly declined for pedestrians of all age groups in the subsequent year. Based on these findings, targeted preventive measures that take into account age differences have been proposed to effectively enhance pedestrian safety.

Abstract:As the Na+/Ca2+ exchanger plays an important role in the regulation of myocyte contractility, it has been suggested that alterations in this system might be involved in the development of insulin resistance and/or diabetes-induced myocardial alterations. Moreover, gene expression and function of the Na+/Ca2+ exchanger in states of combined hypertension and insulin resistance is of a special interest. Thus, we used hereditary hypertriglyceridemic (hHTg) rat (a model of genetically induced insulin resistance and hypertension) to study the effect of losartan, the blocker of type 1 angiotensin receptors, on the Na+/Ca2+ exchanger in the rat heart. We found that gene expression, but not activity of the Na+/Ca2+ exchanger was decreased in the left ventricle of hHTg rats when compared to their normotensive mates. No changes were observed in the right ventricle. In addition, losartan decreased mRNA levels of the Na+/Ca2+ exchanger in the left, but not in the right ventricle of normotensive rats. In hHTg rats, losartan had no effect on the gene expression of this transporter. Our results point to different modulatory pathways of Na+/Ca2+ exchanger in normotensive and hHTg rats.

Abstract:Older pedestrians are vulnerable on the streets and at significant risk of injury or death when involved in crashes. Pedestrians’ safety is critical for roadway agencies to consider and improve, especially older pedestrians aged greater than 65 years old. To better protect the older pedestrian group, the factors that contribute to the older crashes need to be analyzed deeply. Traditional modeling approaches such as Logistic models for data analysis may lead to modeling distortions due to the independence assumptions. In this study, Extreme Gradient Boosting (XGBoost), is used to model the classification problem of three different levels of severity of older pedestrian traffic crashes from crash data in Colorado, US. Further, Shapley Additive explanations (SHAP) are implemented to interpret the XGBoost model result and analyze each feature’s importance related to the levels of older pedestrian crashes. The interpretation results show that the driver characteristic, older pedestrian characteristics, and vehicle movement are the most important factors influencing the probability of the three different severity levels. Those results investigate each severity level’s correlation factors, which can inform the department of traffic management and the department of road infrastructure to protect older pedestrians by controlling or managing some of those significant features.

Abstract:A sensitive (10.0 ng/ml) and stereoselective HPLC method for analysis of S(-)- and R(+)-diprafenone in human plasma has been developed. The assay is based on derivatization with homochiral R(-)-1-(1-naphthyl)ethyl isocyanate to give the diastereomeric derivates and their separation and quantitation using HPLC with UV-detection (220 nm). Details of this stereoselective assay procedure which has already been applied to the multisample analyses from a human pharmacokinetic study are described. The data obtained indicated that the stereoselective disposition of diprafenone enantiomers is quite different from their structural and pharmacological analog propafenone.

Abstract:Pedestrian violations are a major cause of pedestrian accidents. Considering that pedestrians are vulnerable to mortality and severe injury in traffic accidents, it is necessary to conduct a study of the factors influencing the occurrence of unmarked street crossing violations and to improve pedestrian safety in a targeted manner. This paper investigated pedestrian spatial violations on unmarked roadways at six high-traffic locations in Hefei and examined the effects of the road environment, pedestrian crossing behaviour, and pedestrian characteristics on pedestrian spatial violation decisions. Thirty variables with a total of 723 samples were obtained by an unmanned aerial vehicle (UAV) and a camera. The purpose of this study was to gain an understanding of pedestrian spatial crossing violations at unmarked roadways. A random parameter logit model was used to establish a pedestrian crossing decision model for roadway violations. The results indicate that males are more likely to violate than females; the probability of violation with companions is higher than that of crossing alone; when pedestrians cross with other strangers in the same direction, the rate will decrease; and shorter intersection spacing, a higher vehicle flow, and a longer pedestrian crossing length all reduce the illegal crossing propensity. The findings should be indicative to help traffic road construction departments and traffic participants take effective measures to improve road safety.

