Abstract:Objective To explore the quantitative index for evaluating the intermittent hypoxia training effects by analyzing the characteristic changes of electroencephalogram (EEG) complexity and saturation of blood oxygen (SaO2) of high performance fighter pilots. Methods Thirty-two pilots were selected as subjects and undertook a 25 min-training (simulated hypoxia at 3500 m-oxygen concentration 13.1%) with Type DY-84 hypoxia training device once a day for 15 d. Before and after training the subjects were put in simulated 7500 m hypoxia condition (oxygen concentration 7.1%,ventilation volume 15L/min) and their EEG, SaO2, number of red blood cell and hemoglobin level were recorded and analyzed by t-test. Results Training effects showed that the subjects' 7500m EE(G complexity was significantly decreased (P＜0.01), but SaO2 was significantly increased (P＜0.01).Number of red blood cell and hemoglobin level had no obvious change (P＞0.05).Conclusions The simulated 3500 m intermittent hypoxia training could improve pilot's hypoxia tolerance. EEG complexity and SaO2, which are measured under simulated 7500 m hypoxia condition,would be the quantitative indices for evaluating the effects of intermittent hypoxia training for pilot.The results application would be hopefully expanded to the population who work at high altitude or in anoxic environment.

Abstract:The present study systematically compared the effects of fatigue and alcohol intoxication on a range of neurobehavioural tasks. By doing so, it was possible to quantify the performance impairment associated with fatigue and express it as a blood alcohol impairment equivalent. Twenty-two healthy subjects aged 19-26 years participated in three counterbalanced conditions. In the sustained wakefulness condition, subjects were kept awake for 28 h. In the alcohol and placebo conditions, subjects consumed either an alcoholic or non-alcoholic beverage at 30 min intervals, until their blood alcohol concentration reached 0.10%. In each session, performance was measured at hourly intervals using four tasks from a standardised computer-based test battery. Analysis indicated that the placebo beverage did not significantly effect mean relative performance. In contrast, as blood alcohol concentration increased performance on all the tasks, except for one, significantly decreased. Similarly, as hours of wakefulness increased performance levels for four of the six parameters significantly decreased. More importantly, equating the performance impairment in the two conditions indicated that, depending on the task measured, approximately 20-25 h of wakefulness produced performance decrements equivalent to those observed at a blood alcohol concentration (BAC) of 0.10%. Overall, these results suggest that moderate levels of fatigue produce performance equivalent to or greater than those observed at levels of alcohol intoxication deemed unacceptable when driving, working and/or operating dangerous equipment.

Abstract:Reduced opportunity for sleep and reduced sleep quality are frequently related to accidents involving shift-workers1,2,3. Poor-quality sleep and inadequate recovery leads to increased fatigue, decreased alertness and impaired performance in a variety of cognitive psychomotor tests4. However, the risks associated with fatigue are not well quantified. Here we equate the performance impairment caused by fatigue with that due to alcohol intoxication, and show that moderate levels of fatigue produce higher levels of impairment than the proscribed level of alcohol intoxication.

Abstract:Aims: Under current US Department of Transportation (DOT) regulations, some commercial vehicle operators are subject to sanctions for having > or = 00.04 g% blood alcohol concentrations (BACs) on the job. This study investigated the effects of alcohol (between 0.04 and 0.05 g% BAC) on the simulated operation of a commercial ship's power plant. Design: A balanced placebo design was used in which alcohol administration was fully crossed with expectancy that alcohol was administrated. Setting: The study was conducted at the Massachusetts Maritime Academy, Buzzards Bay, MA, USA. Participants: Participants were 18 volunteer engineering students in their senior year, at least 21 years of age, with previous experience on the diesel simulator. Intervention: Following a baseline trial on day 1, on day 2 participants were randomized to expectancy (told alcohol or placebo) and beverage (receive alcohol or placebo). The dosage was 0.6 g/kg for males and 0.5 g/kg for females. In both baseline and performance days, participants were randomized to one of four diesel simulator scenarios, each replicating a system failure in a power plant subsystem (e.g. main propulsion, electrical generating). Measurement: Performance was measured by time required for problem identification and remediation. Findings: A main effect for alcohol administration was found. In the alcohol condition, simulator performance time was almost twice as long (351 sec) as on the placebo condition (186 sec). The expectancy effect was not significant. Conclusion: These findings support the current federal policy on the hazards of this low alcohol level within the context of commercial shipping.
Effect of different alcohol levels on take-over performance in conditionally automated driving

