Abstract:The advantages offered by lasers compared to older technologies for endoscopic surgery for symptomatic benign prostate hyperplasia (BPH) are reviewed. Laser treatments for the endoscopic management of patients with bladder outlet obstruction (BOO) resulting from BPH can be divided into three basic techniques. These techniques are vaporisation (removal of tissue), resection of tissue (excision of small chips and subsequent irrigation from bladder) and enucleation (dissection of the adenoma from the surgical capsule and subsequent morcellation). The decision to offer a transurethral laser approach to patients with BPH depends on their comorbidities, the surgeon’s expertise with the different procedures, and the availability of the relevant technology.

Six Novel Minimally Invasive Therapies for Benign Prostatic Hyperplasia

Abstract:Benign prostatic hyperplasia (BPH) is the most common cause of lower urinary tract symptoms in men. When drug treatment is ineffective or conventional surgery is not suitable, novel minimally invasive therapies can be considered. These include prostatic urethral lift, prostatic artery embolisation, water vapor thermal therapy, Aquablation-image guided robotic waterjet ablation, temporary implantable nitinol device and prostatic stents. These novel therapies can be performed in outpatient setting under local anesthesia, with shorter operative and recovery times, and better protection of ejaculatory function and erectile function. General conditions of the patient and advantages and disadvantages of the each of these therapies should be fully considered to make individualized plans.

Abstract:Lasers are becoming more and more widely used in medicine, gaining significant clinical acceptance in the treatment of diseases of the genitourinary system. The aim of this study is to present contemporary methods of treatment of Benign Prostate Hyperplasia with the use of lasers, taking into account the physical characteristics of these devices and the safety of the therapy. According to the recommendations of the European Urological Association, in the treatment of BPH, laser systems based on laser devices can be used, which are characterized by various physical properties such as wavelength, intensity, power, frequency and duration of the emitted light. An important aspect raised in the study is the safety assessment of these methods in patients using anticoagulants, which are commonly used in the treatment of cardiovascular diseases.
A comparative study of two means of simultaneous treatment for benign prostate hyperplasia and bladder stone

Abstract:Objective To compare the safety and the clinical efficacy of two means of simultaneous treatment for benign prostate hyperplasia (BPH) and bladder stone. Methods Sixty-three cases of BPH and bladder stones were randomly divided into 2 groups: they were treated by combining mini-incisions cystolithotomy (group A) ,transurethral eletroresection with holmium laser (group B). TURP followed after the removal of stone. The removal of stone time, bladder irrigation time, catheterization time, hospital stay , hospital expenses and therapeutic effects were surveyed. Results Compared with group B, the removal of stone time, hospital cost were significantly superiority in group A(t=2.89,17.87, P<0.05).But compared with group A, the bladder irrigation time, catheterization time, hospital stay were significantly superiority in group B(t=2.21,8.04,2.19, P<0.05). The procedures were successful in all cases, a follow up of 3 to 35 months found a good outcome in both groups. All the patients had normal micturation after the urethral catheter was withdrawn and no severe complication. IPSS, Qmax and RUV were significantly improved in both groups. Conclusion The results showed that the two therapies of BPH with bladder stones were safe and effective.
Prostatectomy using different lasers for the treatment of benign prostate hyperplasia in aging males