Abstract:Road traffic safety is an essential component of public safety and a globally significant issue. Pedestrians, as crucial participants in traffic activities, have always been a primary focus with regard to traffic safety. In the context of the rapid advancement of intelligent transportation systems (ITS), it is crucial to explore effective strategies for preventing pedestrian fatalities in pedestrian–vehicle crashes. This paper aims to investigate the factors that influence pedestrian injury severity based on pedestrian-involved crash data collected from several sensor-based sources. To achieve this, a hybrid approach of a random parameters logit model and random forest based on the SHAP method is proposed. Specifically, the random parameters logit model is utilized to uncover significant factors and the random variability of parameters, while the random forest based on SHAP is employed to identify important influencing factors and feature contributions. The results indicate that the hybrid approach can not only verify itself but also complement more conclusions. Eight significant influencing factors were identified, with seven of the factors identified as important by the random forest analysis. However, it was found that the factors "Workday or not" (Not), "Signal control mode" (No signal and Other security facilities), and "Road safety attribute" (Normal Road) are not considered significant. It is important to note that focusing solely on either significant or important factors may lead to overlooking certain conclusions. The proposed strategies for ITS have the potential to significantly improve pedestrian safety levels.

Abstract:Traffic accidents involving pedestrians and drivers pose significant public health and safety concerns. Understanding the differential influences of road physical design attributes on crash frequencies for these two groups is critical for developing targeted safety interventions. Considering that the zero-truncated characteristic of the data is uncertain, the results of the zero-truncated negative binomial models and traditional negative binomial models are calculated to seek the better model. The result revealed that the road surface conditions and vertical and horizontal curvature have greater influence on both pedestrian and driver compared to number of lanes and speed limit. And speed limits were more pronounced for pedestrian crash frequency than driver group. Conversely, the effect of different types of intersections was stronger for driver crash frequency. The differential influences of road physical design attributes on traffic crash frequencies for pedestrians versus drivers highlight the importance of adopting a user-centric approach to transportation safety planning and infrastructure design. Tailoring interventions to address the unique needs and vulnerabilities of different road user groups can lead to more effective safety improvements and better overall traffic safety outcomes.

Abstract:This study intended to investigate the influencing factors of pedestrian injury severity at signalized intersections, considering the heterogeneity issue of unobserved factors at different signalized intersections and the spatial attributes with survival models. To achieve the objectives, a Bayesian multilevel parametric survival model was developed, in which the survival model addressed the pedestrian severity levels varying with time, while the panel data model accommodated the heterogeneity attributed to unobserved factors and spatial features within the Bayesian framework. The pedestrian-related crash data of Hong Kong metropolitan area from 2008 to 2012 were integrated, involving 376 signalized intersections with 2,090 pedestrian severity samples. By comparing the proportional hazard (PH) and accelerated failure time (AFT) models, the PH model with exponential distribution showed priority to the other models. Results revealed that pedestrian age, injury location, pedestrian special circumstance, pedestrian contributory, number of pedestrian stream, obstruction, road type, presence of tram/LRT stops and bus stops were potentially significant factors of increasing the pedestrian injury severity probability. The findings provide useful insights for practitioners and policy makers to improve pedestrian safety at signalized intersections.

Abstract:In this study, our goal was to determine the impact of various risk factors on traffic accidents in the city of Shenyang, China, and to discuss the various common factors that influence pedestrian and non-motor vehicle accidents. A total of 1227 traffic accidents from 2015 to 2017 were analyzed, of which, 733 were accidents involving pedestrians and 494 were non-motor vehicle accidents. Among these traffic accidents, pedestrians and non-motor vehicle users had either minor or no responsibility. Sixteen influencing factors, including main responsible party attributes, pedestrian/non-motor vehicle user attributes, time attributes, space attributes, and environmental attributes were analyzed with regards to their impact on accidents using the binary logistic regression model (BLR) and the classification and regression tree analysis model (CART). Age, administrative division, and time of year were the three most common factors for pedestrian and non-motor vehicle accidents. For pedestrian accidents, the personal influencing factors of the main responsible party included illegal acts while driving and hit-and-run behavior. Factors affecting pedestrian and non-motor vehicle accidents also had different orders of importance.