Abstract:Automated driving systems are getting pushed into the consumer market, with varying degrees of automation. Most often the driver's task will consist of being available as a fall-back level when the automation reaches its limits. These so-called take-over situations have attracted a great body of research, focusing on various human factors aspects (e.g., sleepiness) that could undermine the safety of control transitions between automated and manual driving. However, a major source of accidents in manual driving, alcohol consumption, has been a non-issue so far, although a false understanding of the driver's responsibility (i.e., being available as a fallback level) might promote driving under its influence. In this experiment, N = 36 drivers were exposed to different levels of blood alcohol concentrations (BACs: placebo vs. 0.05% vs. 0.08%) in a high fidelity driving simulator, and the effect on take-over time and quality was assessed. The results point out that a 0.08% BAC increases the time needed to re-engage in the driving task and impairs several aspects of longitudinal and lateral vehicle control, whereas 0.05% BAC did only go along with descriptive impairments in fewer parameters.
Vigilance decrement during a simulated general aviation flight

Abstract:This study examined pilots' vigilance during an extended general aviation flight as measured by their capacity to exercise control over the aircraft during a planned flight. Thirty‐one qualified pilots flew a flight simulator from Wagga Wagga to Bankstown, Sydney, a distance of 207 nautical miles. The flight comprised five separate legs, although three legs were subjected to analysis. On the basis of attentional resource theory, it was hypothesised that task performance would differ based on the requirement for memory retrieval. Consistent with the hypothesis, the results revealed a deterioration in those tasks for which there was a substantial requirement for memory retrieval. Further analysis revealed that the deterioration in performance was best predicted by pilots' perception of the workload associated with the flight and their perception of their ability to exercise control over aircraft during normal conditions. The implications are discussed in terms of system design and training. Copyright © 2010 John Wiley & Sons, Ltd.

Abstract:Excessive intake plays a significant role in the development of alcohol use disorder and impacts 15 million Americans annually, with approximately 88,000 dying from alcohol related deaths. Several facets we contribute to alcohol use disorder include impulsivity, motivation, and attention. Previous studies have used the 5-Choice Serial Reaction Time Task (5-Choice) to analyze these types of behaviors using sugar, but recently we have published using 10% alcohol as the reward. This study analyzed 48 mice that were trained to respond for alcohol in the 5-Choice. All mice distributed and analyzed first by alcohol preference and then by consumption. Here, we became interested in a new classification called "engagement". High-engaged and low-engaged mice were determined by the number of correct responses during final Late-Stage training sessions. Interestingly, during Early-Stage training, the mice began to separate themselves into two groups based on their interaction with the task. Throughout both training stages, high-engaged mice displayed a greater number of trials and correct responses, as well as a lower percentage of omissions compared to low-engaged mice. Following three weeks of intermittent access homecage drinking, low-engaged mice showed greater increase in perseverative responding relative to high-engaged. Additionally, low-engaged mice decreased their reward and correct latencies compared to high-engaged mice suggesting an increase in motivation for alcohol. Overall, engagement analysis presents two clearly different groups, with only one being motivated to work for alcohol. These two distinct phenotypes in the 5-Choice could be used to model alcohol motivated behavior, which could help us further understand alcohol use disorder.

Abstract:Future airspace is expected to become more congested with additional in-service cargo and commercial flights. Pilots will face additional burdens in such an environment, given the increasing number of factors that they must simultaneously consider while completing their work activities. Therefore, care and attention must be paid to the mental workload (MWL) experienced by operating pilots. If left unaddressed, a state of mental overload could affect the pilot's ability to complete his or her work activities in a safe and correct manner. This study examines the impact of two different cockpit display interfaces (CDIs), the Steam Gauge panel and the G1000 Glass panel, on novice pilots' MWL and situational awareness (SA) in a flight simulator-based setting. A combination of objective (EEG and HRV) and subjective (NASA-TLX) assessments is used to assess novice pilots' cognitive states during this study. Our results indicate that the gauge design of the CDI affects novice pilots' SA and MWL, with the G1000 Glass panel being more effective in reducing the MWL and improving SA compared with the Steam Gauge panel. The results of this study have implications for the design of future flight deck interfaces and the training of future pilots.