Abstract:Purpose: Endoscopic lasers have become a treatment option for benign prostate hyperplasia (BPH). The study reported here sought to elucidate the benefits and drawbacks of different laser systems in the treatment of patients with BPH. Methods: The study enrolled 741 patients diagnosed with lower urinary tract symptoms secondary to BPH during the period January 2005 to December 2011. The techniques used in the study were photoselective vaporization of the prostate, thulium laser prostatectomy, and diode laser prostatectomy. Patients were assigned to one of three groups according to the type of laser treatment they received. Outcomes were evaluated using the International Prostate Symptom Score (IPSS), quality of life, maximal urinary flow rate, post-voiding residual urine volume, and prostate-specific antigen (PSA) level. Results: The baseline characteristics of patients who received diode laser prostatectomy show a significant elevated risk and high American Society of Anesthesiology score (P=0.001). Operative time and catheter removal time differed significantly between the three groups (P=0.001). No cases were converted to transurethral resection of the prostate intraoperatively due to bleeding (P=0.142). Among the three groups, there were no significant differences in maximal flow rate, lower post-void residual urine, and postoperative PSA level during the entire follow-up period (P<0.05). Further, no significant differences in postoperative IPSS, quality of life, or bladder neck contracture (P=0.23) were observed. However, a significant difference was observed with regard to prolonged use of Foley catheters and prolonged hospital stay among patients in the diode laser group (P=0.001). Conclusion: Laser prostatectomies are effective in dealing with lower urinary tract symptoms. Early subjective functional results (maximal flow rate, IPSS, and post-void residual urine) appeared the same as those obtained following laser prostatectomy. Thus, it appears that lasers are safe and effective as long as the patients are carefully selected for treatment.

Abstract:We assessed the therapeutic efficacy and safety of laser prostatectomy (LP) for treating benign prostatic hyperplasia (BPH) in patients on oral anticoagulation. We systematically reviewed previous reports, using the Pubmed database and bibliographies of retrieved articles and reviews. The oral anticoagulation included coumarin derivatives and platelet-aggregation inhibitors (PAI). Previous studies do not allow the establishment of definitive conclusions for managing patients on oral anticoagulation and who require BPH surgery. No randomized studies are available. Nevertheless, compared to transurethral resection of the prostate (TURP), LP seems to decrease the risk of haemorrhage in patients taking PAI or coumarin derivatives. Therefore, LP is a useful alternative to TURP for managing patients on oral anticoagulation, and could be proposed as the first intention for those patients. Continuing PAI during the procedure is feasible. A replacement of coumarin derivatives by low molecular weight heparin is preferable. No conclusion can be reached on the preferred type of laser technique to treat these patients, but data on laser enucleation is much less abundant and conclusive than that on laser vaporization.
Usage of GreenLight HPS 180-W laser vaporisation for treatment of benign prostatic hyperplasia

Abstract:Introduction: Laser therapy has gained increasing acceptance as a relatively less invasive treatment for lower urinary tract symptoms (LUTS) due to benign prostatic hyperplasia (BPH). From the early procedure of interstitial laser coagulation through to the use of holmium laser enucleation of the prostate, there has been an expanding body of evidence on the efficacy of such procedures. One of the newer lasers is the Green Light HPS 180 W laser. Studies with this GreenLight laser (GLL) (American Medical Systems, Inc, Minnetonka, MN, USA) showing results as good as those of transurethral resection of the prostate (TURP). In this paper, the efficacy of the new GLL 180-W versus the gold standard TURP in patients with LUTS due to BPH was tested in a prospective clinical trial. Objective: To compare results of Green light laser (GLL) evaporisation of the prostatae and transurethral resection of the prostate (TURP) for treatment of BPH. MATERIJALS AND METHODS: A total of 62 patients with BPH were randomly assigned to two equal groups: TURP or GLL. Results: Both groups were compared regarding all relevant preoperative, operative, and postoperative parameters. Functional results in terms of improvement of International Prostate Symptom Score (IPSS), maximum flow rate (Qmax), and postvoid residual (PVR) urine were assessed at 1, 3, 6 and 12 mo. A total of 62 patients completed 12 mo of follow-up in the TURP and GLL groups, respectively. Baseline characteristics were comparable. Mean operative time was significantly shorter for TURP. Compared to preoperative values, there was significant reduction in hemoglobin levels at the end of TURP only. A significant difference in favor of GLL was achieved regarding the duration of catheterization and hospital stay. In the GLL, no major intraoperative complications were recorded and none of the patients required blood transfusion. Among TURP patients, 6 required transfusion, 1 developed TUR syndrome, and capsule perforation was observed in 5 patients. There was dramatic improvement in Qmax, IPSS, and GLL compared with preoperative values and the degree of improvement was comparable in both groups at all time points of follow-up. Four TURP patients and one GLL patients developed bladder neck contracture treated by bladder neck incision; none in either group experienced urethral stricture or urinary incontinence. Conclusions: Compared to transurethral resection of the prostate, GreenLight HPS 180-W laser photoselective vaporization of the prostate is safe and effective in the treatment of patients suffering from lower urinary tract symptoms due to benign prostatic hyperplasia.