Abstract:This study explores risk factors influencing the at-fault party in traffic accidents and analyzes their impact on traffic accident severity. Based on the traffic accident data of Shenyang City, Liaoning Province, China, from 2018 to 2020, 19 attribute variables including road attributes, time attributes, environmental attributes, and characteristics of the at-fault parties with either full responsibility, primary responsibility, or equal responsibility of the traffic accidents were extracted and analyzed in conjunction with the built environment attributes, such as road network density and POI (points of interest) density at the sites of traffic accidents. Using the RF-SHAP method to determine the relative importance of risk factors influencing the severity of traffic accidents with either motor vehicles or vulnerable groups at-fault, the top ten risk factors influencing the severity of traffic accidents with vulnerable road users as the at-fault parties are: functional zone, density of shopping POI, density of services POI, cause of accident, travel mode, collision type, season, road type, age of driver, and physical isolation. Travel mode, season, and road speed limit are more important risk factors for traffic accidents, with motor vehicle drivers as the at-fault parties. The density of service POI and cause of the accident are less critical for traffic accidents with motor vehicle drivers than traffic accidents with vulnerable road users who are at-fault. Subsequently, the Apriori algorithm based on association rules is used to analyze the important causal factors of traffic accidents, so as to explore the influence mechanism of multiple causal factors and their implied strong association rules. Our results show that most combined factors are associated with the matched Service and Shopping POI features. This study provides valuable information on the perceived risk of fatal accidents and highlights the built environment's significant influence on fatal traffic accidents. Management strategies targeting the most typical combinations of accident risk factors are proposed for preventing fatalities and injuries in serious traffic accidents.

Abstract:With the purpose to improve the road traffic safety,it is important to investigate the factors which have impacts on the severity of traffic crashes.Based on the data of traffic crashes in recently years which include serious colli-sions,the types of crashes are distinguished using Poisson Model and Negative-binomial Model.These models are also used for establishing an estimation model of fatality rate for three typical crash types:rear-end crashes,lateral impact crashes and pedestrian-involved crashes.In this model,the fatality is a dependent variable and highway factors are inde-pendent variables,the numbers of persons involved in each crash is a modified variable.The parameters of this model are determined by the over-dispersion parameter and Akaike Information Criterion (AIC)method.The results indicate that the grade of highway and roadside protective facilities significantly correlate with the fatality of rear-end crashes.Weather, conditions of road surface,locations of segments or intersections,gradient of roads,and structure of pavement are all rel-evant with lateral impact crashes.The factors of conditions of road surface,grade of roads,numbers of lanes,and radius of curve are relevant with pedestrian-involved crashes.This innovated method on severity of traffic crashes proposed in this study can be further used for analyses on extremely serious traffic crashes.This study could be used as a reference for accident analysis and design of roads as well.

Abstract:Road safety has become a worldwide public health concern. Although many factors contribute to collisions, pedestrian behaviors can strongly influence road safety outcomes. This paper presents results of a survey investigating the effects of age, gender, attitudes towards road safety, fatalistic beliefs and risk perceptions on self-reported pedestrian behaviors in a Chinese example. The study was carried out on 543 participants (229 men and 314 women) from 20 provinces across China. Pedestrian behaviors were assessed by four factors: errors, violations, aggressions, and lapses. Younger people reported performing riskier pedestrian behaviors compared to older people. Gender was not an influential factor. Of the factors explored, attitudes towards road safety explained the most amount of variance in self-reported behaviors. Significant additional variance in risky pedestrian behaviors was explained by the addition of fatalistic beliefs. The differences among the effects, and the implications for road safety intervention design, are discussed. In particular, traffic managers can provide road safety education and related training activities to influence pedestrian behaviors positively.

The Effects of Multiple Factors on Elderly Pedestrians’ Speed Perception and Stopping Distance Estimation of Approaching Vehicles

Abstract:To make safe road-crossing decisions, it is necessary for pedestrians to accurately estimate the speed and stopping distance of approaching vehicles. Accordingly, the objective of our study was to examine the effects of multiple factors, such as weather conditions, context time (day or night), and illuminance of the roads, on older pedestrians’ (>60 years old) speed perception and stopping distance estimation of approaching vehicles. The participants in this study included 48 older participants who were asked to estimate the speed and stopping distance of approaching vehicles based on 12 s video clips that were selected from natural conditions. The results revealed that actual speeds, weather, context time, and lighting conditions played important roles in the performance of the participants. Compared with young adults, older pedestrians were found to have smaller accurate estimation intervals that varied by multidimensional influencing factors and thus resulted in missing road-crossing opportunities at lower vehicles’ speeds and increasing road-crossing dangers at higher speeds. The older pedestrians’ performance with respect to speed perception and stopping distance estimation is modeled using a regression model with a complex level of tasks. These models can be used by engineers when establishing speed limits and lighting conditions in the areas with senior residents.
Comparative analysis of middle ear immune response and micronutrient level between mucoid and purulent otitis media.