Abstract:This study was designed to explore the association between CAG repeats in AR gene and major depressive disorder (MDD) in male children and adolescents. The results showed that there were differences between adolescent depressive patients and adolescent controls in CAG repeats’ length and alleles’ distributions, and the severity of depression and anxiety was negatively correlated with the length of CAG repeats in adolescent patients. This suggested that AR gene might be involved in the depressive upset in adolescents, and the age- and sex-related prevalent differences might also be associated to CAG repeats.

Abstract:Situational awareness (SA) and fatigue management are crucial aspects of aviation safety, particularly during demanding flight phases. This study introduces an innovative approach employing flight data, machine learning, and Continuous Performance Test (CPT) metrics to predict pilot performance and SA during instrument approaches under Instrument Meteorological Conditions (IMC). Data were collected from over 10 pilots across more than 40 flights in a high-fidelity Cessna 172 analog flight simulator. Significant correlations were observed between dynamic cognitive performance parameters and the exceedance shape factor, a novel measure of pilot sustained attention introduced in this research. Key variables identified through correlation analysis included variability, interstimulus change, and reaction time standard deviation. Importantly, commission scores and reaction time standard deviation emerged as key predictors in the machine learning model, specifically the Optimizable Gaussian Process Regression (GPR) model with a radial basis function kernel. The model achieved a validation R-squared of 0.90 and a test R-squared of 0.70. These systems could incorporate additional data sources, such as eye-tracking and scan pattern analysis, for a better assessment of pilot SA and fatigue levels. While post-flight measurements are inherently reactive, they are effective for monitoring the degradation of pilot CPT scores after each leg of high-frequency, short-duration flights. Notable limitations include the need to understand individual cognitive differences among pilots, such as age, experience, and cognitive style. The predictive model also requires validation in actual flight conditions to determine its ecological validity. Future research should aim to address these limitations, generalize the findings, and integrate CPT data with other sensor inputs to provide a more comprehensive understanding of pilot performance.

Abstract:A fully portable quantitative EEG assessment system was used to evaluate workload in an advanced technology aircraft simulator. Air refueling and landing approach tasks were each performed at two difficulty levels in 15 Air Force pilots. Averaged and trended EEG spectral data were compared in the 8-12 Hz band to identify functional requirements for increased workload within and between tasks. A progressive suppression of 8-12 Hz activity at medial and right parietal sites accompanied increased workload in the air refueling task, while a sustained suppression at right and left temporal sites was associated with increased workload in the landing task. These findings suggest a potential electrophysiological index for workload. They also identify specific and differential cortical responses to visual integration in air refueling and working memory in ILS approach as primary correlates of the cognitive requirements for these tasks in these subjects.

Abstract:Eight experienced pilots and eight non-rated individuals flew simulated instrument training circuits while eye movements were recorded with infrared oculometry. Each subject flew two, two hour simulations containing 32 data segments. A full circuit contained a nominal workload segment and either a high workload or low workload (monitoring) segment. Differences in performance were noted between high workload and nominal segments, and between morning and afternoon segments. Fixation duration and number of Long Fixations mirrored performance. Long Fixation frequency modelled 83% of dangerous performance deviations with a 31% false alarm rate.

Abstract:Despite technological advancements, human decision errors still contribute to civil aviation accidents. This study investigated whether flight time, cognitive reflection, task-load, metacognition, and perceived stress predicted decision-making (DM) performance during two in-flight training simulations with 104 commercial pilots at Bogota International Airport. Hierarchical regression analysis revealed that the predictors accounted for 56% of the variance. Cognitive reflection, flight time and performance task load emerged as significant positive predictors. Cognitive reflection significantly moderated the relationship between flight time and DM performance, with pilots scoring lower on cognitive reflection showing improved DM with increased flight time, while controlling for performance task load. The study did not find significant relationships between stress metacognition and DM performance. The study emphasises the significance of advanced training methods in improving pilots' DM, especially for those with low cognitive reflection. Future research should expand to multiple airlines, address gender balance, and incorporate direct measures of metacognitive monitoring. This article examines predictors of decision-making performance among commercial pilots during in-flight simulations. Findings suggest that cognitive reflection, flight time and performance task load positively influence decision-making, while task-load has a negative impact. This insight can inform aviation training programs to enhance safety and update pilots' training in uncertain conditions.