Abstract:Benign prostatic hyperplasia (BPH) is one of the most common diseases affecting men and can present with bothersome lower urinary tract symptoms (LUTS). Historically, transurethral resection of the prostate (TURP) has been considered the gold standard in the treatment of LUTS due to BPH. However, TURP and other traditional options for the surgical management of LUTS secondary to BPH are associated with high rates of sexual dysfunction. In the past decade, several novel technologies, including Aquablation therapy, convective water vapor therapy (Rezum), and transperineal prostate laser ablation (TPLA), have demonstrated promising evidence to be safe and effective while preserving sexual function.

Ultrasound-guided transperineal laser ablation for percutaneous treatment of benign prostatic hyperplasia: a new minimally invasive interventional therapy

Abstract:Background Although there are different treatments for benign prostate hyperplasia, their efficacy and safety differ. We are currently exploring a new minimally invasive interventional therapy for benign prostatic hyperplasia (BPH). Purpose To determine the feasibility, effectiveness, and safety of ultrasound-guided transperineal laser ablation (US-TPLA) for the treatment of BPH. Material and Methods Twenty patients with BPH (mean age = 73.9 ± 9.2 years) who underwent US-TPLA from June 2018 to January 2020 with a subsequent six-month follow-up were retrospectively reviewed. After local anesthesia, a 21-G trocar was inserted into the prostate tissue under ultrasound monitoring, followed by 1064 nm diode laser irradiation. Changes in international prostate symptom score (IPSS), quality of life (QoL), maximum urinary flow rate (Qmax), postvoid residual (PVR), prostate volume, and complications were evaluated six months after surgery. Results All patients underwent the operation successfully without serious complications. After six months, the average IPSS improved from 22.7 ± 5.3 to 9.1 ± 3.2 ( P < 0.001), the QoL improved from 4.9 ± 1.7 to 2.3 ± 1.3 ( P < 0.001), the Qmax improved from 8.5 ± 3.0 to 15.2 ± 4.8 mL/s ( P < 0.001), the PVR increased from 78.7 ± 58.8 to 30.3 ± 34.2 ( P < 0.05), and the mean prostate volume ranged from 70.8 ± 23.8 to 54.7 ± 20.9 mL ( P < 0.05). Conclusion US-TPLA is safe and feasible for the treatment of BPH. An evaluation at the six-month follow-up is effective.