Abstract:OBJECTIVE To determine the differences in the outcome of treatment, the middle ear immune response, and micronutrient status between patients with purulent (POM) and mucoid (MOM) otitis media. DESIGN Children with acute otitis media were recruited, treated, and followed up for between 6 and 10 months. The sera and middle ear secretion (MES) were analyzed for immunoglobulins and zinc using enzyme-linked immunoassay and flame atomic absorption spectrophotometry, respectively. The parameters were compared with selected healthy controls. SUBJECTS There were 399 participants: 228 children with acute otitis media selected using the American Academy of Otolaryngology-Head and Neck Surgery criteria and 171 healthy controls, with ages ranging between 6 months and 9 years (mean 7 years; SD 2.32 years). RESULT POM accounted for 126 of 228 patients, and MOM accounted for 102. The chronicity of otitis media (using the 3-month duration cutoff) was seen in 87 subjects (46%): 31 of 126 POM subjects and 56 of 102 MOM subjects (p = .002). The serum IgA to IgG ratios were 0.03 and 0.07 among POM and MOM, respectively; however, the MES IgA to IgG ratios were higher, 0.3 and 0.2, for POM and MOM, respectively. This gives a MES to serum ratio of IgA to IgG value of 0.3 to 0.03 (10) for POM and 0.2 to 0.07 (2.67) for MOM, which showed a significant statistical difference (p = .001). The MES to serum ratios of IgE were 0.89 and 1.3 among POM and MOM, respectively (p = .03). There was a significant difference in the serum IgG (p = .019), serum zinc (p = .009), and MES IgA (p = .028) and IgG (p = .01). CONCLUSION Chronicity was greater in MOM than in POM; POM had a higher middle ear immune response (measured by the MES to serum ratio of IgA to IgG) and serum zinc than MOM but a lower MES IgE.
Towards Healthy Aging: Influence of the Built Environment on Elderly Pedestrian Safety at the Micro-Level

Abstract:As vulnerable road users, elderly pedestrians are more likely to be injured in road crashes due to declining physical and perceptual capabilities. Most previous studies on the influence of the built environment on elderly pedestrian safety focused on intersections or areal units. Using a district of Shanghai as the study area, this research investigated the effects of the built environment at the road segment level with elderly pedestrian collision, taxi tracking point, point of interest, street view image, open street map, land use, housing price, and elderly population datasets. In particular, this research employed both Poisson and geographically weighted Poisson regression (GWPR) models to account for spatial nonstationarity. The Poisson model indicates that green space, sidewalks, and junctions on the roads significantly affected elderly pedestrian safety, and roads around nursing homes, schools, bus stops, metro stations, traditional markets, and supermarkets were hazardous for elderly pedestrians. The results of the GWPR model suggest that the influence of factors varied across the study area. Green space could decrease the risk of elderly pedestrian collisions only in areas without congested environments. Separations need to be installed between roadways and sidewalks to improve elderly road safety.

Abstract:When traffic collisions occur on urban expressways, the consequences, including injuries, the loss of lives, and damage to properties, are more serious. However, the existing research on the severity of expressway traffic collisions has not been deeply explored. The purpose of this research was to investigate how various factors affect the severity of urban expressway collisions. The severity of urban expressway collisions was set as the dependent variable, which could be divided into three categories: slight collisions, severe collisions, and fatal collisions. Ten variables, including individual characteristics, collision characteristics, and road environment conditions, were selected as independent factors. Based on 975 valid urban expressway collisions, an ordered logistic regression model was established to evaluate the impacts of influence factors on the severity of these crashes. The results show that gender, collision modality, road pavement conditions, road surface conditions, and visibility are significant factors that affect the severity of urban expressway collisions. Females were more likely to be involved in more severe urban expressway collisions than males. For collisions involving pedestrians and non-motorized vehicles, the risk of more severe injury was 7.508 times higher than that associated with vehicle–vehicle collisions. The probability of more severe collisions on urban expressways with poor pavement conditions and wet surface conditions is greater than that on urban expressways with good pavement conditions and dry surface conditions. In addition, as visibility increases, the probability of more severe collisions on urban expressways gradually decreases. These results provide more effective strategies to reduce casualties as a result of urban expressway collisions.