Abstract:Abstract This study explores potential disparities between flight instructor evaluations and pilot self-assessments in the context of full flight simulator training. Evaluated performance was based on the Competency-based Training and Assessment framework, a recent development of competency-based education within aviation. Self-assessed performance is derived from survey responses and debriefing interviews. The simulator session involves eight multi-crew pilot training graduates and eight experienced flight captains, encompassing two tasks featuring sudden technical malfucntions during flight. The flight instructor’s evaluations reveal no significant differences in pilot performance. However, disparities become apparent when pilots engaged in reflecting their performance. Novice pilots, despite perceiving both tasks as easy, exhibited an overconfidence that led them to underestimate the inherent risks. Conversely, experienced pilots demonstrated greater caution towards the risks and engaged in discussing possible hazards. Furthermore, this study highlights the challenge of designing flight simulator training that incorporates surprise elements. Pilots tend to anticipate anomalies more readily in simulator training than during actual flights. Thus, this study underscores the importance of examining how pilots reflect on their performance, complementing the assessment of observable indicators and predefined competencies.

Abstract:OBJECTIVE The aim of this study was to evaluate the effectiveness of Er:YAG laser and conventional periodontal therapy in the treatment of acute streptococcal gingivitis both clinically and microbiologically. BACKGROUND DATA This case report describes a 2-year follow-up of a 30-year-old, female, chronic periodontitis patient, presenting severe gingival inflammation, sensitivity, pain, and acute gingival lesions that were treated with Er:YAG laser and conventional hand and ultrasonic instruments. MATERIALS AND METHODS Before the initial periodontal treatment, microbiological samples were taken from the lesion sites with sterile paper points from the sulcuses bilaterally, and excisional biopsies were obtained from the lesions. Following diagnostic tests, the lesions were identified as acute streptococcal gingivitis. Following the measurement of clinical indices, initial periodontal therapy was performed with Er:YAG laser on the right side and conventional hand and ultrasonic instruments on the left side, which were performed as two sessions at weekly intervals. As an adjunct to mechanical periodontal therapy, 500 mg amoxicillin was prescribed t.i.d. for a week. RESULTS Microbiological samples grew mostly Streptococcus sp. and black pigmented obligate anaerobic bacteria. The histopathological examination revealed acanthosis, papillomatosis, and spongiotic lesions in the keratinized stratified squamous epithelium; infiltration of polymorphonuclear neutrophils, lymphocytes, and macrophages and plasma cells in the connective tissue; infiltration and accumulation of polymorphonuclear neutrophils in the epithelium, especially in the spongiotic lesions; and formation of microabscess-like clusters. After the initial periodontal treatment, clinical and microbiological measurements were repeated and reductions in clinical indices and the number of microorganisms were observed. both treatment modalities gave similar results, and no recurrences were observed during the 2-year follow-up. CONCLUSION Er:YAG laser seems to be promising and as effective as conventional periodontal therapy in the treatment of acute streptococcal gingivitis.
Alcohol use disorder and fitness to drive: Discrepancies between health professionals' evaluations and objective measures of alcohol use and cognitive functioning

Abstract:Aims: In this study, we investigated if health professionals' evaluations of driving ability corresponded with measures of severity of alcohol use and measures of cognitive functions necessary for safely driving a car. Methods: A total of 90 participants from a multicentre study were included. Participants were categorised into three groups: (1) the group judged fit to drive (FIT); (2) the group judged not fit to drive (UNFIT); and (3) the group who had lost their driver's licence due to legal sanctions (LEGAL). The participants' AUDIT scores, earlier treatment episodes and results from neuropsychological tests of reaction time, attention and visuospatial ability were included in the analyses. Results: We found a significant difference in the severity of alcohol use disorder (AUD) and visuospatial abilities between the FIT and UNFIT groups. Half of the UNFIT group had at least mild visuospatial difficulties, compared to only a quarter in the FIT group. There were no group differences in reaction time or attentional measures. The LEGAL group had more severe AUD than the other groups. Conclusion: The FIT group did not perform differently from the UNFIT group on attention and reaction time measures. The UNFIT group had more visuospatial impairments, but even half of this group had normal scores. It is uncertain whether the differences between the two groups are of practical significance. The quality of health professionals' evaluations may be questioned, and the results highlight the need for more reliable and valid criteria for doing fitness to drive evaluations.
Pilots' mental workload variation when taking a risk in a flight scenario: a study based on flight simulator experiments