Abstract:Objctives To present the surgical technique of 1470 nm diode laser antegrade enucleation of the prostate for benign prostatic hyperplasia (BPH) and evaluate its clinical efficacy.Methods 30 consecutive patients with BPH who underwent transurethral 1470 nm diode laser antegradeenucleation of the prostate at our institution from September 2016 to January 2017 were reviewed.The mean age of the patients was (72±9) years (range 53 to 86 years),and the prostate volume was (54±25) ml (range 27 to 120 ml).All procedures had been accomplished with antegrade approach:Firstly,two grooves deep into the capsule were created from bladder neck to 1 cm proximalto verumontanum at 5 and 7 o'clock direction.After the median lobe was vaporized,the distal margin of prostate was deepened to the capsule plane and extended to 9 and 3 o'clock position.Then the adenoma at 12 o'clock were vaporized to expose the capsule from the bladder neck to the proximal urethral sphincter.Subsequently,two lateral lobes were enucleated along the capsule with antegrade approach,respectively.The adenoma ＞80 ml were sucked by tissue morcellator and the adenoma ＜80 ml were vaporized segmentally.Results The mean operation time was (65±14) min,and the median estimated blood loss [M(QR)] was 10 (15) ml.The median postoperative hospital stay and mean duration of catheterization were 4 (3.8) d and (4.5±1.5) d (range 2 to 8 d),respectively.Preoperative and postoperative mean hemoglobin and serum sodium concentration were (129±23) versus (125±20) g/L and (141±3) versus (140±3) rnmol/L,respectively,where the differences were not statistically significant.The mean IPSS score,QOL sore and Qmaxpreoperative and 1 month postoperative were (22±6) versus (8± 3),(5.2±0.7) versus (1.9±0.9),and (7.8±2.1) versus (17.2±3.9) ml/s,respectively,with significantly differences.No patient experienced postoperative complications such as incontinence or urethral stricture during a median follow-up of 2.3(2.8) months (range 1 to 4 months).Conclusion Transurethral 1470 nm diode laser antegradeenucleation of the prostate is safe and effective for treatment of BPH which could provide rapid postoperative rehabilitation.

Abstract:Benign prostatic hyperplasia (BPH) is a common condition in adult men and its incidence increases progressively with aging. It has an important impact on the individual's physical and mental health and its natural progression can lead to serious pathological situations. Although the initial treatment is pharmacological, except in specific situations, the tendency of disease progression causes a considerable portion of the patients to require surgical treatment. In this case, there are several options available today in the therapeutic armamentarium. Among the options, established techniques, such as open surgery and endoscopic resection using monopolar energy, still prevail in the choice of surgeons because they are more accessible, both from a socioeconomic standpoint in the vast majority of medical services and in terms of training of medical teams. On the other hand, new techniques and technologies arise sequentially in order to minimize aggression, surgical time, recovery and complications, optimizing results related to the efficacy/safety dyad. Each of these techniques has its own peculiarities regarding availability due to cost, learning curve and scientific consolidation in order to achieve recognition as a cutting-edge method in the medical field. The use of bipolar energy in endoscopic resection of the prostate, laser vaporization and enucleation techniques, and videolaparoscopy are examples of new options that have successfully traced this path. Robot-assisted surgery has gained a lot of space in the last decade, but it still needs to dodge the trade barrier. Other techniques and technologies will need to pass the test of time to be able to conquer their space in this growing market.
Advances in lasers for the minimally invasive treatment of upper and lower urinary tract conditions: a systematic review

Abstract:Purpose: Technological advancements in laser lithotripsy are expanding into numerous fields of urology, like ureteroscopy (URS), percutaneous nephrolithotomy (PCNL), and benign and malignant soft-tissue treatments. Since the amount of research regarding lasers in urology has grown exponentially, we present a systematic review of the most recent and relevant advances encompassing all lasers used in urological endoscopic treatment. Methods: We performed a literature search using PubMed (May 2023) to obtain information about lasers for urological purposes. We included only recent data from published articles between 2021 and 2023 or articles ahead of print. Results: Lasers are widely used in lithotripsy for ureteric, renal, and bladder stones, benign prostate surgery, and bladder and upper tract tumor ablation. While the holmium (Ho:YAG) laser is still predominant, there seems to be more emphasis on pulse modulation and newer lasers such as thulium fiber laser (TFL) and pulsed Tm:YAG laser. Conclusion: The use of lasers and related technological innovations have shown increasing versatility, and over time have proven to be invaluable in the management of stone lithotripsy, treatment of benign and malignant prostate diseases, and urothelial tumors. Laser endoscopic treatment is heavily based on technological nuances, and it is essential to know at least the basics of these technologies. Ultimately the choice of laser used depends on its availability, cost, surgeon experience and expertise.
Transurethral procedures in the treatment of benign prostatic hyperplasia: A systematic review and meta-analysis of effectiveness and complications

Abstract:Background: With the progress of surgical techniques and instruments, various minimally invasive surgical therapies were developed to treat benign prostatic hyperplasisa (BPH). However, the efficacy and safety of different transurethral procedures for the treatment of BPH are still undefined. Method: A systematic search was performed for all randomized controlled trials (RCTs), which compared the transurethral procedures for BPH from 1995 to 2016. The clinical baseline characteristics, International Prostate Symptom Score (IPSS), quality of life (QoL), postvoid residual (PVR), maximum flow rate (Qmax), short-term and long-term complications were analyzed using RevMan and ADDIS software. Result: Eighty-eight randomly controlled trials and fifteen procedures were included in the network meta-analysis. HoLEP greatly inproved PVR. TmLRP had the best efficacy in improving QoL score. Diode laser vaporization of prostate was superior in improving IPSS and Qmax. Diode laser through vaporization required the shortest time in catheterization, while Nd:YAG was the longest procedure. For the hospitalization time, TUR was the longest and HoLEP was the shortest. TmLRP was related to the lowest postoperative hemoglobin decrease. TmLEP had the least rates of occurrence of hematuria, reoperation and erectile dysfunction. HoLEP was the best choice to reduce the incidence of recatheterization, urinary retention, urinary tract infection, stress urinary incontinence and retrograde ejaculation. The complications such as blood transfusion, urethral stricture, bladder neck contracture were relatively rare in the patients who underwent diode laser vaporization of prostate. Conclusion: Compared with other transurethral procedures, thulium, holmium and diode lasers were associated with better efficacy and fewer complications.
Short-term Effect of 980/1470nm Diode Laser Technology on Benign Prostatic Hyperplasia

Abstract:Objective To analyze the short-term effect of 980/1470nm diode laser technology on benign prostatic hyperplasia.Methods 57patientsforsurgicaltreatmentofprostatichyperplasiahospitalizedduringtheperiodfromJune,2014to Mar.,2015 were enrolled in the study;980/1470nm diode laser transuresthral vaporization was performed on 34 patients (laser group) while transurethral plasma kinetic prostatectomy on other 23 patients (TUPKP group);a retrospective analysis was made to the data of the patients in the 2 groups,including age,prostate volume,the tissue cutting effect,length of operation time,bleeding volume in operation,fluidvolumeofpostoperativeperfusion,indwelling catheter time,the decrease of hemoglobin and the changes of IPSS,Qmax and PVR after operation.Results Operations in the 57 patients were successful;there existed no difference in age,prostate volume and preoperative IPSS,Qmax and PVR between the patients in the 2 groups;in laser group,the effective rate of tissue cutting was (43.2 ±5.7) g/h,the length of operation time (1.8±0.5) h,bleeding volume in operation (8.1±3.2) mL,fluid volume of postoperative perfusion (12.1±2.1) L,indwelling catheter time (4.5 ±0.7) d and the decrease of hemoglobin (0.5 ± 1.4) g/L;follow-up 3 months after operation showed a decrease of (14.7±3.2) points in IPSS scores,of (68.1 ± 11.4) mL in PVR but an increase of (13.3 ± 1.6) ml/s in Qmax in laser group;the laser group was of an obvious superiority in bleeding volume in operation,fluid volume of postoperative perfusion and decrease of hemoglobin to TUPKP group;no statistical differences in other data were found between the 2 groups.Conclusions 980/1470nm diode laser transuresthral vaporization is of similar short-term effect to TUPKP in treating prostatic hyperplasia and it is even safer,especially for senile and high-risk patients;the long-term effect needs a further follow-up study.