Abstract:Pilots' operation behavior in flight is associated with their mental state variables such as workload, situation awareness, stress, etc. The objective of this study was to investigate the dynamic process of mental workload for pilots who perform a risky flight task in simulated scenarios. Two empirical experiments were conducted to address this issue. In experiment one, 19 trainee pilots divided into high-risk and low-risk groups performed a target-search task in a low-altitude visual flight. The results showed a statistically significant interaction between groups and segments for heart rate variability (HRV). The same pattern of physiological results was replicated among participants in experiment two, in which 19 airline pilots completed an approach with low visibility. These findings highlighted the relationship between mental workload variation and risk-taking behavior, which could be considered in improving pilot selection and training to improve flight safety.

Abstract:Abstract Rationale Inhibition is a core executive function and refers to the ability to deliberately suppress attention, behavior, thoughts, and/or emotions and instead act in a specific manner. While acute alcohol exposure has been shown to impair response inhibition in the stop-signal and Go/NoGo tasks, reported alcohol effects on attentional inhibition in the Stroop task are inconsistent. Notably, studies have operationalized attentional inhibition variably and there has been intra- and inter-individual variability in alcohol exposure. Objective This study aimed to examine the acute effects of alcohol on attentional inhibition, considering previous limitations. Methods In a single-blind, cross-over design, 40 non-dependent participants with a medium-to-high risk drinking behavior performed a Counting Stroop task (CST) under a baseline and an arterial blood alcohol concentration (aBAC) clamp at 80 mg%. Attentional inhibition was assessed as the alteration of reaction times (RT), error rates (ER), and inverse efficiency scores (IES) between incongruent and congruent trials (interference score). Stroop performance was also assessed regardless of trial-type. Results Compared to saline, acute alcohol exposure via an aBAC clamp did not affect CST interference scores but increased RTs and IES in both incongruent and congruent trials. Conclusions Attentional inhibition (Stroop interference score) was not impaired by clamped moderate alcohol exposure. Acute alcohol impaired Stroop performance evidenced by a general increase in response times. Our findings suggest that response and attentional inhibition do not share the same neurocognitive mechanisms and are affected differently by alcohol. Results could also be explained by automated behaviors known to be relatively unaffected by acute alcohol.

Alcohol Hits You When It Is Hard: Intoxication, Task Difficulty, and Theta Brain Oscillations

Abstract:Background: Alcohol intoxication is known to impair decision making in a variety of situations. Previous neuroimaging evidence suggests that the neurofunctional system subserving controlled processing is especially vulnerable to alcohol in conflict-evoking tasks. The present study investigated the effects of moderate alcohol intoxication on the spatiotemporal neural dynamics of event-related total theta (4 to 7 Hz) power as a function of task difficulty. Methods: Two variants of the Simon task manipulated incongruity via simple spatial stimulus-response mismatch and, in a more difficult version, by combining spatial and semantic interference. Healthy social drinkers participated in both alcohol (0.6 g/kg ethanol for men, 0.55 g/kg for women) and placebo conditions in a counterbalanced design. Whole-head magnetoencephalography (MEG) signals were acquired and event-related total theta power was calculated on each trial with Morlet wavelets. MEG sources were estimated using anatomically constrained, noise-normalized, spectral dynamic statistical parametric mapping. Results: Longer reaction times and lower accuracy confirmed the difficulty manipulation. Response conflict (incongruity) increased and alcohol intoxication decreased event-related theta power overall during both tasks bilaterally in the medial and ventrolateral prefrontal cortices. However, alcohol-induced theta suppression was selective for conflict only in the more difficult task which engaged the dorsal anterior cingulate (dAC) and anterior inferolateral prefrontal cortices. Theta power correlated negatively with drinking levels and disinhibition, suggesting that cognitive control is susceptible in more impulsive individuals with higher alcohol intake. Conclusions: The spatiotemporal theta profile across the 2 tasks supports the concept of a rostrocaudal activity gradient in the medial prefrontal cortex that is modulated by task difficulty, with the dAC as the key node in the network subserving cognitive control. Conflict-related theta power was selectively reduced by alcohol only under the more difficult task which is indicative of the alcohol-induced impairment of conflict monitoring and top-down regulation. Compromised executive control under alcohol may underlie a range of adverse effects including reduced competency in conflict-inducing or complex situations.