Abstract:Purposes: GreenLight laser has gained increasing acceptance as a less invasive treatment for lower urinary tract symptoms due to benign prostatic hyperplasia (BPH/LUTS). Three surgical options were developed: standard photovaporization (PVP), anatomical PVP and GreenLight enucleation of prostate (GreenLEP); however, literature lacks a direct comparison among the procedures. Aim of the present study is to compare the three techniques in a multicentre series of patients. Methods: Data were collected from consecutive patients with indication to surgical management of BPH/LUTS in five institutions. Patients underwent standard PVP, anatomical PVP or GreenLEP according to surgeon preferences. Standard parameters associated with transurethral prostate surgery were documented prior surgery and during the follow-up. Patients' perception of improvement was measured using a single-item scale. Early (within first 30 post-operative days) and delayed post-operative complications were recorded. Descriptive statistics, univariate and multivariate analysis were used. Results: We evaluate 367 consecutive patients (mean age 69.1 years). Median prostate size and PSA were 68 ml (IQR 50-90) and 2.8 ng/ml (IQR 1.7-4.3), respectively. The median operative time and applied energy were 60 min (IQR 45-75) and 250 kJ (IQR 160-364). Catheterization time and median post-operative stay were 1 and 2 days. No patient was transfused. The overall median Q max values increased for 8-19 ml/s (p < 0.05), median International Prostate Symptoms Score decreased from 24 to 7 (p < 0.05). A total of 7.4% urinary retention, 33.4% bothersome storage symptoms, 2.5% short-term stress incontinence were recorded. Three heart attacks, one pulmonary embolism and one death occurred. Prostate volume was a predictive factor for post-operative storage symptoms (p = 0.049). Nine percentage of patients experienced long-term complications (4, 0.9 and 0.9% of urethral stricture, bladder neck contracture and prostatic fossa sclerosis, respectively) with 2.5% of long-term stress urinary incontinence (conservatively managed). The reintervention rate was 6%. Late complications were associated at univariate analysis with pharmacological therapy (combination therapy vs. alpha blockers alone vs. none: p value = 0.042) and with the surgical approach (standard PVP vs. anatomical PVP vs. GreenLEP p value = 0.011). The patients' perception of satisfaction was 68% "greatly improved", 27% "improved", 4% "not changed" and 1% "worsened" with no differences between techniques. Conclusion: The availability of three different GreenLight laser techniques allows surgeons with different skills to safety use this technology that remains effective with high patient satisfaction. Anatomical vaporization seems to guarantee the best balance between functional outcomes, surgical procedures and complications.

Abstract:OBJECTIVE To investigate the causes of the vacuum sealing drainage (VSD) complications during treatment of extremity wounds. METHODS Between February 2005 and February 2010, 174 patients with skin and soft tissue defect of extremities were treated with VSD. Among them, 12 patients (6.9%) had complications and the clinical data were analyzed retrospectively. There were 7 males and 5 females aged from 17 to 65 years (mean, 36.1 years). Injury was caused by traffic accident in 8 cases, and by machine extrusion in 4 cases. The locations were forearm in 3 cases, upper arm in 2 cases, thigh and calf in 1 case, calf in 4 cases, and foot in 2 cases. The size of defect ranged from 10 cm x 7 cm to 90 cm x 40 cm. The time from injury to hospitalization was 3-8 hours (mean, 4.2 hours). RESULTS At 1 day postoperatively, moderate to severe anemia and hypokalemia occurred in 6 cases, and the hemoglobin value increased to 100 g/L and the electrolyte disorder was rectified after blood transfusion and fluid infusion. At 2 days, 2 patients had local skin allergic symptoms, and local rash disappeared after oral administration of Clarityne. At 3 days, 2 patients complained pain and the pain was released after the suction pressure was decreased by 50%; 2 patients had infection and received VSD demolition, debridement and draining, and anti-infection treatment. Tissue necrosis occurred in 1 case at 4 days and pressure ulcer in 1 case at 5 days, VSD was immediately changed and wound was repaired by split thickness skin graft or local flap. CONCLUSION Complications associated with VSD therapy for the wounds of the extremities are not very common. Most causes are related to the location of wound, the technique of the operators, and the conditions of the patients.
